- 0:22The supplement industry, unfortunately, is not very well regulated,
- 0:26and there are quite a lot of people who tend to profit by just going to a country like China,
- 0:34getting that particular substance like NAD+, or magnesium, or fish oil,
- 0:41and packaging it in such a nice way that they can actually market it to people
- 0:46in affluent countries like the US.
- 0:48And this creates a big problem because this space is not regulated and the claims
- 0:53that are made are often mostly driven by hype.
- 0:57So I'm not here to tell you that you should or shouldn't take a supplement.
- 1:01I approach supplements like any prescription medication.
- 1:05I need to know whether the person sitting across from me is going to benefit
- 1:10from the supplement and what the harms are if they're not going to benefit from it.
- 1:14And as always, if I can do it naturally, why would I use a supplement to treat my patient.
- 1:21And just because supplements are readily available and they're inexpensive,
- 1:25it doesn't make them better or worse than other alternative options.
- 1:30The way this industry works is a good business person imports that supplement
- 1:37from another country, they hire a marketing agency, and they publish a bunch of data,
- 1:43they post a bunch of questions on Reddit and Facebook groups,
- 1:47they pay a bunch of doctors to push that particular product,
- 1:50and they will actually give them real research papers that are quite convincing.
- 1:56These research papers often come out of countries where the research studies
- 2:02are a little bit less regulated.
- 2:04And of course, these research studies are always very positive.
- 2:07Unfortunately, they're measuring not actual health outcomes,
- 2:11they're measuring what we call surrogate outcomes.
- 2:15They're basically saying, look, if I give you this NAD supplement,
- 2:18this one molecule in your body increased.
- 2:21Or if I give you this protein supplement, your urine product of protein increased.
- 2:28The problem is that's not the way the human body works. We always want to look
- 2:32at actual results from a study, from the human body in a study,
- 2:37to make sure that that is actually the result that we're after.
- 2:40So if I give somebody fish oil to lower their cholesterol, I don't want to just
- 2:44see their cholesterol go lower.
- 2:46I want to see their heart attack risk and stroke risk decrease.
- 2:51And that's how a really good study is designed. And a bad study,
- 2:54of course, is designed by just
- 2:56showing, hey, look, I give you fish oil and your triglyceride is lower.
- 2:59Well, that's not enough. That's what we call a surrogate marker.
- 3:04And of course, there's tons of supplements out there for berberine and for hair
- 3:09loss and for energy and for cortisol levels. But are they pure?
- 3:14Are they coming from a good source? Even prescription medications that are generic
- 3:18have very little regulation on them.
- 3:22Fortunately, the FDA does oversee these medications, but there's nobody on site
- 3:27to actually take a look at the manufacturing process, which is why even in this
- 3:32highly regulated industry,
- 3:34we see quite a few generic prescription drugs come up with really bad impurities.
- 3:41When it comes to choosing a supplement, the best thing that you can do is look for solid research.
- 3:48Follow people, experts online, who constantly are taking both sides of the argument
- 3:54to assess a supplement's efficacy.
- 3:57If you hear a doctor, an MD, DO, naturopath, any health expert,
- 4:02tout the brilliance of a supplement and how effective it is,
- 4:06chances are they're not giving you the nuanced picture
- 4:09of that supplement and chances are they're not
- 4:12painting the right picture and chances are they're profiting well
- 4:16from giving out that supplement one of
- 4:19the things that you can do of course is to look for research studies yourself
- 4:23these studies should be should have a good power meaning there should be enough
- 4:28people enrolled in that study the people who are enrolled should be double-blinded
- 4:33meaning that the people who are giving these supplements and the placebo don't
- 4:37know who's getting what.
- 4:38And of course, there's tons of other criteria for a good, properly designed study.
- 4:43And sadly, if you go do the search, you're going to see that there's not a lot
- 4:47of such studies available.
- 4:49Does it mean that just because a study is not there, the supplement doesn't work? No, of course not.
- 4:53A supplement can still be incredibly effective, even if the study is not there.
- 4:59But then we have to use other ways of assessing that supplement's effectiveness.
- 5:05And we have to understand that just because you give a supplement,
- 5:08it doesn't mean that it's going to do in the body what we expect it to do.
- 5:13These are the subtleties, the nuances of how the human body works.
- 5:18It's the same thing with prescription drugs. Sometimes I'll give a patient a
- 5:20prescription drug and it doesn't work.
- 5:22And I give it to another person and it works great because our genetics,
- 5:27our molecular chemistry, everything is very subtle and nuanced.
- 5:32And all of those things are just not easy to tease out in all of the research studies.
- 5:37Whenever you have a health concern and you feel like you need a supplement to
- 5:41address it, first ask yourself, what are some lifestyle ways that you can attempt
- 5:45to help improve that particular health outcome?
- 5:49Why? Because lifestyle changes have been proven over and over and over again
- 5:54to be incredibly effective.
- 5:57So the lifestyle choices include stress management, building good relationships
- 6:01with others, having a good self-talk inside your brain, getting enough sleep,
- 6:06exercising, good nutrition, meditation, all of these things.
- 6:10These have been proven, proven to be effective.
- 6:13Now, if that's not enough, then we can start looking at particular nutrients
- 6:18in your food that you can maximize.
- 6:21And finally, if that's not enough, absolutely. A good supplement from a good
- 6:25third-party tested company can be a great, great asset.
