- 0:32This is Madeline Ashley with the Becker CFO
- 0:35and revenue cycle podcast, and I'm excited to
- 0:37be joined today by Robert Chesnutt, senior vice
- 0:40president and CFO of LMH Health. Robert, thanks
- 0:43so much for joining me.
- 0:45It's great being here. Thanks.
- 0:48So excited to have you on today. And
- 0:50before we kick things off, for, for listeners,
- 0:53do you mind just sharing a little bit
- 0:54about yourself and and l
- 0:58Sure. So I,
- 1:00I've been in this role for a little
- 1:02less than two years. Prior to that, I
- 1:04was on the board of trustees for eleven
- 1:06years, so I have kind of an interesting
- 1:08perspective.
- 1:09LMH Health is here in Lawrence, Kansas. We
- 1:12sit about forty five minutes outside the greater
- 1:14Kansas City area.
- 1:16We are the,
- 1:18we have our university here, the University of
- 1:21Kansas, so we have 26,000
- 1:23undergraduates here.
- 1:24Our community is about a 100,000,
- 1:27and we have a service area
- 1:29that's about a 125,000.
- 1:31We're a 174
- 1:32bed hospital.
- 1:34We do employ
- 1:36the majority of our,
- 1:38physicians in our our
- 1:40service area.
- 1:42So we have all of the service lines,
- 1:44orthopedics,
- 1:45cardiology,
- 1:46urology,
- 1:48and so forth, and then, obviously, a very
- 1:50large primary care,
- 1:52division. So,
- 1:53it's pretty much a health system,
- 1:56not just a
- 1:57hospital.
- 1:59And thank you so much for sharing. And,
- 2:00again, as I mentioned, so excited to have
- 2:02you back on the podcast. I believe you
- 2:04and I connected,
- 2:06in late July, early August for podcast. So
- 2:08excited again to to dive deeper in here
- 2:10today and Mhmm. Hear how things are going.
- 2:13And as you know, the the industry continues
- 2:16to change. Probably even in the last few
- 2:17months since we connected last, we've seen so
- 2:19many changes, ebbs and flows.
- 2:22So could you share some of the broader
- 2:23challenges you're closely following right now in the
- 2:26industry
- 2:27and why you're keeping an eye on these
- 2:28from a finance perspective?
- 2:30Sure. I think immediately,
- 2:32we're looking at what the impact's,
- 2:36going to be on our market with
- 2:38the ACA subsidies.
- 2:40It's hard to know exactly where that's all
- 2:42going to end up. We do have some
- 2:44exchange,
- 2:46patients in our in our market,
- 2:48and so we really won't know until,
- 2:52their first encounters in 2026,
- 2:55if they've changed insurance, if if they now
- 2:58are going
- 2:59to not have any insurance. So I think
- 3:01that's probably the largest one. We do have
- 3:03a pretty robust
- 3:05financial counseling group here,
- 3:07at the hospital. So it's really,
- 3:10we try to be proactive in that, but
- 3:12part of it's just that's a big unknown
- 3:14right now. So so that's a big trend.
- 3:16I think the second trend we're looking at
- 3:19is trying to gauge
- 3:21what our inpatient census is gonna be over
- 3:23the winter.
- 3:24Typically,
- 3:26as you know,
- 3:27I'm sure it's true at most hospitals that
- 3:30you have,
- 3:31your your highest census is probably starting now
- 3:34through, let's say, February or March.
- 3:37Our census has been up,
- 3:39during the even the summer, so we're kind
- 3:42of wondering
- 3:43what is that going to mean our census
- 3:45is gonna be even that much higher in
- 3:46the winter or not? So that kinda impacts
- 3:49our staffing models and what we have to
- 3:51open and and how we're doing that. So
- 3:54that's kind of on the ground, a a
- 3:56real,
- 3:57macro
- 3:58trend that we're trying to figure out.
- 4:01So those are two things right now that
- 4:03we're really focused on.
- 4:05Yeah. And you you know, I wanna circle
- 4:06back because you touched a little bit on
- 4:08just the the financial counseling group that you
- 4:10have right now and, you know, everything around,
- 4:12like, the this ACA subsidies and just question
- 4:15marks and unknowns.
- 4:17Could you share a little bit about what
- 4:18those group meetings look like? And maybe for
- 4:21some of our listeners who would who would
- 4:23be interested to to figure out maybe a
- 4:25solid plan right now amid all this unknown.
- 4:28Well, I think that, we're probably slightly unique
- 4:31simply because as a sole community hospital, we're
- 4:34the only game in town. So we feel
- 4:36a real obligation to provide that service to,
- 4:40you know, all of our patients. And Yeah.
- 4:43Whether it be
- 4:44navigating Medicare Advantage plans,
- 4:47whether it be navigating, as you know, on
- 4:49the on this back end of 2026,
- 4:52I think that we will have
- 4:53a lot of of,
- 4:55involvement
- 4:56in Medicaid enrollment
- 4:58and the fact that they're gonna have to,
- 5:01Medicaid patients will have to qualify twice a
- 5:03year.
- 5:04You know, there's a lot there. From from
- 5:06our standpoint, we just feel the obligation
- 5:09to be,
- 5:11I wouldn't say the one stop shop, but
- 5:13certainly the primary source of of information for
- 5:17them to figure out how to navigate this
- 5:19this whole,
- 5:21new world that we're in as far as
- 5:23different,
- 5:25different changes in legislation,
- 5:27because it's just it's it it is,
- 5:30for us unique.
- 5:32I I think that, you know, for for
- 5:34other systems, if you're in larger metro areas,
- 5:37maybe it's a little bit different. But,
- 5:40overall, there are patients, and our patients are
- 5:43critical,
- 5:44to to to understand,
- 5:47because the biggest concern I think we have
- 5:49as a system is to make sure that
- 5:51they're able to receive care.
- 5:53And so,
- 5:55it it requires
- 5:56more, in my opinion, more productivity
- 5:59to be more outreach than it is just
- 6:02kinda waiting to see what happens.
- 6:04Yeah. And, you know, as as a a
- 6:07CFO, as a leader of the finance team,
- 6:10how are you ensuring to keep your team,
- 6:11you know, resilient and engaged right now? Maybe
- 6:14the the secret sauce to success
- 6:16as you guys, you know, kind of prepare
- 6:18and plan.
- 6:19Right. Part of it's education. I think,
- 6:22you know, I have
- 6:24I'm I'm very,
- 6:26blessed, I would think, to have a great
- 6:28team of directors that work for me that,
- 6:32actually have more
- 6:34insight
- 6:35into all the nuance of what's happening, whether
- 6:37it be in,
- 6:39payer negotiate you know, payers and what's happening
- 6:42in Medicare and what's happening with Medicaid and
- 6:44what's happening in coding,
- 6:46what's happening in procurement,
- 6:48so on.
- 6:49So I really encourage that. They they need
- 6:52to be the subject matter experts.
- 6:54I think secondly
- 6:55is just keeping them focused on what is
- 6:58our purpose. Our purpose is to provide
- 7:01high, you know, high quality patient care.
- 7:04It's easy to get kind of trapped in,
- 7:07the numbers
- 7:09and to to kind of of,
- 7:12become sort of siloed into
- 7:14the issues that you're having
- 7:16as a department
- 7:18as it relates to the rest of the
- 7:20ecosystem in health care. But we all have
- 7:22to kinda take a step back and realize
- 7:24why are we doing what we're doing.
- 7:26And,
- 7:27that, I think, provides some motivation for them
- 7:30to say, ultimately, I'm here to make sure
- 7:33that patients receive the best care they can.
- 7:36And as we're looking to the new year,
- 7:38I can't believe we're almost in 2026,
- 7:41but, you know, where's your mind at in
- 7:43terms of planning for next year? Maybe anything
- 7:46that you're keeping an eye on trends wise,
- 7:49that we'll see next year.
- 7:51Well, I think that one of the things
- 7:53for us is that we we are we're
- 7:56under a fairly,
- 7:58expansive,
- 7:59capital
- 8:01expansion right now. We've finished our cancer center
- 8:05this year, and we are opening up,
- 8:08a second,
- 8:10cath lab sometime in late twenty twenty six,
- 8:13early twenty seven,
- 8:15heart center.
- 8:16So we do have some pretty,
- 8:19robust expansion plans. So that's that there's a
- 8:22lot of planning behind that, obviously,
- 8:26obviously. And it does create some short term
- 8:28transitional issues because we have to, for instance,
- 8:31relocate our cardiologist for a period of time,
- 8:34and,
- 8:35we're moving a lot of things around. So
- 8:37so that logistics,
- 8:39those logistics are always challenging.
- 8:41So that's one thing for us.
- 8:43I think what we're probably looking at at
- 8:46a macro level is to make sure that
- 8:49we understand what the landscape is with
- 8:52our our our payer mix and and and
- 8:55just our
- 8:56regional market. Because even though we're a sole
- 8:59community hospital,
- 9:00we do have
- 9:02in the Kansas City market, we have
- 9:05HCA
- 9:06hospital there. We have a big AVID presence
- 9:09there. We have other large large groups there.
- 9:11So we always need to understand,
- 9:13you know, kinda what's going on and making
- 9:16sure that we don't see the kinda,
- 9:19a lot of out migration from this market.
- 9:22So so those are a couple of things
- 9:24that we have the,
- 9:25that we're paying close attention attention to right
- 9:28now.
- 9:30And, you know, with your experience and background
- 9:32in
- 9:33in health care finance,
- 9:35what is or or maybe what are some
- 9:37of the the biggest skills that CFOs need
- 9:40to hone in on right now just amid
- 9:41all these changes?
- 9:43Well,
- 9:45I think to to really understand the the
- 9:47bigger picture, and what I mean by that
- 9:50is that,
- 9:52health care is a very complex ecosystem, and
- 9:55to try to understand
- 9:56all the moving parts is challenging. It takes
- 9:59it takes a village. It really does. It
- 10:01takes all of my colleagues that work with
- 10:03me,
- 10:04in the finance group. It takes our operations
- 10:07directors. It takes our physicians. It's you know,
- 10:10it it is a
- 10:12it is a
- 10:13big a a a big ecosystem to manage.
- 10:16So I think that what you evolve to
- 10:19is you're really realizing that you're
- 10:23well outside of just the financial picture, but
- 10:25just,
- 10:26a lot of things that are happening
- 10:28that affect the financials, but trying to be
- 10:30out in front of it.
- 10:32I think in particular,
- 10:34we're probably more focused right now to make
- 10:36sure how do we manage our our labor.
- 10:40Labor challenges are are fairly large,
- 10:43probably the same issues that a lot of
- 10:45a lot of different systems are experiencing with
- 10:48the shortage of different,
- 10:50different skill sets being either nursing,
- 10:54medical assistants, phlebotomist,
- 10:56all of that. So one of the things
- 10:58we're also involved in is to try to
- 11:01develop some programs, some training programs
- 11:03for some of those positions because
- 11:06I think that, with the shortage of those
- 11:09skills in the market, you have to make
- 11:11up the difference. You're you're gonna have to
- 11:13grow your own.
- 11:15You know, that's a term that we use
- 11:16around here is how do we,
- 11:18give opportunities
- 11:20not only for,
- 11:21colleagues that we have already in our system,
- 11:24but people in our community to really get
- 11:26involved in health care. Because I think if
- 11:29you look at the at the long term,
- 11:31you recognize the fact that there there's going
- 11:34to be less people,
- 11:35and the demand is certainly going to continue
- 11:38to increase. So so we have to find
- 11:40different ways to find the the quality people
- 11:43that work for our, organization.
- 11:46Yeah. And I've even, you know, seen
- 11:49and reported on here at Becker's some, you
- 11:51know, systems. Some hospitals are focusing, you know,
- 11:54on even high school, middle school students, you
- 11:56know, just introducing them to the world of
- 11:58health care saying, hey. You know, come down
- 12:00the line, come work for us.
- 12:02Yeah. We have a we have a a
- 12:05group here in Lawrence,
- 12:07a technical school called Peasley Tech. It's named
- 12:10after
- 12:11a
- 12:12a
- 12:12real,
- 12:13a gentleman that was really involved in the
- 12:15community.
- 12:16But,
- 12:17they probably touch,
- 12:19four or 500,
- 12:21high school students every week. And you're absolutely
- 12:24right that,
- 12:25getting them interested early and maybe some opportunities,
- 12:28because I think the big thing is to
- 12:30to make them see that
- 12:32there are other pathways than just necessarily a
- 12:35college education to be really successful.
- 12:38And,
- 12:39I I think that that we we have
- 12:41to emphasize that because that is the way
- 12:43we're going to have a robust workforce.
- 12:47Right. And it's such a needed and important
- 12:49profession. So Mhmm.
- 12:51Well, Robert, I really appreciate you hopping on
- 12:53this call, and it was so great to
- 12:55to be able to discuss with you, you
- 12:56know, again, some of the industry uncertainties we're
- 12:58seeing and and how LME CHA is going
- 13:01forward tackling it. So thank you so much
- 13:03for for being on again. It's been such
- 13:05a pleasure chatting and excited to connect with
- 13:07you again in in 2026.
- 13:09Okay. Appreciate it.
