- 2:04We want to talk about this morning. We're talking about really large projects.
- 2:09I guess people that have a lot of work that needs to be done,
- 2:13multiple layers, multiple visits, and things that need to be done for the
- 2:16procedures, and we're going to talk about We're want to undwind that a little
- 2:20bit though, why you may have to have it done, that the procedures
- 2:22that Grieve Dental Group goes through to make sure that it's done correctly, and
- 2:24maybe some things that would clean up problems that you had in the past.
- 2:28We want to tie all that in together. Talk about that this morning.
- 2:30We're also going to talk about how sinuses can affect your teeth, which is
- 2:34going to be interesting. I think we've touched on that once before, but
- 2:36it's not some man right now. It is the season, that's for sure.
- 2:38People are feeling it. Well, let's start off. Let's start off
- 2:43with a story that I ran across and then we can do sinuses, and
- 2:46then we'll spend the second half of our show focusing on our main topic,
- 2:49which is these complex cases. One thing I've been writing across, I've almost
- 2:53completely disengaged myself from most social media. I'm on Facebook sometimes for like my
- 2:59parents, from my mom, just in pictures of the grand kid and stuff.
- 3:01You know, I'm not on most other social media. I'm not on
- 3:05Instagram, not on Twitter that much or ex or whatever. But the one,
- 3:08the one thing that I have is I do like TikTok. I know
- 3:12that it's it's like everybody's like it's everybody's like either they love it or look
- 3:16the algorithm. For me, I get nothing but like music and sports and
- 3:21funny stuff. You know. For me, I don't get any of the
- 3:23misinformation stuff. I don't give what you want to say. It's just it's
- 3:27I don't go down the dark rabbit hole. So I don't get this dark
- 3:30stuff that everybody gets, but I know it exists. And the other day
- 3:32I just happened to get one. And this was an AD. So this
- 3:36wasn't something that came up naturally in my algorithm, but it was an AD.
- 3:39And you know, if you've seen these TikTok advertisements, it looks like
- 3:42somebody just doing a regular video, you know. I mean that's the whole
- 3:45point with using an influencer to do it. But I noticed that, you
- 3:49know, I usually don't pay attention to these things, but I noticed this
- 3:51lady right away was starting to talk about her teeth. It's like, I'm
- 3:53always looking for content to talk to doctor rub about. What what's she talking
- 3:57about? And so this was a full, like three and a half minute
- 4:00ad with this lady unboxing her kit to do oil pulling my goodness and from
- 4:04home, and uh, I gotta say that. I was like, are
- 4:09you you know with all the misinformation that people talk about there that are that's
- 4:14on social media, and you know, there's all this stuff in Congress right
- 4:17now about misinformation on TikTok, there is, and they're they're talking about political
- 4:21stuff, but there's political misinformation everywhere. I mean literally, I mean you
- 4:25can watch cable news or whatever else and you're gonna get that. I mean
- 4:29that I think we maybe not everybody, but we know to bolster ourselves a
- 4:32little bit against it's this stuff that maybe you don't know not to trust,
- 4:36you know, so you're just flying around looking or let you look at some
- 4:40person that's just an average looking person, like, hey, I got my
- 4:43oil pulling kit and it's great. It really does everything that I need it
- 4:46to. And I mean that's not cool. Well, well, mean,
- 4:48first of all, it's amazing that someone is I don't know, bright enough
- 4:54to package together something like this. I guess they're selling a product, yeah
- 4:58right, obviously, which is oil pulling. From my understanding, is just
- 5:01like coconut oil you could go buy in the grocery store. So but now
- 5:04they've made a kid out of it. I guess. Yeah. It's like
- 5:08also exciting to open in all this all these super sleek, sleek packaging,
- 5:13so you know, kudos to whoever had the business foresight to do that.
- 5:17But the question that meant you have nice little, nice little additive there.
- 5:25But the question is is it really effective? Is it worth the investment?
- 5:30And probably not is going to be the answer. I mean, I say,
- 5:34obviously not. But some people, you know, if it's something that
- 5:38really benefits you somehow, between you and your dentist making that determination, then
- 5:43you know, hey, I'm not gonna say absolutely, don't do anything.
- 5:46But from everything I've found with oil pulling in particular, is that, I
- 5:50mean, man, it takes a long time. What's it supposed to be
- 5:53supposed to be so with oil pulling from it's an older I believe, coming
- 5:58from the Indian type of you know culture, that what they do is take
- 6:01some coconut oil and put it in their mouth that melts, and then they're
- 6:05swishing with it. Now, the idea is that swishing with this oil somehow
- 6:12leeches toxins out of your gum tissue and helps balance things in there, and
- 6:16instead of brushing and flossing appropriately, this oil pulling is going to magically heal
- 6:21your gums. Now, would the oil give some amount of soothing characteristic to
- 6:27your gums? Maybe? Sure? Sure, But the recommended procedure for this,
- 6:31from my understanding, is that you swish with this oil for ten to
- 6:34fifteen minutes. Wow, what now, I mean, can you imagine oil
- 6:38in your mouth for that long? No? Not really, And I want
- 6:42bacon in my mouth for that long let alone some oil, I mean.
- 6:45And some people feel it does well for them, more power to them.
- 6:48However, I recommend just to take two minutes of brushing. Yeah, you
- 6:51know what I mean? Easier, right, be a little bit, and
- 6:55add another minute for flossing. So we're looking at maybe three minutes. Well,
- 6:58I guess less expensive too, because doesn't look cheap. Yes exactly.
- 7:00Yeah, a roll of full of floss will last you for a long time
- 7:03and it's not very expensive. But yeah, so you spend that time that
- 7:09three minutes instead of the fifteen minutes and doing the regular brushing and flossing,
- 7:14I'm sure your gums will be a lot better than if you're only doing this
- 7:16oil pulling with lackluster other or hygiene. It's odd the things that we replace.
- 7:25You know, a lot of times you think that people want to replace
- 7:27what's going on with something that's way easier or way less expensive or something like
- 7:30that. But it seems like sometimes we go to the opposite direction. We
- 7:32get things that are way more complicated that's way more expensive instead of just doing
- 7:35the thing that works. And you know, the other danger with this in
- 7:39particulars I think that where it really comes from is long term delayed cleanings and
- 7:46treatment. It really gives you periodonal disease that has that really really red irritated
- 7:53gum tissue that bleeds easily. Oh, I see so that's another claim.
- 7:56Although you know you have bleeding gums, let's do this oil pulling. Well,
- 7:59you have bleeding gums because you have tarder under your teeth. So if
- 8:01you add oil to that to soothe it a little bit, yeah, you're
- 8:05going to soothe the symptoms somewhat, but you're not going to treat the source,
- 8:11you know, And so then the underlying problems are still there. That's
- 8:13silently destroying the bone. And then yeah, you might be you can do
- 8:18things like oil pulling, like rinsing with other soothing medicaments over the counter things,
- 8:24but all you're going to do is soothe your way into tooth loss.
- 8:28You know, it's going to make that tooth loss more comfortable for you,
- 8:30I guess in the long run. But let's treat the issue and get you
- 8:35your gums nice and firm and healthy and eliminate the irritation in there. It
- 8:41seems like it makes a lot more sense. My antenta always goes up a
- 8:43little bit when I hear things like removing toxins and stuff like that, especially
- 8:48from a topical standpoint, because I'm no medical expert, but from what I've
- 8:52learned over tom this kind of amorphous toxins that people talk about. There's no
- 8:58speci identification of what these supposed toxins are that they're leaching out of the systems.
- 9:03And this idea that you can just slather something like on your skin or
- 9:07your gums and that's going to leach something of medical equivalents out that's going to
- 9:11actually do some good just doesn't make a lot of sense. I mean that,
- 9:15it just doesn't make sense to think that toxins are coming out that you
- 9:18can leach them out. It reminds me of my father in law. One
- 9:24time he had benign what ended up being a benign growth on his head.
- 9:30He thought it was cancer at the time before he went to the doctor,
- 9:31and he was really into like different alternative medicines and different things like that.
- 9:35He literally strapped tofu to his head. Come on, I'm not kidding,
- 9:39and his thought was it was going to leach out the toxins that were in
- 9:46the cancer that was in his head. So it was making direct con He
- 9:48was a bald man anyway, so it was making direct contact, and his
- 9:52thought was that it was going to leach out all of the toxins that caused
- 9:56the cancer. To magic fruit I mean, I'm sure that he probably made
- 10:00that one up. I don't know that you got that one from you know.
- 10:03I mean this was way before most of the stuff came out on the
- 10:05internet. But that is the type of thing that would catch fire today.
- 10:07It's like, oh, you know, I have a headache or whatever it
- 10:09is, let me put some tofu in my head and that's going to clear
- 10:11it right up. We really have to protect ourselves against believing in stuff that's
- 10:16kind of idie hyak oh man. Yeah. And that's what's tough, is
- 10:18alternative medicines like that. You know, I've been in medicine long enough to
- 10:24know that, you know, not everything's one hundred percent for everybody. I'm
- 10:28going to work effectively, and maybe there's some things out there that work,
- 10:31you know, for a certain you know group of people, and but it's
- 10:35just so hard to test it and to really get into a reliable, reproducible
- 10:41type of result that we can then as a medical professional come across saying,
- 10:46you know, this is really good that we should recommend for everybody. You
- 10:50know. So that's that's the difficulty in these alternative types of medications. Unfortunately,
- 10:56a lot of times it's just trying. Someone trying to sell products right
- 11:01from a medical standpoint from someone who's actually a doctor, has to be really
- 11:05discouraging sometimes when people come with with really kind of wild ideas or things that
- 11:07they're they're I heard this, This has got to be true, and you're
- 11:11sitting here with all your years of experience and knowledge and schooling and everything else,
- 11:15you're like, not really, you know, I mean, I don't
- 11:18know how to how else to describe it. It's got to be a little
- 11:20bit a challenge sometimes. But like you said, people can find things that
- 11:24can help them, and I think individually people can can find things that might
- 11:26help them in some way. A placebo effects. People identify that sometimes as
- 11:30being fake. Placebo is not fake. It actually does make you feel better,
- 11:33So people can feel better when they try something that maybe doesn't work for
- 11:37everyone. Yeah, for sure. And that's where I have those conversations with
- 11:39individuals that come in with with some very outlandish claims and ideas. I say,
- 11:46you know, you can try that, but you know, we also
- 11:48need to do what we can see obviously needs taken care of. You know,
- 11:52if you want to add that to your regiment, that's fine as long
- 11:54as we are, you know, being responsible with the treatment that we can
- 11:58deliver from aria, and you can do the additive things as much as you'd
- 12:03like, as long as it's not detrimental that I can find. It reminds
- 12:07me a little bit. I was reading about dermatology and skin and so this
- 12:11is yet another battle you have to fight, is the battle of plausibility but
- 12:16not actionable in the way that it's being used, you know, like if
- 12:18it's even as oil polling or something. I mean, there is a compound
- 12:22that is something that does something, but it's not doing the thing that you
- 12:24think it does. I had no idea that a lot of the dermatologist stuff
- 12:28like anti aging and making your skin look different ways and different things. Most
- 12:31of the delivery systems that's used in those lotions and things like that can't actually
- 12:35penetrate the skin barrier. You're just slathering the stuff on top of your skin
- 12:39and it's evaporating. It's not actually doing any of this. Like the retino
- 12:43that you get over the counter doesn't do anything. It has to be prescription
- 12:46based, you know for it. So the technical medicine does work, but
- 12:50the application that sold like over the counter in mass quantities or something does not
- 12:54and so there is another It's like, yes, technically retina will work,
- 13:00but it has to be you know, pharmaceutical grade. You can't just get
- 13:01this stuff at the store and smeared on your face and expect for it to
- 13:03work. Yeah, I mean even preparing foods like that. I mean that
- 13:07similar is true of spinach. You know, I heard another physician on you
- 13:11know, you can eat all the spine styles you want, it's not giving
- 13:13you as many nutrients as if it were cooked right and all this stuff.
- 13:16So it's yeah, you have to look at the delivery system as well.
- 13:18It's really interesting the way those things work out and how everything kind of ties
- 13:22in together. And of course Doctor Green's been doing this show for a long
- 13:24time and one thing that we've learned is through good dental hygiene, there's a
- 13:28lot of things that you can diagnose through that. There's early on sets disease
- 13:33that you can find through regular checkups. It's it's all part of your holistic
- 13:37health as you go to your doctors and you have regular checkups. We're in
- 13:39the season right now, and boy, my wife suffers from this every year.
- 13:43Shit, the allergy sign of seasons things along those lines just is basically
- 13:48crippling for her to some degree this time of year. And that yet is
- 13:52something else that has a relationship to our teeth, absolutely, and that's may
- 13:56our teeth live right below our sinus cavity. So it's very, very frequent
- 14:01that patients come in and they say, you know, I have this tooth.
- 14:03I know, I mean, this tooth is killing me and it is.
- 14:07You know, it's driving me crazy. I need to get it out
- 14:09of here, I need to get a root canal on it something. And
- 14:11then once we start looking at it and taking our X rays and doing everything
- 14:15that we need to do, start testing it like, you know what,
- 14:18this tooth is actually quite fine. Let's take a larger X ray and let's
- 14:20look at your sinus cavity as well. So we have that ability in our
- 14:24office to do what we call a panoramic film. Most stentus have that as
- 14:28well. It's the one that goes around your head, shows all of your
- 14:31teeth, also shows your sinus cavities. Many times, what we can see
- 14:33is you know, you're signed. Once we take that X ray, come
- 14:37back and ask the question do you have any sinus issues? And like,
- 14:43oh my goodness, they're killing me right now, you know. All the
- 14:46pollens come out in the air right now, and springtime is such a bad
- 14:48time for it, and even this past winter's has been really tough for everybody
- 14:52too for some reason. And a lot of times what happens is through that
- 14:56film and other diagnosing techniques, we can see that there's some cloudiness in the
- 15:01sinuses and that their sinuses are full of congestion. There can have cyst formation
- 15:07in there, and sometimes these cysts need surgically addressed. And that's where we
- 15:11have good relations with our oral maxiofacial surgeons and our you knows and throat physicians
- 15:16that we can refer to. So you know, we need to instead of
- 15:20jumping the gun and getting onto one tooth and doing an unnecessary root canal or
- 15:26an unnecessary extraction, let's take a look at what our other colleagues say with
- 15:33your sinus issues. And then a lot of times, you know, patients
- 15:37find relief in that and they say, you know, oh wow, you
- 15:39know, I'm happy to hear that we can do something like that. Other
- 15:43times we do our duetial just go through it and find that it could be
- 15:48a combination of issues could be a sinus infection plus a tooth problem, and
- 15:54that tooth problem maybe didn't show up on the X rays immediately. So it's
- 15:58a very can be very complex. Add another layer of complexity to our diagnosing
- 16:03in the dental world right now, while people's sinuses are really flared up and
- 16:07cause them reconhagig on them, create in more way, more more ways than
- 16:11one, what are some of the the pressures or discomforts that a patient might
- 16:15experience that could be signus related that might be worth either getting checked out or
- 16:21that's something that you're going to do at the natural course of things, if
- 16:22they come see you, if they're presenting some of these symptoms. But what
- 16:26with somebody at home maybe that's had some problems finding relief through straight dental examinations
- 16:32or something like that. What kind of pressures are discomfort would they feel?
- 16:33So, first of all, they if you're a regular sinus sufferer, you
- 16:37know that sinus pressure it just you know, goes drives you crazy when that
- 16:41flares up. Sometimes the teeth right below it on the top jaw, especially
- 16:47in the back, can really create it can create a toothache. So if
- 16:52it's really neat to look at when you when you start looking these X rays,
- 16:55how are sinuses. Let's see, for instance, our our roots on
- 16:59our back molars on the top there are three roots and inside of those roots,
- 17:07the sinuses can dip down in between them, so you're having very intimate
- 17:11contact with paper thin bone between the roots and your sinuses, and then there's
- 17:17just one thin membrane between those that if it's inflamed with an infection, it
- 17:23can actually elevate the tooth a little bit. It can constrict the blood flow
- 17:29to the tooth itself. It can create the tooth to feel like it has
- 17:33an infection in it, even though the infection is above it into the sinuses.
- 17:37So what you're gonna feel many times can be pressure in the teeth,
- 17:41very vague pain between the molars in the back. So patients will come in
- 17:48and say, you know, it's just hurting on that top left and I'm
- 17:51not sure which tooth it is. So we go through we test the teeth.
- 17:53There's not one single tooth giving them trouble. Maybe it's like, you
- 17:56know, one day it's one tooth, another day it's another tooth, and
- 18:00it's very inconsistent like that. It very much makes me think, Okay,
- 18:03we're probably looking at sinus issues that are kind of making that whole corner angry
- 18:08in a sense. So then if you may had that top tooth that's hurting
- 18:15and maybe elevated a little bit from some inflammation, then it can actually start
- 18:19banging on a bottom tooth. So patients come in with a toothache on the
- 18:23bottom, but we find that it's related to sinuses on the top. So
- 18:26it can all be kind of a domino effect. So you're looking at pressure,
- 18:33you're looking at vague pain, and also biting pain can cause an issue
- 18:37there too, So if you have biting pain on a tooth, then that
- 18:41that might be related to the sinuses kind of pushing that tooth down in a
- 18:44sense. I think this is something that I've been aware of, maybe not
- 18:48thinking about it in direct line, but there's been times that I know that
- 18:52along the along the back of my mouth my back teeth, when I have
- 18:56a lot of sinus pressure where I feel something in my face or cheeks or
- 18:59whatever, that I get a lot of pressure back there and occasionally it will
- 19:00hurt, and just taking like a ben a drill or something will actually relieve
- 19:03it. It's not terrible. I don't get the sinus issues like my like
- 19:07my like my wife gets. But I do get occasionally, especially after like
- 19:10mowing the lawn or something, I'll get like an influx of stuff up my
- 19:14nose, and my sinuses will get really heavy and active for a while,
- 19:18and I can feel that pressure and sometimes at even borders on pain that's in
- 19:21the back of my teeth, that's set in the back of my jaw.
- 19:23And I came to find out years ago. In fact, I think it
- 19:26might have been a previous expert that was on here, doctor Pollard, who
- 19:29was that eant. He suggested to me, you know, several things,
- 19:32but one of the things he suggested was, you know, try taking some
- 19:33an anti allergy to relieve some of that pressure. See if hell and it
- 19:36did. Sure, And so I could imagine if somebody had a little bit
- 19:40of a problem or more severe sinuss than what I have, because mine are
- 19:42relatively mild, that could really become a, if not depilitating. It's certainly
- 19:48very uncomfortable and distracting type of pain. Yeah, for sure. And you
- 19:52have a great point that of what do we do about that? What's the
- 19:53treatment? Is that many times I recommend get some over the counter pseudo fed.
- 19:57It's not as strong as it used to be, but it's the final
- 20:00effort now, so it seems to work pretty well. Take that for consistently
- 20:04for a day or two on a regular basis to see if that cuts back
- 20:08on some of that sinus congestion. And that's the difficulty is that sometimes the
- 20:15tooth pain outweighs the sinus congestion pain. So patients are saying no, but
- 20:18it's a tooth issue. So if we can try that anti allergy medicine for
- 20:25a couple of days and see if it subsides, makes the tooth issue subside,
- 20:27then we're leaning more toward this being more of a sinus issue than a
- 20:32tooth issue. For sure. That's great information. As always, Doctor Steve

- 22:56I'm Deel Cooper. Thanks for tuning in on this Tuesday Tuesday morning,
- 23:00and looks like we have a pretty nice day ahead of us today. I
- 23:03was checking out the weather before coming in. Yesterday was not a bad day.
- 23:06It was nice and sunny, but a little bit on the chili side
- 23:08got cold. It really was a little cold, especially when you got in
- 23:12the shade. I took a little walk right before my afternoon show. I
- 23:15tend to try to do that, especially when the weather's nice. It's just
- 23:18a good way to clear your head and get a little exercise and things like
- 23:21that. And yesterday when I did it, sun was fine. I've been
- 23:25doing it for years now. I've been taking afternoon walks at wark time,
- 23:29just before COVID, I think is when I started. So we're looking at
- 23:30like five years I've done this. It's not every day, but it's staring
- 23:33closed sometimes when I have the time, you know. But so I've learned
- 23:37to play in my walks. I know what side of the streets which down
- 23:40the day have the sun and don't have the sun and things like that.
- 23:42So yesterday it was one of those days. I was definitely trying to walk
- 23:45in the sun because in the sun it was perfectly warm, it was fine.
- 23:48In the shade, it was actually kind of chilly. And of course
- 23:49downtown Charleston could be a wind tunnel in some places, so it gets chili
- 23:52uplus you get the wind and it gets a little cold. I was a
- 23:56little cold, so you have to seek out the sun. Yeah, and
- 23:57we're into our kids playing baseball right now, two of them. So there's
- 24:02multiple baseball games a week, and you're like, it's not that, not
- 24:06that April baseball's cold and it could be cold. And then you go out
- 24:08there and you're sitting out there for like three hours. Yeah, by hour
- 24:12two it's getting kind of chilly. Yeah, you expect it for football season,
- 24:15you know, because football is a cold weather sports. So you need
- 24:18you bunged to out. You bring your cowboll and your thermo's a hot chocolate
- 24:21or whatever. You're good to go. For baseball, you know, it's
- 24:22supposed to be like hot dogs and shorts. Yeah, you know, I
- 24:25sit out there and enjoy the sun, Yeah, not quite. If you
- 24:27want to join the conversation this morning and talk about some dental hygiene, doctor
- 24:30Stephen Garib is here for you. Three zero four three four five fifty eight
- 24:33fifty eight three four five fifty eight fifty eight. You can call in this
- 24:37morning. You can also texting your responses or questions to three zero four non
- 24:41three five five zeros or eight. So you sent me some information last night,
- 24:44and we wanted to make sure that we have plenty of time to talk
- 24:45on this because one of the things that of course, it's it's a major
- 24:49it's a major component to a patient if they're looking at having to have severe
- 24:52multi stage dental work done. Obviously, as a doctor communicating to a patient,
- 24:57it's it's a major issue to go through. But let's talk about that
- 25:00a little bit. The types of problems that you see come in and the
- 25:03things that you do when you do need multiple layers of work, multiple levels,
- 25:07for sure, and that's something that we started to see. I mean
- 25:10in dentistry. I have mentioned this before. Things just go through cycles.
- 25:12There are times when we see a lot of patients coming in and everyone seems
- 25:15to need to set adventures or everyone seems to need a filling on the single
- 25:22tooth on the top right. That's the same tooth like four times in a
- 25:25row. It's just crazy. It's just strange how the universe works like that.
- 25:27But right now, seeing a lot of patients coming in asking questions and
- 25:30seeking treatment for pretty large, complex issues that have been going on for a
- 25:36very long period of time, and for one reason or another, they come
- 25:38in they say, you know, either they know they have a lot of
- 25:41issues that need treated or they are yet to discover that, but they're on
- 25:48that path. So in order to address that, we treat just about every
- 25:52patient the same when they come in in this fashion, because a lot of
- 25:56times patients call in and say, you know, I need a cleaning and
- 26:00I haven't had cleaning for a couple of years, but I need one,
- 26:02or I just had one a year ago, and we say, you know,
- 26:04well we need to see you first so we can properly diagnose and schedule
- 26:10the amount of time, the right amount of time for us to get your
- 26:11teeth cleaned. Well, well, then they come in and we find out
- 26:14that not only do they need a cleaning, but they have very significant gum
- 26:18issues. We need to schedule multiple appointments to get you healthy before we can
- 26:22start talking about fixing that one tooth that you kind of see in your smile,
- 26:26because no need to fix it if we're going to end up losing it,
- 26:30you know, type of discussion. So these are kind of some minor
- 26:33issues that go on with a regular new patient. But that's why I want
- 26:37to go through a bit of the new patient experience, so we can also
- 26:41direct these patients, if it's a more complicated treatment, down the proper path
- 26:47as well. So when patient calls, our great staff answers the phone and
- 26:51welcomes them and finds a convenient time for them to come in to see the
- 26:55doctor to have a consultation. So that way we bring them in, we
- 26:57take the appropriate X rays, we take, we listen to their concerns,
- 27:02and we see what their main issues are. So, as I mentioned,
- 27:06right now, a lot of patients are coming in saying, you know,
- 27:07I know I have just a lot of things going on. I have maybe
- 27:11one tooth that's hurting me, but cosmetically things are not looking right. I've
- 27:15let this go for too long. Or my mom that's gotten her children through
- 27:18school and now she's saying and now I've put off the lay treatment for myself
- 27:22long enough, it's time to do something with that. So then what we're
- 27:26going to do at that point is we're going to take a full series of
- 27:30what I call cosmetic photos, So we're going to take pictures of the patients
- 27:34face, seeing how their smile exhibits in their natural smile form, and then
- 27:38also close ups of the teeth. And then what I like to do is
- 27:42I like to put those on the screen and talk with the patient through their
- 27:47own mouth. And it's really revealing to so many patients because you know,
- 27:51in your brushing your teeth, you brush your teeth, you look in the
- 27:52mirror a little bit. But when we can get those teeth big on a
- 27:56screen and really talk about the issues, they say, Wow, maybe that's
- 28:00why that tooth has been bothering me. I see this crack that you're talking
- 28:03about. And then we also take a digital scan of their teeth, and
- 28:07then we can really blow things up digitally and do a lot of different visual
- 28:11treatments to the teeth so we can show where their bite really is hitting,
- 28:15we can see the alignment of their teeth better. We can then also simulate
- 28:21movement of their teeth to if we wanted to straighten things out a little bit
- 28:25like that. So we're taking all of these records. We also take a
- 28:27full series of X rays. We take a panoramic film that goes around their
- 28:30head, shows the sinuses that we talked about, but also close ups of
- 28:33the teeth so we can determine if there's any decay there. So that first
- 28:38appointment is super important, so we can determine which path we're going to take
- 28:41from there. Now, as I mentioned, now recently seeing a lot of
- 28:45patients going down the path of more complicated treatment because they come in and for
- 28:52instance, so you know this, I've been somewhere else and I've had a
- 28:57couple fillings done on the other two teeth on the front to make things look
- 29:02a little bit better, but those fillings just keep popping off. Well,
- 29:06then we do our typical pictures and scans and we start talking about the bite
- 29:11and we start showing the patient. You know, of course they're they're popping
- 29:14off because they're the first teeth that hit in your mouth, and those little
- 29:18fillings aren't going to be able to withstand the pressures that you're asking them to
- 29:22In a sense, so now we start talking about, you know, there's
- 29:26there are these two teeth that need corrected, but if we correct these two,
- 29:30what about these on the other side, you know? And then they
- 29:32say, you know, I've always looked at that. That's something that's always
- 29:33bothered me, and i've and no one's been able to explain that to me
- 29:37in the right way. And so suddenly we've gone from two fillings that needed
- 29:42some attention, now we're talking about multiple teeth in the front, and then
- 29:45also maybe some teeth on the bottom that need to be corrected in alignment or
- 29:51with crowns and veneers to give them the right proper aesthetic look that they're looking
- 29:55for. So that's one example of how things can get a little more complex.
- 30:00But also we're having many patients come in that their teeth when they smile
- 30:07in our pictures. So, yeah, give me a big smile. Let's
- 30:10take a picture of that. And they're smiling and you don't see any teeth
- 30:12at all, because their teeth over time have worn down completely and they've really
- 30:18taken away a lot of tooth structure, whether it be by trauma or whether
- 30:22it be from consistent grinding over time, and just naturally and gradually wearing away
- 30:27those teeth. So then we're able to have the conversation, you know,
- 30:33if we could do what you want to know, what would you like to
- 30:36have done? Well, they say, well, I don't know what's exactly
- 30:37wrong, but i'd like to have my smile back, like to be able
- 30:40to see some teeth, you know. So then we start talking about placing
- 30:45their smile in the right position, because we can always go in and fix
- 30:49the tooth where it is, but if it's not showing when you smile,
- 30:53you're still not going to be able to see that. So what we're talking
- 30:56about now is adding tooth structure somehow via either by moving the teeth themselves with
- 31:03invisi line or some type of orthodonic treatment, or by restoring them with porcelain
- 31:08or with composite filling material to give them back what they've lost. And that's
- 31:15why that initial exam and initial consultation is so important, so we can know
- 31:21what records we need, what labs we need to work with, so we
- 31:23can create these prototypes to see where we're going to end up. Kind of
- 31:29the finish line, visualize the finish line before we get there. And I
- 31:33guess the thing that I've picked up over the years that we've done this show
- 31:36and the way things have progressed and the way you've kept the folks up to
- 31:38date on procedures and things that have done. Of course, you can say
- 31:41this about pretty much anytime, but very truly now is about the best.
- 31:45Obviously the best time, but it's a very good time to have these types
- 31:48of procedures and things done. We have the technology to be able to lay
- 31:52it out properly, to take the proper X rays, to stage it properly,
- 31:56to come up with the proper strategy without it being guesswork. There's no
- 32:00guesswork that could be involved with it. You can pretty much determine all of
- 32:02this, and in the past that wasn't necessarily the case. So having this
- 32:07all of this in front of someone able to go through one of these complex
- 32:09issues, I think it's amazing that you can just reskald teeth if that's what
- 32:13you need. You know, you're not regrowing them quite yet, maybe that's
- 32:15in the future somewhere, but but you're you're creating an extending teeth that are
- 32:21there, that are visible, I mean, and that's an incredible thing.
- 32:23You know, even forty years ago or something, I'm sure that that was,
- 32:27if not impossible, was certainly not something that was accessible very easily.
- 32:30And now we're talking about having the materials and the technology to be able to
- 32:34do that for someone that could be life changing for someone who, like you
- 32:37say, missing their smile or maybe through trauma no longer has has a smile
- 32:40that they're confident. That's a that's a pretty big is sure. And you
- 32:44had a great point that materials are so important because right now, think,
- 32:47thank god, we've got these advancements in dental technology to where we've got these
- 32:52very strong materials that before you know, we've we would try to do some
- 32:57of these treatments and the mouth is just too strong, it wouldn't hold up,
- 33:00uh, And we would we try to make things as cosmetic as possible
- 33:06with the materials that we had, but it always seemed to fall short,
- 33:08fall short many times. But now the materials are better than, of course
- 33:13they've ever been. But what we've got to do is properly stage things out.
- 33:16Like we've talked about. So in dentistry, of what you do is
- 33:20you bring a patient in and work did this work up? And we say
- 33:22Okay, here's kind of the We've visualized where we want to go, Now
- 33:27how are we going to get there? So it always starts with the periodonal
- 33:30gum health first, and that's why we have a great set of dental hygenis
- 33:34in all of our offices that we expect a lot from them because we expect
- 33:37our patients to have very good dental hygiene treatment and that is the basis for
- 33:44what we can build upon that of dentally speaking, So of course we're going
- 33:47to bring them in and do the proper types of treatment that are needed to
- 33:52get their gums healthy and where that needs to be first, and then we're
- 33:54also going to look at the most urgent what we call disease removal. We
- 34:01take out cavities basically, so we're going to get your teeth in a relatively
- 34:06healthy position. So we can then build upon that cosmetically what we need to
- 34:12do and functionally. So if you have, you know, five or six
- 34:15cavities that are getting large, wh need to get those cavities taking care of
- 34:19first, and then we can start talking about getting those pretty veneers in the
- 34:22front if that's what we're going to do. But many times we can also
- 34:28combine that with our treatment. Let's say that we are working on these front
- 34:31six teeth to make them look better, but at the same time, you
- 34:36have four fillings, four cavities in those six teeth. So many times what
- 34:40we can do is remove that, remove the cavities and also prepare the teeth
- 34:49for the veneers or the crowns at the same time, because we also want
- 34:52to be cognizant of our patient's schedules. Right now, everybody's busier than ever,
- 34:55so getting you know, when your patient comes in and they' common question,
- 35:00how many how many treatment sessions are we going to have here? How
- 35:04many appointments are we going to have? And so fortunately we're able to condense
- 35:07those many times. But even a very complicated case, we could do that
- 35:13and as little as two to four appointments many times to get the get the
- 35:17final restorations in place. That's pretty incredible, And I know that you guys
- 35:22can do. You can balance things between working between maybe using insurance care credit,
- 35:29staging it between times of the year. You know, I know that's
- 35:30a properly a popular strategy. We've talked, sure, we've talked a ton
- 35:35on how dental insurance is different from medical insurance on this show, so traditional
- 35:39medical insurance. So if you wanted to start later in the year, so
- 35:43you have your stages that crossover December into January and you can get more done.
- 35:46I mean, there's ways that you can stage those things out for yourself
- 35:51too, you know, pay inform and stuff like that to make sure it's
- 35:52done. But the bottom line is, and this is the part that I
- 35:54that I know for me, this would be the biggest issue, the procrastination
- 35:58that's got to be involved. From patient standpoint, it's like to get started.
- 36:01Once you get started, it's probably not all that bad, you know.
- 36:04You know, you get into the process, you schedule appointments, you
- 36:06have it ready to go. It's that mental block that we have to get
- 36:08started for us pay as patients on a big project like that, and that's
- 36:13the one we just do it, you know. I mean, if this
- 36:15is something that you're ready to do, this is a good time for it.
- 36:17There's no there's no way for you to determine if if you for sure
- 36:22want to take the steps and get this taken care of them, then if
- 36:23just go see a doctor, go see doctor Garb. Get the plan in
- 36:25place so you can see if it's something you want to do that's right,
- 36:28and that's where we try to make it as easy as possible for the patients.
- 36:31At that same appointment, we're going to talk about staging the treatment out
- 36:36with the appointments we're going to need to make, and also figuring out any
- 36:38financing questions that they have, any insurance navigation. We try to get it
- 36:43all taken care of it that first appointment, so then from there it's smooth
- 36:45sailing, you know, once we get that, once we cross that hurdle
- 36:49of the first appointment, we have the plan in place. Now we just
- 36:52have to execute it, and then that that can be as quick as one
- 36:57one additional appointment we're going to get all the treat been done, or whether
- 37:00it's going to be four or five or more if we need to. If
- 37:04you do them out in that way, it's just a it's a there's no
- 37:07time like the present, and there's no reason if you're suffering through some discomfort,
- 37:10or if if it's a cosmetic thing that you just can't you can't tolerate
- 37:14anymore, there's no reason to at least not get it evaluated to see if
- 37:16it's something. Let's just check it out. Men, we'll come in.
- 37:19I tell patients, you know, new patients come in. Well, you
