Latest / Rethinking Weight Loss with Eli Luft / Beginners Guide to Ozempic and GLP-1's
Transcript
- 0:00SPEAKER_00: If you're new to OZEPIC, Wigovi, Winjaro, Zeppbound, or any other GLP medications, you probably have a lot of questions.
- 0:08SPEAKER_00: What does it feel like?
- 0:08SPEAKER_00: How do you know it's working?
- 0:10SPEAKER_00: What happens if you eat too much?
- 0:12SPEAKER_00: And do you really have to stay on it forever?
- 0:14SPEAKER_00: I'm a big fan of these medications.
- 0:16SPEAKER_00: I've seen them help a lot of people who had struggled with their weight for years.
- 0:20SPEAKER_00: But the beginning can be rough if you don't know what to expect.
- 0:23SPEAKER_00: I'm Eli Luft, a physician associate at Brevas Medical Weight Loss.
- 0:27SPEAKER_00: So let me show you what the first months usually look like and the things I wish every patient knew before they started.
- 0:33SPEAKER_00: Start with the names, because they cause a lot of confusion.
- 0:35SPEAKER_00: Ozempic and Wigovi are the same medication and they're called semaglutide.
- 0:41SPEAKER_00: Ozempic is the version approved for diabetes, and Wigovi is the version approved for weight loss.
- 0:46SPEAKER_00: Munjaro and Zeppound are also the same medication as each other.
- 0:49SPEAKER_00: They're called terzepitide.
- 0:51SPEAKER_00: Same deal there.
- 0:52SPEAKER_00: Munjaro for diabetes, Zeppound for weight loss.
- 0:55SPEAKER_00: Four names, basically two medications, and a whole lot of similar meds that aren't the name brands at all.
- 1:01SPEAKER_00: Everybody says Ozempic because it got here first, so it sort of became the word for all of them, the way Kleenex did.
- 1:07SPEAKER_00: Nobody asks you to pass the facial tissue or a band-aid.
- 1:10SPEAKER_00: What do you even call a band-aid?
- 1:12SPEAKER_00: If you couldn't call it band-aid, would you call it a sticky thing with a little gauze in the middle?
- 1:17SPEAKER_00: You get the point.
- 1:18SPEAKER_00: There was an earlier one too, Victosa, which when it was approved for weight loss was called Sixenda.
- 1:24SPEAKER_00: Same idea, except it was a daily injection and it wasn't as good.
- 1:28SPEAKER_00: Most people don't want to inject themselves once a week, let alone every single day.
- 1:32SPEAKER_00: So it never really took off.
- 1:33SPEAKER_00: Around 2021 is when this exploded, and that's about when Oprah came out and talked about what it did for her.
- 1:39SPEAKER_00: What she did for weight loss is something Dr.
- 1:41SPEAKER_00: Rivas had been trying to do for more than 30 years, which is make it normal to get help.
- 1:46SPEAKER_00: Your blood pressure's high, you get help for that.
- 1:49SPEAKER_00: And nobody thinks twice about it.
- 1:50SPEAKER_00: With weight, you're supposed to handle that on your own, but not anymore.
- 1:54SPEAKER_00: So what does the medication actually do?
- 1:56SPEAKER_00: You get full and you stay full.
- 1:58SPEAKER_00: Think about Thanksgiving dinner.
- 2:00SPEAKER_00: You finish that meal.
- 2:00SPEAKER_00: Do you want to go run a marathon?
- 2:02SPEAKER_00: Nope.
- 2:02SPEAKER_00: You want to lie down.
- 2:04SPEAKER_00: That's kind of how you'll feel on a GLP1, the full part at least.
- 2:07SPEAKER_00: And because you get full fast and stay that way, you end up eating around 30% less food without thinking about it.
- 2:12SPEAKER_00: That's why you lose weight because you're eating less.
- 2:15SPEAKER_00: One of our patients put it as being satisfied with one bite of a donut instead of two or three.
- 2:19SPEAKER_00: Another patient of ours, a guy who lost about 100 pounds, said he changed absolutely nothing about what he ate.
- 2:25SPEAKER_00: He ate the same food, he just ate less of it.
- 2:28SPEAKER_00: People tell me all the time, I still go to McDonald's, I just get a happy meal instead of two Big Macs, and I can only get through about 12 French fries, where I used to eat all of them.
- 2:36SPEAKER_00: The other thing patients describe is how the food noise goes quiet.
- 2:40SPEAKER_00: A lot of people spend all day thinking about food.
- 2:42SPEAKER_00: They're eating breakfast and thinking about lunch.
- 2:45SPEAKER_00: They're eating lunch and thinking about dinner.
- 2:47SPEAKER_00: They're thinking about the snack in between.
- 2:49SPEAKER_00: They're thinking about whether they're going to go out to eat on Friday.
- 2:52SPEAKER_00: On this medication, a lot of that just stops.
- 2:54SPEAKER_00: I've had people tell me they don't even know what to think about anymore.
- 2:57SPEAKER_00: For most of them, it's a relief.
- 2:59SPEAKER_00: They had no idea that was even possible.
- 3:01SPEAKER_00: So who are GLP ones best for?
- 3:03SPEAKER_00: The people who come into our clinics know that they're overweight, and they've usually known for a long time.
- 3:08SPEAKER_00: But usually what finally causes them to come in is something else that happened.
- 3:12SPEAKER_00: Their blood work came back prediabetic.
- 3:15SPEAKER_00: Their knee or their hip got bad enough that they went to a surgeon and the surgeon told them that they won't operate until they lose some weight.
- 3:20SPEAKER_00: That last one is more common than you'd think, and I know it's a hard thing for people to hear.
- 3:24SPEAKER_00: I had a patient in exactly that spot.
- 3:26SPEAKER_00: They needed a knee replacement, and the weight made them a bad candidate for the surgery.
- 3:30SPEAKER_00: They lost about 90 pounds and then they canceled the knee replacement because they didn't need it anymore.
- 3:35SPEAKER_00: If you're in Maryland, Virginia, or Florida and you want somebody going through all of this with you in person, the link in the description books your first visit.
- 3:42SPEAKER_00: You'll see what's open and pick your own time.
- 3:44SPEAKER_00: For our patients, we give the first shot in office.
- 3:47SPEAKER_00: So nobody's standing in their kitchen at 7 in the morning trying to work out what to do with a needle they've never held before.
- 3:51SPEAKER_00: The dose starts low and goes up slowly over time, so your body gets a chance to get used to it.
- 3:57SPEAKER_00: But not everyone has to increase.
- 3:58SPEAKER_00: So that's why it's important to not do this on your own and end up with a higher dose than you need.
- 4:03SPEAKER_00: Or have your primary care doctor keep you at the same ineffective dose for six months.
- 4:08SPEAKER_00: The most common thing I see is tiredness or fatigue.
- 4:11SPEAKER_00: Same reason as the Thanksgiving thing.
- 4:13SPEAKER_00: You're full and you're eating a lot less, so there's less coming in.
- 4:16SPEAKER_00: For some people though, it's the complete opposite.
- 4:18SPEAKER_00: They were always tired from eating so much that they suddenly have energy that they haven't seen in years.
- 4:23SPEAKER_00: And plenty of people get no side effects at all.
- 4:26SPEAKER_00: Pretty much everyone has a similar experience for the first few meals once you start at GLP1.
- 4:30SPEAKER_00: You'll still get hungry, which surprises people.
- 4:32SPEAKER_00: Taking hunger away completely is very hard.
- 4:35SPEAKER_00: And that isn't what the medication does.
- 4:37SPEAKER_00: It gets you full faster, and that's how you know it's working.
- 4:40SPEAKER_00: Round one goes like this you serve yourself the same plates you always serve yourself.
- 4:44SPEAKER_00: Partway through your brain gives you the signal to stop, and you feel like you just started and it tastes good.
- 4:50SPEAKER_00: So you push past it the way you always have.
- 4:52SPEAKER_00: Then the nausea arrives and you regret not listening to your body.
- 4:56SPEAKER_00: Round two, you're more experienced now.
- 4:58SPEAKER_00: You serve yourself less food, you get that full signal again, but there's two or three little bites still on your plate.
- 5:04SPEAKER_00: Groceries are expensive and you don't want to waste it.
- 5:06SPEAKER_00: And it's not worth dirtying a piece of Tupperware and having to wash the later.
- 5:10SPEAKER_00: So you take those last two bites, and there you go.
- 5:13SPEAKER_00: Nausea comes right back.
- 5:14SPEAKER_00: Round three is where it clicks.
- 5:16SPEAKER_00: Wherever you are in that meal, when that signal says stop, you put your utensils down, you slide the plate away with a stiff hand, and you decide you don't care what happens to that food, just take it away.
- 5:27SPEAKER_00: Take it out from in front of me.
- 5:28SPEAKER_00: That's what people mean when they say this medication retrains you.
- 5:31SPEAKER_00: It's teaching you to listen to a signal you've spent your whole life overriding.
- 5:35SPEAKER_00: Food will do it to you as well.
- 5:37SPEAKER_00: Greasy and oily things are the big one, and so is going way too long without eating, which is tricky for people because they aren't getting hungry the way that they used to.
- 5:44SPEAKER_00: So they skip lunch and feel awful by four in the afternoon.
- 5:48SPEAKER_00: You have to eat at closer intervals even if you're not hungry.
- 5:50SPEAKER_00: And sometimes the nausea just happens because the medication is doing what it does.
- 5:54SPEAKER_00: Then the question I get more than any other, do I have to be on this forever?
- 5:58SPEAKER_00: Most people should plan on staying on it.
- 6:00SPEAKER_00: Whatever you do to lose weight, the day you stop doing that thing, it starts coming back.
- 6:04SPEAKER_00: That's true of a diet, it's true of a nutritionist who tells you to eat half a cup of yogurt, two almonds, and one slice of turkey.
- 6:10SPEAKER_00: Because the day you stop eating like that, you're regaining.
- 6:13SPEAKER_00: And it's true of this.
- 6:15SPEAKER_00: Some people do maintain their weight after they stop, but the ones who manage it are working at it every day.
- 6:21SPEAKER_00: One small thing to do before your first shot is look at the name on the box and know which of the two medications you're holding, semaglutide or trosepatide.
- 6:29SPEAKER_00: You'd be surprised how many people have been on this for three months and don't know which they're on.
- 6:34SPEAKER_00: Then, once you're actually taking it, there's a short list of foods and habits that either make you feel sick or keep you from losing weight.
- 6:40SPEAKER_00: I made a whole video on that one, so go watch that next.
- 6:43SPEAKER_00: Subscribe so you don't miss the next one, and I'll see you there.
