- 0:04Today we're going to talk about the fascinating topic of the
- 0:06GLP1 Agonist. You may know them by their generic
- 0:09names of ozempic or semaglutide
- 0:12or wegovy or zeppelin, and not for their
- 0:15effects on obesity as much as their effects
- 0:18on the brain and how
- 0:21those same centers of the brain may be
- 0:24related to addiction. It's an area that's
- 0:27gaining significant attention in the medical community and
- 0:29beyond. And, today we're going to talk
- 0:32about it.
- 0:36I am your Chief Medical Explanationist, Dr. Terry
- 0:39Simpson, and this is Fork U
- 0:42Fork University, where we bust myths and
- 0:45make sense of the madness and learn a little bit about
- 0:48food and medicine and their
- 0:50interaction.
- 0:52All right, let's get started.
- 0:58So for those of you who have been living Under a
- 1:00rock, GLP1 agonist
- 1:03stands for glucagon.
- 1:05Like polypeptide 1, which
- 1:08is a hormone that is normally made in the
- 1:11guts and plays a critical role in
- 1:14glucose metabolism and appetite
- 1:17regulation. The medications
- 1:20mimic this hormone and they have been
- 1:22synthesized in such a way that when they are
- 1:25injected, they last about a week.
- 1:28But what has really become intriguing is how
- 1:31these medications influence brain
- 1:34activity and behavior,
- 1:37particularly concerning reward
- 1:40addiction and what we now
- 1:42call food noise.
- 1:45So let's start with some basics. How these GLP1
- 1:48agonists, Stepbound, WeGovy, et cetera, work
- 1:51in your body. When you eat,
- 1:53GLP1 is released from your intestines.
- 1:56It helps to lower blood sugar levels by enhancing
- 1:59insulin secretion and reducing something
- 2:02called glucagon levels. But it doesn't stop
- 2:05there. It also signals the brain that
- 2:07you're full, meaning it's time to
- 2:10stop eating or that reduction of
- 2:13appetite. You feel
- 2:15appetite not in your stomach as much
- 2:18as you feel it in your brain.
- 2:21But recent studies have shown that these drugs not only help with
- 2:24the weight loss, but can modify how
- 2:27our, brains respond to that food related
- 2:30rewards, sometimes called a,
- 2:32hedonic response, standing for
- 2:35hedonism. In particular, there
- 2:37are GLP1 receptors found in several key
- 2:40areas of the brain, including the hypothalamus,
- 2:43which is crucial for appetite control, and the brain
- 2:46stem, which helps regulate basic physiologic
- 2:49function. To be more precise,
- 2:52if you want to be a little nerdy about it, the GLP1 receptors
- 2:55are present in the nucleus accumbens and the
- 2:57prefrontal cortex, regions of the
- 3:00brain's reward system.
- 3:03The nucleus accumbens is involved in
- 3:06processing pleasure and reward. And while
- 3:08the prefrontal cortex, or the front part of your brain,
- 3:12plays a role in this decision making and impulse
- 3:14control. By acting on these areas,
- 3:17GLP1 agonists alter how we perceive
- 3:19food rewards and cravings.
- 3:22Sometimes we call this
- 3:25food noise. And I want you to think
- 3:28about, let's say, Halloween, which recently passed.
- 3:31And if you have Halloween candy left over,
- 3:34you might get a hit eating the Reese's.
- 3:36Well, maybe that's just me, but knowing
- 3:39there are Reese's candies sitting up there, and
- 3:42if I get a little bit hungry, my brain
- 3:45starts thinking about the reward I get
- 3:48from when I eat that delicious piece of
- 3:50candy. Now, if you
- 3:53are under the influence of GLP1
- 3:55receptor and that area has been
- 3:58stimulated, you feel less reward
- 4:01for that candy, and there's less
- 4:03interest in going after that extra little bit
- 4:06of reward. But that constant
- 4:09feeling or that constant sensation, that
- 4:12that Reese's candy bar is up there or that there's
- 4:15leftover cheesecake or anything,
- 4:18is part of what we call food
- 4:21noise. Now, how might
- 4:24these medications influence
- 4:26addiction like nicotine or
- 4:29alcohol dependence? Some early research and
- 4:32a lot of anecdotal testimony indicate they might.
- 4:35For instance, one study in animals,
- 4:38the GLP1 agonists reduced alcohol
- 4:40consumption, meaning we had less drunk
- 4:43mice. But that was the first hint
- 4:46that these medications might be useful for those struggling
- 4:49with alcohol use disorder. But we have to approach this
- 4:52cautiously because addictions are complex and
- 4:55we really don't want to say food's an addiction. But clearly,
- 4:58food noise is something that is very hard for some
- 5:00people to control, like me with
- 5:03rhesus. Anecdotal evidence, though,
- 5:06has shown this. In a query of
- 5:08GLP1 users, people who use semaglutide or
- 5:11zembic wegovine, they all noted that they
- 5:14are drinking less alcohol and not driven to drink the
- 5:17alcohol. Some individuals quit smoking. Some
- 5:20individuals find that they just don't have an
- 5:22interest in coffee in the morning, which horrifies
- 5:25me. But be that as it, may, coffee is
- 5:28a perfectly acceptable, wonderful morning
- 5:31gift. But as we consider the effects of these
- 5:34GLP1 agonists, it's essential to understand
- 5:37the implications of targeting
- 5:40those brain regions. While
- 5:43dampening the reward response to food, it can aid in weight
- 5:46loss. That food noise is
- 5:48gone. It could also have an impact on other
- 5:51addictions. If GLP1 influences
- 5:54the dopamine release in the reward pathways, it
- 5:57opens up intriguing possibilities for addiction
- 5:59treatment and maybe raises some ethical questions
- 6:02as we blur the line between treating medical
- 6:05conditions and behavior.
- 6:08But for people who truly want to
- 6:11lose weight when they get upon the
- 6:14GLP medications, these aren't people that are
- 6:17morbidly obese. These are people that maybe have
- 6:2030, 40, 50 pounds to lose. They'll tell
- 6:23you they didn't even realize there was food noise going
- 6:26on in their brain until this
- 6:29now GLP1s act on the stomach. But we're today just
- 6:32talking about the brain. So personalized
- 6:34medicine is always the key in this discussion. Not everyone's going to
- 6:37respond to these GLP1 agonists in the same way.
- 6:40Genetics, environment, blah blah blah, they all shape
- 6:43how we process rewards and make choices.
- 6:47But they do offer a really
- 6:49interesting possibility, not only for treating weight
- 6:52loss and food noise, which
- 6:55clearly drives people with ultra processed food to
- 6:58want more. And this also
- 7:00changes perhaps one of the other ways we consider
- 7:03food. In the recent ultra
- 7:06processed food discussions, many people
- 7:09say, well there's not fiber in
- 7:12Doritos, for example, so therefore you can
- 7:15eat a whole family pack of Doritos and not feel
- 7:17full. So if we were to put
- 7:20fiber in Doritos, you might
- 7:23feel uncomfortable eating that family pack.
- 7:26But that doesn't change the reward center in your
- 7:28brain. That doesn't change your desire
- 7:31to have those Doritos.
- 7:35So my future prediction goes something
- 7:38like this. More researchers clearly need
- 7:41to understand the impact of the GLP1s on the
- 7:44brain. And probably we will get Some very
- 7:47specific GLP1 agonists
- 7:50for the brain to help people with food noise and
- 7:53reward.
- 7:54Anyway, I really hope that you've enjoyed
- 7:57Today's topic on GLP1. It's a
- 8:00fascinating topic that we have seen here on
- 8:024Q. It is fascinating how those reward
- 8:05systems in the brain hack. My interest of
- 8:08course is in people who struggle with weight loss,
- 8:11but they may give us a key for
- 8:14addiction treatment in general. If you want
- 8:17to see the references associated with this, you
- 8:20can find it on forku.com or on my
- 8:22regular blog, which is yourdoctorsorders.com
- 8:25today's production is brought to you by.
- 8:28Well, me. No
- 8:31seriously, we're not asking for sponsors, but we do
- 8:34appreciate them. But thank you for listening. If you've enjoyed
- 8:37this, please give us a nice 5 star review.
- 8:39This has been distributed by our friends at Simpler Media and the
- 8:42Pod God Evotera. I'm Dr. Terry
- 8:45Simpson. This was written and researched
- 8:48by me. If you need medical
- 8:51advice, I'm not your doctor. Please
- 8:54find your own doctor. Specifically, I'd like you to find
- 8:57an obesity specialist. If you are
- 8:59concerned about your excess weight
- 9:02and want to have an impact with some of the
- 9:05new GLP1 agonists like Ozempic, WeGovy et
- 9:08cetera. There are plenty of people out there who are
- 9:11obesity specialists who are actually trained on obesity like the
- 9:13people@acomplishhealth.com until next
- 9:16time, keep questioning, keep learning, and
- 9:19remember to think critically about the
- 9:21choices that you make.
- 9:28Hey Evil, can you imagine?
- 9:31Food noise goes away. I wonder
- 9:34if I'd still like that Reese's candy bar though,
- 9:39to help with candy, I'm thinking about using it to fight
- 9:42the munchies.
