- 0:00Welcome to the GLP-1 Newspaper, your daily update

- 0:11looking at Eli Lilly's registration of two phase
- 0:14three trials for EloraTZP. Right, which is an
- 0:18investigational once weekly co-formulation.
- 0:20Yeah, of the amylin agonist eloralintide and
- 0:24tirzepatide. Exactly. And we have the initial
- 0:27details for both of these phase 3 trials
- 0:29because they were first posted on a public registry
- 0:32of clinical studies on October 19, 2026. Right.
- 0:36And, you know, looking at the timeline for both
- 0:38of these, they are not yet recruiting. No, not
- 0:41yet. They are currently estimated to finish in
- 0:44late 2029. And for both trials, they are going
- 0:48to measure the percent change in body weight
- 0:50at week 100. Yeah, that week 100 mark is the
- 0:53primary focus. So looking at the first trial,
- 0:56which is called AMPLIFY-1. Right. AMPLIFY-1 plans
- 0:59for about 2,115 adults. And these are adults
- 1:03with obesity and a BMI of 35 or more? Exactly.
- 1:07And importantly, this specific trial is for individuals
- 1:10without diabetes. Yeah, that is a key detail
- 1:12for AMPLIFY-1. And it has five EloraTZP dose
- 1:16arms. Right, plus an arm for tirzepatide alone.
- 1:19And an arm for eloralintide alone. Yeah. And
- 1:22finally, a placebo arm. So there are quite a
- 1:24few arms in that first trial. Then there is the
- 1:27second trial, AMPLIFY-2. Right. And AMPLIFY-2
- 1:31plans for about 1,000 adults. With obesity or
- 1:34overweight and type 2 diabetes. Yeah. And the
- 1:38design here is different because AMPLIFY-2 compares
- 1:41EloraTZP with tirzepatide alone. Right.
- 1:44And it has no placebo arm. Exactly. No placebo
- 1:47arm for that one. So to get some context for
- 1:50these phase 3 registrations, I think we should
- 1:52look back at the phase 2b trial data. Right.
- 1:55From September 2026. Yeah. That trial had 367
- 1:59adults with type 2 diabetes. Over 48 weeks, right?
- 2:02Yeah. But a really specific detail to note about
- 2:05that phase 2b trial is how the medication was
- 2:08administered. Oh, right. Because in that trial,
- 2:11eloralintide and tirzepatide were given as separate
- 2:13injections. Oh, right. Rather than as one combined
- 2:16shot. Exactly. Administered separately. Well,
- 2:20looking at the results, there was up to 23.3%
- 2:23weight loss with EloraTZP. Versus 14.8%
- 2:27with tirzepatide 15 milligrams. And 3.0% with
- 2:31the placebo. I should interject here just to
- 2:34state plainly that those weight figures assume
- 2:36everyone stayed on treatment. Right. That is
- 2:38how those specific figures are calculated. And
- 2:41we also need to state that the comparisons with
- 2:43tirzepatide alone were secondary endpoints in
- 2:45arms of about 37 people. Yeah, so just keeping
- 2:49that specific sample size in mind. The trial
- 2:51also measured A1C reductions. Right, which were
- 2:55up to 2.9 with EloraTZP. Versus 2.4 with
- 2:58tirzepatide. Well, I do want to push back a little
- 3:01on the efficacy numbers and bring your attention
- 3:03to the adverse events. Right, the tolerability
- 3:06side of the data. Because treatment discontinuations
- 3:08due to adverse events ranged from 10.8% to
- 3:1327.0% across the EloraTZP arms. And I
- 3:18need to clarify that 27% was the rate in the
- 3:21highest arm, not all patients. Right. But still,
- 3:24that compares to a discontinuation rate of 2.9%
- 3:26with tirzepatide. Yeah. And 16.7% with
- 3:31the placebo. So what exactly was the nature of
- 3:34these side effects? Well, they were mostly mild
- 3:37to moderate gastrointestinal events during dose
- 3:39escalation. Which actually brings us to a perspective
- 3:42from the investor side. Oh, from Kevin Gade?
- 3:44Yeah, Kevin Gade, chief operating officer of
- 3:46Bahl and Gaynor, an investment firm that holds
- 3:49Lilly shares. What did he say about it? He said,
- 3:52quote, the outsized weight loss benefit is being
- 3:54questioned relative to the discontinuation rates
- 3:56and the reality that this is a dual drug regime.
- 4:00Well, regarding the dose escalation, Lilly says
- 4:02phase 3 will use an optimized escalation
- 4:05schedule. Right, an optimized schedule going
- 4:08forward. We also have some comments from Kenneth
- 4:10Custer, president of Lilly's Cardiometabolic
- 4:13Health Division. Yeah, he stated that he expects
- 4:15the combination to offer the best balance of
- 4:17efficacy, safety, and tolerability. He also noted
- 4:20that it might produce greater weight loss than
- 4:22retatrutide in later trials. And for some factual
- 4:26context there, retatrutide's up to 28.3% weight
- 4:29loss over 80 weeks came from a late-stage trial
- 4:32in people without diabetes. Right. And, you know,
- 4:36just looking at the broader space, Novo Nordisk's
- 4:38CagriSema is an amylin combination under the
- 4:41U.S. regulatory review. Yeah. We also heard
- 4:44from Citi analyst Geoff Meacham about the Lilly
- 4:47data. What were his thoughts? He said the findings
- 4:50provide strong combination proof of concept and
- 4:52broaden Lilly's cardiometabolic portfolio beyond
- 4:55today's incretin standards. Right. He also noted
- 4:58that achieving tirzepatide-like weight loss
- 5:00in patients with type 2 diabetes is significant
- 5:02because diabetes typically tempers treatment
- 5:05efficacy. Yeah. So to wrap up today's reporting,
- 5:08Morningstar analyst Karen Andersen said, if Lilly
- 5:12can get a handle on the discontinuations and
- 5:14GI tolerability issues, then I think this is
- 5:17looking stronger than retatrutide as a high-efficacy
- 5:19option for patients. Thanks for listening to
