- 0:35Hello, everyone, and welcome to Becker's Healthcare podcast.
- 0:38I'm Scott King. Thrilled today to be joined
- 0:40by a very special guest, Dawn Maroney, president,
- 0:43Alignment Health and CEO
- 0:45of Alignment Health Plan. Dawn, how are you
- 0:47doing? Thanks so much for joining us. Appreciate
- 0:48it. I'm doing wonderful. Thank you for asking.
- 0:51Of course. No. So great to have you.
- 0:53We're gonna dive into a lot of big
- 0:55payer topics that that I know we're gonna
- 0:57lean on your your expertise for. But before
- 0:59we do that, I was wondering if you
- 1:00could please just kinda share a little bit
- 1:01about your background and your journey in in
- 1:04health care with health plans.
- 1:05Well, thank you for asking.
- 1:07I actually
- 1:09have been with Alignment going on twelve years
- 1:12and, you
- 1:13know, lead the health
- 1:14plan for all of our contracts and,
- 1:16the company for other, areas.
- 1:20And, I've been in the Medicare space for
- 1:22going on thirty gosh, it'll be thirty nine
- 1:24well, thirty eight years. I'll I'll give it
- 1:26that that number.
- 1:28And I just have always loved working with
- 1:30seniors. I've worked for larger companies and a
- 1:33couple of smaller companies. In fact, the smaller
- 1:35company that when we first originally started out
- 1:38to kind of grow it was a company
- 1:39called CareMore.
- 1:41And,
- 1:42you know, so either way,
- 1:45you know,
- 1:46we've tried to do something unique here at
- 1:48Alignment, and we've just,
- 1:49have done exceptional and very excited about seeing
- 1:52how alignment is just
- 1:55blooming,
- 1:55to be able to support as many,
- 1:58seniors in the country as possible.
- 2:01That's great to hear, Don. And not just
- 2:02about all your experience, you know, twelve years,
- 2:04I'm sure you've seen a lot and and
- 2:06a lot of things evolve. But, you know,
- 2:08with seniors, it's just to follow that up,
- 2:09it's it's really interesting that, you know, at
- 2:12our conferences, especially, you hear about
- 2:14I guess people are surprised
- 2:16how
- 2:17much seniors are able to adapt to, like,
- 2:19technology. Have you found that too? Just kind
- 2:21of being able to do things with with
- 2:23their accounts and their own plans digitally?
- 2:25Yeah. Yes. I you know, it's it's interesting.
- 2:28Although it is improving. So I've seen a
- 2:30big difference, in the last several years. Prior,
- 2:33it was much more difficult. And
- 2:35and the other piece is, you know, you
- 2:37know, this audience, you know, they don't trust
- 2:40the system and there's so much
- 2:42fraud or spam or other things that are
- 2:44going around. So
- 2:46they're very wise in the sense of, you
- 2:48know, what do I click on, what do
- 2:50I use. And so there's a lot of,
- 2:52extra effort to make sure that they know
- 2:54if we're doing something digitally
- 2:56as an organization or asking them to utilize
- 2:59technology more
- 3:01to to go through the process and educating
- 3:03them on why they can trust us.
- 3:05Absolutely. And, you know, one of the topics
- 3:07I wanted to get your expertise on, Don,
- 3:09was just that payer provider relationship and how
- 3:12it obviously changes
- 3:14and especially changed lately. But how are how
- 3:17are you seeing that relationship change as both
- 3:19sides
- 3:20face cost pressure and and workforce shortages in
- 3:23particular?
- 3:24Well, what's interesting to me I mean, depending
- 3:27on the areas,
- 3:29you know, the payer can be a number
- 3:32of areas. Right? So it can be the
- 3:33provider
- 3:34well, the health care company, the health insurance
- 3:36company,
- 3:38as well as you can even look at
- 3:39original Medicare as the payer.
- 3:41So you got Medicare Advantage, the original payer.
- 3:43You've got,
- 3:44you know, primarily in the insurance,
- 3:47organizations. And then you've got the provider,
- 3:50system, which we play literally both sides in
- 3:53the sense of how do you support the
- 3:55provider through our clinicians who work for us.
- 3:58But the providers,
- 4:00you know, are, you know, the ones that
- 4:02are closest to the individual on providing actual
- 4:05care. That is, primary care specialist, hospital,
- 4:09post acute facility, SNF, home health, ancillary service,
- 4:11a number of things.
- 4:13So
- 4:14the hybrid between both, there's a lot of
- 4:17changes from a regulatory standpoint going on. And
- 4:20it's it's it's really,
- 4:22you know,
- 4:23it becomes challenging,
- 4:24you know, for both sides. But I believe
- 4:27the payer actually gets, you know, kind of
- 4:30more,
- 4:31immediacy
- 4:32tied to what's going to change or more
- 4:34involved in the policy,
- 4:36than you'll see even on the provider side
- 4:37other than the providers,
- 4:39or the hospital systems that you'll see across
- 4:41the board.
- 4:42Right.
- 4:44Where do you see the biggest gap today
- 4:46between payer strategy and operational execution?
- 4:52Well, it's it's data. And so how do
- 4:55you make sure
- 4:57that you're getting data real time,
- 5:00specifically, no matter who's, you know, who's involved
- 5:03in caring for your members and or your
- 5:05patients?
- 5:06And it can't just be claims based data.
- 5:08So, you know, the more you know about
- 5:11a consumer or a patient, the more you
- 5:13can be proactive in integrating care. And so
- 5:16it's not at the level that you're getting
- 5:19it as close as possible.
- 5:21So we really work hard at alignment through
- 5:24our technology platform is to try to get
- 5:26that data sooner than later,
- 5:28whether it's lab, pharma,
- 5:30or a hospital,
- 5:32you know, even the census primarily from directly
- 5:35from CMS.
- 5:36So,
- 5:38you know, it's just a
- 5:39it's, you know, it's a process. But it,
- 5:42the closer you can get to that data
- 5:44to understand,
- 5:45individuals
- 5:46about that individuals,
- 5:48you can actually be better at providing care
- 5:51and more in a timely and accessible way
- 5:54that this consumer really deserves.
- 5:56What do you think some of the hang
- 5:58ups are? And, you know, people talk about
- 5:59clean data. What do you think some of
- 6:01the hang ups are in getting clean data
- 6:02and having it, you know, readily available like
- 6:04you were saying? I mean, it's, you know,
- 6:062026
- 6:06and with with all the emerging technology we've
- 6:08seen in in health care and with health
- 6:10plans, what what do you think the issue
- 6:11is there?
- 6:12You still see the same things. You know,
- 6:14I mean, it would be great to get
- 6:16to a point where you can actually pay
- 6:17a claim, you know, immediately at the the
- 6:19time of service,
- 6:21specifically,
- 6:22to where it's super clean. But you're you
- 6:24need to integrate more.
- 6:26But we're still using we're still pulling out
- 6:28our cards when we're going into doctor's offices.
- 6:31Right. Even though you're filling out a form
- 6:33electronically or you're doing it through a chart,
- 6:35you know, portal,
- 6:36you're doing all the information, uploading, scanning, whatever
- 6:39you need to do, but you're still showing
- 6:41up to the practice and you're still pulling
- 6:42out that card and your ID and, you
- 6:44know, it and even if there's a barcode
- 6:47or you know, it's it's
- 6:48it's still
- 6:49not at the level that it needs to
- 6:51be.
- 6:52So having
- 6:54that
- 6:55ingestion
- 6:55of data where everybody's aligned,
- 6:58to make sure that we're all working together
- 7:01to share that information as much as possible.
- 7:04I think what you will see,
- 7:06Scott, with this administration is you're gonna see
- 7:09probably more forward looking,
- 7:12opportunities because we've got a pretty sophisticated
- 7:17educated,
- 7:18group. And what I mean educated through the
- 7:20like,
- 7:21the value of technology
- 7:23with our current administration.
- 7:25I'm quite impressed. And I think if there's
- 7:27gonna be movement,
- 7:29you know, to similar to what we're doing
- 7:31here at alignment, we're gonna see that, at,
- 7:33CMS trying to push and drive that with
- 7:35this team.
- 7:37If you could change one regulatory
- 7:40or industry practice tomorrow to improve affordability and
- 7:43access, what would it be and why?
- 7:45Well,
- 7:46immediately, I would make sure if a
- 7:49hospital
- 7:50system
- 7:51accepts
- 7:52Medicare today in a fee for service or
- 7:54original Medicare status, Medicare
- 7:58sub not supplemental insurance, but Medicare,
- 8:01then I would make that hospital
- 8:04available even at the same rates as Medicare
- 8:06is paying, 100% of Medicare if you don't
- 8:08have a contract,
- 8:09have a requirement that that hospital would need
- 8:12to be contracted
- 8:13with a Medicare Advantage plan or frankly, any
- 8:16participating Medicare Advantage or Medicare,
- 8:19program.
- 8:20And I think that policy needs to drive
- 8:22that. This would would push more organizations to
- 8:25be in rural areas where, you know, local,
- 8:28state, or government officials as well as at
- 8:30the federal level would like to see us.
- 8:34But you can't get there without that hospital
- 8:36saying, no. I don't wanna contract with you,
- 8:38or I wanna charge a 150 or maybe
- 8:40even 200% of Medicare, some crazy number where
- 8:43it prevents organizations from contracting.
- 8:46And then the last thing I wanna ask
- 8:48you, Dawn.
- 8:49What issue
- 8:50do you think is putting the most pressure
- 8:52on health plan margins right now, and and
- 8:54how are you responding differently in in 2026
- 8:57so far with it? It's a great question
- 8:59on, margins.
- 9:02You know, there's a lot of changes. There's
- 9:04a lot of headwinds coming in, hitting the
- 9:06industry as a whole provider and, insurer.
- 9:09And,
- 9:10you know, and it's
- 9:12as well as, you know, the cost continue
- 9:14to increase when it comes to health care
- 9:15expenses, especially in the, when when you think
- 9:18of the acute or institutional cost.
- 9:21And it's not going away. And as well
- 9:23as if you think of the, inflation reduction
- 9:26act as tied to pharma,
- 9:28you know, these costs, the
- 9:30the liabilities hitting more on the payer side,
- 9:33specifically.
- 9:34So you've gotta really you gotta do it
- 9:37right, and you gotta have discipline. And it
- 9:39can't be just looking at it, three months
- 9:42ago or six months back. You have to
- 9:44have
- 9:45discipline in this business to prepare for the
- 9:47regulatory
- 9:48environment
- 9:49as well as the discipline when it comes
- 9:51to how you manage care.
- 9:54And for alignment,
- 9:56you know, because we're really you know, we're
- 9:58one of few plans,
- 10:00that are focused primarily on seniors,
- 10:02but let alone, we have a very rigorous
- 10:06discipline on how we look at this business.
- 10:08And we know we still need to improve,
- 10:10but it's helped us when these headwinds have
- 10:13come
- 10:14to be able to to kind of, you
- 10:17know, surpass the storm unlike, what we're seeing
- 10:20of others. But I want
- 10:22our other competitive
- 10:25partners out there to succeed because, you know,
- 10:27we still need competition. Competition is extremely important
- 10:31for this consumer. I want these,
- 10:33members or seniors who are eligible for these
- 10:35types of products
- 10:37to have choice. And you need to have
- 10:39a couple of plans, more than a couple
- 10:40plans, usually in a county, to have
- 10:43where you're kind of pushing and forcing
- 10:46each of those plans to perform.
- 10:49Because if you only have one org one
- 10:50plan, there's nothing there to force that plan
- 10:53to perform.
- 10:54And so it's really, really important that we
- 10:56still have competition and we have
- 10:59performance across these organizations.
- 11:01But if they're gonna be in it, they're
- 11:02gonna have to do Medicare Advantage done right.
- 11:06Dawn, thank you so much for a great
- 11:07conversation and for joining the podcast. We're really
- 11:09looking forward to having you speak with us
- 11:11at the Spring Payor Issues Roundtable as well.
- 11:13But, thank you so much, Dawn. You're welcome.
- 11:18Thank you.
