- 0:00Welcome to the Becker's pharmacy leadership podcast. I'm
- 0:02Ella Jeffries, pharmacy reporter here with Becker's Healthcare.
- 0:06Today, I'm joined by doctor Mark Mackensen, senior
- 0:08director of outpatient pharmacy at Mount Sinai Health
- 0:11System.
- 0:12In this episode, we'll be exploring digital innovation
- 0:14in specialty pharmacy, how technology is transforming operations,
- 0:18driving better patient outcomes, and creating new opportunities
- 0:21for pharmacy leaders. Doctor Mechenson, thank you so
- 0:24much for joining me today. To start off,
- 0:26can you please introduce yourself and share a
- 0:27bit about your role and how specialty pharmacy
- 0:30fits within your organization's overall pharmacy strategy?
- 0:33Thank you, Ella. Good to be here with
- 0:35you. So my name is Mark Mackensen. I
- 0:37am the senior director of outpatient specialty pharmacy
- 0:38for the
- 0:41specialty pharmacy for the Mount Sinai Health System
- 0:43in New York City. We're a large academic
- 0:44medical center, consisting of seven hospitals and approximately
- 0:49500
- 0:50outpatient
- 0:51specialty clinics
- 0:53across the New York City area, New York,
- 0:54New Jersey, Connecticut.
- 0:57And I I oversee all the outpatient
- 1:00pharmacy
- 1:01activities across the system. We have seven
- 1:04pharmacies,
- 1:05across New York City,
- 1:07three of which are specialty accredited pharmacies. We
- 1:11have a you know, we started in
- 1:13about 2020,
- 1:14so pretty young operation,
- 1:18as a whole,
- 1:19but we've been we've grown,
- 1:21exponentially over the last five years.
- 1:24So we service the system as a whole.
- 1:27So all the the practices across system, we
- 1:30are integrated
- 1:31in about 90% of practices with our specialty
- 1:34services.
- 1:36So
- 1:37the overall goal of the the outpatient pharmacy
- 1:40department is is really to
- 1:43keep the care of the patients
- 1:45within the continuum of the health system.
- 1:48And, you know, we we see when patients
- 1:50stay within the health system and all of
- 1:52their care originates in the same place,
- 1:54they get they get better outcomes.
- 1:57And we can drive system level initiatives such
- 2:00as, like, the stay challenges,
- 2:03decreasing length of stay
- 2:05and increasing compliance,
- 2:06increasing adherence,
- 2:08and decreasing readmission.
- 2:10Great. Amazing to hear about your background. With
- 2:13that perspective in mind, I'd love to dig
- 2:15into how your team is approaching innovation. You
- 2:17know, specialty pharmacy continues to evolve quickly. So
- 2:20what does digital innovation look like at Mount
- 2:22Sinai right now?
- 2:24Absolutely.
- 2:25So right now, we are in the middle
- 2:27of some really big changes.
- 2:30We are actively
- 2:31engaged with with several
- 2:33vendors to to bring on some some automation,
- 2:36AI technologies,
- 2:38which I'll I'll get into in a little
- 2:39bit.
- 2:40But, you know, our our pharmacy platform is
- 2:43currently operating on on a legacy platform that
- 2:46has very little
- 2:48digital reach, to our patients.
- 2:50Right. So if you can imagine a retail
- 2:53pharmacy, an independent retail pharmacy with an independent
- 2:55system
- 2:56where a patient needs to call in,
- 2:59and
- 2:59interact with a legacy IVR system or speak
- 3:03to a member of the pharmacy staff to
- 3:04get a status on a prescription,
- 3:06that's where we're at now. We but we
- 3:08are
- 3:09really quickly
- 3:11moving towards a digital first strategy. Right?
- 3:15So in trying to meet our our goal
- 3:18of keeping as many patients within the system,
- 3:21we need to offer
- 3:22a similar level of service and similar experience
- 3:25that patients get elsewhere. Right? So when patients
- 3:28go
- 3:29online,
- 3:31for a typical shopping experience, whether they're going
- 3:33to Amazon or they're going to CVS or
- 3:36to any other retailer,
- 3:37they're used to a a digital experience that
- 3:41is easy
- 3:42and streamlined and and meets their needs and
- 3:45meets them where they are.
- 3:47And that's where we're trying to go. So
- 3:50developing a front end digital platform where a
- 3:52patient can
- 3:54log on, interact with their medication,
- 3:57refill,
- 3:58check out,
- 3:59schedule delivery,
- 4:01things of that nature. But then
- 4:03going a step further, right, we wanna bring
- 4:05in the clinical aspect. So
- 4:07where can they get their clinical information?
- 4:11Can they access their
- 4:13their
- 4:13their discharge summary in one place and have
- 4:16that linked to their medications that they got
- 4:18from the pharmacy
- 4:19to understand what the side effects are on
- 4:21the medication and when to take the medication?
- 4:24And, you know, how can they use the
- 4:27the
- 4:28health systems
- 4:30digital presidents
- 4:31to answer all their health care needs?
- 4:34That's that's where we're trying to go. So
- 4:37digital first strategy from a patient experience perspective.
- 4:41But then on the back end, right,
- 4:44there are a lot of workflows that we
- 4:46have that are very manual and legacy workflows
- 4:49that require a lot of people to to
- 4:52do
- 4:53repetitive tasks day in and day out, which
- 4:56is what we're we're trying to
- 4:58streamline and and automate at this moment. Right?
- 5:01So there there's a few main processes
- 5:04that we we do every day, including benefits
- 5:06investigation, prioritization,
- 5:09financial assistance,
- 5:10and delivery coordination.
- 5:12We are currently
- 5:14able to automate two of those, two out
- 5:16of four, but we are in the process
- 5:19of trying to bring them all into
- 5:21a single ecosystem
- 5:23so that everything lives in one place and
- 5:26everything is
- 5:27is automated and streamlined.
- 5:30And at the end of the day, we
- 5:31we really want to
- 5:33focus
- 5:34on our employees, whether it's our technicians, our
- 5:37pharmacists,
- 5:38working at top of our license and being
- 5:40able
- 5:41to have those conversations
- 5:43with patients
- 5:44that are are value add for patients. Right?
- 5:47We want our people to be a resource
- 5:49for our patients
- 5:50somewhere that they can go to, that they
- 5:53trust for information
- 5:54versus
- 5:55individuals that are doing repetitive tasks day in
- 5:58and day out.
- 5:59For sure. That's all really interesting and very
- 6:01important changes that sounds like your team has
- 6:04embraced a lot in a very short amount
- 6:05of time. I'm curious, you know, what specific
- 6:08new technologies or you mentioned those workflows or
- 6:10just any digital tools that have had the
- 6:12biggest impact on your specialty farmer pharmacy operations
- 6:15or patient outcomes?
- 6:17Yeah.
- 6:18So I think the first big one that
- 6:20that we really piloted and and implemented was
- 6:24prior authorization support. So being able to
- 6:28have technology
- 6:29pull
- 6:30the correct answers for prior authorizations out of
- 6:33the medical record and populate
- 6:35the prior authorization form.
- 6:37Previously, this was a fully manual process. So
- 6:40you can imagine a patient gets a prescription.
- 6:42It needs a prioritization.
- 6:44The that liaison has to go into the
- 6:46medical record,
- 6:48pull a diagnosis code, pull previous
- 6:50therapies that the patient has failed,
- 6:53answer the multitude of questions that that
- 6:57come from a a payer.
- 6:59And every payer is different, and every payer
- 7:01has different questions and different criteria.
- 7:03So the these prior authorizations
- 7:06take up a lot of time and take
- 7:08a lot of expertise to handle.
- 7:10So by bringing in a ace an AI
- 7:13solution,
- 7:14it's integrated into the medical record. And once
- 7:18a prescription needs a prioritization,
- 7:20the software
- 7:22pulls the appropriate form
- 7:24and populates it. So answers the questions,
- 7:28with reference for the individual.
- 7:30So now, the liaison, instead of having to
- 7:33dig through a medical record, whether whether it's
- 7:36digital,
- 7:37media or or legacy media that's been uploaded,
- 7:40the liaison just needs to verify the answers.
- 7:43Right? So
- 7:45the question is populated. Liaison verifies that the
- 7:48answer is is appropriate, checks the the reference,
- 7:51and then submits the prior authorization.
- 7:53This leads to a
- 7:55significant decrease in the time to complete a
- 7:58prior authorization.
- 7:59We were trending at twenty to twenty five
- 8:01minutes per PA.
- 8:03That's come down,
- 8:05by at least 50%. We're we're targeting at
- 8:07least a 75 to 80% decrease in time
- 8:10to complete PA.
- 8:12But, secondarily,
- 8:14our first pass approval of prioritizations
- 8:17has increased significantly.
- 8:19Right? So reducing the
- 8:21the work of a pharmacist writing a letter
- 8:24letter of medical necessity or going back to
- 8:27a provider to ask
- 8:29additional questions.
- 8:31All this
- 8:32gets the patient to therapy sooner
- 8:35with with less with with less interference,
- 8:39which which is the ultimate goal. And like
- 8:41I said previously,
- 8:42those those liaisons are now doing more patient
- 8:45facing work and and
- 8:47being of more value to the patient,
- 8:50in their care journey.
- 8:51So that that's the first one,
- 8:54which really utilize AI.
- 8:56And then the second one is the the
- 8:58digital
- 8:59front door. So developing a digital marketplace
- 9:03for a patient to to interact with their
- 9:05medications
- 9:06that's fully integrated
- 9:07within,
- 9:08the MyChart experience.
- 9:10So, basically, if a patient gets a prescription
- 9:13and they get a text message,
- 9:15they'll
- 9:16click the link there. It'll take them into
- 9:18the MyChart experience,
- 9:19and they'll have a similar
- 9:22shopping experience as they would anywhere else on
- 9:24the Internet.
- 9:25Those are great examples.
- 9:27And I love hearing about those, success outcomes
- 9:30that you described.
- 9:32Kind of on that, you know, same note
- 9:33of the metrics, how are you and your
- 9:35organization defining success when it comes to digital
- 9:38innovation as a whole for pharmacy?
- 9:41So I think it all comes back to,
- 9:43number one, the patient experience,
- 9:45and whether
- 9:47they are have a a an experience that
- 9:50that they like and that they they can
- 9:52keep coming back to. Right? Keeping that patient
- 9:55engaged with with the health system,
- 9:58on an ongoing basis is is beneficial. Right?
- 10:00So it the patient engagement, but then secondarily,
- 10:04you know, what what is our efficiencies
- 10:07on the back end. Right? Can we
- 10:10repurpose staff to do more beneficial work for
- 10:13the patient?
- 10:14You know, one example is, you know, we
- 10:16have 20 liaisons just doing
- 10:19GLP one prior authorizations.
- 10:21What does the world look like when we
- 10:23can bring that work work from 20 down
- 10:25to five?
- 10:26You know, maybe we can repurpose
- 10:28those 15 liaisons to do
- 10:30more financial
- 10:31navigation
- 10:32for the patient. Right? If we can get
- 10:35patients more financial assistance
- 10:38so that they can continue on therapy and
- 10:40have access to more therapies,
- 10:43we have better outcomes. Right? If we can
- 10:46get those employees to
- 10:48to do outreach to patients to ensure adherence,
- 10:52then they can have better outcomes. So at
- 10:54the end of the day,
- 10:55any any goals that we set for ourselves
- 10:58is always with the patients at the heart
- 11:01of it and ensuring that the patients'
- 11:03outcomes and experience are are best.
- 11:06For sure. And I think those are important
- 11:08things that any health system should be keeping
- 11:10in mind as well. So thank you for
- 11:12sharing.
- 11:13And, of course, with any new technology, there
- 11:15are always a few bumps along the way.
- 11:17What challenges have you encountered in implementing digital
- 11:20tools, and how have you addressed these issues?
- 11:23Yeah. So anyone that's that's worked in a
- 11:25health system or a large matrixed organization,
- 11:29knows the word governance.
- 11:30So there is a lot of governance that
- 11:32that happens
- 11:33especially within a health system,
- 11:36for for many reasons. And, you know, many
- 11:38many of these reasons are are are valid.
- 11:41But it just it takes a lot of
- 11:43time and effort and persistence
- 11:45to
- 11:47to to get any anything
- 11:49live within the health system.
- 11:52And then
- 11:53secondarily,
- 11:54you know, the the the difficulty of of
- 11:57such an endeavor
- 11:59increases when you start talking about AI,
- 12:02because, you know, the governance is being built
- 12:04as we speak. So,
- 12:07really working with
- 12:09your partners within your organization,
- 12:12specifically your your IT partners,
- 12:15your legal partners,
- 12:18is
- 12:19is so important
- 12:20in order to
- 12:22to be efficient and get some of these
- 12:25products to, to market and to to a
- 12:28live state.
- 12:30You know,
- 12:31a lot of times,
- 12:33vendors come to us with with new ideas
- 12:36and and
- 12:38and not really fully baked,
- 12:41products.
- 12:42And, you know, we we a lot of
- 12:44times, we we just do not move forward
- 12:47with them because, you know, we know that
- 12:49in in order to be successful in in
- 12:51a health system setting,
- 12:54we need to have something that that is
- 12:56is fully baked and
- 12:58ready to be deployed and and has a
- 13:01proven
- 13:02use case. So
- 13:04partnering
- 13:05internally,
- 13:06as well as externally and and being fully
- 13:08transparent with with everybody that's involved,
- 13:12on on the process and timeline.
- 13:15For sure. That's a really helpful perspective, and
- 13:17it's clear that your team has learned a
- 13:18lot from the digital implementation process.
- 13:21I'd like to shift now to the patient
- 13:23side of things for a moment. How are
- 13:24digital tools changing the patient experience in specialty
- 13:27pharmacy at Mount Sinai for you guys?
- 13:30Again, it's it's going back to
- 13:33transparency
- 13:34and access.
- 13:36I think
- 13:38with with our digital offering, now we're able
- 13:42to to give them
- 13:44a more
- 13:45streamlined
- 13:46way of of interacting with us. But then
- 13:49secondarily,
- 13:50you know, it it comes down to
- 13:53a patient's ability
- 13:55to access a medication, whether that's a prior
- 13:57authorization, whether it that's a payer putting
- 14:01some limitations on on access to medication or
- 14:04whether that's cost.
- 14:06A lot of times, the cost of a
- 14:08medication is
- 14:10is too high,
- 14:12and the patient doesn't have any any way
- 14:14of
- 14:15accessing.
- 14:17So
- 14:18utilizing our our digital
- 14:20landscape
- 14:22to to to give them the the tools
- 14:24to, to find the financial assistance. Right? Whether
- 14:27it's linking to a
- 14:29manufacturer discount card, whether it is
- 14:32giving them a,
- 14:34information about a foundation.
- 14:37Those those things are so important. So really
- 14:40utilizing the the digital
- 14:43front door,
- 14:44to to give that patient that trust trusted
- 14:48source
- 14:49for financial information,
- 14:51and then backing that up with with our
- 14:53liaisons
- 14:54who can do that financial navigation.
- 14:57That's the thing that I'm most excited about.
- 14:59I think that's something that is a very
- 15:02gray area within pharmacy, specialty pharmacy especially,
- 15:06and that there are a lot of different
- 15:08funding options,
- 15:10outside of the traditional payers that patients are
- 15:14not aware of. So being able to
- 15:16to direct the patients to to these options
- 15:19so that they have access to these really
- 15:23expensive therapies. Medications are not getting any cheaper.
- 15:26You know, they're may getting more complex and
- 15:28more expensive.
- 15:29So it's so important to have that that
- 15:32ability.
- 15:33For sure. That's such an important point, especially
- 15:35as, you know, patient expectations continue to evolve.
- 15:39Now looking ahead, where do you see the
- 15:40greatest opportunities for digital transformation in specialty pharmacy
- 15:44over the next few years?
- 15:46You know,
- 15:47the landscape,
- 15:49especially now,
- 15:51with direct to consumer
- 15:54becoming more and more of an option, the
- 15:56landscape is changing.
- 15:58And there is a lot of
- 16:01untrustworthy
- 16:02information
- 16:03out there. And a lot of times when
- 16:07a patient
- 16:08goes online to to search about their medical
- 16:11condition
- 16:12or about their medication
- 16:14or anything health related,
- 16:16they're they can be led astray.
- 16:19So having a
- 16:22trusted source that they can always come back
- 16:25to,
- 16:26that is
- 16:28is is fully,
- 16:29you know,
- 16:30fully capable of of
- 16:32leading the patient on that care journey, is
- 16:35is so important. So I I see health
- 16:38systems
- 16:39and health system pharmacies
- 16:41being that source of truth in the future.
- 16:43Right?
- 16:44Imagine a world where a patient
- 16:46can log on to their their health system
- 16:48portal
- 16:49and and access
- 16:51any kind of information that they want about
- 16:53their their health,
- 16:55whether it's a question about an acute condition,
- 16:58whether it's a question about medication access, or
- 17:02or, you know, something about a a side
- 17:05effect
- 17:06for their long term condition?
- 17:09What if it's a a
- 17:11a drug interaction with an herbal medication that
- 17:14they're taking?
- 17:16There are a lot of resources out there,
- 17:18but it's it's unknown which ones are to
- 17:21be trusted and which ones are not. But
- 17:23the health system name and logo
- 17:26is is always a trusted resource. So
- 17:29having a one stop shop for a patient
- 17:31to to go for all their health care
- 17:33needs, that's where I see the future,
- 17:36of
- 17:37of digital
- 17:39specialty pharmacy within health system.
- 17:42Gotcha.
- 17:43Well, thank you so much, doctor Mecheson. This
- 17:45has been such an insightful conversation, and thank
- 17:47you for sharing your perspective and experience
- 17:50with us.
- 17:51That's all for today's episode of the Becker's
- 17:53Pharmacy leadership podcast. A big thank you again
- 17:55to doctor Mackensen for sharing his insights on
- 17:58digital innovation and specialty pharmacy. Thanks for listening,
- 18:01and we'll see you next time. Thank you,
- 18:03Ella.
