- 0:34Dot com,
- 0:35top 231
- 0:37page Pdf study guide.
- 0:40Alright. So number 36
- 0:43is
- 0:44pam. Brand name in this medication is Ada.
- 0:48This is classified as a benz,
- 0:52kind of a broader class, I guess you
- 0:53could consider it an an analytic.
- 0:56Primary use is as you could expect,
- 1:00use
- 1:01short term for anxiety, ideally, short term. I've
- 1:04definitely seen patients on it chronically.
- 1:07Other
- 1:09indications, you know, alcohol withdrawal,
- 1:12seizures
- 1:13potentially.
- 1:14You do see it in hospice patients a
- 1:16fair amount for
- 1:17you know, agitation and anxiety,
- 1:21rare conditions you may see it utilized for
- 1:24serotonin syndrome and neuro malignant syndrome.
- 1:28Those are
- 1:29kinda 2 drug induced states where we could
- 1:31have some increase in
- 1:34anxiety and agitation there as well. So
- 1:37those are some of the the common uses
- 1:38or some of the the
- 1:40situations where you may see this this medication
- 1:42utilized.
- 1:441 nice thing with
- 1:45maybe compared to some of the other
- 1:48benz
- 1:49is it does have a ton of different
- 1:51dosage forms.
- 1:53So we've got,
- 1:55tablet
- 1:56formulations. We've got oral liquid
- 1:58formulations. We've got injectable
- 2:00formulations.
- 2:02So that can certainly be
- 2:04advantageous depending upon what we're trying to do
- 2:07with that medication.
- 2:10Adverse drug reactions.
- 2:12So if you remember alcohol in a pill,
- 2:14that's the easiest way to remember the majority
- 2:16of adverse effect. With.
- 2:20So it is a depress type medication, so
- 2:23it's gonna cause some s edema
- 2:26can increase the risk for confusion. This is
- 2:28more problematic in our elderly patient population.
- 2:33I can increase falls risk, which is obvious
- 2:35see a a challenge in our older patient
- 2:37population as well.
- 2:39Slur speech, another example of an adverse effect.
- 2:42That you may see as doses escalate with
- 2:46pam.
- 2:48As far as drug actions, go, probably the
- 2:51biggest thing is other sedative type medications on
- 2:54top of using,
- 2:58So I think of,
- 2:59you know, first generation anti anti,
- 3:02t,
- 3:04you know, older, skeletal muscle relax and cabo
- 3:07pre ga,
- 3:09other
- 3:10medications and 1 that has a box skeletal
- 3:13warning as far as drug interactions go is
- 3:15with opioids.
- 3:16I can increase the risk for respiratory depression.
- 3:20So that's definitely something to monitor for closely
- 3:23if you've got somebody on
- 3:25in combination
- 3:27with an opioid
- 3:29medication.
- 3:30Last thing I wanted to mention, maybe a
- 3:32couple things here. It is a controlled substance,
- 3:35so it's a schedule for... Controlled substance, so
- 3:37there is the risk of addiction dependence,
- 3:40just why we try to minimize its use
- 3:43to short term if possible, controlled
- 3:46and I also wanted to mention there were
- 3:47some new
- 3:49draft guidelines out, 20 24 draft guidelines on
- 3:52benz
- 3:53tapering.
- 3:54And I did post about that at med
- 3:5701:01. So if Google search,
- 3:59med ed 01:01 20 24 Benz,
- 4:02tapering,
- 4:03you'll see kind of those
- 4:05draft guidelines,
- 4:07any information on that if you're looking for,
- 4:09more detail, there.
- 4:11Alright. Moving on. The next
- 4:13medication, number 37
- 4:15is fin p. It's And brand name of
- 4:18this medication is iridium.
- 4:20This medication is a urinary anal.
- 4:25It helps with painful
- 4:26urination.
- 4:28Most commonly, I've I've seen it used in
- 4:31conjunction for a couple of days.
- 4:33Particularly,
- 4:35in patients who've maybe then diagnosed with the
- 4:37Uti, and they've got significant painful
- 4:39urination,
- 4:40you may see fin as purity kinda added
- 4:43on top of an antibiotic.
- 4:46Now I will caution you, be careful with
- 4:48patients out in
- 4:50the community. So if you're in a community
- 4:53pharmacy, for example,
- 4:54or if somebody's asking you for you know,
- 4:57ambulatory care type
- 4:59recommendations saying, hey, I've got some painful urination?
- 5:02Is there a medication used to treat this?
- 5:04You wanna make sure that those patients have
- 5:07been assessed,
- 5:08whether it be for urinary an tract infection
- 5:11or some other medical
- 5:13neurological
- 5:14condition.
- 5:15You definitely wanna
- 5:17get those those patients step assess because,
- 5:21painful urination definitely isn't
- 5:23an incredibly common thing for most patients. So
- 5:26usually, there's something else going on with it.
- 5:28So again,
- 5:30if you see somebody, you know, trying to
- 5:32buy this medication, get this medication,
- 5:35you definitely might wanna,
- 5:37inquire a little bit, ask a few questions.
- 5:39And make sure they've been assessed and get
- 5:41a, you know, kind of a background on
- 5:43their symptoms and what's been going on
- 5:45just to see if we need to dig
- 5:47a little bit further
- 5:49into that diagnoses
- 5:50and, have them tested and assessed for other
- 5:52things. So,
- 5:54important thing, patient education
- 5:56thing to remember there with ph.
- 6:00Adverse effect
- 6:02profile,
- 6:031 unique thing with the medication is it
- 6:06can change urine color,
- 6:09and that color is a red or orange
- 6:12type color.
- 6:14That is
- 6:15definitely probably 1 of the more common unique
- 6:18adverse effects. Typically, it doesn't
- 6:20result in anything, but obviously, it can be
- 6:23pretty
- 6:24unnerving to the patient if they see that.
- 6:27Other adverse drug reactions usually they're pretty rare.
- 6:30You may see you know, on rare radiation
- 6:32skin reaction or maybe some Gi upset.
- 6:36Kidney function is important to take a peek
- 6:38out.
- 6:39If you've got a patient
- 6:41whose
- 6:42credit clearances is less than 50 emails per
- 6:44minute, it's recommended to a void,
- 6:47fin,
- 6:48there's rare situations, case reports
- 6:51associated with acute kidney injury,
- 6:54certain types of anemia.
- 6:57So definitely some some potential issues there.
- 7:01In using ph if patients have poor renal
- 7:04function.
- 7:05Alright. Moving on, the next drug we've got
- 7:08is hydro.
- 7:09Quinn. Brand name of this medication is Plaque
- 7:12elm.
- 7:13This was definitely a,
- 7:15hot topic for debate
- 7:17as far as whether it was effective for
- 7:19Covid or not, several years ago and
- 7:23the whole Covid thing was, in full swing.
- 7:27But it's a medication that's primarily used for
- 7:31rh
- 7:33disorders.
- 7:33It is classified as a dm, that's a
- 7:36disease modifying
- 7:38anti
- 7:39drug.
- 7:40It is also classified
- 7:42has an anti malaria drug. It has some
- 7:44anti
- 7:46malaria
- 7:46activity.
- 7:48Primary use I and practice on a chronic
- 7:50basis. You've rheumatoid arthritis,
- 7:53and then,
- 7:54Lupus is probably the other 1 that I
- 7:56see it
- 7:57utilized most often.
- 8:00Alright. Adverse drug reactions with hydro.
- 8:04So
- 8:051 of the ones that's probably the most
- 8:06highly tested on, and definitely 1 that I've
- 8:09seen come up and practice quite a bit
- 8:11is hydro
- 8:13can cause some,
- 8:16toxicity.
- 8:17More specifically retinopathy.
- 8:20And this is a
- 8:21concentration dependent adverse effect. So obviously, higher higher
- 8:24the dose more frequently, we're giving it.
- 8:27The more likely
- 8:28this eye adverse effect
- 8:31is going to occur. So
- 8:33definitely making sure that patients are having
- 8:37routine eye exams and monitoring their eyes.
- 8:41And definitely should report any issues
- 8:43if they exert to experience some,
- 8:46unique changes in their vision, for example.
- 8:50There are some other rare things associated with
- 8:53hydro,
- 8:55hem anemia,
- 8:57particularly in patients with
- 9:00g6 6 pd deficiency.
- 9:03Hyper sensitive rack have been reported,
- 9:07Q prolong prohibition has been reported as well.
- 9:10So again, kinda some unique things with hydro
- 9:13when there is a bigger list as well.
- 9:16But again, probably the most important thing, if
- 9:18you're taking an exam,
- 9:20and really unique thing,
- 9:22is that risk for retinopathy and eye toxicity.
- 9:27Alright. Next
- 9:29medication is lid
- 9:31and we are specifically focusing on,
- 9:35the topical
- 9:36patch, so that would be lighted der would
- 9:39be the brand name there.
- 9:41This is a local an aesthetic.
- 9:44So the patch is essentially supposed to be
- 9:47placed.
- 9:49On the location where the
- 9:51pain is.
- 9:53Most common type of pain
- 9:56that lid patch is going to be beneficial
- 9:59for as neuro type pain
- 10:02and post
- 10:03neurology. Those are probably the 2 most common
- 10:06situations
- 10:07that I see that lid patch used for.
- 10:12An important thing to remember with the administration
- 10:14of those
- 10:15patches. So, first and foremost, they they act
- 10:19locally,
- 10:20but you can get absorption over I'm into
- 10:23the
- 10:25systemic circulation.
- 10:27So
- 10:28an important patient education point is for my
- 10:30patients that it should be
- 10:33kept on no longer than 12 hours. And
- 10:35that's really to try to reduce
- 10:37that accumulate.
- 10:39Now when we compare a lighter king patch
- 10:42compared to a fentanyl patch, for example,
- 10:45fentanyl patches actually have systemic absorption
- 10:49and have
- 10:51systemic and central nervous system type effects.
- 10:55Fentanyl is an an opioid of course there.
- 10:58So that's definitely different.
- 11:01A fentanyl patch is not meant to be
- 11:03used in a local area and placed on
- 11:05a local location.
- 11:07If your pain is on your elbow, you
- 11:09aren't supposed to put a fentanyl patch on
- 11:11your elbow,
- 11:13that's not the way it works. It requires
- 11:15systemic absorption and getting adequate concentrations into the
- 11:18bloodstream. Versus lid patch, that's definitely more of
- 11:22a localized type effect. So if you've got,
- 11:24you know, low back pain, that type of
- 11:27thing,
- 11:28you'll see that lid
- 11:30patch
- 11:31placed on the side of action. So good
- 11:34little
- 11:35differentiator there
- 11:36with that lid patch.
- 11:39Now, I mentioned the 12 hours on maximum
- 11:42in a 24 hour period.
- 11:46When and if
- 11:48lid can accumulates in the bloodstream.
- 11:51There is risk for
- 11:53cardiac effects and changing
- 11:55the way the heart heart works and can
- 11:57cause arrhythmias and things of of that nature.
- 11:59And I think
- 12:01if you've got any experience with, you know,
- 12:03acute care or emergency room care, that type
- 12:05of thing.
- 12:07You'll know that we use or can use
- 12:09lid.
- 12:10In the Ac
- 12:12algorithm as
- 12:14medication used to treat certain types of arrhythmias.
- 12:17So
- 12:18that's the deal. That's the issue with
- 12:21accumulation of lid. So
- 12:23good 1 to remember there potentially.
- 12:27Alright. And our last
- 12:29medication of the day, number 40
- 12:33is going to be d.
- 12:36Brand name in this medication is cat.
- 12:39I'm focusing more so here on the oral
- 12:43formulation of this medication.
- 12:45It is an nsa. That's a non steroid
- 12:48anti inflammatory drug.
- 12:50Obviously, good at reducing pain and inflammation.
- 12:55But we've got plenty of adverse drug reactions
- 12:58with Nsa, in general,
- 13:01this is
- 13:02worsened as we get older, some of those
- 13:04adverse effects,
- 13:06can occur at a higher frequency or be
- 13:08more severe?
- 13:10So first 1 I wanna mention, of course,
- 13:12with Nsa,
- 13:14is Gi bleed.
- 13:16So it can increase the risk for Gi
- 13:18bleed, gi,
- 13:20and that is definitely a a big issue,
- 13:23with Nsa in general there.
- 13:26There is a boxed warning for Gi bleed
- 13:28as well, So I did wanna mention that
- 13:30as well.
- 13:31Edema, swelling can happen,
- 13:33so an increased risk for heart failure ex
- 13:37exaggeration, for example,
- 13:40the
- 13:41medication can cause some
- 13:43potential sodium and and fluid retention, so that
- 13:45can be a a problematic thing.
- 13:48Renal impairment. The way
- 13:51nsa work on the kidney.
- 13:53They can
- 13:54potentially,
- 13:55kinda impair the function of the kidney
- 13:58and, increase the risk acute renal failure. This
- 14:01is typically exacerbated
- 14:04in patients who are older,
- 14:06at, you know, obviously, at risk for renal
- 14:08impairment for and on other medications that may
- 14:12increase that risk for renal impairment.
- 14:14I think of, you know, diuretics, for example,
- 14:17can increase that risk.
- 14:19Ace inhibitors and Ar can increase that risk,
- 14:23as well as, you know, Iv medications like
- 14:27I amino,
- 14:28you know, gen mice, and to mice. They
- 14:30can increase the risk for renal problems as
- 14:33well.
- 14:35And then last, I did wanna mention
- 14:38there is warnings on cardiovascular
- 14:40risks with
- 14:42Nsa, and
- 14:44d cloth act being nsa falls certainly in
- 14:46that category.
- 14:47So that boxed warning is for an increased
- 14:50risk of t product events.
- 14:54And then lastly, finishing up on drug interactions,
- 14:58so we've gotta think about
- 15:00medications that can increase the risk
- 15:04for Gi bleed. So cor,
- 15:06they can kinda have an additive effect.
- 15:09So if we take prednisone own you know,
- 15:11with d,
- 15:13that can increase the risk for Gi bleed,
- 15:16And then, of course, any medications that thin
- 15:18the b. So any type of anti coa
- 15:22medication, you know, picks a band
- 15:24war, for example,
- 15:26any type of anti platelet medication.
- 15:28Cl,
- 15:29p,
- 15:31Aspirin. These are all medications that are gonna
- 15:33increase that risk of Gi bleed when used
- 15:37in combination with d.
- 15:40Alright Well, I think that's gonna wrap up
- 15:42the podcast for today. Do me a huge
