- 0:00Hey,
- 0:00everybody.
- 0:00Welcome
- 0:00back.
- 0:01today
- 0:01I have
- 0:02guest
- 0:02Doctor
- 0:03Padia,
- 0:03who is
- 0:04here.
- 0:04He's an
- 0:05interventional
- 0:05radiologist
- 0:06at UCLA
- 0:07from
- 0:07California.
- 0:08And today
- 0:09we're going
- 0:09to be
- 0:09chatting
- 0:10about liver
- 0:10directed
- 0:10therapies
- 0:11for
- 0:11metastatic
- 0:12disease
- 0:13with some
- 0:13exciting
- 0:13updates
- 0:14about
- 0:14Hepzato
- 0:14kit.
- 0:15So
- 0:15thanks
- 0:15so much for
- 0:16joining me
- 0:16doctor
- 0:16Padia.
- 0:17Thanks and
- 0:18thanks for
- 0:18the
- 0:18invitation.
- 0:19glad
- 0:19to, talk
- 0:20about this
- 0:20super
- 0:21exciting
- 0:21topic.
- 0:22it's super
- 0:22exciting
- 0:23and it's
- 0:23such an
- 0:24important
- 0:24thing
- 0:24for us
- 0:25as patients
- 0:25If you're
- 0:26dealing
- 0:26with
- 0:26metastatic
- 0:27disease,
- 0:27if you're
- 0:27potentially
- 0:28going to
- 0:28be dealing
- 0:29with it
- 0:29in the future
- 0:30or you're
- 0:30worried
- 0:30about that,
- 0:31it can help
- 0:31a lot
- 0:32to get
- 0:32this
- 0:32information
- 0:33and to
- 0:33just have
- 0:33a better
- 0:34understanding
- 0:34of what's
- 0:35out there
- 0:35so that
- 0:36when or
- 0:36if the
- 0:37need
- 0:37arises,
- 0:37you're not
- 0:38just trying
- 0:39to scramble
- 0:40for
- 0:40information,
- 0:40you now
- 0:41have a
- 0:41knowledge
- 0:42base,
- 0:42you know,
- 0:43kind of
- 0:43where to go
- 0:43and
- 0:44you know
- 0:44what to do.
- 0:44So,
- 0:45Doctor
- 0:45Padia,
- 0:45can you
- 0:46just help
- 0:46me, define
- 0:47Hepzato
- 0:47kit,
- 0:47what is it
- 0:48and
- 0:48how does
- 0:49it work?
- 0:49So great
- 0:50question.
- 0:51step back
- 0:51one
- 0:52step back.
- 0:52I'm an
- 0:52inventor
- 0:53for
- 0:53radiologists.
- 0:53And what
- 0:54we do,
- 0:54what most
- 0:55interventional
- 0:56radiologists
- 0:56do is
- 0:57we use
- 0:57imaging
- 0:58guidance
- 0:59to do
- 1:00minimally
- 1:00invasive
- 1:00procedures.
- 1:01And that's
- 1:01most often
- 1:03things
- 1:03in the
- 1:03blood
- 1:03vessels
- 1:04in your
- 1:04arteries
- 1:04and veins.
- 1:05the
- 1:05Hepzato kit
- 1:07or the
- 1:07Hepzato
- 1:07procedure,
- 1:08is
- 1:09a procedure
- 1:09that
- 1:09is done
- 1:10through the
- 1:10arteries
- 1:11and veins.
- 1:12Specific
- 1:13in your liver.
- 1:14And this
- 1:16device
- 1:16and
- 1:16procedure
- 1:17is designed
- 1:17for people
- 1:19who have
- 1:19uveal
- 1:20melanoma,
- 1:20where
- 1:21the uveal
- 1:22melanoma
- 1:22has
- 1:22metastasized
- 1:23to their
- 1:24liver
- 1:24just to
- 1:25the procedure
- 1:25itself,
- 1:26has become
- 1:27commercially
- 1:27available
- 1:28in the
- 1:28United
- 1:28States
- 1:29in 2024.
- 1:31It was FDA
- 1:32approved
- 1:33in 2023.
- 1:35and the
- 1:35reason
- 1:36it was
- 1:36FDA
- 1:36approved
- 1:37was
- 1:37there was
- 1:37a clinical
- 1:38trial done,
- 1:39in multiple
- 1:40centers,
- 1:40both in the
- 1:40United
- 1:41States
- 1:41and in
- 1:41Europe,
- 1:42where
- 1:43they did
- 1:43this
- 1:44procedure
- 1:44and
- 1:44they compared
- 1:45it to
- 1:46what is
- 1:47considered
- 1:47the best
- 1:48available
- 1:48care in
- 1:48so in
- 1:49every
- 1:49institution.
- 1:50That might
- 1:50be
- 1:50different.
- 1:51It might be
- 1:51immunotherapy,
- 1:52it might be
- 1:52chemotherapy,
- 1:53it might be
- 1:53something
- 1:54else.
- 1:55And so
- 1:55they compared it
- 1:56to that.
- 1:57And
- 1:57they found
- 1:58a
- 1:58significantly
- 1:59higher,
- 2:00outcome.
- 2:01And
- 2:01that's in
- 2:01two
- 2:02respects.
- 2:02Number one,
- 2:03it shrunk
- 2:04tumors
- 2:05to a much
- 2:05higher
- 2:06degree,
- 2:06compared to
- 2:07the
- 2:07patients
- 2:08who
- 2:08received,
- 2:09let's say,
- 2:09standard
- 2:09immunotherapy.
- 2:11And number
- 2:11two, it
- 2:12prolongs
- 2:12survival,
- 2:13which is,
- 2:13I think
- 2:14probably
- 2:15the most
- 2:15important
- 2:15thing.
- 2:16and number
- 2:16three,
- 2:17which
- 2:17I think is
- 2:17equally
- 2:18as
- 2:18important,
- 2:18is it
- 2:19preserve
- 2:19people's
- 2:19quality
- 2:20of life.
- 2:20So in other
- 2:21words,
- 2:21the side
- 2:22effects
- 2:22and the
- 2:22complication
- 2:23rate was
- 2:24pretty
- 2:24minimal
- 2:24and
- 2:25manageable,
- 2:26where,
- 2:26most people
- 2:27can kind of
- 2:28go on their
- 2:29daily
- 2:29lives.
- 2:30And that's
- 2:30what
- 2:30we've
- 2:30seen in
- 2:30our
- 2:31experience.
- 2:32So what
- 2:33is the
- 2:33Hepzato KIt
- 2:34it is
- 2:34a, it's
- 2:35a device.
- 2:36It's using
- 2:36a
- 2:36procedure.
- 2:37The procedure
- 2:38is done
- 2:38in the
- 2:38hospital.
- 2:40Currently,
- 2:40there are
- 2:40about 7
- 2:42to 10
- 2:43centers
- 2:44in the
- 2:44United States.
- 2:45And I think
- 2:45that number
- 2:46is probably
- 2:46growing
- 2:47over
- 2:47the next
- 2:48several
- 2:48months
- 2:49as certain
- 2:50centers,
- 2:51kind of
- 2:51come
- 2:52online.
- 2:52so few
- 2:53centers
- 2:53currently
- 2:53doing this,
- 2:54as you
- 2:54can
- 2:55imagine,
- 2:55we tend
- 2:55to get
- 2:56a lot
- 2:56of people
- 2:56coming in
- 2:57from
- 2:58outside of
- 2:58the Los
- 2:59Angeles
- 2:59area.
- 2:59In fact,
- 3:00I would say
- 3:00half
- 3:00the patients
- 3:01that I've
- 3:01treated
- 3:01have been
- 3:02from
- 3:02outside of
- 3:03Los Angeles
- 3:04County.
- 3:05the procedure
- 3:05is done
- 3:05in the
- 3:06hospital.
- 3:06It's done
- 3:07under
- 3:07anesthesia.
- 3:08So we,
- 3:08have an
- 3:08anesthesiologist
- 3:10who, puts
- 3:10you to
- 3:11sleep
- 3:11just like
- 3:11any other
- 3:12surgery.
- 3:13And
- 3:13then what
- 3:14we do is
- 3:14we isolate
- 3:15the liver
- 3:16via
- 3:16the blood
- 3:16vessels.
- 3:17That's done
- 3:18two ways.
- 3:19So the way
- 3:21blood flow
- 3:21works
- 3:22in the
- 3:23liver and
- 3:23in,
- 3:24let's say,
- 3:24every organ,
- 3:25you have
- 3:25an artery
- 3:26that takes
- 3:27blood
- 3:28from the
- 3:28heart
- 3:29to the liver,
- 3:31and you
- 3:31have a vein
- 3:32that takes
- 3:32blood
- 3:33from the
- 3:34liver
- 3:34back to
- 3:35the heart.
- 3:35Okay.
- 3:35So
- 3:36and that
- 3:36that
- 3:37concept
- 3:37works
- 3:37for every
- 3:38single
- 3:38organ.
- 3:38So if we're
- 3:39in
- 3:39the brain,
- 3:39you have an artery
- 3:40that goes
- 3:40to
- 3:40the brain,
- 3:41from the heart.
- 3:42And
- 3:42then you
- 3:42have a vein
- 3:43that goes
- 3:43from the,
- 3:44from the
- 3:45brain
- 3:45back
- 3:45to the heart.
- 3:46So that's
- 3:46how we do
- 3:46our
- 3:46circulation.
- 3:48Your your
- 3:48liver,
- 3:49your,
- 3:49brain,
- 3:49your toes,
- 3:50your
- 3:50fingers,
- 3:51etc..
- 3:52The goal
- 3:53is to
- 3:53isolate
- 3:54them.
- 3:54What's
- 3:54that?
- 3:55I just said
- 3:56all the
- 3:56parts.
- 3:57All the
- 3:57parts.
- 3:57So,
- 3:58arteries
- 3:58carry blood
- 3:59from
- 3:59the heart.
- 3:59Veins carry
- 4:00blood
- 4:01back
- 4:01to the heart.
- 4:01And so
- 4:02the goal
- 4:02when we do
- 4:03this
- 4:03procedure,
- 4:04which is,
- 4:04known
- 4:05as
- 4:05percutaneous
- 4:06hepatic
- 4:07perfusion
- 4:07with
- 4:07melphaian
- 4:08also known
- 4:08as hepzato
- 4:09kit
- 4:10is to
- 4:10isolate
- 4:11both
- 4:12the artery
- 4:12and the
- 4:13vein
- 4:13in the liver.
- 4:14And so
- 4:14where
- 4:14we're
- 4:15giving
- 4:15very high
- 4:16doses
- 4:17of
- 4:17chemotherapy
- 4:18the name of
- 4:19the
- 4:19chemotherapy
- 4:20is
- 4:20Melphalan
- 4:21It's
- 4:21actually
- 4:22a quite
- 4:22an old
- 4:23chemotherapy.
- 4:24but
- 4:25the challenge
- 4:25is that
- 4:26when we
- 4:26give this
- 4:27chemotherapy,
- 4:28let's say
- 4:28through
- 4:28an IV
- 4:29in your
- 4:29hand,
- 4:30it goes
- 4:30whole body.
- 4:31And there's
- 4:32only so
- 4:33much of
- 4:33a dose
- 4:33that
- 4:34your whole
- 4:34body can
- 4:34tolerate.
- 4:36That
- 4:36dose of
- 4:37that
- 4:37chemotherapy
- 4:38is not
- 4:39effective
- 4:40for
- 4:40your liver.
- 4:41And so by
- 4:42isolating
- 4:42the liver
- 4:43and
- 4:43delivering
- 4:43it directly
- 4:45into
- 4:45the artery
- 4:46that
- 4:46goes into
- 4:46your liver,
- 4:48and then
- 4:49sucking the
- 4:50chemo out
- 4:51via
- 4:52the vein
- 4:53so it does
- 4:53not
- 4:54circulate
- 4:55throughout
- 4:55your whole
- 4:56entire
- 4:56body.
- 4:56We can give
- 4:57much higher
- 4:575 to 10
- 4:58times
- 4:59the dose,
- 5:00because
- 5:00almost
- 5:01all of it
- 5:02stays
- 5:02within
- 5:02the liver
- 5:03or is
- 5:03trapped
- 5:03outside
- 5:04of it.
- 5:05Very little
- 5:05of it
- 5:06circulates
- 5:06throughout
- 5:07the rest
- 5:07of the body.
- 5:08So how does
- 5:09this.
- 5:09Let's take
- 5:10one more
- 5:10step
- 5:10backwards.
- 5:11How does this
- 5:11work?
- 5:11Okay,
- 5:12so
- 5:12you're put
- 5:13under
- 5:13anesthesia
- 5:13in the
- 5:14hospital.
- 5:14And our
- 5:14interventional
- 5:15radiology
- 5:16suite.
- 5:17All of this
- 5:17is done
- 5:18through
- 5:18tiny little
- 5:18skin.
- 5:19So
- 5:19at the end
- 5:20you just have
- 5:20bandage.
- 5:21There's no
- 5:21big
- 5:21stitches or
- 5:23sutures
- 5:23or anything
- 5:24like that.
- 5:25we have one
- 5:26tiny little
- 5:26catheter.
- 5:27It's
- 5:27the width
- 5:28of, maybe
- 5:29like the
- 5:29tip of
- 5:29a pen.
- 5:30It goes
- 5:31in the
- 5:31upper, left
- 5:32thigh.
- 5:34That
- 5:34catheter
- 5:34goes
- 5:34through
- 5:35the
- 5:35arteries
- 5:36all the way
- 5:36into the
- 5:37artery
- 5:37going into
- 5:38your liver.
- 5:38That is
- 5:39called the
- 5:39hepatic
- 5:40artery.
- 5:41We
- 5:41then have
- 5:42another
- 5:42catheter
- 5:43in the
- 5:43right side.
- 5:44In the
- 5:44right vein,
- 5:46in
- 5:46your upper
- 5:46thigh.
- 5:47That goes
- 5:48straight up
- 5:49into
- 5:50what's
- 5:50called
- 5:50the inferior
- 5:51vena cava,
- 5:52which
- 5:52is the
- 5:52blood
- 5:53vessel
- 5:53that drains
- 5:53the veins
- 5:55from the liver.
- 5:56So now
- 5:57we have
- 5:57a catheter
- 5:57in the
- 5:58veins
- 5:58from
- 5:58the liver.
- 5:59We have
- 5:59catheter
- 5:59the arteries
- 6:00going to
- 6:00the liver,
- 6:01and
- 6:01we infuse
- 6:03the
- 6:04melphalan
- 6:05or the
- 6:05chemotherapy
- 6:06through the
- 6:07catheter
- 6:07in the
- 6:08liver
- 6:09artery
- 6:09or the
- 6:09hepatic
- 6:10artery.
- 6:11At the same
- 6:12time,
- 6:13we remove
- 6:14the blood
- 6:15from
- 6:16the vein
- 6:16of
- 6:17the liver.
- 6:17So
- 6:17what it does
- 6:18is it
- 6:18washes
- 6:19through
- 6:19the liver,
- 6:20similar to
- 6:21if you
- 6:21were
- 6:21putting
- 6:21like a
- 6:22sponge
- 6:22under
- 6:22your
- 6:23kitchen
- 6:23sink
- 6:23running
- 6:24the faucet,
- 6:25the water
- 6:26will
- 6:26come
- 6:26down into
- 6:26the sponge
- 6:27and then
- 6:27it'll
- 6:27kind of
- 6:28come out
- 6:28at the
- 6:28bottom of
- 6:29the sponge
- 6:29as it
- 6:29percolates
- 6:30through
- 6:30the sponge.
- 6:31The chemo
- 6:32does
- 6:33the same thing,
- 6:34but instead
- 6:35of leaving
- 6:35and going,
- 6:36returning
- 6:36back
- 6:36into the heart,
- 6:37we suck it
- 6:38out.
- 6:39Through
- 6:39that
- 6:39catheter.
- 6:40We put it
- 6:41through a
- 6:41filtration
- 6:42circuit
- 6:43to extract
- 6:44all the chemo,
- 6:46and we
- 6:46return
- 6:47the blood
- 6:47back into
- 6:48another
- 6:48catheter,
- 6:49into the patient.
- 6:50So it's
- 6:50essentially
- 6:51a closed
- 6:52circuit
- 6:52where we
- 6:53filter out
- 6:53all
- 6:54the chemo.
- 6:55The
- 6:55procedure
- 6:56itself
- 6:56takes
- 6:57about two
- 6:58and a half
- 6:58to four
- 6:59hours.
- 6:59It's done
- 7:00under
- 7:00anesthesia
- 7:00so
- 7:01you don't
- 7:01really feel
- 7:01any pain.
- 7:02And
- 7:02at the end
- 7:03we remove
- 7:03all these
- 7:04catheters
- 7:04and we
- 7:04put, you
- 7:05know, 3
- 7:05or 4
- 7:05Band-Aids
- 7:06on
- 7:07the
- 7:07recovery
- 7:07is,
- 7:08you know,
- 7:08you recover
- 7:08from
- 7:08anesthesia.
- 7:09And
- 7:10in our practice,
- 7:10we've been
- 7:11sending
- 7:11patients
- 7:11home either
- 7:12at the end
- 7:12of the day
- 7:13or the
- 7:14next morning,
- 7:15and
- 7:16it takes
- 7:16about 3
- 7:18to 4 days
- 7:18for you
- 7:19to really,
- 7:19there's 3
- 7:20or 4 days
- 7:20where
- 7:20you're
- 7:20feeling
- 7:20a
- 7:21little bit
- 7:21of fatigue.
- 7:22But most of
- 7:23the patients
- 7:23we've
- 7:23treated
- 7:23have gone
- 7:24back
- 7:24to work
- 7:25either
- 7:25by
- 7:25the end of
- 7:25the week or
- 7:26early
- 7:26next week.
- 7:28this sounds
- 7:29awesome.
- 7:29I don't
- 7:30know about
- 7:30you guys,
- 7:30I know that
- 7:31the more
- 7:31that I
- 7:31understand
- 7:32about this
- 7:32procedure,
- 7:33the more excited
- 7:33I am.
- 7:34But it's
- 7:34available
- 7:34and that
- 7:34it's,
- 7:35you know,
- 7:35like you
- 7:35said, it's
- 7:35FDA
- 7:36approved
- 7:36here
- 7:36in the United States.
- 7:38And I think
- 7:38we'll we'll
- 7:39probably
- 7:39allude
- 7:39to this
- 7:40a
- 7:40little bit later,
- 7:40but it's
- 7:41also
- 7:41approved
- 7:42for use
- 7:43in Europe.
- 7:43It's been
- 7:44approved
- 7:45for several
- 7:46countries
- 7:47in Europe,
- 7:47including
- 7:47the United
- 7:48Kingdom,
- 7:48Germany,
- 7:49Netherlands,
- 7:50those are
- 7:50probably
- 7:50the three
- 7:50main
- 7:51countries.
- 7:51And they've
- 7:52been
- 7:52doing it
- 7:52for
- 7:52some time
- 7:53now.
- 7:53There are
- 7:54centers
- 7:54in Europe
- 7:55that have
- 7:55done,
- 7:56in fact,
- 7:56a probably
- 7:57a few
- 7:57hundred
- 7:58by now.
- 7:58and
- 7:58it is nice
- 7:59that it is
- 7:59FDA
- 8:00approved,
- 8:00in the
- 8:00United States
- 8:01because of
- 8:01the data.
- 8:02That also
- 8:02means
- 8:03that
- 8:03insurance
- 8:03carriers
- 8:04routinely
- 8:05cover this.
- 8:05Now,
- 8:05even though
- 8:06it is new.
- 8:07it was a little bit,
- 8:09I would say
- 8:09rocking
- 8:10the start
- 8:10because
- 8:10they never
- 8:10even
- 8:11heard of this.
- 8:12but now
- 8:12I would
- 8:12say that
- 8:12most
- 8:13insurance
- 8:13carriers
- 8:14have,
- 8:14if not
- 8:14all of them
- 8:15have
- 8:15put this
- 8:16into their
- 8:16policies.
- 8:17It is designed
- 8:18for people
- 8:18with liver
- 8:19metastases.
- 8:20So
- 8:20it's not,
- 8:20you know,
- 8:21if you have
- 8:21you'll be
- 8:22able
- 8:22melanoma
- 8:22and it's
- 8:23spread
- 8:24all over
- 8:25and
- 8:25in the lungs
- 8:26or in
- 8:26the lymph
- 8:26nodes.
- 8:27It's not
- 8:27going to do
- 8:27that.
- 8:27It's not
- 8:28going
- 8:28to really
- 8:29be
- 8:29effective
- 8:29for that.
- 8:30So
- 8:30the key thing
- 8:30to keep
- 8:31in mind
- 8:31is
- 8:31that it's
- 8:31really
- 8:32addressing,
- 8:32people
- 8:33where
- 8:34either all
- 8:35or at least
- 8:36most of
- 8:37your tumors
- 8:37have
- 8:38metastasized
- 8:38to the
- 8:39liver.
- 8:40just for
- 8:41any of
- 8:41us who are
- 8:41visual
- 8:42learners,
- 8:42do you have
- 8:43maybe
- 8:43a video
- 8:44or a short,
- 8:45kind of
- 8:45a picture
- 8:45that we
- 8:45can show
- 8:46this closed
- 8:47circuit?
- 8:47Because
- 8:47I think
- 8:48that that's
- 8:48one of the
- 8:48most unique
- 8:49things.
- 8:50Is this,
- 8:50I think
- 8:50I've heard
- 8:51it compared
- 8:51to almost
- 8:52like
- 8:52putting the
- 8:52liver on
- 8:53bypass.
- 8:54I think
- 8:54that's a
- 8:54maybe a
- 8:55good way
- 8:56to
- 8:56describe
- 8:56it.
- 8:56Do you.
- 8:58That's
- 8:58absolutely
- 8:59right.
- 8:59And I'm
- 8:59going
- 8:59to try
- 9:00to share
- 9:00my other
- 9:00screen
- 9:01here.
- 9:08Awesome.
- 9:08All right.
- 9:09So
- 9:10let's talk
- 9:11through
- 9:11kind of
- 9:11the visual.
- 9:12And
- 9:12what are we
- 9:12what are
- 9:13some of
- 9:13these
- 9:13kind of
- 9:13images mean
- 9:14in real
- 9:14time in
- 9:14real life.
- 9:15Because
- 9:15this looks
- 9:15like an
- 9:16actual
- 9:16image of
- 9:16the liver
- 9:17or the
- 9:17veins.
- 9:18So the
- 9:19so
- 9:19these are
- 9:19x rays
- 9:20and it's
- 9:21actually
- 9:21almost
- 9:21the same
- 9:22x ray.
- 9:22One is just
- 9:23what's
- 9:23called
- 9:23digitally
- 9:24subtracted.
- 9:25So that one
- 9:25on the left
- 9:26where it's
- 9:26almost
- 9:26all white,
- 9:27which is
- 9:27some black
- 9:28in
- 9:28the middle
- 9:28that's
- 9:28digitally
- 9:29subtracted.
- 9:29So let's
- 9:29go on the
- 9:30on the
- 9:31right side
- 9:31of the image
- 9:31you see
- 9:32these two
- 9:32circles,
- 9:33right.
- 9:33And it's
- 9:34connected
- 9:34with this
- 9:34big thick
- 9:35line.
- 9:35So that's
- 9:36the catheter
- 9:37that
- 9:37catheters
- 9:38inserted in
- 9:38the upper
- 9:39right
- 9:40thigh.
- 9:40And
- 9:40your vein.
- 9:41And it goes
- 9:42all the way
- 9:42up into
- 9:42a blood
- 9:43vessel
- 9:43called
- 9:43the IVC.
- 9:45We inflate
- 9:46these two
- 9:47balloons.
- 9:47Would you
- 9:48see these,
- 9:48this big
- 9:48circle
- 9:49on the top
- 9:49of
- 9:49the image
- 9:50and this
- 9:50oval on
- 9:51the bottom
- 9:51of the image.
- 9:52And so what
- 9:53that does
- 9:54is when
- 9:55we inflate
- 9:56those two
- 9:56balloons,
- 9:57it isolates
- 9:59that
- 10:00segment of
- 10:01the vein
- 10:02that's in
- 10:02between
- 10:02those two
- 10:03balloons.
- 10:04That
- 10:04segment of
- 10:05the vein is
- 10:05what drains
- 10:06the veins
- 10:07in the liver.
- 10:07So
- 10:07the veins
- 10:08in the liver
- 10:08which is
- 10:09on the left
- 10:10side of
- 10:11the image
- 10:12and the
- 10:12right side
- 10:12of your body,
- 10:13because
- 10:13we look
- 10:13at
- 10:13everything
- 10:14in kind of
- 10:14in portrait
- 10:14mode,
- 10:15drains into
- 10:17the region
- 10:18that is
- 10:18isolated
- 10:19between the
- 10:19two
- 10:19balloons.
- 10:20Now,
- 10:20if you
- 10:21look,
- 10:21at that
- 10:21bottom
- 10:22balloon,
- 10:23there's
- 10:23this
- 10:23horizontal
- 10:25wavy line.
- 10:27It's like a
- 10:27and that's
- 10:28the
- 10:28catheter
- 10:29or it's
- 10:30another
- 10:30catheter.
- 10:31That
- 10:31catheter is
- 10:32the width
- 10:33of the
- 10:34tip of
- 10:35a pen.
- 10:36that
- 10:36catheter
- 10:37comes from
- 10:37the left
- 10:39leg.
- 10:39So comes
- 10:40all the way
- 10:40from the
- 10:41bottom of
- 10:41the image.
- 10:42And then
- 10:43kind of
- 10:43courses
- 10:44to the
- 10:44right upper
- 10:45quadrant of
- 10:46the patient,
- 10:47in this
- 10:47case
- 10:47the left
- 10:48side
- 10:48of the image.
- 10:49And that is
- 10:49in the
- 10:50hepatic
- 10:50artery.
- 10:51So where
- 10:52your mouse
- 10:52cursor is,
- 10:53that's what
- 10:53the tip of
- 10:54the
- 10:54catheter.
- 10:55And that
- 10:56at the back
- 10:57end outside
- 10:58the patient
- 10:58is
- 10:58connected
- 10:59to the
- 10:59chemo.
- 11:00It's
- 11:00infused
- 11:02into
- 11:02the liver.
- 11:03And it is
- 11:04then
- 11:05withdrawn
- 11:07through the
- 11:07two
- 11:08balloons.
- 11:08So on
- 11:09the left
- 11:10image where
- 11:11you see
- 11:11that
- 11:11black
- 11:12stuff,
- 11:12that's us
- 11:13injecting
- 11:13contrast
- 11:14dye.
- 11:16So that
- 11:16thick
- 11:17column
- 11:18of black,
- 11:19that is the IVC.
- 11:20And then
- 11:21those,
- 11:21streaks
- 11:22going to
- 11:22the left
- 11:22side of
- 11:23the image.
- 11:23Those are
- 11:24the veins
- 11:25from
- 11:25the liver.
- 11:25So
- 11:26you can see
- 11:26everything
- 11:26is
- 11:26isolated.
- 11:27So it's
- 11:27basically
- 11:28withdrawing
- 11:29all that
- 11:30contrast
- 11:30which is
- 11:30blood,
- 11:31outside of
- 11:32the body
- 11:33filters
- 11:34the chemo out
- 11:35and then
- 11:35re infuses
- 11:36it, into
- 11:37a different
- 11:38catheter
- 11:38in the into
- 11:39the veins.
- 11:40So that's
- 11:41the
- 11:41summary.
- 11:42of what
- 11:42we're
- 11:42doing here.
- 11:44Now, this
- 11:44is that.
- 11:46Remember
- 11:46that wavy
- 11:47catheter
- 11:47we had
- 11:48in the
- 11:48hepatic
- 11:49artery.
- 11:49So this
- 11:49is just an
- 11:50isolated
- 11:51picture
- 11:51of that.
- 11:53The black
- 11:53stuff is
- 11:54contrast
- 11:54dye.
- 11:54So
- 11:55what we do
- 11:55is before
- 11:56we infuse
- 11:56the chemo
- 11:57we inject
- 11:58contrast
- 11:59dye.
- 12:00And that
- 12:00lights up
- 12:01as black.
- 12:02And
- 12:02what happens
- 12:03is we
- 12:03digitally
- 12:04subtract
- 12:05out
- 12:05the background,
- 12:05which is
- 12:06why
- 12:06everything
- 12:06looks,
- 12:07faded out
- 12:07or white
- 12:08and all
- 12:09these
- 12:09squiggly
- 12:10black
- 12:10lines,
- 12:11those are
- 12:11the
- 12:11arteries
- 12:12in your liver.
- 12:12And in this
- 12:13case,
- 12:13it is
- 12:14the right
- 12:14hepatic
- 12:14lobe,
- 12:14the right
- 12:15side
- 12:15of the liver.
- 12:16And
- 12:16those big
- 12:17blotchy
- 12:18gray
- 12:18things,
- 12:19those are
- 12:19tumors in
- 12:20the liver.
- 12:21Okay.
- 12:21So what
- 12:21we're going
- 12:21to do
- 12:22is we
- 12:22inject
- 12:22the
- 12:22contrast.
- 12:23I make sure
- 12:24we're in
- 12:25a good spot.
- 12:26And in this
- 12:26case,
- 12:26the right
- 12:27side
- 12:27of the liver.
- 12:28And then
- 12:28we, switch
- 12:29the contrast
- 12:29dye for
- 12:30the chemo.
- 12:31The chemo
- 12:31you can't
- 12:32see
- 12:32on the screen.
- 12:33and
- 12:33then the
- 12:33chemo
- 12:33infuses
- 12:34into these
- 12:35black
- 12:35lines,
- 12:36these
- 12:36arteries
- 12:37into the
- 12:37tumors
- 12:37themselves.
- 12:38And again,
- 12:39this is
- 12:39when we're
- 12:40doing this.
- 12:40The
- 12:40balloons
- 12:41are
- 12:41inflated
- 12:42in the vein
- 12:43withdrawing
- 12:44the
- 12:44blood
- 12:44at the
- 12:44same time.
- 12:46It's so
- 12:47incredible
- 12:48what
- 12:48medicine
- 12:49can do
- 12:49and how
- 12:50like how
- 12:50advanced
- 12:51this is.
- 12:52Not
- 12:52only that,
- 12:53if
- 12:53you think
- 12:53about like
- 12:54the
- 12:54resolution,
- 12:55this is
- 12:55obviously
- 12:56magnified
- 12:57up.
- 12:57this catheter
- 12:58is about, 3
- 13:00or 4
- 13:00thousandths
- 13:01of an
- 13:01inch
- 13:02in diameter
- 13:03so that
- 13:03that artery
- 13:04is super
- 13:05small.
- 13:05It's, it's
- 13:06like an
- 13:06eighth
- 13:06of an
- 13:07inch
- 13:07in width.
- 13:09and so
- 13:09those
- 13:09distal
- 13:10arteries,
- 13:10the ones
- 13:11downstream,
- 13:11they're
- 13:12basically
- 13:12the size
- 13:13of,
- 13:13you know,
- 13:14the human
- 13:14hair.
- 13:15That's
- 13:15incredible.
- 13:16so
- 13:16we have covered
- 13:18a lot
- 13:18of really
- 13:19neat stuff.
- 13:19Is there
- 13:20anything
- 13:20that,
- 13:20you know,
- 13:21for anyone
- 13:21who does
- 13:22have liver
- 13:22metastases
- 13:23just as
- 13:23far as kind
- 13:24of
- 13:24qualifications,
- 13:25like for a
- 13:25good
- 13:26candidate
- 13:26for hepzato
- 13:27kit,
- 13:28what are some
- 13:28of the
- 13:28kind of the
- 13:28baseline
- 13:29markers
- 13:29aside
- 13:30from
- 13:30obviously
- 13:31liver Mets,
- 13:31that
- 13:31you guys
- 13:32are looking
- 13:32for as
- 13:33far as
- 13:33a patient
- 13:34health
- 13:34and all of
- 13:34that? Yeah.
- 13:35So that's a
- 13:36great question.
- 13:36So of
- 13:37course
- 13:37you have
- 13:37to have
- 13:38liver
- 13:38metastases
- 13:39ideally
- 13:41really
- 13:41no
- 13:41significant,
- 13:42tumors
- 13:43outside
- 13:43the liver,
- 13:44although
- 13:44we have
- 13:44treated
- 13:45patients
- 13:46who have
- 13:46predominantly
- 13:47the
- 13:47metastases
- 13:48because
- 13:48it is
- 13:49widely
- 13:49believed
- 13:50amongst
- 13:51most
- 13:51medical
- 13:51oncologist
- 13:52that once
- 13:53you develop
- 13:53liver
- 13:54metastases,
- 13:55that is
- 13:55the
- 13:55bleeding
- 13:56edge of
- 13:57the problem.
- 13:57Even if you
- 13:58have,
- 13:58let's say
- 13:59lung
- 13:59metastases
- 14:00and or
- 14:00lymph
- 14:00nodes,
- 14:01there are
- 14:02several
- 14:02other
- 14:03different
- 14:03types
- 14:04of liver
- 14:04directed
- 14:05therapies
- 14:05that we can
- 14:06perform in
- 14:06the liver.
- 14:08And
- 14:09all of them
- 14:09have their
- 14:10advantages
- 14:11and
- 14:11disadvantages.
- 14:12So when
- 14:12I see
- 14:13a patient
- 14:13in
- 14:13consultation,
- 14:15let's say
- 14:15for this,
- 14:16I
- 14:16look at them,
- 14:17look
- 14:18at what
- 14:18they want
- 14:19or what
- 14:19what
- 14:20they have
- 14:20in mind.
- 14:21I
- 14:21look at their
- 14:22liver, on
- 14:22their
- 14:22imaging,
- 14:23typically
- 14:23an MRI
- 14:25to figure
- 14:25out how
- 14:26many tumors
- 14:26they have,
- 14:27where
- 14:27the
- 14:27distribution
- 14:28of the
- 14:28tumors are.
- 14:28And then I
- 14:29kind of
- 14:30make
- 14:30recommendations
- 14:31as to,
- 14:31okay,
- 14:31you have
- 14:32option A,
- 14:32B, and C in
- 14:33terms of,
- 14:34your
- 14:34treatment
- 14:34options
- 14:34for
- 14:35your liver.
- 14:35These are
- 14:36the pros
- 14:36and cons of
- 14:36each.
- 14:37So for
- 14:38example,
- 14:39most people
- 14:40for hepzato
- 14:40this is
- 14:41meant as a
- 14:41whole liver
- 14:42treatment
- 14:42okay.
- 14:42So we don't
- 14:43normally
- 14:43just treat
- 14:44one side
- 14:45of a liver.
- 14:46So most
- 14:47people
- 14:47should have
- 14:48multiple
- 14:49hepatic
- 14:50metastases.
- 14:51And in both
- 14:52sides of
- 14:52the liver
- 14:52let's say
- 14:53left and
- 14:53right.
- 14:55in
- 14:55contrast,
- 14:56if you had
- 14:56let's say
- 14:56one tumor
- 14:58just on
- 14:59the left
- 14:59side,
- 15:00I would
- 15:01recommend
- 15:01some other
- 15:02procedure,
- 15:03for
- 15:03example, a
- 15:04ablation
- 15:05procedure
- 15:05or a y90
- 15:07radioembolization
- 15:07procedure.
- 15:08Those
- 15:09are really
- 15:10more
- 15:10designed
- 15:10for focal
- 15:11treatments
- 15:12within
- 15:12the liver.
- 15:13The great
- 15:14thing about
- 15:14Hepzato
- 15:15is that
- 15:15it is
- 15:16designed
- 15:16for a whole
- 15:17liver
- 15:17treatment.
- 15:18Additionally,
- 15:19there are,
- 15:20certain
- 15:20other
- 15:20qualifications
- 15:21that one
- 15:21must have
- 15:22since you
- 15:22are
- 15:22undergoing
- 15:23general
- 15:23anesthesia.
- 15:24We want to
- 15:25make sure
- 15:25that people
- 15:26can
- 15:26tolerate
- 15:27that.
- 15:27And so what
- 15:28are the
- 15:28main risk
- 15:29factors
- 15:29for general
- 15:29anesthesia?
- 15:30Anyone
- 15:30who's had
- 15:30a heart
- 15:31attack,
- 15:32coronary
- 15:32artery
- 15:33disease,
- 15:34congestive
- 15:34heart
- 15:35failure,
- 15:35those people
- 15:36wouldn't be
- 15:36at
- 15:36relatively
- 15:37high risk
- 15:38for general
- 15:39anesthesia
- 15:39and would
- 15:40probably
- 15:40not be
- 15:41candidates.
- 15:42Additionally,
- 15:42since
- 15:42you are
- 15:43put to
- 15:43sleep
- 15:43with
- 15:43anesthesia,
- 15:44we want to
- 15:44make sure
- 15:44people
- 15:45don't have
- 15:45any
- 15:46breathing
- 15:46or lung
- 15:47problems.
- 15:48The
- 15:48most common
- 15:48things are
- 15:49like
- 15:49emphysema
- 15:50or COPD.
- 15:51and also
- 15:53since
- 15:53we are
- 15:53putting
- 15:53people
- 15:54on blood
- 15:54thinners
- 15:55simply
- 15:56during
- 15:57the
- 15:57procedure,
- 15:57not afterwards
- 15:58or
- 15:58beforehand,
- 15:59we want to
- 16:00make sure
- 16:00that they
- 16:00can
- 16:00tolerate
- 16:01blood
- 16:01thinners.
- 16:01So,
- 16:02for
- 16:02example,
- 16:03people
- 16:03who have
- 16:03had prior
- 16:04bleeding
- 16:04episodes
- 16:05when may
- 16:06not be
- 16:06a candidate
- 16:07or people
- 16:07who have
- 16:08metastases
- 16:09in their
- 16:09brain
- 16:10may not be
- 16:10a candidate.
- 16:11So
- 16:12we have basically
- 16:13what we do
- 16:13is when
- 16:14we see
- 16:14someone
- 16:14in
- 16:14consultation,
- 16:15we have a checklist
- 16:16and
- 16:16we kind of
- 16:17go through
- 16:17the
- 16:17checklist,
- 16:18to make
- 16:19sure
- 16:19that they
- 16:19are
- 16:20candidates.
- 16:20I would say
- 16:21at least
- 16:22in our
- 16:23practice,
- 16:23there's
- 16:24a good
- 16:24proportion
- 16:25of patients
- 16:25that are
- 16:25candidates
- 16:26for the
- 16:26hepzado
- 16:27procedure.
- 16:27and there's
- 16:27also a
- 16:28lot of
- 16:28patients
- 16:28who are
- 16:29better
- 16:30candidates
- 16:30for an
- 16:31ablation
- 16:31procedure
- 16:32or a y90
- 16:33radioembolization
- 16:34procedure,
- 16:34as well.
- 16:39Okay.
- 16:39So let's
- 16:40kind of
- 16:41do a
- 16:42little
- 16:42bit of
- 16:42a, I guess
- 16:43our role
- 16:43play here.
- 16:44So
- 16:44you're an
- 16:45interventional
- 16:45radiologist
- 16:46I'm
- 16:46a metastatic
- 16:47patient
- 16:47who.
- 16:48Well,
- 16:48let's
- 16:48just
- 16:48pretend
- 16:49I've just
- 16:49learned
- 16:49that
- 16:50I've got
- 16:50liver Mets
- 16:51and I
- 16:52unfortunately,
- 16:53and I'm
- 16:54going
- 16:54to just
- 16:54kind
- 16:54of pull
- 16:54from some
- 16:55prior
- 16:55experience
- 16:56because
- 16:56I've
- 16:56actually
- 16:56talked
- 16:56to quite
- 16:57a few of
- 16:57you guys
- 16:58in,
- 16:59the omie
- 16:59community
- 16:59this week
- 17:00alone.
- 17:00And it's
- 17:01been
- 17:01kind of
- 17:02incredible
- 17:02to me.
- 17:02and it's
- 17:03I think
- 17:03it's just
- 17:04one of
- 17:04those,
- 17:04you know,
- 17:04the more
- 17:04things
- 17:05you're
- 17:05aware of,
- 17:07or the,
- 17:07you know,
- 17:08when you
- 17:08say you
- 17:08want to buy
- 17:09a blue car
- 17:09and then
- 17:10suddenly
- 17:10you're
- 17:10seeing
- 17:10blue cars
- 17:11everywhere.
- 17:11Well, I'm
- 17:12a young
- 17:12patient
- 17:13with
- 17:13metastatic
- 17:14disease.
- 17:14And so I'm
- 17:15constantly
- 17:16aware of
- 17:16the younger
- 17:17patients
- 17:17like me
- 17:18who are
- 17:18also being
- 17:19diagnosed.
- 17:19Not to say
- 17:20that we are
- 17:20the largest
- 17:21percentage
- 17:21of the
- 17:22metastatic
- 17:22community,
- 17:23but
- 17:23it feels
- 17:24that way
- 17:24sometimes
- 17:24when I
- 17:25have, you
- 17:25know, 6
- 17:25or 7
- 17:26patients
- 17:26who
- 17:26message me
- 17:27in a week.
- 17:27so let's
- 17:28just
- 17:28pretend
- 17:28I just got
- 17:29MRI results
- 17:30on Friday
- 17:30and I'm
- 17:31consulting
- 17:31you
- 17:31because
- 17:32I've got
- 17:32like, one
- 17:33tumor up
- 17:34here in
- 17:34my liver
- 17:34and one
- 17:35kind of
- 17:35down here
- 17:36and, and
- 17:37the
- 17:37imaging
- 17:37and, you
- 17:38know,
- 17:38all of the
- 17:38things are
- 17:38pointing to
- 17:39I need
- 17:39liver
- 17:40directed
- 17:40therapy.
- 17:41And
- 17:41I've heard
- 17:42about
- 17:42hepzato kit
- 17:43can you
- 17:43tell me why
- 17:43or why not?
- 17:44Would
- 17:44hepzato kit
- 17:45be a good
- 17:45option
- 17:46for me.
- 17:46Okay.
- 17:47Great
- 17:47question.
- 17:48So
- 17:48let's say
- 17:49you have
- 17:49that
- 17:49scenario
- 17:50where
- 17:50you have
- 17:50two small
- 17:50tumors,
- 17:51one up
- 17:51here, one
- 17:52down there.
- 17:52and it's
- 17:53decided
- 17:53that,
- 17:54you know,
- 17:54you want
- 17:54to pursue
- 17:55liver
- 17:55directed
- 17:55therapy.
- 17:55And
- 17:55the first thing
- 17:56we do is,
- 17:57you know,
- 17:57we always
- 17:58get in
- 17:58touch
- 17:59with your
- 17:59medical
- 18:00oncologist,
- 18:00and we make
- 18:00this kind
- 18:01of a joint
- 18:01decision.
- 18:02That is,
- 18:02is this
- 18:03the right
- 18:03approach
- 18:04for you
- 18:05just to
- 18:05take one
- 18:06step back?
- 18:06In general,
- 18:07any liver
- 18:08directed
- 18:08therapy
- 18:09we perform
- 18:09can be done
- 18:11at the
- 18:11same time
- 18:12that
- 18:12you are
- 18:12getting
- 18:13systemic
- 18:13therapy
- 18:14if you
- 18:14are on
- 18:15systemic
- 18:15therapy.
- 18:15So, for
- 18:16example,
- 18:17if you are
- 18:17on a
- 18:18medication
- 18:18called
- 18:18Tebentafusp,
- 18:19also known
- 18:20as
- 18:20Kimmtrak,
- 18:21if
- 18:21you're on
- 18:22and
- 18:22immunotherapy
- 18:23agents such
- 18:23as
- 18:24nivolumab
- 18:24and
- 18:24ipilimumab
- 18:26those are
- 18:26just
- 18:27examples.
- 18:27And two
- 18:27of the more
- 18:28common
- 18:28ones,
- 18:29we don't
- 18:29even need
- 18:31to
- 18:31discontinue
- 18:32that.
- 18:32We can do it
- 18:33exactly
- 18:34at the
- 18:34same time.
- 18:35There's
- 18:35no
- 18:35interaction
- 18:37that we are
- 18:37aware of.
- 18:38So for
- 18:39someone
- 18:39with two
- 18:39small liver
- 18:40metastases,
- 18:41the goal is
- 18:41to do
- 18:42focal
- 18:42treatments
- 18:43of those
- 18:43two.
- 18:44And
- 18:44the reason
- 18:45is why
- 18:46put your
- 18:46liver,
- 18:47your entire
- 18:48liver
- 18:49through
- 18:49a procedure
- 18:50when
- 18:50we can just
- 18:51knock off
- 18:51those two
- 18:52tiny lesions.
- 18:54So it
- 18:54depends on
- 18:54the size
- 18:55and
- 18:56the location.
- 18:57But
- 18:57the more,
- 18:58common
- 18:58things
- 18:59that we
- 18:59would
- 18:59recommend
- 19:00in those
- 19:00cases
- 19:01is either
- 19:02number one,
- 19:03a thermal
- 19:03ablation
- 19:05or number
- 19:05two,
- 19:06a y-90
- 19:07radioembolization
- 19:08So what is
- 19:08each one,
- 19:09a thermal
- 19:09ablations
- 19:10where we
- 19:10bring
- 19:11you into
- 19:11the
- 19:11hospital?
- 19:11It's an
- 19:12outpatient
- 19:12procedure.
- 19:13We put you
- 19:13to sleep
- 19:14under
- 19:14anesthesia.
- 19:15We take a
- 19:16tiny,
- 19:17thin needle
- 19:18similar to,
- 19:19you know,
- 19:19the needle
- 19:19that you
- 19:19may have
- 19:20had during
- 19:20a biopsy
- 19:21procedure.
- 19:22Numb up
- 19:22your skin
- 19:23using
- 19:23an ultrasound
- 19:24and a CT
- 19:25scan,
- 19:26we
- 19:27guide it
- 19:27into
- 19:27your tumor.
- 19:28So we're
- 19:29cheating
- 19:29the whole
- 19:30way
- 19:30through.
- 19:30so we know
- 19:31exactly
- 19:31where
- 19:31we're going.
- 19:32So we know
- 19:33not to
- 19:33go through
- 19:34dangerous
- 19:34things.
- 19:36At the end
- 19:36of that
- 19:37needle,
- 19:37there's an
- 19:37electrical
- 19:38cord
- 19:38connected
- 19:39to a
- 19:39generator
- 19:40box.
- 19:40We flip
- 19:41the switch,
- 19:42the tip
- 19:43of the
- 19:43needle
- 19:44heats up to
- 19:45about 150
- 19:45to 180
- 19:46degrees.
- 19:47We
- 19:48literally
- 19:48cook
- 19:49the tumor
- 19:50for 10
- 19:50to 15
- 19:51minutes,
- 19:52turn
- 19:52the switch
- 19:52off,
- 19:53take the
- 19:53needle out,
- 19:54put a
- 19:54Band-Aid on
- 19:55you, send
- 19:55you home.
- 19:56That works
- 19:57extraordinarily
- 19:58well.
- 19:59It is
- 19:59designed
- 19:59for small
- 20:00tumors,
- 20:01just like,
- 20:02you know,
- 20:02you have
- 20:02a shish
- 20:03kebab
- 20:03on your
- 20:03barbecue
- 20:03at home to
- 20:04the heat.
- 20:05Will start
- 20:06to
- 20:06dissipate
- 20:07the farther
- 20:08you go away
- 20:08from the heat.
- 20:09So I
- 20:09can't do a
- 20:10large tumor
- 20:11because of that.
- 20:12Because
- 20:12then
- 20:12the heat
- 20:12won't reach
- 20:13all the way
- 20:13to the
- 20:14margin.
- 20:14It also has
- 20:15to be done
- 20:16in a tumor
- 20:17that is
- 20:17in a
- 20:18favorable
- 20:19location.
- 20:19In other
- 20:19words,
- 20:19I have to be
- 20:20able to get
- 20:20from
- 20:21the skin
- 20:22all the way
- 20:22into the
- 20:23tumor
- 20:24safely
- 20:24without
- 20:25damaging
- 20:26any
- 20:26surrounding
- 20:27structures
- 20:27which
- 20:27may be
- 20:28sensitive
- 20:29to heat.
- 20:30So
- 20:30a lot of
- 20:30people
- 20:30are
- 20:31candidates
- 20:31for that.
- 20:32some people
- 20:32are not.
- 20:33And if,
- 20:35you're not
- 20:35a
- 20:35candidate,
- 20:36then
- 20:36we would
- 20:36kind of,
- 20:37for safety
- 20:38reasons
- 20:38or let's
- 20:38just say,
- 20:39you're in a
- 20:39tenuous
- 20:40location
- 20:40where
- 20:41I don't
- 20:41want to
- 20:41deliver heat
- 20:42because
- 20:42there's
- 20:42the colon
- 20:43in the way,
- 20:43for
- 20:44example.
- 20:44Then
- 20:45we would
- 20:45switch to
- 20:46the next
- 20:46treatment,
- 20:46which is a
- 20:47y-90
- 20:48radioembolization.
- 20:49Okay.
- 20:50are there
- 20:51and I
- 20:51know, like,
- 20:52this is
- 20:52this is
- 20:53maybe
- 20:53just me
- 20:54because I'm
- 20:54a little
- 20:54more aware
- 20:55of it.
- 20:55And
- 20:55it's
- 20:55kind of
- 20:56a buzzword
- 20:56now that is
- 20:57being
- 20:57talked
- 20:57about in
- 20:58liver
- 20:58directed
- 20:58therapies.
- 20:59But for
- 20:59like 1 or 2
- 21:00small
- 21:00spots.
- 21:01has your
- 21:01interventional
- 21:02radiology
- 21:02team had
- 21:03any contact
- 21:04with anyone
- 21:04who's done
- 21:05histotripsy
- 21:05as an
- 21:06ablation
- 21:07option?
- 21:07And when
- 21:08do
- 21:08you think
- 21:08that would
- 21:08be best
- 21:09in your
- 21:09opinion?
- 21:10So
- 21:10histotripsy
- 21:11also just
- 21:12came out
- 21:13probably in
- 21:13the last
- 21:14year.
- 21:15histotripsy
- 21:16is a
- 21:16relatively
- 21:17new method
- 21:18of
- 21:18ablation.
- 21:18So what
- 21:19we've
- 21:19typically
- 21:19done is
- 21:20something
- 21:20called
- 21:20thermal
- 21:21ablation.
- 21:21Thermal
- 21:21meaning
- 21:22temperature.
- 21:22We heat the tissue
- 21:23or
- 21:24in some case
- 21:24some people
- 21:25will freeze
- 21:25the tissue.
- 21:26histotripsy
- 21:27is done
- 21:28by
- 21:28ultrasound
- 21:29waves.
- 21:29So there's
- 21:29no heat
- 21:30or cold.
- 21:31So the nice
- 21:32thing is
- 21:32remember
- 21:32I mentioned
- 21:33that
- 21:33structures
- 21:33may be
- 21:34damaged
- 21:35by heat,
- 21:36histotripsy
- 21:36doesn't
- 21:37have that
- 21:38issue
- 21:38because
- 21:38there's no
- 21:39heat
- 21:39deliver.
- 21:40So that's
- 21:40the great
- 21:41advantage
- 21:42of
- 21:42histotripsy
- 21:43in that,
- 21:44the damage
- 21:45to
- 21:46surrounding
- 21:47structures
- 21:47is
- 21:47essentially,
- 21:48not there.
- 21:49Now,
- 21:50there are
- 21:50some
- 21:50challenges
- 21:50of
- 21:51histotripsy
- 21:51number one,
- 21:52when it was studied
- 21:54and
- 21:54when the trials
- 21:54came out,
- 21:55it wasn't
- 21:56studied
- 21:56specifically.
- 21:57In uveal
- 21:58melanoma
- 21:59metastases,
- 22:00they did it
- 22:01in
- 22:01all comers.
- 22:02So people
- 22:03with
- 22:04primary
- 22:04liver
- 22:04cancer,
- 22:05people
- 22:05with
- 22:06metastases
- 22:07from
- 22:07colorectal
- 22:08cancer
- 22:08or breast
- 22:08cancer
- 22:09or
- 22:09melanoma.
- 22:10So
- 22:10it wasn't
- 22:11really
- 22:11they didn't
- 22:12do a trial
- 22:12specifically
- 22:13for, uveal
- 22:14melanoma.
- 22:15Number two,
- 22:16you also have
- 22:17to be, able
- 22:18to
- 22:18technically
- 22:20get to
- 22:21the lesion
- 22:21via
- 22:22histotripsy
- 22:23And
- 22:23there are a
- 22:23lot of
- 22:23places in
- 22:24the liver
- 22:25where
- 22:25the histotripsy
- 22:26device
- 22:27in its
- 22:27current
- 22:27version
- 22:28doesn't
- 22:28work or
- 22:29will not
- 22:30see
- 22:30the lesion.
- 22:30So it has
- 22:31to be
- 22:31approachable,
- 22:33via
- 22:33the histotripsy device
- 22:35but I
- 22:35think
- 22:35it's a
- 22:36great
- 22:36option for,
- 22:38small
- 22:38tumors
- 22:39where the
- 22:39ablation
- 22:40that our
- 22:40typical
- 22:41ablation
- 22:41modalities,
- 22:42such as
- 22:42microwave
- 22:43ablation
- 22:43or
- 22:43radiofrequency
- 22:44ablation
- 22:44or
- 22:44cryoablation,
- 22:45are
- 22:46let's say,
- 22:46high risk.
- 22:48you're
- 22:48telling me
- 22:48that I have
- 22:49a couple spots
- 22:50and that
- 22:50we should
- 22:50do spot
- 22:51treatment,
- 22:51essentially,
- 22:52but
- 22:52why wouldn't
- 22:53I want to
- 22:53just do
- 22:54the whole
- 22:54treatment?
- 22:54Like,
- 22:55why wouldn't
- 22:55I want
- 22:55to have
- 22:56my entire
- 22:56liver
- 22:56just dosed
- 22:57and get it
- 22:57over with?
- 22:58Make sure
- 22:58that
- 22:58anything
- 22:59tiny, you
- 22:59know, can't
- 23:00come back.
- 23:01What's
- 23:02the harm of that?
- 23:03So what
- 23:03we're
- 23:03trying to do
- 23:04and what
- 23:05this is
- 23:05something
- 23:05that I
- 23:05think
- 23:05that we've
- 23:06learned
- 23:06in the last
- 23:07ten years,
- 23:09what
- 23:09we want
- 23:09to do
- 23:10is design
- 23:10these
- 23:10therapies
- 23:11as a
- 23:11marathon
- 23:12and
- 23:12not a sprint.
- 23:13So
- 23:13what does that
- 23:14mean?
- 23:14That means
- 23:14that
- 23:15let's say
- 23:15you come
- 23:16into
- 23:16my office
- 23:16for
- 23:17those two
- 23:17tiny
- 23:17lesions,
- 23:19it's,
- 23:19you know,
- 23:20August
- 23:212024.
- 23:21I want to
- 23:22design
- 23:23treatments
- 23:23for you
- 23:24for the
- 23:24next
- 23:25several
- 23:25years.
- 23:26What
- 23:26I don't
- 23:26want to do
- 23:27is design
- 23:27a treatment
- 23:28for you
- 23:28for August
- 23:29and
- 23:29September
- 23:30and October
- 23:30of 2024,
- 23:32and then
- 23:33if you
- 23:33pop up
- 23:34new tumor
- 23:34six months
- 23:35later
- 23:35or a year
- 23:36later,
- 23:37I tell you,
- 23:38I don't
- 23:38have,
- 23:39you know,
- 23:39all
- 23:39all of our
- 23:40tools are
- 23:41kind of,
- 23:42used up.
- 23:43And so
- 23:44this becomes
- 23:45we want to
- 23:45stretch
- 23:46things out
- 23:47so
- 23:47you can
- 23:47live
- 23:48as long
- 23:48as possible
- 23:49with,
- 23:51complete
- 23:51preservation,
- 23:52in terms of
- 23:52quality
- 23:53of life.
- 23:53So today
- 23:54you might
- 23:55have
- 23:55two lesions
- 23:56and
- 23:56we can get
- 23:57rid of
- 23:57those.
- 23:57And in
- 23:58many
- 23:58patients
- 23:58we've done
- 23:59that.
- 23:59And,
- 24:00we just
- 24:01monitor
- 24:01them with
- 24:02an MRI
- 24:02every
- 24:02three months,
- 24:03and
- 24:03they don't
- 24:03ever pop up
- 24:04new ones,
- 24:05at least
- 24:05for several
- 24:06years.
- 24:06And in
- 24:06others,
- 24:07they may
- 24:08pop up
- 24:08a bunch of
- 24:09new ones.
- 24:09And then
- 24:10what we do
- 24:11is we start
- 24:12all over
- 24:12We'd say,
- 24:12okay,
- 24:12what
- 24:13is your best
- 24:14option
- 24:14at that
- 24:15point?
- 24:16Okay.
- 24:16If we're
- 24:16careful
- 24:17and
- 24:17selective,
- 24:19and
- 24:19preserve
- 24:20the rest of
- 24:20the liver,
- 24:21we can
- 24:21repeat
- 24:22these
- 24:22treatments
- 24:23over
- 24:23and over
- 24:24and over
- 24:25again
- 24:25as long as
- 24:26you
- 24:26tolerate
- 24:26them.
- 24:27So
- 24:27we can get
- 24:28the most
- 24:29amount
- 24:30of
- 24:30longevity
- 24:31for you
- 24:32again,
- 24:33while
- 24:33preserving
- 24:34your
- 24:34quality
- 24:34of life.
- 24:35The goal is
- 24:35to.
- 24:36If you're
- 24:36working
- 24:36a job,
- 24:37I want you
- 24:37to keep
- 24:37on working.
- 24:38If you're,
- 24:39you know,
- 24:40like to
- 24:40go fishing,
- 24:41I want you
- 24:41to keep on
- 24:42going
- 24:42fishing.
- 24:42The goal is
- 24:42not to
- 24:43put you
- 24:43in bed
- 24:44all day.
- 24:45really,
- 24:45in general,
- 24:45when you're
- 24:46doing
- 24:46any liver
- 24:46directed
- 24:47therapy,
- 24:47I think
- 24:48what
- 24:48you told me
- 24:48before, was
- 24:49you want to
- 24:49treat the
- 24:49cancer?
- 24:50Of course,
- 24:50like that's
- 24:51number
- 24:51one goal.
- 24:52And then
- 24:52it sounds
- 24:52like
- 24:52patient
- 24:53learning
- 24:53with
- 24:53longevity
- 24:54is number
- 24:54two goal.
- 24:55And then
- 24:55that also
- 24:56kind of
- 24:56encompasses
- 24:57that high
- 24:57quality
- 24:57of life
- 24:58for as long
- 24:58as
- 24:58possible.
- 24:59Like those
- 25:00three
- 25:00kind of
- 25:00main
- 25:00things.
- 25:01So having
- 25:02the entire
- 25:02liver
- 25:03treated
- 25:03isn't
- 25:03necessarily
- 25:04going to be
- 25:04the most ideal
- 25:05because
- 25:05like you
- 25:05said, it's
- 25:06going to
- 25:07almost,
- 25:07use it
- 25:08too early
- 25:08in the marathon,
- 25:09so to
- 25:09speak.
- 25:10Exactly.
- 25:10So
- 25:11let's say
- 25:11you did
- 25:11come in
- 25:12and I
- 25:12did decide
- 25:12to do
- 25:13a hepzato
- 25:13procedure
- 25:13which is
- 25:14fine
- 25:14and would
- 25:14probably
- 25:15work in
- 25:15someone
- 25:16with a
- 25:16that low
- 25:17of a tumor
- 25:17burden.
- 25:18And
- 25:19then you
- 25:19came to me
- 25:20a year
- 25:20later
- 25:21and
- 25:21you sprouted
- 25:23a bunch
- 25:23of
- 25:23new tumors.
- 25:24So then
- 25:25I got a problem
- 25:26because
- 25:26I've
- 25:26already
- 25:26used up
- 25:27my main
- 25:27tool,
- 25:28which is the hepzato
- 25:28So I don't
- 25:29I can't
- 25:30necessarily
- 25:30go back
- 25:31and do
- 25:31ablation
- 25:32if
- 25:32let's say
- 25:32you
- 25:32sprout up
- 25:33ten tumors.
- 25:34And so we
- 25:35may have
- 25:35used up
- 25:36that option
- 25:37when I
- 25:37could
- 25:38have used
- 25:38something
- 25:39else. Yeah.
- 25:40And so
- 25:40that's
- 25:40where we're
- 25:41trying to
- 25:42kind of
- 25:43do this
- 25:44in a
- 25:44sequential
- 25:45stepwise
- 25:46and
- 25:47thoughtful
- 25:47manner,
- 25:48fully
- 25:48realizing
- 25:50that
- 25:51there's no
- 25:51data as
- 25:52to which
- 25:53therapy
- 25:54you should
- 25:54do first.
- 25:56this is really
- 25:57where
- 25:57the art
- 25:58of
- 25:58medicine,
- 25:59becomes
- 25:59just as
- 25:59important
- 26:00as the
- 26:00science.
- 26:01Yeah,
- 26:02exactly.
- 26:02Okay.
- 26:03okay, so
- 26:04for
- 26:04those of
- 26:04you guys
- 26:04listening,
- 26:05let's
- 26:05just say
- 26:06this isn't
- 26:06an option
- 26:07for you,
- 26:07right?
- 26:07Like,
- 26:08you can't
- 26:08just have
- 26:081 or 2
- 26:09tumors
- 26:09treated.
- 26:10Let's say
- 26:10you've got
- 26:10a little
- 26:11bit more
- 26:11involvement.
- 26:11Maybe
- 26:12you've
- 26:12got a
- 26:12larger
- 26:12tumor,
- 26:13something
- 26:13bigger than
- 26:14a
- 26:14centimeter
- 26:15or two.
- 26:15you
- 26:15mentioned
- 26:16some y-90
- 26:17options
- 26:17and just
- 26:18some other
- 26:18kind of
- 26:19a little
- 26:20bit more
- 26:20than just
- 26:21an ablation
- 26:21or,
- 26:22histotripsy
- 26:22treatment.
- 26:23what would be
- 26:24kind of the
- 26:25ideal
- 26:25situation
- 26:26for that
- 26:26patient?
- 26:27Okay.
- 26:28So let's
- 26:28say we
- 26:29actually
- 26:29just saw
- 26:30somebody
- 26:30you
- 26:30follow up
- 26:30two days
- 26:31ago.
- 26:32he had a single
- 26:33tumor
- 26:34that was
- 26:35six
- 26:36centimeters.
- 26:36So about
- 26:37two and
- 26:38a half
- 26:38to three
- 26:38inches
- 26:39and a
- 26:39second
- 26:40smaller one
- 26:41next to it.
- 26:42again,
- 26:42the rest of
- 26:42the liver
- 26:43was fine.
- 26:43It was all
- 26:43in
- 26:43the middle
- 26:44of the liver.
- 26:45and so
- 26:45I didn't
- 26:46want to
- 26:46treat the
- 26:47whole liver
- 26:48at the same
- 26:48time.
- 26:48The tumor
- 26:49was too big
- 26:50for to
- 26:50for me
- 26:51to get a
- 26:51successful
- 26:52ablation.
- 26:53Because
- 26:53you can
- 26:53imagine
- 26:54if I take
- 26:54a tumor
- 26:55that's
- 26:55the size
- 26:55of a golf
- 26:56ball,
- 26:56and I put
- 26:57a tiny
- 26:57needle
- 26:58in the middle,
- 26:59the heat may
- 27:00not go
- 27:00to the edge
- 27:01of that
- 27:01golf ball.
- 27:02And so
- 27:03the y- 90
- 27:04radioembolization
- 27:04is a great
- 27:05option
- 27:05in this
- 27:05case
- 27:06similar to
- 27:07the Hepzato
- 27:08procedure.
- 27:08It is done
- 27:09through
- 27:09the artery
- 27:10in the liver.
- 27:10So remember
- 27:11the artery
- 27:11is the one
- 27:12that
- 27:12carries
- 27:12blood
- 27:12from the
- 27:12heart
- 27:13to the liver.
- 27:14And we
- 27:14infuse
- 27:16tiny
- 27:17radioactive
- 27:17particles.
- 27:18These
- 27:18particles
- 27:18are so small
- 27:19you can't
- 27:20even see
- 27:20them.
- 27:21There are
- 27:21several
- 27:22million
- 27:22of them.
- 27:23and
- 27:24the infusion
- 27:25is is
- 27:25completely
- 27:25painless.
- 27:26So
- 27:26we do this
- 27:27under
- 27:27sedation,
- 27:28not under
- 27:28anesthesia.
- 27:30So what
- 27:30we do is
- 27:31we position
- 27:31that
- 27:31catheter
- 27:32in the
- 27:33specific
- 27:34artery,
- 27:35that goes
- 27:35towards,
- 27:36towards
- 27:37that tumor
- 27:38to
- 27:38make sure
- 27:39that we're
- 27:39not going
- 27:39somewhere else
- 27:40or even in
- 27:40the rest of
- 27:41the liver.
- 27:41We infuse
- 27:42those, tiny
- 27:44radioactive
- 27:45particles,
- 27:46we take
- 27:46the
- 27:46catheter
- 27:47out, put
- 27:47a Band-Aid
- 27:48on you,
- 27:48send
- 27:48you home.
- 27:49That
- 27:49procedure
- 27:49again,
- 27:49takes about
- 27:501 to 2
- 27:51hours,
- 27:51and it's
- 27:51done under
- 27:51sedation.
- 27:52It is
- 27:53also very
- 27:54well
- 27:54tolerated.
- 27:55there are,
- 27:57again,
- 27:57just like
- 27:58every
- 27:58therapy,
- 27:59there are
- 27:59advantages
- 28:00and
- 28:00disadvantages.
- 28:01So that
- 28:01works
- 28:02extraordinarily
- 28:03well.
- 28:03When we're
- 28:04targeted
- 28:04and focal,
- 28:05we can give
- 28:06very high
- 28:06doses
- 28:07of
- 28:07radiation,
- 28:08to that
- 28:08tumor.
- 28:09And there's
- 28:09no spread
- 28:11of the
- 28:11radiation
- 28:12elsewhere.
- 28:13The downside
- 28:14comes
- 28:14when
- 28:15we have
- 28:15to treat
- 28:15someone's
- 28:16whole
- 28:16liver.
- 28:17Let's say
- 28:17someone
- 28:17presents
- 28:18with 10
- 28:18or 12
- 28:19lesions,
- 28:20then
- 28:20we have to
- 28:21irradiate
- 28:22the entire
- 28:22liver.
- 28:23And that's
- 28:23where,
- 28:24I think
- 28:24we have
- 28:25some
- 28:25challenges
- 28:25and where
- 28:26the hepzato
- 28:27procedure
- 28:27might be
- 28:28a better
- 28:28option.
- 28:29Yeah.
- 28:29For sure.
- 28:30Okay,
- 28:31so we've
- 28:32got the y-90.
- 28:33what
- 28:33can you
- 28:33tell us
- 28:33just
- 28:34briefly
- 28:34about Linac
- 28:35radiation
- 28:35and the IHI
- 28:37and the C
- 28:37procedures?
- 28:38feel free
- 28:38to touch
- 28:39on either
- 28:39or both
- 28:40of those
- 28:40briefly.
- 28:42So
- 28:42the line of
- 28:42radiation
- 28:43is
- 28:43I would say
- 28:44right now,
- 28:45it's done
- 28:46again
- 28:46by
- 28:46radiation
- 28:46oncologist.
- 28:47It's still
- 28:48very early
- 28:49in its
- 28:50development
- 28:50and phases.
- 28:51there's
- 28:52no
- 28:52significant
- 28:53or really
- 28:54large scale
- 28:55data sets
- 28:55right now.
- 28:56So I think
- 28:56we're still
- 28:57learning
- 28:57about it.
- 28:58Okay.
- 28:58and that's
- 28:58probably
- 28:59mostly
- 28:59what I can
- 29:00say.
- 29:00it's still
- 29:01kind of
- 29:01a work
- 29:02in
- 29:02progress.
- 29:03Okay.
- 29:03So kind
- 29:04of the
- 29:04buzzword
- 29:05for,
- 29:05you know,
- 29:06for one
- 29:07lobe or for
- 29:07kind of
- 29:08whole lobe
- 29:09or whole
- 29:09liver
- 29:10treatment
- 29:10where
- 29:10you're
- 29:10treating
- 29:11both of
- 29:11the lobes.
- 29:12the buzz
- 29:12conversation
- 29:13in the
- 29:13metastatic
- 29:14community,
- 29:15like we're
- 29:15talking
- 29:15about
- 29:16things like
- 29:16ie And ce
- 29:17so
- 29:18the immuno
- 29:18embolization
- 29:19for those
- 29:19of you
- 29:19listening
- 29:20and chemo
- 29:20embolization,
- 29:21how is this
- 29:22different
- 29:22than
- 29:22hepzato kit.
- 29:23And
- 29:24what do you think
- 29:24are the
- 29:25risks,
- 29:25the data
- 29:26maybe
- 29:26the
- 29:26benefits
- 29:26okay.
- 29:27So immune
- 29:28embolization
- 29:28and chemo
- 29:29embolization
- 29:29are similar
- 29:30to that y90
- 29:31where
- 29:31we put that
- 29:32catheter
- 29:32in.
- 29:32And instead
- 29:33of infusing
- 29:33radioactive
- 29:34particles
- 29:34you give
- 29:35chemo
- 29:36laced
- 29:36particles
- 29:37or immuno
- 29:38laced
- 29:38particles.
- 29:39I would say
- 29:39two things.
- 29:40Number one,
- 29:41it compared
- 29:41to let's
- 29:42say hepzato
- 29:42kit,
- 29:43the efficacy
- 29:44in terms
- 29:45of tumor
- 29:45shrinkage
- 29:46is far
- 29:47lower
- 29:48and
- 29:49the side
- 29:49effects are
- 29:50far higher.
- 29:51So
- 29:51I think
- 29:51it's really
- 29:52fallen
- 29:52out of
- 29:53favor
- 29:53to do chemo
- 29:54embolization
- 29:55or immuno
- 29:55embolization
- 29:57with the
- 29:58arrival of
- 29:59hepzato
- 29:59kit.
- 30:02in our
- 30:02experience
- 30:04in patients
- 30:04undergoing
- 30:05chemo
- 30:05embolization
- 30:05or immuno
- 30:06embolization,
- 30:06it's
- 30:07not easy
- 30:07to
- 30:07tolerate.
- 30:08You're
- 30:09quite sick
- 30:10for the
- 30:10next
- 30:10several
- 30:11days
- 30:11afterwards,
- 30:12and you
- 30:12have to
- 30:13repeat the
- 30:13procedure
- 30:14time and
- 30:15time again,
- 30:15you know,
- 30:15several
- 30:16times.
- 30:17So what?
- 30:18We've
- 30:18always
- 30:19kind of,
- 30:20philosophically
- 30:21thought
- 30:21in our
- 30:22field
- 30:22in
- 30:22interventional radiology,
- 30:24which is
- 30:24really why
- 30:25I went
- 30:25into this
- 30:26field,
- 30:26is that
- 30:27I like
- 30:28to see
- 30:28patients
- 30:29go home
- 30:29that day
- 30:30and not
- 30:31be
- 30:32miserable
- 30:32in the hospital.
- 30:33That was
- 30:34kind
- 30:34of what
- 30:35drove me
- 30:35into this.
- 30:36and so
- 30:37when I hear
- 30:37about
- 30:37patients
- 30:38not doing
- 30:39well
- 30:39afterwards
- 30:40or feeling
- 30:40really
- 30:40sick,
- 30:41that
- 30:41kind of
- 30:42disheartened
- 30:42me.
- 30:42So
- 30:43the nice
- 30:44thing about
- 30:44the
- 30:44hepzato kit
- 30:45okay
- 30:45procedure,
- 30:46other than
- 30:47the
- 30:47recovery
- 30:47from the
- 30:48anesthesia,
- 30:48the recovery
- 30:49is actually
- 30:49quite
- 30:50rapid.
- 30:51same thing
- 30:51with the
- 30:51radioembolzation
- 30:52Same thing
- 30:53with
- 30:53thermal
- 30:53ablation.
- 30:54So
- 30:54I foresee
- 30:56that chemo
- 30:57embolization
- 30:58and immuno
- 30:59embolization
- 31:00will
- 31:01probably
- 31:02fall out
- 31:03of favor
- 31:03significantly.
- 31:05as hepzato
- 31:06kit becomes
- 31:08more
- 31:08widespread
- 31:09use and
- 31:10widely
- 31:10available.
- 31:11Perhaps it
- 31:12might
- 31:12be used
- 31:13in those
- 31:14patients
- 31:14where,
- 31:15the hepzato
- 31:16kit
- 31:16procedure
- 31:16failed or,
- 31:17you know,
- 31:18patients
- 31:18progressed
- 31:18after it
- 31:19and
- 31:19they didn't
- 31:19have
- 31:20any other options.
- 31:21and so
- 31:22hopefully
- 31:22we really
- 31:22won't
- 31:23see that
- 31:23too often.
- 31:24but I think
- 31:25it's going
- 31:25to be
- 31:25relegated
- 31:26to what
- 31:26I would
- 31:27say, the
- 31:27salvage
- 31:28setting.
- 31:29Okay.
- 31:30we've
- 31:30talked
- 31:30a little
- 31:31about
- 31:31the y90
- 31:31and we
- 31:32talked
- 31:32about some
- 31:32of these
- 31:32kind
- 31:33of like
- 31:33maybe one
- 31:34or like one
- 31:34side of
- 31:35the liver.
- 31:35It really
- 31:35just needs
- 31:36a little
- 31:36more
- 31:36directed
- 31:37treatment.
- 31:38Hepzato
- 31:38Kit, you've
- 31:39said,
- 31:39is a
- 31:39little bit
- 31:40more,
- 31:41heavily
- 31:41used
- 31:42right now,
- 31:42at least
- 31:43when
- 31:43you have
- 31:44a larger
- 31:45amount
- 31:45of tumor
- 31:46involvement
- 31:47or liver
- 31:47involvement.
- 31:47let's
- 31:48just say
- 31:48I've got 2
- 31:49or 3
- 31:49moderate
- 31:50sized
- 31:50tumors
- 31:50I can see
- 31:51on the
- 31:51imaging and
- 31:51my doctor
- 31:52is telling
- 31:52me, oh,
- 31:53there's
- 31:53kind of
- 31:53this
- 31:53peppering
- 31:54like
- 31:54there's,
- 31:55there's
- 31:55like all
- 31:55of these
- 31:56little tiny
- 31:56little
- 31:57specks
- 31:57everywhere
- 31:57that we're
- 31:58pretty
- 31:58sure,
- 31:58you know,
- 31:58just
- 31:59based on
- 31:59how it
- 31:59presents.
- 32:00This is
- 32:01more
- 32:01cancer.
- 32:02I'm
- 32:02just kind
- 32:02of breaking
- 32:02this down
- 32:03for
- 32:03those of
- 32:03you guys
- 32:03listening,
- 32:04just
- 32:04so that
- 32:04if you're
- 32:05imagining
- 32:05your scans,
- 32:05you can
- 32:06kind of
- 32:06picture
- 32:06what your
- 32:07doctor
- 32:07might be
- 32:07saying.
- 32:08what what
- 32:09would this
- 32:09look like
- 32:10as far as
- 32:11why
- 32:11has auto
- 32:11might be
- 32:12a better
- 32:12option?
- 32:13That's
- 32:13actually
- 32:14a quite
- 32:14a common
- 32:15occurrence.
- 32:15We see,
- 32:16you know,
- 32:16multiple
- 32:16lesions,
- 32:17but you see
- 32:17a lot
- 32:17of little
- 32:17dots on
- 32:18the MRI.
- 32:20It's always
- 32:20hard to
- 32:20say what
- 32:21those dots
- 32:21are.
- 32:21But
- 32:22we kind of
- 32:22know just
- 32:23historically
- 32:24that
- 32:24those dots
- 32:25are very
- 32:26small
- 32:26tumors
- 32:27in its infancy.
- 32:28So
- 32:28in that case,
- 32:29when we see
- 32:30that it
- 32:30makes
- 32:31more sense
- 32:31to do
- 32:32a more
- 32:32whole liver
- 32:33for global
- 32:34approach.
- 32:35And that's
- 32:35where I do
- 32:36think that
- 32:36hepzato
- 32:37kit, plays
- 32:37a role.
- 32:38In fact,
- 32:38the last
- 32:39two
- 32:39patients
- 32:40that we did
- 32:40have
- 32:41exactly
- 32:41that
- 32:42appearance.
- 32:43the nice
- 32:44thing
- 32:44is that,
- 32:44it really
- 32:46does.
- 32:46You know,
- 32:46when we're
- 32:47infusing
- 32:47the chemo
- 32:48with the
- 32:48chemo,
- 32:48keep in
- 32:48mind, is
- 32:49a liquid,
- 32:49right?
- 32:49Just like
- 32:50turning on
- 32:51that
- 32:51faucet,
- 32:51running it
- 32:52through
- 32:52that
- 32:52sponge,
- 32:53it'll
- 32:53fill up
- 32:54the whole sponge
- 32:54so
- 32:55the chemo
- 32:55will
- 32:55fill up
- 32:55the whole liver.
- 32:56So it'll
- 32:56get to
- 32:57all those
- 32:58tiny
- 32:58little dots
- 33:00as well as
- 33:00the
- 33:00big tumors.
- 33:01So that's
- 33:01that's
- 33:02really
- 33:02the
- 33:02advantage.
- 33:03The second
- 33:03advantage
- 33:04is this,
- 33:05when even
- 33:05despite
- 33:06doing a
- 33:06whole liver
- 33:07treatment
- 33:07with the
- 33:07hepzato kit
- 33:08okay,
- 33:09the risk
- 33:10of liver
- 33:11toxicity
- 33:12or liver
- 33:13damage
- 33:15is
- 33:15exceptionally
- 33:16low
- 33:17in all
- 33:18the patients
- 33:18that we
- 33:19have
- 33:19treated
- 33:19so far,
- 33:20we have
- 33:21measured
- 33:21all their
- 33:22liver
- 33:22enzymes,
- 33:23immediately
- 33:24prior
- 33:24to the procedure
- 33:25and then
- 33:25afterwards
- 33:26and then
- 33:26even two
- 33:27weeks
- 33:27afterwards
- 33:28and one
- 33:28month
- 33:28afterwards.
- 33:29And not
- 33:30a single
- 33:30person
- 33:31has had
- 33:32elevation
- 33:33of their
- 33:33liver
- 33:33function
- 33:34tests.
- 33:35So it's
- 33:36quite
- 33:36remarkable,
- 33:37almost
- 33:37surprising,
- 33:38that we're
- 33:39giving such
- 33:40a high dose
- 33:40of chemo
- 33:41right
- 33:41into their
- 33:42liver.
- 33:43And for
- 33:43whatever
- 33:44reason,
- 33:45the
- 33:45hepatocytes,
- 33:46the normal
- 33:47liver
- 33:47cells,
- 33:48are not
- 33:48reacting
- 33:49to it.
- 33:50and so
- 33:50the liver
- 33:51function
- 33:51remains
- 33:51completely
- 33:52preserved.
- 33:53So
- 33:53what does that
- 33:54mean?
- 33:54That means
- 33:54also that
- 33:56we can give
- 33:57this
- 33:58multiple
- 33:58all times
- 33:59if needed.
- 34:00And we can
- 34:00give this
- 34:01with
- 34:01immunotherapy
- 34:02or
- 34:02chemotherapy
- 34:03or
- 34:04tebentafusp
- 34:05And there's
- 34:05not going
- 34:05to be
- 34:06any
- 34:06significant
- 34:07interaction
- 34:08that we are
- 34:08aware of.
- 34:10Oh that's
- 34:10incredible.
- 34:10So a really
- 34:11sounds like
- 34:12there's
- 34:12a lot
- 34:12of benefits
- 34:13to the
- 34:14treatment
- 34:14landscape.
- 34:14And just
- 34:15kind of
- 34:15what
- 34:15you guys
- 34:16creatively,
- 34:17creatively
- 34:17can do,
- 34:18you know,
- 34:18as doctors,
- 34:19as
- 34:19interventional
- 34:20radiologists,
- 34:20you've got
- 34:21so much
- 34:21more to
- 34:21work with
- 34:22when you
- 34:23when you
- 34:23can trust
- 34:24that
- 34:24the liver
- 34:24is not
- 34:24going to
- 34:25be
- 34:25compromised.
- 34:26Yeah.
- 34:26It gives us
- 34:27a lot more,
- 34:29it gives us
- 34:29a lot
- 34:30more
- 34:30latitude
- 34:31in what
- 34:31we can do.
- 34:32And I would
- 34:33say even
- 34:34with this
- 34:34procedure,
- 34:35we're
- 34:35still just
- 34:35starting
- 34:36out.
- 34:36so far,
- 34:37the procedures
- 34:38have gone
- 34:38extremely
- 34:39smoothly.
- 34:40the
- 34:40procedure
- 34:41time
- 34:41ranging
- 34:42from 2.5
- 34:43hours
- 34:44to three
- 34:45hours.
- 34:46Patients
- 34:46all getting
- 34:47discharged
- 34:48at the end
- 34:48of the day
- 34:49or the
- 34:49next morning.
- 34:50But I
- 34:51actually
- 34:51foresee
- 34:52several
- 34:53technical
- 34:53improvements
- 34:54for the
- 34:54procedure
- 34:54over
- 34:55the next
- 34:55two years
- 34:56where
- 34:56we can
- 34:56make it
- 34:56even more
- 34:58efficient
- 34:59as we share
- 35:00and learn
- 35:01our ideas.
- 35:02I'll
- 35:03give you
- 35:03one
- 35:03example.
- 35:04initially
- 35:05the procedure
- 35:05was done
- 35:06in a
- 35:06clinical
- 35:06trial
- 35:07where
- 35:07the all
- 35:07these
- 35:07catheters
- 35:08and sheaths
- 35:10were left
- 35:10in place
- 35:11and
- 35:11in coming
- 35:12out
- 35:12of the body
- 35:13while they
- 35:14were
- 35:14recovering
- 35:14in the ICU.
- 35:16and what we
- 35:17did is one
- 35:18small
- 35:18adaptation
- 35:19that we did
- 35:19is before
- 35:20they even wake
- 35:20up, we
- 35:20remove
- 35:21them.
- 35:21We use
- 35:22something
- 35:22called
- 35:22a closure
- 35:23device
- 35:23to close
- 35:24the
- 35:24arteries
- 35:24and veins.
- 35:25and
- 35:25then they
- 35:26get a band
- 35:26aid
- 35:26afterwards.
- 35:27So now
- 35:28it's nice
- 35:29that
- 35:29you wake
- 35:29up.
- 35:30You don't have
- 35:31anything
- 35:31sticking
- 35:31out
- 35:31of your body
- 35:32except just
- 35:32maybe
- 35:33in your hand.
- 35:34and so
- 35:34that's just
- 35:35a small
- 35:35technical
- 35:36improvement
- 35:36that
- 35:36we did.
- 35:37And I
- 35:37foresee
- 35:37several
- 35:38improvements,
- 35:39that will
- 35:39occur over
- 35:40the next
- 35:40two years.
- 35:42Well,
- 35:42and I
- 35:42think
- 35:42that's,
- 35:43you know,
- 35:43that's
- 35:43the beauty
- 35:44of medicine
- 35:44is when
- 35:44we have
- 35:45modern
- 35:45medicine
- 35:46and it's
- 35:46really been
- 35:47some
- 35:47established
- 35:48care
- 35:48of some
- 35:48kind, like
- 35:49there is so
- 35:49much
- 35:50more room
- 35:50for rapid
- 35:51improvements.
- 35:51And,
- 35:52and I know
- 35:52you guys
- 35:52as doctors,
- 35:53that's
- 35:53your goal.
- 35:53Like
- 35:54your goal
- 35:54is to
- 35:54make sure
- 35:55patients
- 35:55have
- 35:56as good
- 35:56of an
- 35:56experience
- 35:57as possible
- 35:57and can get
- 35:58back to
- 35:59regular,
- 35:59you know,
- 36:00regular
- 36:00life
- 36:00programing,
- 36:01so to
- 36:01speak.
- 36:02Yeah,
- 36:02absolutely.
- 36:02And that's
- 36:03something
- 36:03that we
- 36:03always have
- 36:04in the back
- 36:04of our
- 36:04minds.
- 36:05And
- 36:05any time
- 36:06let's say
- 36:06I do this
- 36:07or any
- 36:07other
- 36:07procedure,
- 36:08we do
- 36:09a, sit down
- 36:10consultation,
- 36:10either
- 36:11in person
- 36:11or via
- 36:12video
- 36:12or
- 36:12whatever,
- 36:13you know,
- 36:13the
- 36:13patient's
- 36:13preferences.
- 36:14And
- 36:14we go over
- 36:15their
- 36:15goals,
- 36:15we go over,
- 36:16you know,
- 36:16their needs
- 36:17and what
- 36:17their wants,
- 36:18and
- 36:19you know
- 36:20how
- 36:20aggressive
- 36:21they want
- 36:21to be.
- 36:23you know,
- 36:23and I
- 36:24try to
- 36:24get a
- 36:25very good
- 36:25gauge
- 36:26of their
- 36:26current
- 36:27physical
- 36:27status.
- 36:28In
- 36:28other words,
- 36:29are you
- 36:29sitting on
- 36:29your couch
- 36:30all day?
- 36:30Are you
- 36:30in bed
- 36:31all day?
- 36:31Are you
- 36:32working
- 36:32out?
- 36:33The last person
- 36:34I treated,
- 36:34he was
- 36:35in the gym
- 36:36lifting
- 36:36weights
- 36:37two hours
- 36:37a day
- 36:37before
- 36:37the procedure.
- 36:38And
- 36:38fortunately
- 36:39it two
- 36:39weeks later
- 36:39he was back at it.
- 36:42and so
- 36:42we want to
- 36:43get that
- 36:43gauge
- 36:43because I
- 36:44want to
- 36:44return
- 36:44people back
- 36:46to their
- 36:47previous
- 36:47lifestyle.
- 36:49Yeah.
- 36:49It's just
- 36:49I mean,
- 36:50I know
- 36:50that's
- 36:50something
- 36:51that's so
- 36:51important
- 36:51to me
- 36:51personally,
- 36:52and I hope
- 36:52you guys
- 36:53like, can
- 36:53maybe
- 36:54you can
- 36:54relate
- 36:54that.
- 36:54Like,
- 36:54you don't
- 36:55want cancer
- 36:55to take
- 36:56anything
- 36:56more
- 36:56from you
- 36:57than it
- 36:57has to.
- 36:58And so it's
- 36:58so it's
- 36:59so
- 36:59validating.
- 37:00I think, to
- 37:00hear that
- 37:01doctors
- 37:01are
- 37:01understanding
- 37:02that
- 37:02they're
- 37:02value
- 37:02in that
- 37:03and that,
- 37:04you know,
- 37:04that we
- 37:04have
- 37:04professionals
- 37:05like you
- 37:05who are
- 37:06looking for
- 37:06how do I make
- 37:07sure
- 37:07my patients
- 37:08have
- 37:08the best
- 37:08quality
- 37:09of life
- 37:09and
- 37:09the most
- 37:10longevity,
- 37:11like,
- 37:11you know,
- 37:11as they're
- 37:12dealing
- 37:12with
- 37:12metastatic
- 37:12disease?
- 37:14you know.
- 37:14I would say
- 37:14that's
- 37:15not done.
- 37:15I
- 37:15unfortunately,
- 37:16it's
- 37:16not universal.
- 37:17I think
- 37:18especially
- 37:18in our field,
- 37:19it tends
- 37:19to be much
- 37:20more
- 37:20common.
- 37:21I know
- 37:21I've had,
- 37:22family
- 37:23members
- 37:23who have,
- 37:24gone
- 37:24through
- 37:25some pretty
- 37:25tough times
- 37:26in terms
- 37:26of their
- 37:27cancer
- 37:27care,
- 37:28and had,
- 37:29you know,
- 37:29I still
- 37:30still
- 37:30remember
- 37:30my
- 37:31grandmother
- 37:31who had
- 37:32chemotherapy
- 37:33for breast
- 37:33cancer
- 37:33at 85.
- 37:34And it
- 37:35you know,
- 37:35it took
- 37:35a very
- 37:36vital
- 37:37person
- 37:37and
- 37:37basically
- 37:38put her
- 37:39into bed,
- 37:39you know,
- 37:40within weeks.
- 37:40And she
- 37:41really
- 37:41never
- 37:41kind of
- 37:42came out
- 37:42of that.
- 37:43And that's
- 37:43still
- 37:44that was
- 37:44ten years
- 37:45ago.
- 37:45That still
- 37:45sticks in
- 37:46my head.
- 37:47And I think
- 37:49not only
- 37:49myself,
- 37:50but I think
- 37:50most in the
- 37:51radiology
- 37:51community,
- 37:52actually,
- 37:53most ocular
- 37:54melanoma
- 37:54oncologists
- 37:55I work
- 37:55with, feel
- 37:55the same
- 37:56way.
- 37:56We're
- 37:56trying to
- 37:57avoid that,
- 37:58and trying
- 37:59to preserve
- 37:59people's
- 38:00quality
- 38:00of life
- 38:01as much
- 38:01as possible.
- 38:02and I think
- 38:03that's,
- 38:03you know,
- 38:04an equal
- 38:04goal in
- 38:05terms of,
- 38:06comparing
- 38:06to quantity
- 38:07of life
- 38:07as well.
- 38:08Yeah.
- 38:09Well,
- 38:10and you
- 38:10mentioned
- 38:10too
- 38:11that like,
- 38:11this is
- 38:11the kind
- 38:11of
- 38:12treatment
- 38:12that you
- 38:12can have
- 38:13multiple
- 38:13times.
- 38:14So like
- 38:14let's just,
- 38:15you know,
- 38:15let's
- 38:15just say
- 38:16I've got
- 38:16this, this,
- 38:16you know,
- 38:17this, this,
- 38:18this
- 38:18presentation
- 38:19of all of
- 38:19these
- 38:19tumors
- 38:20in my
- 38:20liver.
- 38:20It's
- 38:20obviously
- 38:21super
- 38:21concerning.
- 38:22and you're
- 38:22telling me
- 38:23about,
- 38:23okay, it's
- 38:23a
- 38:23really good
- 38:24option.
- 38:24So how many
- 38:25treatments
- 38:26could
- 38:26I
- 38:26realistically
- 38:26have,
- 38:27you know,
- 38:27without it
- 38:28producing
- 38:29side
- 38:29effects
- 38:30or things
- 38:30like that?
- 38:31Okay.
- 38:31So great
- 38:32question.
- 38:32So I think
- 38:33right now
- 38:34we don't
- 38:34truly know.
- 38:35But in
- 38:36general
- 38:36we're
- 38:37looking at
- 38:37around six.
- 38:38So
- 38:38what does
- 38:39six
- 38:39treatments
- 38:39mean.
- 38:40We do
- 38:40these in
- 38:41groups
- 38:41of two.
- 38:42So
- 38:42we do one.
- 38:43And then
- 38:43we do
- 38:43another one
- 38:446 to 8
- 38:44weeks
- 38:44later.
- 38:45And then
- 38:46we get
- 38:46imaging
- 38:47afterwards.
- 38:48In general
- 38:49it tends
- 38:50to be
- 38:51more more
- 38:51tolerated
- 38:52with each
- 38:53treatment.
- 38:54As an
- 38:54example,
- 38:55I saw
- 38:55somebody
- 38:56last week
- 38:56in
- 38:56the office
- 38:57after
- 38:58they
- 38:58received
- 38:58their first
- 38:59two
- 38:59treatments,
- 39:01their
- 39:01tumors
- 39:02shrunk
- 39:02considerably.
- 39:04So
- 39:04I sat down
- 39:05with them.
- 39:05I said,
- 39:06look, here
- 39:06are your options.
- 39:07We can
- 39:08either just
- 39:09you can
- 39:09continue
- 39:10your
- 39:10immunotherapy.
- 39:11You can get
- 39:11scans every
- 39:12few months,
- 39:13or I can do
- 39:14rounds
- 39:15three
- 39:15and four.
- 39:16I've have
- 39:16decided to
- 39:17shrink them
- 39:17even
- 39:18further.
- 39:19It really
- 39:20depends on
- 39:21how you did
- 39:21after
- 39:22the last
- 39:22two.
- 39:22If you
- 39:23didn't
- 39:23tolerate
- 39:23the last two,
- 39:24I probably
- 39:24would
- 39:24hold off.
- 39:25If you tolerated
- 39:25the last
- 39:26two,
- 39:26I'd go
- 39:27for it.
- 39:27He's like,
- 39:28well,
- 39:28the last one
- 39:29I had on
- 39:29a
- 39:29Wednesday.
- 39:30I was back
- 39:30at work
- 39:30Monday
- 39:31morning.
- 39:31I don't
- 39:31really feel
- 39:32any different,
- 39:33so let's
- 39:34just do it.
- 39:35So he's
- 39:35going
- 39:35to get
- 39:36scheduled
- 39:36for rounds
- 39:37three
- 39:37and four
- 39:39because
- 39:39he's
- 39:39totally up
- 39:40for it.
- 39:41there are certain
- 39:42other
- 39:43considerations.
- 39:43We want
- 39:44to make
- 39:44sure that
- 39:44they
- 39:45continue
- 39:45to not have
- 39:46heart
- 39:46disease or
- 39:46develop it.
- 39:47the one
- 39:48main side
- 39:49effect of
- 39:50the Hepsato
- 39:50kit
- 39:50okay,
- 39:50procedure
- 39:51is that it
- 39:52can lower
- 39:53one of
- 39:53your blood
- 39:54counts.
- 39:55That's
- 39:55called the
- 39:55white blood
- 39:56cell count.
- 39:57So we want
- 39:58to make
- 39:58sure your
- 39:58white blood
- 39:59cell count
- 39:59is within
- 40:00an normal
- 40:01range.
- 40:02So
- 40:02sometimes
- 40:03we'll wait
- 40:03another
- 40:04few months
- 40:04to make
- 40:05sure it
- 40:05comes back
- 40:06to normal
- 40:06range.
- 40:07It tends
- 40:07to not be a
- 40:08significant
- 40:09problem.
- 40:10so we want
- 40:10to
- 40:10make sure
- 40:10that you're
- 40:11still safe
- 40:11for
- 40:11the procedure.
- 40:12And I
- 40:13basically
- 40:13I ask them
- 40:14if they're up
- 40:15for it,
- 40:16and they
- 40:16want to
- 40:16go ahead.
- 40:18Okay.
- 40:18Well, I
- 40:19mean,
- 40:19I think
- 40:19that's
- 40:19fair.
- 40:19And I
- 40:20think it's
- 40:20so
- 40:20important
- 40:21that we,
- 40:21as patients
- 40:22have the
- 40:22opportunity
- 40:22to be
- 40:23involved in
- 40:24the
- 40:24decision
- 40:24making of like,
- 40:25what do I want?
- 40:26so I think
- 40:27that's a
- 40:27that's a
- 40:27really
- 40:27good thing
- 40:28to find for
- 40:28you guys.
- 40:30okay.
- 40:30So around
- 40:32six
- 40:32treatments
- 40:33in
- 40:33groups of two.
- 40:34And then
- 40:34you
- 40:34mentioned
- 40:35that I
- 40:35could
- 40:35still have
- 40:36my systemic
- 40:36therapy.
- 40:37Yeah.
- 40:37So
- 40:38the kind of
- 40:38main
- 40:39options
- 40:39we've
- 40:39got for
- 40:40the
- 40:40systemic
- 40:40therapy are
- 40:41ipi nivo,
- 40:42which is
- 40:42generally
- 40:43for anyone
- 40:43but we know
- 40:44doesn't
- 40:44have or
- 40:45historically
- 40:45hasn't
- 40:46had the
- 40:46best
- 40:46response
- 40:47rate as far
- 40:47as I know.
- 40:48And then
- 40:48we also
- 40:49have the
- 40:49HLA
- 40:49dependent
- 40:50Kimmtrack
- 40:50for, for
- 40:51anybody
- 40:51who,
- 40:52you know,
- 40:52maybe
- 40:52you're
- 40:52confused
- 40:53about
- 40:53Kimmtrack
- 40:54versus tebn
- 40:54They are
- 40:55the same thing.
- 40:56so let's
- 40:56just say
- 40:57I am I'm
- 40:57hla
- 40:57positive
- 40:58I can have
- 40:58Kimmtrack.
- 40:59or I can
- 41:00have ipi
- 41:00nivo like,
- 41:02how soon do
- 41:02I have to
- 41:03kind of
- 41:04pause
- 41:04my
- 41:04immunotherapy
- 41:05before
- 41:06I can
- 41:07have one
- 41:07of the
- 41:07hepzato
- 41:08treatments,
- 41:08or vice
- 41:09versa?
- 41:10From what
- 41:11we can
- 41:11tell, you
- 41:12don't
- 41:12need to
- 41:12pause it
- 41:12at all.
- 41:13we have treated
- 41:14patients
- 41:15both on
- 41:15Kimmtrack,
- 41:16and we have
- 41:17treated
- 41:17patients
- 41:18both on
- 41:19nivolumab
- 41:19We've also
- 41:20treated
- 41:20patients
- 41:21on other
- 41:21third line,
- 41:22immuno
- 41:23agents
- 41:23as well.
- 41:25there does
- 41:25not seem
- 41:26to be
- 41:26any cross
- 41:27reaction
- 41:28in terms of
- 41:28the side
- 41:29effects.
- 41:30and so
- 41:32we have
- 41:33recommended
- 41:34that you
- 41:35do not stop
- 41:36or even
- 41:37temporarily
- 41:38discontinue
- 41:39your systemic
- 41:40therapy.
- 41:41That's
- 41:42kind of
- 41:42the nice
- 41:42thing
- 41:42about this.
- 41:43There's
- 41:43no
- 41:43it doesn't,
- 41:44you know,
- 41:44add to
- 41:45the side
- 41:45effect
- 41:46profile
- 41:46of what
- 41:47you already have.
- 41:49So I don't
- 41:49really
- 41:49foresee
- 41:50any reason,
- 41:52to stop
- 41:53your
- 41:53systemic
- 41:54therapy
- 41:54at this
- 41:55time.
- 41:56There is
- 41:56an ongoing
- 41:57trial
- 41:58in Europe
- 41:58called
- 41:58a Chopin
- 41:59trial of
- 42:00hepzato kit
- 42:00plus
- 42:02immunotherapy.
- 42:03The results
- 42:03should
- 42:04there's
- 42:04some prelim
- 42:05results
- 42:05right now,
- 42:06but they're
- 42:06very,
- 42:06very prelim
- 42:07and
- 42:07the final
- 42:08results
- 42:08should be out.
- 42:09I would say
- 42:10shortly,
- 42:10perhaps,
- 42:11you know,
- 42:11later
- 42:12this year,
- 42:13which will
- 42:13confirm,
- 42:14hopefully
- 42:15confirm
- 42:15the safety
- 42:16of,
- 42:17let's say
- 42:17Nebo
- 42:18and ipi
- 42:18with
- 42:19hepzato
- 42:19kit.
- 42:19Enough.
- 42:20But
- 42:20honestly,
- 42:20I don't
- 42:20foresee
- 42:21any issues.
- 42:22Okay.
- 42:22Awesome.
- 42:23and just
- 42:24just for,
- 42:24you know,
- 42:25brief
- 42:25reference
- 42:26for anybody
- 42:26who's
- 42:26looking
- 42:27for, like,
- 42:27the data
- 42:27and the
- 42:28studies
- 42:28around
- 42:28Hepzato
- 42:29kit itself
- 42:29and like,
- 42:30the efficacy
- 42:30and how
- 42:31well it
- 42:31works.
- 42:32what where
- 42:33where could
- 42:33somebody
- 42:34find some
- 42:34of that
- 42:34good data
- 42:35on, on
- 42:36hepzato kit
- 42:37So the best
- 42:37data is a
- 42:38recently
- 42:39released
- 42:39trial
- 42:39that was
- 42:40published
- 42:41in the
- 42:41Journal of
- 42:41Surgical
- 42:42Oncology,
- 42:43in this
- 42:43past
- 42:44spring.
- 42:45It is
- 42:45called the
- 42:46Focus
- 42:47trial.
- 42:48efficacy U.
- 42:49S
- 42:50and
- 42:50the first
- 42:51author is
- 42:52Doctor
- 42:52Zager
- 42:53z a g e r
- 42:55who is a
- 42:55surgical
- 42:56oncologist
- 42:56at of
- 42:57Moffitt
- 42:57in Florida.
- 42:58So he led
- 42:59he was the
- 43:00primary
- 43:00investigator
- 43:01on this
- 43:01multicenter,
- 43:02international
- 43:03trial,
- 43:04of hepzato
- 43:05kit.
- 43:05This is the trial
- 43:06that was
- 43:07presented
- 43:08at a
- 43:09oncology
- 43:09conference
- 43:10in 2023,
- 43:11which then
- 43:12resulted in
- 43:13the FDA
- 43:14approval
- 43:15in the
- 43:15United States.
- 43:16And that's
- 43:17amazing.
- 43:17Okay.
- 43:17Well,
- 43:18we'll
- 43:18make sure
- 43:18that we
- 43:18link that
- 43:19and we'll
- 43:19share that
- 43:20in
- 43:20the focus
- 43:20trial.
- 43:21just
- 43:21so that
- 43:21you guys
- 43:22can see
- 43:22the
- 43:22breakdown,
- 43:23because
- 43:23it's
- 43:23amazing
- 43:24how like,
- 43:24just how
- 43:25much
- 43:25stronger of
- 43:26an efficacy
- 43:26that happens
- 43:27out. Okay.
- 43:27It shows
- 43:28in the focus
- 43:28trial.
- 43:28So
- 43:29just as we
- 43:29end,
- 43:30doctor,
- 43:30doctor
- 43:31Polya,
- 43:31where do
- 43:32you see
- 43:32the future
- 43:32going
- 43:33for out?
- 43:33Okay,
- 43:33now
- 43:34that we're
- 43:34just a
- 43:34year out
- 43:35from FDA
- 43:35approval
- 43:35from last
- 43:36summer.
- 43:38So I see
- 43:39a couple
- 43:39of things.
- 43:39Number one,
- 43:40in
- 43:40the short
- 43:41term,
- 43:41I see
- 43:41several
- 43:42technical
- 43:42improvements,
- 43:43that we can
- 43:44potentially
- 43:44make,
- 43:45to make
- 43:46the
- 43:46procedure
- 43:47easier and
- 43:48faster and,
- 43:49more
- 43:50adaptable
- 43:50to other
- 43:51institutions
- 43:52as well.
- 43:53One of
- 43:53the
- 43:53things
- 43:53I didn't
- 43:53mention is
- 43:54it does
- 43:54take a
- 43:55good amount
- 43:55of
- 43:55coordination
- 43:56between
- 43:57intervention,
- 43:57radiology,
- 43:58anesthesia
- 43:59and a
- 44:00perfusion
- 44:00team which
- 44:01handles
- 44:02the
- 44:02filtration
- 44:03of
- 44:03the medication.
- 44:03So a large
- 44:04part of us
- 44:05setting
- 44:05this up
- 44:06at our center
- 44:06at UCLA,
- 44:07we spent
- 44:08about
- 44:08six months
- 44:08prepping
- 44:09this,
- 44:10between
- 44:10the three teams
- 44:11so we could
- 44:11work,
- 44:12seamlessly
- 44:13in the room
- 44:14and the
- 44:15room.
- 44:15For
- 44:15example,
- 44:15there's
- 44:16about 10 to
- 44:1712
- 44:17different
- 44:17personnel,
- 44:18working
- 44:19on this
- 44:19one
- 44:19patient.
- 44:20And so
- 44:20we want
- 44:21to make
- 44:21sure that
- 44:22everything
- 44:22goes well,
- 44:23seamlessly,
- 44:24especially
- 44:24from a
- 44:25communication
- 44:25standpoint.
- 44:27so I think
- 44:27that we
- 44:27can make
- 44:28a lot
- 44:28of
- 44:28improvements
- 44:29in that,
- 44:29in terms
- 44:30of the
- 44:30technical
- 44:30know how
- 44:31and
- 44:31capabilities
- 44:32of the
- 44:32procedure.
- 44:33there's
- 44:33probably
- 44:33some
- 44:34improvements
- 44:34we can make
- 44:34in
- 44:35the filter
- 44:35as well
- 44:36to change
- 44:37or vary
- 44:38how much of
- 44:39the chemo
- 44:40gets
- 44:40withdrawn.
- 44:41we need to
- 44:43further
- 44:43refine the
- 44:44patient
- 44:45population.
- 44:46I would
- 44:46love to
- 44:47figure out
- 44:48in the next
- 44:493
- 44:49to 4 years
- 44:51who it's
- 44:51going to
- 44:51work in
- 44:52and
- 44:52who it's not,
- 44:53because
- 44:53what
- 44:53I would
- 44:53like to do
- 44:54is see
- 44:55this person
- 44:56in
- 44:56my office
- 44:57before
- 44:57the
- 44:57procedure
- 44:58and say,
- 44:59this is
- 44:59going to
- 44:59work
- 45:00in you,
- 45:00or even
- 45:01conversely,
- 45:02this is not
- 45:03going to
- 45:03work
- 45:03in you.
- 45:04I want to
- 45:04offer
- 45:04something
- 45:05else.
- 45:05right now,
- 45:07we kind of
- 45:08offer this
- 45:08in all
- 45:09patients
- 45:09with
- 45:10multifocal
- 45:11tumors
- 45:11throughout
- 45:12their
- 45:12liver.
- 45:13It doesn't
- 45:14work in
- 45:14everyone.
- 45:14And
- 45:15we would love
- 45:15I
- 45:15would love
- 45:16to figure
- 45:16this out
- 45:17as to
- 45:17who it's
- 45:17going
- 45:18to work
- 45:18in, who
- 45:18it's not
- 45:19in a way
- 45:20similar to
- 45:21tebentafusp
- 45:22in a
- 45:22certain
- 45:22HLA
- 45:23subtype.
- 45:23We know
- 45:24it works
- 45:24in those
- 45:24patients.
- 45:25It does not
- 45:25work in
- 45:26the other
- 45:26HLA
- 45:27subtype.
- 45:27So,
- 45:28if I could
- 45:28figure out
- 45:29a marker
- 45:29or
- 45:30some kind
- 45:31of way
- 45:33to
- 45:33prognosticate
- 45:35people
- 45:35to say that
- 45:36it's
- 45:36going to be
- 45:37a success
- 45:37or it's
- 45:38not going
- 45:38to be
- 45:38a success,
- 45:39I think
- 45:40that's the
- 45:40next option
- 45:42or the
- 45:43next,
- 45:43advancement
- 45:44for this
- 45:45therapy.
- 45:46Now, I
- 45:46love that.
- 45:48Well,
- 45:48Doctor
- 45:48padia
- 45:49this has
- 45:49been
- 45:49incredible.
- 45:50Thank you
- 45:50so much
- 45:51for joining
- 45:51us today.
