- 0:00Hi, everyone, and thanks for tuning in to
- 0:01the Becker's Healthcare Podcast. My name is Lucas
- 0:04Voss, and I'm very excited to be joined
- 0:07by Trina Histon, PhD and VP at clinical
- 0:11product strategy at Wolbot Health to discuss scaling
- 0:14mental health support with health systems. Trina, thanks
- 0:18so much for joining us today.
- 0:20Lucas, it's a pleasure to be here. Looking
- 0:22forward to today's conversation.
- 0:23Yes. And it's so so interesting to have
- 0:26you on. You've done so much work
- 0:29in the health care space. You've been around
- 0:31for such a long time. I briefly scanned
- 0:33over some of your papers that you've done
- 0:35on on overweight and obesity and a lot
- 0:38of that work, and it's there's it's so
- 0:40multifaceted,
- 0:41which I love. You have such a deep
- 0:43background in health care. Can you just give
- 0:45us a little bit of an introduction to
- 0:46yourself and and your work at Wolcott?
- 0:49Sure. Sure. Happy to.
- 0:51So I'm a health psychologist by background. And
- 0:53as as you referenced, I've over 2 decades
- 0:56of experience
- 0:57in health care. And a lot of that
- 0:59in health care was kind of building end
- 1:01to end clinical pathways
- 1:03for prevention and treatment. And I would say
- 1:06the last 7, 8 years of my time
- 1:08in health care before I joined robot health
- 1:10and startup world was building and scaling a
- 1:12digital mental health ecosystem
- 1:14that deployed in primary care and in specialty
- 1:17care. So in in doing that work, it's
- 1:19really
- 1:20translating the evidence base into care delivery and
- 1:23doing that with methodologies
- 1:24like human centered design and performance improvement. So
- 1:27you're really thinking deeply about not just the
- 1:30what you're deploying,
- 1:31but where you're deploying it and who you're
- 1:33deploying it with at both in the care
- 1:34team side and on the patient side. So,
- 1:37delighted to bring that skill set to what
- 1:39about health where I really lean into clinical
- 1:41enablement with our partners. So it's been it's
- 1:44been great so far. Start up world is
- 1:45different to health care, but I'm in my
- 1:47happy place for sure. That's fantastic. And it
- 1:49it like you mentioned, it's become such an
- 1:51important part of the industry, I feel, like,
- 1:53to be able to look to those new
- 1:54solutions, to look to new developments, technology,
- 1:57digital solutions, etcetera. I think that that's really
- 2:00the way of the future. And you've mentioned
- 2:02you've you've started to work with health systems
- 2:04on this quite extensively.
- 2:05One of the systems you work with is
- 2:07Virtua Health.
- 2:09What were the initial goals when you first
- 2:11started that collaboration
- 2:13and and sort of the objectives as you
- 2:15were looking into how can we help patients?
- 2:17How can we improve care?
- 2:20Absolutely. Quick question. So I would say Virtua
- 2:22Health for those who aren't familiar as a
- 2:24health system in New Jersey. In fact, it's
- 2:26Southern New Jersey's largest healthcare provider.
- 2:29And when we started talking with the Virtua
- 2:31team,
- 2:32you know, we're thinking about primary care and
- 2:34the primary care setting and patients coming in
- 2:36to see their doctor. And, you know, one
- 2:39of the initial sort of pain points not,
- 2:41unfamiliar to pro perhaps most of health care
- 2:43in the US was making a referral to
- 2:46behavioral health and then having a wait list.
- 2:49I think Virtua really wanted to explore might
- 2:52there be other ways
- 2:53that patients who might need support in living
- 2:56better with
- 2:57symptoms of anxiety, depression,
- 2:59might there be other tools? And, of course,
- 3:01in the last several years, digital tools have
- 3:04become more and more common in deployments in
- 3:07in primary care and specialty care. So the
- 3:09initial pain point we were looking to solve
- 3:11was those patients seen in primary care who
- 3:13may get a referral and then they're on
- 3:15a wait list. But I think we we
- 3:17quickly then explored more deeply with the team
- 3:20and,
- 3:21you know, these tools are an adjunct to
- 3:23care. So again, robot health, in adults, that's
- 3:26the tool we've deployed with with the virtual
- 3:28team can be that support that's 247 for
- 3:31that patient.
- 3:32So I think, you know, that was really
- 3:34the initial discussion that we had with the
- 3:36virtual team. But but as I'll explain later
- 3:38on, we've we've expanded. So it's been tremendously
- 3:41exciting, and they've been a great team to
- 3:43partner with.
- 3:44Lots of options. And I feel like, again,
- 3:45that's that's where really those those digital solutions
- 3:48can really fill some of those gaps, fill
- 3:49some of those care gaps, and be able
- 3:51to to truly make an impact on the
- 3:53patient at the end of the day. And
- 3:54I think that's what we want. That's what
- 3:56we Absolutely. We're looking for help in the
- 3:58moment. Mhmm. Yeah. To to supplement
- 4:01to supplement and being able to help. I
- 4:02think that that's really key.
- 4:04Again, a lot of this is new for
- 4:07a lot of folks. A lot of people
- 4:08are might be hesitant.
- 4:10There's all of these solutions, all of these
- 4:12different companies that are doing this. What are
- 4:14some of the challenges
- 4:16in integrating a digital solution, especially if you
- 4:19already have a built out infrastructure?
- 4:22Sure. Sure. Great great question. I think, you
- 4:24know, I feel that the work we've been
- 4:27doing in partnership with Virtua really does, you
- 4:29know, offer a blueprint for success. So
- 4:33when we're we're the first sort of digital
- 4:34mental health tool that Virtua is deploying.
- 4:37And so we think of scale at the
- 4:39beginning. So you're not just sort of adding
- 4:40on and iteratively doing piecemeal, but you're thinking
- 4:42of your end goal in mind. And so
- 4:44we tend to implement in phases or or
- 4:46kind of what we call internally crawl, walk,
- 4:48run. And so, you know, when you're crawling,
- 4:51you're learning, like, you're exploring something new. I
- 4:54would say, you know, I think about the
- 4:55ABCs of implementation. And so I I think
- 4:58I've talked about the a, which was access,
- 5:00which which is a pain point for many
- 5:02systems in the US. And and I think
- 5:04the b is not to be pity, but
- 5:05it's it's beginning and being bold. I think
- 5:07a lot of folks who might be hesitant,
- 5:09you just you just have to take that
- 5:11first step. And so what does that look
- 5:12like? Well, we actually started in telehealth in
- 5:16in primary care in VirTra. In fact, our
- 5:18clinical lead, doctor Skrinsky leads leads that that
- 5:21team.
- 5:22And in doing that, like, the the c
- 5:24of the ABC's is context. Deep understanding and
- 5:26respect for
- 5:28how patients move through their visits pre, post,
- 5:31and after.
- 5:33How doctors are spending their time with patients
- 5:35kind of where's their attention going, what are
- 5:37they holding space for, what are they listening
- 5:38for. You know, we know from the literature
- 5:40that on average primary care doctors might have
- 5:42about 17 minutes and there are 6 to
- 5:447 topics that are usually covered in those
- 5:4717 minutes. So there's a lot happening and
- 5:49they're they're doing a lot and they're obviously
- 5:51documenting in the EMR as well and and
- 5:53Virtua use epic. So in the crawl phase,
- 5:56you're really understanding,
- 5:58you know, where might they be there'd be
- 5:59good points to get robot in in a
- 6:01patient's hands. What does the workflow look like?
- 6:04So
- 6:05making sure that you're not adding it undue
- 6:08burden to the care team because we know
- 6:10clinician burnout is very real and we know
- 6:12that teams are doing more and more and
- 6:14more, in in a visit. So we didn't
- 6:16wanna be one more thing that adds burden.
- 6:18So really listening for where are the opportunities
- 6:22for us to deploy,
- 6:24and really have that minimal impact on workflow,
- 6:26but really lean into where there's already good
- 6:29muscle memory on on different places in the
- 6:31EMR that a doctor is going to provide
- 6:33care. And so we really wanted to do
- 6:35that in crawl and we sort of launched
- 6:37in January
- 6:38in telehealth and then quickly added some, what
- 6:41I'll call more traditional primary care face to
- 6:43face care,
- 6:44sites in virtual health care footprint as well.
- 6:47We learned a lot, you know, in those
- 6:48first 15 to 20 patients in, how are
- 6:51the conversations going, what quest questions do patients
- 6:54have because,
- 6:55you know, part part of the getting ready
- 6:58is, you know, developing clinical training because
- 7:01a lot of doctors, you know, they've obviously
- 7:03deep expertise
- 7:04in in their degree, but things like mental
- 7:06health and digital mental health and and concepts
- 7:09from cognitive behavioral therapy may not be something
- 7:11they've had a lot of exposure to. So
- 7:13making sure that they really are comfortable
- 7:15with robot health and that they can talk
- 7:17about it knowledgeably
- 7:19in making the referral and that they get
- 7:21to ask and answer all their questions. You
- 7:23know, doctors are very discerning and and rightly
- 7:26so. And so we had a lot of
- 7:27great conversations to get ready and a number
- 7:29of clinical trainings to get ready. And so
- 7:32that would cover, you know, what robots is
- 7:34for adults and, you know, who might be
- 7:36a good fit for robots and maybe who
- 7:37isn't a good fit for robots and then
- 7:39how you make the referral.
- 7:41And and I would say one of the
- 7:42nice things Virtua has to,
- 7:44if I think of kind of workforce of
- 7:46the future and sort of care team configuration
- 7:48of the future, they have also got,
- 7:50technical consultants that support
- 7:53telehealth. And I think these are newer roles
- 7:56in in health care that we're seeing pop
- 7:58up like digital navigators or care coordinators,
- 8:00and they are really in a really nice
- 8:02connection then between the care team and the
- 8:04patient.
- 8:05So in the crawl phase, you know, figuring
- 8:07all those moving pieces
- 8:09out, looking at how the flow of the
- 8:11first, as I said, 10 to 20 patients
- 8:13is looking and might there be anything we
- 8:15need to optimize. And then we scaled quite
- 8:18quickly across their primary care footprint
- 8:20in the springtime and continue to to learn
- 8:23as we go. In fact, you know, Virtua
- 8:26made the decision to to bring in other
- 8:28specialties because, you know, the other thing that's
- 8:30happening in health care and I'm personally applauding
- 8:33it is the recognition that the the head
- 8:35is connected to the body. And so physical
- 8:37health and mental health are are intertwined. And
- 8:40and Yes. You know, if you're struggling
- 8:42with your mental health, it's gonna impact how
- 8:44well you might manage your diabetes or or
- 8:46anything else of chronic conditions. So we've actually
- 8:48launched another specialties like GI, like cardiopulmonary,
- 8:51and and those care settings and and are
- 8:53about to add some more because those specialties
- 8:56we're recognizing, look, we we wanna really support
- 8:59our patients and giving them tools to kind
- 9:01of help them at at at 4 in
- 9:02the morning. I mean, candidly, about 77% of
- 9:05patients who are using robots in our virtual
- 9:07deployment are doing so outside of traditional office
- 9:09hours. So we can act as that digital
- 9:11layer of support
- 9:13that's available in the moment. And then, obviously,
- 9:15we're adjunctive to care. So, those patients know
- 9:18they can always go back to their, you
- 9:20know, referring provider as well.
- 9:22Yeah. And I think, again, that's what I
- 9:24love about digital about these digital solutions and
- 9:27specifically what you've mentioned with WoBot Health. Right?
- 9:29It's it's ax we're also solving an access
- 9:32issue. We're providing access to people that normally
- 9:36might not have it or that might not
- 9:38be able to get the help that they
- 9:39need, and these digital tools are able to
- 9:41do that in a way that's so much
- 9:43more just inclusive for everyone. And I think
- 9:47that that's such a key such a key
- 9:49piece to being able to have a strategy
- 9:51like this
- 9:52for for health systems, I think. Absolutely. And
- 9:54and Virtu the Virtu team exceed those opportunities
- 9:57early. Yes. And and they want to avail
- 10:00of them. So that's been, you know, such
- 10:02a joy to to partner and think through,
- 10:04okay, might we adjust any of the materials
- 10:07for different specialties or or do we sort
- 10:10of point to different parts of robot that
- 10:12might be supportive in different ways for for
- 10:14different kinds of patients. So they've they've just
- 10:17really been very innovation mindset, which is wonderful.
- 10:20I definitely really enjoy the partnership. And and,
- 10:22you know, you're building the company, you know,
- 10:24and now I'm on the other side, right,
- 10:25that I wasn't healthcare and I'm in a
- 10:27startup. But, you know, you move at the
- 10:29speed of trust and you're building
- 10:31relationships with folks every day. And so, you
- 10:33know, our data is growing every day, but
- 10:35you're learning from that. And then you're infusing
- 10:38the learnings from data in the context of
- 10:40say virtual health footprint. And then, you know,
- 10:42you're continually, you know, moving and optimizing and
- 10:45it's a dynamic process. But, you know, you
- 10:47always want to keep at the heart of
- 10:48it is, you know, you're being deployed
- 10:51in in a clinically mediated way and it's,
- 10:54you know but the relationship between a clinician
- 10:56and a patient. And so we never take
- 10:58that for granted and and really sort of
- 11:00hold that that trust as well because we
- 11:02are also supporting those patients.
- 11:05Yeah. And and I think it comes back
- 11:06to your your ABCs that you've mentioned earlier
- 11:08in the conversation. Right? And I think the
- 11:10the b here is really crucial. As you've
- 11:13mentioned, you have to be bold.
- 11:15You have to Uh-huh. You have to innovate.
- 11:17You have to start. And I I think
- 11:19that's such a key piece. And and looking
- 11:21ahead
- 11:22right now for you as you as you've
- 11:24been in the industry for such a long
- 11:26time, you've been on different sides of things.
- 11:28Where do you see this going? Where do
- 11:30you see the partnership with Virtua going? But
- 11:32in general, also, how do you see this
- 11:35overall evolving for other health systems as well?
- 11:38Yeah. Yeah. Great question. I think there are
- 11:40a couple things with Virtua. Obviously, we're we're
- 11:42expanding into other specialties. So I think we're
- 11:45gonna learn a lot as those mature and
- 11:47evolve in terms of different types of patient
- 11:50populations and and what's resonating with them as
- 11:52as we are providing that support.
- 11:54We're also doing a case study with Virtua.
- 11:56So, you know, again, they're, you know, very
- 11:59innovative, but they also want, you know, as
- 12:01do we want to understand how are we
- 12:03impacting
- 12:04patient care, down the line. So really looking
- 12:07at the context of the deployment in different
- 12:09settings. So I would say keep your eyes
- 12:11peeled for for that case study.
- 12:13And if I zoom out to the whole
- 12:14field, I mean, there are many big moving
- 12:16pieces now happening in the US. So recently,
- 12:19you know, with parity, you know, seeing how,
- 12:22that's going to land with parity between mental
- 12:24health and physical health.
- 12:26Also, CMS will be releasing their physician fee
- 12:29schedule final ruling
- 12:31in early November. And that is, you know,
- 12:33with the goal to supporting the deployment of
- 12:35digital mental health. I'm personally very excited about
- 12:37that because it kind of creates a a
- 12:39strong reimbursement pathway.
- 12:42So I'm excited that those big pieces are
- 12:44moving. The other things I'm seeing also are
- 12:47looking at more whole person care. And I
- 12:49think Virtu have been one of the early
- 12:50movers in this in terms of addressing social
- 12:53determinants of health. And so both assessing it
- 12:55but also addressing it. So I think in
- 12:56healthcare and I can say this after 2
- 12:592.5 decades and, you know, a lot of
- 13:00times you're assessing a lot of things, but
- 13:02maybe you're not able to act on them
- 13:04because there may not be something in place
- 13:06to do something candidly. And, you know, I
- 13:08think mental health might have been one of
- 13:10those areas where that was challenging. But I
- 13:11think again with digital mental health growing and
- 13:14growing year on year, that's now becoming less
- 13:16of an issue. But newer things like social
- 13:18terms of health and what you need to
- 13:20have in place to support whole person care,
- 13:22I think there's tremendously
- 13:24exciting efforts,
- 13:25you know, underway and understanding, you know, the
- 13:28assessment and also the deployment of solutions to
- 13:30really support that human being. And we're part
- 13:33of that story and that architecture. So we're
- 13:35excited to to partner with our healthcare partners
- 13:38and and lean in and see how might
- 13:39we be part of that touch point for
- 13:42that patient over, say, a 24 hour period.
- 13:44What else can we do? So I'm excited
- 13:46by all of that because the promise of
- 13:48digital mental health, I think we're in the
- 13:50the second era, if I can frame it
- 13:52that way Mhmm. In the field. And, you
- 13:54know, I'm excited for the future.
- 13:56Absolutely. That sounds so fascinating. And we'll have
- 13:59obviously, Trina, we'll have to do a second
- 14:01podcast once the study comes out. Right? Absolutely.
- 14:04Happy to come back and talk. Once it
- 14:06comes out, we'll have to talk about the
- 14:07findings. We'll have to talk about all the
- 14:09interesting things that come out of that. Again,
- 14:11thank you so much for sharing your time
- 14:13today with our audience and your insights, certainly.
- 14:16What a great conversation. Thanks for being here.
- 14:18Thanks a million, Lucas. Take care. Thank you.
- 14:20And I'd also like to thank WolBot Health
- 14:22for sponsoring today's content. And to our listeners,
- 14:25thanks for joining us today, and make sure
- 14:27to check out other Becker's podcasts.
- 14:29Have a wonderful rest of your day, and
- 14:31make sure to tune in for the next
- 14:33one.
