- 0:04>> Dr. Terry Simpson: We talk a lot about aging, aging with good health,
- 0:07aging with a good health span and longevity. And
- 0:10we talk a lot about supplements that don't work or
- 0:12to diets that are rather silly. But today, the
- 0:15biggest anti aging story isn't in a pill or a cold
- 0:19plunge or a fasting app. Uh, it's in your
- 0:21metabolism. And it starts with a class of drugs we
- 0:24once thought were just for diabetes, the GLP1
- 0:28receptor agonists, you know, Ozempic, Zepbound,
- 0:31etc. These medications are now being primarily
- 0:34used for obesity. And they're doing remarkable
- 0:36things, not only reducing weight, but reversing
- 0:39many of the diseases that travel with and maybe
- 0:42because of obesity. And since obesity clearly
- 0:46shortens your health span, it's not surprising
- 0:48that these drugs are now being looked at as tools
- 0:51to improve health. Spanish the years you live
- 0:54well, not just lengthening a, uh, miserable life.
- 0:58But it turns out there is much more to these drugs
- 1:01than weight loss. These drugs have anti
- 1:03inflammatory effects, cardiovascular protection,
- 1:06reductions in stress physiology, and emerging
- 1:10evidence they may delay cognitive decline. And
- 1:14many of those benefits appear to be independent of
- 1:17weight loss. So the idea that GLP1 drugs, and now
- 1:21triple agonists like pitoutatride might represent
- 1:24a new class of anti aging medicine isn't hype.
- 1:27It's a serious scientific question. Even Peter
- 1:31Attia, whose clinic focuses almost exclusively on
- 1:35longevity and health span, has discussed using
- 1:37GLP1 drugs at lower doses in select patients to
- 1:41support metabolic health and long term health.
- 1:43Spanish this week, Eli Lilly released striking
- 1:46phase three data on their newest compound,
- 1:49retatrutide, a triple hormone drug that isn't just
- 1:52helping people lose weight. It may be resetting
- 1:55the biology of aging. Today on 4Q, we're going to
- 2:03make sense of the madness of Healthspan and GLP1.
- 2:06I am, um, your chief medical explanationist, Dr.
- 2:09Terri Simpson, and this is 4Q Fork University,
- 2:12where we bust myths, make sense of the madness,
- 2:15and teach you a little bit about food and
- 2:17medicine. When we talk about longevity, we're
- 2:24really talking about metabolic stability, blood
- 2:27sugar control, lower inflammation, lower oxidative
- 2:31stress, the ability of cells to repair. And GLP1
- 2:35agonists touch all of these systems. They were
- 2:39designed to mimic a gut hormone, the glucagon,
- 2:42like peptide 1, which signals satiety and improves
- 2:46glucose regulation. But over the last decade,
- 2:49we've learned they do far more than lower
- 2:51hemoglobin A1C and make diabetes better control.
- 2:55They reduce systemic inflammation, they improve
- 2:58mitochondrial efficiency, they lower oxidative
- 3:02stress, and they improve vascular function. The
- 3:05same biology that reduces appetite may also make
- 3:08cells behave younger. Enter UH retatrutide. In
- 3:14Lilly's Phase 3 Triumph 4 trial, participants lost
- 3:17nearly 29% of their body weight over 70 pounds on
- 3:20average, and saw UH major improvements in
- 3:22inflammation, blood pressure, lipids and joint
- 3:25pain. But weight loss is just the surface.
- 3:27Retatutride targets three key pathways. There's
- 3:31the GLP1 pathway, which reduces appetite and
- 3:34inflammation. There's the GIP, which improves
- 3:37insulin sensitivity and nutrient handling. And
- 3:40this one adds a glucagon, which increases energy
- 3:43expenditure and fat oxidation. If GLP1 calms the
- 3:47system and GIP balances it, glucagon fights a
- 3:51controlled metabolic fire, keeping metabolism from
- 3:54slowing down as weight is lost. That's not just
- 3:57slimming, that's metabolic rejuvenation. Let's
- 4:01talk about inflammaging. Aging isn't just wear and
- 4:04tear. It's driven by chronic low grade
- 4:07inflammation, where gerontologists call
- 4:09inflammaging. Inflammaging fuels heart disease,
- 4:13strokes, type 2 diabetes, fatty liver, arthritis
- 4:16and likely cognitive decline. In the Triumph 4
- 4:20trials, retatutride decreased CRP, triglycerides
- 4:25and blood pressure. Classic anti aging biomarkers.
- 4:29Less inflammation means fewer senescent cells,
- 4:31healthier blood vessels and better organ function.
- 4:35Let's talk about heart disease and GLP1s. And if
- 4:38you want to talk about aging, you have to talk
- 4:40heart disease because nothing ages you faster than
- 4:43a heart attack. Across multiple cardiovascular
- 4:47outcome trials, GLP1 receptor agonist, which is
- 4:50all these new drugs, have shown a 13% reduction in
- 4:53cardiovascular death, a uh, 9% reduction in non
- 4:57fatal heart attacks or myocardial infarctions. In
- 5:01the so called leader trials, liraglutide reduced
- 5:04cardiovascular mortality by 22%. Now semaglutide
- 5:09and lugotide show similar reductions, enough that
- 5:12the FDA now recognizes certain GLP1 drugs as
- 5:16cardiovascular red risk reducing therapies in
- 5:19patients with type 2 diabetes and established
- 5:21heart disease. These benefits are not just about
- 5:24sugar control. Turns out the GLP1s lower blood
- 5:28pressure, reduce inflammation, improve endothelial
- 5:31function. That's the cells that surround the
- 5:34vessels, reduce oxidative stress and decrease what
- 5:37we call RAS activation, which means at the
- 5:40cellular level they protect heart cell muscles
- 5:43from multiple forms of cell death and enhance
- 5:45autophagy and mitophagy. That means the heart is
- 5:49cleaning up old cells and old mitochondria. They
- 5:53don't dramatically treat heart failure the way
- 5:55SGLT2 inhibitors do, but they are safe and reduce
- 5:58atherosclerotic risk. Avoiding a heart attack is
- 6:01one of the most powerful anti aging interventions
- 6:04and powerful ways to increase your health span.
- 6:08People who live a long time aren't free from heart
- 6:10disease, but what they are free from is heart
- 6:13disease for a little while. Meaning healthspan
- 6:16puts it off.
- 6:18Now, let's talk about the brain. Let's be careful
- 6:20here. GLP1 drugs do not reverse dementia. They do
- 6:25not improve cognition once dementia is
- 6:28established. Recent trials in people with
- 6:31diagnosed Alzheimer's disease have shown no
- 6:34meaningful cognitive improvement. And that's
- 6:37critical for me to tell you that clearly the
- 6:39promise of GLP1 drugs is prevention and delay, not
- 6:44cure. And in large observational drugs, GLP1 users
- 6:48had roughly 33 to 45% lower risk of developing
- 6:51dementia compared with other diabetic drugs. The
- 6:552025 Journal of the American Medical association
- 6:57of Neurology had a study that showed uh, a 33%
- 7:00lower risk of Alzheimer's and related dementias,
- 7:03especially in people who have cardiovascular
- 7:05disease. Randomized trial meta analysis show a
- 7:09real but m more modest protective signal stronger
- 7:12than what we actually see with other types of
- 7:14drugs. Mechanistically, GLP1s reduce
- 7:17neuroinflammation. That's inflammation of the
- 7:20brain. They improve insulin signaling in the
- 7:23brain, they enhance vascular health in the brain.
- 7:26They perform neurogenesis and they may reduce
- 7:30amyloid and tau pathology. Age matters. The
- 7:34benefit appear stronger when started earlier,
- 7:37before neurodegeneration is established. But our
- 7:40goal here is not immortality. The goal is to push
- 7:43dementia far enough into the future that many
- 7:46people never reach it. That's healthspan.
- 7:50Let's talk about ultra processed food and brain
- 7:53aging. This may be the real Ultra processed food
- 7:57isn't a terribly difficult concept, but you
- 8:00probably get the general idea. These foods hijack
- 8:03the brain's dopamine reward system, increasing
- 8:05wanting while reducing pleasure. They're soft,
- 8:08fast, hyperpalatable and engineered to be consumed
- 8:11before the gut can signal fullness. Ultra
- 8:14processed food intake is linked to higher
- 8:16inflammation, lower gray matter density, worse
- 8:19metabolic health and faster brain aging. They
- 8:22disrupt that gut brain access, damage the
- 8:24microbiome and worsen insulin signaling in the
- 8:27brain. GLP1 drugs are in many ways repairing
- 8:31damage caused by a food environment designed to
- 8:34exploit our biology. What about the Mediterranean
- 8:37diet and alcohol? Well now, here's the empowering
- 8:40part. If there is one dietary pattern with the
- 8:44strongest evidence for protecting the heart and
- 8:46the brain, it's the Mediterranean diet.
- 8:49Vegetables, legumes, fruit, whole grains, olive
- 8:52oil, fish. Minimally ultra processed food. This
- 8:54pattern reduces inflammation Supports the
- 8:56microbiome, um, improves vascular health and slows
- 8:59cognitive decline. GLP1 drugs calm the biology.
- 9:04The Mediterranean diet keeps it calm. And one more
- 9:08hard truth, we've talked about this before.
- 9:10Alcohol. The idea that alcohol protects the brain
- 9:14has never been held up. Even moderate drinking is
- 9:17associated with higher dementia risk, worse sleep,
- 9:19hippocampal injury and increased neuro
- 9:22inflammation. If your goal is to protect your
- 9:25brain, thus alcohol is better. No, alcohol is
- 9:29best. So what does this really mean now? Aging
- 9:32isn't about the years, it's about systems. GLP1
- 9:36drugs support metabolism. The Mediterranean diet
- 9:39supports biology. Avoiding alcohol protects the
- 9:42brain and the heart and the kidneys and the eyes,
- 9:46movement and sleep. Lock it in. If we delay heart
- 9:49disease, dementia and disability long enough, many
- 9:52people will never live long enough to experience
- 9:54them. That's not cheating death, that's winning.
- 9:58At Healthspan. If you're thinking about GLP1
- 10:01drugs, work with a qualified obesity or metabolic
- 10:04health specialist. Not one of those peptide mills.
- 10:06Avoid research grade compounds sold online. That's
- 10:09lab reagent. That's not medicine. And if you want
- 10:12to go deeper into longevity medicine and
- 10:14Mediterranean eating, metabolic health and
