- 0:41Hello and welcome to Cancer Conversations On Talking Facts.
- 0:45This is your host,
- 0:46Doctor Jennifer Hunter,
- 0:47assistant Director for Family Consumer Sciences Extension at the University of Kentucky.
- 0:53Today,
- 0:53I'm pleased to be joined by Doctor Timothy Mullett and Miss Sherry Toll with the University of Kentucky Mary Cancer Center.
- 1:01Doctor Mullett.
- 1:02I'd like to start with you.
- 1:03Could you introduce yourself and tell us a little bit about your role with Marquee?
- 1:08Hello,
- 1:09I'm Tim Mullett.
- 1:10I'm a uh thoracic surgeon at the University of Kentucky and have been here for 26 years and have enjoyed working with a lot of different aspects of patient care.
- 1:20Most recently,
- 1:21I've been involved in the Marquee Cancer Center affiliate network as the medical director.
- 1:27And so I got the opportunity to work with physicians from across the state to be able to talk about how we can improve cancer care together.
- 1:34And Sherry,
- 1:35would you mind to introduce yourselves to our listeners?
- 1:37Yes.
- 1:38Good morning.
- 1:39Uh Sherry Toll.
- 1:40I'm the administrative director for the Mary Cancer Center affiliate Network.
- 1:44I am going on my 35th year with uh with UK,
- 1:49always with the Mary Cancer Center and I'm delighted to be with you this morning today.
- 1:55I'm excited to talk about the UK Marquee Cancer Center Affiliate Network.
- 2:01And um especially since our podcast is shared quite broadly across the state of Kentucky that we have reached into all 120 counties.
- 2:09And today we're gonna talk about community hospitals and how they can collaborate with the UK Marquee Cancer Center.
- 2:17And I wanted to start with just a basic question of,
- 2:21can you tell us more or tell us exactly what is the UK Marquee Cancer Center Affiliate network?
- 2:28The Market Cancer Center affiliate network is a collaboration between community hospitals and the Market Cancer Center,
- 2:36which is an,
- 2:37a National Cancer Institute designated Cancer Center and it is also the academic medical Center at the University of Kentucky.
- 2:45We started in 2006 with three hospitals.
- 2:49And 15 years later,
- 2:51we have 19 hospitals across the state and it's an opportunity for community hospitals to work with the Marquee Cancer Center to ensure high quality cancer care,
- 3:03to provide patients with the option to be close to home for their cancer care if at all possible.
- 3:09But if for whatever reason,
- 3:11the care is too complex or there are procedures or treatments that are not available locally,
- 3:19we are available to provide that service for them at the Marquee Cancer Center in Lexington and now Sherry,
- 3:25when we're talking about the UK Marquee Cancer Center affiliate network,
- 3:30y'all often use an acronym that is simply MC C A N said mccann.
- 3:37Is that correct?
- 3:38That is correct.
- 3:39We are affectionately known as mccann and many of the folks that are familiar with our program will refer to us as mccann.
- 3:47Excellent.
- 3:48And so can y'all tell me a little bit?
- 3:50Um How did mccann get started?
- 3:53Originally,
- 3:54we had a few of our medical oncologists that were providing outreach clinics at some of our sites and these community hospitals were really looking for a couple of things.
- 4:06They were looking for a more formal relationship with an academic medical center.
- 4:12And they also wanted to initially look for opportunities for additional education,
- 4:18for their staff.
- 4:19And this was primarily in the beginning for their nurses and they were very interested in how they could improve some of the educational opportunities that their nurses had.
- 4:31And as we started working with them and looking at a more formal process we realized that there was an opportunity to develop a network of community hospitals that could not only be connected with the Market Cancer Center but could also be connected with one another.
- 4:49And so in 2006,
- 4:51we started with three hospitals,
- 4:53Rockcastle Regional Hospital in Mount Vernon,
- 4:56Saint Clair Regional Medical Center in Morehead,
- 4:59and Harrison Memorial Hospital in Cynthiana.
- 5:02And with those three hospitals,
- 5:05we started providing professional education opportunities for their staff.
- 5:09We offered community outreach and educational programs uh for their communities and um a more formalized relationship between the providers with those hospitals and with the Mar Cancer Center.
- 5:24Excellent.
- 5:25So,
- 5:25Doctor Mullet,
- 5:26you shared with me that Marquee Cancer Center is Kentucky's only National Cancer Institute designated Cancer Center.
- 5:33Why is the N C I Cancer Center designation?
- 5:37Important?
- 5:39Well,
- 5:39thank you,
- 5:40Jennifer.
- 5:40That's a great question.
- 5:41Our interest in maintaining uh the uh designation by the National Cancer Institute or the N C I is that it provides access to certain clinical trials to certain procedures that are only available to those designated programs.
- 5:58It's a grant system that is offered through the National Institutes of Health and it recognizes the quality of research that's being done at an institution.
- 6:08And we really feel that this is a uh a great way for us to collaborate with our community partners through the mccann.
- 6:17Because what we can offer here at Mary Cancer Center in terms of advanced level research uh is something that they can't offer in their own facility.
- 6:26And so our opportunity to be able to,
- 6:29to share with our colleagues in the community,
- 6:33the research that we have available for their parents is a unique opportunity and one that uh can only be offered at Market Cancer Center.
- 6:43Excellent.
- 6:43Can you tell me a little bit about how high quality cancer care is defined?
- 6:49Sure,
- 6:51you know,
- 6:51we,
- 6:51we felt for several years now that if a program wanted to be identified as an affiliate with Mary and that we wanted to make sure that they were being held to a standard,
- 7:03that was an expectation for quality cancer care.
- 7:06And so examining opportunities for how to define that,
- 7:10we lean on the commission on cancer accreditation.
- 7:14That's a program that's offered nationally and programs of all sizes can participate in the multiple standards that exist and quality measures uh which allow us to evaluate the quality of care that's being delivered.
- 7:29It looks at the effectiveness of training of physicians and nurses.
- 7:33It looks at the data that they collect and it looks at the at how they take care of patients and programs and services that are being offered.
- 7:42And so one of the unique features of mccann is that we have the ability to monitor programs when they are striving to maintain accreditation by the commission on cancer.
- 7:55So we help them with educational programs with other services,
- 8:00with Consultative Services to be able to help them work through those standards so that they are maintaining that accreditation and can be really proud of that.
- 8:10So that's a great resource for a community program to be able to offer accreditation by a national.
- 8:16Uh And we help them move right on through.
- 8:19So it's been uh excellent for all of our programs to be able to see the pride that develops in their own community and in their own facility.
- 8:27and a chance for us to be able to help them uh maintain that excellent.
- 8:32Excellent Sherri.
- 8:33Both you and Doctor Mullett have talked a little bit about the services that um community hospitals receive as part of the marquee network.
- 8:43Would you care to expand on that?
- 8:44A little bit?
- 8:45Sure.
- 8:46I mentioned that we do offer um professional education and training programs for our sites and initially it was primarily for nurses,
- 8:56but as we know,
- 8:57cancer is a multidisciplinary uh disease.
- 9:00And so as we have grown over the years,
- 9:03our opportunities have expanded to,
- 9:05to all kinds of health professional disciplines.
- 9:08So we offer both web based and in person educational programs and opportunities for nurses,
- 9:16P A S um social workers,
- 9:19registered dieticians and other support staff at our sites.
- 9:23We also offer shadowing and mentoring opportunities.
- 9:26This,
- 9:27this was suspended uh during the pandemic,
- 9:30but we are starting to get back into inviting folks if one of our sites um hires a new social worker or hires a new chemotherapy nurse and would like to shadow somebody at Mary.
- 9:42We provide the opportunity to do that.
- 9:45We also have what we call an affiliate liaison program and that is staffed by nurse navigators nurse liaisons that can help our sites with streamlined referrals.
- 9:58If they have patients that cannot uh be cared for locally.
- 10:02At a hospital,
- 10:03we have nurse liaisons that can work with the providers to make sure that that's a,
- 10:08a smooth process for that patient to come to Lexington for either a second opinion or for a,
- 10:15a type of treatment uh that isn't available locally.
- 10:19And those nurse liaisons also talk with the patient so that the patient has someone before they even come to Mary that can assess any barriers,
- 10:29talk to them about what to expect,
- 10:31help them in,
- 10:32in feeling more comfortable coming to Lexington.
- 10:34And as you know,
- 10:35in many of our rural communities,
- 10:37Lexington is perceived as a big city and for somebody who's worked at UK for decades,
- 10:42we know that sometimes it may be challenging for parking or um because of the different additions for buildings,
- 10:50you're not sure where to go.
- 10:51And so our affiliate liaison program can help uh patients feel a little bit more comfortable coming to market for the first time.
- 10:59And then as doctor Mullet mentioned,
- 11:01uh with our quality mandate for the mission on cancer,
- 11:06we have a quality team of coordinators who work with our sites to provide that extra set of eyes and that expertise for their commission on Cancer Accreditation with our community outreach education and screening opportunities.
- 11:22We work closely with our folks to make sure that,
- 11:25that the communities are aware of all of the programs and services that are available at that community hospital and that they um uh are part of the Marquee affiliate network.
- 11:36Thank you,
- 11:37Doctor Mullett.
- 11:38I think that through our conversation today,
- 11:40it's been clear the benefits that are provided through the Marquee Cancer Center Affiliate Network.
- 11:46Um As we wrap up today from a patient perspective,
- 11:50why would a patient want to go to a hospital that is part of the marquee network?
- 11:55So I think you touch on something that's important for us and that's that we really want to make sure that we provide value to those facilities.
- 12:04Part of that is through education and part of it is through our assistance with their C O C accreditation.
- 12:09We wanna make sure that the that we have value to market so that patients that need to be in those clinical trials that we offer,
- 12:16have ready access to be able to get into that.
- 12:19And then we provide advantages to the patient and by extension to the community.
- 12:23So patients who participate in care at an affiliate network site know that should know that they have access to what we believe is a high standard of quality for a community program.
- 12:37But they also have the backup of collaboration with our clinicians,
- 12:42our physicians and experts in uh in Marquee Cancer Center because that's really what we want to provide.
- 12:50We want that to be the first choice of care for a patient should be to go to their community program to know that when it's,
- 12:57when the time is right,
- 12:58Marquee Cancer Center stands next to them uh with a,
- 13:02an ability to offer those specialized services in a seamless referral path through our uh nurse liaisons.
- 13:11And to be able to make sure that if they have questions,
- 13:15then they can communicate directly with us to be able to answer some of those complex questions that come up about patients.
- 13:21Maybe that means the patient can be cared for directly in their own community with the assistance of information from our experts.
- 13:29Maybe it means the patient needs to be transferred to Mary for a certain portion of their care and then returned to their community.
- 13:36But our hope is that all of these patients find that they are uh getting better care through the collaboration between the community site and Marquee Cancer Center.
- 13:46Excellent Doctor Mullet and Sherry.
