- 0:04>> Dr. Terry Simpson: As we age, our mitochondria get sloppy. They break
- 0:07down, pile up, and gum up the machinery that keeps
- 0:10our cells running smoothly. And for the last few
- 0:13years, one molecule, urolithin A, has been making
- 0:17headlines for its supposed ability to reactivate
- 0:20the cellular cleanup system that slows as we get
- 0:24older. Before we talk hype, let's start with what
- 0:27urolithin A actually is. It's not something you
- 0:31eat. It's something that your gut bacteria make
- 0:34after you consume foods rich in elegant. These are
- 0:39polyphenols found in pomegranates, walnuts,
- 0:41berries, and green tea. Green tea is famous for
- 0:44its catkins, but it's also a real source of
- 0:47elligechanins like strychnine, meaning it provides
- 0:50the raw ingredients your microbes need to make
- 0:53urolithin A. And if you're lucky enough to have
- 0:55the right gut bacteria, you're lucky. When I first
- 0:59heard about urolithin A, I thought it was a
- 1:01commercial for a urology product. Something for
- 1:04benign prostatic hypertrophy. Maybe a middle aged
- 1:07man named Greg talking about improved flow. I'm
- 1:11sorry, urolithin A. You really deserve better
- 1:14marketing. So today we're going to skip the
- 1:17infomercials and look to the real science. Where
- 1:21urolithin A comes from, how it works, and why your
- 1:24microbiome decides whether pomegranates turn into
- 1:27longevity molecule or just a fruit stain on your
- 1:31favorite white shirt.
- 1:36Today we're continuing our longevity series and
- 1:39making sense of the madness of urolithin A. Yes,
- 1:43you've seen the tiktoks for it. I am, um, your
- 1:46Chief Medical Explanationist, Dr. Terry Simpson,
- 1:48and this is for Q Fork University, where we bust a
- 1:53few myths, make sense of the madness, and teach
- 1:56you a little bit about food and medicine.
- 2:02Urolithin A is created when your gut bacteria take
- 2:06alga channins and break them down into various
- 2:09forms that convert to urolithin A. You're not
- 2:12absorbing them directly. Your microbiomes are
- 2:15doing the chemistry for you like it does for a lot
- 2:18of things. It activates this pathway that does
- 2:22cellular housekeeping, that tags and recycles old
- 2:26broken mitochondria. So mitochondria, uh, are the
- 2:29powerhouse of the cell. I want you to imagine a
- 2:32cell. So the cell has, if you may recall from
- 2:35biology, a surrounding membrane, a, uh, nucleus
- 2:39and mitochondria. And it's the mitochondria that
- 2:42provide the energy and do the, a lot of the repair
- 2:45and that your cell needs. When you're young. This
- 2:49system works beautifully. After age 40, well, it
- 2:53kind of works like the TSA line during the
- 2:55government shutdown. Slow backed up and full of
- 2:58complaints. And urolithin A doesn't create new
- 3:01mitochondria. It helps your body clean out the bad
- 3:05ones so the good ones can perform better. Here's,
- 3:10uh, where urolithin A becomes a tale of two guts.
- 3:14Your microbiome decides your urolithin a destiny.
- 3:18It turns out only 12 to 40% of adults naturally
- 3:22produce urolithin A after eating foods that are
- 3:25rich in that substance. The urolithin A producers
- 3:29have gut microbiomes that look like a botanical
- 3:31garden. Rich diversity. They have a number of key
- 3:35bacteria that seem to carry the exact genes that
- 3:38are needed to make urolithin A. But most people
- 3:41are non producers. They don't have the microbiomes
- 3:45or the microbiomes with the functional genes. And
- 3:47no matter how many pomegranates they eat, how much
- 3:50green tea they drink, they make zero urolithin A.
- 3:54Do diets fix this? No. No diet. Keto, low carb,
- 3:59etc. No biohacker, no ancestral diet has been
- 4:02shown to turn a non producer of urolithin A into a
- 4:05producer. Now, the Mediterranean diet might give
- 4:08you more elegant, but it cannot install the
- 4:13bacteria required to convert them into ua and keto
- 4:16bacon. It does not care about your mitochondria.
- 4:20Here's the important part. If you happen to be a
- 4:22urolithin A producer, diet matters because you
- 4:25have the right microbiomes. If you're a non
- 4:28producer, food will never get you there. Hence
- 4:32supplement, bypass the entire microbiome
- 4:36bottleneck. Let's look at some actual trials and
- 4:39the human data. There have been tons of
- 4:42randomization trials in older adults. Typically 4
- 4:46months of urolithin A supplementation led to
- 4:49improved muscle endurance, improved mitochondrial
- 4:53biomarkers, reduced C reactive protein and
- 4:56inflammation marker by but importantly, there was
- 4:59not an improvement in 6 minute walk distance,
- 5:02there was not an improvement in ATP production,
- 5:05and there were no changes in major mobility
- 5:08outcomes. So your mitochondria looked happier. But
- 5:12in the big clinical outcomes, things didn't
- 5:15change. Another systemic review where they looked
- 5:19at five randomized controlled trials involving
- 5:21about 250 people, had the same story. Better
- 5:25autophagy markers, modest endurance improvements,
- 5:29lower inflammatory markers, but no effect on
- 5:33cardiovascular outcomes, weight or physical
- 5:36function. Urolithin A is biologically promising,
- 5:40but it's clinically modest.
- 5:43Now let's talk about the Hayflick senescent Cells.
- 5:46As you may recall, our cells are programmed to
- 5:50die. They will divide 50 plus or minus 10 times.
- 5:53They go into senescence and retire. Cells stop
- 5:58dividing and they just sort of hang out. But they
- 6:02don't hang out in a positive way like grandpa at
- 6:05the nursing home. They spend their time spewing
- 6:08inflammatory molecules which really make your
- 6:12whole body sick. Now, in vitro, meaning in the
- 6:16laboratory, you, Urolithin A does some kind of
- 6:19impressive things. It decreases some of the
- 6:22senescent cells, it suppresses some of the
- 6:24inflammatory markers, it restores mitophagy, it
- 6:28improves oxidative stress markers, and it boosts
- 6:31circadian rhythm amplitude in senescent
- 6:33fibroblasts. So urolithin A behaves kind of like
- 6:37a. It tames senescent cells rather than killing
- 6:41them. But none of this has been shown in human
- 6:45dishes. It's petri dishes, not clinical outcomes.
- 6:49In some cancer cell models, urolithin A actually
- 6:53induces senescence. And that's kind of promising
- 6:57also. But context matters. This is why we don't
- 7:01turn lab data into longevity promises. Well, we at
- 7:044Q don't. Now let's be clear. You cannot get
- 7:07urolithin A directly from food. You can only get
- 7:10it from the precursors. And, and people who have
- 7:12the right microbiome can convert them. But if you
- 7:16don't have the right microbiome, you can't. And we
- 7:19don't seem to be able to give you probiotics to do
- 7:22this. So what we actually know from safety data in
- 7:26urolithin A is from studies that last about four
- 7:29months of daily use. Four months, that's not four
- 7:33years. In that window when we give patients 250 to
- 7:371,000 milligrams a day, there was no significant
- 7:39severe adverse outcome. There was no dose limiting
- 7:43toxicity. The side effects were similar to
- 7:46placebos. There were no liver changes, no kidney
- 7:49changes, no blood abnormalities, some mild GI
- 7:52symptoms. Bloating, loose stools is the most
- 7:54common things. What about long term data? We don't
- 7:58have it. What about pregnancy data? We don't know
- 8:01and clearly don't recommend it for people who are
- 8:03pregnant. There's no multi year use data, there's
- 8:06no chronic disease safety data. So if someone says
- 8:09that urolithin A is proven safe long term, they're
- 8:12selling confidence, they're not selling evidence.
- 8:16So should you take it? Well, here's a verdict from
- 8:19us. At 4q, Urolithin A does something real. At the
- 8:22mitochondrial level. It improves biomarkers, helps
- 8:25endurance in older adults, it's safe short term.
- 8:27It bypasses that microbiome bottleneck, but it
- 8:31does not change major clinical outcomes, it does
- 8:35not reverse aging, and it doesn't replace the
- 8:39basics. The basics of lifting weight, moving your
- 8:42body, eating a Mediterranean style diet, getting
- 8:44better sleep, managing stress by taking more
- 8:46walks, and avoiding quacks. Urolithin A is
- 8:50scientifically interesting, but it is not
- 8:52scientifically transformative. Save the money for
- 8:56the supplement. Come on our Mediterranean cruise

- 10:11still sounds like something for your prostate.
- 10:16>> Speaker B: Yeah, but you medical people have weird names for
- 10:19most things like that. Still, I'm sure someone
- 10:22will try to use it for that. Man, we still got
- 10:26work to do.
