- 0:00Good morning and welcome to another episode of Morning Medical with Dr.
- 0:04Chow.
- 0:04I am your host, Dr.
- 0:06Chi-Ming Chow.
- 0:07Today, we are diving into some of the latest developments in the world of health and
- 0:12medicine.
- 0:12Our first segment focuses on a significant milestone in Ontario's healthcare system.
- 0:18Let's explore how the province is connecting its residents to family physicians.
- 0:25Ontario's health minister, Sylvia Jones, recently announced that more than five hundred
- 0:30eleven thousand Ontarians have been connected to family doctors since the province launched
- 0:35its primary health care plan last year.
- 0:38This initiative aims to ensure every Ontarian has access to a publicly-funded family
- 0:44physician by the year two thousand twenty-nine.
- 0:47This ambitious goal is part of a larger three point four billion dollar primary health care
- 0:52plan that began in January of two thousand twenty-five.
- 0:57Since two thousand eighteen, Ontario has added approximately twenty thousand doctors to its
- 1:03workforce, and there has been a fourteen percent increase in the number of family
- 1:07doctors.
- 1:08Health Minister Jones highlighted that the province is not stopping there, emphasizing the
- 1:13ongoing efforts to build a robust healthcare system that will last for generations.
- 1:20The province is also expanding medical education by adding three hundred forty
- 1:24undergraduate seats and five hundred fifty-one postgraduate positions, which will result in a
- 1:30sixty-seven percent increase in family medicine graduates each year.
- 1:35This expansion includes building eighteen primary care teaching clinics, one of which has
- 1:40already opened in Brampton to support Toronto Metropolitan University's new medical school.
- 1:46In addition to expanding medical education, Ontario is enhancing its healthcare workforce
- 1:52by recruiting over two thousand three hundred nurses and five hundred seventy doctors from
- 1:57the United States last year.
- 1:59The province is investing three hundred million dollars to create up to one hundred seventy
- 2:04primary care nurse practitioner seats and one hundred fifty physician assistant positions.
- 2:11This progress comes at a time when Ontario hospitals are facing financial challenges, with
- 2:17some announcing job cuts to address budgetary constraints.
- 2:20Despite these challenges, the province remains committed to its goal of connecting all
- 2:25Ontarians to a primary care provider by two thousand twenty-nine.
- 2:30A call for proposals is currently open to create or expand seventy-eight primary care
- 2:36teams, which officials say should connect another six hundred thousand people to primary
- 2:41care.
- 2:42This concerted effort demonstrates Ontario's commitment to providing accessible and
- 2:46comprehensive healthcare to its residents.
- 2:51Our next topic today delves into the concerning link between obesity in young adults and the
- 2:56increased risk of multiple early-onset cancers.
- 3:00A recent observational study from South Korea has shed light on this association,
- 3:05highlighting that the elevated cancer risk persists into middle age.
- 3:11The study, which utilized a large population-based sample, found that men were at
- 3:16a higher risk of developing cancer than women.
- 3:19Interestingly, the presence of abdominal fat further increased the risk beyond what body
- 3:24mass index alone would suggest.
- 3:27The cancers with the highest rates in this study were thyroid, endometrial, and ovarian
- 3:32cancers, followed by colorectal, kidney, liver, and pancreatic cancers.
- 3:38Researchers reported that the ten-year cumulative incidence of any cancer ranged from
- 3:44one point eight four percent in individuals with normal weight to two point six two percent
- 3:50in those with class two obesity.
- 3:52It's worth noting that the association between obesity and cancer was particularly strong
- 3:58among men, with abdominal and visceral fat adding to the risk.
- 4:03In terms of specific cancer types, thyroid, endometrial, and ovarian cancers were the most
- 4:10prevalent over the ten-year period.
- 4:12Interestingly, the study also found that breast cancer rates were lower in people with obesity
- 4:18compared to normal-weight young adults.
- 4:21This finding suggests that the relationship between weight and cancer can be complex and
- 4:26varies by cancer type.
- 4:29The authors of the study emphasized the importance of early-life weight management,
- 4:34noting that although the absolute risk differences were modest, the consistent
- 4:39associations across multiple cancer types underscore the potential significance of
- 4:44addressing obesity early on.
- 4:46They called for future research to explore whether intentional weight loss in early
- 4:51adulthood could reduce subsequent cancer risk and to determine the clinical settings in which
- 4:56early cancer screening for young adults with obesity might be appropriate.
- 5:01Alpa Patel, a researcher with the American Cancer Society, noted that this study provides
- 5:07additional insights into early-onset cancers, which are not as well represented in older
- 5:13studies.
- 5:14The study's contemporary population and the start of the follow-up period between two
- 5:19thousand nine and two thousand twelve provide a more current perspective on these trends.
- 5:25One limitation of the study was the lack of detailed information on smoking intensity or
- 5:30duration, even though smoking status was reported.
- 5:34This is an important factor as it also contributes to cancer risk.
- 5:39In a broader context, more than a dozen cancers have been significantly associated with
- 5:45overweight and obesity.
- 5:47A recent study even suggested that up to ten percent of the global cancer burden might be
- 5:52attributed to these conditions.
- 5:55As obesity prevalence rises globally, we are seeing increases in early-onset cancers.
- 6:01Most studies have focused on middle-aged and older populations, but this research highlights
- 6:06the need to pay attention to younger adults as well.
- 6:11Our next segment takes a closer look at a recent study on the use of artificial
- 6:15intelligence-driven ambient scribes in the healthcare setting.
- 6:20These AI scribes are being explored as a potential solution to reduce the documentation
- 6:25burden faced by clinicians, which is a significant contributor to burnout.
- 6:30The study, conducted at Beth Israel Deaconess Medical Center, evaluated the impact of an AI
- 6:36scribe system known as Heidi, implemented across a fourteen-hospital system.
- 6:42The findings revealed that while the use of Heidi reduced the time clinicians spent
- 6:47actively taking notes and increased the length of those notes, it did not lead to improvements
- 6:53in same-day note closure or overall clinical productivity.
- 6:57Specifically, the median note-active time decreased from twenty-one minutes to eighteen
- 7:02point six minutes, and the median note length increased from four thousand eight hundred
- 7:07fifty-four characters to five thousand one hundred eighty-seven characters after
- 7:12clinicians started using the system.
- 7:15Despite the reduction in time spent on note-taking, the study found no significant
- 7:20changes in other productivity measures.
- 7:23This suggests that while AI scribes can help with documentation efficiency, they might not
- 7:28fully alleviate the broader challenges associated with clinician burnout.
- 7:33The researchers highlighted the need for further research to examine the impact of AI
- 7:38scribes on clinician productivity, documentation accuracy, and health equity.
- 7:45Interestingly, the study also noted that the reduction in note-active time was more
- 7:50pronounced among clinicians who used the system more frequently.
- 7:54High users experienced a larger decrease in time spent on notes and a notable increase in
- 8:00note length, compared to low users and nonusers.
- 8:03This suggests that frequent users may derive more benefits from the system.
- 8:09However, there are potential unintended consequences of using AI scribes.
- 8:14An accompanying editorial pointed out that the time saved might be offset by the additional
- 8:20time required for reviewing and editing AI-generated notes.
- 8:24Moreover, the increase in note length raises questions about the transformation of patient
- 8:29narratives and the potential impact on the quality of care.
- 8:34The editorial also emphasized the need for more regulation around AI scribes, which are
- 8:39currently classified as low-risk medical devices not requiring review by the Food and
- 8:45Drug Administration.
- 8:46The long-term effects of incorporating AI-generated content into clinical notes remain
- 8:52uncertain, particularly concerning inaccuracies or omissions.
- 8:57The study's authors acknowledged several limitations, including the inability to
- 9:02distinguish between AI-scribed and clinician-authored notes in their data.
- 9:07This, along with potential unmeasured confounding factors, suggests that further
- 9:12investigation is necessary to fully understand the implications of AI scribes in clinical
- 9:18practice.
- 9:21Today, we're shedding light on a critical aspect of health that often goes unnoticed
- 9:26until it's too late: mental health.
- 9:29It's a topic that bears repeating, as mental health issues can affect anyone at any time.
- 9:35An article from Yahoo Life highlights some subtle signs that your mental health might be
- 9:40starting to slip.
- 9:41Recognizing these signs early can be crucial in seeking the help and support you might need.
- 9:48One of the first signs people often notice is a reluctance to respond to text messages.
- 9:53When you're feeling burnt out or depressed, even the simplest tasks, like replying to a
- 9:58friend, can feel overwhelming.
- 10:01This withdrawal from communication can be a red flag indicating that your mental health needs
- 10:07attention.
- 10:08Another subtle indicator is making mistakes in tasks that you usually perform with ease.
- 10:15For instance, a skilled carpenter might start making errors in their work, which could be a
- 10:20sign of underlying stress or emotional turmoil.
- 10:23It's worth checking in with yourself or others when you notice such changes in behavior.
- 10:29The desire to stay at home and avoid social interactions is another common sign.
- 10:35When leaving the house feels daunting, it's often a sign that something deeper might be
- 10:39going on.
- 10:41Similarly, when doing nothing starts to feel like a full-time job, it might be time to
- 10:46evaluate your mental health state.
- 10:49Emotional responses can also be telling.
- 10:52If you find yourself getting teary over seemingly trivial matters, like the weather, it
- 10:57could indicate that you're experiencing emotional distress.
- 11:01Pay attention to these emotional cues, as they can be a window into your mental health.
- 11:07Physical spaces can reflect our mental state, too.
- 11:11If your home becomes cluttered and you lack the motivation to clean it, this might be a sign
- 11:16that your mental health is declining.
- 11:19A messy environment often mirrors the chaos happening internally.
- 11:24Anxiety about things that usually do not bother you, or paranoia, can also be a subtle sign.
- 11:31If you notice an increase in anxious thoughts or behaviors, it might be a good time to reach
- 11:36out for support.
- 11:38Loss of interest in social interactions or hobbies you once enjoyed is another indication
- 11:44that your mental health might be struggling.
- 11:47When activities that used to bring joy no longer appeal to you, it's important to take
- 11:52note.
- 11:53Memory loss and difficulty maintaining routines that usually bring happiness are also
- 11:59concerning signs.
- 12:00When you start forgetting things or find it hard to stick to your daily routines, it might
- 12:06be time to consider your mental well-being.
- 12:09A decrease in appetite or feeling like a bystander in your own life can also suggest
- 12:14that something is amiss.
- 12:16These feelings of disconnection and disinterest in basic needs can be important indicators of
- 12:22mental health challenges.
- 12:25Nightmares, increased fatigue, and spending excessive amounts of time in bed are additional
- 12:31signs that your mental health might be slipping.
- 12:34These symptoms can create a cycle of exhaustion and stress, further impacting your overall
- 12:40well-being.
- 12:42Lastly, physical symptoms like random sighing, imaginary arguments in your head, and a general
- 12:48run-down feeling can all be linked to mental health issues.
- 12:52It's vital to recognize these symptoms as potential indicators that you might need to
- 12:57address your mental health.
- 13:00It is essential to understand these subtle signs and take them seriously.
- 13:05Early recognition can lead to timely intervention, which can make a significant
- 13:09difference in managing mental health.
- 13:12Everyone deserves support and understanding as they navigate life's challenges, so if you
- 13:17notice these signs in yourself or others, consider seeking help or reaching out to
- 13:22someone you trust.
