Transcript
- 0:00Hi, everyone. This is Lucas Voss with Becker's
- 0:02Healthcare. Thanks so much for tuning in to
- 0:03the Becker's Healthcare podcast series. It's great to
- 0:06have you. An exciting topic today, wide ranging
- 0:09certainly, why managing denials isn't enough. It's really
- 0:12time to eliminate them, and I'm so excited
- 0:15to talk about this with Patrick D'Angelo, president
- 0:18of CoroHealth. Patrick,
- 0:19so excited to have you. Thanks for being
- 0:21here.
- 0:22Happy to be here, Lucas.
- 0:24Great to, be a part of this.
- 0:26Yes. There's a lot to talk about, and
- 0:29I I'm I'm going to aid you here.
- 0:31I apologize.
- 0:33You have more than twenty years of experience
- 0:35in this space. Again That's being kind, actually.
- 0:39There you go. You're you're again, that that
- 0:41positioning is so important because you're not just
- 0:44with one specific health system. Right? You have
- 0:47this view
- 0:49of what's going on across the country,
- 0:51across dozens of health systems. Right? I'd love
- 0:54to start off with just sort of the
- 0:56patterns that you're seeing
- 0:58from where you're sitting that health system leaders
- 1:01right now might not be able to see
- 1:03from where they're at.
- 1:05Yeah.
- 1:06You know, I think that's a great place
- 1:08to start because being on,
- 1:11you know, more of the service and the
- 1:14service provider,
- 1:15you're able to look across the spectrum
- 1:18of
- 1:19all health care institution,
- 1:21be it a large health system, be it
- 1:24a small
- 1:25private
- 1:26physician practice, be it even payers and and
- 1:28and what they're dealing with. So seeing that
- 1:30spectrum kinda gives you a different light of
- 1:33exactly,
- 1:33you know, or somewhat of what what's going
- 1:36on,
- 1:37in in the in the health care space.
- 1:40You know? And I think when you you
- 1:42represent providers, which is what I represent at,
- 1:45at Coral Health and and support them,
- 1:48you know, you just see this constant trend,
- 1:50and it's it's almost it's turning into, like,
- 1:52a broken record of, you know, denials, managing
- 1:55denials.
- 1:56How do I get the right people? The
- 1:59shift in the workforce,
- 2:01you know, today is way different than it
- 2:03was, you know, pre, pandemic. Right?
- 2:06You have, you know, employees that, you know,
- 2:07pretty much, you know, work from home. We've
- 2:09never had that in our space before. Right?
- 2:11So being able to, you know, manage that
- 2:14shift of employees knowing that,
- 2:17you know, your your lifeline is
- 2:20is always been reliant on your resources or
- 2:23your vendors.
- 2:25And now you have the,
- 2:27you know, the the parallel track of AI
- 2:31and and how that can support,
- 2:33you know, each each individual and improve overall
- 2:37results and so forth. So, you know, it
- 2:39is an it's a true interesting time, but
- 2:41I will say the themes are are pretty
- 2:43common
- 2:45across, you know, health systems, across practices,
- 2:48physician practices,
- 2:49you know, across the board and and and
- 2:51where they struggle. And it's, you know, it's
- 2:53having
- 2:54the the right people, the right amount of
- 2:56people, having the right,
- 2:58partner or vendor, if you will, to support
- 3:00them in different areas.
- 3:02You know? Do you,
- 3:03do you outsource everything? Do you compliment?
- 3:06Do you use staff aug? You know? How
- 3:08do you how do you, you know, how
- 3:10do you work this? And it's it's different
- 3:12depending on, you know, the health system and
- 3:15and how they wanna approach things.
- 3:17And then, you know, like I mentioned, you
- 3:19got AI, and and what are the decisions
- 3:21there? Where do I start? Do I start
- 3:23big? Do I start small? Do I trust
- 3:25it?
- 3:26Do I have to have a a an
- 3:27overlay to to keep a a close eye
- 3:30on it? And, you know, I think there
- 3:32there are just a lot of questions that
- 3:33the organizations
- 3:35are truly
- 3:36trying to work through and understand and so
- 3:38forth.
- 3:40I wanna touch on something that you just
- 3:41briefly mentioned, which again is the importance of
- 3:43technology. We'll talk about AI here in a
- 3:44little bit, but in a recent conversation that
- 3:46we've had in in a webinar that I
- 3:48hosted with Coral Health that you should definitely
- 3:51also check out if you haven't done so.
- 3:53You said something that really stuck with me
- 3:55in that conversation, which is that things have
- 3:57changed to where we're it used to be
- 3:59people process technology, so that three pronged piece
- 4:01that a lot of folks still mention today,
- 4:04and it changed to technology
- 4:07people.
- 4:08What's driving that shift right now, if you
- 4:11wanna talk just a little bit about what
- 4:12you mean by that? And and, again, for
- 4:14leaders that are still early in that journey,
- 4:16what is that first move to get there?
- 4:19Yeah. Yeah. Well, I I I think it's
- 4:22really in the
- 4:24the age now of AI. Right?
- 4:26You know, technology was always a critical part,
- 4:30but it wasn't the first part. It was
- 4:31always driven mainly by people.
- 4:35And now as you look at the shift,
- 4:39you know, technology
- 4:40is really the process. Right? You're gonna load
- 4:43technology.
- 4:44It's gonna tell you everything you wanted to
- 4:46do. Right? I mean, it's even, you know,
- 4:48your your epic work lists are are really
- 4:51built around how do you want your process
- 4:54set up. I think
- 4:56fundamentally, every workflow
- 4:57is driven by
- 4:59technology of some sort. And that technology then
- 5:02has some
- 5:03pretty intelligent
- 5:06work listings
- 5:07or intelligent
- 5:08areas that
- 5:10can
- 5:11drive the workload
- 5:13based
- 5:15off demand, based off skill, based off need,
- 5:18you know, it that ultimately help reduce
- 5:22what you're trying
- 5:23to do or what you've done in the
- 5:25past manually.
- 5:26So, you know, I I think on our
- 5:29call last week, Lucas, you know,
- 5:33the ladies that we had on, you know,
- 5:35said, yeah. We still use people and the
- 5:37the technology. Right? They were really people focused.
- 5:39And I I don't disagree with that at
- 5:41all. I think that's actually the the right
- 5:43approach for sure. But you gotta meet have
- 5:46that technology, I think, today,
- 5:48drive where people are,
- 5:51or drive where you want your people to
- 5:53be and and be able to focus on
- 5:56areas or what I would call, you know,
- 5:59pits within
- 6:00your
- 6:01AR process or within your entire revenue cycle
- 6:04flow, it'd be at front end, mid cycle,
- 6:06and back end, have that technology
- 6:09drive,
- 6:10you know, where you need to focus on
- 6:13and be able to report upon it. Because
- 6:16I do think
- 6:18you're not gonna eliminate
- 6:20the people aspect.
- 6:22Right? But when the technology identifies
- 6:25routine
- 6:26denials,
- 6:27routine areas, routine fail points,
- 6:31you're gonna have to go up and reengineer
- 6:34or tinker the technology
- 6:37to improve that feedback. Now,
- 6:39unfortunately, I think
- 6:41it's gonna be a cat and mouse game
- 6:42with payers and providers that you're constantly tweaking.
- 6:45They'll make their change on their end and
- 6:47so forth. But the goal ultimately
- 6:49is if you have
- 6:51poor registration on the front end and you're
- 6:54not fixing it through
- 6:56resources or technology,
- 6:58you're you're gonna be in a you're gonna
- 7:01it's gonna continue to snowball, if you will.
- 7:03So
- 7:04having technology be that first one and the
- 7:06people that really drive the feedback to improve
- 7:09the technology,
- 7:10that's where we've evolved. And that's that's where
- 7:12we are today.
- 7:14Speaking of these processes, I do wanna get
- 7:15a little bit more granular and maybe also
- 7:17use this as an example to illustrate what
- 7:19we just touched on. Right?
- 7:21You talk often about the distinction between self
- 7:24pay after insurance and pure self pay, right,
- 7:26when it comes to AI, specifically in in
- 7:28patient financial conversations. It's very, very crucial. It's
- 7:31a very important topic for patients, obviously, for
- 7:33providers as well. Again, that's, however, also a
- 7:36nuance that a lot of folks don't necessarily
- 7:38talk about.
- 7:40Can you walk through that distinction a little
- 7:41bit more and what it means for health
- 7:44systems
- 7:45that where they should be
- 7:47deploying that AI on the patient facing side?
- 7:51Yeah. And we we talked about this a
- 7:52little bit last week as well.
- 7:55You know, in in where I look at
- 7:57it from a self pay perspective,
- 7:59we you carve out your pure self pay
- 8:02versus your, self pay after insurance. And, you
- 8:06know, in most cases,
- 8:07other than, you know, mostly Medicare, but your
- 8:10self pay after insurance,
- 8:13these are more of your
- 8:16working class, if you will. Nothing against the
- 8:19self pay or the non working class or
- 8:21people who are are are are working there,
- 8:24but they're more tech savvy.
- 8:26Right? They are more likely to
- 8:30pay their bills. They're more likely to use,
- 8:34your your online pay, your things like that.
- 8:38And sometimes they just need that little nudge
- 8:41or that little reminder. And utilizing AI to
- 8:44make that call
- 8:46or send that message, that email, that text
- 8:49is is what gets it done. And and
- 8:51me personally, just because I'm in the space,
- 8:54I'm the one that waits to see
- 8:57if I get a balance after. I actually
- 8:59wait to see the technology. I wanna learn.
- 9:01I wanna I wanna see what they're doing.
- 9:03So I'm the type of guy that doesn't
- 9:04pay right away, and I wait till really
- 9:06the end just to catch the dunning cycle.
- 9:09What is the process? How long between calls?
- 9:12Are they really doing it? So it help
- 9:14gives me the actual feedback to either get
- 9:16back to that client or to my customers
- 9:19to say, hey. This seemed to work for
- 9:21me or or I've seen it work a
- 9:22little bit better. So I look at the
- 9:24self pay after as being more
- 9:27they'll adapt to
- 9:30the change in trends of AI and just
- 9:32trying to get the bill pay. On the
- 9:35self pay side,
- 9:37I I don't speak for all of them,
- 9:38but most of them will use that AI
- 9:40as a way, well, it didn't get through,
- 9:42or I didn't hear it, or, yeah, my
- 9:44check's in the mail. And it's a lot
- 9:45easier to do that
- 9:47with
- 9:49a pure self pay
- 9:51because
- 9:52they're either gonna pay or not gonna pay.
- 9:54And and based off their their current situation,
- 9:57likely use that not to pay. It it
- 10:00you'll need more of a human touch to
- 10:02hopefully keep them on the phone, get them
- 10:04the assistance they need, find out what charity
- 10:06care is, can we offer a prompt pay
- 10:08discount
- 10:09altogether. But one of the things I did
- 10:11talk about last week, which
- 10:13goes over self pay and self pay after,
- 10:16even with AI,
- 10:18you have to be sensitive
- 10:20to the
- 10:23patient you're calling or guarantor you're calling
- 10:26depending on the type of illness or why
- 10:29they've had this bill.
- 10:31Right? So if you have a family member
- 10:34or yourself
- 10:36that's going through a horrible health issue
- 10:39and you're struggling with work, you're struggling with
- 10:43making payments,
- 10:44and you having a robot constantly call you,
- 10:48it, I think, impacts
- 10:51the community
- 10:52and the ethic of the hospital. Right? And
- 10:55and I think having a human still call
- 10:58in those situations,
- 10:59for example, a children's,
- 11:02a a children's hospital.
- 11:03Right? You you're there there's so much sensitivity
- 11:06going there, and and you don't wanna lose
- 11:08that. And I think a lot of children's
- 11:10hospital promote themselves with being the forefront and
- 11:13being the provider
- 11:15for those families during these hard times. And
- 11:18having an AI just constantly ping them, you
- 11:21sort of lose somewhat of the sensitivity. And
- 11:23I I just want, you know, everybody to
- 11:25realize that that could be a hard barrier
- 11:28that will take longer for
- 11:30the community, the patients, the guarantors, the sufferers,
- 11:33the ones who are struggling
- 11:35to understand
- 11:36having a human touch versus an an,
- 11:39an AI robot, if you will.
- 11:42And I wanna talk a little bit more
- 11:43about that specific fact. We've we've touched on
- 11:46it in the beginning of our conversation. Again,
- 11:48you you see this work in a lot
- 11:50of cases. You see especially the AI agent
- 11:52piece make a difference for people, specifically when
- 11:54it comes to workload.
- 11:56Unfortunately, you're also part of the other side.
- 11:57Right? You you see when it doesn't work.
- 11:59You have the experience to say, hey. This
- 12:01might not have worked. This this didn't work.
- 12:03This needed to be adjusted, etcetera. Right? I'd
- 12:06love to know what some of the warning
- 12:08signs are that you would like to point
- 12:11to with leaders when an AI implementation
- 12:15doesn't work. What are some of the things
- 12:17that you are pointing to that leaders should
- 12:19be aware of today of, like, okay. We
- 12:22need to adjust this.
- 12:23Yeah. I I think there's probably multiple areas
- 12:26you can look at. I mean,
- 12:28you know, financial decline would be, like, the
- 12:30first one. If you start to see, you
- 12:32know, your metrics starting to slip, you know,
- 12:34be it cash,
- 12:35be it anything, you see your, you know,
- 12:37your AR shifting
- 12:39into higher age buckets,
- 12:42you know something's wrong. Right? Now I'm not
- 12:44saying that would be a 100% AI. There
- 12:46could be other shifts in the business that
- 12:47would drive that. But for the most part,
- 12:50it would be those AR metrics.
- 12:53Moving back to the self pay area, you
- 12:55know, if you're making a lot of, quote,
- 12:57unquote, contacts,
- 12:59yet you're not seeing the shift, or the
- 13:03inflow
- 13:04of cash or resolution rates to those contacts,
- 13:07I think that would be
- 13:09a a huge indicator
- 13:10of what you have to do and and
- 13:12maybe
- 13:13adjust or think. And that's where we go
- 13:16back to the technology and people.
- 13:19In the past, you had to retrain the
- 13:20people, redo that, and, you know, it was
- 13:23a it was a longer shift to improvement.
- 13:26Now, you tinker with the technology
- 13:29and you've changed the process and you have
- 13:31a a a you can have a quicker
- 13:33impact to improvement.
- 13:35And you you have the ability to say,
- 13:37okay. Hey. Like, this didn't work that well.
- 13:39Let's shift it this way. Or based off
- 13:42the metrics, let's shift it that way. But,
- 13:44you know, again, I think you're gonna have
- 13:47to have
- 13:48if you're using that technology
- 13:51to drive a process or person,
- 13:54you're gonna need to make sure you have
- 13:56the right tools or the people
- 13:58to watch those shifts in metrics to make
- 14:01sure you're on top of it so it
- 14:03doesn't just keep snowballing into something bigger.
- 14:06I think, again, you've mentioned something very important
- 14:10here again, the fact that this is
- 14:12it's this that snowball effect. And and I
- 14:15like to link this to to the fact
- 14:17that everything is changing all the time. Right?
- 14:19This is a fast fast moving space. Organizations
- 14:22have to be aware of that. One of
- 14:24the things that we talk about quite a
- 14:26lot is the fact that payers are also
- 14:28adapting. Right? Health systems are adapting. Payers are
- 14:32adapting. Health systems a health system do something,
- 14:35then the payer does something. Right? It's sort
- 14:37of this cat and mouse. Folks are adjusting
- 14:40because this is a constantly moving space as
- 14:42we just highlighted.
- 14:44What does this race
- 14:45look like from your perspective? And as we
- 14:48close our conversation here today, right, how should
- 14:51revenue cycle leaders think about this space with
- 14:54that in mind?
- 14:56Yeah. I mean, if I had an answer
- 14:57to this, Lucas, my goodness, what we could
- 14:59do.
- 15:00It's you know, it's one of those things
- 15:02because each payer is is different. Right? And
- 15:05and they get categorized different. And that's what
- 15:08I think that's a lot of the complexity.
- 15:10Right? Is it's not, you know, one size
- 15:12fits all with payers and and providers and
- 15:15and how they work.
- 15:17But,
- 15:18you know, I I do say it's
- 15:21it's
- 15:22capturing
- 15:23what you know is failing.
- 15:25Regardless if payers right or aren't capturing it,
- 15:29categorizing it as how do I fix this
- 15:32solution bundled. Right?
- 15:34Fix this solution for multiple
- 15:36and try to stay on top of it.
- 15:38Right?
- 15:39It's hard to pinpoint where the fixed point
- 15:41would be, right, the chicken and the egg
- 15:43here for the most part. But being able
- 15:46to
- 15:47just categorize
- 15:49where your biggest pain points are
- 15:51and basic blocking and tackling. And that's with
- 15:54technology.
- 15:55That's with humans. That's being able to stay
- 15:58on top.
- 15:59And, you know, I you always like to
- 16:02say you can work with your payer. Right?
- 16:04You always like to say that you're big
- 16:05payers. You can sit down and and figure
- 16:08out how to work it. But if you
- 16:09you can't really do that
- 16:11unless you have the data points and the
- 16:14proof points
- 16:15on where you feel
- 16:17they're failing you. Right? And I do think
- 16:20sometimes we just categorize payers as they're just
- 16:23horrible. They're just gonna do it because they're
- 16:25gonna maybe they do. I'm not gonna go
- 16:27go there with this conversation. But I would
- 16:30say that if you don't have the data
- 16:33and the factual points,
- 16:35you're not gonna improve anything. One with technology,
- 16:39one with your people, and most importantly, the
- 16:41payer relation.
- 16:42Patrick, thanks so much again. It's great to
- 16:45have you. Same here, Lucas. Appreciate it.
