  - a large health system sits down with the
• 0:05
  - CEO of a small community mental health organization,
• 0:08
  - the conversation sounds different than most conference panels.
• 0:12
  - Nobody's selling a framework.
• 0:13
  - They're comparing notes on what's actually failing and
• 0:16
  - what's starting to work.

  - That's the room Becker's Fall Behavioral Health Summit
• 0:20
  - is built to be. 30 plus speakers,
• 0:23
  - 300 plus executive level attendees, and two days
• 0:25

  - Welcome to the Becker's Healthcare podcast. I'm Chris
• 0:52
  - Sosa, your host. I'm thrilled to be joined
• 0:54
  - today by doctor Nick Holkamp. He's chief health
• 0:56
  - transformation officer at Ranken Jordan Pediatric Bridge Hospital
• 1:00
  - in Missouri.
• 1:01
  - Nick, thank you for joining us today. Thank
• 1:03
  - you for having me. Wonderful. So, Nick, you're
• 1:05
  - here to discuss how to improve care for
• 1:07
  - children with medically complex conditions.
• 1:10
  - But before we dive into that, could you
• 1:11
  - please just tell us about yourself and about
• 1:13
  - your career?
• 1:15
  - Well, I'm, trained as a pediatrician,
• 1:17
  - spent nine years at a a general hospital
• 1:20
  - here in Saint Louis as a pediatrician
• 1:23
  - hospital based. And then in 2000 came to
• 1:26
  - be the medical director of a then
• 1:29
  - small,
• 1:30
  - pediatric
• 1:31
  - nursing home actually, Ranken Jordan at the time,
• 1:34
  - and,
• 1:36
  - have been here twenty six years during which
• 1:38
  - time the hospital has
• 1:40
  - become a hospital and evolved into really a
• 1:43
  - leading provider of transitional care services for children
• 1:47
  - with medical complexity of all ages, really bridging
• 1:50
  - the gap between hospital and home for those
• 1:53
  - kids and their families.
• 1:55
  - Thank you for laying that out for us,
• 1:57
  - Nick. So
• 1:58
  - as we dive into the crux of our
• 2:00
  - conversation today, so you are adamant that play
• 2:03
  - is critical for children's recovery.
• 2:05
  - You've been saying that for years. This is
• 2:07
  - not something that's new at all in terms
• 2:09
  - of your approach to care for for children
• 2:11
  - in general. So first question I have for
• 2:13
  - you is simply, what should leaders understand about
• 2:15
  - that approach?
• 2:17
  - Well, for me, it's as simple as
• 2:20
  - play is an essential part of childhood. Play
• 2:23
  - is the work of childhood,
• 2:26
  - and childhood is critical in becoming a high
• 2:28
  - functioning adult. The absence of play means
• 2:32
  - these kids would not be getting
• 2:34
  - the all the important developmental
• 2:36
  - and social and emotional
• 2:38
  - building blocks that,
• 2:40
  - they depend on when they continue to grow
• 2:43
  - and evolve as adult humans.
• 2:45
  - So play has to be the central feature,
• 2:48
  - especially for children
• 2:50
  - challenged by the added insult of having complex
• 2:53
  - medical conditions and,
• 2:56
  - being confined in, let's just be frank, not
• 3:00
  - developmentally ideal settings,
• 3:02
  - I e hospital rooms and hospital beds.
• 3:05
  - Nick, I love that turn of phrase. It
• 3:07
  - plays the work of childhood.
• 3:10
  - Is that something that that you've been hammering
• 3:13
  - home as far as,
• 3:15
  - how we should think about it? How, more
• 3:17
  - importantly, how health care leaders should think about
• 3:20
  - that?
• 3:21
  - I think so. Although it's well understood and
• 3:24
  - recognized and reinforced daily by the American
• 3:27
  - Academy of Pediatrics, for example, who says play
• 3:29
  - is essential. Get more play into a child's
• 3:32
  - life,
• 3:33
  - deviate away from screens and isolation
• 3:36
  - and get kids playing. So it's well understood.
• 3:39
  - It's just sort of,
• 3:41
  - forgotten a little bit in hospitals, and these
• 3:44
  - are the kids who spend
• 3:46
  - lots of time in hospitals,
• 3:48
  - sometimes the majority of their childhood if it's
• 3:50
  - the first year or two or three.
• 3:53
  - So make sure that
• 3:54
  - play
• 3:55
  - is present because that's I I say it's
• 3:58
  - work of childhood. It's the central therapeutic
• 4:02
  - element for a recovering child too. If you
• 4:05
  - are playing, you are getting better. You are
• 4:07
  - healing. You are getting stronger,
• 4:09
  - socializing,
• 4:10
  - reengaging
• 4:11
  - with your community.
• 4:12
  - If not, you're not able to do those
• 4:15
  - things optimally.
• 4:17
  - Well, Nick,
• 4:18
  - as you and Rankin Jordan at Rankin Jordan
• 4:21
  - and other health care leaders
• 4:24
  - try to to expand this approach, I understand
• 4:26
  - there's a new research department,
• 4:28
  - that's been developed to study, you know, the
• 4:30
  - care model. So what would you say are
• 4:32
  - its goals?
• 4:34
  - Well, the first major goal was to assess
• 4:38
  - our own
• 4:39
  - environment,
• 4:40
  - because,
• 4:41
  - obviously, that's one of the things we have
• 4:43
  - in our control is the environment in which
• 4:46
  - children are unfortunately stuck for medical reasons.
• 4:50
  - And when we looked, nobody had ever
• 4:53
  - assessed
• 4:54
  - the,
• 4:55
  - environment in terms of where kids are, who
• 4:58
  - they're with, what they're doing, what impact that
• 5:00
  - was having on their
• 5:02
  - recovery.
• 5:03
  - And so,
• 5:05
  - that was sort of
• 5:07
  - the starting point. How are we doing on
• 5:09
  - giving kids,
• 5:12
  - helping enhance their environment to give them those
• 5:14
  - developmental opportunities alongside the intensive medical support
• 5:19
  - that they're getting to recover from whatever insult,
• 5:22
  - prematurity, or a terrible accident.
• 5:25
  - So,
• 5:26
  - really,
• 5:27
  - that was the starting point. We've made a
• 5:29
  - lot of enhancements to our care environment. As
• 5:31
  - a result of that, we're remeasuring that. And
• 5:34
  - along the
• 5:35
  - way, we've
• 5:36
  - then,
• 5:38
  - taken the next step to try and assess
• 5:41
  - what impact does improving a child's environment have
• 5:44
  - on their outcomes, and what outcomes are we
• 5:47
  - interested in, how well they're healing, how well
• 5:50
  - small children are developing.
• 5:53
  - Small children stuck in hospitals for extended periods
• 5:56
  - have
• 5:58
  - a very high incidence of severe developmental delay.
• 6:02
  - Might we have an impact on that? That's
• 6:05
  - our real focus at the moment and probably
• 6:07
  - for a number of years to come is,
• 6:09
  - can we improve
• 6:11
  - the course
• 6:13
  - of what is commonly not a great track
• 6:15
  - record on developmental outcomes for, high medical needs
• 6:19
  - kids stuck in hospitals for extended periods.
• 6:23
  - Nick, as you mentioned enhancements, I also wanna
• 6:25
  - touch on some of the innovations and even
• 6:28
  - potential innovations that may be farther down the
• 6:31
  - road,
• 6:32
  - that you're
• 6:33
  - developing,
• 6:34
  - you know, down at Rankin Jordan.
• 6:36
  - So
• 6:37
  - what would you say are are the top
• 6:39
  - two or maybe three innovations that are really
• 6:42
  - exciting you right now? Well, the one that's
• 6:44
  - been there really since Mary Reinking Jordan's time
• 6:46
  - in 1941
• 6:48
  - is what we now call care beyond the
• 6:49
  - bedside, just to focus on getting kids up
• 6:52
  - and out of bed and out of their
• 6:54
  - rooms, enhancing their environment. That's step number one.
• 6:58
  - When we did our study and found we
• 7:00
  - could improve on that, we looked at what's
• 7:02
  - missing even in our environment that is normal
• 7:06
  - and accepted and expected for every other child
• 7:09
  - is
• 7:11
  - engagement with other kids and learning. That's what
• 7:13
  - happens in preschool or day care. That doesn't
• 7:17
  - happen in hospitals. That's when we developed the
• 7:20
  - Oz program,
• 7:21
  - short for the optimization
• 7:23
  - zone, because we're trying to optimize every child's
• 7:25
  - neurodevelopmental
• 7:26
  - recovery, which
• 7:28
  - is going to school and playing with other
• 7:30
  - kids. This can't happen for these kids,
• 7:33
  - stuck in hospitals.
• 7:34
  - And and the more general one since that
• 7:37
  - first study
• 7:39
  - are enhancing play opportunities
• 7:41
  - in addition to us,
• 7:43
  - organized play spaces
• 7:45
  - on the,
• 7:46
  - inpatient units
• 7:48
  - and the hiring of new group of employees
• 7:51
  - called patient play associates whose job it is
• 7:54
  - to engage with kids when they're not getting
• 7:56
  - the medical care that they need intermittently throughout
• 7:58
  - the day.
• 7:59
  - Nick, do you find yourself,
• 8:01
  - at Rankin Jordan getting an overwhelming amount of
• 8:05
  - applications
• 8:06
  - for her patient play associates? It sounds like
• 8:08
  - a pretty good job to have in health
• 8:10
  - care.
• 8:11
  - We
• 8:12
  - we don't have a shortage of folks who
• 8:14
  - think like you. That's a great job. And
• 8:16
  - who wouldn't want that? Unfortunately, for the hospital,
• 8:19
  - it's not a high skilled job. It's,
• 8:23
  - something that
• 8:25
  - students on the way to a career in
• 8:27
  - health care might like, and, boy, how rewarding
• 8:30
  - it is to see
• 8:32
  - a child with tubes in the neck and
• 8:34
  - the stomach and,
• 8:36
  - monitors attached,
• 8:38
  - get them up and out of their bed,
• 8:40
  - even just to the play area adjacent to
• 8:43
  - their hospital room or out of the room
• 8:45
  - into bigger
• 8:47
  - play and therapy areas or outside
• 8:49
  - so that they can have experiences kids not
• 8:52
  - stuck in hospitals would have. So it it
• 8:54
  - it's a it is a popular,
• 8:56
  - position to be applied for.
• 8:58
  - That's wonderful to hear.
• 9:00
  - As you think about
• 9:03
  - how the department is evolving
• 9:07
  - and all the efforts that are going on
• 9:09
  - as far as
• 9:10
  - using play to
• 9:12
  - treat children with complex care conditions
• 9:16
  - or complex medical conditions, I should say. What
• 9:18
  - do you think other hospitals can learn? And,
• 9:20
  - obviously, you're constantly learning this whole part of
• 9:23
  - this process. Right? But in this moment in
• 9:25
  - time, what can other hospitals learn from Michael
• 9:27
  - Jordan? It's a great question, and it'll be
• 9:30
  - the focus or one of the main
• 9:32
  - focus points of a talk we're giving to
• 9:34
  - a group of pediatric,
• 9:36
  - hospital,
• 9:37
  - medicine specialists
• 9:39
  - at a national conference in Kansas City early
• 9:42
  - next month where we're gonna say, this is
• 9:46
  - possible at Ranken Jordan. It's really hard to
• 9:48
  - do upstream in the acute care environment, but
• 9:51
  - here's a few things you can do. You
• 9:53
  - can take notice.
• 9:55
  - Instead of ignore, take notice of that child
• 9:57
  - sitting in bed by themselves doing nothing but
• 10:00
  - staring at a screen and say, we need
• 10:03
  - to do better and encourage your colleagues and
• 10:05
  - administrators
• 10:06
  - to find ways instead of excuses for leaving
• 10:09
  - kids in beds too long. And then,
• 10:13
  - little things,
• 10:14
  - getting a mat in the room or outside
• 10:17
  - the room and getting his up and out
• 10:18
  - of bed and out. Getting kids with other
• 10:21
  - kids is really hard because there's such a
• 10:24
  - high bar for infection prevention purposes.
• 10:28
  - We have to go to extra lengths to
• 10:31
  - overcome that concern.
• 10:33
  - I will tell you that it's not only
• 10:34
  - possible. We do it really well and don't
• 10:36
  - increase a child's risk of of infection.
• 10:40
  - So,
• 10:42
  - test
• 10:43
  - but more generally,
• 10:44
  - test and sort of push on the conventional
• 10:47
  - hospital norms that require kids to be stuck
• 10:50
  - in their room. They're not all as valid
• 10:52
  - and,
• 10:54
  - well,
• 10:55
  - well seated in science as you might have
• 10:57
  - been taught.
• 10:59
  - Nick, is there anything else you would like
• 11:01
  - our audience to know about,
• 11:02
  - your program or Rankin Jordan in general?
• 11:05
  - I think it's, again, just a matter of
• 11:08
  - paying attention to common sense. I often say
• 11:10
  - these kids are,
• 11:13
  - sometimes
• 11:14
  - missing in plain sight. They sort of disappear,
• 11:17
  - and yet they're,
• 11:19
  - a third
• 11:20
  - they're they're they're only 1% of kids generally,
• 11:23
  - you know, children with medical complexity, but they,
• 11:26
  - account for over a third of health care
• 11:28
  - resource use and
• 11:31
  - the majority of hospital days, acute care hospital
• 11:34
  - days.
• 11:35
  - Pay attention to this population and
• 11:38
  - do what we ought to to really optimize
• 11:42
  - their outcomes, not just medically, but,
• 11:44
  - holistically for the whole child, especially their their
• 11:47
  - development in their future life.
• 11:50
  - Well, Nick, here's hoping this approach catches on
• 11:53
  - and very quickly. Thank you so much for,
• 11:55
  - joining us on the Doctors podcast today, and
• 11:58
  - we can't wait till the next time our
• 11:59
  - pass goes. I really sincerely appreciate you having
• 12:02
  - me today. Thank you so much.
