- 0:00This is Madeline with the Becker CFO and
- 0:01Revenue Cycle podcast, and I'm thrilled to be
- 0:04joined today by Todd Roberts, CFO of Cheshire
- 0:06Medical Center, a member of Dartmouth Health. Todd,
- 0:09thanks for joining me. Thank you for having
- 0:12me. So, Todd, I'm so excited to chat
- 0:14with you today. But before we begin, can
- 0:15you share a little bit about yourself with
- 0:17our listeners and more about Cheshire Medical Center?
- 0:21Sure.
- 0:22So I have been in health care for
- 0:23a little over twelve years now.
- 0:25And I was at a a Alispect A
- 0:28Memorial Hospital for just over eleven years.
- 0:31And Alispect A Memorial Hospital is another, Dartmouth
- 0:34Health System member, and I recently joined, Cheshire
- 0:37back in April. So, a little less than
- 0:40a year, in my current position as a
- 0:42chief financial officer.
- 0:44Chief Financial Officer. Is, located in, King, New
- 0:47Hampshire and, has a a a pretty extensive
- 0:49history, within the community here.
- 0:52Wonderful. Thanks so much for sharing that, Todd.
- 0:54So let's dive right in. You've been in
- 0:56the industry for a while now as you
- 0:58mentioned at the beginning of the podcast. So
- 1:01what are some, from the financial seat, strategic
- 1:03priorities that are taking up the most of
- 1:05your attention right now?
- 1:07Yeah. So,
- 1:08we recently went through a strategic partnership with
- 1:10with the health system around our three forty
- 1:12b program.
- 1:13And this was a a a project that
- 1:15was implemented in a really short period of
- 1:18time. Took a lot of coordination and efforts,
- 1:21from system members as well as the the
- 1:23folks from, Chester Medical Center. There was a
- 1:26financial tail on this project,
- 1:28and we're continuing to to work through that,
- 1:31from a transactional reporting perspective.
- 1:33We knew that we would face some challenges,
- 1:35but did not anticipate it to take as
- 1:37long as it has.
- 1:39This issue that we're we're struggling with has
- 1:41made data and reporting very challenging and with
- 1:44such a reliance on our accurately
- 1:46accurate and timely data,
- 1:48it's taken a lot of my team's attention.
- 1:50And one of the other strategic priorities that's,
- 1:52taken a lot of my attention is strengthening
- 1:54obviously, strengthening our financial performance.
- 1:57Being in a rural region,
- 1:59with an aging demographic, extremely important for us
- 2:02to, look for opportunities and efficiencies,
- 2:04across the organization.
- 2:07We recently kicked off an ops excellence committee,
- 2:10which is identifying some areas that, across multiple
- 2:13disciplines that will help our overall financial performance,
- 2:16you know, and includes efficiencies as well as
- 2:18that potential areas for growth.
- 2:21It's extremely focused work group, and, you know,
- 2:23I'm really excited to be a part of
- 2:25it, because it gets me involved with our
- 2:27frontline operations
- 2:28as well as working across multiple areas, within
- 2:31the organization.
- 2:32And, you know, one critical,
- 2:34component that, when we
- 2:36start discussing strategic priorities is the importance of
- 2:39being being a part of the health system.
- 2:40We don't wanna get, out of line or
- 2:42or off on our own when when looking
- 2:44at these priorities and and keeping the system
- 2:46members involved is critical.
- 2:49Yeah. And I I kinda wanna circle back
- 2:51real quick. So at the beginning of this,
- 2:52you mentioned one of the priorities is, you
- 2:54know, obviously, some of the challenges you're facing
- 2:56with the three forty b program. Could you
- 2:58dive a little deeper into some of those
- 2:59challenges there and and some solutions that have
- 3:02maybe been working for you from this perspective?
- 3:05Yeah. We,
- 3:06we're very
- 3:07for the past couple of years, we've been
- 3:09very close to, losing our our three forty
- 3:11b designation. So,
- 3:13you know, really thinking strategically in how we
- 3:16can,
- 3:17help ourselves down the road,
- 3:20and and not have to worry about that.
- 3:22Took a lot of, efforts and
- 3:25coordination with the system and coming up with
- 3:28some real
- 3:29interesting ideas and and
- 3:31implementing those ideas is is what's,
- 3:34been very challenging.
- 3:36This kinda segues into my next question for
- 3:39you. So, obviously, when we think about, like,
- 3:40teamwork, communication,
- 3:42what would you say your approach is to
- 3:44building financial resilience especially when there's so many
- 3:47unknowns and there can be so much volatility
- 3:50in in the industry?
- 3:52Yeah. So, you know, as as as I've
- 3:55mentioned, being part of Dartmouth Health, you know,
- 3:57I rely on our internal experts when and
- 3:59where. You know, being a larger health system,
- 4:02it is, we've got specific expertise across the
- 4:05organization,
- 4:07but not one person that knows
- 4:09or is an expert in in every area.
- 4:11So any, you know, potential changes, regulatory changes,
- 4:15we need to be obviously focused on this
- 4:17at all times
- 4:18And partnering with our specialists,
- 4:21across the system,
- 4:23is is critical.
- 4:24You know, without knowing what we might be
- 4:26facing, we'd be behind before, anything was ever
- 4:28implemented. So
- 4:30utilizing the the appropriate resources obviously helps us
- 4:33get ahead and, you know, be strategic on
- 4:35this.
- 4:36And, you know, as a finance leader, I
- 4:38love to ask CFOs this question because it
- 4:40does seem like, you know, the traditional CFO
- 4:43role of, you know, numbers crunching behind the
- 4:45desk has changed.
- 4:47So how are you working to partner differently
- 4:49with clinical and operational colleagues to drive sustainable
- 4:52performance?
- 4:54Yeah. So,
- 4:56for me, being somewhat still new to the
- 4:58organization, you know, less than a year, it's
- 5:00it's building relationship.
- 5:01You know, being able to get to know
- 5:03current operations and the way things are being
- 5:05done is is really important to me as
- 5:07opposed to just coming in and and assuming
- 5:09that I know. Each organization is different. And,
- 5:13you know, I think as a CFO showing
- 5:15interest in how things are currently being done,
- 5:18helps to build relationships.
- 5:20I think it's much easier working together and
- 5:21making changes as opposed to, you know, just
- 5:24coming in and and demanding changes.
- 5:26Yeah. Those partnerships are so critical.
- 5:29And, you know, again, to segue, what would
- 5:31you say some major decisions today what are
- 5:34some of the major decisions that would require
- 5:36CFOs to even think beyond that traditional financial
- 5:39management perspective and act maybe even as like
- 5:42a enterprise strategist?
- 5:45Yeah. I I think it's, have an important
- 5:48relationship with with our CEO,
- 5:50you know, and and being open,
- 5:53being engaged. And, you know, as you said,
- 5:55it's not just,
- 5:57as a CFO, just about crunching numbers anymore.
- 5:59It's being strategic
- 6:01and really buying into, the, you know, the
- 6:04overall mission and and vision of of the
- 6:06organization, which, you know, we just went through
- 6:09a refresh on that with our board of
- 6:10trustees approving it. And, you know, it it
- 6:13it really aligns with,
- 6:15you know, what I, I have looked for,
- 6:18in in a career. And, you know, I'm
- 6:20excited to
- 6:21partner with with our executive team as well
- 6:24as our board to to make sure that
- 6:26we see that through.
- 6:27Looking to the future now, maybe even three
- 6:30to five years out, what would you say
- 6:31are some capabilities
- 6:33that will separate
- 6:34thriving organizations
- 6:36from maybe some of those that will continue
- 6:37to struggle?
- 6:39Yeah. We, we recently saw some real eye
- 6:41opening data, regarding our clinical care staffing, the
- 6:45expected shortfall in the future,
- 6:48specifically around the physicians and nurses. I mean,
- 6:50we're already seeing this, but, you know, the
- 6:52the the further outlook doesn't doesn't look like
- 6:54it's getting any better. So being in a
- 6:56rural community, it's it's already challenging to recruit
- 6:59for our area. Yeah. But being able to
- 7:02partner or think strategically around housing or other
- 7:04other factors to
- 7:06help us recruit young talented staff is is
- 7:09gonna be key for us to to continue
- 7:11to strive and, you know, kind of separate
- 7:13ourselves.
- 7:15The other critical component is, you know, we're
- 7:18we're dealing with a multi generational workforce,
- 7:21you know, when there's another generation that, supposed
- 7:23to be joining us shortly.
- 7:25So being able to adapt to those generational
- 7:28differences and offer, you know, something that each
- 7:31of those generations is looking for will will
- 7:33also be key to closing that, you know,
- 7:34staffing gaps as well as being able to
- 7:37continue to strive.
- 7:39Yeah. Todd, you know, as you mentioned, you've
- 7:41been in your role here for around a
- 7:44year. So anything that you're looking forward to?
- 7:46Anything exciting down the pipeline for 2026?
- 7:51I think, you know, as I I mentioned,
- 7:53you know, health care always has its challenges,
- 7:56and it seems like, you know, every day
- 7:58we come to the office, something new pops
- 8:00up. So,
- 8:01you know, I I think that's exciting. I
- 8:04like,
- 8:05we like challenges.
- 8:06And, you know, within health care, I think
- 8:08that, there's there's gonna be plenty of those,
- 8:12moving forward. So,
- 8:14you know, I I I look to the
- 8:16next day and kind of, wondering what I'm
- 8:18gonna get into. And, you know, for 2026,
- 8:21you know, we're making some changes,
- 8:23from a payer perspective,
- 8:25you know, making the, making making some shifts
- 8:28to incentive based models as well as value
- 8:30based care models from a reimbursement perspective. And,
- 8:33you know, I think that brings an opportunity
- 8:35for us to,
- 8:37really partner within the organization and and and
- 8:39try to get ahead of things.
- 8:41Wonderful. Well, Todd, it has been such a
- 8:43pleasure getting to connect with you and hear
- 8:45about, you know, everything that's on your mind
- 8:47right now. Again, such a interesting time we
- 8:50live in and especially with health care. So
- 8:52appreciate your sharing your thoughts and insights and
- 8:54excited to connect again down the line. Sounds
- 8:57good. Thank you so much for having me.
- 8:59Thank you.
