- 0:00Scott Webb: Between friends and family, social media and commercials, it seems
- 0:04that everybody is talking about the benefits of GLP one medications for the
- 0:07treatment of diabetes and weight loss.
- 0:10And my guest today is here to explain the benefits and risks of GLP one's,
- 0:14who's a good candidate And so much more.
- 0:16Scott Webb (Host): I'm joined today by Dr.
- 0:17Jaqui Jones.
- 0:18She's a Family Medicine Doctor with PRISMA Health.
- 0:23This is Flourish, the podcast brought to you by PRISMA Health.
- 0:26I'm Scott Webb.
- 0:27Scott Webb: Dr.
- 0:27Jones, nice to have you here today we're going beyond blood sugar.
- 0:30We're gonna talk GLP ones And the future of diabetes care And so much more.
- 0:35But before we get to all that, and I don't wanna jump too far ahead for
- 0:38listeners, like what is diabetes and why is blood sugar control so important?
- 0:43Dr. Jaqui Jones:
- 0:43So diabetes?
- 0:44There are two types.
- 0:45we're talking about type two diabetes, which is, really more of a.
- 0:49Disease of lifestyle.
- 0:51So, there's an endocrine dysfunctional problem.
- 0:54We become hyperinsulinemic, meaning an insulin.
- 0:57That thing we use to bring sugar into our body does not function as well as
- 1:01we want to because of lots of different reasons, how much we've eaten, how little
- 1:05we've exercised, a combination of both that results in elevated blood sugars,
- 1:09and when they become high enough, we can test you and diagnose you with diabetes.
- 1:12That's it in a scientific nutshell.
- 1:14But the problem is that we, we.
- 1:16Really become a society that's eating too much processed foods, refined
- 1:19sugars, And that result in us being overflowing with fats and sugars,
- 1:23and that's resulting in disease.
- 1:25Scott Webb: Yeah, as you say, it's behavior, lifestyle, diet, lack of
- 1:29exercise, maybe a little genetics, family history mixed in, and doctor we.
- 1:34Hear all the time, seemingly now about GLP one medications.
- 1:38Every time I turn on the TV, I see commercials and they're on social media.
- 1:42What are they and how do they work?
- 1:45Dr. Jaqui Jones:
- 1:45So GLP ones have become all the rage.
- 1:48Now, they're not as new as people think, but I think that's
- 1:50always the case in medicine.
- 1:52They're very popular right now, and they're the.
- 1:54New kid on the block, but the science has been studying GLP ones for a long time.
- 1:58So glucagon like peptide one, agonist medications were created for the
- 2:02treatment of type two diabetes and now labeled for the treatment of obesity.
- 2:07These medicines are injectables, although they're recently become oral
- 2:10medications, but they have been great in dropping a one Cs, larger than
- 2:15any medicine that we have with the exception of insulin and So, It has
- 2:19really been unheard of to see a drop of.
- 2:21You know, plus one point.
- 2:23So to go from like maybe a 7.2
- 2:25down to a 6.2
- 2:27has been hard to do on a single medication, but these medications do it.
- 2:31And so they've been very beneficial in helping us, , with the glucagon-like
- 2:35peptide, receptors, which help us decrease blood sugars and help with hunger.
- 2:39Scott Webb: Right.
- 2:41Yeah.
- 2:41Like I said, they're seemingly everywhere.
- 2:43Everybody seem, talking about them.
- 2:45How do they differ, doctor, than the more traditional or older diabetes treatments?
- 2:50Dr. Jaqui Jones:
- 2:50They have a completely different approach.
- 2:51So insulin, we all know it's just simply hormone replacement, right?
- 2:54Yeah.
- 2:55So I am putting insulin in your system to make it do what it's always done.
- 2:58There have been medicines that used to either increase how much insulin.
- 3:03That you were producing, or they were trying to prevent absorption of
- 3:07sugars in the gut, like metformin.
- 3:09There have never been ones that actually attack this particular,
- 3:14receptor that really got at hunger and So, It slows down gut motility.
- 3:19It has a sensation of feeling more full, and it has done a great job by
- 3:23doing that, making sure that people eat less, um, or eat more appropriately.
- 3:29Scott Webb: Right.
- 3:30So then let's talk about who's a good candidate, right?
- 3:33As I said, it's seemingly everywhere and everyone's talking about it,
- 3:35and I'm sure lots of patient's are asking you about it, GLP ones That is.
- 3:39But who's the right person, the right candidate for this therapy?
- 3:43Dr. Jaqui Jones:
- 3:43Anybody with diabetes I think can be a candidate for, the GLP
- 3:47ones,, from a diabetes standpoint, just to stay the course there.
- 3:50I think anyone that has an elevated.
- 3:53A1C of 6.5
- 3:54and higher is an absolute good candidate which will also be covered by insurance.
- 3:58So I think from a medicine standpoint, I think anyone with an elevated
- 4:01A1C would be a great candidate.
- 4:03Understanding that cost matters in our society.
- 4:06People wanna know that it's gonna be covered by most of their insurance
- 4:08carriers, a diagnosis of diabetes.
- 4:11Where you've not had success in controlling that through some of the
- 4:14traditional or older medications, I think makes you a great candidate
- 4:18for having a conversation with your doctor about a GLP one.
- 4:21Scott Webb: Right.
- 4:21At the very least, at least have a conversation.
- 4:23And it makes me wonder, are there some benefits beyond just
- 4:27blood sugar control for GLP ones?
- 4:30Dr. Jaqui Jones:
- 4:30Absolutely.
- 4:30There are anecdotal things that you've heard, I'm sure, sure.
- 4:33Through the grapevine, but we do know that, you know, Ozempic, and Wegovy
- 4:38have been studied to, , know improvement in people with chronic kidney disease.
- 4:43People with existing heart failure have seen some improvement
- 4:45in morbidity and mortality.
- 4:47And then also the one that everybody's in love with, weight loss.
- 4:51Scott Webb: Yeah, that, uh, no doubt that, that's music to our EARS.
- 4:56Of course.
- 4:57Yes, we need to eat better And we need to, you know, get some
- 5:00more exercise and all of that.
- 5:01But the weight loss is key.
- 5:03Are there any side effects or risks that patient's need to be aware of?
- 5:07Dr. Jaqui Jones:
- 5:07There are some people that we are concerned about.
- 5:10So if you ever had a history of pancreatitis, then unfortunately
- 5:13GLP ones may or may not be for you.
- 5:16It can cause in some rare instances, a repeat of your pancreatitis.
- 5:20I do not think there's a reason for you to absolutely not have
- 5:23it, but you have to be cautious.
- 5:25There is an absolute contraindication, and that's anyone with what we call
- 5:28MEN, multiple endocrine neoplasia.
- 5:32If you have been diagnosed with one of those or you have a family history with
- 5:35that, you likely may not be able to take advantage of a GLP one, and I strongly
- 5:40recommend that you let your doctor know about any history or any family
- 5:44history before starting this medication.
- 5:46Scott Webb: Sure.
- 5:47I wanna talk to you about integrating GLP ones, if you
- 5:50will, with the lifestyle changes.
- 5:52Would recommend diet, exercise, And so forth.
- 5:54How do you kind of merge those things together?
- 5:57How do you integrate them?
- 5:58Dr. Jaqui Jones:
- 5:58Well first with education.
- 6:00So I think right now the GLP one, sound magical, that you take them and you
- 6:05wake up one morning and you are skinny.
- 6:09But that's not exactly how that works.
- 6:12I think the average weight loss is somewhere between five and
- 6:1515% of the starting body weight.
- 6:18, Over the first six to 12 months.
- 6:21We are really trying hard to.
- 6:23Help people understand patterns and behaviors.
- 6:27It has been great that it has decreased our appetite and our hunger drive, and I
- 6:31see that as an opportunity to really buy you time to be thoughtful about the food.
- 6:35So I think in addition to educating people on their diabetes, the
- 6:40advantages of the GLP one.
- 6:42We also are trying to spend a good chunk of time saying, now that we have
- 6:45your attention, and now that you're not driven so much by your cravings
- 6:48and your hunger, let's talk about what you should be eating right versus
- 6:52all the things you should not eat.
- 6:53So, it without that key component.
- 6:56I think we cannot see the sustained longevity of the weight loss And the
- 7:00success that people are seeing upfront.
- 7:03Scott Webb: I like to, do a little myth-busting when I can, doctor.
- 7:06So let's bust any myths that are out there, misconceptions about GLP ones.
- 7:11Anything out there that you've heard anecdotally or otherwise
- 7:14that you want to clear up.
- 7:15Dr. Jaqui Jones:
- 7:15The big one is that they're magic.
- 7:17Um, they're, they're not magic.,
- 7:19I've had many conversations, , with patient's, some from the
- 7:23surprise, I didn't think I could lose this much weight.
- 7:26And the other one's from, I can't believe I'm paying this money for this medication,
- 7:29and I have not lost enough weight.
- 7:31do think that the five to 15% is a pretty reasonable,
- 7:35expectation, for your weight loss.
- 7:38I think anything past That is a combination of both the tools that you
- 7:41use, like GLP one and your lifestyle.
- 7:44So I think that That is probably the biggest one.
- 7:48Everybody's super excited to try it and I don't want to set a high
- 7:53expectation that's gonna leave people disappointed because if you are a
- 7:56stress eater or an over eater or just.
- 7:59Love eating.
- 8:00Nothing better than drive that craving than a big chunk of
- 8:04disappointment when you did not hit a goal that you were expecting.
- 8:07So setting the expectations of this not being magical, but being
- 8:10a tool is probably the biggest myth I would like to bust.
- 8:14Scott Webb: so you mentioned earlier about, cost and insurance.
- 8:17I just wanted to touch on that a little bit more.
- 8:19Like what should patient's know and or consider when you're thinking about
- 8:24the cost of GLP ones and insurance And what they cover or don't cover?
- 8:28This is still, I think, an expensive medication for most people if
- 8:34you're paying out of POCAT.
- 8:35I think that, the makers of the GLP ones have, put things in place where
- 8:40you can buy them directly from.
- 8:42You know, the manufacturer to try to cut down on the cost, but
- 8:46they are an expensive, adventure.
- 8:49So I've seen cost as low as 2 99 a month to as high as 1500 a month when
- 8:55not covered by insurance, depending on where you're purchasing it.
- 8:59And so I want people to know that there can be a cost associated
- 9:02with obtaining this medication if you're paying out of POCAT.
- 9:05What I would like for people to do is check with their insurance
- 9:08companies because we did a big swing of insurance companies not
- 9:12covering weight loss medications.
- 9:14But I think that that may reverse, , over time and So, it will not hurt
- 9:18to call your insurance company to ask, do you cover GLP ones?
- 9:21And more importantly, for what conditions do you cover them?
- 9:26Right?
- 9:26So they may say, yes, we cover them if you have diabetes.
- 9:29And you can always ask directly, will you cover them for weight loss?
- 9:33And if they say, yes, that's great.
- 9:34That's great information for us to know.
- 9:36If they say no, then it allows me to have a conversation about
- 9:39the expectation on pricing.
- 9:41Right.
- 9:43It's amazing to me, doctor, that we're talking about these GLP ones
- 9:46and we're still learning so much about them, the sort of unintended
- 9:49benefits that folks are experiencing.
- 9:51But what's next in GLP ones
- 9:53Dr. Jaqui Jones:
- 9:53We just recently started to, get oral options.
- 9:56So, you know, I don't know how many people know, but the GLP ones today have
- 10:00been primarily injectable medications.
- 10:03Yeah.
- 10:03But they are now offering some oral options out there
- 10:06that are starting to come out.
- 10:07So I think as we provide, different mediums, because some people.
- 10:11Really want to take advantage of this tool, but they cannot
- 10:14get over self-injecting.
- 10:16So as the oral medications come out, I think that's gonna be lots to
- 10:19talk about and lots to experience.
- 10:20But I think for now the GLP ones are gonna be the hot commodity of the moment.
- 10:27Scott Webb: Yeah, gonna be here for a while.
- 10:29I said, we're still learning so much about them and they may not be magic, and if
- 10:33your insurance doesn't pay for 'em in some circumstances, they're pretty expensive.
- 10:36But they are amazing nonetheless.
- 10:39And I appreciate your time today.
- 10:40just great to learn more.
- 10:41Dr. Jaqui Jones:
- 10:41Thank you so much for having me, Scott.
