1. The warning before the Urda crash
Spain's rail accident investigators warned in May 2025 that poor communication between traffic control centres and train drivers was not a one-off but a "systemic" problem. Two days after the high-speed crash near Urda, in Toledo, where at least 69 people died, the probe is focusing on the orders the control room gave the drivers. Transport minister Óscar Puente says the people in the control centre "could see what was happening".
