- 0:47Doctor
- 0:47Ulrich,
- 0:48welcome
- 0:48to the stage.
- 0:50Thank you.
- 0:52Well,
- 0:52that was a
- 0:53long
- 0:53introduction,
- 0:54but
- 0:54it all
- 0:54basically
- 0:55means
- 0:55that I'm
- 0:55a little
- 0:56pocket
- 0:56nerd.
- 0:57I studied
- 0:58the
- 0:58immune
- 0:58system
- 0:59for a
- 0:59very
- 0:59long time
- 1:00in
- 1:00grad school,
- 1:01and now
- 1:01I work
- 1:02with a
- 1:02company,
- 1:03a
- 1:03disclaimer
- 1:04called.
- 1:04Caris
- 1:05but
- 1:05essentially
- 1:06we do
- 1:06somatic
- 1:06testing,
- 1:07like a
- 1:07lot of
- 1:07other tests
- 1:08that
- 1:08I'm sure
- 1:08you've
- 1:08heard of.
- 1:09And so
- 1:10I was
- 1:10invited
- 1:10by Doctor
- 1:11Moser
- 1:11today
- 1:12to talk,
- 1:12just in
- 1:13general
- 1:13about
- 1:14somatic
- 1:14testing.
- 1:15What is it?
- 1:16Why do we do
- 1:17it?
- 1:17How
- 1:17do we do
- 1:17it?
- 1:19slides,
- 1:20but we're
- 1:20going to
- 1:20kind
- 1:21of wing
- 1:21it today
- 1:21and just
- 1:22we can
- 1:23make this
- 1:23a
- 1:23discussion
- 1:24based
- 1:24and
- 1:24ask your
- 1:25questions
- 1:25very
- 1:25informally.
- 1:28I think
- 1:28we all
- 1:28know
- 1:28about the
- 1:29genetic
- 1:29testing
- 1:30of
- 1:30primary
- 1:30tumors
- 1:30through
- 1:31castle
- 1:31So I'll
- 1:32just
- 1:32start
- 1:32by giving
- 1:33kind of a
- 1:33high
- 1:33level
- 1:34overview
- 1:34about,
- 1:35you know,
- 1:35the
- 1:35different
- 1:35types of
- 1:36genetic testing.
- 1:37And then
- 1:37we can
- 1:38talk
- 1:38about
- 1:38the
- 1:39somatic
- 1:39tests
- 1:39in
- 1:39metastatic
- 1:40tumors
- 1:40if that's
- 1:41okay.
- 1:41Yeah.
- 1:42So you
- 1:43know, as
- 1:43most
- 1:43of us
- 1:43are aware,
- 1:44when
- 1:44you have
- 1:45a tumor
- 1:45in the eye,
- 1:46we can
- 1:46do
- 1:47genetic
- 1:47testing
- 1:47on that
- 1:48tumor
- 1:48with a
- 1:48biopsy
- 1:49through
- 1:49Castle.
- 1:50And
- 1:50that's
- 1:50really
- 1:51a focused
- 1:52genetic test
- 1:53where
- 1:53they're
- 1:53looking
- 1:54at
- 1:54certain
- 1:55types
- 1:55of RNA
- 1:57or DNA
- 1:58to try
- 1:58to
- 1:59predict
- 1:59how
- 1:59aggressive
- 2:00the tumor
- 2:00is
- 2:00going to be.
- 2:02And so
- 2:02when
- 2:02you have
- 2:03genetic
- 2:03tests,
- 2:03you can always
- 2:04gene test
- 2:05the person
- 2:06or
- 2:06the tumor.
- 2:08So that's
- 2:08one
- 2:08of the tests
- 2:09where
- 2:09we test
- 2:09the tumor.
- 2:10And we're
- 2:10doing it
- 2:11in a
- 2:11focused
- 2:12way
- 2:12to try
- 2:12to
- 2:12predict
- 2:13something.
- 2:14We
- 2:14then have
- 2:15where
- 2:15you can
- 2:15test
- 2:15the person.
- 2:16So
- 2:17I think
- 2:17this is
- 2:18most
- 2:18commonly
- 2:19seen in
- 2:20breast
- 2:20cancer
- 2:21right.
- 2:21We
- 2:21know that
- 2:21there's
- 2:22different
- 2:22mutations
- 2:23you can be
- 2:23born with
- 2:23maybe
- 2:24in brca
- 2:25where
- 2:25we can
- 2:25test
- 2:26your DNA
- 2:27to see
- 2:27if
- 2:28you are
- 2:28at higher
- 2:28risk for
- 2:29getting cancer.
- 2:31And then
- 2:31once
- 2:31patients
- 2:32develop
- 2:32metastatic
- 2:33cancer
- 2:33that
- 2:33spread
- 2:34and we
- 2:34can't
- 2:35cut it
- 2:35out,
- 2:35we can actually
- 2:36test
- 2:36the tumor
- 2:37genetics,
- 2:38not for a
- 2:39specific
- 2:39test or
- 2:40prognosis,
- 2:41but
- 2:41kind of
- 2:41as a
- 2:41shotgun
- 2:42approach
- 2:42and say,
- 2:43I,
- 2:43I call it
- 2:44a fishing
- 2:44expedition.
- 2:45We're
- 2:46basically
- 2:46testing
- 2:47the
- 2:47cancer
- 2:47to say
- 2:48what
- 2:48mutations
- 2:49or
- 2:49changes
- 2:50in
- 2:50the DNA
- 2:50or
- 2:51genetics
- 2:52has this
- 2:52tumor
- 2:53developed?
- 2:54either
- 2:55to better
- 2:55understand
- 2:56it or try
- 2:56to find
- 2:57a way
- 2:57that
- 2:57we can
- 2:58exploit
- 2:58that and
- 2:59use it
- 2:59to guide
- 3:00treatment.
- 3:00And
- 3:01that's
- 3:01really
- 3:01what this
- 3:02this talk
- 3:02is
- 3:02focused
- 3:03on, that
- 3:03kind of
- 3:04fishing
- 3:04expedition
- 3:05where we
- 3:05test
- 3:06everything
- 3:07to try to
- 3:08figure out
- 3:08is there
- 3:09a better
- 3:09treatment
- 3:10option
- 3:10or a
- 3:11personalized
- 3:11treatment
- 3:12option
- 3:12based
- 3:12on that
- 3:12genetic
- 3:13testing.
- 3:14And
- 3:14that's
- 3:14really
- 3:14what
- 3:15peaches
- 3:15specializes
- 3:16in.
- 3:16In her
- 3:17career.
- 3:18Thank you.
- 3:18I'm
- 3:18big on
- 3:19analogies
- 3:20and
- 3:20metaphors.
- 3:21I'm
- 3:21a visual
- 3:21person,
- 3:22so
- 3:22apologies
- 3:22for not
- 3:23having
- 3:23the slides,
- 3:23but I
- 3:24think
- 3:24about
- 3:24somatic
- 3:25testing
- 3:26as
- 3:26essentially
- 3:27looking
- 3:27at
- 3:28a house.
- 3:29So as
- 3:29an individual
- 3:30we are
- 3:30organisms.
- 3:31We're
- 3:31made
- 3:32up of
- 3:32different
- 3:32organs,
- 3:33and
- 3:33our organs
- 3:34are made
- 3:34up of
- 3:34different
- 3:34cells
- 3:35in the
- 3:35same way
- 3:36we have
- 3:37if
- 3:37we think
- 3:37about
- 3:38a house,
- 3:39a house
- 3:39is made
- 3:40up of
- 3:40different
- 3:40rooms,
- 3:41and
- 3:42every
- 3:42room has
- 3:43different
- 3:43features
- 3:44that
- 3:44make it
- 3:44a
- 3:44characteristic
- 3:45of
- 3:45that room.
- 3:46So
- 3:46if I walk
- 3:47into a room
- 3:48and
- 3:48there's
- 3:48a toilet,
- 3:49I think,
- 3:49hey,
- 3:49that's a
- 3:50bathroom.
- 3:50If I see
- 3:52an oven
- 3:52and a
- 3:53stove,
- 3:53I'm
- 3:53thinking
- 3:54the
- 3:54kitchen,
- 3:54a bed,
- 3:55a
- 3:55bedroom,
- 3:56and
- 3:56our bodies
- 3:57work
- 3:57the same way
- 3:58and
- 3:58just like
- 3:59you can
- 3:59have
- 3:59a house,
- 4:00it
- 4:00could be
- 4:00built in
- 4:01the 1960s
- 4:02or it
- 4:02could
- 4:02have been
- 4:03built
- 4:03yesterday.
- 4:04There's
- 4:04this
- 4:04master
- 4:05blueprint
- 4:05for houses.
- 4:06And
- 4:06that's
- 4:07how our
- 4:07DNA is.
- 4:08It's
- 4:08our DNA
- 4:08is the
- 4:09master
- 4:09blueprint
- 4:10of
- 4:10our bodies,
- 4:11whether
- 4:11it's,
- 4:12normal,
- 4:13healthy
- 4:13tissue.
- 4:14And
- 4:14sometimes
- 4:15what happens
- 4:15is those
- 4:16blueprints
- 4:16get
- 4:17messed up
- 4:17a little bit,
- 4:18and that
- 4:18leads to
- 4:19our cancer.
- 4:20And so
- 4:21just like
- 4:22a house
- 4:23can be
- 4:23made,
- 4:23and you,
- 4:24you walk
- 4:25in the door
- 4:25and
- 4:25there's
- 4:26a squeaky
- 4:26door.
- 4:27Hey,
- 4:27WD 40
- 4:28kind of
- 4:29an easy
- 4:29fix
- 4:29for it.
- 4:30And
- 4:30that's
- 4:30what
- 4:30we're
- 4:30looking
- 4:31at in
- 4:31cancer
- 4:32and
- 4:32somatic testing.
- 4:33What are those
- 4:34differences
- 4:35from the
- 4:35healthy
- 4:36tissue
- 4:36from the
- 4:37the
- 4:37manual
- 4:38blueprint.
- 4:39And how
- 4:39can we fix it.
- 4:41And so
- 4:41with
- 4:41a lot of
- 4:42traditional
- 4:42medicine,
- 4:43you know,
- 4:43our
- 4:43chemo,
- 4:44radiation,
- 4:44surgery,
- 4:45those are
- 4:45all
- 4:46part of
- 4:46the
- 4:46toolbox
- 4:46that are
- 4:47still
- 4:47used
- 4:47today.
- 4:48They're great.
- 4:49But with
- 4:50somatic
- 4:50testing
- 4:51it
- 4:51doesn't
- 4:51only
- 4:52tell you
- 4:52that
- 4:52there is a problem,
- 4:54it tells
- 4:54you more
- 4:55about
- 4:55what
- 4:55the problem
- 4:56is to
- 4:56inform
- 4:57treatment.
- 4:58So
- 4:58if you
- 4:58walk
- 4:58in your house
- 4:59and
- 4:59there's
- 5:00water
- 5:00all over
- 5:00the floor,
- 5:01is
- 5:02obviously
- 5:02there's
- 5:02a problem.
- 5:03But is
- 5:04it due to
- 5:05the
- 5:05faucet
- 5:05being
- 5:05left on,
- 5:06a pipe
- 5:07broke?
- 5:08Or maybe
- 5:08the city
- 5:09did some
- 5:09construction
- 5:09down
- 5:10the street,
- 5:11knowing
- 5:11which of
- 5:12those problems
- 5:12it is.
- 5:13The source
- 5:14is important
- 5:15because
- 5:15if you
- 5:16know the
- 5:16source of
- 5:16the problem,
- 5:17it
- 5:17gives you
- 5:17a better
- 5:18informed
- 5:18on the
- 5:18treatment,
- 5:19on how
- 5:19you're
- 5:19going,
- 5:20how
- 5:20the oncologist
- 5:21like
- 5:21doctor
- 5:21knows
- 5:22you're
- 5:22going to
- 5:22then
- 5:22treat
- 5:22your cancer.
- 5:24And so
- 5:24somatic
- 5:25testing
- 5:25is just
- 5:26that
- 5:26other
- 5:27side of
- 5:27traditional
- 5:28medicine
- 5:28that informs
- 5:29you more
- 5:30about how
- 5:30what is going
- 5:31what is different
- 5:32in your tumor
- 5:33then
- 5:33the healthy
- 5:33tissue
- 5:34and
- 5:34how can
- 5:35that
- 5:35information
- 5:36inform
- 5:36treatment.
- 5:37And so
- 5:38there's
- 5:38a lot of
- 5:38companies
- 5:39out
- 5:39there,
- 5:39just like
- 5:40Kerry's
- 5:40Tempest
- 5:40Foundation,
- 5:41lots
- 5:41that do
- 5:42genetic
- 5:42testing,
- 5:43but
- 5:43all of them
- 5:44are
- 5:44essentially
- 5:44looking
- 5:45for three
- 5:45major
- 5:46things
- 5:47changes
- 5:47in your
- 5:47DNA, in
- 5:48your RNA
- 5:50and
- 5:50protein,
- 5:51all these
- 5:52different
- 5:52things
- 5:52that can be
- 5:53changed
- 5:53in the,
- 5:54tumor
- 5:55tissue
- 5:55versus
- 5:55your
- 5:55healthy
- 5:56tissue.
- 5:59Great.
- 5:59So I
- 5:59think
- 6:00that's a
- 6:00really
- 6:00good
- 6:00overview
- 6:01of kind
- 6:01of what
- 6:01this
- 6:02testing
- 6:02is.
- 6:03So any
- 6:03questions
- 6:04on
- 6:04kind
- 6:04of the
- 6:04basics of
- 6:05what is
- 6:06somatic
- 6:06testing
- 6:07which is also
- 6:08tumor
- 6:08testing.
- 6:09Right.
- 6:09Somatic
- 6:10just
- 6:10means
- 6:10tumor
- 6:10testing.
- 6:13about
- 6:13that
- 6:13before
- 6:14we kind
- 6:14of go
- 6:14in deeper
- 6:15and start
- 6:15talking
- 6:15about
- 6:16the
- 6:16different
- 6:16types
- 6:16of tests
- 6:17that are
- 6:17out
- 6:17there.
- 6:17when we
- 6:18talk
- 6:18about,
- 6:19somatic
- 6:19testing
- 6:20features,
- 6:21that just
- 6:21means
- 6:21we're
- 6:21testing
- 6:22the tumor.
- 6:22But
- 6:22there's
- 6:23obviously
- 6:23different
- 6:23platforms
- 6:24out
- 6:24there.
- 6:24There's
- 6:25ways
- 6:25where
- 6:25you can
- 6:25test.
- 6:27let's
- 6:27start
- 6:27big
- 6:27picture
- 6:27first.
- 6:28So
- 6:28there's
- 6:28a lot of,
- 6:29tumor
- 6:30biopsy
- 6:30tests
- 6:30out
- 6:31there.
- 6:31And those
- 6:31a lot
- 6:31of blood
- 6:32tests
- 6:32out there.
- 6:33And
- 6:34you know
- 6:34I think
- 6:35the blood
- 6:35tests
- 6:35have
- 6:35really
- 6:35come on
- 6:36in
- 6:36the past
- 6:36couple
- 6:36of years
- 6:37where
- 6:37we can
- 6:37take your
- 6:38blood
- 6:38and ....
- 6:39your tumor.
- 6:40What are the
- 6:40big
- 6:40differences
- 6:41and
- 6:41advantages
- 6:42and
- 6:42disadvantages
- 6:43to
- 6:43taking a
- 6:44biopsy
- 6:45tissue
- 6:46and doing
- 6:46the
- 6:46sequencing
- 6:47of the
- 6:47tumor
- 6:47versus
- 6:48a blood
- 6:48sample of
- 6:49the patient,
- 6:49and
- 6:50trying
- 6:50to do
- 6:51the
- 6:51sequencing
- 6:51of
- 6:51the tumor.
- 6:52this
- 6:53whole
- 6:53expansion
- 6:54of,
- 6:55technology
- 6:56and
- 6:56science
- 6:56and
- 6:56everything,
- 6:56we have
- 6:57these two
- 6:57major
- 6:57groups,
- 6:57like you
- 6:58said,
- 6:58you take,
- 6:59a piece
- 7:00of tissue
- 7:01and we
- 7:01call it
- 7:01a
- 7:01snapshot.
- 7:03So
- 7:03when you
- 7:03take
- 7:03a biopsy,
- 7:04there's
- 7:05just a
- 7:05snapshot.
- 7:06It's like
- 7:06taking
- 7:06a
- 7:06picture.
- 7:07You're
- 7:07capturing
- 7:08what
- 7:08you took out
- 7:10and
- 7:10that's
- 7:10it.
- 7:11You're
- 7:11not
- 7:11getting
- 7:11anything
- 7:12that's
- 7:12surrounding
- 7:13nothing
- 7:13that
- 7:13may be
- 7:13circulating
- 7:14the blood.
- 7:14If
- 7:14there's
- 7:15metastatic
- 7:15sites,
- 7:16you're
- 7:16not
- 7:16capturing
- 7:17that.
- 7:17You're
- 7:17solely
- 7:17capturing
- 7:18that
- 7:19portion
- 7:19of tumor
- 7:20that
- 7:20you took out.
- 7:21And
- 7:21that is
- 7:21really
- 7:22advantageous
- 7:22for
- 7:23multiple
- 7:23reasons,
- 7:24is
- 7:24that
- 7:25you are
- 7:25getting
- 7:25that full
- 7:26picture.
- 7:26You're
- 7:27able to
- 7:27send that
- 7:28for
- 7:28sequencing.
- 7:29And
- 7:29tissue
- 7:30biopsies
- 7:30right now
- 7:31are
- 7:32highly
- 7:32more
- 7:32sensitive
- 7:33since
- 7:33it's,
- 7:35tissue
- 7:35that
- 7:35you can
- 7:35take and
- 7:36fully
- 7:37look at,
- 7:38you're
- 7:39able to
- 7:39determine
- 7:40the DNA,
- 7:40the RNA
- 7:41and the
- 7:41protein,
- 7:41what's
- 7:42going on
- 7:42inside
- 7:42the cells
- 7:43and on
- 7:44the cell
- 7:44surface.
- 7:45And
- 7:45so it's
- 7:46a
- 7:46snapshot
- 7:46where
- 7:47you're
- 7:47able to
- 7:47capture
- 7:48heterogeneity
- 7:49and
- 7:49everything
- 7:49like
- 7:49that.
- 7:50Now in
- 7:51contrast
- 7:52liquid
- 7:53biopsies
- 7:54are
- 7:54very advantageous
- 7:55because
- 7:56if a
- 7:56patient
- 7:57does have
- 7:57metastatic
- 7:58sites,
- 7:59essentially
- 7:59when you
- 7:59take a
- 8:00blood
- 8:00draw,
- 8:01you're
- 8:01taking
- 8:01a hodgepodge.
- 8:02If you're
- 8:03having,
- 8:04metastatic
- 8:04sites
- 8:05that are
- 8:05essentially
- 8:05shedding
- 8:06tumor
- 8:07into the
- 8:07body,
- 8:08into the
- 8:09blood,
- 8:09you're
- 8:10able to
- 8:10take a
- 8:10draw,
- 8:11all of that.
- 8:11So
- 8:12I think
- 8:12of doing
- 8:13a
- 8:13recording
- 8:14versus
- 8:14taking
- 8:14a snapshot.
- 8:15So
- 8:16your chances
- 8:16are that
- 8:16you're
- 8:17going
- 8:17to
- 8:17capture
- 8:18cancer
- 8:18information
- 8:19from
- 8:19multiple
- 8:20metastatic
- 8:20sites.
- 8:22And
- 8:22really
- 8:23one of
- 8:23the
- 8:23pros of
- 8:23doing
- 8:24this
- 8:24is that
- 8:25it's less
- 8:25invasive.
- 8:26No one wants
- 8:27to have
- 8:27a biopsy.
- 8:28And
- 8:28sometimes
- 8:28most of
- 8:28the time
- 8:29they
- 8:29can't
- 8:29have an
- 8:30additional
- 8:30biopsy.
- 8:31So that's
- 8:31where
- 8:32liquid
- 8:32testing
- 8:32really
- 8:33comes in.
- 8:33As a pro,
- 8:35however,
- 8:36there's a
- 8:36sensitivity
- 8:37issue
- 8:38when you're
- 8:38able
- 8:39to take
- 8:39that
- 8:39snapshot
- 8:40or that
- 8:40picture
- 8:41and do a
- 8:41tissue
- 8:41biopsy,
- 8:42the material is
- 8:43right
- 8:44there.
- 8:44You can
- 8:44see the
- 8:45tumor
- 8:45material,
- 8:45you know
- 8:46you have it
- 8:46and
- 8:47that can be
- 8:47analyzed.
- 8:48Whereas
- 8:49not all
- 8:50tumors
- 8:50shed and
- 8:51or mean
- 8:52that they
- 8:52spread.
- 8:53And
- 8:53you can
- 8:53capture it
- 8:54in the blood.
- 8:55And so
- 8:55it's a
- 8:55little
- 8:55harder
- 8:56in
- 8:56sensitivity
- 8:57to
- 8:57be
- 8:57able to
- 8:57do that.
- 8:58But
- 8:58in the
- 8:59absence
- 8:59of
- 8:59tissue,
- 9:00at least
- 9:00you have
- 9:00a backup
- 9:01which
- 9:01we use
- 9:02with
- 9:02liquid
- 9:02biopsies.
- 9:03And
- 9:03when we
- 9:03talk
- 9:03about
- 9:04sensitivity,
- 9:04what does that
- 9:05mean
- 9:05practically?
- 9:06Does
- 9:06that mean
- 9:06we're
- 9:06more
- 9:06likely to
- 9:07catch
- 9:07something?
- 9:07We're
- 9:08more
- 9:08likely to
- 9:08miss
- 9:08something
- 9:09that's there.
- 9:10What does that
- 9:11mean in
- 9:11kind of
- 9:11layman's
- 9:12terms.
- 9:12Yeah.
- 9:13So
- 9:13sensitivity
- 9:14is just
- 9:14what
- 9:14you said
- 9:15is that
- 9:15we're
- 9:15able to
- 9:16be highly
- 9:16sensitive
- 9:17in
- 9:17tissue,
- 9:18meaning
- 9:18that it's
- 9:19the
- 9:19tissue is
- 9:19actually
- 9:20there.
- 9:20You see
- 9:20the tumor.
- 9:21You've
- 9:21captured
- 9:22it versus
- 9:23in a
- 9:23liquid
- 9:23biopsy,
- 9:24there's
- 9:25less
- 9:25sensitivity
- 9:25in
- 9:26the fact
- 9:26that you
- 9:26can't
- 9:27always
- 9:27detect it
- 9:28because
- 9:28again,
- 9:28it's a
- 9:29hodgepodge.
- 9:29There's
- 9:29a lot of
- 9:30other
- 9:30things
- 9:30in that
- 9:31blood
- 9:31specimen
- 9:32outside
- 9:32of tumor.
- 9:33And so
- 9:33you're
- 9:34looking
- 9:34at your
- 9:34blood
- 9:34cells,
- 9:35your
- 9:35white
- 9:35blood
- 9:36cells,
- 9:38and
- 9:38everything else
- 9:38that can be
- 9:39found in
- 9:39blood.
- 9:40So
- 9:40you have to sift
- 9:41through
- 9:41things
- 9:41more.
- 9:42So
- 9:42if you have
- 9:43to sift
- 9:43through
- 9:43things
- 9:44more,
- 9:44your
- 9:44sensitivity
- 9:45kind of
- 9:45goes
- 9:45down.
- 9:46Whereas
- 9:46in tissue
- 9:47the tumor
- 9:48is
- 9:48blatantly
- 9:48right
- 9:49there.
- 9:49You can
- 9:49assess
- 9:50it.
- 9:51And
- 9:51I think
- 9:51one of
- 9:51the
- 9:51things
- 9:52that
- 9:52we're
- 9:52going
- 9:52to
- 9:52discuss
- 9:53in one
- 9:53of the
- 9:53later
- 9:54sessions
- 9:54about our
- 9:54circulating
- 9:55biomarkers
- 9:56is that,
- 9:57like
- 9:58peaches
- 9:58mentioned,
- 9:58not
- 9:59all cancers
- 10:00let their
- 10:01DNA float
- 10:01in the
- 10:02blood.
- 10:02And uveal
- 10:03melanoma
- 10:03is one of
- 10:04the ones
- 10:04where
- 10:04we know
- 10:05from
- 10:05really
- 10:05good data
- 10:06and
- 10:06really
- 10:06good
- 10:06studies
- 10:07that
- 10:08you're
- 10:08less
- 10:08likely
- 10:08to find
- 10:09uveal
- 10:10melanoma
- 10:10DNA
- 10:11in the
- 10:11blood of
- 10:12in
- 10:12patients
- 10:13with UVA
- 10:13melanoma
- 10:14than
- 10:14you are
- 10:14other
- 10:14types
- 10:15of
- 10:15cancer,
- 10:15like
- 10:15cutaneous
- 10:16melanoma
- 10:17or breast
- 10:17cancer.
- 10:18And so
- 10:19these
- 10:19liquid,
- 10:20biopsies
- 10:21can be
- 10:21easier.
- 10:22They can give
- 10:23you a
- 10:23more
- 10:23holistic
- 10:24picture
- 10:24because
- 10:24you're
- 10:24looking
- 10:25at
- 10:25everything
- 10:25versus
- 10:26just
- 10:26one spot,
- 10:27but
- 10:27you are
- 10:27more
- 10:28likely to
- 10:28miss
- 10:29stuff
- 10:29that's
- 10:29there
- 10:30because
- 10:30of that.
- 10:31And the
- 10:32other
- 10:32thing,
- 10:32just
- 10:32taking
- 10:33a step
- 10:33back,
- 10:33you know,
- 10:34here when
- 10:35we're
- 10:35talking
- 10:35about
- 10:36that
- 10:36liquid
- 10:37biopsy,
- 10:38we're
- 10:38talking
- 10:39about
- 10:39that
- 10:39fishing
- 10:39expedition
- 10:40where
- 10:40we're
- 10:40doing
- 10:41sequencing
- 10:41to say
- 10:42we want
- 10:42to
- 10:42sequence
- 10:43everything
- 10:43in the tumor
- 10:44and
- 10:44figure out
- 10:45what is
- 10:47what
- 10:47changes.
- 10:48There's
- 10:48a tumor.
- 10:48Have our
- 10:49conversation
- 10:50in about
- 10:50an hour
- 10:50or two
- 10:51where
- 10:51we're
- 10:51looking
- 10:51at
- 10:52circulating
- 10:52DNA is
- 10:53really
- 10:53more of a
- 10:54focused
- 10:54panel
- 10:55that
- 10:55we're
- 10:55using
- 10:56to guide
- 10:56and
- 10:57measure,
- 10:57not to
- 10:58really
- 10:58understand
- 10:58the biology.
- 10:59So
- 10:59they are
- 10:59a little
- 11:00different
- 11:00tests,
- 11:01that's
- 11:01important
- 11:02in
- 11:02regards
- 11:02to the
- 11:03likelihood
- 11:04of
- 11:04finding
- 11:04something
- 11:04or not.
- 11:05Because
- 11:05with
- 11:05liquid
- 11:06biopsies,
- 11:07the more things
- 11:08you look
- 11:08at
- 11:09and with
- 11:10tumor
- 11:10testing
- 11:11we want
- 11:11to look
- 11:11at
- 11:12everything
- 11:12for this
- 11:13discussion,
- 11:14the more likely
- 11:14you
- 11:14are to
- 11:15miss
- 11:15stuff
- 11:16where
- 11:16if you
- 11:16look at
- 11:17one
- 11:17thing,
- 11:17you're
- 11:17more
- 11:17likely
- 11:18to find
- 11:18that one
- 11:18thing.
- 11:19talking
- 11:19about
- 11:19the,
- 11:20tumor
- 11:20biopsies,
- 11:21you know,
- 11:21you're
- 11:22less
- 11:22likely
- 11:22to miss
- 11:23any changes
- 11:24that are
- 11:24there.
- 11:25There are
- 11:26different
- 11:26types.
- 11:26There's
- 11:27targeted
- 11:27panels.
- 11:28I think
- 11:28some are
- 11:29do
- 11:29a couple
- 11:29hundred
- 11:30genes.
- 11:30There's
- 11:30some
- 11:31that,
- 11:32sequenced
- 11:32the
- 11:32entire
- 11:33genome.
- 11:34There's
- 11:34some that
- 11:34sequence
- 11:35both DNA
- 11:36and RNA.
- 11:37Can
- 11:37you kind
- 11:37of
- 11:38discuss
- 11:38the
- 11:38differences
- 11:38of
- 11:39the panels
- 11:39and the
- 11:39advantages
- 11:40to the
- 11:40different
- 11:41methods
- 11:41that are
- 11:42out
- 11:42there?
- 11:43doing DNA
- 11:44and RNA
- 11:45assessment
- 11:45is as
- 11:46you can
- 11:46imagine,
- 11:47we have
- 11:47a lot
- 11:47of 23,000
- 11:48plus
- 11:49genes
- 11:49in our
- 11:49body,
- 11:49lots and
- 11:50lots of
- 11:50genes
- 11:51to
- 11:52be able
- 11:52to look
- 11:52at all
- 11:53of those.
- 11:53The
- 11:54technology
- 11:54has advanced
- 11:55quite
- 11:55a bit
- 11:56and so,
- 11:57so that
- 11:57it's
- 11:58somewhat
- 11:58cost
- 11:58efficient
- 11:59for us
- 11:59to be
- 11:59able to
- 11:59do this.
- 12:00Now
- 12:01when we
- 12:01look at,
- 12:02ihc
- 12:03or
- 12:03basically
- 12:04proteins,
- 12:04what's on
- 12:05the cell
- 12:05surface
- 12:06of your
- 12:07tumors,
- 12:08that's
- 12:08very
- 12:08advantageous
- 12:09for us.
- 12:09But
- 12:10that's
- 12:10one
- 12:10kind of
- 12:10test we
- 12:11call
- 12:11that,
- 12:12you know,
- 12:12surface
- 12:12and ihc
- 12:13a surface
- 12:14test.
- 12:14So
- 12:14tells us
- 12:14what's on
- 12:15the
- 12:15actual
- 12:15outside
- 12:16of it.
- 12:16Can it
- 12:17be
- 12:17targeted.
- 12:18There's
- 12:18a lot of
- 12:18your PDL
- 12:19ones or,
- 12:19and
- 12:20other,
- 12:21proteins.
- 12:22And so
- 12:22a lot
- 12:23of your
- 12:23local
- 12:23labs and
- 12:24requests
- 12:25and
- 12:25whatnot,
- 12:25they'll
- 12:25do those
- 12:26in-house.
- 12:26They can
- 12:26typically do that,
- 12:28but
- 12:28they don't
- 12:28have the
- 12:28technology
- 12:29to to do
- 12:30full
- 12:31exome
- 12:32or genome
- 12:32sequencing.
- 12:34And that
- 12:34comes to
- 12:35a lot
- 12:35of your
- 12:35commercial
- 12:36labs
- 12:36that many of
- 12:36you
- 12:37may be
- 12:37familiar
- 12:37with.
- 12:38So when
- 12:39we're
- 12:39talking
- 12:39about
- 12:39whole
- 12:40exome
- 12:40sequencing,
- 12:41we're
- 12:41basically
- 12:42looking
- 12:42or
- 12:43looking
- 12:43at at
- 12:44analyzing
- 12:45DNA
- 12:46and RNA
- 12:47that
- 12:47are,
- 12:49basically
- 12:50with
- 12:50whole
- 12:50genome
- 12:51sequencing.
- 12:51I'll just
- 12:52start
- 12:52there.
- 12:52That's a
- 12:52bigger
- 12:53picture.
- 12:53Yeah,
- 12:54it's it's
- 12:54a lot
- 12:54harder
- 12:55to do
- 12:55because
- 12:55you're
- 12:55looking
- 12:55at
- 12:56everything,
- 12:56everything
- 12:56that's
- 12:57going
- 12:57to be
- 12:57made
- 12:57or not
- 12:58be made.
- 12:58Whereas
- 12:58whole
- 12:59exome
- 12:59sequencing,
- 12:59it's
- 13:00a lot
- 13:00more
- 13:00targeted.
- 13:01So
- 13:01a lot of
- 13:02your DNA
- 13:02is what
- 13:03we call
- 13:03junk DNA.
- 13:04Yeah,
- 13:04it's
- 13:04there and
- 13:05there's
- 13:05nothing.
- 13:06And so
- 13:07genomic
- 13:08whole
- 13:08genome is
- 13:09you
- 13:09sequence
- 13:09everything,
- 13:10including
- 13:10the junk
- 13:11where
- 13:11a whole
- 13:12exome is
- 13:12saying,
- 13:13we're
- 13:13just
- 13:13going
- 13:13to
- 13:13sequence
- 13:14the stuff
- 13:14that
- 13:14we know
- 13:15actually
- 13:15has a
- 13:15biological
- 13:16purpose.
- 13:16Exactly.
- 13:17Yeah.
- 13:17And
- 13:18that's
- 13:18the
- 13:18difference.
- 13:18And
- 13:18it really
- 13:19and the
- 13:19importance
- 13:19of that
- 13:19goes back
- 13:20to what
- 13:20we
- 13:20discussed
- 13:20before.
- 13:21The more
- 13:21things
- 13:22you look
- 13:22at,
- 13:22the harder
- 13:23it is
- 13:23to know
- 13:23what's
- 13:23there.
- 13:24And so
- 13:25trying
- 13:25to
- 13:25focus on
- 13:26the non
- 13:26junk
- 13:27we think
- 13:27is
- 13:28advantageous.
- 13:28Exactly.
- 13:29And you
- 13:30know,
- 13:30again
- 13:31with
- 13:31hotspot
- 13:31panel.
- 13:32So I
- 13:32you know
- 13:33we always
- 13:33say ten
- 13:35years ago
- 13:35there were
- 13:36maybe 50
- 13:36or 100
- 13:37genes
- 13:37that were
- 13:38actually
- 13:38relevant
- 13:39in cancer
- 13:39that we
- 13:39needed to
- 13:40know
- 13:40about.
- 13:41Then
- 13:41it was
- 13:41300
- 13:42genes.
- 13:43So when
- 13:43we started,
- 13:44we
- 13:44call them
- 13:44hotspot
- 13:44panels,
- 13:45where
- 13:45we just
- 13:45look at
- 13:46those
- 13:46100 genes
- 13:47or those
- 13:47300
- 13:47genes,
- 13:48because
- 13:48those are
- 13:48the ones
- 13:48most
- 13:49valuable
- 13:49in
- 13:49knowing
- 13:50about.
- 13:51But we're
- 13:51seeing
- 13:51is that
- 13:52that
- 13:52number
- 13:53keeps
- 13:53increasing.
- 13:55Now
- 13:55it's up
- 13:55to 700
- 13:56and
- 13:56something
- 13:57genes
- 13:58that are
- 13:58relevant
- 13:58in
- 13:59cancer.
- 14:00And so
- 14:00we've
- 14:01moved out
- 14:01of this
- 14:02thought
- 14:03process
- 14:03of just
- 14:03doing
- 14:04what's
- 14:04currently
- 14:05relevant
- 14:05in cancer
- 14:06and
- 14:06expanding
- 14:07that
- 14:08by
- 14:08looking
- 14:08at whole
- 14:08exome,
- 14:09looking
- 14:09at
- 14:09everything
- 14:10essentially
- 14:11here,
- 14:11a lot of
- 14:11people
- 14:12say,
- 14:12well,
- 14:12that's
- 14:12a lot.
- 14:13Why would you be doing
- 14:14that?
- 14:14It's not
- 14:14even relevant
- 14:15right
- 14:15now.
- 14:16But then
- 14:16I can
- 14:17argue
- 14:18this gene
- 14:18wasn't
- 14:19relevant,
- 14:19you know,
- 14:19six
- 14:20months ago.
- 14:20And now
- 14:21it is.
- 14:21And so
- 14:22the
- 14:22advantage,
- 14:23the advantage
- 14:23of
- 14:23basically
- 14:24doing
- 14:24whole
- 14:24exome
- 14:25sequencing,
- 14:25not
- 14:25looking
- 14:26at just
- 14:26700
- 14:27genes,
- 14:27but
- 14:28looking
- 14:28at 20,000
- 14:30genes,
- 14:31is that
- 14:31when you
- 14:32do that,
- 14:32you're
- 14:32basically
- 14:33getting
- 14:33that
- 14:33whole
- 14:34molecular
- 14:34blueprint
- 14:35of your
- 14:35tumor
- 14:36right
- 14:36now.
- 14:37And so if
- 14:38there is
- 14:38something
- 14:39that may
- 14:39be
- 14:39changed
- 14:40in
- 14:40the tumor,
- 14:40if
- 14:41there's a
- 14:41clinical
- 14:41trial
- 14:42or
- 14:42therapy
- 14:43that may be
- 14:43upcoming,
- 14:44you don't have
- 14:45to retest
- 14:46for that
- 14:46tumor.
- 14:46You already know
- 14:47that that
- 14:48tumor
- 14:48harbors
- 14:48that
- 14:48alteration.
- 14:50And so we
- 14:50always
- 14:50say that
- 14:51every
- 14:51gene is
- 14:52going
- 14:52to be
- 14:52a cancer
- 14:52gene
- 14:53at some point.
- 14:54So
- 14:54why don't
- 14:54we test
- 14:55for it
- 14:55now?
- 14:55Why don't
- 14:56we get
- 14:56that full
- 14:56picture
- 14:58of all
- 14:58the
- 14:58genes and
- 14:59their
- 14:59status?
- 15:00And then
- 15:00if you do
- 15:00another
- 15:01biopsy,
- 15:01you can also
- 15:02see how
- 15:02it
- 15:02changes
- 15:03later on.
- 15:03But
- 15:04at least
- 15:04you get
- 15:04a
- 15:04snapshot
- 15:05snapshot
- 15:06of what's
- 15:06happening
- 15:07and
- 15:07not just
- 15:07a limited
- 15:08view of
- 15:08those
- 15:09hundred
- 15:09or 300
- 15:10genes,
- 15:10but
- 15:10everything
- 15:11that's
- 15:11going on.
- 15:12So that's
- 15:12a
- 15:12difference
- 15:12between
- 15:13a hotspot
- 15:14just
- 15:14looking
- 15:14at
- 15:14a couple
- 15:15hundred
- 15:15genes
- 15:16versus
- 15:16full
- 15:16exome
- 15:17sequencing,
- 15:17when you're
- 15:17looking
- 15:18at
- 15:18hundreds
- 15:19of genes
- 15:20at a
- 15:20time.
- 15:21And
- 15:21I think
- 15:21what
- 15:21you said
- 15:22is
- 15:22important,
- 15:22you know,
- 15:23in
- 15:23the future,
- 15:24everything,
- 15:24every gene
- 15:25will
- 15:25probably
- 15:25be
- 15:26a cancer
- 15:26gene.
- 15:26You know,
- 15:27doctor,
- 15:28may me
- 15:28who's
- 15:29over
- 15:29there
- 15:29and
- 15:29doctor
- 15:29McKean
- 15:30will be
- 15:30talking,
- 15:31soon
- 15:31about
- 15:31clinical
- 15:32trials.
- 15:32And we're
- 15:33seeing
- 15:33clinical
- 15:33trials
- 15:34come in
- 15:34with
- 15:35different
- 15:35ways of
- 15:36targeting
- 15:37vast
- 15:38number of
- 15:39mutations,
- 15:40that
- 15:40are
- 15:41coming in
- 15:41the pipeline
- 15:42that will be
- 15:42available
- 15:42both now
- 15:43and in 2
- 15:44or
- 15:443 years.
- 15:44And so
- 15:45having
- 15:45that
- 15:45information
- 15:46is
- 15:46hugely important.
- 15:47So
- 15:47I always
- 15:48tell
- 15:48people
- 15:48if you're
- 15:48going
- 15:49to do
- 15:49genetic
- 15:50testing
- 15:50on
- 15:50the tumor,
- 15:51like I've
- 15:52already
- 15:52said,
- 15:52that's a
- 15:52fishing
- 15:53expedition.
- 15:53So I tell
- 15:54people
- 15:54if you're
- 15:54going
- 15:54to fish,
- 15:55fish
- 15:56don't
- 15:56half way
- 15:57fish,
- 15:57right.
- 15:58If you're
- 15:58going
- 15:58to do it,
- 15:58do it.
- 15:59And so
- 16:00it's
- 16:00really
- 16:00important
- 16:01if you're
- 16:01going
- 16:01to do
- 16:02these tests
- 16:02that you
- 16:02do these
- 16:03full
- 16:03panels,
- 16:04because
- 16:04most of
- 16:05the time
- 16:05you're
- 16:05not
- 16:06going to
- 16:06find
- 16:06anything
- 16:06that
- 16:07will affect
- 16:08how
- 16:08your cancer
- 16:08is
- 16:09treated
- 16:09or how
- 16:09your family
- 16:10member's
- 16:10cancer is
- 16:10treated.
- 16:11But
- 16:11you could
- 16:12and
- 16:12we've all
- 16:12found
- 16:13something
- 16:13that has
- 16:13changed
- 16:14the
- 16:14management
- 16:14and
- 16:14benefited
- 16:15the patients.
- 16:16And
- 16:16you also
- 16:17raise a
- 16:17good point
- 16:17if you're
- 16:18gonna get
- 16:19NGS
- 16:20testing.
- 16:21I feel like
- 16:21there's
- 16:22a lot
- 16:22of jargon
- 16:22in the field
- 16:23right
- 16:23now.
- 16:24NGS is
- 16:25used as
- 16:25a huge
- 16:26umbrella
- 16:26term
- 16:26for what
- 16:27we were
- 16:27talking
- 16:27about,
- 16:28about
- 16:28whole
- 16:28exome
- 16:29sequencing
- 16:29and
- 16:29whatnot.
- 16:30And so
- 16:30it's
- 16:30important
- 16:31I always
- 16:31advocate
- 16:32people
- 16:32to know
- 16:33what
- 16:34they're
- 16:34looking
- 16:34for
- 16:35and
- 16:35having
- 16:35those
- 16:35conversations
- 16:36with your
- 16:37oncologist
- 16:38as
- 16:38they're
- 16:38able to
- 16:38tell you
- 16:39what
- 16:40tests
- 16:41they'll
- 16:41be
- 16:41sending for
- 16:43and what
- 16:43that test
- 16:43is
- 16:44actually
- 16:44going
- 16:45to
- 16:45give you
- 16:45the
- 16:45results
- 16:46for.
- 16:46You can ask them
- 16:47how many
- 16:47genes
- 16:47are
- 16:47tested
- 16:48in my panel,
- 16:49and
- 16:49if they say
- 16:50100, 200,
- 16:51you
- 16:51kind of
- 16:52know it's
- 16:52a hotspot
- 16:53and maybe
- 16:53you can
- 16:53advocate
- 16:54for
- 16:54something
- 16:55larger.
- 16:55But
- 16:55of course
- 16:56it's
- 16:56always up
- 16:56to you
- 16:57and
- 16:57the oncologist,
- 16:58but just
- 16:58knowing
- 16:58kind of
- 16:59what
- 16:59you're
- 16:59looking for
- 17:00and that
- 17:01there are
- 17:01differences
- 17:02in these
- 17:02tests,
- 17:02I think
- 17:03are
- 17:03really
- 17:03important
- 17:03in your
- 17:04own
- 17:04journey.
- 17:05And and
- 17:06going
- 17:06there,
- 17:06speaking
- 17:07to that
- 17:07as well,
- 17:07you know,
- 17:08these
- 17:08platforms,
- 17:09this next
- 17:10generation
- 17:10sequencing
- 17:11or NGS
- 17:11testing
- 17:12is not
- 17:13cheap.
- 17:13Most of
- 17:14the time.
- 17:14Insurance
- 17:15covers
- 17:15it.
- 17:15And so
- 17:16insurance
- 17:16is
- 17:16paying a
- 17:17hefty price.
- 17:18But
- 17:18if you're
- 17:18going to
- 17:19have your
- 17:20insurance
- 17:20pay
- 17:21thousands
- 17:21of
- 17:21dollars
- 17:22for the
- 17:22test,
- 17:23pay
- 17:23thousands
- 17:23of
- 17:23dollars
- 17:24for the
- 17:24complete
- 17:24test
- 17:25is what I
- 17:25always
- 17:25say.
- 17:25Make sure
- 17:26you're
- 17:26getting
- 17:26all the
- 17:27information
- 17:27if you're
- 17:27going
- 17:28to pay
- 17:28for it.
- 17:29And
- 17:29thankfully,
- 17:30within
- 17:30the past
- 17:31couple
- 17:31of years,
- 17:31most of
- 17:31these
- 17:32panels
- 17:32have
- 17:32become
- 17:33very
- 17:33comprehensive.
- 17:34Pretty
- 17:34much
- 17:35complete
- 17:35genome
- 17:36or
- 17:36complete
- 17:37non junk
- 17:37genome.
- 17:39still are
- 17:39some very
- 17:40focused
- 17:40panels
- 17:40out
- 17:41there.
- 17:41And so
- 17:41just
- 17:41making
- 17:42sure you
- 17:42send the
- 17:42most
- 17:43broad one
- 17:44you can
- 17:44find is
- 17:45important.
- 17:46So that's
- 17:46kind of
- 17:47a big
- 17:47overview
- 17:47of the
- 17:48different
- 17:48tests.
- 17:48What
- 17:49questions
- 17:49do
- 17:49you guys
- 17:50have
- 17:50before
- 17:50we start
- 17:50talking
- 17:51about
- 17:51uveal
- 17:52melanoma
- 17:52genetics
- 17:53and how
- 17:53this is
- 17:53impacting
- 17:54treatment?
- 17:55in the
- 17:56next
- 17:56part.
- 18:01the
- 18:01question
- 18:01was
- 18:01have we
- 18:02heard of
- 18:02the
- 18:02gallery,
- 18:03tests
- 18:03that are
- 18:04supposed
- 18:04to detect
- 18:05cancer?
- 18:06And do
- 18:06we think
- 18:06it could
- 18:07pick up
- 18:07melanoma
- 18:08or uveal
- 18:09melanoma?
- 18:09So that's
- 18:10a great
- 18:11question.
- 18:11And
- 18:11the gallery
- 18:12test is a
- 18:12little
- 18:13different.
- 18:13So
- 18:14most of the time
- 18:15when
- 18:15we're
- 18:16when
- 18:16we're
- 18:16talking
- 18:17about
- 18:17these
- 18:17genetic
- 18:18tests
- 18:18and these
- 18:19liquid
- 18:19biopsies
- 18:20that
- 18:20we're
- 18:20discussing,
- 18:21we're
- 18:22looking to
- 18:22say
- 18:23are there
- 18:24changes
- 18:24in the DNA.
- 18:25So
- 18:25has the
- 18:25DNA
- 18:26changed?
- 18:27Its
- 18:27sequence
- 18:27has
- 18:27gotten
- 18:28shorter,
- 18:28gotten
- 18:28longer.
- 18:30This
- 18:30gallery
- 18:30test
- 18:31actually
- 18:31looks at
- 18:32the
- 18:32methylation.
- 18:33So on
- 18:33your DNA
- 18:34you have
- 18:34these
- 18:34little
- 18:35methyl
- 18:35groups.
- 18:36And then
- 18:37there's a
- 18:37different
- 18:37pattern
- 18:38of how
- 18:38those
- 18:39groups
- 18:39can be
- 18:39on there.
- 18:40And
- 18:40so that
- 18:40really
- 18:41looks
- 18:41at this
- 18:41methylation
- 18:42pattern.
- 18:42And then
- 18:43can
- 18:43predict
- 18:43cancer
- 18:45in the
- 18:45current
- 18:45version
- 18:46of the test
- 18:47you know
- 18:47they can
- 18:47detect
- 18:48all cancers.
- 18:49And these
- 18:49were
- 18:49validated
- 18:50on very
- 18:51big trials
- 18:52I don't know
- 18:53off
- 18:53the top
- 18:53of my head
- 18:54if uveal
- 18:55melanoma,
- 18:56how
- 18:56well that
- 18:57was
- 18:57represented
- 18:58in those
- 18:58trials
- 19:00usually
- 19:00rare
- 19:01cancers
- 19:01like this
- 19:02are not
- 19:02that well
- 19:03represented.
- 19:03And so
- 19:04the tests
- 19:04are not
- 19:04as good
- 19:06for rare
- 19:07tumor
- 19:07such as
- 19:07uveal
- 19:08melanoma
- 19:08as
- 19:08they are.
- 19:09And more
- 19:09common
- 19:09cancers
- 19:10like
- 19:10lung,
- 19:11breast,
- 19:11prostate,
- 19:12etc..
- 19:13The other
- 19:13thing
- 19:13about
- 19:14those
- 19:14tests is
- 19:15they're
- 19:16still new
- 19:16and we're
- 19:16still
- 19:16learning,
- 19:17you know,
- 19:17in the
- 19:18current
- 19:18version
- 19:18of the test,
- 19:19if
- 19:19it comes back
- 19:20saying
- 19:21that you
- 19:21have
- 19:21cancer in
- 19:22your
- 19:22blood
- 19:23for,
- 19:24let's
- 19:24say, for
- 19:24example,
- 19:25breast
- 19:25cancer,
- 19:26there's a
- 19:2644%
- 19:27chance
- 19:27that you
- 19:27actually
- 19:28don't.
- 19:29That is a
- 19:29false
- 19:29positive.
- 19:30And so
- 19:31I think
- 19:32the is
- 19:33advancing
- 19:33the newer
- 19:34versions
- 19:34of that
- 19:34test
- 19:35are in trial.
- 19:36So
- 19:36I think
- 19:36they will
- 19:36get
- 19:37better
- 19:37with
- 19:37time.
- 19:38Right now
- 19:38I'd be hesitant
- 19:39to you
- 19:40put
- 19:40too much
- 19:40weight
- 19:41on it
- 19:41for uveal
- 19:42melanoma.
- 19:42Just
- 19:42because I
- 19:43don't
- 19:44know if
- 19:44it was
- 19:44adequately
- 19:45represented
- 19:46and how
- 19:46the test
- 19:46was
- 19:46developed.
- 19:47And
- 19:47a test
- 19:48is only
- 19:48as good
- 19:48as
- 19:49how
- 19:49well you
- 19:49trained
- 19:49it.
- 19:51Yeah,
- 19:51I don't
- 19:52know
- 19:52if you
- 19:52have any
- 19:52thoughts
- 19:52on that.
- 19:53I think
- 19:53it's
- 19:53exciting.
- 19:54It's
- 19:55again,
- 19:55that
- 19:55whole
- 19:56expansion
- 19:56of liquid
- 19:57biopsies
- 19:58and it's
- 19:58the
- 19:58new kid
- 19:59on the
- 19:59block
- 19:59essentially
- 20:00in
- 20:00testing,
- 20:01again,
- 20:02the data
- 20:02is going
- 20:03to take
- 20:03a little
- 20:03while
- 20:04to catch
- 20:04up.
- 20:04So I'm
- 20:05always a
- 20:05strong
- 20:05advocate
- 20:06like the
- 20:06Nccn
- 20:07guidelines.
- 20:07And
- 20:08if you
- 20:08have
- 20:08tissue,
- 20:09that's
- 20:09what you
- 20:10want to
- 20:10send.
- 20:10But
- 20:10with all
- 20:11these
- 20:11additional
- 20:11tools
- 20:12with
- 20:12liquid,
- 20:12it's it's
- 20:13getting
- 20:13more and
- 20:13more
- 20:13exciting,
- 20:14especially
- 20:15with,
- 20:15you know,
- 20:15early
- 20:16detection
- 20:17and
- 20:17being able
- 20:18to
- 20:18determine
- 20:19if it's
- 20:19there's
- 20:20likely to
- 20:20be
- 20:20recurrence
- 20:21and
- 20:21everything.
- 20:21So we're
- 20:22in a
- 20:22really
- 20:23neat time
- 20:24of of
- 20:25science
- 20:25and
- 20:25advancements.
- 20:27So
- 20:27the question
- 20:28was for
- 20:28people
- 20:28who are
- 20:28like five
- 20:29years out
- 20:29are there
- 20:30any research
- 20:30looking
- 20:31at liquid
- 20:31biopsies
- 20:32to try
- 20:32to
- 20:32understand
- 20:33what's
- 20:33going on
- 20:33in the
- 20:34genetics
- 20:34of
- 20:34the tumor?
- 20:35And from
- 20:36my
- 20:36understanding,
- 20:37there's
- 20:37nothing
- 20:37in that
- 20:38particular
- 20:39area.
- 20:40Now.
- 20:40There's
- 20:41a lot
- 20:41of work
- 20:42on trying
- 20:42to
- 20:42develop
- 20:43blood
- 20:43based
- 20:44biomarkers
- 20:44so
- 20:45that can
- 20:45be
- 20:45circulating
- 20:46tumor
- 20:46cells,
- 20:46circulating
- 20:47hybrid
- 20:47cells,
- 20:48circulating
- 20:48DNA.
- 20:49And I
- 20:50believe
- 20:50our
- 20:50we have a panel
- 20:51on that
- 20:51later
- 20:52this afternoon.
- 20:53Those are
- 20:53more
- 20:54focused
- 20:54panels
- 20:55or
- 20:55focused
- 20:55tests
- 20:55really
- 20:56trying
- 20:56to ask
- 20:57the question
- 20:57of can
- 20:58we
- 20:58detect
- 20:59the
- 20:59cancer
- 21:00in the blood
- 21:00so we can
- 21:01follow
- 21:01and
- 21:02monitor
- 21:02it to
- 21:02predict
- 21:03recurrence
- 21:04or
- 21:04response?
- 21:05that
- 21:06is it's a
- 21:06similar
- 21:07technology,
- 21:08but it's
- 21:08a different
- 21:08application
- 21:09of it
- 21:09than
- 21:10saying
- 21:11we're
- 21:11going
- 21:11to try
- 21:11to
- 21:11sequence
- 21:12the
- 21:12entire
- 21:13tumor, to
- 21:13try to
- 21:14understand
- 21:14what's
- 21:14driving
- 21:15the tumor
- 21:15and
- 21:15how
- 21:15we can
- 21:16treat it.
- 21:17And
- 21:17a lot
- 21:17of that
- 21:17goes back
- 21:18to
- 21:18how big
- 21:19of a scope
- 21:19you look
- 21:20at.
- 21:20You know,
- 21:20to
- 21:20really do
- 21:21the full
- 21:21sequencing,
- 21:22you need
- 21:22a good amount of DNA.
- 21:24So
- 21:24I don't know
- 21:25if
- 21:25there
- 21:26ever
- 21:26will be
- 21:26a situation
- 21:27where
- 21:27someone
- 21:28has no
- 21:29disease
- 21:29on
- 21:29their scan,
- 21:30and
- 21:31we're
- 21:31able
- 21:31to do
- 21:31a somatic
- 21:32tumor
- 21:33test like
- 21:33Peter's
- 21:34is
- 21:34talking
- 21:34about,
- 21:34where
- 21:35we can
- 21:35get all
- 21:35that
- 21:35information
- 21:36just
- 21:37because
- 21:37if we
- 21:37if it's
- 21:38not on
- 21:38scans,
- 21:38there's
- 21:39probably
- 21:39not going
- 21:39to be
- 21:39enough
- 21:40DNA
- 21:40available
- 21:41to
- 21:42actually
- 21:42run the
- 21:42test.
- 21:45Yeah,
- 21:45absolutely.
- 21:45that's
- 21:47very
- 21:47important
- 21:48and we
- 21:48take it
- 21:48very
- 21:48seriously
- 21:49as a
- 21:49scientific
- 21:50community,
- 21:51is that
- 21:51there are
- 21:52a lot of
- 21:52different
- 21:52changes
- 21:53that
- 21:53can happen
- 21:54in your
- 21:54DNA, RNA,
- 21:55protein.
- 21:55There
- 21:56actually
- 21:56aren't
- 21:56going to
- 21:56go on
- 21:57to have
- 21:58a
- 21:58functional
- 21:59effect.
- 21:59They're
- 21:59not
- 22:00going to
- 22:00cause
- 22:00cancer.
- 22:01And
- 22:01maybe
- 22:01we just
- 22:01don't
- 22:02know
- 22:02enough
- 22:02about
- 22:03that
- 22:03alteration
- 22:04right
- 22:04now.
- 22:05And so
- 22:05there is
- 22:06a
- 22:06classification
- 22:08of how
- 22:08we,
- 22:09provide
- 22:10a title,
- 22:11basically
- 22:11label
- 22:12if it's
- 22:12going to
- 22:12be
- 22:12cancerous
- 22:13or not,
- 22:13if it's
- 22:14going to
- 22:15contribute
- 22:15to cancer
- 22:15or not.
- 22:16And so
- 22:18you don't
- 22:18want
- 22:18to talk
- 22:18about,
- 22:19oh,
- 22:19we sequenced
- 22:1922,000
- 22:20genes.
- 22:20All this
- 22:21information
- 22:21that
- 22:21would be
- 22:21in an
- 22:22encyclopedia,
- 22:23essentially
- 22:24way too
- 22:24much
- 22:24information.
- 22:25So
- 22:26I can speak
- 22:27to cares
- 22:27at least.
- 22:27And a
- 22:28lot of
- 22:28this
- 22:28happens
- 22:29in the
- 22:29background
- 22:29with
- 22:29much of
- 22:30the
- 22:30bioinformatics,
- 22:31is we
- 22:31look at
- 22:32the
- 22:32mutations
- 22:33we see
- 22:33about the
- 22:33relevance
- 22:34in that
- 22:35specific
- 22:35tumor
- 22:36type,
- 22:36the
- 22:36relevance
- 22:37of that,
- 22:38mutation
- 22:39in other
- 22:39tumors.
- 22:40Does it
- 22:41play a
- 22:41role in,
- 22:42in the
- 22:42cell
- 22:42growth
- 22:43or in,
- 22:46in
- 22:46basically,
- 22:47is it
- 22:47function
- 22:47altered
- 22:48enough to
- 22:48cause
- 22:49cancer?
- 22:49And so we
- 22:50will
- 22:50classify
- 22:51them,
- 22:51we'll
- 22:51say, hey,
- 22:52this is a
- 22:52pathogenic
- 22:53mutation.
- 22:53This is
- 22:53something
- 22:54that's
- 22:54going to
- 22:54contribute
- 22:55to it
- 22:55or it's a
- 22:56likely
- 22:57pathogenic
- 22:57meaning
- 22:58we
- 22:58see that
- 22:58it has
- 23:00contributes
- 23:00to cancer
- 23:01in in
- 23:01lung
- 23:02cancer,
- 23:02but not
- 23:02so much in uveal
- 23:04but
- 23:04it's good
- 23:04for you
- 23:04to be
- 23:05aware
- 23:05of it.
- 23:06And
- 23:06then
- 23:06we have
- 23:06other ones
- 23:07where
- 23:07there's
- 23:07mutations
- 23:08that
- 23:08we know
- 23:09nothing's
- 23:09going to
- 23:10happen
- 23:10with that
- 23:10gene.
- 23:11Essentially
- 23:12you have
- 23:12that
- 23:12information
- 23:13there
- 23:13in case
- 23:14it does
- 23:14become
- 23:14relevant
- 23:15in
- 23:15the future.
- 23:16You could
- 23:16always
- 23:17give
- 23:17a call
- 23:17and get
- 23:17the information,
- 23:18but
- 23:19we know
- 23:19that it's
- 23:20nothing
- 23:20that you
- 23:20really
- 23:21need
- 23:21to pay
- 23:21attention
- 23:21to
- 23:22right now
- 23:22because
- 23:22it's
- 23:23just,
- 23:24you know,
- 23:24a word
- 23:27that's
- 23:27basically
- 23:28misspelled,
- 23:28but it's
- 23:29not
- 23:29caught.
- 23:29You can still
- 23:30understand
- 23:30the
- 23:30sentence.
- 23:31It's how
- 23:31I think
- 23:32about it.
- 23:33so but
- 23:33it is
- 23:34very
- 23:34important
- 23:34because
- 23:35you don't
- 23:35want
- 23:36anyone
- 23:36to be
- 23:37looking
- 23:37at a
- 23:37report
- 23:38and see
- 23:38all these
- 23:38mutations
- 23:39and
- 23:39get scared
- 23:40when
- 23:40they're
- 23:40actually
- 23:41not doing
- 23:41anything
- 23:42in the body.
- 23:43And so
- 23:43we do
- 23:43make sure
- 23:44to do
- 23:44that and
- 23:45classify
- 23:45these mutations
- 23:46so that
- 23:46we know
- 23:47that
- 23:47the ones
- 23:47that are
- 23:47relevant
- 23:48are
- 23:48actually
- 23:49the ones
- 23:49that are
- 23:49going to
- 23:49be
- 23:49provided
- 23:50to the
- 23:50clinician.
- 23:52point,
- 23:52Doctor
- 23:52Mamie.
- 23:53So,
- 23:53you know,
- 23:53when
- 23:53we're
- 23:54interpreting
- 23:54this
- 23:54data,
- 23:55there's
- 23:55a lot of
- 23:56questions
- 23:56we have to ask.
- 23:57I think
- 23:57the first question
- 23:58we have
- 23:58to ask
- 23:59is,
- 24:00does
- 24:00the change
- 24:01actually
- 24:01affect
- 24:02that
- 24:02protein
- 24:03which
- 24:04peaches
- 24:04discussed.
- 24:05And then
- 24:05there's
- 24:05a second
- 24:06question
- 24:06of does
- 24:07that
- 24:07matter to
- 24:08this tumor.
- 24:09And that
- 24:10one's
- 24:10sometimes
- 24:11a harder
- 24:11question
- 24:11to answer.
- 24:13But
- 24:13you know,
- 24:14like
- 24:14Doctor
- 24:14alluded
- 24:15to, we
- 24:15do have
- 24:16what
- 24:16we call
- 24:16driver
- 24:17mutations,
- 24:18which are
- 24:18mutations
- 24:18that
- 24:19we know
- 24:19are
- 24:19driving
- 24:19the cancer.
- 24:20And then
- 24:21we have
- 24:21what
- 24:22we call,
- 24:22standby
- 24:23or
- 24:23bystander
- 24:24mutations,
- 24:24which
- 24:25happen
- 24:26in the cancer
- 24:27but
- 24:27probably
- 24:27aren't
- 24:27driving
- 24:28it.
- 24:28And those
- 24:29can be
- 24:30sometimes
- 24:30near
- 24:30impossible
- 24:32to
- 24:32differentiate.
- 24:34one of
- 24:35the
- 24:35things
- 24:35you can
- 24:35look at
- 24:36is
- 24:36the percentage
- 24:37within
- 24:37the tumor.
- 24:38So
- 24:38anytime
- 24:38you get
- 24:38these
- 24:39reports,
- 24:39you'll
- 24:39get a percent
- 24:40that says
- 24:41this
- 24:41change
- 24:41is in
- 24:42this percent.
- 24:43And
- 24:43sometimes
- 24:44we will
- 24:44see, you
- 24:45know,
- 24:45people
- 24:46being
- 24:47treated
- 24:47with
- 24:47drugs
- 24:47where
- 24:48some
- 24:48mutations
- 24:49have
- 24:494,050% in
- 24:50the tumor,
- 24:51they find
- 24:52one
- 24:52mutation
- 24:52that's 1
- 24:53to 2%
- 24:53in the tumor.
- 24:54And we'll
- 24:55say, oh,
- 24:55we should
- 24:56try to
- 24:56treat
- 24:56that.
- 24:57Probably
- 24:57not a
- 24:57good
- 24:58treatment
- 24:58option
- 24:59if it's
- 25:00only
- 25:00in that
- 25:01few
- 25:01percentage
- 25:01of the tumor,
- 25:02because
- 25:02you're
- 25:02not
- 25:02actually
- 25:03covering
- 25:03at all.
- 25:03So
- 25:04you do have
- 25:05to
- 25:05interpret
- 25:05these
- 25:05reports
- 25:06in these
- 25:06changes,
- 25:07because
- 25:07sometimes
- 25:07with a
- 25:07grain
- 25:08of salt,
- 25:08by asking
- 25:09the questions,
- 25:10does that
- 25:10change
- 25:11actually
- 25:11affect
- 25:11biology.
- 25:12And does
- 25:13that
- 25:13biology
- 25:13actually
- 25:14drive
- 25:14this
- 25:14tumor?
- 25:15because
- 25:16sometimes
- 25:16you
- 25:16can get
- 25:17a little
- 25:18lost in
- 25:18the weeds
- 25:18in these
- 25:19reports
- 25:20if you don't
- 25:20look at
- 25:20those
- 25:21or
- 25:21don't ask
- 25:22those
- 25:22questions.
- 25:24let's
- 25:24talk
- 25:25about,
- 25:25what
- 25:26do we
- 25:26expect to
- 25:26find in
- 25:28in kind
- 25:29of a
- 25:29testing
- 25:30of
- 25:30a uveal
- 25:31melanoma.
- 25:31So
- 25:32as most of
- 25:33us know,
- 25:34most
- 25:34uveal
- 25:35melanomas
- 25:35are
- 25:36driven
- 25:37by one of
- 25:38two
- 25:38mutations,
- 25:39either
- 25:39a mutation
- 25:39in gnaQ
- 25:40or gna11.
- 25:42And so
- 25:43when
- 25:43we send
- 25:43these
- 25:43reports,
- 25:44we 96%
- 25:45of the
- 25:46time find
- 25:46a mutation
- 25:47in one of
- 25:47these two
- 25:48proteins.
- 25:49Those are
- 25:50proteins
- 25:50as in
- 25:51right now
- 25:52where
- 25:52we can't
- 25:52actually
- 25:53target
- 25:53there's
- 25:53no drugs
- 25:54that can
- 25:54actually
- 25:55turn that
- 25:55on
- 25:55or off.
- 25:56There's
- 25:56a lot of
- 25:57work
- 25:58going
- 25:58on in
- 25:58the
- 25:58scientific
- 25:59community
- 26:00trying to
- 26:00figure out
- 26:00how
- 26:00we can
- 26:01do that,
- 26:02but just
- 26:02the type
- 26:03of
- 26:03protein
- 26:03that
- 26:04these
- 26:05mutations
- 26:05are
- 26:06actually
- 26:07makes it
- 26:07really
- 26:07difficult
- 26:08to block.
- 26:08There's
- 26:09different
- 26:09types
- 26:09of
- 26:09proteins
- 26:10out
- 26:10there.
- 26:10Some
- 26:10are very
- 26:11easy to
- 26:11turn off
- 26:12and turn
- 26:12on.
- 26:13This particular
- 26:14type is
- 26:14a little
- 26:14bit more
- 26:15difficult.
- 26:15So it's
- 26:15been a
- 26:16little
- 26:16harder.
- 26:16then
- 26:16a lot of
- 26:17the other,
- 26:18we see,
- 26:19or the
- 26:19other
- 26:19most
- 26:19common
- 26:20mutation
- 26:20we see
- 26:20in uveal
- 26:21melanoma
- 26:21is BAP 1
- 26:23which
- 26:23again is
- 26:23very
- 26:24hard.
- 26:25necessarily
- 26:26a
- 26:26therapeutic
- 26:27option.
- 26:27So
- 26:28a lot of times
- 26:29when
- 26:29we do
- 26:29this type
- 26:30of
- 26:30testing
- 26:31in
- 26:32patients
- 26:32with
- 26:32uveal
- 26:32melanoma
- 26:33where
- 26:33their cancer
- 26:34has
- 26:34spread,
- 26:34we see
- 26:35a mutation
- 26:36in one of
- 26:36those
- 26:36two, Gina
- 26:37Q or Gina
- 26:38Levin,
- 26:39maybe
- 26:40with a
- 26:40mutation
- 26:40in BAP 1
- 26:41or a
- 26:41couple
- 26:42other,
- 26:44So
- 26:44most of
- 26:45the time,
- 26:45the fishing
- 26:46expedition
- 26:46of doing
- 26:47this
- 26:47testing
- 26:48doesn't
- 26:48necessarily
- 26:49change
- 26:49your
- 26:49treatment
- 26:50option,
- 26:51but it's
- 26:51really
- 26:52when
- 26:52we find
- 26:52something
- 26:53unexpected,
- 26:53because
- 26:54one
- 26:54of the
- 26:54big
- 26:55shifts
- 26:55that
- 26:55we see
- 26:56right now
- 26:57in
- 26:57the oncology
- 26:58community
- 26:58as a
- 26:59whole is
- 27:00redefining
- 27:01cancer,
- 27:01not
- 27:02based on
- 27:02where
- 27:02it came,
- 27:03but
- 27:03from the
- 27:03genetic
- 27:04mutations.
- 27:05You know,
- 27:05100
- 27:06years ago
- 27:06when we
- 27:07started
- 27:07treating
- 27:07cancer,
- 27:08the only way
- 27:08to
- 27:08kind of
- 27:09lump
- 27:09cancer
- 27:09into
- 27:10groups
- 27:10is to
- 27:10say,
- 27:10this is where
- 27:11it came
- 27:11from.
- 27:12That's
- 27:12why we
- 27:12call
- 27:12breast
- 27:12cancer,
- 27:13breast
- 27:13cancer
- 27:13and
- 27:13prostate
- 27:14cancer,
- 27:14prostate
- 27:15cancer.
- 27:16But we're
- 27:16learning
- 27:16now
- 27:17that the
- 27:17molecular
- 27:18signatures
- 27:19for some
- 27:20cases
- 27:20are
- 27:21actually
- 27:21more
- 27:21important
- 27:22than
- 27:22where
- 27:22it came
- 27:23from.
- 27:23So
- 27:24I have had
- 27:24patients
- 27:24with
- 27:25uveal
- 27:25melanoma
- 27:26that's
- 27:26had
- 27:26molecular
- 27:27signatures
- 27:28more
- 27:28consistent
- 27:28with
- 27:29types
- 27:29of colon
- 27:30cancer
- 27:30that can
- 27:30respond
- 27:31to a mean
- 27:31therapy,
- 27:32and that
- 27:33will change
- 27:33what
- 27:33we do.
- 27:34And
- 27:34so most
- 27:35of
- 27:35the time
- 27:35when
- 27:35we're
- 27:35doing
- 27:36this
- 27:36testing,
- 27:36we're
- 27:36not going
- 27:37to find
- 27:37something.
- 27:38But
- 27:38if we
- 27:38find
- 27:39a mutation
- 27:39or
- 27:39a protein
- 27:41that is
- 27:41more
- 27:42similar
- 27:42to
- 27:42another
- 27:42cancer,
- 27:43that can
- 27:44actually
- 27:44change
- 27:45how we
- 27:45treat the
- 27:45patient
- 27:46and
- 27:46benefit
- 27:46the patient.
- 27:47So, like
- 27:48I said,
- 27:49it's a
- 27:49fishing
- 27:49expedition.
- 27:50Most of
- 27:50the time
- 27:50you're
- 27:50going to
- 27:50come up
- 27:51negative,
- 27:51but
- 27:52it can
- 27:52actually
- 27:53change,
- 27:54what
- 27:54what we do
- 27:55and
- 27:55how your cancer
- 27:56is
- 27:56treated.
- 27:56I feel like
- 27:57with
- 27:57the more
- 27:57information
- 27:58we get
- 27:59from
- 27:59all of
- 27:59these
- 27:59tests,
- 28:00we're
- 28:00going to
- 28:00learn
- 28:01more
- 28:01about the
- 28:01cancer
- 28:02itself.
- 28:03And so
- 28:03we know,
- 28:04you know,
- 28:04gnac and
- 28:05all
- 28:06the other
- 28:06ones,
- 28:06which ones
- 28:07are
- 28:07prevalent.
- 28:07We can't act
- 28:08on them
- 28:08yet.
- 28:08There's
- 28:09clinical
- 28:09trials,
- 28:10but we're
- 28:10learning
- 28:11about
- 28:12what
- 28:12shouldn't
- 28:12be there,
- 28:13what's
- 28:13commonly
- 28:14not
- 28:14there.
- 28:14And all
- 28:15these
- 28:15signatures
- 28:15that
- 28:16we can
- 28:16potentially
- 28:16develop
- 28:17in the
- 28:17future
- 28:18to
- 28:18identify
- 28:18these or
- 28:19maybe
- 28:19find
- 28:19future
- 28:20targets.
- 28:20So
- 28:21that's
- 28:21a whole
- 28:21other
- 28:21research
- 28:22side
- 28:22of it.
- 28:22But,
- 28:23you know,
- 28:23the more
- 28:24we test,
- 28:24the more
- 28:24we know.
- 28:25And
- 28:25hopefully
- 28:25you can
- 28:26advance
- 28:26it in
- 28:26the
- 28:26future.
- 28:29I think
- 28:29the other
- 28:30side
- 28:30benefit
- 28:31I'll talk
- 28:31about
- 28:31is most
- 28:32of the,
- 28:33tumor
- 28:33testing
- 28:34platforms
- 28:34that are out there
- 28:35right now
- 28:36will
- 28:36actually
- 28:36give you
- 28:37your HLA
- 28:37type,
- 28:38which is not the type
- 28:40which
- 28:40is not
- 28:41the
- 28:42cancer
- 28:42per say.
- 28:42It's more
- 28:43you HLA
- 28:44type is
- 28:44what
- 28:45I tell
- 28:45people
- 28:45is your
- 28:46blood
- 28:46type,
- 28:47but
- 28:47it's not
- 28:47the blood
- 28:47type
- 28:48we use
- 28:48for blood
- 28:48donation.
- 28:49It's a
- 28:49blood
- 28:49type
- 28:50we use
- 28:50for organ
- 28:50transplant.
- 28:51And for
- 28:52most
- 28:52tumors
- 28:53it's not
- 28:53that
- 28:53relevant
- 28:54for uveal
- 28:54melanoma,
- 28:55it's
- 28:55highly
- 28:55relevant.
- 28:56We know
- 28:57tebentafusp
- 28:57for
- 28:58Kimmtrac
- 28:58is only
- 28:59effective
- 29:00and
- 29:00approved
- 29:01for
- 29:01people
- 29:01who have
- 29:02a certain
- 29:02blood
- 29:02type
- 29:03HLA 8
- 29:04A02 O 1.
- 29:05And
- 29:05we know
- 29:06that
- 29:06there's
- 29:06clinical
- 29:06trials
- 29:07such as
- 29:07the
- 29:08ideaya
- 29:08clinical
- 29:09trial
- 29:09that's
- 29:09available
- 29:10right now
- 29:10for
- 29:10people
- 29:11who don't
- 29:11have that
- 29:12blood
- 29:12type.
- 29:13So it
- 29:14it is
- 29:14advantageous
- 29:15to get
- 29:15this
- 29:15testing
- 29:16because
- 29:16you're
- 29:16getting
- 29:17that
- 29:17information
- 29:17as well.
- 29:18And
- 29:19you can
- 29:19really
- 29:19get
- 29:19kind of
- 29:20both of
- 29:21those
- 29:21and
- 29:21questions
- 29:22answered
- 29:22in
- 29:23one test,
- 29:23which is
- 29:23great.
- 29:24Where
- 29:25historically
- 29:25they were
- 29:25separate
- 29:26tests.
- 29:31Any other
- 29:32questions?
- 29:32No.
- 29:34Anything else
- 29:34you want
- 29:35to add
- 29:35or
- 29:35discuss?
- 29:36Peaches,
- 29:36I know
- 29:37the talk
- 29:37has been
- 29:37pretty
- 29:38informal
- 29:38since
- 29:38we didn't
- 29:38have
- 29:38slides.
- 29:39Yeah,
- 29:39yeah.
- 29:39just
- 29:40I always
- 29:40advocate
- 29:41for
- 29:41asking,
- 29:42having
- 29:42the
- 29:42conversations
- 29:43with your
- 29:43clinicians.
- 29:44I know,
- 29:44Doctor
- 29:44moser
- 29:44you're
- 29:45fantastic
- 29:45with
- 29:46taking
- 29:46time
- 29:46with your patients.
- 29:47And I
- 29:47know,
- 29:48you know,
- 29:48sometimes
- 29:49that's
- 29:49a little
- 29:50hard
- 29:50to get
- 29:50from
- 29:50clinicians.
- 29:51So,
- 29:51you know,
- 29:52advocating
- 29:52for
- 29:52yourself,
- 29:53asking
- 29:53those
- 29:54questions
- 29:54about,
- 29:55again,
- 29:55somatic
- 29:56testing
- 29:56can mean
- 29:56a lot
- 29:57of things
- 29:58to a
- 29:58lot of
- 29:58different
- 29:59companies.
- 30:00So take
- 30:00the time
- 30:01with the
- 30:01clinician
- 30:02who
- 30:02should know
- 30:02essentially
- 30:03what
- 30:03you're
- 30:03looking
- 30:03for to
- 30:04get the
- 30:04right
- 30:04test.
- 30:06Right.
- 30:07So,
- 30:07Doctor
- 30:08Moser,
- 30:08peaches,
- 30:09I have
- 30:09a question
- 30:10for you
- 30:10guys.
- 30:11at least
- 30:11are
- 30:11familiar
- 30:12with the
- 30:12Castle
- 30:12Biosciences
- 30:13report
- 30:14that
- 30:14comes in.
- 30:15we can
- 30:15see
- 30:15lots of
- 30:16different
- 30:16things.
- 30:17I know
- 30:17I've
- 30:17looked at
- 30:18one of
- 30:18my caris
- 30:18reports.
- 30:19with
- 30:19Doctor
- 30:19Moser in
- 30:20an appointment,
- 30:20but I'm
- 30:21curious,
- 30:21do
- 30:21you guys
- 30:22have any
- 30:22tools at
- 30:23caris
- 30:23Life
- 30:23Sciences?
- 30:25you guys
- 30:25have any
- 30:25tools for
- 30:26helping
- 30:26patients
- 30:26to,
- 30:27understand
- 30:27and
- 30:27interpret
- 30:28their
- 30:28report
- 30:28aside
- 30:29from
- 30:29obviously
- 30:29talking
- 30:29to their
- 30:30oncologist?
- 30:32It's a
- 30:32great
- 30:32question.
- 30:33So,
- 30:35we do
- 30:35we have a
- 30:35whole
- 30:36patient
- 30:36navigation.
- 30:37Okay.
- 30:38Essentially
- 30:38that will
- 30:38help.
- 30:39I like
- 30:40to work
- 30:41closely
- 30:41with
- 30:41my oncologist
- 30:42so
- 30:43that if
- 30:43they have
- 30:44questions,
- 30:45I can be
- 30:45that
- 30:46resource.
- 30:47in my
- 30:47role,
- 30:48technically
- 30:48not
- 30:48allowed
- 30:48to speak
- 30:49to
- 30:49patients
- 30:50in that
- 30:50capacity.
- 30:51Yeah.
- 30:51but so
- 30:52we do
- 30:53really
- 30:53good.
- 30:54we offer
- 30:54a lot of
- 30:55education
- 30:55things.
- 30:55So we
- 30:56work
- 30:56with RN's
- 30:57and the
- 30:57whole
- 30:57clinical
- 30:58staff,
- 30:58if they
- 30:58want to
- 30:59learn
- 30:59how to
- 30:59interpret
- 31:00the reports
- 31:01as well,
- 31:01just said
- 31:02a basic
- 31:02level.
- 31:03Yeah.
- 31:03so that
- 31:04they can
- 31:04at least.
- 31:04What is the
- 31:05green box
- 31:05mean?
- 31:05What is
- 31:06a red
- 31:06box?
- 31:06You know,
- 31:07where
- 31:07am I
- 31:07looking
- 31:08on this
- 31:08report?
- 31:08Yeah.
- 31:09because
- 31:09I think
- 31:10it's
- 31:10valuable
- 31:10a lot of
- 31:10the time
- 31:11they're
- 31:11spending
- 31:12time with
- 31:12the
- 31:12patient
- 31:12as well,
- 31:13answering
- 31:13these
- 31:13questions.
- 31:14so
- 31:16I would
- 31:16say a
- 31:17lot of
- 31:18companies
- 31:18do have
- 31:19some sort
- 31:19of
- 31:19patient
- 31:20navigation
- 31:20team that
- 31:21can
- 31:21help them
- 31:21walk
- 31:22through
- 31:22those
- 31:22reports.
- 31:23but we
- 31:23leave it
- 31:24to the
- 31:24clinician
- 31:24most
- 31:25of the time
- 31:25because
- 31:26you don't
- 31:26want
- 31:26anything
- 31:26to kind
- 31:27of get
- 31:27misinterpreted.
- 31:28You know,
- 31:28for sure.
- 31:29That
- 31:29totally
- 31:29makes
- 31:30sense.
- 31:31question,
- 31:32kind of
- 31:32just
- 31:32regarding
- 31:32what
- 31:32you guys
- 31:33do on
- 31:33your end,
- 31:34obviously,
- 31:35biopsy
- 31:35testing,
- 31:36rare
- 31:36diseases.
- 31:37We know
- 31:37from
- 31:38many of us
- 31:39who
- 31:39have had
- 31:39castle
- 31:39testing,
- 31:40and
- 31:40I think
- 31:40I know
- 31:40I've
- 31:41seen this
- 31:41on my
- 31:41Kara's
- 31:42tests.
- 31:42We get,
- 31:43you know,
- 31:43an
- 31:43explanation
- 31:44of
- 31:44benefits
- 31:44and your
- 31:45insurance
- 31:45comes
- 31:45back
- 31:45and says,
- 31:46this
- 31:46is an
- 31:46experimental test
- 31:47and
- 31:47we don't
- 31:48cover it.
- 31:49what does
- 31:49caris do
- 31:50for
- 31:50patients
- 31:50as far
- 31:50as a
- 31:51financial
- 31:51standpoint
- 31:51to help
- 31:52them
- 31:53figure
- 31:53that out?
- 31:54Yeah.
- 31:54And and
- 31:54that's
- 31:55it.
- 31:55The two
- 31:55most
- 31:56common
- 31:56questions
- 31:56I get.
- 31:57yeah,
- 31:58that's an
- 31:58excellent
- 31:58question.
- 31:59So again,
- 31:59we have
- 32:00an entire
- 32:01patient
- 32:01team that
- 32:02will do
- 32:02that.
- 32:02One thing
- 32:03I can
- 32:03advocate
- 32:04with
- 32:04caris is
- 32:04I know
- 32:05the test
- 32:05is
- 32:05expensive,
- 32:06just like
- 32:06most
- 32:07tests.
- 32:07It really
- 32:08depends
- 32:08on
- 32:08insurance
- 32:09and where
- 32:09you're at
- 32:09with your
- 32:10deductible
- 32:10as well.
- 32:11That
- 32:11plays a
- 32:11huge part
- 32:12versus
- 32:12getting
- 32:13the test
- 32:13in the
- 32:13beginning
- 32:14of
- 32:14the year
- 32:14versus
- 32:14the end
- 32:14of the
- 32:15year.
- 32:15All of
- 32:15those
- 32:15logistics,
- 32:16which
- 32:16I've
- 32:17recently
- 32:17learned,
- 32:19but we do
- 32:20I do
- 32:20pride
- 32:21myself in
- 32:21the
- 32:21patient
- 32:22navigator
- 32:22team that
- 32:22we have.
- 32:23They
- 32:23essentially
- 32:24we don't
- 32:24direct
- 32:24dealer.
- 32:25I don't
- 32:27balance
- 32:27Bill.
- 32:27Thank you.
- 32:28And so
- 32:28the patient
- 32:29isn't
- 32:29going
- 32:29to
- 32:30receive
- 32:30a bill
- 32:30like
- 32:31that.
- 32:31Okay.
- 32:33we've
- 32:33had it
- 32:34maybe
- 32:34happen
- 32:34by mistake
- 32:35once you
- 32:35contact
- 32:36us
- 32:36immediately
- 32:36and we're
- 32:37going to
- 32:37work to
- 32:37to figure
- 32:38that out.
- 32:39We also know
- 32:39that some
- 32:40have to
- 32:40do
- 32:40self-pay.
- 32:41And so
- 32:42how do we
- 32:42work with
- 32:42that.
- 32:43So
- 32:43basically.
- 32:45We.
- 32:46Even if
- 32:46there
- 32:47were
- 32:47a bill
- 32:47and maybe
- 32:48a couple
- 32:48hundred
- 32:49dollars,
- 32:50there's
- 32:50still
- 32:50a program
- 32:51for that.
- 32:51Okay.
- 32:52So we're
- 32:52not
- 32:52going to
- 32:52dig into
- 32:53finances.
- 32:54We
- 32:54don't do
- 32:54any
- 32:54of that
- 32:55in the
- 32:55background,
- 32:55which
- 32:56I know
- 32:56some some
- 32:57companies
- 32:57unfortunately.
- 32:58You know,
- 32:59I just
- 32:59I think
- 32:59it's
- 32:59important
- 33:00to point
- 33:00out,
- 33:00especially
- 33:01for
- 33:01those of
- 33:01us who
- 33:02are
- 33:02dealing
- 33:02with,
- 33:03who are
- 33:04dealing
- 33:04with,
- 33:04you know,
- 33:05the
- 33:05financial
- 33:05aspect
- 33:06of things
- 33:06and just
- 33:06the
- 33:07confusion
- 33:07and the
- 33:07insurance
- 33:08battles
- 33:08and
- 33:09just all
- 33:09of the
- 33:09things
- 33:10that can
- 33:10come into
- 33:10our
- 33:11diagnosis,
- 33:11it's
- 33:12important
- 33:12to
- 33:12recognize
- 33:12we do have
- 33:13companies
- 33:13who are doing
- 33:14these tests
- 33:15who
- 33:15understand
- 33:15that
- 33:16and
- 33:16who are
- 33:16making it
- 33:17as much
- 33:17of a
- 33:17priority
- 33:18as
- 33:18possible
- 33:19for
- 33:19patients
- 33:20to not be
- 33:20financially
- 33:21burdened
- 33:21by having
- 33:22this test
- 33:22done.
- 33:23And
- 33:23I guess
- 33:24my
- 33:24you
- 33:24guys can
- 33:25correct me
- 33:25if
- 33:25I'm wrong,
- 33:26but my
- 33:26understanding
- 33:26of it
- 33:27is that
- 33:27these
- 33:27companies
- 33:28want
- 33:29they want
- 33:29to do
- 33:30the research,
- 33:30they want
- 33:30to see
- 33:31what's
- 33:31happening.
- 33:32And and
- 33:32it's
- 33:33worth
- 33:33it's
- 33:33worth
- 33:34doing
- 33:34the
- 33:34tests,
- 33:35and it's
- 33:35worth
- 33:35seeing
- 33:36the results
- 33:36from as
- 33:37many
- 33:37patients
- 33:37as possible
- 33:38to get
- 33:38that
- 33:38information.
- 33:39Like, I,
- 33:40I don't
- 33:40know,
- 33:40I guess
- 33:41for me,
- 33:41as
- 33:41a patient,
- 33:41it
- 33:41kind of
- 33:42feels
- 33:42like the
- 33:42information
- 33:43feels
- 33:43more
- 33:44valuable
- 33:45than the
- 33:46cash flow.
- 33:47If
- 33:47that makes
- 33:47some
- 33:48level of
- 33:48sense.
- 33:49Like
- 33:49I know
- 33:49the cash
- 33:50flow is
- 33:50important,
- 33:50the company
- 33:51has to
- 33:51continue.
- 33:51But,
- 33:52I just
- 33:52appreciate
- 33:53that that
- 33:53caris
- 33:54Castle
- 33:54like
- 33:54that,
- 33:55they make
- 33:55a priority
- 33:55of trying
- 33:56not to
- 33:56put that
- 33:56burden
- 33:56on
- 33:57patients.
- 33:57Yeah.
- 33:57And
- 33:58both of
- 33:58the
- 33:58companies,
- 33:59I mean,
- 33:59we work
- 33:59closely
- 33:59with
- 33:59Castle
- 34:00two.
- 34:00They're
- 34:00great is
- 34:01how much
- 34:02more can
- 34:03we learn
- 34:03about this
- 34:04so that
- 34:05we can
- 34:05have
- 34:06advancements?
- 34:06It's a
- 34:07question.
- 34:08Yeah.
- 34:08So
- 34:08translational.
- 34:09So
- 34:09that's
- 34:09the whole
- 34:10goal.
- 34:10Yeah.
- 34:10Is that
- 34:11we learn
- 34:12all of
- 34:12this
- 34:12information.
- 34:13Find
- 34:13this.
- 34:13So
- 34:14I would even
- 34:15say that
- 34:15the
- 34:16the drugs
- 34:17that we
- 34:17have now
- 34:17for these
- 34:18specific
- 34:18markers
- 34:19like the
- 34:19HLA or
- 34:22you
- 34:22know, PDL
- 34:22ones and
- 34:23stuff
- 34:23that all
- 34:23came from
- 34:24learning
- 34:25about it
- 34:26first.
- 34:26So that
- 34:26being
- 34:27in the experiment
- 34:27and
- 34:27now you
- 34:28went
- 34:28through
- 34:28the
- 34:29clinical
- 34:29trials
- 34:30to now
- 34:30be a drug
- 34:31that's
- 34:31available.
- 34:31So it all
- 34:32begins
- 34:32with
- 34:32research
- 34:32and with
- 34:33caris
- 34:34We're
- 34:34twofold.
- 34:35So first
- 34:35and
- 34:35foremost
- 34:36it's
- 34:36about
- 34:36getting
- 34:37those
- 34:37reports
- 34:37to our
- 34:37clinician
- 34:38to help
- 34:38with
- 34:38treatment.
- 34:39But
- 34:40our second
- 34:40branch is
- 34:41research.
- 34:42So how do
- 34:42we use
- 34:42this not
- 34:43to sell
- 34:43the information
- 34:44but to
- 34:44learn about it
- 34:46and then
- 34:46partner
- 34:46to figure
- 34:47out like,
- 34:47hey,
- 34:47these
- 34:48these
- 34:48genes are
- 34:49coming up
- 34:49a lot
- 34:49in this
- 34:50cancer
- 34:51based
- 34:51off of
- 34:51how they
- 34:51work.
- 34:52Can
- 34:52a drug
- 34:52be
- 34:53developed
- 34:53to then,
- 34:54you know,
- 34:54go into
- 34:55trials
- 34:56for use?
- 34:57So
- 34:57just to
- 34:58recap
- 34:58that
- 34:58question,
- 34:59for
- 34:59those of
- 34:59you
- 34:59who
- 34:59didn't
- 34:59hear the
- 34:59question,
- 35:01just does
- 35:01the kind
- 35:02of the
- 35:02more
- 35:02testing
- 35:03that
- 35:03happens
- 35:03make it
- 35:04so that
- 35:04the drug
- 35:05becomes
- 35:06approved
- 35:06as
- 35:06something
- 35:07that
- 35:07is no
- 35:07longer
- 35:07experimental
- 35:08by
- 35:08insurance?
- 35:09Anyway,
- 35:10kind
- 35:11of to
- 35:11go along.
- 35:11I'll
- 35:11go ahead.
- 35:12I want to
- 35:12I'll
- 35:12answer
- 35:13that too.
- 35:13So you're
- 35:13absolutely
- 35:14right.
- 35:14Right.
- 35:14If
- 35:14we find
- 35:15that
- 35:15something's
- 35:15there
- 35:16on doing
- 35:16these
- 35:16tests,
- 35:17that
- 35:17drives
- 35:17the
- 35:17development
- 35:18of ways
- 35:18to try to
- 35:19target
- 35:19it.
- 35:20And so by
- 35:20doing
- 35:21more
- 35:21tests,
- 35:21we might
- 35:22better
- 35:22understand
- 35:22it.
- 35:23I think
- 35:23the other
- 35:24thing,
- 35:24too, is,
- 35:27tests
- 35:27are done,
- 35:28the companies
- 35:28can share
- 35:29the data
- 35:30kind of
- 35:30anonymously
- 35:31in
- 35:32aggregate.
- 35:32So
- 35:32they'll
- 35:33say, here
- 35:33is
- 35:33the data
- 35:34of,
- 35:35you know,
- 35:35so many
- 35:35patients
- 35:36with this
- 35:36that's
- 35:36done.
- 35:37And
- 35:38we are
- 35:38starting
- 35:38to do
- 35:39that.
- 35:39And
- 35:39through
- 35:39academic
- 35:40partnerships,
- 35:41you know,
- 35:41I've been
- 35:42involved
- 35:42with this
- 35:42with
- 35:42Caris
- 35:43and other
- 35:43companies.
- 35:44We're
- 35:44going
- 35:44back and
- 35:45saying,
- 35:45okay,
- 35:45so you do
- 35:46all the RNA
- 35:47and
- 35:47that's
- 35:47not part
- 35:47of the report,
- 35:48but
- 35:48can
- 35:49we try
- 35:49to
- 35:49analyze
- 35:50that to
- 35:50better
- 35:50understand
- 35:51the tumor
- 35:51biology?
- 35:52So both,
- 35:53you know,
- 35:54getting
- 35:54the test
- 35:54done
- 35:55helps
- 35:55potentially
- 35:56drive
- 35:56new
- 35:56therapies
- 35:57by
- 35:57finding
- 35:57stuff
- 35:58to
- 35:58target.
- 35:59But
- 35:59we also
- 35:59are also
- 36:00exploring
- 36:01that
- 36:01in an
- 36:01anonymized
- 36:02way
- 36:03or non
- 36:03patient
- 36:04specific
- 36:04way to
- 36:05try to
- 36:05understand
- 36:06the biology
- 36:07so we can
- 36:07better
- 36:07treat it
- 36:08in that
- 36:08way too.
- 36:08Yeah.
- 36:09Awesome.
- 36:10Do
- 36:10we have
- 36:10any other
- 36:10questions
- 36:11from the
- 36:11audience?
- 36:14Doctor
- 36:14Mimi
- 36:15yeah, I
- 36:15was
- 36:16going.
- 36:16To
- 36:16just turn
- 36:16just my point
- 36:18that,
- 36:18you know,
- 36:19when we
- 36:20collect
- 36:20these,
- 36:21ngs
- 36:22or DNA
- 36:23sequencing
- 36:24or RNA
- 36:24sequencing
- 36:25data,
- 36:26we can
- 36:26pair it
- 36:27with
- 36:28the outcomes
- 36:29that have
- 36:29happened
- 36:29in a
- 36:30patient's
- 36:31mutations
- 36:32and how
- 36:33they did
- 36:33with
- 36:33certain
- 36:34treatment
- 36:34and
- 36:34what kind
- 36:35of side
- 36:35effects
- 36:36they had.
- 36:37And
- 36:37hopefully
- 36:38that
- 36:38could
- 36:38drive
- 36:39clinical
- 36:39trials
- 36:40and
- 36:40research
- 36:40to
- 36:40predict
- 36:42responses
- 36:43or
- 36:43predict
- 36:44toxicity
- 36:45in
- 36:45patients.
- 36:45So,
- 36:46you know,
- 36:47if we can
- 36:47say that,
- 36:48hey,
- 36:48there's a
- 36:49marker Z
- 36:50in this
- 36:51patient's,
- 36:52tumor
- 36:53and that
- 36:53predicts
- 36:54for
- 36:54toxicity,
- 36:55but
- 36:55it doesn't
- 36:56predict
- 36:56for a
- 36:56response,
- 36:57then
- 36:57we can
- 36:58potentially
- 36:59avoid
- 36:59those
- 36:59therapies.
- 37:00Right.
- 37:01And
- 37:01and so
- 37:03that will
- 37:03only come
- 37:04if
- 37:04we have
- 37:05paired
- 37:06data,
- 37:06paired
- 37:07data from
- 37:08the patient
- 37:09and
- 37:09from our
- 37:10industry
- 37:10partners,
- 37:11caris
- 37:11compass
- 37:11and
- 37:12all of them,
- 37:13and
- 37:13kind of
- 37:14putting
- 37:14that
- 37:14together
- 37:15in an
- 37:15anonymous
- 37:16way.
- 37:16So we're
- 37:16not
- 37:17really,
- 37:17you know,
- 37:18sort of
- 37:19the red
- 37:20tape
- 37:20to buy
- 37:20a lot of
- 37:21the
- 37:21things
- 37:21that
- 37:21happens
- 37:22in this
- 37:22kind of
- 37:22research.
- 37:24Thank
- 37:25you so
- 37:25much,
- 37:25Doctor
- 37:26I think
- 37:27one of
- 37:27the
- 37:28things
- 37:28that
- 37:28I take
- 37:28away from
- 37:29this is,
- 37:29is these
- 37:30kinds
- 37:30of tests
- 37:30help
- 37:31eliminate
- 37:31trial
- 37:31and
- 37:31error,
- 37:32like,
- 37:32right
- 37:32now,
- 37:33I remember
- 37:33when,
- 37:34when
- 37:34Doctor
- 37:34Moser
- 37:35came out
- 37:35to, like,
- 37:36with
- 37:36Meredith
- 37:36McKeen
- 37:37in
- 37:37Nashville,
- 37:38one of the
- 37:38things
- 37:38that we
- 37:39talked
- 37:39about
- 37:39at the
- 37:39very
- 37:39beginning
- 37:40of
- 37:40some of
- 37:40those
- 37:40sessions,
- 37:41we were
- 37:41talking
- 37:41about,
- 37:41you know,
- 37:42navigating
- 37:42clinical
- 37:43trials.
- 37:43We were
- 37:44talking
- 37:44about the
- 37:44new FDA
- 37:45approval
- 37:45of
- 37:46aspect.
- 37:46Kimmtrac.
- 37:47I guess
- 37:48that's
- 37:48the same
- 37:48thing,
- 37:48in case
- 37:49you
- 37:49didn't
- 37:49know.
- 37:51but
- 37:51one of
- 37:52the
- 37:52things
- 37:52that
- 37:52doctor
- 37:52from
- 37:53doctor
- 37:53McKean
- 37:53and
- 37:53Doctor
- 37:54Moser
- 37:54both
- 37:54touched on
- 37:55is that
- 37:56at that
- 37:56time,
- 37:57you know,
- 37:57two years
- 37:57ago, most
- 37:58uveal
- 37:59melanoma
- 37:59oncologists
- 38:00were kind
- 38:00of like
- 38:00flying
- 38:01by the
- 38:01seat of
- 38:01their
- 38:01pants.
- 38:02It was
- 38:02guesswork.
- 38:03We're
- 38:03getting
- 38:03closer
- 38:04and
- 38:04closer
- 38:04to having
- 38:05less
- 38:05guesswork
- 38:06for
- 38:06certain
- 38:07parts
- 38:07of the
- 38:08demographic
- 38:08of
- 38:08patients.
- 38:09And
- 38:09the more
- 38:09of these
- 38:10kinds
- 38:10of tests
- 38:10that happen,
- 38:11the more
- 38:12that
- 38:12guesswork
- 38:12goes
- 38:12away.
- 38:13And it's
- 38:13no longer
- 38:13trial
- 38:14and
- 38:14error.
- 38:14Quite
- 38:14as scary
- 38:15feeling
- 38:16as it,
- 38:16you know,
- 38:17was five,
- 38:17ten years
- 38:18ago.
- 38:20Okay.
- 38:21Any other
- 38:21questions?
- 38:22Comments?
- 38:24let's
- 38:24give a
- 38:25round of
- 38:25applause
- 38:25to Doctor
- 38:26Ulrich
- 38:26and
- 38:26Doctor
- 38:27Mosher,
- 38:27and,
- 38:28we'll
- 38:28bring
- 38:28them back
- 38:29up
- 38:29a little
- 38:29later.
