- 0:03>> Dr. Terry Simpson: M. She came into the emergency room in full
- 0:06cardiac arrest. Paramedics were doing CPR on her.
- 0:10They'd already placed a tube down her throat so
- 0:12she could breathe. They shocked her heart twice,
- 0:16trying to bring her back. Her name was Eileen. 52
- 0:19years old. Fit, worked out several times a week,
- 0:23ate well, enjoyed a glass of wine now and then. No
- 0:25medical problems. But that night at her company
- 0:28holiday party, she laughed, she toasted, she
- 0:30danced, and then she collapsed in the ER. Uh, her
- 0:33blood alcohol level was 0.09. The legal limit.
- 0:36Yes, but not unusual for a party. She smelled of
- 0:40alcohol. A nurse muttered, another Holiday heart.
- 0:43And someone else shrugged. Probably a closet
- 0:46alcoholic. But she wasn't. Her cardiology workup
- 0:49was clean. No blocked arteries, no structural
- 0:51disease. Just a healthy woman whose heart decided
- 0:54that night to stop. She, uh, lived, thanks to the
- 1:00over caffeinated paramedic who refused to quit.
- 1:02But she never drank again. Every December, ers
- 1:06fill with people like Eileen. Healthy, active
- 1:09people whose hearts suddenly forget how to beat
- 1:11after a few drinks. We call it Holiday Heart
- 1:14Syndrome. They don't always come in drunk, they
- 1:17come in dead. And if they're lucky, sometimes we
- 1:21bring them back. I'm going to make an appeal you
- 1:25might not want to hear. I'd like you to stop
- 1:28drinking. If you're serious about healthspan, not
- 1:31just lifespan. If you want to cut your risk of
- 1:34heart disease, cancer and dementia, stop drinking.
- 1:37I'm a boomer. Alcohol was what you drank at every
- 1:41party. But the data has changed and so have I.
- 1:47Millennials are leading the way. Drinking less,
- 1:49living longer. Maybe it's time we boomers Gen Z,
- 1:53Gen X caught up. And while some people scold you
- 1:57about glyphosate in their Cheerios or BPA in
- 2:00canned beans, they'll say it while sipping their
- 2:02second eight ounce glass of Cabernet. They'll warn
- 2:05you about forever chemicals while doing more
- 2:07metabolic damage with an Old Fashioned than a
- 2:09lifetime of canned chickpeas ever could. The
- 2:12single greatest threat to longevity you can remove
- 2:16isn't sugar. It isn't seed oil. It isn't soda.
- 2:20It's alcohol. I am your Chief Medical
- 2:26Explanationist, Dr. Terry Simpson, and this is
- 2:29Fork U Fork University, where we make sense of the
- 2:32madness, bust a few myths, and teach you a little
- 2:35bit about food and medicine. Every December,
- 2:42emergency room doctor, doctors and staff brace for
- 2:45the holiday Heart. Someone with no heart history
- 2:48suddenly flips into atrial fibrillation after just
- 2:50a few drinks. Alcohol irritates the electrical
- 2:53system of the heart, shifts electrolytes inflames
- 2:56tissues, and the heart starts drumming like a jazz
- 2:59solo. Atrial fibrillation may sound benign, but if
- 3:02that misfire lands on the wrong millisecond, the
- 3:05heart doesn't flutter. It stops. And when the
- 3:09heart stops, you die. Now, maybe someone starts
- 3:13cpr. Maybe there's a defibrillator. But even if
- 3:15you've come back, you've probably lost some brain
- 3:18cells. Holiday heart isn't acute flutter. It's the
- 3:22beat before the beat stops. Do you remember the
- 3:26French paradox? The myth that red wine explains
- 3:28France's low heart disease rates? That's been
- 3:31debunked. Their advantage, Frances, comes from
- 3:34smaller portions, fewer processed foods, and
- 3:36walking the not Bordeaux. Alcohol is a direct
- 3:40cardiac or heart toxin. Ethanol, the active
- 3:43ingredient. Alcohol and acetaldehyde damage heart
- 3:46muscles, scramble calcium signals, they impair
- 3:49mitochondria. They trigger oxidative stress,
- 3:51fibrosis, and metabolic collapse inside the cell
- 3:54of the heart. And chronic exposure to alcohol
- 3:57leads to what we call alcohol cardiomyopathy.
- 4:00That's a dilated, weakened heart that struggles to
- 4:03pump blood. And that can begin at just four drinks
- 4:07a week. Women are hit harder. You know what the
- 4:10cure is? Abstinence. Stop drinking, and the heart
- 4:13has a chance to heal. Keep drinking. It doesn't.
- 4:17You're not drinking for your heart, you're
- 4:19drinking against it. Now, alcohol doesn't just
- 4:23hurt your heart. It fuels cancer. Both ethanol and
- 4:27its metabolite, acetaldehyde, are carcinogens.
- 4:30They damage DNA, block DNA, repair, ignite
- 4:33inflammation and oxidative stress. The spark and
- 4:37the oxygen for cancer. They raise estrogen,
- 4:40heightening breast cancer risk, and act as
- 4:42solvents to other carcinogens, like tobacco smoke.
- 4:45They disrupt folate and retinol metabolism. They
- 4:48blunt the immune surveillance. They reshape the
- 4:50gut microbiome to produce even more acetaldehyde.
- 4:53According to a 2025 journal of the American
- 4:56Medical association review, alcohol is causally
- 4:59linked to seven cancers. Oral cavity, pharynx,
- 5:02larynx, esophagus, liver, colorectum, and the
- 5:06breast. The international association classifies
- 5:09alcoholic beverages as Group 1 carcinogens. That's
- 5:13the same category as asbestos, tobacco, and
- 5:16radiation.
- 5:17Globally, 741,000 new cancer cases in 2020 were
- 5:22due to alcohol. 4% of all cancers in the U.S.
- 5:27alcohol is the third largest preventable cancer.
- 5:305% of new cases, 4% of deaths. Even one drink a
- 5:34day raises the risk of breast cancer by 5 to 15%.
- 5:38Roughly 44,000 United States cases. Each year, the
- 5:42risk starts at zero. There is no safe threshold.
- 5:46Beer Wine, liquor doesn't matter. Ethanol is the
- 5:50common denominator. And for heavy drinkers, five
- 5:53fold higher risk for head and neck cancers than
- 5:56esophageal cancers. Sharply higher for liver and
- 5:58colorectal. Roughly one third of East Asians carry
- 6:02the ALDH2 gene variant that slows acetaldehyde
- 6:06breakdown, dramatically increasing esophageal
- 6:08cancer risk, even with light drinking. And tobacco
- 6:12magnifies alcohol's carcinogenicity. So when
- 6:16someone says they're drinking for their health,
- 6:18remember, every sip raises a cancer risk. The
- 6:21safest amount is zero. And the French, yeah, they
- 6:25don't die of heart disease. A lot of them die of
- 6:28carcinoma. Now, we've all heard the story. People
- 6:31in Icara or Sardinia drink wine every night and
- 6:33live to 100. What's missing? Well, first of all,
- 6:36they drink about three to four ounces. That's not
- 6:39a glass, that's not a pour. That's what we in the
- 6:42United States call a, uh, tasting. And their
- 6:44rustic wines that most of them make run 9 to 11%
- 6:48alcohol, not the 15 to 16% of the Sonoma Zinfandel
- 6:52or Cabernet. And a 3 ounce pour barely reaches a
- 6:56quarter of a cabernet glass. The same pour you get
- 6:59in a tasting or a wine flight, the one that you
- 7:01swirl and sniff and pretend your palate isn't
- 7:04exhausted. The longevity in Sardinia isn't from
- 7:07the wine. It's from everything around it. Walking
- 7:10miles a day, gardening, hills, naps, beans,
- 7:13belonging. The wine is cultural, not clinical. If
- 7:17you want their health, copy their food, copy their
- 7:20movement, copy their sleep, copy their purpose,
- 7:23not their nightly pour. Now, do you want to lose
- 7:27weight? Stop drinking? Forget detox teas or keto
- 7:30cleanses. Nothing sabotages the metabolism faster
- 7:34than alcohol. Two martinis you think are 200
- 7:37calories. After all, an ounce of liquor is 100
- 7:39calories. Have you seen the size of the martini
- 7:41glasses these days? The average martinis that are
- 7:43found in a bar, about 500 calories. So those two
- 7:47martinis, that's about 1,000 calories a day. Keep
- 7:51drinking, the belly will grow, the liver will
- 7:53fatten, the scale will climb in years. As a weight
- 7:57loss surgeon, I saw patients undo their results of
- 8:00surgery with alcohol. Alcohol hijacks the liver,
- 8:04halts fat metabolism, and wrecks every diet plan.
- 8:08Want to wreck your diet drink? Do you want to save
- 8:11it? Stop. What about holiday seasons and choices?
- 8:16Well, I know this is going to be a gradual thing
- 8:18for some people, so let's start the night not with
- 8:21a cocktail, but with sparkling water, hold it like
- 8:24a martini and own the room. And better yet, be the
- 8:27designated driver. Now, if someone offers you a
- 8:30$3,000 glass of screaming eagle, fine. Take a sip
- 8:33for science. Everything else, treat it like pruno
- 8:37brewed in a prison toilet. The same one you prayed
- 8:39to in college. And what about these mocktails?
- 8:42Have you ever noticed their mocktails? More and
- 8:44more showing up. And that's because people aren't
- 8:46drinking as much. But most of those mocktails are
- 8:48overpriced soda and elderberry syrup. Instead, try
- 8:51iced tea soda with cranberry juice. Or get an
- 8:54espresso martini without the vodka. Uh, you don't
- 8:57need alcohol to mark a moment. You need presents.
- 9:00And maybe a good garnish. So the next time someone
- 9:03lectures you about toxins, look at what's in their
- 9:06hand. The vegan warning you about red meat while
- 9:09sipping on a gin and tonic? The guy biohacking his
- 9:12coffee, getting hair plugs, buying a Porsche, and
- 9:14bragging about his longevity hack with that scotch
- 9:17collection that could stock a pub in Edinburgh.
- 9:19They're not hacking their biology, they're
- 9:22pickling it. Because the real anti longevity
- 9:25compound isn't your steak or your cheerios. The
- 9:28anti longevity compound is alcohol. But here's the
- 9:32good. The body wants to heal. Within days of
- 9:36quitting alcohol, blood pressure will drop. Within
- 9:38weeks, inflammation quit, sleep returns, rhythms
- 9:41steadies within months, the heart recovers within
- 9:44a year, cancer risk declines within a decade, and
- 9:47it's almost as if you never drank at all. All you
- 9:51have to do is let your body do its job. Alcohol is
- 9:56woven into celebration, grief, and awkward office
- 10:00parties. But biology? It's a poor partner for
- 10:04aging. Alcohol as a social lubricant doesn't make
- 10:07you socially any better. You're still that awkward
- 10:10person that hasn't dealt with the things you need
- 10:12to deal with in therapy. All it's doing is giving
- 10:15you a fake maladaption to the reality you're in.
- 10:18So as you raise your glass or don't remember, your
- 10:21cells don't take holidays. I'm Dr. Terry Simpson,
- 10:28your chief medical explanationist. This has been
- 10:30forku, where we make sense of longevity, bust
- 10:33myths, and teach you a little bit about food and

- 10:49longevity cruise for the summer. Of 2026 with
- 10:51world class scientists, physicians and a food
- 10:53network chef. Real longevity without the woo. The

- 11:00visit these longevity centers for, you're going to
- 11:02get for the price of a cruise. Sailing the
- 11:04Mediterranean, eating incredible food, and
- 11:06actually enjoying life. Details coming soon. Have
- 11:10a good week, everybody, and happy holidays. Let's
- 11:13see you next year. Keep the alcohol out. Be the
- 11:16hero. Be the designated driver. Hey, Evo. You
- 11:29know, when I stopped drinking, my biologic age was
- 11:32in the 70s. It's been climbing down ever since.
- 11:35Meanwhile, my real age is creeping up. Damn. I
- 11:44feel attacked. Simpson attacked. I say.
