- 0:00Hello, everyone, and welcome to Becker's Healthcare Podcast.
- 0:02I'm Scott King. Thrilled today to be joined
- 0:04by a very special guest, Gretchen Wagner, associate
- 0:07vice president, risk management with Humana. Gretchen, how
- 0:10are you doing? Thanks so much for joining
- 0:12us. Sure. Thanks for having me, Scott.
- 0:14Yeah. You know, we have a lot of
- 0:16big topics to get to here, you know,
- 0:18on the podcast in in health care and
- 0:20with health plans, and we're we're definitely looking
- 0:22forward to having you speak with us at
- 0:23our spring payer issues roundtable.
- 0:25But before we, before we get to that,
- 0:27I was wondering if you could just tell
- 0:28us a little bit about your background and
- 0:30your career.
- 0:31Sure. So I actually started out in education,
- 0:35a little bit nontraditional to get into health
- 0:37care, but then made my shift into internal
- 0:40audit. And that is how I ended up
- 0:43making my way into health care coming in
- 0:45from an internal audit perspective,
- 0:47and found that was a great way to
- 0:49really learn a company as a new associate
- 0:52and a young employee at that point in
- 0:54time because it gave me exposure to a
- 0:56lot of different parts of
- 0:58a company. Right? I got to learn different
- 1:00parts and pieces and how they work and
- 1:02meet leadership.
- 1:03And from there,
- 1:05I really moved up and found my niche
- 1:08from a clinical perspective,
- 1:10at Humana and have been with the the
- 1:13organization now for over fourteen years
- 1:16and lead our SNP governance.
- 1:18So work closely with our SNP models of
- 1:20care, serve as a first line risk and
- 1:22compliance partner,
- 1:24support a lot of our quality governance components.
- 1:27Doing a lot right now with our integrated
- 1:29D SNPs as we're seeing our Medicaid and
- 1:32Medicare programs come more closely together
- 1:35across that D SNP landscape,
- 1:37and also work very closely with our clinical
- 1:39operations team. So,
- 1:41excited to be here and,
- 1:43excited to talk about really kind of the
- 1:45the things we're seeing from a health care
- 1:47perspective,
- 1:48right now in the state of our our
- 1:50health players.
- 1:52Well, appreciate you sharing that background info there,
- 1:54Gretchen. And, you know, the first kind of
- 1:55big topic I wanted to get to
- 1:57is how are your relationships with providers changing
- 2:00as both sides face cost pressure and workforce
- 2:02shortages?
- 2:04Sure. So I would say
- 2:06we are seeing very significant changes in the
- 2:09last few years.
- 2:11Right? We're starting to see
- 2:14unprecedented
- 2:15cost pressures
- 2:16that are only continuing
- 2:18from especially an MA perspective and a Medicaid
- 2:21perspective as well as workforce shortages.
- 2:24You know, when we think about historically what
- 2:27relationships look like between payers and providers, we
- 2:30always had very transactional
- 2:33based
- 2:34relationships that were very fee for service type
- 2:36payments where providers were reimbursed for individual services
- 2:41or procedures,
- 2:42and payers really focused on that claims processing
- 2:44and cost containment.
- 2:46And we found that model created silos and
- 2:49at times really misaligned
- 2:50incentives. So one of the things we're really
- 2:53doing at Humana today is seeing a shift
- 2:55towards a much more collaborative approach
- 2:58where we think about partnership and accountability
- 3:01with a value based model.
- 3:03So in those arrangements, we're really incentivizing
- 3:06our providers to not just think about the
- 3:08volume of care, but really how are we
- 3:11delivering on quality
- 3:13and achieving outcomes.
- 3:15And it really is thinking about how can
- 3:17we align our goals together
- 3:19so that we can see our patients get
- 3:22healthier,
- 3:23avoid unnecessary
- 3:25hospitalizations,
- 3:26and, ultimately, we really wanna make sure our
- 3:29members are receiving care
- 3:31at the right time.
- 3:33We've also found, I think, through that as
- 3:35we think about that value based care model,
- 3:38that it allows more innovation and care delivery,
- 3:41and it really allows us to think about
- 3:44recognizing
- 3:44and rewarding
- 3:46our payers,
- 3:48when they have better coordination,
- 3:51fewer gaps in care, and a much more
- 3:53personalized approach. So really thinking about a much
- 3:56more holistic and streamlined experience. So
- 3:59as we think about
- 4:01how things have changed with this cost pressure,
- 4:05the workforce shortages that we're having,
- 4:08we've really got to rethink how we incentivize
- 4:11our payers and how we incentivize
- 4:13those relationships.
- 4:15So it's definitely been a shift as we
- 4:18make that shift and that change towards those
- 4:20value based
- 4:21care arrangements.
- 4:23But we are finding
- 4:24it truly is
- 4:26allowing us to be more effective and efficient
- 4:29and also make sure we've got better
- 4:31partnerships
- 4:33across our our payers,
- 4:36but also allowing to make sure that,
- 4:39we can put the member first
- 4:41when it comes to their health and their
- 4:43outcomes as well when we take that approach.
- 4:47Yeah. Absolutely. And I love the idea of
- 4:49of focusing on quality there. It's something to
- 4:51kind of put in the middle of that
- 4:52payer provider relationship to kind of get both
- 4:54sides to come to the table. I I
- 4:56think that's great. And I also wanna ask,
- 4:58where do you see the biggest
- 4:59gap today between
- 5:01payer strategy and operational execution?
- 5:03Yeah.
- 5:04So
- 5:06I honestly think the biggest bur
- 5:08burden that we see
- 5:10is how do we take that long term
- 5:12vision
- 5:13that we have as leaders
- 5:15and translate that into day to day operations.
- 5:18Right? Oftentimes, we have
- 5:21big big rocks that we all are working
- 5:23towards from a health care perspective as we
- 5:26think about innovation and quality,
- 5:28even think about things like utilization of AI
- 5:31from a strategy perspective.
- 5:33And it's how can we take that and
- 5:37realize that
- 5:39in the daily work of our frontline teams.
- 5:42That's something that is not a unique gap
- 5:46to Humana by any means.
- 5:48It's something that the entire industry is facing,
- 5:51particularly with this current pace of change.
- 5:54One of my favorite sayings that I continue
- 5:56to hear, both internal to Humana
- 5:59but external in the environment we're living in,
- 6:03is that the pace of change today
- 6:06is as slow as it will ever be.
- 6:09And that's kind of hard to take in
- 6:11because we are facing a rapid pace of
- 6:14change
- 6:15from a technology
- 6:16perspective,
- 6:17from a structure and infrastructure
- 6:19perspective,
- 6:21from a way in which we live perspective,
- 6:23from a,
- 6:24you know, a day to day function perspective.
- 6:27There is so much that is happening
- 6:30rapidly,
- 6:31and we are only gonna continue to see
- 6:34that progress.
- 6:35And that can be hard for a lot
- 6:37of our frontline associates,
- 6:39to adapt to
- 6:41as well as our members
- 6:43and our patients to adapt to,
- 6:45as we go through this this change acceleration.
- 6:49So with me saying that, right, of how
- 6:52do we take this
- 6:54strategic vision
- 6:55where we want to innovate, where we wanna
- 6:58drive quality,
- 6:59and make a better experience for our members
- 7:03is
- 7:04now how do we how do we go
- 7:05about doing that and doing that in the
- 7:07right way
- 7:08in this time of of growth and technology?
- 7:11So I think one is, like, how do
- 7:12we start thinking about digital innovation?
- 7:15Because our our senior population
- 7:19is no longer
- 7:21a population that is just working on
- 7:24on telephone calls and and letters in front
- 7:27of them.
- 7:28They are someone who have started to adapt
- 7:31things like
- 7:32cell phones, mobile apps, online platforms. They want
- 7:36virtual care options. They want telehealth options. And
- 7:39so we've gotta find
- 7:41coordinated ways
- 7:43to bring multiple teams together
- 7:45across not only our operational teams, but our
- 7:48clinical teams and our provider functions
- 7:51to make sure that we're doing this and
- 7:53doing it appropriately.
- 7:55So I think that's one of the things
- 7:57that we really gotta find is how can
- 7:59we find ways as organizations
- 8:02to create cross functional implementation
- 8:05that includes
- 8:07feedback loops from providers,
- 8:10from members,
- 8:11and things like I think we've got some
- 8:13really good platforms at Humana
- 8:16that are,
- 8:16in many ways, regulatory
- 8:18requirements for us and things like our D
- 8:21SNP and our Medicaid
- 8:23enrollee advisory committees where we can seek out
- 8:26member feedback
- 8:27and try out some of our our new
- 8:29technology with them. But it's also making sure
- 8:32we're getting the feedback from associates and from
- 8:35our frontline teams
- 8:36to make sure that they are front and
- 8:38center
- 8:39when it comes to
- 8:41the execution
- 8:42and how they can execute on that strategy
- 8:46at a day to day level. So it's
- 8:48really bringing everyone to the to the table,
- 8:51closing that gap, and making sure that we're
- 8:53building systems and processes
- 8:56that consider continuous improvement
- 8:58and collaboration and communication.
- 9:01Yeah. And I like what you said about
- 9:03change. And, you know, it's kind of a
- 9:04it's kind of a bit jarring, isn't it,
- 9:05too? Like It is. Like emerging tech, and
- 9:07is it if this is not gonna slow
- 9:08down, so you No. You have to jump
- 9:10on board right now.
- 9:12Yeah. It it really is. Like,
- 9:14it's one of those that I feel like
- 9:16we are in such a time of transformation,
- 9:19both as a society, but for us in
- 9:21health care. This is our time to
- 9:24challenge what we have been doing
- 9:26and really think about the utilization of technology
- 9:30in new ways. Right. Because there are other
- 9:34industries
- 9:35who are doing things differently
- 9:38to think about that member experience in a
- 9:41unique way
- 9:42that we haven't yet executed on in the
- 9:45in the health care industry.
- 9:47And so I think this is this is
- 9:48really our opportunity
- 9:50as we are seeing
- 9:52technology advance
- 9:54advancement and just the changes that are happening
- 9:56from a regulatory perspective
- 9:59as well as a societal perspective, right, and
- 10:01the adoption of
- 10:03more people being open to the utilization of
- 10:05technology and those digital platforms
- 10:08coming together.
- 10:09No. I I agree. I think you're you're
- 10:11right 100%.
- 10:12I also wanna ask what's one investment or
- 10:15initiative you believe will most reshape how health
- 10:18plans operate over the next two to three
- 10:20years? Yeah. Honestly, it goes back as what
- 10:22we've been talking about, and that's that's digital
- 10:25it's digital analytics.
- 10:27And getting even more specific, I think it's
- 10:30the analytics
- 10:31and the interoperability, but it's across the health
- 10:34care ecosystem.
- 10:35Right? Right now,
- 10:37there is a lot of data,
- 10:39and there is a lot
- 10:40of information
- 10:41that we have
- 10:44across health plans,
- 10:45across health systems,
- 10:48but it's fragmented.
- 10:49And when it's fragmented,
- 10:51we aren't seeing a holistic view of a
- 10:54patient's health journey or a member's health journey.
- 10:58And so one of the things that I
- 11:00think we need to invest in and one
- 11:02of the things we're focusing on at Humana
- 11:04is investing heavily in data infrastructure
- 11:07and advancing analytics
- 11:08so that we can really bring all of
- 11:10that information together from multiple sources.
- 11:13So So think about things like
- 11:15clinical records,
- 11:17pharmacy data, member interactions. So when did we
- 11:20call a member? Who called a member? When
- 11:23did they pick up the phone? And even
- 11:25things we might know about them,
- 11:27such as social determinants of health,
- 11:29right, based upon
- 11:31maybe they have food insecurity,
- 11:34maybe they have a transportation need. So it's
- 11:37really how can we build comprehensive
- 11:39views
- 11:41of our membership
- 11:42to know
- 11:44how to be more pro proactive
- 11:46in outreaching members and especially outreaching members
- 11:50at the right time.
- 11:52Right? We've gotta make sure that we can
- 11:55take data
- 11:56and utilize it for good
- 11:58and not just take data
- 12:00to have data
- 12:02and report on data.
- 12:04We've gotta make sure we're using data
- 12:07in the right way and pulling it together
- 12:09through analytics,
- 12:10through interoperability,
- 12:12sharing that data
- 12:14to make sure we're really tailoring it to
- 12:16drive efficiency
- 12:18in the payer system, but across the broader
- 12:21health care system and really focusing on that
- 12:23member experience through it.
- 12:26If you could change
- 12:27one regulatory or industry practice tomorrow to improve
- 12:30affordability and access, what would it be and
- 12:33why? Sure.
- 12:35Well, I feel like I'm I keep talking
- 12:37about the same thing a little bit, but
- 12:39I I think it also shows how important
- 12:41out it is, and that's interoperability.
- 12:43Right? And that is something we have got
- 12:46to change and speed up even
- 12:48more so than we have today.
- 12:50We have got to
- 12:53I think about this from my own perspective
- 12:55frequently
- 12:56both as
- 12:58a a patient and as a caregiver,
- 13:02for my mother who has complex conditions and
- 13:05sees a number of specialists and doctors across
- 13:07networks and networks and provider groups, and how
- 13:11frequently
- 13:12we don't see the full picture of a
- 13:14member because we don't have that information,
- 13:17and how frequently we we have inefficiency
- 13:22in the work that we do.
- 13:24How often do we create duplicate tests? How
- 13:28often do we miss something?
- 13:30How often do we have the chance to
- 13:32take action we can't because we don't have
- 13:35the right information,
- 13:36but it's sitting in another payer's health system,
- 13:39or it's sitting in another
- 13:41health system system at a hospital or at
- 13:44a doctor's office.
- 13:46Think about how how many hours just we
- 13:49as
- 13:50patients would say if we didn't have to
- 13:52fill out the same form every time we
- 13:54went to the doctor's office because that information
- 13:57was up to date and timely.
- 14:00We could save ourselves, could save hours
- 14:03over the course of years in doing that.
- 14:05And so we really have a chance to
- 14:08rethink about data
- 14:10and bringing that together
- 14:12to create
- 14:14a better member experience,
- 14:16but truly
- 14:18having the chance
- 14:19to create
- 14:20a more complete picture for our providers, for
- 14:23them to make better decisions
- 14:25and create a better treatment plan.
- 14:27And for health plans, it's gonna mean that
- 14:30we can improve the accuracy of what we
- 14:32do
- 14:33and make sure we're supporting,
- 14:36our populations in a more effective manner. And
- 14:39so going back to where we started,
- 14:42talking about some of the cost pressures we're
- 14:44feeling.
- 14:45The more that we can streamline data and
- 14:47have the right data, that's only going to
- 14:50help us as health plans
- 14:52in a time where we're facing more from
- 14:54a a margins perspective
- 14:57and and the cost pressure from a regulatory
- 14:59aspect that we're seeing right now. So this
- 15:01is definitely a place where
- 15:04while
- 15:05a lot is moving quickly
- 15:07from a regulatory aspect for us to get
- 15:09there from an interop aspect,
- 15:11there's an opportunity for us to move even
- 15:14quicker here.
- 15:15And that kinda ties into the last thing
- 15:17I wanted to ask you, Gretchen, along the
- 15:18lines of margins. And you you might have
- 15:20touched on this already, but what issue do
- 15:22you think is putting the most pressure on
- 15:23health plan margins right now, and how how
- 15:26are you responding differently in 2026?
- 15:29Yeah. It's it's interesting. I feel like it's
- 15:31such a timely question as we've been seeing
- 15:35all of the,
- 15:37the end of year
- 15:39financial
- 15:40reports come out from the large health plans
- 15:43and just how
- 15:44things from the industry perspective that are coming
- 15:46out right right now about what is happening
- 15:49in the health care industry. So it's an
- 15:51interesting time for us.
- 15:53And there definitely is pressure that we have
- 15:56not seen,
- 15:58especially in the MA space or the Medicaid
- 16:00space.
- 16:01And I don't think it's gonna slow down.
- 16:03And I think there are really
- 16:04three large contributors
- 16:06to that. So one is just rising health
- 16:08care cost.
- 16:11Primarily,
- 16:12we're seeing that through the price of specialty
- 16:14medications and advanced treatments.
- 16:17But in general, we're just seeing the cost
- 16:19of health care go up. Everything from a
- 16:21cost perspective is rising.
- 16:23Two, I think we're seeing greater utilization.
- 16:26Our boomer population is aging.
- 16:29We're seeing chronic conditions become more prevalent.
- 16:32But I also think we've created
- 16:34where people are becoming more proactive
- 16:38and seeking
- 16:39care, which is positive, but that drives up
- 16:42that utilization.
- 16:44And then really the last piece is just
- 16:45the administrative costs, so the cost to do
- 16:48business is going up everywhere.
- 16:50That doesn't matter what industry you're in. That's
- 16:53not unique to health care, but it's something
- 16:55we're seeing across industries.
- 16:57And I think the last piece, as we
- 16:58know, is the regulatory
- 17:00requirements. Right? We've seen cuts in Medicaid.
- 17:03We have
- 17:04the proposed
- 17:06you know, payment that came out on the
- 17:08MA space with the reimbursement rate standing pretty
- 17:12flat with a slight increase,
- 17:15that's been a lot of the stocks dropping
- 17:17massively last week.
- 17:19So
- 17:20this is not something
- 17:21that's gonna go away for sure. And so
- 17:24I think,
- 17:25you know, when we think about this, we
- 17:27have to definitely think from a health plan
- 17:29perspective.
- 17:30We've got to be
- 17:33thinking
- 17:34about efficiency,
- 17:36innovation, and value.
- 17:39And this really is our chance to think
- 17:41differently
- 17:43about how we show up,
- 17:46who we are as a company,
- 17:48and how we're gonna serve our members. It
- 17:50really is our chance to distinguish ourselves from
- 17:53each other
- 17:54and really think about that member experience
- 17:57if we ground ourselves in the efficiency, innovation,
- 18:00and value,
- 18:02from an organization
- 18:03perspective. And that's something we're really focused on
- 18:05here at Humana.
- 18:07So
- 18:08I I would say when I think about
- 18:09this
- 18:10operational efficiency,
- 18:12we've all gotta think about leveraging automation
- 18:15right now, process optimization.
- 18:18How can we reduce administrative
- 18:20cost?
- 18:20Right? We've gotta think about things like
- 18:23automated claims processing,
- 18:25analytics. This is a place where I think
- 18:27we have a chance to think about AI
- 18:29and how we can utilize
- 18:31that in the right way to support
- 18:34some efficiency in our especially in our administrative
- 18:36processes.
- 18:38And then I think innovation is something that's
- 18:40really exciting.
- 18:41I talked about this is a chance to
- 18:42do things differently.
- 18:44How can we think about
- 18:46things like virtual care,
- 18:48remote monitoring,
- 18:50new care management practices?
- 18:53How can we think
- 18:55about improving outcomes differently than we have in
- 18:58the past. So I think the more that
- 18:59we can think about cost containment over time
- 19:02by improving health outcomes,
- 19:04that's gonna help us from an innovation perspective,
- 19:08but also from a member perspective.
- 19:10And then the last piece I would ask
- 19:11is just that value based partnership piece. You
- 19:14know, the more we can incentivize our providers
- 19:17with quality,
- 19:18partnership,
- 19:19efficiency,
- 19:21that's gonna help all of us as we
- 19:23we work through these cost pressures.
- 19:25But it definitely is gonna be something that
- 19:28we see and continue to have as payers
- 19:32going into 2026.
- 19:34I don't see that something that is going
