- 0:04Hello and welcome to the Orthodontic Products podcast.
- 0:07I'm your host, Alison Werner.
- 0:08In this episode, we're gonna talk about increasing adult patients as share of
- 0:12chair.
- 0:12And with us to break down this topic is Oliver Gelles, Chief Marketing Officer at
- 0:16OrthoFi, the software and service solution that provides patient onboarding and
- 0:20includes insurance, patient billing and collection services.
- 0:23Oliver has over 20 years of experience in the orthodontic industry and is a frequent
- 0:27speaker on patient acquisition.
- 0:29Oliver, thank you for joining me.
- 0:31Thanks for having me, Allison.
- 0:32Great.
- 0:33So to get started, I know OrthoFi has some great data around adult orthodontic starts
- 0:38and I know there's some misconceptions around trends there.
- 0:42So what are you seeing?
- 0:44Yeah, I've been on the circuit a little bit recently because I think that in the
- 0:50last few years, we've had such a roller coaster ride with COVID and then what
- 0:54I call the year of confirmation bias with the 20 plus percent growth year in 2021
- 0:59and then a correction in 2022 and then some economic tightness and softness that
- 1:06people experience, especially in the back half of last year, which we're seeing
- 1:11continuing into this year.
- 1:13And so, you know, for, for the audience out there, you know, first of all, the 21
- 1:18% growth that, that everyone experienced in, in, you know, in 2021, you know, much
- 1:25of that was fueled by anomalous factors, stimulus, nowhere to spend the stimulus,
- 1:31and then really a surge in adult treatment demand.
- 1:35They surged from what was 25 % of the cases to almost 35 % within our cohort
- 1:42group.
- 1:43And so they represented a disproportionate aspect of that growth.
- 1:48And then of course, when the economy comes back and tightens, that's usually the
- 1:52first place that that happens.
- 1:54That said, the 3.5 % shrinkage in starts last year is not entirely attributable to
- 2:03just the economy.
- 2:04I think that that's lazy.
- 2:06And I think that what we've been doing is slicing and dicing the data to understand
- 2:11like more of a root cause.
- 2:13And so what we've seized upon is this.
- 2:16So when you look at the starts, you know, the, the, the child starts actually were
- 2:20flat or increasing the overall, you know, uh, exams were, you know, a little bit
- 2:26down, but not as down as the start.
- 2:28So we lost some case acceptance, the adult,
- 2:31demand was a little down, which is pretty indicative of economic tightness.
- 2:35We've seen that in other times like 2008, 2009.
- 2:39So their exams were down 4 % for adults.
- 2:43Adult fixed treatment was also down 4%, which is what you'd want.
- 2:47If the phone rings 4 % less at the time, you would expect a 4 % drop in adult fixed
- 2:52appliance starts.
- 2:54The outlying factor, though, is that adult aligner starts were down almost 11%.
- 3:01And so what accounts for that seven point delta?
- 3:05And there's really only one distinguishing factor between aligner treatment and adult
- 3:11fixed appliance treatment.
- 3:13And I think we can all agree that aligners didn't spontaneously become less popular,
- 3:17less marketed or any of that.
- 3:19So it wasn't the demand for that solution.
- 3:22It was because orthodontists routinely are still requiring the lab fee as a minimum
- 3:28down.
- 3:29And so during a time of economic tightness, the worst thing you can do
- 3:33during that time is to require an adult who, you know, by the numbers for only
- 3:404 out of 10 have a thousand dollars to put towards treatment to put 1500 down.
- 3:45So you're, we self -inflicted the equivalent of a 2 % impact on start.
- 3:53So if you think about what I said, 3.6 % drop in starts.
- 3:572 out of that 3.6, so more than half was attributable to that seven-point delta
- 4:03with adults.
- 4:04So that's actually self -inflicted.
- 4:07Okay.
- 4:08Okay.
- 4:09So let's talk about who these patients, these adult patients are.
- 4:15In large part, we're talking about Millennials and Gen Z.
- 4:18So if you're going to go after these adult patients and figure out a way to rebound
- 4:23here, what do you need to know about this population and their decision making?
- 4:28So there are a lot brighter folks than I about actually seizing on the behavioral
- 4:33trends of millennials.
- 4:36Tracy Martin in particular has made a career out of really educating the
- 4:40industry of millennials and she's forgotten more about millennial
- 4:43psychographics than I'll ever know.
- 4:45But I do think that it's worthwhile to be a student of that game.
- 4:49What I can add to the picture is this, which is what everyone on the call needs
- 4:54to remember and know is that...
- 4:57The adult target and the adolescent target are very different in these two important
- 5:02ways.
- 5:03Number one is they were not planning on this treatment.
- 5:08The parents of an 11 year old have likely been in OBS or at least seen their kid
- 5:14knowing that their kid has some malocclusion or some crooked teeth or buck
- 5:18teeth.
- 5:19And they've known that they were going to have to be saving for this.
- 5:22So they've been preparing for this expense.
- 5:25And so you can sometimes get a higher down from a family that's been saving for
- 5:31adults.
- 5:32Many of them make the decision very spontaneously.
- 5:35It's far more whimsical.
- 5:36You know, they have a life event, they get divorced, they have a wedding they're
- 5:41going to be going to or be in.
- 5:44They are their friend just got aligners or got them off and they're motivated in that
- 5:51moment.
- 5:51So they don't have a bunch of saved up and.
- 5:54All the Fred savings date data shows that people had lots of savings and lots of
- 6:00cash back in 2021, but it not only has snapped back, it's actually snapped back
- 6:04lower than before.
- 6:06So people have even less sitting in their bank accounts ready to write a check on.
- 6:10So that's one of the reasons why of all your targets, you can't ask an adult for a
- 6:17high down.
- 6:17That's just not a great idea.
- 6:20So that's one important thing.
- 6:21And then the other thing is,
- 6:23They've been living with this malocclusion for likely 10 to 15 to 20 years.
- 6:30And so they can live another year with it.
- 6:33So we got to be really great at getting that same day contract from them while
- 6:39their attention is in this place right now before they get distracted with other
- 6:43expenses and other things.
- 6:45And so if you don't get an adult to same day start, you're probably not going to
- 6:49get them for a while.
- 6:51but you need to be patient and stay with them using automated pending technologies
- 6:56like OrthoFi has, but there are other technologies that do this and stay with
- 7:01them so that when the thought comes back up, you are just in time, you're in their
- 7:06inbox and ready to remind them that they deserve this treatment and that it's a
- 7:13great time to get a great smile.
- 7:17Well, and another reason that targeting this adult population and getting them to
- 7:21that decision is because adults are going to make up a larger portion of cases going
- 7:28forward based on the demographics, right?
- 7:31Yeah, I mean, this is another thing that a lot of orthodontists are either sleeping
- 7:36on or bearing their heads about.
- 7:38So we are in one of the greatest specialties and industries in the world
- 7:43for many reasons.
- 7:44But one of the reasons is that we're very lucky is that we have birth rate data,
- 7:49which essentially is the equivalent of total addressable market data that tells
- 7:53us how many targets are we going to have.
- 7:56for the largest part of our market, right?
- 7:58So we know how many 11 year olds there are gonna be because we have the birth rate
- 8:02from 11 years ago.
- 8:04And so what's super interesting is the birth rate actually peaked in 2010 and it
- 8:10has been declining ever since.
- 8:12So it makes sense that in 2021, we had a nice number of kids.
- 8:19The bad news is, is that by the data, not only are there gonna be less kids this
- 8:24year to treat,
- 8:25they're going to be less kids for the next 10 years to treat.
- 8:30So if you want to grow your practice, it's not going to be by treating more kids
- 8:34unless you're going to be stealing them from your neighbors and getting a market
- 8:38share play.
- 8:39You're going to need to treat more adults if you want to increase your total volume
- 8:43of starts.
- 8:47Okay, so with all that in mind, what are the best strategies for winning these
- 8:53patients over and building a long -term relationship?
- 8:55And you talked a little bit there about dealing with their financial situation.
- 9:00So what else should doctors know?
- 9:02So from a conversion workflow, I think one of the most important things in general,
- 9:09but especially for adults.
- 9:11So it's almost like all these things are important to convert kids, but acutely
- 9:15important for adults.
- 9:16Right.
- 9:17The first thing is really lean into the pre -suasion aspect.
- 9:23Right.
- 9:23The job to convert the patient does not begin when they walk through your door.
- 9:27The job begins from the first call.
- 9:31how you establish rapport, how you establish reciprocity with that patient,
- 9:36make them feel like a patient even before they walk in.
- 9:40We have lots of data connecting their engagement with intake forms, which is why
- 9:47we have mobile friendly intake forms, using technology, making them feel like
- 9:52they are part of the practice, high impact appointment reminder calls with video
- 9:57texts, like investing time and energy in the...
- 10:00persuasion so that they're already pretty warm when they come in and then having a
- 10:06great playbook to do same -day conversion.
- 10:09So a lot of practices have leaned into same -day starts.
- 10:13If you have not, now is the time because this target needs to be converted that
- 10:20way.
- 10:21And then of course have great technology that follows them immediately after and
- 10:27doesn't let a lot of dust settle.
- 10:30you know, on that opportunity of a very short window after the exam, but then in
- 10:35the middle have to have flexible terms.
- 10:40You can have a nice fee.
- 10:41If you're doing the best technology in your neighborhood, I say this all the
- 10:44time.
- 10:45I want you to have the highest fee.
- 10:48I also want you to be the most affordable.
- 10:50And the only way to have both of those be true is by being flexible in our form of
- 10:56flexibility.
- 10:57We feel is the best.
- 10:57And that is the open choice concept.
- 10:59where the adult patient designs their payment plan based on what they can do.
- 11:06And what that ends up doing is it blends growth and cash flow and risk.
- 11:12And that's the ultimate formula that we see.
- 11:17Okay.
- 11:18Now you mentioned, you know, the fact that a lot of orthodontic practices have
- 11:24aligner patients pay that lab fee upfront.
- 11:27And I know that OrthoFi recently entered into an agreement with Align Technology
- 11:31that will allow OrthoFi clients to spread those Align lab fees over 10
- 11:35months.
- 11:36Can you talk a little bit more about what that means in practice and how practices
- 11:40can kind of implement that?
- 11:41Absolutely.
- 11:42Now I will say like OrthoFi already did a pretty decent job of helping them to
- 11:46balance that anyway, because we have carrot and stick incentives for patients
- 11:51to put higher downs that then create a blended balance cashflow that's really
- 11:56healthy.
- 11:56So we already were doing a pretty good job of solving for this, but it always did
- 12:01feel like, trust me, it'll happen.
- 12:04It'll work itself out.
- 12:05And some did and they, and they've,
- 12:07profited and thrive from that and some, you know, have hesitated even within our
- 12:11own customer group.
- 12:12Some, you know, lean into the system and some not.
- 12:15But now I'm excited because we actually have a real solution where I can tell you
- 12:21that your cashflow is going to be fine because if you are a qualifying practice,
- 12:26meaning that you're in good standing with Align, it's, you know, it is only to sole
- 12:31proprietor practices.
- 12:32So DSOs likely have a different, you know, different arrangement.
- 12:36But if you're in a sole proprietor practice and you're in good standing with
- 12:39Invisalign, you will likely qualify for what's called the FPT -10 program, which
- 12:44like you said, Alison, very simply is imagine that if you click your clin check
- 12:49and approve it, you will get the first of 10 invoices over the next 10 months that
- 12:56through ACH, and it does have to be ACH, but there's a huge ROI for that.
- 13:01that you will automatically be on a 10 month installment program.
- 13:06So what we've seen by running all the modeling numbers is that the average
- 13:11practice would see, even if they didn't increase their volume of Invisalign at all,
- 13:16they would see roughly a $45 ,000 cashflow benefit over the first 12 months, which is
- 13:24the equivalent of getting a zero interest small business loan.
- 13:29And so,
- 13:30That's pretty nice as a benefit.
- 13:32But I am also telling everybody, don't use it to go buy a boat, right?
- 13:38That cash is, I want you to reinvest it in the business.
- 13:42And if you have designs to increase your mix of aligners in your practice, and you,
- 13:47let's say, want to go from 20 % mix to roughly 40 % mix, this is the vehicle to
- 13:53get there.
- 13:54And if you do that, that $45 ,000 is actually more like over $70 ,000.
- 14:00that you would have as an advantage versus just trying to climb up the ladder
- 14:05yourself.
- 14:10Now for anyone who's interested, I know that you're going to be going deeper into
- 14:14this topic, or our topic, which was adult starts.
- 14:17And you'll be leading a mini masterclass as part of the AEO's Lunch and Learn
- 14:21program in the exhibit hall with consultant Tracy Martin, who you mentioned
- 14:25is really enmeshed in the Gen Z millennial behavior.
- 14:30So if you really want to dive in deeper on that, go check this out on Saturday, May
- 14:344th at 1150 a .m.
- 14:36in the exhibit hall.
- 14:37What else should potential attendees know about this session?
- 14:40Yeah, we're going at this as hard as it can possibly go.
- 14:44And for any of you on that has heard Tracy's content in particular, it will
- 14:49make you uncomfortable.
- 14:51It is definitely cutting edge stuff.
- 14:53There are a lot of, you know, staid notions in our industry that she's going
- 14:59to challenge hard.
- 15:01And, you know, for me to be saying that being I usually am that role.
- 15:06is saying something, but it will be very, you will not want to miss it.
- 15:11You will hear stuff that you are not hearing in other halls.
- 15:17Yeah.
- 15:18Oh, great.
- 15:19Well, Oliver, thank you so much for taking the time to speak to me.
- 15:21I know we're going to have you back again soon to talk on other topics, but I really
- 15:26appreciate you kind of delving into this.
- 15:27And I hope everybody goes and checks you out at the AO and maybe gets a little
- 15:31scared, but it's for a good it's for a good reason.
- 15:34All right.
- 15:36Thank you.
- 15:38Great.
- 15:39Thanks.
