- 0:00This is Laura Dierda with the Becker's Healthcare
- 0:02podcast. I'm thrilled today to be joined by
- 0:04doctor Isaac Sachemetchi,
- 0:06who is a physician in endocrinology,
- 0:08diabetes, and metabolism,
- 0:11at NYC Health and Hospitals Queens. Doctor Sachemes,
- 0:14it's a pleasure to have you on the
- 0:15podcast today.
- 0:17Thank you for inviting me.
- 0:21Absolutely. Now I know we've got a lot
- 0:23to talk about November being,
- 0:25diabetes awareness month. And so I'm wondering, can
- 0:27you tell us a little bit more about
- 0:29your program at NYC Health and Hospitals and
- 0:32some of the cool things that you're doing
- 0:34there to really,
- 0:36meet the needs of your patient population, especially
- 0:38those with diabetes?
- 0:39Excellent.
- 0:41Okay. So
- 0:42we have diabetes center of excellence, and I'm
- 0:44the director of diabetes center of excellence.
- 0:49We are recognized by American Diabetes Association,
- 0:54and we are going to most likely apply
- 0:57for pre recognition,
- 1:00by July
- 1:022026.
- 1:05We have the most diverse
- 1:07diabetic population in the Queens area,
- 1:10and the highest rate of
- 1:13prevalence of diabetes in Queens is about thirteen
- 1:17percent,
- 1:18more than even the New York City area.
- 1:23And the most diverse ethnic group of population
- 1:26of diabetic,
- 1:28You see,
- 1:29about, I would say, eighty percent of this
- 1:32diabetic are either overweight
- 1:35or,
- 1:36obese.
- 1:37So we not only treat the diabetes, but
- 1:40treat the obesity.
- 1:43We try to use
- 1:45the best modalities of therapy that available currently
- 1:48in the market
- 1:50for management of diabetes.
- 1:52We in the diabetes center of excellence, we
- 1:55have a nutritionist.
- 1:57We have diabetes
- 1:58educator.
- 2:01We have a team of,
- 2:04four, four physician
- 2:06and,
- 2:07two nurse practitioners,
- 2:09nutritionist, diabetes
- 2:11educator.
- 2:12We are very
- 2:14adjacent
- 2:15in,
- 2:16localization
- 2:18of the diabetes center is adjacent to
- 2:21to the ophthalmology
- 2:22clinic that we can refer the patient for
- 2:25retinal exam that is essential
- 2:28to for the diagnosis of diabetic
- 2:30eye disease.
- 2:32We have podiatrist
- 2:34that that running clinic
- 2:36in the diabetes,
- 2:38clinic
- 2:39and, seeing
- 2:41different
- 2:42problem of wound care and
- 2:45podiatric
- 2:45problem that is common in
- 2:48diabetic patient.
- 2:49We
- 2:51have classes for diabetes
- 2:53education
- 2:54at the diabetes center.
- 2:57And, we try to also,
- 3:00to get the best,
- 3:03modalities
- 3:04of,
- 3:07measuring blood sugar, like continuous glucose monitoring
- 3:11to the patient that we can provide if
- 3:14their,
- 3:16insurance
- 3:17cover that,
- 3:18to get a better glycemic control in those
- 3:21patient.
- 3:22Not only diabetes care is important, but,
- 3:26also the complication of diabetes, like hypertension,
- 3:31hyperlipidemia.
- 3:32So when we see diabetic patient,
- 3:34we address,
- 3:35both glycemic control, the blood pressure,
- 3:38the the dyslipidemia
- 3:40that they have, and,
- 3:42address all those problem with appropriate management.
- 3:47A really excellent team of physician,
- 3:51that really they have a big knowledge of
- 3:54diabetes care.
- 3:56We have the diabetes,
- 3:58nutritionist, diabetes educator
- 4:01that they are excellent in in teaching and
- 4:05giving a nutritionist
- 4:07advice to this patient.
- 4:09And,
- 4:10also to nurse practitioners
- 4:12that working with me for several years,
- 4:15and,
- 4:16one of them became actually also certified diabetes
- 4:19educator
- 4:20that help our diabetes educator
- 4:23that currently working with us.
- 4:25So I think it's one of the center
- 4:28of excellence in Queens area that recognized by
- 4:32American Diabetes Association.
- 4:36Oh, that's really impressive to hear and amazing
- 4:38that you've got such talent on your team
- 4:40there, especially to serve an important patient population.
- 4:44And given,
- 4:45the rates of obesity
- 4:47and diabetes that you mentioned already within the
- 4:49community,
- 4:50it seems like an extremely needed service.
- 4:53Now
- 4:54Yeah. I have to mention also
- 4:55that we have a clinical research unit
- 4:58in the diabetes center of excellence at,
- 5:01health and hospital Queens. I have a clinical
- 5:04research coordinator
- 5:06and few volunteers
- 5:08that helping her, and we are running
- 5:10phase three, phase four clinical studies
- 5:13that's sponsored by pharmaceutical companies. So patient that
- 5:17attend our diabetes clinic also,
- 5:20they have the option
- 5:22to participate,
- 5:24in those studies. Some of them is a
- 5:26management of weight, but an obesity study with
- 5:30or post
- 5:33bariatric surgery, hypoglycemia
- 5:35that we are running now.
- 5:37We do other studies in management of diabetes
- 5:40also.
- 5:41And patient can really participate in those,
- 5:44clinical studies.
- 5:48That's great to hear. And, having such an
- 5:50eye on innovation in the clinical studies I
- 5:52know push the field forward in a a
- 5:55meaningful way. I'm curious,
- 5:57when you look at the different research that's
- 5:59been do done as well as, the achievements
- 6:02and accomplishments that you've,
- 6:04gotten in recognition
- 6:05for the diabetes center, What makes it really
- 6:08special and unique? What are some of the
- 6:10elements you've been able to build over time
- 6:12that truly,
- 6:14ensure you're giving the best quality care to
- 6:16patients possible and you can achieve these types
- 6:18of recognitions?
- 6:19Yes. So they're putting together
- 6:22really the ancillary services that we need
- 6:26for management of diabetic patient,
- 6:28having the diabetes center of excellence
- 6:31that has nutritionist,
- 6:33test diabetes educator,
- 6:36and providers
- 6:37that they are knowledgeable in management of diabetes,
- 6:40I think made us
- 6:43one of the center of excellence. Actually, I
- 6:45did a study
- 6:47that we collected data about,
- 6:50you know, intervention
- 6:52of patient in diabetes center compared to
- 6:56medical clinic, and we
- 6:58we compare our outcome data to Joslin clinic
- 7:02that is one of the best
- 7:04diabetes center of excellence. And we shout that,
- 7:07actually,
- 7:08we are very, very,
- 7:10comparable to Joslin clinic in term of achieving
- 7:14data,
- 7:15a one c reduction,
- 7:17and improving quality of care of patient.
- 7:23And that's great to hear. And it's certainly,
- 7:27awesome that you're able to put all that
- 7:28together to have those ancillary services and be
- 7:31able to manage patients,
- 7:33in in a meaningful way. I'm curious. When
- 7:35you look into the future, what are you
- 7:36most excited about? What's coming down the pipe
- 7:39that could really be a a continued differentiator
- 7:41for the care of diabetic patients? You see,
- 7:44having having,
- 7:45continuous glucose monitoring
- 7:48that help us to get a better control
- 7:50of diabetes.
- 7:51And, you know, the future is coming with
- 7:53these all these,
- 7:57insulin pumps,
- 7:59the autonomous,
- 8:00insulin delivery system
- 8:02that will really help us to
- 8:05really,
- 8:07better take care of patient with,
- 8:09diabetes who requiring insulin.
- 8:12That's, fantastic.
- 8:13In the pipeline
- 8:15of medication, there are
- 8:17other medication that
- 8:19will most likely will be approved in close
- 8:21future, 2006,
- 8:232007,
- 8:24that,
- 8:25making combination of,
- 8:28hormones like GLP one, GIP,
- 8:31amylin,
- 8:32and glucagon
- 8:34that, they are promising in term of getting
- 8:37better reduction,
- 8:39of weight in this,
- 8:41diabetic population,
- 8:43especially the type two that they are overweight
- 8:45or obese.
- 8:47And I think this is exciting future for,
- 8:51diabetes management.
- 8:55And that's great to hear. Now, from your
- 8:58perspective, is there anything you would like to
- 8:59share with other physicians or health care professionals,
- 9:03especially,
- 9:04knowing this is diabetes month, how they can
- 9:06think about,
- 9:07you know, what's happening in this space and
- 9:09what they need to do in order to
- 9:10make sure that patients with their in their
- 9:12own communities are really getting the best level
- 9:14of care possible?
- 9:15Yes. I think we should we should really
- 9:18have
- 9:19a a system
- 9:21educating the patient,
- 9:23about diabetes and its complication,
- 9:26educating patient that they are overweight and obese,
- 9:30that weight management
- 9:31is one of the essential,
- 9:34part of the management of diabetes.
- 9:37Because,
- 9:38when you achieve weight loss, you improve insulin
- 9:41sensitivity,
- 9:42improve glycemic control.
- 9:44And in addition, you improve
- 9:46the,
- 9:47blood pressure,
- 9:48control and and,
- 9:51lipid control in this patient.
- 9:53So I think it's
- 9:55very, very important to put the patient in
- 9:58the center
- 9:59of the therapy
- 10:00and provide them the tools
- 10:02that they can use really to achieve these
- 10:06goals.
- 10:09Absolutely. That's very well said.
- 10:11Well, thank you so much for your time
- 10:13today. This has been really, such a fascinating
- 10:15conversation.
- 10:16I appreciate it, doctor Sachemeji, and I look
- 10:18forward to connecting with you again soon. Thank
- 10:20you. Thank you very much for your time.
