- 0:00here
- 0:00is the
- 0:01agenda
- 0:01for what
- 0:01we're going
- 0:02to be
- 0:02talking
- 0:02about
- 0:02today.
- 0:03so I'm
- 0:04going to
- 0:04introduce
- 0:05a few
- 0:05different
- 0:05types
- 0:05of therapy.
- 0:06Modality is
- 0:07that are
- 0:08best
- 0:08practice
- 0:09from
- 0:09I always
- 0:10say
- 0:10there's no
- 0:10right
- 0:11or wrong,
- 0:12therapy
- 0:12modality to
- 0:13choose
- 0:14from.
- 0:14and
- 0:15at the same
- 0:15time,
- 0:15it can be
- 0:16helpful
- 0:16to hear
- 0:16a little
- 0:17bit about,
- 0:17like
- 0:17the nuances
- 0:18and the
- 0:18differences
- 0:19between
- 0:20all of these.
- 0:21and for
- 0:21each
- 0:22modality,
- 0:22I'll go
- 0:23into giving
- 0:24a basic
- 0:25preview
- 0:25over what
- 0:26this
- 0:26therapy
- 0:27modality
- 0:27is.
- 0:28consideration.
- 0:29So
- 0:29how do you know
- 0:30which one
- 0:30might be a
- 0:31the best
- 0:32fit
- 0:32for you?
- 0:33And I'll
- 0:33also give a
- 0:34preview
- 0:34into some
- 0:35of like
- 0:35the skills
- 0:36and
- 0:36interventions
- 0:37we use
- 0:37for each of
- 0:38these
- 0:38modalities.
- 0:39And
- 0:40then we'll
- 0:40end the
- 0:41presentation
- 0:41talking
- 0:42about,
- 0:42you know,
- 0:43if you
- 0:43are
- 0:44interested
- 0:44in seeking
- 0:45out
- 0:45therapy,
- 0:46some of the challenges
- 0:46that
- 0:47come up.
- 0:47And as a
- 0:48therapist,
- 0:48just
- 0:48some of
- 0:49my
- 0:49suggestions
- 0:49as to how
- 0:50to navigate
- 0:50those
- 0:51challenges.
- 0:53So first
- 0:54we'll talk
- 0:54about
- 0:55something
- 0:55called
- 0:55cognitive
- 0:56behavioral
- 0:56therapy.
- 0:57And this
- 0:57diagram
- 0:58here is
- 0:58called
- 0:58the
- 0:58cognitive
- 0:59triangle
- 1:00which is
- 1:00essentially
- 1:01the basis
- 1:01of this
- 1:02therapy.
- 1:03And so
- 1:04the idea
- 1:04here is
- 1:05that our
- 1:05thoughts
- 1:05or
- 1:06behaviors
- 1:06our emotions
- 1:07they're all
- 1:08interconnected.
- 1:08They all
- 1:09affect
- 1:09one
- 1:09another.
- 1:10So the way
- 1:10I think
- 1:11affects how
- 1:11I feel,
- 1:12the way
- 1:12I feel
- 1:13affects how
- 1:13I behave,
- 1:14etc..
- 1:16And they're
- 1:16all
- 1:16interconnected.
- 1:17And so
- 1:18in CBT,
- 1:18what we do
- 1:19is we
- 1:19intervene
- 1:20at one
- 1:20point
- 1:20in the
- 1:21triangle,
- 1:22meaning
- 1:22that if
- 1:23I try
- 1:24to change
- 1:24my
- 1:25behavior,
- 1:25that
- 1:26in turn
- 1:26to change
- 1:27my thinking
- 1:27patterns,
- 1:28that should
- 1:28change
- 1:29my feelings
- 1:29so we can
- 1:30intervene
- 1:30at any point.
- 1:32And so I'll
- 1:32take a minute
- 1:33now
- 1:33to talk about
- 1:34what does
- 1:34this
- 1:34look like
- 1:34if,
- 1:34let's say
- 1:35we
- 1:35intervene
- 1:35at the
- 1:36cognitive
- 1:36level.
- 1:37So
- 1:37the thought
- 1:38point of
- 1:38the
- 1:38triangle.
- 1:40So let's
- 1:41say you
- 1:42get a
- 1:42cancer
- 1:43diagnosis
- 1:44and you
- 1:44have the
- 1:44thought
- 1:45of I'll
- 1:45never feel
- 1:46happy
- 1:46again.
- 1:48And that
- 1:49causes your
- 1:49emotions
- 1:50to turn
- 1:50into
- 1:50hopelessness.
- 1:51Maybe
- 1:51feeling
- 1:52depressed.
- 1:53And let's
- 1:53say the
- 1:53intensity
- 1:54of this
- 1:54is a 90
- 1:55out of
- 1:55a hundred.
- 1:56And then
- 1:57the start,
- 1:57this
- 1:57emotion
- 1:58impacts
- 1:58your behavior.
- 1:59So
- 1:59maybe you
- 2:00stay in bed
- 2:00all day.
- 2:01You stop
- 2:01responding
- 2:01to text
- 2:02messages
- 2:02from
- 2:03friends,
- 2:03from loved
- 2:04ones.
- 2:05So
- 2:05this is a
- 2:06completely
- 2:07understandable
- 2:07thought
- 2:08to have
- 2:08if all of
- 2:09a sudden
- 2:09you get a
- 2:09cancer
- 2:10diagnosis.
- 2:11And
- 2:11at the same time,
- 2:12we know
- 2:13that this
- 2:13thought
- 2:13might not
- 2:14necessarily
- 2:14be fully
- 2:15true,
- 2:15that you'll
- 2:16never feel
- 2:17happy
- 2:17again.
- 2:18And so
- 2:18something
- 2:19we do
- 2:19in CBT,
- 2:20and I'm
- 2:20giving
- 2:21a very
- 2:21watered
- 2:21down
- 2:22explanation
- 2:22of this,
- 2:23is we help
- 2:23people
- 2:23challenge
- 2:24their thoughts.
- 2:25So
- 2:26you might
- 2:27help
- 2:27someone
- 2:27challenge
- 2:27that
- 2:28initial
- 2:28thought
- 2:28by creating
- 2:29a more
- 2:29balanced
- 2:30thought.
- 2:30So maybe
- 2:30the more
- 2:31balanced
- 2:31thought is
- 2:32I'm going
- 2:32to be
- 2:32really sad
- 2:33for a
- 2:33while,
- 2:33and it's
- 2:34going
- 2:34to take
- 2:34some time
- 2:35to feel any
- 2:35sense of
- 2:36happiness.
- 2:37The
- 2:38emotions
- 2:38are still
- 2:38probably
- 2:39going
- 2:39to be
- 2:39there.
- 2:39Feeling
- 2:39hopeless
- 2:40and
- 2:40depressed.
- 2:41But maybe
- 2:41instead
- 2:42of it
- 2:42being at
- 2:42a 90%,
- 2:43maybe it's
- 2:43a 70%.
- 2:45And so
- 2:46maybe
- 2:46instead of
- 2:47sitting
- 2:47in bed
- 2:47all day,
- 2:47you stay
- 2:48in bed
- 2:48for
- 2:48two hours
- 2:49and you
- 2:49respond to
- 2:49some
- 2:50of your
- 2:50text great
- 2:51So the idea
- 2:51here is
- 2:52if we can
- 2:52change
- 2:52our thoughts
- 2:53that might
- 2:53be able
- 2:53to change
- 2:54our
- 2:54emotions
- 2:55and
- 2:55actually
- 2:56change
- 2:56our
- 2:56behaviors.
- 2:59Another
- 2:59example is
- 2:59if we
- 3:00intervene
- 3:00at,
- 3:00let's say,
- 3:01like the
- 3:01behavioral
- 3:02point of
- 3:02the
- 3:02triangle.
- 3:03I imagine
- 3:04a lot of
- 3:04people
- 3:04have had
- 3:04this
- 3:04experience.
- 3:05I know
- 3:05I've had
- 3:06this
- 3:06experience
- 3:06where
- 3:07maybe
- 3:07you're
- 3:07not in
- 3:07the mood
- 3:08to go
- 3:08socialize,
- 3:08but
- 3:09you have
- 3:09plans
- 3:09and you're
- 3:10like,
- 3:10okay, I'm
- 3:10going to go
- 3:10anyway.
- 3:11You don't
- 3:12think
- 3:12you're
- 3:12going
- 3:12to have
- 3:12a good
- 3:12time,
- 3:13you're
- 3:13really not
- 3:13in the mood.
- 3:14And you go
- 3:15and you end
- 3:15up actually
- 3:15feeling
- 3:16better.
- 3:17Right.
- 3:17So
- 3:17behaviorally,
- 3:18if
- 3:18we change
- 3:19our
- 3:19behavior
- 3:19and act
- 3:20opposite
- 3:21to what
- 3:21our emotion
- 3:22is
- 3:22telling us
- 3:22to do,
- 3:23that made
- 3:24in turn
- 3:24change our
- 3:25emotions
- 3:26that we're
- 3:26feeling.
- 3:28So some
- 3:29other
- 3:29information
- 3:30about
- 3:30cognitive
- 3:30behavioral therapy.
- 3:31So we have
- 3:32the
- 3:32cognitive
- 3:32piece right
- 3:33which
- 3:33is what
- 3:33we're
- 3:33talking about.
- 3:34So we help
- 3:35patients
- 3:36increase
- 3:37their
- 3:37awareness
- 3:37of thoughts
- 3:38that are
- 3:38maybe not
- 3:39fully
- 3:39accurate,
- 3:41help them
- 3:41identify
- 3:42maladaptive
- 3:43patterns
- 3:43of
- 3:43thinking.
- 3:44And we're
- 3:44going to
- 3:44talk more
- 3:45about that
- 3:45on
- 3:45the next slide.
- 3:46So an
- 3:46example
- 3:47of that
- 3:47might be
- 3:47catastrophizing,
- 3:49and help
- 3:49people
- 3:49reframe
- 3:50unrealistic
- 3:51thoughts
- 3:51and
- 3:51creating
- 3:52more
- 3:52balanced
- 3:53adaptive
- 3:53thoughts.
- 3:55Now
- 3:55I always
- 3:56also tell
- 3:56people
- 3:57when I talk
- 3:57about
- 3:57reframing
- 3:58our
- 3:58thoughts,
- 3:58I'm not
- 3:59talking
- 3:59about
- 3:59toxic
- 4:00positivity
- 4:01or
- 4:01invalidating
- 4:02yourself
- 4:02or
- 4:03minimizing
- 4:03your
- 4:03thoughts,
- 4:04but really
- 4:05trying to
- 4:05find
- 4:06thoughts
- 4:06that are
- 4:06more
- 4:07accurate
- 4:07based on
- 4:08the
- 4:08facts of
- 4:09the situation.
- 4:10So
- 4:10we have the
- 4:10cognitive
- 4:11piece.
- 4:11Now
- 4:12we have the
- 4:12behavioral
- 4:13piece
- 4:14behaviorally.
- 4:15So
- 4:15what do we
- 4:15we help
- 4:16clients
- 4:16with.
- 4:17we help
- 4:17clients
- 4:18engage
- 4:18in more
- 4:18goal
- 4:19dependent
- 4:19behavior
- 4:20as opposed
- 4:20to mood
- 4:21dependent
- 4:21behavior.
- 4:22so an
- 4:23example of,
- 4:24of this is,
- 4:25last night
- 4:26I had
- 4:26dishes
- 4:27to clean.
- 4:27I really
- 4:28wasn't in
- 4:28the mood
- 4:28to
- 4:28clean them.
- 4:29And
- 4:30if I were
- 4:30to engage
- 4:31in mood
- 4:31dependent
- 4:31behavior
- 4:32last night,
- 4:33I would
- 4:33have not
- 4:33cleaned
- 4:33my dishes.
- 4:34But I've
- 4:34just, like,
- 4:34gone
- 4:34to bed.
- 4:35Great.
- 4:35But I
- 4:36don't want
- 4:36to have
- 4:36a dirty kitchen.
- 4:37So
- 4:37I engaged
- 4:38in goal
- 4:38dependent
- 4:39behavior.
- 4:39So even
- 4:40though
- 4:40I wasn't
- 4:40in the mood
- 4:41to, let's
- 4:42say, clean
- 4:42my dishes
- 4:42right,
- 4:43I acted
- 4:44based on
- 4:44my goal.
- 4:45And
- 4:45the problem
- 4:46is that
- 4:46when we
- 4:47there's
- 4:47there's
- 4:48no issue
- 4:48with acting
- 4:49based on
- 4:49your mood.
- 4:50We all do
- 4:50that.
- 4:51The problem
- 4:51becomes
- 4:52when
- 4:52we do that
- 4:52too much.
- 4:53And that's
- 4:54what can
- 4:54often
- 4:55result
- 4:55in
- 4:55depression
- 4:56and anxiety.
- 4:58so we help
- 4:58clients act
- 4:59in more
- 4:59goal
- 5:00dependent
- 5:00manners.
- 5:01especially
- 5:01when
- 5:02it comes
- 5:02to
- 5:02depression,
- 5:03we do
- 5:03something
- 5:03called
- 5:03behavioral
- 5:04activation.
- 5:05so we would
- 5:05actually
- 5:06help you
- 5:07identify
- 5:08what are
- 5:09your
- 5:09values,
- 5:09what's
- 5:10important
- 5:10to you,
- 5:11and help
- 5:11you start
- 5:12to schedule
- 5:12into
- 5:13your week
- 5:14activities
- 5:15that make
- 5:15you feel
- 5:15a sense
- 5:15of joy,
- 5:16and
- 5:17activities
- 5:17that make
- 5:17you feel
- 5:18a sense of
- 5:18mastery
- 5:19or a sense
- 5:20of
- 5:20accomplishment.
- 5:22And then
- 5:22we also
- 5:23help
- 5:23clients
- 5:24decrease
- 5:24avoidant
- 5:25behaviors,
- 5:26that
- 5:26maybe feel
- 5:27good in
- 5:27the moment
- 5:28because
- 5:28more issues
- 5:29in the long
- 5:29term.
- 5:30Right.
- 5:30So let's
- 5:31say you get
- 5:31a cancer
- 5:32diagnosis,
- 5:33right?
- 5:33And now
- 5:34you are
- 5:34just so
- 5:35scared
- 5:35to ever go
- 5:36to
- 5:36the doctor
- 5:36again.
- 5:37And it's
- 5:38just
- 5:38traumatizing.
- 5:39Right?
- 5:39So you stop
- 5:40going to
- 5:41the doctor.
- 5:41You stop,
- 5:43watching TV
- 5:44shows
- 5:44or movies
- 5:44that talk
- 5:45about
- 5:45doctors,
- 5:45right?
- 5:46You just
- 5:46your world
- 5:47becomes
- 5:47smaller
- 5:48and smaller
- 5:48and
- 5:48smaller.
- 5:49And so we
- 5:49help
- 5:49people,
- 5:51basically
- 5:51face their
- 5:51fears, face
- 5:52these
- 5:52anxieties.
- 5:55So now,
- 5:56within CBT,
- 5:57I mentioned
- 5:57that
- 5:57maladaptive
- 5:58patterns
- 5:59of
- 5:59thinking,
- 5:59also known
- 6:00as
- 6:00cognitive
- 6:00distortions.
- 6:01I know
- 6:01there's
- 6:01a lot on
- 6:02this slide,
- 6:02but bear
- 6:02with me
- 6:03here.
- 6:03So
- 6:04cognitive
- 6:05distortions
- 6:05is the
- 6:05definition
- 6:06of that
- 6:07is
- 6:07distorted
- 6:08thought
- 6:08patterns
- 6:09that are
- 6:09often
- 6:10exaggerated.
- 6:11And it
- 6:11skews our
- 6:12perception
- 6:12of reality.
- 6:13It makes us
- 6:14feel really
- 6:14intense
- 6:15emotions.
- 6:15So if
- 6:15you think about
- 6:16it, if
- 6:16you have thoughts
- 6:17that are
- 6:18more
- 6:18exaggerated
- 6:19or extreme,
- 6:20you're
- 6:20going to
- 6:20have more
- 6:20extreme,
- 6:21intense
- 6:22emotions.
- 6:24And we're
- 6:24going to
- 6:24talk about
- 6:25this at
- 6:25the end.
- 6:25But
- 6:25cognitive
- 6:26distortions
- 6:26can
- 6:26actually
- 6:27be helpful
- 6:27at times.
- 6:28And a lot
- 6:29of the time
- 6:29it can
- 6:29be
- 6:29unhelpful.
- 6:30And we all
- 6:30engage in
- 6:31cognitive
- 6:31distortions.
- 6:32So
- 6:33sometimes
- 6:33it gets
- 6:33to a place
- 6:34where
- 6:34we're
- 6:34having so
- 6:34many of
- 6:35these,
- 6:35it can make
- 6:36us feel
- 6:36more
- 6:36depressed,
- 6:37more
- 6:37anxious.
- 6:38So I'm
- 6:38going to
- 6:38briefly
- 6:39go through
- 6:39these,
- 6:40because I
- 6:40really want
- 6:41to give
- 6:41a window in
- 6:42into
- 6:42some of the stuff
- 6:43we focus on
- 6:43in CBT.
- 6:45So we have
- 6:45all or
- 6:46nothing
- 6:46thinking,
- 6:47also known
- 6:47as black
- 6:48and white
- 6:48thinking.
- 6:49So an
- 6:49example.
- 6:50If I'm not
- 6:50perfect,
- 6:51I have
- 6:51failed.
- 6:52And I
- 6:52also
- 6:53included
- 6:53some
- 6:54examples
- 6:54of thoughts
- 6:55that maybe
- 6:55especially
- 6:56with
- 6:56cancer,
- 6:56people
- 6:57might have
- 6:57that fall
- 6:58into these
- 6:58different
- 6:58distortions.
- 6:59So for all
- 7:00or nothing
- 7:00thinking,
- 7:01right,
- 7:01maybe
- 7:02you are
- 7:03venting to
- 7:04your spouse
- 7:05about
- 7:06your
- 7:06feelings
- 7:06about your
- 7:06diagnosis
- 7:07and they
- 7:08see
- 7:08something
- 7:09that just
- 7:09doesn't
- 7:10feel
- 7:10helpful
- 7:10for you.
- 7:11And
- 7:11you have
- 7:11this
- 7:11thought.
- 7:11They always
- 7:12see the
- 7:13wrong thing,
- 7:13right?
- 7:14The word
- 7:14always
- 7:15there
- 7:15probably
- 7:15isn't fully
- 7:16accurate.
- 7:17And we have
- 7:18mental
- 7:18filters,
- 7:18so we only
- 7:19pay
- 7:19attention
- 7:20to certain
- 7:20types
- 7:21of
- 7:21evidence.
- 7:22Right.
- 7:22So I only
- 7:23ever get
- 7:23bad news
- 7:24right?
- 7:25Again.
- 7:25Maybe
- 7:25you've
- 7:26got in
- 7:26a lot
- 7:26of bad news.
- 7:27And that is
- 7:28not to
- 7:28say that
- 7:28hasn't
- 7:29happened.
- 7:29Right.
- 7:29But
- 7:30the word
- 7:30only ever
- 7:31probably
- 7:32not fully
- 7:33accurate.
- 7:34And we have
- 7:34jumping to
- 7:35conclusions.
- 7:35So we have
- 7:36two types.
- 7:36We have mind
- 7:37reading.
- 7:38We imagine
- 7:38we know
- 7:39what other
- 7:39people are
- 7:40thinking.
- 7:40Right.
- 7:41So they're
- 7:41probably
- 7:41judging me.
- 7:42They think
- 7:42I'm weird,
- 7:44or fortune
- 7:44telling,
- 7:45predicting
- 7:45the future.
- 7:46Right.
- 7:46So I know
- 7:47I've had
- 7:48this
- 7:48thought for
- 7:48me of like,
- 7:49my cancer
- 7:50is going to
- 7:50spread,
- 7:51right?
- 7:51But I don't
- 7:52know that
- 7:53to be true.
- 7:53Right?
- 7:53I don't
- 7:54know what
- 7:55the future
- 7:55is.
- 7:55I
- 7:55don't have
- 7:56a crystal
- 7:56ball right?
- 7:57Or a
- 7:58thought
- 7:58that
- 7:58someone
- 7:58else might
- 7:59have.
- 7:59I'll never
- 7:59feel happy.
- 8:01And we have
- 8:02over
- 8:02generalizing.
- 8:03So seeing
- 8:04a pattern
- 8:04based upon
- 8:05a single event,
- 8:06and then
- 8:06being
- 8:07overly
- 8:07broad
- 8:07in the
- 8:08conclusions
- 8:08that we
- 8:08draw.
- 8:09So
- 8:09nothing
- 8:09ever
- 8:10nothing in
- 8:10my life
- 8:11ever goes
- 8:11right,
- 8:12disqualifying
- 8:13the positive.
- 8:14So we just
- 8:14count
- 8:14the good
- 8:15things
- 8:15that
- 8:15happen,
- 8:16right?
- 8:17Or that
- 8:17we've done
- 8:18right.
- 8:18I always
- 8:19mess up
- 8:20right,
- 8:20and we're
- 8:20not looking
- 8:21at
- 8:21the times
- 8:21where maybe
- 8:21we did
- 8:22something
- 8:22right.
- 8:24Then
- 8:24we have
- 8:24catastrophizing,
- 8:25so blowing
- 8:26things out
- 8:26of
- 8:26proportion
- 8:27or the
- 8:27opposite
- 8:28minimizing.
- 8:29Right.
- 8:29So someone
- 8:29asks you
- 8:30how you're
- 8:30doing after
- 8:31your
- 8:31diagnosis,
- 8:32treatment
- 8:32or years
- 8:33later
- 8:33and
- 8:34you say,
- 8:34I'm doing
- 8:34fine,
- 8:35even though
- 8:35maybe
- 8:35or not.
- 8:36or maybe
- 8:37you think
- 8:37that your
- 8:38diagnosis
- 8:38isn't a
- 8:39big deal,
- 8:39right?
- 8:39You're
- 8:40trying to
- 8:40minimize
- 8:40your
- 8:41feelings
- 8:41and
- 8:41thoughts
- 8:42around it.
- 8:43Our four
- 8:44last ones
- 8:44we have
- 8:45emotional
- 8:45reasoning.
- 8:47This one,
- 8:47I think is
- 8:47sometimes a
- 8:48little bit
- 8:48confusing
- 8:49to fully
- 8:50grasp,
- 8:50but
- 8:50the idea
- 8:51here is
- 8:51that
- 8:52it's
- 8:53assuming
- 8:53that
- 8:54because
- 8:54we feel a
- 8:54certain
- 8:55way, what
- 8:55we think
- 8:56must be
- 8:57true.
- 8:57So I feel
- 8:58embarrassed.
- 8:59So we must
- 8:59be
- 8:59an idiot.
- 9:00I feel
- 9:01so overwhelmed.
- 9:02So
- 9:02that must
- 9:03mean I'll
- 9:03never feel
- 9:03better.
- 9:04I feel anxious,
- 9:05so that
- 9:06must mean
- 9:06I'm going
- 9:06to get
- 9:06bad results
- 9:07on my scan.
- 9:08Right?
- 9:08I feel anxious
- 9:09about going
- 9:09to
- 9:09this party,
- 9:10so
- 9:10I shouldn't
- 9:10go because
- 9:11I won't
- 9:11have
- 9:11a good time.
- 9:13And we have
- 9:13labeling.
- 9:14So
- 9:14assigning
- 9:14a label
- 9:14to yourself
- 9:15or someone
- 9:15else.
- 9:16I'm so
- 9:16ugly.
- 9:16I'm so
- 9:17lazy.
- 9:18then
- 9:18we have
- 9:18should
- 9:18or must.
- 9:19This is my
- 9:19favorite
- 9:20out of
- 9:20all of these.
- 9:21so
- 9:22the idea here
- 9:23is that
- 9:23when we
- 9:24use these
- 9:24critical
- 9:24words like
- 9:25should
- 9:25or must,
- 9:26it
- 9:26can make us
- 9:27feel guilty
- 9:27or like
- 9:28we've done
- 9:28something
- 9:29wrong.
- 9:29Or when
- 9:30we apply
- 9:30these words
- 9:31to other
- 9:31people,
- 9:32it's
- 9:33inevitably
- 9:33going to
- 9:34make us
- 9:34feel
- 9:34incredibly
- 9:35frustrated.
- 9:36Right?
- 9:36So an
- 9:37example
- 9:37I should be
- 9:38over my
- 9:39diagnosis
- 9:39by now.
- 9:40I shouldn't
- 9:41be
- 9:41so upset,
- 9:42right?
- 9:42So we're
- 9:42placing
- 9:43these
- 9:43unfair
- 9:43expectations
- 9:44on ourself.
- 9:45But
- 9:45who says
- 9:46you should
- 9:47or
- 9:47shouldn't
- 9:47be over
- 9:48your
- 9:48diagnosis,
- 9:48right?
- 9:49Who gets
- 9:49to decide
- 9:50these
- 9:50things?
- 9:51I am a
- 9:52New Yorker.
- 9:52I live,
- 9:53like, right
- 9:53outside of
- 9:53Manhattan,
- 9:54and I am
- 9:56a fast
- 9:56walker.
- 9:56And
- 9:57sometimes
- 9:57when I'm in
- 9:57the city,
- 9:58people are
- 9:58walking
- 9:59pretty
- 9:59slow,
- 10:00and I have
- 10:00those
- 10:01thoughts of like,
- 10:01they should
- 10:01just walk
- 10:02faster,
- 10:02right?
- 10:02They shouldn't
- 10:03walk so
- 10:03slow.
- 10:04who am I
- 10:05to say
- 10:05that they
- 10:05should
- 10:05or
- 10:05shouldn't
- 10:06walk faster
- 10:07or slower?
- 10:08and so
- 10:08something
- 10:08that we
- 10:09talk about
- 10:09in therapy
- 10:10is instead
- 10:10of using
- 10:11these
- 10:11should
- 10:11statements,
- 10:12it can be
- 10:12helpful
- 10:13to use
- 10:13preferential
- 10:14thinking.
- 10:15So words
- 10:16such as
- 10:16I wish
- 10:17I would
- 10:17prefer,
- 10:18I would
- 10:18like.
- 10:19Right.
- 10:19So I would
- 10:20prefer
- 10:20they would
- 10:20walk
- 10:20faster.
- 10:21Right?
- 10:22I wish they
- 10:22walked
- 10:22faster,
- 10:23right?
- 10:24I wish she
- 10:24didn't
- 10:24say that.
- 10:26Then our
- 10:26last one
- 10:27is
- 10:27personalization.
- 10:28So taking
- 10:29full
- 10:29responsibility
- 10:30for
- 10:30something
- 10:30that maybe
- 10:30wasn't
- 10:31your fault
- 10:31and vice
- 10:32versa,
- 10:32right?
- 10:32So I caused
- 10:33my cancer.
- 10:36So some
- 10:36considerations
- 10:37when it comes to CBT.
- 10:38How
- 10:38do you know
- 10:39if
- 10:39this is a
- 10:39good choice
- 10:40of therapy
- 10:41for you.
- 10:41so
- 10:42if you're
- 10:42finding
- 10:42yourself
- 10:43feeling
- 10:43depressed,
- 10:44you're
- 10:44no longer
- 10:44finding
- 10:44that you
- 10:45enjoy the
- 10:45things,
- 10:46the
- 10:46activities,
- 10:46hobbies
- 10:47that you
- 10:47once
- 10:47enjoyed.
- 10:49maybe you
- 10:49find it
- 10:49difficult
- 10:50to do
- 10:50daily
- 10:51tasks.
- 10:51It's
- 10:51hard to get
- 10:52out of bed.
- 10:52It's hard
- 10:52to shower.
- 10:53It's hard
- 10:53to cook.
- 10:54Do all of
- 10:54those
- 10:55things.
- 10:56CBT can
- 10:56also
- 10:57be good
- 10:57if you find
- 10:58yourself
- 10:58ruminating,
- 10:59and
- 10:59experiencing
- 11:00a lot
- 11:00of negative
- 11:01thoughts.
- 11:02Right.
- 11:02And you
- 11:02just keep
- 11:02replaying
- 11:03the same
- 11:04thing
- 11:04in your head
- 11:04again
- 11:04and again.
- 11:06Or if
- 11:06you're
- 11:06finding
- 11:07yourself
- 11:07experiencing
- 11:08post-traumatic
- 11:08stress
- 11:09symptoms,
- 11:10some PTSD,
- 11:11which can
- 11:12often
- 11:12happens
- 11:12with
- 11:12medical
- 11:13diagnoses
- 11:14and, a
- 11:15lot of
- 11:15medical traumas.
- 11:16And
- 11:16there are
- 11:17just some
- 11:17symptoms
- 11:18that we
- 11:18often see
- 11:18with PTSD.
- 11:21So some of
- 11:22these I
- 11:22touched on.
- 11:23But to just
- 11:23sort of
- 11:24wrap up
- 11:24this part
- 11:24of the
- 11:25presentation,
- 11:25CBT,
- 11:26some of
- 11:26the main
- 11:27interventions
- 11:28that we as
- 11:29therapists
- 11:29use with
- 11:30clients
- 11:31are coming
- 11:31into us
- 11:32use
- 11:33behavioral
- 11:33activation
- 11:34for
- 11:34depression.
- 11:35So
- 11:35that's what
- 11:35I mentioned
- 11:35before.
- 11:36Increasing
- 11:36pleasurable
- 11:37activity
- 11:38is a sense
- 11:39of
- 11:39accomplishment.
- 11:40We have
- 11:40exposure
- 11:41and
- 11:41response
- 11:42prevention
- 11:42for
- 11:43anxiety.
- 11:44Right.
- 11:44So
- 11:44let's say
- 11:45you have a
- 11:45phobia of
- 11:46needles
- 11:47right.
- 11:47We'll do
- 11:48something
- 11:48called
- 11:48exposure
- 11:49work where
- 11:50maybe first
- 11:50I have
- 11:51you look at
- 11:51photo
- 11:52of needles
- 11:52and then
- 11:54watch
- 11:54a video
- 11:54of needles
- 11:55and then
- 11:55watch
- 11:56a video
- 11:56of someone
- 11:56getting,
- 11:57a blood
- 11:57draw and
- 11:58so on
- 11:58and so
- 11:59forth.
- 11:59And we
- 11:59gradually
- 12:00expose
- 12:01someone to,
- 12:02something
- 12:02that feels
- 12:03more
- 12:03and more
- 12:03anxiety
- 12:04provoking
- 12:04with
- 12:05the hope
- 12:05that
- 12:05eventually
- 12:05they'll
- 12:06be able
- 12:06to tolerate
- 12:07this thing
- 12:07that
- 12:07they've
- 12:08been
- 12:08avoiding,
- 12:09cognitive
- 12:10restructuring,
- 12:11that more
- 12:12so ties
- 12:12back
- 12:13to what
- 12:13I was
- 12:13talking
- 12:13about,
- 12:13about those
- 12:14cognitive
- 12:14distortions
- 12:15and helping
- 12:15us reframe
- 12:16our thoughts
- 12:17that can be
- 12:17really
- 12:17helpful
- 12:18for
- 12:18depression
- 12:18and anxiety.
- 12:20and we also
- 12:20have
- 12:20cognitive
- 12:21processing
- 12:21therapy,
- 12:21which is a type
- 12:22of therapy
- 12:23we use for
- 12:23people
- 12:23with PTSD.
- 12:26So that was
- 12:27cognitive
- 12:27behavioral
- 12:27therapy.
- 12:28Now we move
- 12:29into
- 12:29acceptance
- 12:29and
- 12:29commitment
- 12:30therapy,
- 12:30also known
- 12:31as Act.
- 12:32this is
- 12:32sort of
- 12:33stems
- 12:33from CBT.
- 12:34It's
- 12:34a newer
- 12:34type of
- 12:35therapy.
- 12:36it's one
- 12:36that I
- 12:37definitely
- 12:37use myself
- 12:38in my
- 12:38personal
- 12:39life.
- 12:39I use
- 12:39a lot of
- 12:40these
- 12:40skills.
- 12:41so
- 12:41the main
- 12:42idea
- 12:42behind acts
- 12:44they,
- 12:44they basically
- 12:45say that
- 12:45painful
- 12:46emotions
- 12:46and
- 12:46thoughts
- 12:47are not
- 12:47the problem.
- 12:48We all have
- 12:48painful
- 12:49emotions
- 12:49and
- 12:50thoughts.
- 12:51The problem
- 12:52happens
- 12:52when we try
- 12:53to avoid
- 12:54thinking
- 12:55and feeling
- 12:55these things.
- 12:57So we say
- 12:57control
- 12:58is
- 12:58the problem,
- 12:59not
- 12:59the solution.
- 13:00so to flush
- 13:01that out
- 13:01a little
- 13:02bit, right?
- 13:03If you have
- 13:03someone who
- 13:04feels like
- 13:04they can't
- 13:05tolerate
- 13:05these
- 13:05thoughts
- 13:06and
- 13:06emotions,
- 13:06they might
- 13:07turn to
- 13:07alcohol
- 13:08or drugs
- 13:08to numb
- 13:09out.
- 13:09They might
- 13:10throw
- 13:10themselves
- 13:11into
- 13:11their work
- 13:11and neglect
- 13:12all
- 13:12the other
- 13:12parts
- 13:13of their lives,
- 13:14because
- 13:14they
- 13:14just want
- 13:15to avoid
- 13:15feeling
- 13:16and
- 13:16thinking
- 13:16these
- 13:17things.
- 13:18And so,
- 13:19you know,
- 13:20we don't
- 13:20have
- 13:21a good way
- 13:21of helping
- 13:22people
- 13:22completely
- 13:23avoid
- 13:23their
- 13:23painful
- 13:24thoughts
- 13:24and
- 13:24emotions.
- 13:25Right?
- 13:25We
- 13:25know that
- 13:26when we
- 13:26avoid them,
- 13:27it often
- 13:28ends up
- 13:28coming back
- 13:29even
- 13:29stronger.
- 13:30at least,
- 13:31I and
- 13:31the field,
- 13:32we don't
- 13:32have a way
- 13:32of getting
- 13:33rid of
- 13:33these
- 13:33things.
- 13:33If
- 13:34any of you
- 13:34do, please
- 13:34let me
- 13:35know.
- 13:35I'll be out
- 13:35of a job,
- 13:36but
- 13:36that would
- 13:36be great.
- 13:37right.
- 13:38But
- 13:38we don't
- 13:39have a way
- 13:39of getting
- 13:39rid of
- 13:39these
- 13:40things.
- 13:40And our
- 13:40attempts
- 13:41to push
- 13:41them away
- 13:42is what
- 13:42ends up
- 13:42causing
- 13:43a lot
- 13:43of issues.
- 13:45So
- 13:45with Act,
- 13:46what we do
- 13:47is we
- 13:48stop
- 13:49pulling
- 13:50on the rope.
- 13:51So what
- 13:52I mean by
- 13:52that is
- 13:53imagine
- 13:53a tug
- 13:54of war
- 13:54between
- 13:54you
- 13:54and your
- 13:55brain,
- 13:55right?
- 13:56So you have
- 13:56the painful
- 13:57thoughts
- 13:57and
- 13:57emotions
- 13:58tugging you
- 13:58and you're
- 13:59like, no,
- 13:59I don't want
- 14:00to feel
- 14:00or think
- 14:01about these
- 14:01things.
- 14:02You try and
- 14:02pull away,
- 14:03but you're
- 14:04never
- 14:04really
- 14:04successful,
- 14:05right?
- 14:05Because
- 14:05those
- 14:05painful
- 14:06emotions
- 14:06and
- 14:06thoughts
- 14:06still
- 14:07come back.
- 14:07They come
- 14:08rushing
- 14:08back.
- 14:09So with
- 14:09Act, what
- 14:10we say is,
- 14:11can we
- 14:11learn
- 14:11to just
- 14:12drop that
- 14:12rope?
- 14:13Can
- 14:13we learn
- 14:14to accept
- 14:15these
- 14:15painful
- 14:16thoughts
- 14:16and
- 14:16emotions
- 14:17that
- 14:17come up
- 14:17for us?
- 14:19And
- 14:19what act
- 14:19does is
- 14:20helps us
- 14:21change
- 14:21our
- 14:21relationships
- 14:22to these
- 14:23painful
- 14:23that and
- 14:24these
- 14:24painful
- 14:25feelings.
- 14:26So that's
- 14:26not to say
- 14:27we have to
- 14:27like these
- 14:28thoughts.
- 14:28We have
- 14:28to like
- 14:29these
- 14:29feelings.
- 14:30Right?
- 14:31But
- 14:31what we do
- 14:32is we
- 14:32say, okay,
- 14:32you know
- 14:32what?
- 14:33Our
- 14:33attempts
- 14:34to try
- 14:34and get rid
- 14:34of them,
- 14:35these attempts
- 14:35aren't
- 14:35working
- 14:36and they're
- 14:37wasting
- 14:37my time.
- 14:37They're
- 14:37wasting
- 14:38my energy.
- 14:38And so
- 14:39I'm going
- 14:39to stop
- 14:39struggling
- 14:40with them.
- 14:43And so
- 14:43with acts,
- 14:44one of the
- 14:44main points
- 14:45is that
- 14:45we help
- 14:46people
- 14:46build a
- 14:46meaningful
- 14:47life,
- 14:48even when
- 14:48there are
- 14:48painful
- 14:49parts
- 14:49of that
- 14:49life.
- 14:50Right.
- 14:50Because
- 14:50pain is
- 14:51inevitable.
- 14:51We're all
- 14:52going to
- 14:52go through
- 14:52painful
- 14:53situations,
- 14:54especially
- 14:55when
- 14:55it comes
- 14:55to cancer
- 14:55right so
- 14:56much pain
- 14:57that comes
- 14:58with that.
- 14:59and so
- 14:59with act
- 14:59we focus
- 15:00on
- 15:01mindfulness.
- 15:02When I
- 15:02say
- 15:02mindfulness
- 15:03and don't
- 15:03necessarily
- 15:04mean
- 15:04meditation,
- 15:05I
- 15:05personally
- 15:05am not,
- 15:06I don't
- 15:06find
- 15:06meditation
- 15:07is helpful
- 15:07for me.
- 15:08So when I
- 15:08say
- 15:08mindfulness,
- 15:09what I
- 15:09mean is
- 15:10being
- 15:10present
- 15:11in the moment,
- 15:12because
- 15:12so many times
- 15:14we live
- 15:14our lives
- 15:15in the
- 15:15future,
- 15:15we're
- 15:16worrying
- 15:16about
- 15:16what's
- 15:16going to
- 15:17happen,
- 15:17or we're
- 15:18ruminating
- 15:19on the past
- 15:19and
- 15:19it takes us
- 15:20out of the
- 15:21present
- 15:21moment.
- 15:21We
- 15:21miss out.
- 15:23we also
- 15:23focus on
- 15:23acceptance,
- 15:25right?
- 15:25So, how
- 15:27can we
- 15:27accept
- 15:28our
- 15:28thoughts?
- 15:29Right?
- 15:29That
- 15:29doesn't
- 15:30mean you
- 15:30have to
- 15:30like
- 15:30your thoughts
- 15:31right?
- 15:31But how can
- 15:32we change
- 15:32our
- 15:32relationship
- 15:33to them?
- 15:34And values.
- 15:35We help
- 15:36people
- 15:36identify
- 15:36what their
- 15:37values are
- 15:38and
- 15:38identify
- 15:39the
- 15:39barriers
- 15:39to living
- 15:40their life
- 15:40in
- 15:41alignment
- 15:41with
- 15:41those values,
- 15:42and then
- 15:43actually
- 15:43figure out
- 15:43a way,
- 15:44how do I do
- 15:44things
- 15:45that align
- 15:46with these
- 15:46values
- 15:46of mine,
- 15:47these things
- 15:47that are
- 15:47important
- 15:48to me?
- 15:50So some
- 15:51considerations,
- 15:52how
- 15:52do you know
- 15:52if Act is a
- 15:53good choice
- 15:54of therapy
- 15:54for you?
- 15:55So
- 15:55you're just
- 15:55feeling
- 15:56stuck,
- 15:56maybe
- 15:56stagnant in
- 15:57your life
- 15:57and in your
- 15:58thoughts?
- 15:59you
- 15:59feel like
- 15:59you
- 15:59don't know
- 16:00who you are
- 16:00or what
- 16:01your values
- 16:01are, and
- 16:02you feel a
- 16:02little bit
- 16:03lost.
- 16:04you find
- 16:04you're
- 16:05always
- 16:05trying to
- 16:05push away
- 16:06your
- 16:06thoughts,
- 16:06your
- 16:06memories,
- 16:07your emotions,
- 16:08and it
- 16:08just
- 16:08doesn't
- 16:08work.
- 16:10you're
- 16:10consumed
- 16:11by the past
- 16:12or future,
- 16:12and it's
- 16:13hard to be
- 16:13in the
- 16:13present
- 16:14in your life.
- 16:15and more
- 16:16specifically
- 16:16to cancer,
- 16:17right?
- 16:17Maybe
- 16:17you're
- 16:17denying
- 16:18your
- 16:18diagnosis.
- 16:19You have
- 16:20difficulty
- 16:20accepting
- 16:21your new
- 16:22normal.
- 16:23and this is
- 16:23very
- 16:24common,
- 16:24right?
- 16:24When you
- 16:25get, like,
- 16:25a painful
- 16:26diagnosis,
- 16:26you might
- 16:26have times
- 16:27where
- 16:27you just
- 16:28you're
- 16:28sort of
- 16:28in denial
- 16:28about it.
- 16:29And that's
- 16:29okay.
- 16:30That's not
- 16:30to say
- 16:30you
- 16:31need therapy,
- 16:31but
- 16:32if that's
- 16:32happening
- 16:32for a long
- 16:33period
- 16:33of time,
- 16:34therapy
- 16:35could
- 16:35potentially
- 16:36be helpful.
- 16:37and what's
- 16:38actually
- 16:38interesting
- 16:39is
- 16:39there's
- 16:39been
- 16:39a lot of research
- 16:40that's
- 16:40come out
- 16:41that shown
- 16:41that act
- 16:42is actually
- 16:43really
- 16:43effective
- 16:44for people
- 16:45with
- 16:45terminal
- 16:46diagnoses
- 16:46or chronic
- 16:47health
- 16:48issues.
- 16:50So this
- 16:50thing
- 16:50called a
- 16:51hexa flex.
- 16:52and
- 16:53these six
- 16:53things here
- 16:54are the
- 16:55main things
- 16:55we focus
- 16:56on.
- 16:56And but
- 16:57for today
- 16:57I'm just
- 16:57going to
- 16:58give you
- 16:58like a tiny
- 16:59preview
- 16:59window
- 17:00into
- 17:01one of these,
- 17:02which is diffusion.
- 17:03It says
- 17:04observe
- 17:04your thoughts
- 17:05without
- 17:05being ruled
- 17:06by them.
- 17:07So
- 17:09let's first
- 17:09define what
- 17:10cognitive
- 17:11fusion is.
- 17:13So
- 17:13cognitive
- 17:13fusion
- 17:13as it says
- 17:14here, is
- 17:15when
- 17:15you give
- 17:16your
- 17:16thought
- 17:17your full
- 17:17attention,
- 17:18you buy
- 17:18into
- 17:18the thought.
- 17:19You believe
- 17:20the thought
- 17:20as facts.
- 17:21And
- 17:21then what
- 17:22often
- 17:22happens is
- 17:22you fall
- 17:23into a
- 17:23thought
- 17:23spiral.
- 17:24You have
- 17:24maybe an
- 17:25unhelpful
- 17:25thought,
- 17:26and then
- 17:26it leads
- 17:27to ten
- 17:27more
- 17:27unhelpful
- 17:28thoughts,
- 17:28right?
- 17:28You create
- 17:29this whole
- 17:29narrative.
- 17:31Opposite of
- 17:31cognitive
- 17:32fusion
- 17:32is
- 17:32cognitive
- 17:33fusion,
- 17:34which is
- 17:34what
- 17:35we help
- 17:35people do
- 17:36in
- 17:36this therapy.
- 17:37So
- 17:37as it says
- 17:37here,
- 17:38we want to
- 17:38help people
- 17:39create
- 17:39distance
- 17:40between
- 17:40themselves
- 17:41and
- 17:41the thoughts
- 17:42so that
- 17:42the thoughts
- 17:42have less
- 17:43power
- 17:43over them.
- 17:44So
- 17:44instead
- 17:45of using
- 17:45these
- 17:45thoughts
- 17:45as facts,
- 17:46we try
- 17:47and view
- 17:47them
- 17:47for what
- 17:47they are.
- 17:48A string
- 17:48of letters
- 17:49that your
- 17:49brain has
- 17:50generated,
- 17:50string
- 17:51of words,
- 17:51nothing
- 17:52more,
- 17:52nothing
- 17:52less.
- 17:53It's really
- 17:53just words.
- 17:55And
- 17:55we don't
- 17:56need to
- 17:56become so
- 17:56attached,
- 17:57infused to
- 17:58our thoughts.
- 17:59But
- 17:59we also know
- 18:00that we
- 18:00can't
- 18:00get rid of
- 18:01our
- 18:01thoughts.
- 18:02So with
- 18:02cognitive
- 18:02diffusion,
- 18:03we change
- 18:04our
- 18:04relationship
- 18:05to our
- 18:05thoughts.
- 18:06We change
- 18:06how we
- 18:07respond
- 18:07to them.
- 18:08So instead
- 18:08of fully
- 18:09buying
- 18:09into them
- 18:10and
- 18:11being on
- 18:11autopilot
- 18:12and taking
- 18:12our thought
- 18:12as fact,
- 18:13we can
- 18:14notice
- 18:14the thought
- 18:14without
- 18:15judgment.
- 18:16Ask
- 18:16ourselves,
- 18:16is this
- 18:17not helpful?
- 18:18Or is this
- 18:18unhelpful?
- 18:20And if it's
- 18:20unhelpful,
- 18:21what we do
- 18:21and I'm
- 18:22going
- 18:22to talk
- 18:22in a minute
- 18:22or two
- 18:23about how
- 18:23we actually
- 18:23practically
- 18:24do this.
- 18:25We create
- 18:25some
- 18:25separation
- 18:26between
- 18:26ourselves
- 18:27and the
- 18:27thought
- 18:28that we
- 18:28can
- 18:28recenter
- 18:29our attention
- 18:30to the
- 18:30things
- 18:30that are
- 18:30more
- 18:30important
- 18:31to us.
- 18:31So with
- 18:32cognitive
- 18:32diffusion,
- 18:32we're not
- 18:33getting rid
- 18:33of the thought,
- 18:34but we're
- 18:34changing
- 18:35our
- 18:35relationship
- 18:36to it
- 18:36so that
- 18:36we can
- 18:37coexist
- 18:38with that.
- 18:40So what we
- 18:41do with
- 18:41cognitive
- 18:42diffusion
- 18:42is we ask,
- 18:42is this
- 18:43not helpful
- 18:43or
- 18:44unhelpful?
- 18:45So
- 18:45you may be
- 18:46asking
- 18:46like,
- 18:46how do we know
- 18:47if a
- 18:47thought is
- 18:47hopeful or
- 18:48unhelpful,
- 18:48right.
- 18:49So
- 18:49if you're
- 18:49diagnosed
- 18:49with
- 18:49cancer,
- 18:50right.
- 18:50And let's
- 18:51say you're
- 18:51worried
- 18:51that it's
- 18:52going to
- 18:52spread
- 18:52rate.
- 18:53And
- 18:53the thought
- 18:53is
- 18:54I'm worried
- 18:55my
- 18:55cancer is
- 18:55going to
- 18:56spread
- 18:57in some
- 18:57capacity.
- 18:58That's
- 18:58false
- 18:58actually.
- 18:59Be helpful.
- 19:00It might
- 19:00prompt you
- 19:00to do
- 19:01some
- 19:01research.
- 19:01It might
- 19:02prompt you
- 19:02to get
- 19:02your scans,
- 19:03get
- 19:03all the information
- 19:05right.
- 19:06But there
- 19:06might be times
- 19:06where
- 19:06that
- 19:07thought
- 19:07really
- 19:07isn't
- 19:07helpful,
- 19:08right?
- 19:09It just
- 19:09makes you
- 19:09ruminate.
- 19:09It
- 19:10makes you
- 19:10sad.
- 19:10It
- 19:10makes you
- 19:10worry.
- 19:11Right?
- 19:12So,
- 19:13you know,
- 19:13it's sort
- 19:14of context
- 19:14dependent
- 19:15whether
- 19:15the thought
- 19:15is helpful
- 19:16or
- 19:16unhelpful.
- 19:17So let's go
- 19:18through
- 19:18an example.
- 19:19so
- 19:19let's say
- 19:19you have
- 19:19the thought
- 19:20that
- 19:20nothing's
- 19:20ever going
- 19:21to get
- 19:21better.
- 19:22If you're
- 19:22fuzed with
- 19:23that
- 19:23thought,
- 19:23what's
- 19:24going to
- 19:24happen
- 19:24is you're
- 19:24going
- 19:24to have
- 19:25a lot
- 19:25of other
- 19:25unhelpful
- 19:26thoughts,
- 19:26right?
- 19:27You're
- 19:27going
- 19:27to have
- 19:27the thought,
- 19:28I'm going
- 19:28to feel
- 19:28this way
- 19:28forever.
- 19:29There's
- 19:29no point
- 19:30in trying
- 19:30and
- 19:30nothing's
- 19:31going to
- 19:31get better,
- 19:32and that's
- 19:32going
- 19:33to impact
- 19:33your
- 19:33behavior
- 19:34right
- 19:34Maybe you
- 19:34stop
- 19:35responding
- 19:35to phone
- 19:35calls
- 19:36and texts.
- 19:36You stay
- 19:37in bed
- 19:37all day.
- 19:37You no
- 19:38longer
- 19:38do
- 19:38the things
- 19:39that
- 19:39you once
- 19:39enjoyed
- 19:39doing.
- 19:41If we were
- 19:42to practice
- 19:42cognitive
- 19:42diffusion.
- 19:43And again
- 19:43on the
- 19:44next slide,
- 19:44we'll
- 19:44talk about
- 19:45how
- 19:45to do this.
- 19:46So
- 19:46you have
- 19:46that
- 19:47initial
- 19:47thought.
- 19:47Nothing's
- 19:48ever
- 19:48going to
- 19:48get better.
- 19:49And
- 19:50actually
- 19:50I'm gonna
- 19:51go up for
- 19:51a second.
- 19:52So one
- 19:52strategy
- 19:53for
- 19:53cognitive
- 19:54diffusion
- 19:54is when you
- 19:55notice
- 19:55yourself
- 19:56having
- 19:56a thought
- 19:56that's
- 19:57unhelpful.
- 19:57It's not
- 19:58helping
- 19:58you.
- 19:59What you do
- 19:59is you
- 19:59externalize
- 20:00the
- 20:00thoughts.
- 20:01So the way
- 20:01we do this,
- 20:02as we say
- 20:03in your
- 20:03head,
- 20:03I notice
- 20:04I'm having
- 20:05the thought
- 20:06that
- 20:07nothing's
- 20:07ever going
- 20:08to get
- 20:08better,
- 20:08right?
- 20:09Or I notice
- 20:09I'm having
- 20:10the thought
- 20:10that
- 20:12tomorrow
- 20:13might suck
- 20:14great.
- 20:14And
- 20:14what it does
- 20:15is it
- 20:15creates
- 20:15some
- 20:15distance.
- 20:16It helps us
- 20:16notice
- 20:17that, okay,
- 20:17this is
- 20:18just a
- 20:18thought,
- 20:19right.
- 20:19So with
- 20:19cognitive
- 20:19diffusion,
- 20:20right.
- 20:20The thought
- 20:21I notice
- 20:21I'm having
- 20:22the thought
- 20:22that
- 20:22nothing is
- 20:23going to
- 20:23get better
- 20:24or my brain
- 20:24is
- 20:24telling me
- 20:25nothing
- 20:25is going
- 20:25to get
- 20:26better.
- 20:26Right.
- 20:27And that
- 20:27might
- 20:27change
- 20:28your
- 20:28behavior,
- 20:28right?
- 20:28Instead of
- 20:29just
- 20:29isolating
- 20:30yourself
- 20:30right,
- 20:31you still
- 20:31might
- 20:31engage
- 20:32in your life
- 20:32and do
- 20:32things
- 20:33that feel
- 20:33meaningful
- 20:34to you.
- 20:36So a lot
- 20:37of words
- 20:37on here.
- 20:38I'm not
- 20:38going to
- 20:38go through
- 20:38all of these,
- 20:39but I just
- 20:39wanted to,
- 20:41share
- 20:41some
- 20:42examples.
- 20:43And
- 20:43actually,
- 20:44there's one
- 20:45other thing
- 20:45I want to
- 20:45share
- 20:45before
- 20:45we even go
- 20:46to the
- 20:46examples.
- 20:47an analogy
- 20:48for
- 20:48cognitive
- 20:49diffusion
- 20:49that I
- 20:49sometimes
- 20:50use is
- 20:51if
- 20:51you've ever
- 20:52been in
- 20:52a room
- 20:52that has
- 20:53a clock,
- 20:54right?
- 20:54And
- 20:54the clock
- 20:55is ticking
- 20:55and
- 20:55all of a
- 20:56sudden you
- 20:56hear, right
- 20:57the second
- 20:57hand moving
- 20:58right,
- 20:58you hear it
- 20:58ticking,
- 20:59right?
- 21:00And maybe
- 21:01you didn't
- 21:01notice it
- 21:01for
- 21:02the past
- 21:02hour,
- 21:02but
- 21:02all of a
- 21:02sudden
- 21:03you
- 21:03notice it,
- 21:03and then
- 21:04maybe you
- 21:04tune it,
- 21:05you tune
- 21:05it out,
- 21:05you don't
- 21:05notice it.
- 21:06And an
- 21:06hour later
- 21:07it comes
- 21:07back.
- 21:08That's sort
- 21:08of what
- 21:09cognitive
- 21:09diffusion
- 21:09is, is
- 21:10we create
- 21:11this degree
- 21:11of
- 21:11separation
- 21:12from
- 21:12our thought,
- 21:13where
- 21:13we can
- 21:13sort of
- 21:13maybe tune
- 21:14it out.
- 21:14It's
- 21:14sort of
- 21:15in the
- 21:15background.
- 21:16And then at
- 21:17some point
- 21:17that
- 21:17thought's
- 21:17probably
- 21:18going to
- 21:18come back.
- 21:19And so
- 21:19we use
- 21:20cognitive
- 21:20diffusion
- 21:20again and
- 21:21again
- 21:21and again.
- 21:22and I
- 21:23want to
- 21:23share an
- 21:23example of
- 21:24a time
- 21:24I use
- 21:24cognitive
- 21:25diffusion
- 21:25recently.
- 21:27so I was on
- 21:28a vacation
- 21:29with my
- 21:29family over
- 21:30the summer
- 21:30is
- 21:30the first
- 21:31vacation
- 21:31I had
- 21:31been on in
- 21:32a very
- 21:32long time.
- 21:33Really
- 21:33excited.
- 21:34And while
- 21:34we were
- 21:34there,
- 21:35my sister
- 21:36and her
- 21:36husband
- 21:36got Covid.
- 21:37and I
- 21:38was really,
- 21:38really
- 21:39worried
- 21:39that I
- 21:40was going
- 21:40to catch
- 21:41it.
- 21:41I didn't
- 21:42know
- 21:42how bad
- 21:42I would
- 21:42get it, b
- 21:44I was like,
- 21:44oh my God,
- 21:44I spent all
- 21:45this money
- 21:45on
- 21:45a vacation.
- 21:46It's
- 21:46all going
- 21:46to go down
- 21:46the drain.
- 21:47And I
- 21:47was so
- 21:48incredibly
- 21:49anxious.
- 21:50and it was
- 21:50taking me
- 21:50out of the
- 21:51present
- 21:51moment.
- 21:51I couldn't
- 21:52focus on
- 21:52what was in
- 21:53front of
- 21:53me.
- 21:53I couldn't
- 21:53enjoy
- 21:54myself.
- 21:54So I was
- 21:55like, okay,
- 21:55let me
- 21:56practice
- 21:56what I
- 21:56preach, use
- 21:57some
- 21:57cognitive
- 21:57diffusion.
- 21:58So I start
- 21:59trying
- 21:59to
- 21:59externalize
- 22:00some of
- 22:00these thoughts
- 22:01I have
- 22:01right.
- 22:01The thought
- 22:02of, I'm
- 22:02going to
- 22:02get Covid
- 22:04right
- 22:04or this
- 22:04vacation
- 22:05is ruined.
- 22:06So I start
- 22:07practicing it.
- 22:08But
- 22:08then what
- 22:09I realized
- 22:09is I start
- 22:10to have
- 22:10these
- 22:10thoughts
- 22:11of,
- 22:12you know,
- 22:12I hate that
- 22:13I'm having
- 22:14these
- 22:14anxious
- 22:14thoughts.
- 22:15You know,
- 22:15these
- 22:15thoughts
- 22:16are going
- 22:16to like,
- 22:16ruin my
- 22:17trip.
- 22:18I wish
- 22:18I didn't
- 22:19feel so
- 22:19anxious,
- 22:20right?
- 22:20I was
- 22:20sort of
- 22:21like
- 22:21fighting
- 22:21against
- 22:21that
- 22:22anxiety,
- 22:22judging
- 22:23my thoughts
- 22:24right,
- 22:24and
- 22:24becoming
- 22:25so fuzed
- 22:26with this
- 22:26thought
- 22:26that,
- 22:28oh my God,
- 22:28these
- 22:28anxious
- 22:28thoughts
- 22:29are going
- 22:29to ruin
- 22:29my trip,
- 22:30right?
- 22:30And so
- 22:31then what
- 22:31I tried
- 22:31to do
- 22:32is diffuse
- 22:33from that
- 22:34thought
- 22:34because I
- 22:34was like,
- 22:35okay,
- 22:35this thought of like,
- 22:37oh my God,
- 22:37these
- 22:37anxious
- 22:37thoughts
- 22:38are going
- 22:38to make me
- 22:38have
- 22:38an awful
- 22:39time, is
- 22:39not hopeful
- 22:40and not
- 22:40necessarily
- 22:41true.
- 22:42And so what
- 22:42I really
- 22:43tried to do
- 22:43is practice
- 22:44this
- 22:44diffusion
- 22:44and remind
- 22:45myself
- 22:45I can
- 22:46coexist
- 22:47with
- 22:47unhelpful,
- 22:48anxious
- 22:48thoughts.
- 22:49I don't
- 22:49need to get
- 22:49rid of them
- 22:50right.
- 22:51I just need
- 22:51to know
- 22:52that it's
- 22:52just a
- 22:53thought
- 22:54and that
- 22:54this
- 22:55doesn't
- 22:55have to
- 22:55control.
- 22:58so some
- 22:58other
- 22:58examples
- 22:59here.
- 22:59I'll just
- 22:59go through
- 22:59two of
- 23:00these
- 23:00very
- 23:00briefly.
- 23:02number two,
- 23:02it talks
- 23:03about name
- 23:03the story,
- 23:04the book on
- 23:05the shelf.
- 23:06right.
- 23:06So if
- 23:07there's
- 23:07a thought
- 23:08that comes
- 23:08up for you,
- 23:09maybe
- 23:10the thought
- 23:10is no
- 23:11one likes.
- 23:12Right?
- 23:12So
- 23:12what you do
- 23:13is you
- 23:14imagine
- 23:14that
- 23:14that's the,
- 23:15title of a
- 23:16book,
- 23:16right?
- 23:16That no one
- 23:17likes,
- 23:17neighbor.
- 23:18And
- 23:18instead of
- 23:19opening
- 23:19that book
- 23:20and reading
- 23:21all
- 23:21the thoughts
- 23:22that come
- 23:22thereafter
- 23:23about why
- 23:23nobody
- 23:24likes you
- 23:24and how
- 23:25awful
- 23:25you are
- 23:26and
- 23:26worthless,
- 23:27I'm opening
- 23:28that book.
- 23:28We're going
- 23:28to notice
- 23:29when that
- 23:29thought
- 23:30comes up.
- 23:30We're going
- 23:31to
- 23:31visualize
- 23:32that
- 23:32thought
- 23:33as the
- 23:33title of
- 23:34the book,
- 23:35and
- 23:35instead of
- 23:35opening
- 23:35the book,
- 23:35we're
- 23:36just going
- 23:36to place it
- 23:36back
- 23:37on the shelf.
- 23:37So it's
- 23:38no longer,
- 23:39you know,
- 23:39in
- 23:39the center
- 23:40of our
- 23:40vision, no
- 23:41pun
- 23:41intended.
- 23:41Right.
- 23:41It's in
- 23:42our
- 23:42periphery,
- 23:42right?
- 23:43The
- 23:43thoughts
- 23:43still there,
- 23:43but
- 23:44we don't
- 23:44need to
- 23:44open it.
- 23:45We don't
- 23:45need to be
- 23:45fuzed
- 23:46with it.
- 23:47there's
- 23:47some other
- 23:47ideas here,
- 23:48and I
- 23:48figure
- 23:49maybe
- 23:49we could
- 23:49send out
- 23:49these
- 23:49slides
- 23:50and people
- 23:50can look
- 23:51at some
- 23:51other ideas
- 23:52for
- 23:52practicing
- 23:52cognitive
- 23:53confusion.
- 23:55So now
- 23:56we go
- 23:56into
- 23:56dialectical
- 23:57behavioral
- 23:57therapy,
- 23:58which is another
- 23:59type
- 23:59of therapy
- 23:59that I
- 24:00specialize in
- 24:00and that I
- 24:01love.
- 24:02So it was
- 24:02initially
- 24:03created
- 24:03for people
- 24:04with
- 24:05suicidality
- 24:06and
- 24:06personality
- 24:07disorders.
- 24:08But
- 24:08what they
- 24:08found over
- 24:09the years
- 24:09is that
- 24:10it's
- 24:10effective
- 24:10for
- 24:10a lot of
- 24:11different
- 24:11mental
- 24:12health
- 24:12issues,
- 24:13particularly
- 24:14when
- 24:14people find
- 24:15there's
- 24:15a lot
- 24:15of
- 24:15instability
- 24:16in their lives.
- 24:17So whether
- 24:17that's
- 24:18intense,
- 24:18unstable
- 24:19emotions,
- 24:20low
- 24:20self-image
- 24:21issues
- 24:22in
- 24:22relationships,
- 24:24and
- 24:25what DBT
- 24:25is
- 24:25comprised
- 24:26of, it's
- 24:26a more
- 24:26intensive
- 24:27treatment
- 24:27than
- 24:27the other ones
- 24:28we were
- 24:28talking
- 24:29about.
- 24:30You have your
- 24:30individual
- 24:31therapy
- 24:31appointments
- 24:32every week.
- 24:32You have
- 24:33group
- 24:33therapy
- 24:33where you
- 24:34learn
- 24:34concrete,
- 24:35tangible
- 24:35skills
- 24:36to help you.
- 24:37And
- 24:37you also
- 24:38get
- 24:38something
- 24:38called
- 24:38phone
- 24:38coaching
- 24:39where you
- 24:39can call
- 24:40your
- 24:40therapist
- 24:40typically
- 24:41any time
- 24:42of day
- 24:42or night.
- 24:43For some
- 24:43coaching.
- 24:44And the
- 24:45four things
- 24:45DBT
- 24:46focuses on
- 24:47are
- 24:47mindfulness,
- 24:48interpersonal
- 24:49effectiveness,
- 24:49or
- 24:50helping you
- 24:50with your
- 24:50interpersonal
- 24:51relationships.
- 24:52Asserting
- 24:52yourself,
- 24:53maintaining
- 24:54these
- 24:54relationships.
- 24:55Managing
- 24:55conflict.
- 24:57Emotion
- 24:57regulation.
- 24:58So
- 24:58how do I cope
- 24:58with
- 24:59painful
- 24:59emotions
- 24:59that
- 24:59come up
- 25:00and
- 25:00distress
- 25:01tolerance?
- 25:01So
- 25:02what do I do
- 25:02when I'm in
- 25:03like
- 25:03literal
- 25:03crisis mode
- 25:04and my
- 25:04emotions
- 25:05are a ten
- 25:05out of ten?
- 25:07So who
- 25:07is DBT
- 25:08good for?
- 25:09If you find
- 25:09yourself
- 25:09struggling
- 25:10with
- 25:10self-harm
- 25:11urges
- 25:12and
- 25:12thoughts
- 25:12of suicide,
- 25:14you
- 25:14find yourself
- 25:14having
- 25:15a lot
- 25:15of
- 25:15relationship
- 25:16issues,
- 25:16both
- 25:17platonically,
- 25:17romantically.
- 25:19You feel
- 25:19a lot
- 25:20of high,
- 25:20intense
- 25:21emotions
- 25:21that lasts
- 25:22for a long
- 25:22time.
- 25:23DBT is also
- 25:24really
- 25:25helpful
- 25:25for people
- 25:25who've
- 25:26experienced
- 25:26a lot
- 25:26of trauma
- 25:27or
- 25:27invalidation.
- 25:28You've
- 25:28been told
- 25:29that
- 25:29your emotions
- 25:30are wrong.
- 25:31You're,
- 25:31too
- 25:32sensitive,
- 25:33or you find
- 25:34yourself
- 25:34just
- 25:34struggling
- 25:34with a lot
- 25:35of mental
- 25:35health
- 25:36issues.
- 25:37So I'm
- 25:38going to
- 25:38focus
- 25:38on one
- 25:39particular
- 25:40skill
- 25:41in CBT
- 25:42that I want
- 25:43to share
- 25:43with
- 25:43everybody
- 25:44here.
- 25:44It's called
- 25:44a radical
- 25:45acceptance.
- 25:45And I
- 25:45would say
- 25:46out of
- 25:46every
- 25:47therapy
- 25:48technique
- 25:48I'm
- 25:48aware of
- 25:49or skill,
- 25:50I think
- 25:50this is
- 25:50absolutely
- 25:51the
- 25:51hardest one
- 25:52and is
- 25:53also,
- 25:54can be
- 25:54the most
- 25:54life
- 25:55changing
- 25:55skill.
- 25:56So when
- 25:57I was in
- 25:57grad
- 25:57school,
- 25:58I actually
- 25:58had a
- 25:58professor
- 25:59who
- 25:59specialize
- 26:00in DVT.
- 26:00She's like
- 26:01very well
- 26:01known
- 26:01in the field,
- 26:02and she
- 26:03shared this
- 26:03equation
- 26:04with me,
- 26:04which
- 26:04I felt
- 26:05was life
- 26:05changing
- 26:06for myself.
- 26:07So
- 26:07the idea
- 26:08here
- 26:08is that
- 26:09you have
- 26:09something
- 26:10painful
- 26:10that happens
- 26:11in life.
- 26:12If we can accept
- 26:12that
- 26:12it happens,
- 26:13it doesn't
- 26:14get rid of
- 26:14the pain.
- 26:15Pain is
- 26:15still there.
- 26:16But when we
- 26:17deny that
- 26:18pain,
- 26:18so pain
- 26:19plus
- 26:19denial,
- 26:20that pain
- 26:21turns into
- 26:21suffering.
- 26:23So when
- 26:23we deny
- 26:24the pain,
- 26:24we're
- 26:24basically
- 26:24adding on
- 26:25layers
- 26:26of pain.
- 26:26And it
- 26:26intensifies
- 26:28and
- 26:28exacerbates
- 26:29the pain
- 26:29and
- 26:29turns it
- 26:30into
- 26:30suffering.
- 26:31When I
- 26:32say denial,
- 26:33what I mean
- 26:34when I go
- 26:34to the
- 26:34next slide
- 26:35real quick,
- 26:36when I
- 26:36say denial,
- 26:37but I mean
- 26:37it's
- 26:37non-acceptance,
- 26:39what does
- 26:39that
- 26:39look like
- 26:40in terms
- 26:40of thoughts
- 26:41that might
- 26:41look like,
- 26:42I can't deal
- 26:42with this.
- 26:43This isn't
- 26:44fair.
- 26:45Things
- 26:45shouldn't
- 26:45be this
- 26:46way.
- 26:46It's not
- 26:47right.
- 26:48Things
- 26:48should be
- 26:48different.
- 26:50And none of
- 26:50this is
- 26:50to say that
- 26:51it's wrong
- 26:51to have
- 26:52these
- 26:52thoughts.
- 26:52These are
- 26:53very normal
- 26:53thoughts,
- 26:54especially
- 26:54with a
- 26:54cancer
- 26:55diagnosis.
- 26:56The problem
- 26:57becomes
- 26:57when we
- 26:57get stuck
- 26:58in these
- 26:59thoughts
- 26:59for a long
- 27:00period of time.
- 27:01So this is
- 27:01what I mean
- 27:02by denial.
- 27:04and so
- 27:04what
- 27:05radical
- 27:05acceptance
- 27:05is, is
- 27:06when we
- 27:07practice
- 27:07accepting
- 27:08our situation
- 27:10and
- 27:13really
- 27:13trying to
- 27:14move away
- 27:14from that
- 27:16line of
- 27:16thinking of
- 27:17what
- 27:17should be.
- 27:18Right.
- 27:18So the idea
- 27:19here,
- 27:19suffering
- 27:20doesn't
- 27:20come from
- 27:20the pain
- 27:21itself.
- 27:22Suffering
- 27:22happens
- 27:23when we
- 27:23become
- 27:23so attached
- 27:24to what
- 27:25we feel
- 27:25life should
- 27:26look like.
- 27:27And again,
- 27:27that's not
- 27:28to say that
- 27:29cancer
- 27:29is fair
- 27:30or okay,
- 27:31or that
- 27:31we should
- 27:31be happy
- 27:32about it.
- 27:32Not at all.
- 27:33Right?
- 27:34But when we
- 27:34get lost
- 27:35in those
- 27:35thoughts
- 27:36of it
- 27:36shouldn't
- 27:36be this
- 27:36way, it
- 27:37shouldn't
- 27:37be this
- 27:37way, it's
- 27:37not fair,
- 27:38right?
- 27:39It ends up
- 27:40making
- 27:40us feel
- 27:41that much
- 27:41worse.
- 27:43and so
- 27:44acceptance
- 27:44again, it
- 27:45doesn't
- 27:45mean that
- 27:45you're
- 27:45agreeing
- 27:46with
- 27:46what's
- 27:46happening
- 27:47or
- 27:47resigning
- 27:47yourself
- 27:48to it
- 27:48or
- 27:48justifying,
- 27:49and
- 27:49it doesn't
- 27:49mean that
- 27:50you're not
- 27:50taking
- 27:50action
- 27:51to maybe
- 27:51try and
- 27:51pull yourself
- 27:53just means
- 27:53that you're
- 27:53accepting
- 27:54that this
- 27:54situation
- 27:55has
- 27:55happened
- 27:56right?
- 27:57Accepting
- 27:58what has
- 27:58happened
- 27:59in this
- 27:59present
- 27:59moment.
- 28:00I once had
- 28:01a patient
- 28:01who said
- 28:02this line
- 28:02on
- 28:02the bottom,
- 28:03which
- 28:03I love.
- 28:03She said,
- 28:04radical
- 28:04acceptance.
- 28:05It's not
- 28:05accepting
- 28:06what
- 28:06happened.
- 28:07It's
- 28:07accepting
- 28:08that
- 28:08it happens.
- 28:10so again,
- 28:11there is
- 28:11a place,
- 28:12especially
- 28:13with
- 28:13cancer,
- 28:14there is a place
- 28:15for having
- 28:16being stuck
- 28:16in denial
- 28:17and on
- 28:17acceptance.
- 28:18Absolutely.
- 28:18Grief,
- 28:19disappointment,
- 28:19anger,
- 28:20bitterness.
- 28:21It is
- 28:21so normal.
- 28:23The
- 28:23suffering
- 28:23result
- 28:24from when
- 28:26the initial
- 28:27pain is
- 28:27prolonged.
- 28:28Because
- 28:28we're stuck
- 28:29in this
- 28:29non-acceptance
- 28:30place.
- 28:33So I
- 28:34want
- 28:34to give
- 28:34two
- 28:34examples,
- 28:35radical
- 28:36acceptance
- 28:36to really
- 28:37like paint
- 28:37a picture
- 28:38as to what
- 28:38this
- 28:38looks like.
- 28:39And then
- 28:39we'll talk
- 28:39about
- 28:40practically
- 28:40how do we
- 28:40practice?
- 28:42so imagine
- 28:43that you
- 28:44are in
- 28:45prison
- 28:45for life.
- 28:46You are
- 28:46convicted
- 28:46for a crime
- 28:47you did
- 28:47not commit.
- 28:48You
- 28:49objectively
- 28:49did not
- 28:50commit
- 28:50this crime.
- 28:50You are
- 28:51falsely
- 28:51accused.
- 28:52You appealed
- 28:53the verdict.
- 28:54but
- 28:54the ruling
- 28:55wasn't
- 28:55overturned.
- 28:56And you are
- 28:56stuck
- 28:57in prison
- 28:57for life.
- 28:58right.
- 28:58So
- 28:59what are your
- 29:00options?
- 29:01Well,
- 29:01you're not
- 29:01going to
- 29:01be able
- 29:02to get out
- 29:02of jail,
- 29:03right?
- 29:03And it's
- 29:04also
- 29:05unrealistic
- 29:05to
- 29:06expect you
- 29:06to be happy
- 29:07that
- 29:07you are
- 29:07in prison
- 29:08for life.
- 29:08Right.
- 29:10But we do
- 29:11sort of
- 29:11have two
- 29:11options,
- 29:12right?
- 29:13so option
- 29:14one, right.
- 29:15You can stay
- 29:15in that
- 29:15non-acceptance
- 29:16place,
- 29:17being
- 29:17miserable
- 29:18and telling
- 29:18yourself
- 29:19this isn't
- 29:19fair,
- 29:20things
- 29:20shouldn't
- 29:21be this
- 29:21way.
- 29:21And again,
- 29:22that's not
- 29:22to say that
- 29:22those
- 29:23thoughts
- 29:23are not
- 29:23true,
- 29:24right?
- 29:24It isn't
- 29:25fair.
- 29:26Right?
- 29:26But
- 29:26those thoughts
- 29:27just aren't
- 29:27helping
- 29:28you, right?
- 29:28It's going
- 29:28to make
- 29:29you feel
- 29:29worse,
- 29:29right?
- 29:29You can spend
- 29:30every day
- 29:30crying,
- 29:31beating
- 29:31people up,
- 29:32beating
- 29:32yourself
- 29:32up.
- 29:33or you can
- 29:35radically
- 29:36accept the
- 29:36situation.
- 29:37Doesn't
- 29:37mean you're
- 29:37happy about
- 29:38it, right?
- 29:38You can be
- 29:38really angry
- 29:39about it.
- 29:40You can be
- 29:40angry about
- 29:41a situation
- 29:41and
- 29:42accept it.
- 29:42Right?
- 29:43And you can
- 29:43find a way
- 29:43to build
- 29:44a
- 29:44meaningful
- 29:45life there.
- 29:45Right.
- 29:46So one
- 29:46example of
- 29:47radical
- 29:47acceptance,
- 29:48right.
- 29:48So with
- 29:49radical
- 29:49acceptance,
- 29:49we can turn
- 29:50unendurable
- 29:51agony into
- 29:52endurable
- 29:52pain.
- 29:54One other
- 29:55example,
- 29:56that some
- 29:57of us
- 29:57can
- 29:57probably
- 29:57all relate
- 29:58to.
- 29:58You're
- 29:58driving in
- 29:59traffic.
- 30:00Let's say
- 30:00you have
- 30:00an
- 30:00important
- 30:01holiday
- 30:01dinner
- 30:02with
- 30:02your family.
- 30:03and you and
- 30:04your spouse
- 30:04are stuck
- 30:04in traffic,
- 30:05bumper
- 30:05to bumper
- 30:06traffic.
- 30:06You're
- 30:06going
- 30:07to be like
- 30:07an hour
- 30:07late,
- 30:08right?
- 30:08You can
- 30:09get
- 30:09saturated
- 30:10in your
- 30:10anger
- 30:10and
- 30:11frustration,
- 30:12which would
- 30:13look like
- 30:13maybe these
- 30:13thoughts,
- 30:14right?
- 30:14This always
- 30:15happens
- 30:15to me.
- 30:15I'm such
- 30:16an idiot.
- 30:16I should have
- 30:17left
- 30:17earlier.
- 30:18The whole day
- 30:18is ruined.
- 30:19You might
- 30:19even notice
- 30:20some
- 30:20cognitive
- 30:20distortions
- 30:21in here,
- 30:21right?
- 30:21You
- 30:21ruminate,
- 30:22your
- 30:23emotions
- 30:23intensify,
- 30:24and the
- 30:24drive
- 30:24sucks,
- 30:25right?
- 30:26Or
- 30:27we can try
- 30:28and
- 30:28practice
- 30:28some
- 30:28radical
- 30:29acceptance
- 30:29around it.
- 30:30Realize
- 30:30that given
- 30:31the
- 30:31circumstance,
- 30:31there's
- 30:32no way
- 30:32to
- 30:32change it
- 30:33right?
- 30:33These thoughts
- 30:33aren't
- 30:33going to
- 30:34help you
- 30:34change
- 30:34the
- 30:34outcome.
- 30:36Right?
- 30:36And it's
- 30:37normal
- 30:37to feel
- 30:37frustrated,
- 30:38right?
- 30:39that makes
- 30:40sense,
- 30:40right?
- 30:41But maybe
- 30:41instead
- 30:42of these
- 30:42initial
- 30:42thoughts,
- 30:43we practice
- 30:43medical
- 30:44acceptance
- 30:44and have
- 30:45these
- 30:45thoughts.
- 30:45Right?
- 30:45I'm going
- 30:46to be late.
- 30:46I'm
- 30:46disappointed.
- 30:48I wish
- 30:48I left
- 30:48earlier,
- 30:49right?
- 30:49Instead
- 30:49of that,
- 30:50I should have
- 30:50right?
- 30:51And I
- 30:51can't
- 30:51change the
- 30:51situation.
- 30:52And instead
- 30:53of
- 30:53ruminating
- 30:53and your
- 30:53emotions
- 30:54intensify,
- 30:55your
- 30:55emotions
- 30:55remain
- 30:56the same.
- 30:56But maybe
- 30:57you listen
- 30:57to a
- 30:57podcast,
- 30:58right?
- 30:58Or
- 30:58you have
- 30:58like an
- 30:58interesting
- 30:59conversation
- 30:59with your
- 31:00spouse.
- 31:02So how do
- 31:02we actually
- 31:03practice
- 31:04radical
- 31:04acceptance?
- 31:05it's not
- 31:06easy.
- 31:07You don't
- 31:07arrive at
- 31:08a destination
- 31:09where
- 31:09you have
- 31:09a radically
- 31:10accepted
- 31:10a situation.
- 31:11We
- 31:11vacillate.
- 31:12It's normal
- 31:13to
- 31:13vacillate
- 31:13between
- 31:13acceptance
- 31:14and
- 31:14non-acceptance,
- 31:15right?
- 31:15So we
- 31:15constantly
- 31:16have to
- 31:17sort of
- 31:17pull our
- 31:17brains
- 31:18back
- 31:18to this
- 31:18radical
- 31:19acceptance
- 31:19place.
- 31:20So what
- 31:20we do,
- 31:21we allow
- 31:22ourselves
- 31:22to first
- 31:22notice
- 31:23any urges.
- 31:24We have to
- 31:24not want
- 31:25to accept
- 31:26the
- 31:26situation.
- 31:27Right?
- 31:27So practice
- 31:28radical
- 31:28acceptance
- 31:29around your
- 31:29reluctance
- 31:30to want
- 31:30to practice
- 31:31radical
- 31:31acceptance.
- 31:32Right.
- 31:33validate
- 31:34that it's
- 31:34normal,
- 31:34that, okay,
- 31:35maybe
- 31:35in this
- 31:35moment
- 31:35I want to stay
- 31:36in this
- 31:36not
- 31:36acceptance
- 31:37based.
- 31:38Describe
- 31:38in detail
- 31:39what
- 31:39you need
- 31:39to accept
- 31:40without
- 31:40exaggerating
- 31:41or
- 31:41minimizing
- 31:42just
- 31:42the fact.
- 31:43When we are
- 31:44in pain,
- 31:45we tend to
- 31:45catastrophize
- 31:47right?
- 31:47And so what
- 31:48we want
- 31:48to do here
- 31:49is
- 31:49we want to
- 31:49radically
- 31:49accept
- 31:50just
- 31:50the facts
- 31:50of the
- 31:51situation.
- 31:51Not all
- 31:52the worst
- 31:52case
- 31:53scenarios.
- 31:54Think
- 31:55of the
- 31:55benefits of
- 31:55radically
- 31:56accepting
- 31:56the
- 31:56situation.
- 31:57I'll made
- 31:57it make
- 31:57me feel
- 31:57better.
- 31:59Remind
- 31:59yourself
- 31:59in this
- 32:00moment,
- 32:00reality
- 32:01cannot be
- 32:01changed.
- 32:01That's
- 32:02not to say
- 32:02you can't
- 32:03try and
- 32:03change
- 32:04things
- 32:04in the future,
- 32:05but in this
- 32:05moment,
- 32:06reality is
- 32:06as it is.
- 32:08Notice
- 32:09those should
- 32:10statements,
- 32:11right?
- 32:11Or notice
- 32:12any
- 32:12judgments
- 32:12you're
- 32:12having of
- 32:13yourself.
- 32:14Avoid
- 32:15labeling
- 32:15your emotions
- 32:16as good
- 32:16or bad.
- 32:18Remind
- 32:18yourself
- 32:18that there
- 32:18are causes
- 32:19for this
- 32:19reality
- 32:20outside of
- 32:20your
- 32:20control,
- 32:21right?
- 32:21If we're
- 32:21thinking
- 32:22about
- 32:22cancer
- 32:23there,
- 32:23right?
- 32:23We're not
- 32:24saying
- 32:24that,
- 32:24you know,
- 32:25we want you
- 32:26to get
- 32:26that
- 32:26diagnosis
- 32:27right
- 32:28or that
- 32:28we're happy
- 32:28about it.
- 32:29Right.
- 32:29But
- 32:29there are
- 32:30causes
- 32:30for this
- 32:30reality,
- 32:31right?
- 32:31Maybe
- 32:32there was
- 32:32some
- 32:32genetic
- 32:33predisposition.
- 32:33Right.
- 32:34And even
- 32:34if we
- 32:35don't
- 32:35understand
- 32:35the causes
- 32:36or know
- 32:36what
- 32:36they are,
- 32:37right,
- 32:37we we, we,
- 32:38benefit
- 32:39from just
- 32:39accepting
- 32:40there are causes,
- 32:41even if
- 32:41we're
- 32:41not sure
- 32:41what
- 32:42they are
- 32:43allowing
- 32:43yourself
- 32:44to feel
- 32:44any emotion
- 32:45that arises
- 32:46and
- 32:47validate
- 32:47those
- 32:47emotions
- 32:48for yourself
- 32:49and also
- 32:49remind
- 32:50yourself
- 32:50these
- 32:50emotions
- 32:51are
- 32:51temporary.
- 32:51There's
- 32:52never been
- 32:52an emotion
- 32:52that you
- 32:53have
- 32:53experienced
- 32:54for weeks
- 32:55and weeks
- 32:55on end,
- 32:56right?
- 32:57Emotions
- 32:57come,
- 32:57they go,
- 32:58they leave,
- 32:59and
- 32:59they come
- 32:59back.
- 33:00imagine
- 33:01what life
- 33:01would be
- 33:01like
- 33:01if you
- 33:02accepted
- 33:02this
- 33:02situation,
- 33:03right?
- 33:03What would you
- 33:03do?
- 33:04What
- 33:04would you be
- 33:04doing with
- 33:05your time
- 33:05and try
- 33:06doing
- 33:06those things.
- 33:07and then we
- 33:08can also
- 33:08use
- 33:08relaxation,
- 33:09mindfulness
- 33:10to help
- 33:10us cope
- 33:10with
- 33:11painful
- 33:11emotions
- 33:11that come.
- 33:14And I said
- 33:14this
- 33:15earlier on
- 33:15in the
- 33:15PowerPoint.
- 33:16Right.
- 33:16But
- 33:16accepting
- 33:17that life
- 33:17can be
- 33:17worth
- 33:18living
- 33:19even when
- 33:19we
- 33:19experience
- 33:20such awful,
- 33:21painful,
- 33:21horrendous
- 33:22things
- 33:22in our lives.
- 33:24So the last
- 33:25therapy
- 33:26to briefly
- 33:26talk about
- 33:27is
- 33:27psychodynamic
- 33:28therapy.
- 33:28and I
- 33:30will also
- 33:31share, as
- 33:31a caveat
- 33:32that I do
- 33:33not
- 33:33specialize
- 33:33in
- 33:33psychodynamic
- 33:34therapy.
- 33:35so I'm
- 33:35probably
- 33:36not going
- 33:36to do this
- 33:36one
- 33:36justice,
- 33:37but
- 33:37I wanted to
- 33:37include it,
- 33:38because I
- 33:39myself,
- 33:39I've been
- 33:39in
- 33:39psychodynamic
- 33:40therapy,
- 33:40even though
- 33:41I don't
- 33:41specialize
- 33:42in it.
- 33:42I think
- 33:42it's a
- 33:43wonderful,
- 33:43wonderful
- 33:44type of
- 33:44therapy
- 33:45can be
- 33:45incredibly
- 33:46helpful.
- 33:47so
- 33:47it's
- 33:47more of
- 33:48that
- 33:48traditional
- 33:48talk
- 33:48therapy
- 33:49that maybe
- 33:49you see on
- 33:50like a
- 33:50TV show
- 33:51or movie.
- 33:52main idea
- 33:52that helps
- 33:53people
- 33:53recognize
- 33:54repressed
- 33:54and
- 33:54unconscious
- 33:55emotions
- 33:56and
- 33:56thoughts
- 33:56and beliefs
- 33:57that might
- 33:58be
- 33:58impacting
- 33:58their
- 33:59behavior.
- 34:00there's
- 34:00a huge
- 34:00focus
- 34:01on early
- 34:02childhood
- 34:03experiences
- 34:04or early
- 34:04life
- 34:04experiences,
- 34:05and helping
- 34:06you
- 34:06understand
- 34:07how those
- 34:07things
- 34:07have shaped
- 34:08and molded
- 34:09you
- 34:09and
- 34:09impacted
- 34:10and
- 34:10continue
- 34:11to impact
- 34:11your
- 34:11current
- 34:12relationships
- 34:13that you've
- 34:13you,
- 34:13yourselves,
- 34:14and the
- 34:14world
- 34:14around you.
- 34:18some
- 34:18considerations
- 34:19here.
- 34:19Who is
- 34:19psychodynamic
- 34:20therapy
- 34:20good for?
- 34:22it's
- 34:22specifically
- 34:22good if
- 34:23you feel
- 34:23like you
- 34:24want to
- 34:24be
- 34:24self-reflective.
- 34:25Right.
- 34:26If you have
- 34:26the capacity
- 34:26to do that,
- 34:28and think
- 34:28more
- 34:28deeply,
- 34:29share
- 34:29more
- 34:29deeply,
- 34:29this can be
- 34:30really good
- 34:30for you.
- 34:31you're
- 34:31willing to
- 34:32delve
- 34:32deeper
- 34:32into
- 34:32your past?
- 34:34you're
- 34:34looking for
- 34:34longer term
- 34:35therapy.
- 34:35Psychodynamic
- 34:36therapy is
- 34:36a longer
- 34:37term
- 34:37therapy.
- 34:37The first
- 34:38three that
- 34:38I mentioned
- 34:39are more
- 34:39short
- 34:40term, short
- 34:41term.
- 34:41We're
- 34:41talking
- 34:42maybe six
- 34:42months to
- 34:43a year,
- 34:43maybe
- 34:43a few years
- 34:44psychodynamic
- 34:45therapy.
- 34:45Sometimes
- 34:46that can be
- 34:46a decade.
- 34:47psychodynamic
- 34:48therapy,
- 34:49you know,
- 34:49it's not
- 34:49as much of
- 34:50a focus on
- 34:51like
- 34:51concrete,
- 34:51tangible
- 34:52strategies,
- 34:54versus
- 34:54like the
- 34:54first three
- 34:55where it's
- 34:55a little
- 34:55bit more
- 34:56concrete.
- 34:58and so
- 34:59if you're
- 34:59looking for
- 34:59a place
- 35:00to just
- 35:00sort of
- 35:00talk
- 35:01openly,
- 35:01freely,
- 35:02not really
- 35:02have much
- 35:03structure
- 35:03to your
- 35:03session,
- 35:04right.
- 35:05psychodynamic
- 35:06therapy can
- 35:06be helpful.
- 35:08an analogy
- 35:08I use for
- 35:09this is
- 35:09if
- 35:09your house
- 35:10is on fire,
- 35:11the first
- 35:12three types
- 35:12of therapy
- 35:13can
- 35:13be helpful
- 35:13because
- 35:14they're
- 35:14focused
- 35:14on the
- 35:14present
- 35:15moment
- 35:15and helping
- 35:16you
- 35:16figure out
- 35:17how to
- 35:17problem
- 35:17solve.
- 35:18How
- 35:18do I fix
- 35:19this
- 35:19once you're
- 35:21out of the
- 35:21burning
- 35:22house?
- 35:23Right.
- 35:23And now
- 35:23your house.
- 35:24Awfully
- 35:25has burnt
- 35:25down,
- 35:26right?
- 35:26Psychodynamic
- 35:26therapy
- 35:27can be
- 35:27really
- 35:27helpful to
- 35:28assess
- 35:28and to
- 35:28talk
- 35:29through
- 35:29those things.
- 35:30So just
- 35:31to
- 35:32highlight
- 35:32the
- 35:32differences,
- 35:33the first
- 35:33three
- 35:34therapies
- 35:35are more
- 35:35short term.
- 35:36They're
- 35:36more
- 35:36focused on
- 35:37the
- 35:37present.
- 35:38They're
- 35:38more goal
- 35:38oriented.
- 35:39So
- 35:39what are my goals.
- 35:40How
- 35:40am I going
- 35:41to get
- 35:41there.
- 35:41And it's
- 35:41more
- 35:41structured.
- 35:42The
- 35:42sessions
- 35:43are more
- 35:43structured.
- 35:44You're
- 35:44likely
- 35:45going to
- 35:45have
- 35:45homework in
- 35:46between
- 35:46sessions.
- 35:48we always
- 35:48say 80%
- 35:49of change
- 35:49happens
- 35:50outside of
- 35:50session.
- 35:51Only 20%
- 35:52happens
- 35:52inside of
- 35:53session.
- 35:53So homework
- 35:54might
- 35:54look like
- 35:55I might
- 35:56tell a
- 35:56client,
- 35:56I want you
- 35:57to exercise
- 35:57three times
- 35:58this week.
- 35:58I want you
- 35:58to
- 35:58challenge
- 35:59your
- 35:59thoughts.
- 35:59I want
- 36:00you to
- 36:00hang out
- 36:00with a
- 36:00friend.
- 36:01Psychodynamic
- 36:02therapy
- 36:02more long
- 36:03term,
- 36:04more
- 36:04focused on
- 36:05both
- 36:05the past
- 36:06and present
- 36:06and is
- 36:06less
- 36:07structured.
- 36:09So
- 36:09wrapping
- 36:10up here,
- 36:11challenges
- 36:12and
- 36:12suggestions.
- 36:13If you are
- 36:14looking for
- 36:15a
- 36:15therapist,
- 36:16and let's
- 36:16see you go
- 36:16to
- 36:17Psychology
- 36:17TODAY.com.
- 36:18Great place
- 36:18to look for
- 36:19a therapist.
- 36:20And you
- 36:20leave
- 36:21this talk
- 36:21and you're
- 36:21like,
- 36:22I want
- 36:22acceptance
- 36:23and
- 36:23commitment
- 36:23therapy
- 36:25when you're
- 36:25looking
- 36:26online.
- 36:26Many
- 36:27therapists
- 36:27will say
- 36:28that they
- 36:28specialize
- 36:29in every
- 36:29therapy
- 36:30under
- 36:30the sun,
- 36:31when
- 36:31in fact
- 36:31they do not.
- 36:33So if
- 36:33you go to
- 36:33a
- 36:33therapist,
- 36:35what you can
- 36:35do is
- 36:36ask them
- 36:36even some
- 36:37questions
- 36:38based on
- 36:38my
- 36:38PowerPoint
- 36:39here and
- 36:39be like,
- 36:39hey,
- 36:40I want like
- 36:40acceptance
- 36:41and
- 36:41commitment
- 36:41therapy.
- 36:42I saw
- 36:42you post
- 36:43that
- 36:43online, but
- 36:43like I want
- 36:44to get
- 36:44a better
- 36:44sense
- 36:44of like
- 36:45how much
- 36:46you
- 36:46actually
- 36:46incorporate
- 36:47that into
- 36:47your work
- 36:48because
- 36:48oftentimes
- 36:48people say
- 36:49they do
- 36:49one thing,
- 36:49but they
- 36:50don't
- 36:50actually,
- 36:52what's
- 36:52really
- 36:52important
- 36:53to note,
- 36:53research
- 36:54tells us
- 36:54that the
- 36:55therapeutic
- 36:55relationship.
- 36:56So how
- 36:57you feel
- 36:57about your
- 36:58therapist
- 36:58and the
- 36:58relationship
- 36:59there is
- 37:00the biggest
- 37:01predictor
- 37:01for
- 37:01positive
- 37:02outcomes
- 37:02in therapy.
- 37:04so
- 37:04at the end
- 37:05of the day,
- 37:05the therapy
- 37:06modality
- 37:07isn't
- 37:07as
- 37:07important
- 37:08as the
- 37:08relationship.
- 37:09So don't
- 37:10be afraid
- 37:10to
- 37:10shop around
- 37:11if you feel
- 37:12like
- 37:12you and
- 37:12your
- 37:12therapist
- 37:12are not
- 37:13a good fit.
- 37:14If you have
- 37:14financial
- 37:14constraints,
- 37:15especially
- 37:16with a
- 37:16cancer
- 37:16diagnosis,
- 37:17that often
- 37:17comes up
- 37:18and you
- 37:18can't
- 37:19afford
- 37:19to see
- 37:19a therapist.
- 37:20if you
- 37:21go to,
- 37:22like a
- 37:22local
- 37:22university,
- 37:23oftentimes
- 37:24they have
- 37:24clinics
- 37:24that are
- 37:25being run
- 37:25by
- 37:26therapists
- 37:26in
- 37:26training,
- 37:27and they're
- 37:27being
- 37:27supervised
- 37:28by seasoned
- 37:29therapists.
- 37:29So
- 37:29you should
- 37:30get good
- 37:30quality
- 37:31therapy.
- 37:31Oftentimes
- 37:32they offer
- 37:32very cheap
- 37:33sliding
- 37:34scale
- 37:34therapy
- 37:34for as
- 37:35little
- 37:35as like
- 37:355 to $10.
- 37:37Group
- 37:37therapy
- 37:38can also
- 37:38be really
- 37:39helpful,
- 37:39especially
- 37:39when we're
- 37:40thinking
- 37:40about
- 37:40cancer
- 37:41and how
- 37:41isolating
- 37:42it can be.
- 37:43It can help
- 37:43decrease
- 37:44that
- 37:44loneliness
- 37:44or shame
- 37:45that you're
- 37:45feeling
- 37:46make
- 37:46you feel
- 37:47more
- 37:47connected.
- 37:48also,
- 37:48there are
- 37:49therapists
- 37:49who
- 37:49specialize
- 37:50in cancer
- 37:50or chronic
- 37:51health
- 37:51issues that
- 37:52can be
- 37:52helpful to
- 37:53look into.
- 37:54and
- 37:54ironically,
- 37:55I also
- 37:55believe not
- 37:56everybody
- 37:56needs
- 37:57therapy.
- 37:57I think
- 37:57people can
- 37:58benefit
- 37:58from it,
- 37:58but I also
- 37:59don't think
- 38:00we
- 38:00always
- 38:00need to be
- 38:01in therapy.
- 38:03so
- 38:03with that,
- 38:04I guess
- 38:05I will
- 38:05open it up
- 38:06and see
- 38:06if anybody
- 38:07has any
- 38:08questions.
- 38:09And I'm
- 38:09looking at
- 38:09the chat
- 38:09and seeing
- 38:10there are
- 38:10potentially
- 38:11questions
- 38:11in there.
- 38:13Yeah.
- 38:13I see
- 38:13the same.
- 38:14I see
- 38:14question
- 38:15from Val
- 38:15and she
- 38:16just says,
- 38:16what's
- 38:16your
- 38:17recommendation
- 38:18for the
- 38:18occasional
- 38:19brief
- 38:20backsliding
- 38:20into the
- 38:21grieving
- 38:21process
- 38:22of a
- 38:22om
- 38:23diagnosis?
- 38:23Yes.
- 38:24I live
- 38:24my life
- 38:25pretty much
- 38:26in between
- 38:26those
- 38:26quarterly
- 38:27scans,
- 38:28and I
- 38:28feel
- 38:28like I'm
- 38:28doing
- 38:29great,
- 38:29and then
- 38:29I'm not.
- 38:30And I
- 38:30tend to
- 38:31judge myself
- 38:31for having
- 38:32these
- 38:32feelings,
- 38:32even if
- 38:33briefly.
- 38:34can I just
- 38:34before
- 38:35you jump
- 38:35in, I will
- 38:36just say
- 38:36that
- 38:36I think
- 38:37that when
- 38:37you have
- 38:38quarterly
- 38:38scans,
- 38:39it's like
- 38:40a new level
- 38:41of hell.
- 38:42It's it's
- 38:43it's own
- 38:44crazy
- 38:45because
- 38:45when you
- 38:46have scans
- 38:46once a
- 38:46year,
- 38:47you're
- 38:47kind of
- 38:48thrust back
- 38:48into that
- 38:49place
- 38:49every 12
- 38:50months,
- 38:50maybe every
- 38:5111 months
- 38:52if
- 38:52you're, you
- 38:52know,
- 38:52lead up is
- 38:53is build
- 38:54ups,
- 38:54is that
- 38:55build up
- 38:55is lasting
- 38:56for a month
- 38:56or two.
- 38:57When you
- 38:57have scans
- 38:58every three
- 38:58months,
- 38:58you can
- 38:59almost
- 38:59barely
- 39:00get through
- 39:00the scan
- 39:01and the
- 39:01recovery
- 39:02process
- 39:02from the
- 39:02trauma
- 39:03of having
- 39:03the scan
- 39:04all over
- 39:04again,
- 39:04and the
- 39:05fear coming
- 39:05back up,
- 39:06and then
- 39:06you're
- 39:07just thrust
- 39:07right back
- 39:07into it
- 39:08again.
- 39:08So
- 39:10just
- 39:10knowing
- 39:10that has
- 39:11helped me,
- 39:11at least
- 39:12over
- 39:12the last
- 39:12three years
- 39:13of
- 39:13of having
- 39:14scans every
- 39:15three
- 39:15months.
- 39:16the other
- 39:17thing too,
- 39:17is that
- 39:18if you are
- 39:18finding it
- 39:19too hard
- 39:20to deal
- 39:20with,
- 39:21just,
- 39:22you know,
- 39:22having the
- 39:22conversation
- 39:23with your
- 39:23medical
- 39:24oncologist
- 39:24and just
- 39:25saying,
- 39:25look,
- 39:25this is
- 39:26affecting me
- 39:27on an
- 39:27emotional
- 39:28and a
- 39:28mental
- 39:28capacity
- 39:29level,
- 39:29like
- 39:30I need to
- 39:30I need to
- 39:31talk about
- 39:31maybe
- 39:31doing scans
- 39:32every four
- 39:32months
- 39:33or every
- 39:33six months
- 39:34and just
- 39:34consider a
- 39:35different
- 39:35frequency
- 39:36just for you
- 39:36for a time,
- 39:37so that
- 39:37you can
- 39:38kind of
- 39:38get a
- 39:38better
- 39:38handle on
- 39:39how you're
- 39:39feeling,
- 39:40because
- 39:41that is an
- 39:41important
- 39:41part
- 39:42of what
- 39:42you
- 39:42experience
- 39:43in this
- 39:43diagnosis
- 39:43for sure.
- 39:45I would
- 39:45totally agree
- 39:45with
- 39:45everything
- 39:46you said.
- 39:46danet
- 39:47I think
- 39:48to Val
- 39:48like
- 39:48something
- 39:49that I
- 39:49think is
- 39:49so key
- 39:50here.
- 39:50And what
- 39:51you said
- 39:51in
- 39:51your last
- 39:51sentence
- 39:52of,
- 39:52your chat,
- 39:53I
- 39:53judge myself
- 39:54for having
- 39:55the
- 39:55feelings,
- 39:55even if
- 39:56briefly.
- 39:57Right.
- 39:58and that's
- 39:58where the
- 39:59radical
- 39:59acceptance
- 40:00piece
- 40:00can be
- 40:00really
- 40:01helpful,
- 40:02accepting
- 40:03that,
- 40:03radically
- 40:03accepting
- 40:04these
- 40:04feelings
- 40:05and,
- 40:06you know,
- 40:06really
- 40:07working on
- 40:07noticing
- 40:08when you're
- 40:08judging
- 40:09yourself.
- 40:10Right?
- 40:10Because
- 40:11you're
- 40:11absolutely
- 40:11right.
- 40:12This is as
- 40:13human
- 40:13beings,
- 40:13we do this
- 40:14all the
- 40:14time,
- 40:14right?
- 40:15We feel sad
- 40:16because
- 40:16we're
- 40:16anxious,
- 40:17right?
- 40:17We're like,
- 40:17oh, like,
- 40:18I hate
- 40:18that.
- 40:18I feel
- 40:19so anxious.
- 40:19This sucks,
- 40:21right?
- 40:21We're
- 40:21judging
- 40:22our
- 40:22anxiety,
- 40:22our
- 40:23emotion,
- 40:23or
- 40:23ourselves,
- 40:24and
- 40:24it makes us
- 40:24feel worse
- 40:25where
- 40:25we have
- 40:25anxiety
- 40:26about our
- 40:26anxiety.
- 40:26Right?
- 40:27Oh my God,
- 40:27I know
- 40:27before
- 40:28my scans
- 40:28of
- 40:29conscious
- 40:29oh my God.
- 40:30On top
- 40:30of it.
- 40:31Exactly
- 40:31right.
- 40:32Like,
- 40:32oh my God,
- 40:32I know
- 40:32these next
- 40:33few weeks
- 40:33are going
- 40:33to be
- 40:33awful.
- 40:34I'm going
- 40:34to be
- 40:34so anxious
- 40:35and I
- 40:35don't want
- 40:35to be
- 40:35anxious
- 40:35and anxious
- 40:36about
- 40:36feeling
- 40:36anxious.
- 40:37Right.
- 40:38And so
- 40:38I think
- 40:39one of
- 40:39the biggest
- 40:39things
- 40:39about is
- 40:40I think
- 40:40these emotions
- 40:41that
- 40:41come up,
- 40:41they're
- 40:42so normal
- 40:43and
- 40:43they suck.
- 40:44it sucks.
- 40:45There's no
- 40:46way around
- 40:46it.
- 40:47I think
- 40:47one of the
- 40:47best things
- 40:48to do is,
- 40:49like I
- 40:49said,
- 40:49notice
- 40:50when you're
- 40:50having
- 40:50those
- 40:50judgments,
- 40:51right?
- 40:51Notice
- 40:51when you're
- 40:52judging
- 40:52yourself
- 40:52and saying,
- 40:53oh, why
- 40:53am I
- 40:54feeling
- 40:54this way?
- 40:55Right.
- 40:55Because
- 40:55you can't
- 40:56control
- 40:56your
- 40:56emotions,
- 40:57right?
- 40:57We cannot.
- 40:58Our
- 40:58emotions
- 40:59happen,
- 41:00regardless
- 41:00of
- 41:00whether we
- 41:01want them
- 41:01to or not.
- 41:02Right.
- 41:02And like
- 41:02I was
- 41:03saying,
- 41:03can
- 41:03we find a way
- 41:05to change
- 41:05our
- 41:05relationship
- 41:06to our
- 41:06emotions?
- 41:07Right.
- 41:07So
- 41:07one thing
- 41:07we can do
- 41:08is ask
- 41:08ourselves,
- 41:09what is the
- 41:09function of
- 41:10this
- 41:10emotion,
- 41:11right.
- 41:11What's the
- 41:12function
- 41:12of anger?
- 41:12I was
- 41:13talking to
- 41:13a client
- 41:13about this
- 41:14the other day,
- 41:14right?
- 41:15We have anger
- 41:15because it
- 41:16indicates
- 41:17to us
- 41:17that
- 41:18our goal
- 41:18has been
- 41:18blocked
- 41:19or
- 41:19something
- 41:19important
- 41:20to us,
- 41:21is being
- 41:21impeded
- 41:21against.
- 41:22Right?
- 41:22Or a value
- 41:23of ours
- 41:23is being
- 41:23blocked
- 41:24Right.
- 41:24An anger
- 41:24motivates
- 41:25us to fight
- 41:26to change
- 41:27it, right?
- 41:28What's the
- 41:28function
- 41:28of anxiety?
- 41:29Anxiety
- 41:30tells us
- 41:30that
- 41:30something is wrong
- 41:32and it
- 41:32motivates
- 41:33us to
- 41:33fix it.
- 41:33Now,
- 41:33of course,
- 41:34especially
- 41:35with
- 41:35cancer,
- 41:35right.
- 41:35We can't
- 41:36necessarily
- 41:36always do
- 41:37that,
- 41:38right?
- 41:38But
- 41:38it can be
- 41:38helpful
- 41:39instead
- 41:39of judging
- 41:39our
- 41:39emotions
- 41:40to get
- 41:40curious
- 41:41about it
- 41:41right.
- 41:42What
- 41:42is my emotion
- 41:42trying to
- 41:43tell me?
- 41:44And
- 41:44instead of
- 41:44viewing it
- 41:44as this
- 41:45awful
- 41:45thing,
- 41:46right,
- 41:46trying to
- 41:47view it
- 41:47as okay,
- 41:47it's
- 41:48my brain
- 41:48is
- 41:49trying to
- 41:49send me
- 41:49a message,
- 41:50what is this
- 41:50message?
- 41:51And then
- 41:52pairing
- 41:52that
- 41:52with some
- 41:53coping
- 41:53skills
- 41:53and
- 41:54especially
- 41:54something
- 41:54like that
- 41:55acceptance
- 41:56and
- 41:56commitment
- 41:56therapy
- 41:57or
- 41:57dialectical
- 41:58behavioral
- 41:58therapy,
- 41:59those
- 41:59can offer
- 41:59really,
- 42:00really
- 42:00hopeful
- 42:01tools
- 42:02to help
- 42:02calm you
- 42:03down.
- 42:03It's not
- 42:03going to
- 42:03get rid
- 42:04of the
- 42:04emotion,
- 42:04right?
- 42:04It makes
- 42:05sense that
- 42:05you're
- 42:05anxious.
- 42:06So
- 42:06understandable.
- 42:07Right.
- 42:07But it's
- 42:08going
- 42:08to help
- 42:08you
- 42:08feel like
- 42:08you can
- 42:09cope
- 42:09with it
- 42:09a little
- 42:09bit
- 42:10better
- 42:10and
- 42:10take the
- 42:10edge off.
- 42:13Oh,
- 42:13that's such
- 42:14a good
- 42:14point.
- 42:14And I
- 42:15just
- 42:15wanted to
- 42:16to
- 42:16point out
- 42:16something
- 42:16that
- 42:17you said
- 42:17earlier in
- 42:18the session,
- 42:18and it was
- 42:19so I guess
- 42:19it was
- 42:19just
- 42:20towards
- 42:20the end
- 42:20of your,
- 42:20your
- 42:21radical
- 42:21acceptance
- 42:22section.
- 42:22It was
- 42:23this idea
- 42:23that it's
- 42:23not there's
- 42:24not some
- 42:24destination
- 42:25to that
- 42:26you reach,
- 42:27right?
- 42:27There's not
- 42:27some
- 42:27destination
- 42:28in therapy
- 42:29in, in
- 42:29self-help
- 42:30and in
- 42:31mental
- 42:31health
- 42:32that you
- 42:32just
- 42:32ultimately
- 42:33reach
- 42:33this point
- 42:34where
- 42:34you just
- 42:34suddenly
- 42:34never have
- 42:35an issue,
- 42:35you never have
- 42:36an
- 42:36intrusive
- 42:37thought
- 42:37again, or
- 42:38you never
- 42:38have
- 42:38a moment
- 42:39where
- 42:39you're
- 42:39judging
- 42:40your emotions
- 42:40when they come
- 42:41up.
- 42:41Those are always
- 42:42going to be
- 42:42things
- 42:43that
- 42:43happen.
- 42:43The
- 42:43differences,
- 42:44your
- 42:44capacity
- 42:45and your
- 42:46awareness
- 42:46around
- 42:47them,
- 42:47your capacity
- 42:48to
- 42:48handle it,
- 42:48and
- 42:48the tools
- 42:49that you
- 42:49have.
- 42:50when,
- 42:50say, fear
- 42:51and anxiety
- 42:51come up
- 42:52versus
- 42:53maybe
- 42:53the tools
- 42:54and
- 42:54the resources
- 42:55you had
- 42:56before
- 42:56you were
- 42:56involved
- 42:57in therapy,
- 42:58or you did
- 42:58some
- 42:58self-help,
- 42:59or you read
- 43:00a book
- 43:00or you,
- 43:01you know,
- 43:01experienced
- 43:02the podcast,
- 43:02you
- 43:03were part
- 43:03of a group
- 43:03therapy
- 43:04group.
- 43:05there's,
- 43:05someone
- 43:05that I
- 43:05listened to
- 43:06and
- 43:06they talked
- 43:06about,
- 43:07how about this
- 43:08idea that
- 43:09when we
- 43:09are
- 43:09experiencing
- 43:10fear,
- 43:11it's that
- 43:12we're
- 43:12forgetting
- 43:13every
- 43:13available
- 43:14resource.
- 43:15And I love
- 43:15that
- 43:16acronym,
- 43:16because
- 43:18a lot of
- 43:18the times
- 43:19we get
- 43:19caught
- 43:19in that,
- 43:20that spiral
- 43:20of the
- 43:21judging
- 43:21of the
- 43:22thoughts
- 43:22and having
- 43:22of the
- 43:23thoughts
- 43:23and the
- 43:24the kind of
- 43:24like you
- 43:25said,
- 43:25they just
- 43:25kind of
- 43:25keep going.
- 43:27And,
- 43:28ultimately,
- 43:28when we're
- 43:28experiencing
- 43:29fear,
- 43:29which
- 43:29I think
- 43:30is probably
- 43:30one of the
- 43:30most common
- 43:31feelings
- 43:32that we
- 43:32have fear
- 43:32and anxiety
- 43:33and a
- 43:33cancer
- 43:34diagnosis.
- 43:35And it just
- 43:36leads us
- 43:36to that
- 43:36place where
- 43:37if we're
- 43:37not
- 43:38careful,
- 43:38like you
- 43:38said,
- 43:39we can
- 43:39get stuck
- 43:40in the
- 43:40thoughts.
- 43:40And
- 43:40if we're
- 43:41stuck in
- 43:41the
- 43:41thoughts,
- 43:41then fear
- 43:42is winning.
- 43:43but if we
- 43:44can just
- 43:44kind of
- 43:44remember
- 43:46to question
- 43:47and to,
- 43:47like you
- 43:47said,
- 43:48ground
- 43:48yourself.
- 43:48And once
- 43:48you know,
- 43:49ground
- 43:49yourself
- 43:49and in the
- 43:50things that
- 43:51you're,
- 43:52that are
- 43:52true,
- 43:53then
- 43:53those are,
- 43:54those
- 43:54are some
- 43:54really
- 43:55helpful
- 43:55skills.
- 43:56And, I
- 43:56mean, Ali
- 43:57has broken
- 43:57down three,
- 43:59three,
- 43:59four, four,
- 44:00I think for
- 44:01you
- 44:01broke down
- 44:02for like
- 44:03modalities
- 44:03of therapy.
- 44:04And I mean,
- 44:05I have
- 44:06I am fairly
- 44:07certain
- 44:07in all of
- 44:07the years
- 44:08that
- 44:08I've done
- 44:08various
- 44:08different
- 44:09types of
- 44:09therapy,
- 44:09that
- 44:09I've done
- 44:10a little
- 44:10bit of all
- 44:10of these
- 44:11at
- 44:11some level,
- 44:12I think
- 44:12that that's
- 44:13maybe
- 44:13normal.
- 44:13If you have
- 44:14been
- 44:14involved in
- 44:15therapy for
- 44:15a long
- 44:15time, like
- 44:16I have. but
- 44:18I will say
- 44:19that
- 44:19like the
- 44:19awareness
- 44:20and, and
- 44:21the practice
- 44:22that comes
- 44:22from that,
- 44:23it all adds
- 44:24up
- 44:24to health.
- 44:25And I think
- 44:25that that's
- 44:26that's
- 44:26an
- 44:26important
- 44:27thing
- 44:27to
- 44:27recognize
- 44:27is that
- 44:28you build
- 44:28those
- 44:28resources
- 44:29over time.
- 44:30And so if
- 44:30this looks
- 44:30overwhelming,
- 44:31like maybe
- 44:32just pick
- 44:32the one
- 44:32that
- 44:32resonates
- 44:33with you
- 44:33the most
- 44:34and just
- 44:34start
- 44:35somewhere
- 44:35small.
- 44:36Yeah.
- 44:37And like
- 44:37danet
- 44:37said,
- 44:37right.
- 44:38Like you've
- 44:38you're
- 44:39looking
- 44:39at these
- 44:39and you're
- 44:40like, okay.
- 44:40Like
- 44:40I feel like
- 44:40I've done
- 44:41a little
- 44:41bit of
- 44:41all of
- 44:41these,
- 44:42right?
- 44:42Sometimes
- 44:42I say
- 44:43that like
- 44:43at the core
- 44:44of it,
- 44:44a lot of
- 44:45these
- 44:45therapies
- 44:45are the
- 44:45same.
- 44:46We just
- 44:47get to
- 44:48to
- 44:48this idea
- 44:49or this
- 44:49skill in
- 44:50a different
- 44:50way
- 44:51based on
- 44:51the
- 44:51therapy.
- 44:53one one
- 44:54last
- 44:54thought,
- 44:54I was
- 44:54also just
- 44:55having
- 44:55today,
- 44:56I was thinking
- 44:56about your
- 44:56question
- 44:57is, I
- 44:58promise
- 44:58you,
- 44:58this is
- 44:59related,
- 44:59but,
- 44:59my sister's
- 45:00a big
- 45:00rock
- 45:00climber,
- 45:01and I go
- 45:01rock
- 45:01climbing
- 45:02with her
- 45:02sometimes
- 45:03and gotten
- 45:04into it
- 45:05more so
- 45:06in
- 45:06the past,
- 45:06like year
- 45:06or two.
- 45:07And
- 45:08sometimes
- 45:08I'm like,
- 45:09rock
- 45:09climbing,
- 45:10and I
- 45:10look down
- 45:10and I'm
- 45:11like,
- 45:11oh crap,
- 45:12this rope
- 45:13breaks,
- 45:13I'm done.
- 45:14This is it.
- 45:15and I,
- 45:15like, start
- 45:15to freak
- 45:16out
- 45:16a little.
- 45:16And then I
- 45:16remind
- 45:17myself,
- 45:17okay, like,
- 45:18I am already
- 45:18on this
- 45:19wall.
- 45:20Me
- 45:20worrying
- 45:21about
- 45:21if
- 45:22this rope
- 45:22breaks.
- 45:23It's not
- 45:23going to
- 45:23change
- 45:24the outcome.
- 45:24Either
- 45:25it breaks
- 45:26or it
- 45:26doesn't.
- 45:27Right?
- 45:27Me worrying
- 45:28about this,
- 45:28no matter
- 45:29how much,
- 45:29no matter
- 45:30how many
- 45:30anxious
- 45:30thoughts
- 45:31I come up,
- 45:31it has no
- 45:32bearing on
- 45:32what
- 45:32actually
- 45:33happened
- 45:34right?
- 45:34And so
- 45:34I know
- 45:34for myself,
- 45:35once again,
- 45:35sometimes
- 45:36I remind
- 45:36myself
- 45:36of that,
- 45:37right?
- 45:37No matter
- 45:37how much
- 45:38anxiety
- 45:38I have,
- 45:39it's it's
- 45:40not
- 45:40helping me
- 45:41to
- 45:41ruminate.
- 45:42It
- 45:42makes sense
- 45:43that I'm
- 45:43feeling
- 45:43anxious.
- 45:44I'm
- 45:44going to
- 45:44feel
- 45:44anxious.
- 45:44Right?
- 45:45But
- 45:45ruminating on
- 45:46it is not
- 45:46going to
- 45:47change
- 45:47the outcome.
- 45:48And so
- 45:48I try
- 45:49to remind
- 45:49myself
- 45:49I will
- 45:50deal with
- 45:52whatever
- 45:53awful
- 45:53outcome
- 45:54could
- 45:54potentially
- 45:54happen
- 45:55if
- 45:55it happens,
- 45:56because
- 45:57ruminating
- 45:58on it
- 45:58is making
- 45:59me go
- 45:59through
- 45:59that pain
- 46:00two times,
- 46:01right?
- 46:01Instead of
- 46:01potentially,
- 46:02what's.
- 46:04Yes, such
- 46:04a good
- 46:04point.
- 46:06Well, Ali,
- 46:07that is all
- 46:07the time
- 46:07we have
- 46:08for now
- 46:08for
- 46:08questions.
- 46:09Val,
- 46:09that was an
- 46:09excellent
- 46:10question.
- 46:10So
- 46:10thank you
- 46:11for sharing
- 46:11that.

- 46:15you need me
- 46:15to send
- 46:15something
- 46:15over
- 46:16to Ali.
- 46:16I'm happy
- 46:17to send
- 46:17that over
- 46:18to her
- 46:18after
- 46:18the fact.
- 46:19And
- 46:19if you're
- 46:19listening
- 46:19to
- 46:20the recording,
- 46:20we can also
- 46:21do the
- 46:21same.
- 46:22Ali, can
- 46:23you see my
- 46:24my slide

- 46:25screen now?
- 46:25Yes, I
- 46:26can okay.
- 46:27All right.
- 46:28So, with

- 46:32so much
- 46:33for your
- 46:33time, Ali.
