- 0:02- There's a small circle of people who truly
- 0:04- understand what it takes to lead a health
- 0:05- system right now. The decisions are enormous.
- 0:08- The variables are constantly shifting, and the stakes
- 0:11- of getting a call wrong don't stay contained.
- 0:14- That's not a complaint. It's a structural reality.
- 0:17- And it's why the most effective c suite
- 0:19- leaders are deliberate about where they go to
- 0:21- pressure test their thinking.
- 0:22- Becker's fourteenth annual CEO and CFO roundtable

- 0:35- workforce strategy, and the leadership questions that don't
- 0:38- have easy answers.

- 0:50- This is Madeline with the Becker CFO and
- 0:53- revenue cycle podcast, and I'm thrilled to be
- 0:55- joined today by Saurabh Tapati,
- 0:57- CFO and executive vice president of Ascension. Saurabh,
- 1:01- thanks for joining me. Hey, Madeline. Thank you
- 1:03- for inviting me. It's a pleasure to be
- 1:05- on Becker's health care podcast.
- 1:08- Amazing. So before we kick things off, do
- 1:10- you mind sharing just a little bit about
- 1:12- yourself, your background on health care, and more
- 1:14- on Ascension with our listeners?
- 1:17- Certainly.
- 1:18- I've been with Ascension as CFO of Ascension
- 1:20- for two and a half years, and I
- 1:22- have the privilege of serving Ascension across
- 1:26- all finance functions, including m and a, treasury,
- 1:29- traditional finance and accounting,
- 1:31- Ascension ventures,
- 1:33- and Ascension investment functions.
- 1:35- Prior to this role, I was CFO of
- 1:37- Highmark Health, which is an integrated payer provider
- 1:41- network
- 1:42- based out of Pennsylvania.
- 1:44- And I was responsible for all finance functions
- 1:46- there for almost four and a half years.
- 1:49- And before that, I spent, almost,
- 1:52- my,
- 1:53- entire twenty five year career in health care,
- 1:57- a little bit in GE health care and
- 2:00- Fresenius health health care as well in Boston.
- 2:03- Thanks so much for that background. I really
- 2:05- appreciate it. So I wanna dive right in
- 2:08- with you here today. So in mid May,
- 2:11- Ascension released its third quarter results.
- 2:14- And so I was hoping you might be
- 2:15- able to just kick us off and share
- 2:18- how you're feeling, about the third quarter results
- 2:20- before we dive into the logistics there.
- 2:23- Yeah. Great question, Marlene. And if I may
- 2:25- just go back a little bit in history,
- 2:27- Ascension has been on a financial turnaround for
- 2:30- almost, two and a half, three years now.
- 2:32- Yeah. In the last two and a half
- 2:34- years, we have seen almost 2,600,000,000
- 2:37- of financial turnaround. This last quarter wasn't any
- 2:40- surprise
- 2:41- either.
- 2:42- We saw 426,000,000
- 2:45- improvement
- 2:45- in our net income, almost 218%
- 2:48- year over year improvement.
- 2:50- And this is frankly driven by consistent
- 2:53- operational improvement
- 2:55- through all our operating metrics
- 2:57- as well as mission focused,
- 3:00- clinical care that our clinicians
- 3:02- offer to our patients every single day.
- 3:05- Yeah. Thank you for that. And, actually got
- 3:08- right into the my next question I was
- 3:10- gonna ask you. So as you said, and
- 3:11- just again for our listeners here, third quarter
- 3:14- fiscal year twenty twenty six, four hundred and
- 3:17- twenty six million or 218%
- 3:19- year over year net income growth. Could you
- 3:22- dive a little deeper into what you feel
- 3:24- is really most responsible here for that turnaround
- 3:27- just to get some some more details there?
- 3:30- Yeah. If I could unpack the result a
- 3:31- little bit, Madeline. 400,
- 3:34- 26,000,000
- 3:35- of net income improvement
- 3:37- was a combination of 262,000,000
- 3:40- improvement in operating income,
- 3:42- which was driven by higher volume,
- 3:46- more acute patients coming to Ascension facilities,
- 3:49- a higher commercial mix that we are serving
- 3:52- across our footprint of roughly
- 3:54- a 100 or so hospitals.
- 3:57- And then we also saw,
- 3:59- really, truly,
- 4:01- good operational productivity
- 4:03- when it comes to contract labor management,
- 4:06- when it comes to length of stay improvement
- 4:09- in our hospitals,
- 4:10- when it comes to supplies
- 4:12- and,
- 4:13- drugs,
- 4:14- cost efficiency.
- 4:16- We have seen improvement in pretty much all
- 4:18- metrics across,
- 4:20- and that led to 262,000,000
- 4:22- in operating margin improvement.
- 4:25- We also benefited from strong investment markets,
- 4:28- and that helped our investment income come in.
- 4:31- So the combination of higher investment income and
- 4:35- operating income helped us with the 426,000,000
- 4:38- improvement year over year. Yeah. Thank you for
- 4:41- that breakdown there. And I I'm gonna look
- 4:43- at total operating revenue now. So that did
- 4:46- fall from 19,500,000,000
- 4:48- to 18,100,000,000
- 4:49- for the nine months ending March 31.
- 4:52- And, you know, Ascension has reduced its hospital
- 4:54- portfolio from and keep me honest here, from
- 4:56- a 139
- 4:57- to 90 facilities.
- 4:59- So in
- 5:01- your view as CFO, at what point does
- 5:03- smaller, more focused portfolio begin to show up
- 5:06- more clearly in in the operating line?
- 5:09- Yeah. Another great question, Madeline. So Ascension did
- 5:12- right size our portfolio from 139
- 5:15- to 90 fully owned hospitals
- 5:17- and 29 joint venture hospitals. So roughly 190
- 5:22- if you add all of them.
- 5:24- And we have been really intentional in the
- 5:26- last two and a half years of our
- 5:27- turnaround
- 5:29- in making sure our acute hospital portfolio is
- 5:32- focused on
- 5:33- less legacy structure
- 5:36- and find the right partners in The States
- 5:39- where we divested our facilities.
- 5:41- And I'll give you a few few examples.
- 5:43- You know, Chicago,
- 5:45- we found a partner with Trine Healthcare,
- 5:48- where we have given some of our facilities
- 5:50- to them.
- 5:51- In Alabama,
- 5:52- we found a partner in University of Alabama,
- 5:54- and we divested some of our facilities there
- 5:57- as well.
- 5:59- By rightsizing our footprint, we have been able
- 6:01- to build a predictable and consistent operating performance
- 6:05- and focus our capital deployment in those facilities.
- 6:09- And most importantly, now Ascension is pivoting towards
- 6:13- ambulatory
- 6:14- care. And you may have heard recently we
- 6:16- acquired AmSurge.
- 6:18- We just got a by FTC.
- 6:21- So this freed us the capacity
- 6:23- and capital
- 6:25- so that we can focus on ambulatory care.
- 6:29- Yeah. Definitely.
- 6:30- And I would like to hear from you
- 6:31- too. When you think of growth, what would
- 6:34- you say smart growth looks like to you?
- 6:37- I know that there's been some consolidation with
- 6:39- the portfolio, but looking to the future, what's
- 6:41- that smart growth look like in your eyes?
- 6:44- That's a great question, Madeline. When we think
- 6:46- about growth, we don't think about brick and
- 6:48- mortar growth. We don't think about number of
- 6:51- hospitals.
- 6:52- We think about how many patients we are
- 6:54- serving
- 6:55- and how do we provide the best clinical
- 6:58- care to our patients
- 6:59- in the most optimal setting where they need
- 7:02- it, when they need it. And hence, our
- 7:04- pivot towards ambulatory care settings. Because, let's face
- 7:08- it,
- 7:09- no one wants to stay in a hospital
- 7:11- longer than they have to. You know, people
- 7:13- want to go into an ambulatory care,
- 7:16- get their procedures done, and quickly go back
- 7:19- to their loved one loved ones in the
- 7:21- hospital. So
- 7:23- when we think about smart growth, we think
- 7:25- about how do we take care of patients
- 7:27- holistically,
- 7:29- when they need it, where they need it
- 7:31- in the most efficient care setting,
- 7:34- not necessarily number of hospitals and then brick
- 7:37- and mortar settings.
- 7:39- Yeah. I wanna backtrack just a tiny bit.
- 7:41- I know you just mentioned the AmSurge deal
- 7:43- there. Anything you can share with us? I
- 7:46- know you said this literally just happened. So
- 7:48- any any additional details there?
- 7:50- It's an exciting,
- 7:52- growth in our strategy.
- 7:54- And, Madeline, you mentioned how do we think
- 7:56- about smart growth.
- 7:58- If you think about ASC settings, these are
- 8:01- places where patients go for
- 8:04- less severe surgeries, nips knee surgery, hip surgery,
- 8:09- less acute procedures.
- 8:11- These procedures
- 8:12- don't necessarily need to be done in an
- 8:14- acute care in a hospital setting.
- 8:17- So as Ascension
- 8:18- vitalize our portfolio
- 8:20- and we reduce our acute footprint,
- 8:23- we wanted to make sure we expand in
- 8:26- the areas where patients
- 8:27- are going or where the payers want the
- 8:30- patients to be taken care of.
- 8:32- And that's where ambulatory strategy came in. So
- 8:35- Amsearch brings in roughly 250
- 8:38- ASCs to our portfolio.
- 8:41- Ascension already has about 60 ASCs, so this
- 8:44- brings us roughly 310
- 8:46- or so ASCs
- 8:48- across our portfolio in thirty four days.
- 8:51- And that will make Ascension one of the
- 8:53- largest ASC platform in the country
- 8:57- along with our acute care
- 8:59- hospitals as well.
- 9:01- Hopefully, we can bring better clinical care in
- 9:04- lower cost setting to our patients,
- 9:07- and this will help our patients go to
- 9:09- the places they need to versus have to
- 9:11- go to a hospital for their clinical care.
- 9:15- Yeah. Definitely exciting time of growth right there
- 9:17- in the ASC space.
- 9:19- Wanna shift gears just a tiny bit with
- 9:21- you now and talk about, you know, back
- 9:23- to the financial report of long term,
- 9:25- growth. You know, of course, long term debt
- 9:27- right now at Ascension does stand a little
- 9:29- bit over 9,000,000,000.
- 9:31- So in this instance, what would you say
- 9:33- are ways that you're looking to manage that
- 9:36- moving forward?
- 9:37- Another great question, Madeline. I'll say Ascension has
- 9:40- a very strong balance sheet. We have roughly
- 9:43- 45,000,000,000
- 9:44- of total assets,
- 9:46- and, our rating agencies have recognized it. We
- 9:50- enjoy a very strong rating,
- 9:53- double a from all three rating agencies,
- 9:56- Moody's, S and P, and Pitch.
- 9:58- And that allows us to borrow
- 10:00- smartly, wisely at a lower cost of capital.
- 10:04- So the 9,000,000,000 debt that you mentioned, you
- 10:06- know, one of the metrics they measure
- 10:09- is what is your
- 10:11- debt to cap ratio,
- 10:13- Debt to capital ratio.
- 10:15- Ascension enjoys a very high debt to cap
- 10:18- ratio.
- 10:19- We also have
- 10:20- the debt to cash, which is how many
- 10:23- times of the debt can you pay,
- 10:25- from your existing cash.
- 10:28- Ascension can pay almost the entire debt plus
- 10:32- some more.
- 10:33- Our
- 10:34- debt service ratio is 10.4
- 10:37- x, which is we can service our debt
- 10:39- interest
- 10:40- almost 10 times more.
- 10:43- So all that is we manage our debt
- 10:45- very wisely. We make sure our borrowing cost
- 10:48- is low,
- 10:50- and we make sure our debt to cap
- 10:51- ratio
- 10:52- and our debt service coverage ratio remains strong
- 10:55- for our rating agencies.
- 10:57- Yeah. So sitting in a pretty solid position
- 10:59- there for sure.
- 11:01- Absolutely. And
- 11:02- in the press release that,
- 11:05- Ascension shared,
- 11:06- for, again, its most recent, financial report,
- 11:09- you mentioned that higher volumes, better efficiency, and
- 11:13- more consistent execution are the system's key drivers.
- 11:16- So this in mind looking towards the rest
- 11:18- of the year, can't believe we're already more
- 11:20- than almost halfway through.
- 11:22- What must Ascension do to continue that momentum?
- 11:25- Great question, Marlene. I'll say we must stay
- 11:28- firmly on the path of transformation. As I
- 11:30- mentioned, it's been two and a half years,
- 11:32- almost three years now.
- 11:34- But we cannot stop listening and, cannot be
- 11:37- distracted by what's going on.
- 11:40- So I'm blessed to have a very strong
- 11:41- operating leaders
- 11:43- with us here in Ascension
- 11:45- who are everyday serving our patients, driving operational
- 11:49- execution and accountability.
- 11:51- We must need to, continue to improve
- 11:54- care coordination,
- 11:56- reduce our administrative friction,
- 11:58- and also take care where the patients need
- 12:01- it in the most efficient manner, as I
- 12:03- mentioned in ambulatory care settings.
- 12:06- Sustaining all of this momentum requires a lot
- 12:08- of collaboration
- 12:09- and a lot of communication.
- 12:12- And with the leadership from right from our
- 12:14- CEO, Eduardo Conrado,
- 12:17- and all the way down to a 100,000
- 12:19- associates,
- 12:20- I'm confident we'll be able to deliver that
- 12:23- mission.
- 12:24- And to to round us off here today,
- 12:26- I've got a fun one for you. So
- 12:28- you oversee
- 12:29- one of the largest systems in The US.
- 12:31- And so this in mind from a a
- 12:33- financial perspective,
- 12:35- what would you say to some of our
- 12:36- listeners who are maybe newer into health care
- 12:38- finance or looking to work their way up?
- 12:40- What advice would you have for them?
- 12:43- That's a great question, Marilyn. I've been in
- 12:44- health care for twenty five years, and I'm
- 12:46- blessed to be part of Ascension and in
- 12:48- this role. You know, one piece of advice
- 12:51- I'll give to, younger professionals in finance is
- 12:54- find your purpose.
- 12:56- Mhmm. You know, if you age
- 12:58- in a purposeful,
- 13:01- vocation,
- 13:02- your job doesn't remain a job. It becomes,
- 13:07- something that you are waking up every day
- 13:10- and living the dream. You are following the
- 13:12- passion.
- 13:13- And I coach this to my kids as
- 13:16- well. You know, then at that point,
- 13:19- you're not chasing a title. You're not chasing
- 13:21- an office. You're not chasing a certain amount
- 13:24- of compensation.
- 13:25- You are chasing a dream, and it becomes
- 13:28- a passion. So follow your passion, find your
- 13:31- purpose,
- 13:32- and you'll be very successful.
- 13:34- Well, that is some great advice, and I
- 13:36- think that can go across multiple industries as
- 13:38- well. So thank you so much. It's been
- 13:40- such a pleasure having you on, sir Rob.
- 13:42- I really appreciate you breaking down, Ascension's q
- 13:45- three earnings and, you know, just sharing what
- 13:47- the future's looking like at the system. So
- 13:49- I look forward to staying in touch and
- 13:51- connecting with you again soon.
- 13:52- Well, thank you for the opportunity, Madeline, for
- 13:54- me to tell the Ascension story. Awesome. Thank
- 13:57- you.
