- 0:00And welcome to ask the doctor. And the doctor is Carrie Williams with med
- 0:03Club bartles Direct primary Care. Some folks have heard it, but they don't
- 0:07know exactly what it means, but it pretty much means what it says.
- 0:10It does. It means that that passion contracts directly with that position for primary
- 0:15care. And you do that at med Club Bartlesville, right here in Bartlesville,
- 0:19across from Jimmy's Egg on seventy five. Yes, and we've been doing
- 0:22it in Bartlesville since twenty seventeen. Now. This is kind of a cool
- 0:26model. It's something that got started really up in Kansas, didn't it.
- 0:29It did, Yeah, and it started kind of when Obamacare was coming about
- 0:34and people were looking for different ways to affordable see a doctor and make it
- 0:40work not only for the patient, but for the doctor and for the health
- 0:42of a community. As a matter of fact, Yes, and doctors are
- 0:45a lot happier because they're not dealing with a lot of insurance companies and administration.
- 0:51No. In fact, you have one monthly fee and that takes care
- 0:55of a lot of things. It doesn't take care of everything, but by
- 0:57all he takes care of a pretty good swath of it and you know what
- 1:02to expect. There's transparency, Oh yeah, very much. So it's all
- 1:04right there in black and white. Plus you'll tell them too. Yes.
- 1:07And also at mcclub we offer discounted labs and prescriptions and you know how much
- 1:12it's going to cost before you leave. My goodness sakes, So all of
- 1:17the mystery is taken out and you can get down to medicine. Yes,
- 1:22Oh, how rare is that? That is very rare? In our medical
- 1:26comment today? Oh my goodness. You know, I was taking a look
- 1:30at some of these facts figures here. Whenever I need a medical constitution or
- 1:37looked at it for anything I call, I'm either there that afternoon or the
- 1:42next day, which try very hard, ninety nine times out of a one
- 1:46hundred, but going the traditional route, we got some late times that are
- 1:51kind of I wasn't aware, and I'm glad that I didn't go that route.
- 1:55Well, in traditional practices, yeah, there's nearly a thirty day wait
- 2:00to get an appointment, and as a new patient it will be even longer.
- 2:04And if you go to a specialty clinic, it'll be over thirty days,
- 2:08more like thirty five or forty days to get an appointment because there's not
- 2:13enough doctors in the United States. Well, you know, I just took
- 2:16a look at a chart there, and you're absolutely correct. When we talk
- 2:21about primary care workforce projections, we start out with what looks like a lot
- 2:27of doctors, but in comparison to how many people there are, the ratio
- 2:31pardner my grammar ain't that great, No, sir, it's not. In
- 2:36primary care physicians. They're basically what populates rule Oklahoma, rural America, and
- 2:45there's not enough of them. And because our population as a nation is growing,
- 2:52so you ratio it and doctors can see so many patients today and that
- 2:58limits how many patients they have. Indeed, and when we talk about the
- 3:05demand for doctors, it's it's almost off the scale and it all comes down
- 3:09in numbers. But it's nice to know that you're here and you're abailable.
- 3:15Yes, And you know, because there's a decrease ratio physicians of patients in
- 3:23the United States because of the increase in our populations are filling in with other
- 3:28medical providers like nurse practitioners and physicians assistants. Now I look at family positions,
- 3:37Yeah, maybe you get six percent a year coming into the field,
- 3:40but the increase is thirteen percent by thirty. Wow, kind of losing ground
- 3:47there. We are losing ground well goodness. As a general internal physicians,
- 3:53it's almost double. That's a that's crazy. And the ratios here our nurse
- 4:00practitioners in primary care there seems to be a bunch of those. But the
- 4:03increase in demand is only sixteen percent. So we've got like a little crossing
- 4:09there the of the of the diagram as far as supply and demand. But
- 4:14it all depends on what you're looking for it does, and what that patience
- 4:16goals are. And one of the questions I asked my patients with age,
- 4:20do you think you're going to get healthier? And Andrews absolutely not no.
- 4:27And you think your medical conditions are going to get less complicated with age?
- 4:32With age, well you can agree. And so you need someone with the
- 4:38experience that has has spent a lot of time with patients and it's seen a
- 4:43lot of patients. By the time we complete residency, which in family medicine
- 4:46is three years, we've seen a lot of patients. We've worked a lot
- 4:53of hospitals, We've worked a lot of clinics, and I did a fellowship
- 4:58in emergency medicine and also can do that and boards ordified in hospice and pality
- 5:04medicine. It's so seeing that many patients having much experience, you can kind
- 5:13of weed through the complications a lot more quickly. You can now death rates.
- 5:19I'm just taking a look at that because you know, I'm sixty four
- 5:25and I didn't quite really care about this until he turned like sixty two.
- 5:29I don't know why I was. At sixty two was like, hey,
- 5:30I better check up on this. But it's amazing here in the United States.
- 5:34You think you're going to live forever, Well, none of us get
- 5:36out of here alive. But there are hard facts and hard numbers and real
- 5:42reasons why you should really be in good hands of a physician knows what's going
- 5:46on. Well, you know, with age, you know ourselves get damaged,
- 5:53Yeah, just by injury and stress and DNA mutations, and our body
- 5:58has a lesson to ability to correct those problems, so the disease processes will
- 6:04creep up on you, and you know, the deterioration of the organs.
- 6:11In the United States, cardiovascular disease is the number one cause of death.
- 6:15Cancer is right behind it. So those two together of all the death in
- 6:19the United States, contribute to over forty percent. And a lot of times
- 6:25we can kind of control the encroachment of that. We can we can start
- 6:30doing preventiveness. And a lot of people will not address their health concerns until
- 6:36they have a catastrophic event, they have their heart attack, their stroke,
- 6:41their cancer. They won't keep up with their maintenance. They will spend more
- 6:46time on their car, changing the oil, doing the maintenance on their car
- 6:49than they will themselves. And then when a catastrophic event happens, then it's
- 6:55expensive. Wow. Wait, that is that? That is something else?
- 6:58You know, when we think that our body is what we have that's gonna
- 7:02take us to where we need to go, that the folks wait to think
- 7:05about their conveyances more than you know, what's keeping them alive, what's keeping
- 7:10you you? And we've got to change the mind shift here on this.
- 7:13It's hard, yeah, because you have to come to groups with your own
- 7:17mortality, and most people don't like doing it. No one likes doing it.
- 7:23Let's do it that way. But it's an inevitable event and a lot
- 7:28to die. So let's see if we can keep the misery out of it,
- 7:31if we can anticipate it, if we can ward off, we can
- 7:34catch things early and take care of them. Now. The model you have
- 7:38is ideal for what catching things early and to get ahead of things. Because
- 7:45with the way you're set up, one fee, monthly fee, you can
- 7:48get checked in just about any old time we want making the appointment. You're
- 7:53usually there in the day or half a day, and you give your patients
- 7:59anywhere from a half hour to an hour of your time each time they're there
- 8:03in order to get things explained, examined properly and help them help themselves in
- 8:07getting ahead of the curve of what's coming ahead. Any education of it is
- 8:11a lot and the understanding that the patient has of the medical system, the
- 8:18system, the system, and people go, well, that's not right,
- 8:22they shouldn't do it that way. I'm sorry they do do it that way,
- 8:26and we have to learn to live with it. But we have to
- 8:28also learn how to manage it. Wow and what I also have a degree
- 8:35in a master's in healthcare administration to try to understand this and to understand the
- 8:43system and break it down and put it into final English for us common folk
- 8:48to how this all operates and how it's going to affect you. Absolutely,
- 8:52And for a doctor to speak English, let alone Oki English, it's difficult.
- 8:58We've spent a lot of years learning to speak that high pollutant vocabulary.
- 9:03But the thing is is that when people are looking for answers, they're looking
- 9:07for direct response, in the very direct communication. And with med Club Bartlefield,
- 9:16everything is based on communication and very direct communicating. Yes, and we
- 9:20have several means of communication, not only a face to face appointment time,
- 9:22but we text an email. I do telephone appointments. There are time times
- 9:28I'm going to really almost manage to come in because I need to see something,
- 9:33examine something. But a lot of the questions, a lot of the
- 9:35labrardies and things are done via the telephone or face to face something like that.
- 9:43Also, some of medicine isn't what it appears. Do tell things like
- 9:48metabolic problems, they don't show up until there's a problem. We can kind
- 9:54of anticipate them. Also, as a primary care physician, I'm a gatekeeper.
- 10:00Most of the information comes through me or to me, and so getting
- 10:05different specialists their information and getting hospital information plus my information and being able to
- 10:11put a canulative together and see trends, see reasons, see timings of problems
- 10:20is really important. It is it truly is. Now how can folks get
- 10:26started with becoming a member of med Club, artilesfelt, We'll go to our
- 10:31website at www dot medclub dot clinting. That website will answer almost every question
- 10:39you may have about direct primary care and med Club. Also, there's a
- 10:43page on there to sign up as a patient, and my electronic medical record
- 10:50will let me know that someone new has signed up and we will get back
- 10:54with you. Very good. And you offer an opportunity for folks to come
- 10:58in once they go through that portion I mean, and just ask questions and
- 11:03that's great. A free there is an appointment available that we don't provide medical
- 11:09advice, but we answer the questions about direct primary care in mid Club.
- 11:13Sure, so you know everything, what to expect and why to the best
- 11:18of our ability, the best of our ability. Now with this one month
- 11:22fee, that is all kinds of checkups, that's visits, that's consultations and
- 11:28everything else. However, you do provide some other service at reduced costs,
- 11:33like labs them right there, Yes, in our office we have lab pick
- 11:39up every day, blood work every day we're open and we do blood work
- 11:45you'reine, and some other things like pap smears and the contract with the lab.
- 11:52We also have discounted and that is gravely discounted. And we also have
- 11:56discounted prescriptions. As direct primary care doctor, we're going to buy in group
- 12:01with other direct primary care practices and can buy purchase prescription medications that are very
- 12:07discounted rate and we don't have the markup of the retail pharmacies. Another bonus
- 12:13for the patient. Yes, and for I have some patients who are are
- 12:20snowbirds who go to different states. We mail them prescriptions that are a small
- 12:24fee. So it's the one stop shop works on so many different levels for
- 12:31so many different people. It does, and the main thing is accessibility and
- 12:37time. Time is everything. It's fun. It's something that if you're a
- 12:43patient, you want someone who's able to take the time to explain exactly what's
- 12:48going on with you in a way that you'll understand it. And you can't
- 12:52do that in five minutes. I've been on the receiving end of that,
- 12:56and if it doesn't work well and being able to explain having resources there.
- 13:03Some people are very visual. They need to see pictures and understand it.
- 13:09They need to know where certain organs are located on their body and to understand
- 13:13what certain things mean. If I poke you here and it shows up over
- 13:18here, it means so and so. Yeah. So it's great that we
- 13:24have that at the time to get that that will explain. And I'm going
- 13:28to tell you folks full transparency. I'm a patient of doctor Williams It she'lldn't
- 13:31even haul out the grave anatomy and and they'll show you exactly what's going on,
- 13:37and she'll tell you where to find other information so you can do some
- 13:41homework on your own and really absorb it in your own way and in your
- 13:45own time as well. On top of the long but that's the key to
- 13:48medicine is understanding and it has been an enigma to so many people, and
- 13:54they use great big words and getting past that. I send almost every new
- 14:01patient, I will send some type of link to a website of some kind
- 14:05for them to start their learning. Because a lot of medicine has turned into
- 14:11online college classes where you have to do a lot of it yourself to get
- 14:15caught up and stay caught up any things is get medical advance has changed that
- 14:22is pertinent to that patient. Sometimes out of the blue, you'll get something
- 14:24from me that is relevant to you, and that's because the doctor really takes
- 14:31the time to get to know each and every individual patient. You don't have
- 14:35thousands of patients like other clinics do that. Club Barnelsfield. You have a
- 14:39set number and that's by design. Yes, we limit our number of patients
- 14:43at seven hundred and we're still accepting new patients. And in my old practice
- 14:48and a traditional method, I had over three thousand. How'd you keep track?
- 14:56You don't you have a staff? My staff is limited where I'm at
- 15:03and a lot of the time I'm the one doing things because and more than
- 15:07anything that trust for the time and the communications is built both ways. And
- 15:13our phone number is nine one eight two one four eight two zero zero.
- 15:18Very good. And of course the website the all important website to get the
- 15:22ball rolling. Yes, it's www dot medclub dot clinic
