- 0:00Joey Wahler (Host): With GLP one, medications becoming so popular, who
- 0:04should consider Bariatric Surgery?
- 0:06Our guest is Dr.
- 0:08John Scott, a bariatric surgeon.
- 0:14This is Flourish, a podcast from Prisma Health.
- 0:18Thanks so much for joining us.
- 0:19I'm Joey Wahler.
- 0:21Hi there, Dr.
- 0:21Scott.
- 0:21Welcome.
- 0:23Dr John Scott: nice to be here.
- 0:24Joey Wahler (Host): Great to have you with us.
- 0:25We appreciate the time.
- 0:26So first, how has the rise of GLP one meds like Ozempic of
- 0:32course, and other's, changed the conversation around obesity treatment?
- 0:37What are you hearing?
- 0:38Dr John Scott: So one of the most important things about the new
- 0:41GLP one era, it is changed how society thinks about obesity.
- 0:45You know, for decades obesity was framed as this failure of willpower,
- 0:49but now we have medications that clearly demonstrate that obesity is more about
- 0:53the biology of appetite regulation and gut hormones and energy balance.
- 0:58And that's really shifted the conversation, from the blame
- 1:01from the patient to biology.
- 1:04And the second major change about obesity treatment, it's finally being discussed
- 1:08alongside other chronic diseases.
- 1:11Nobody expects a patient with like hypertension or cancer to manage
- 1:15their condition with willpower.
- 1:17so we're starting to apply that same framework to obesity, for, doctors,
- 1:21the conversation has expanded.
- 1:24Instead of having a, choice of just between, surgery and lifestyle, we now
- 1:28have a whole spectrum of therapies.
- 1:31So there's behavioral treatment, medications like
- 1:33GLP one and metabolic surgery.
- 1:36The real progress is that we can start matching the right treatment to the
- 1:39right patient instead of pretending that there is just one single solution.
- 1:44Joey Wahler (Host): Gotcha.
- 1:45And that being said, in this age of these highly effective GLP one drugs,
- 1:50how should patient's and clinicians alike think about the role of bariatric
- 1:55and metabolic surgery these days?
- 1:58Dr John Scott: I think the key point is that, the difference between GLP
- 2:02one medications and bariatric surgery is that they're not competitors.
- 2:05They're different tools that treat the same disease.
- 2:08Now, GLP ones are very powerful.
- 2:10They're powerful pharmacologic therapy that can help patient's
- 2:13lose meaningful weight.
- 2:15And bariatric surgery, on the other hand, remains the most effective and
- 2:19durable treatment that we have for severe obesity and metabolic surgery.
- 2:22The way that I frame it for patient's is this, medications and surgery
- 2:26exist on the same treatment continuum.
- 2:29For some patient's, medication will be the right first step.
- 2:32But for other's, especially for patient's with severe obesity, diabetes,
- 2:37obesity-related complications, surgery may offer a more durable metabolic reset.
- 2:42The goal is not medication versus surgery.
- 2:45The goal is the right therapy for the right patient at the right time.
- 2:50Joey Wahler (Host): So let's delve into that a little bit more.
- 2:53Who would you say is an ideal candidate for GLP one therapy, And what are
- 2:58the expectations that patient's should realistically have regarding
- 3:02weight loss and using them long-term?
- 3:05Dr John Scott: So for patient's that want a non-surgical approach to
- 3:09treatment, I think GLP one therapy is, the perfect option for them.
- 3:14if you look at clinical trials And in real-world practice, patient's often lose
- 3:18about maybe 10 to 20% of their body weight depending on which medication you use.
- 3:23And that can have a significant, I impact on some of the diseases
- 3:26like diabetes, hypertension.
- 3:29fatty liver disease, sleep apnea.
- 3:31But expectations are important.
- 3:32These medications were designed to treat a chronic disease, And that
- 3:36means you have to take them long-term.
- 3:38When patient's stop the medications, weight regain is common because
- 3:41the biology hasn't changed.
- 3:44so that's what I tell patient's.
- 3:46these work best, but they're viewed the same way that you view
- 3:49taking a blood pressure medication or a medication for cholesterol.
- 3:53They are a long-term therapy for a chronic condition.
- 3:56As far as surgery's concerned, surgery is a much more durable,
- 4:01long lasting option for patient's.
- 4:04Joey Wahler (Host): so when is GLP one therapy?
- 4:07Not an option because of other factors, side effects, access,
- 4:14cost, or maybe the challenges of adhering to what needs to be done.
- 4:20Dr John Scott: The biggest barriers that we see for patient's when it comes to
- 4:22GLP one therapy are cost and access.
- 4:25Many insurance plans don't cover anti BC medications, And the
- 4:29out-of-pocket cost can be substantial.
- 4:31And there's also an issue of long-term adherence.
- 4:33These medications require long-term ongoing use, and some patient's
- 4:37prefer a more definitive treatment.
- 4:39So when patient's cannot tolerate the medications or can't access it
- 4:43or simply just prefer a different approach, metabolic surgery
- 4:46becomes a very important option.
- 4:48There was a recent study that showed that almost 50% of patient's that
- 4:51were placed in GLP one therapy.
- 4:53We'll discontinue use of those medications within the first year.
- 4:57Joey Wahler (Host): Interesting indeed.
- 4:57So which patient's would you say, doctor, better serve them by
- 5:02metabolic and bariatric surgery rather than just medication?
- 5:07Dr John Scott: So recently, the indications for bariatric
- 5:09surgery, were updated.
- 5:11Patient's that have A BMI over 30 with comorbidity related to weight, or
- 5:16patient's that have A BMI greater than 35 are potential candidates for bariatric
- 5:20surgery and would show the most benefit.
- 5:23Surgery is very powerful for patient's that have type two diabetes,
- 5:27metabolic syndrome, or significant obesity-related complications.
- 5:31In these cases, surgery does more than just produce weight loss.
- 5:34It fundamentally changes the metabolic physiology and can
- 5:37lead to remission of diabetes.
- 5:39And other diseases related to weight.
- 5:42Another group of patient's, that have benefits are those that have
- 5:45struggled for many years with multiple different treatments,
- 5:48multiple different medications, and want a more durable intervention.
- 5:52So when we talk about surgery, we're not talking about a last resort.
- 5:55We're talking about the most effective therapy that we have for certain
- 5:58patient's with metabolic disease.
- 6:01Joey Wahler (Host): Gotcha.
- 6:02And so naturally doc people joining us, I'm sure, wondering more so
- 6:08than anything else about outcomes, results at the end of the road, if.
- 6:14There is such a thing here.
- 6:15How do outcomes compare then between GLP one meds and bariatric surgery in terms
- 6:22of things like weight loss, durability, metabolic disease, remission, and of
- 6:27course overall quality of life, period.
- 6:30Dr John Scott: In real-world studies, GLP medications typically
- 6:33produce anywhere from 10 to 20% total body weight loss, depending on
- 6:36the medication that you're taking.
- 6:38And if you continue with treatment, bariatric surgery, once again, varices
- 6:43between which, metabolic procedure you have produces about 25 to 30.
- 6:48Total body weight loss, and most importantly, much of that weight loss
- 6:52can be maintained long-term after your surgery from a metabolic standpoint.
- 6:57Surgery also has very high rates of type two diabetes, remission,
- 7:00and significant improvements in cardiovascular risk factors.
- 7:04Quality of life often improves with both therapies.
- 7:07What patient's care about most, mobility, energy, metabolic
- 7:11health longevity can substantially improve with either approach.
- 7:15So the difference isn't what works And what doesn't.
- 7:18but what's been shown substantially is that surgery currently provides
- 7:23the most powerful And the most durable metabolic effect of the two options.
- 7:28Joey Wahler (Host): so.
- 7:29What about GLP one meds and bariatric surgery working together?
- 7:34Medication before surgery or after surgery, or perhaps as part
- 7:38of long-term weight maintenance?
- 7:40Dr John Scott: Absolutely.
- 7:41One of the most exciting things about the current area is
- 7:44that we can combine therapies.
- 7:46Some patient's benefit from medication before surgery to
- 7:49help reduce operative risk.
- 7:51Other patient's use medications after surgery to treat weight recurrence,
- 7:54or optimize metabolic control.
- 7:56We're increasingly thinking about obesity the way we think about
- 7:59cancer or cardiovascular disease by using combination therapy.
- 8:03So rather than asking whether medications replace surgery, the better question
- 8:07is how can we integrate medications and surgery together to achieve the
- 8:10best long-term outcomes for patient's?
- 8:13Joey Wahler (Host): Misconceptions, of course, are always a factor here with
- 8:17people reading things on the internet that may or may not be as accurate as possible.
- 8:23What are the misconceptions you find that patient's commonly have about
- 8:28bariatric surgery And this GLP one era, and how do you help patient's make
- 8:33individualized evidence-based decisions rather than poorly informed ones here?
- 8:39Dr John Scott: One of the most common misconceptions about metabolic
- 8:42surgery is that it's somehow either outdated or an extreme treatment.
- 8:46Now that medications exist In reality, metabolic surgery is probably one
- 8:50of the most studied operations in modern medicine and remains the most
- 8:54effective treatment for severe obesity.
- 8:56another misconception that people have about, these surgeries, that
- 8:59it's purely about weight loss.
- 9:01In truth, it fundamentally, changes the metabolism of a patient.
- 9:05It alters hormones.
- 9:07It changes the way your body signals about appetite.
- 9:10It alters your glucose metabolism in ways that medications
- 9:13alone cannot fully replicate.
- 9:15The way that I help patient's think about This is obesity
- 9:18is a complex chronic disease.
- 9:21Some patient's do well with medications.
- 9:24Other patient's will do better with surgery.
- 9:26Some patient's will do better with a combination of both.
- 9:29The most important step is not choosing a specific treatment first.
- 9:33It's making sure that patient's are evaluated by clinicians who can offer
- 9:36the full spectrum of therapy and help them choose the best approach
- 9:39that fits their weight loss goals.
- 9:41Joey Wahler (Host): Couple of other things for you.
- 9:43One being, how important is it for experts here like yourself to remind
- 9:48those, using either of these routes that.
- 9:52They're going to have work to do for the remainder of their lives,
- 9:55regardless of which option they choose.
- 9:58Right.
- 9:59You're gonna have to change your ways when it comes to exercise
- 10:03and diet one way or the other.
- 10:05Yes.
- 10:06Dr John Scott: I think that's a true statement.
- 10:07there's no doubt about it that, you know, behavioral.
- 10:10changes, in combination with these different options like surgery
- 10:15and medications will definitely enhance, weight loss and some of
- 10:19the resolution of the comorbidities that we see, with these therapies.
- 10:24This is a total lifestyle change and regardless of which route you decide
- 10:28to go with medication or surgery.
- 10:30We are trying to make patient's.
- 10:33Able to eat healthy, be more active, and live a full lifestyle.
- 10:39Joey Wahler (Host): And then in summary here, doctor, having said all of the
- 10:42above, what's the best way for those joining us to decide inevitably,
- 10:47which weight loss route is best?
- 10:49For them.
- 10:50Dr John Scott: The best way to get on the path to this healthy lifestyle is to
- 10:53have a conversation with your physician.
- 10:56Physicians can prescribe GLP one therapies themselves, or they can refer
- 10:59you to a comprehensive center that has access to all of these treatment
- 11:03modalities, like surgery and medications.
- 11:07Joey Wahler (Host): Well, doc, no question, it's an area of medicine
- 11:10that's growing in terms of participation and a thirst for knowledge out there
- 11:15on the part of patient's, so very important to discuss it right here.
- 11:19We appreciate all your expertise, folks.
- 11:22We trust you are now more familiar with bariatric surgery in this GLP one era.
- 11:28Dr.
- 11:28Scott, keep up all your great work and thanks so much again.
- 11:31Dr John Scott: Appreciate it.
