- 0:00Welcome to the Becker's pharmacy leadership podcast. I'm
- 0:03Ella Jeffries, pharmacy reporter with Becker's Healthcare. Today,
- 0:06I'm joined by Nilesh Desai, chief pharmacy officer
- 0:09at Baptist Health System. In this episode, we'll
- 0:11be discussing the headwinds pharmacy leaders are navigating
- 0:14today along with the opportunities emerging for pharmacy
- 0:17teams as health care to continues to evolve.
- 0:19Nilesh, thank you so much for joining me
- 0:21today. To start us off, can you share
- 0:23a bit about your role as chief pharmacy
- 0:25officer and what's top of mind for you
- 0:26as you lead pharmacy across your system today?
- 0:29Thank you, Ella, for having me.
- 0:32So, Nilesh Shasai. I'm the chief pharmacy officer
- 0:34at Baptist Health. I've been here almost six
- 0:37years now,
- 0:38and,
- 0:40I oversee all of pharmacy services across our
- 0:43health system. We are,
- 0:45eight hospital system.
- 0:47We have 12 retail pharmacies. We have a
- 0:49big central pharmacy that we just went live
- 0:51last year. And,
- 0:53we're in the middle of,
- 0:55completing our implementation
- 0:57across our system.
- 0:59And,
- 1:01you know, as as,
- 1:03we have done a lot of work, in
- 1:05the pharmacy space, we really brought pharmacy
- 1:09to the forefront at Baptist with all our
- 1:11initiatives,
- 1:13and, you know, as recent as opening the
- 1:15100,000 square feet central pharmacy.
- 1:18But there are obviously a lot more headwinds,
- 1:20You
- 1:21know, three forty
- 1:23space with all the changes, the Inflation
- 1:25Reduction Act,
- 1:27all the initiatives that are kicking off, oneone
- 1:29hundred and twenty six with the 10 drugs
- 1:31or nine drugs now. And then,
- 1:34ongoing
- 1:34changes with 340B
- 1:36Medicaid,
- 1:37you know, reduction in Medicaid patients that is
- 1:39resulting in some
- 1:41issues on the dish percentage.
- 1:44And then really,
- 1:45medication access continues to be the biggest challenge
- 1:49as as a lot of these,
- 1:51pharmacies,
- 1:53are closing and or gonna close depending on
- 1:56how,
- 1:58the financial viability turns out.
- 2:01You know, end of the day, we still
- 2:02have to figure out a way how to
- 2:04provide patients with access to medications.
- 2:08You know, a lot of our retail pharmacies
- 2:10are not open, twenty four seven, except, you
- 2:13know, we may have some weekend coverage here
- 2:15and there at couple other sites.
- 2:17So we're looking at other options where, you
- 2:20know, we can deploy kiosks or some type
- 2:22of technology
- 2:24where you can you can have at least
- 2:26the top medications that folks when they come
- 2:28to the ED and they need some medications,
- 2:30we can have those medications be available for
- 2:33them.
- 2:34So those are the kind of things that
- 2:35we're looking at right now.
- 2:37Again, sustaining,
- 2:38the growth,
- 2:39sustaining,
- 2:41and maintaining or increasing growth.
- 2:43But it it's just that,
- 2:45I wanna make sure that,
- 2:48you know, pharmacy continues to contribute to the
- 2:52health of our,
- 2:54our patients and then really to the bottom
- 2:56line to our, from a health system perspective.
- 2:59Great. Thank you for sharing. That was really
- 3:01helpful context.
- 3:03You mentioned, you know, three four two b
- 3:04and those pressures. What is your health system
- 3:07doing or what are you doing with your
- 3:08team to prepare for those?
- 3:10Yeah. A lot of, preparation
- 3:12in order to get our,
- 3:14data sharing and everything that we need to
- 3:17do,
- 3:18and
- 3:19in in leveraging technology again where we can.
- 3:23You know, one thing we don't wanna do
- 3:25is keep adding FTEs,
- 3:27because as you all know,
- 3:29there's a shortage of staffing as it is.
- 3:31You know, we've been dealing with pharmacy technician
- 3:33shortage for the last fifteen years, and that
- 3:36hasn't improved significantly. So that continues to be
- 3:39a challenge. We're also dangerously entering pharmacist shortages.
- 3:43So, you know, we wanna make sure that
- 3:45judiciously
- 3:47use our staff.
- 3:48So the goal is, if we can leverage
- 3:50technology
- 3:52to to,
- 3:53you know, continue to do what we need
- 3:54to do,
- 3:56because these these requirements that that are out
- 3:58there now,
- 4:00you know, hope
- 4:01I I know a lot of vendor,
- 4:03community that's out there. They're trying to work
- 4:05on a solution here,
- 4:07to to help us out. So we're exploring
- 4:09the right partnership,
- 4:11so that
- 4:12we can do things, you know, the right
- 4:14way.
- 4:15Mhmm. For sure. Thank you for sharing that.
- 4:18But, you know, given all that, it does
- 4:19sound like there's a lot competing for attention.
- 4:20How are you prioritizing where pharmacy focuses its
- 4:20time, resources, and leadership energy?
- 4:26You
- 4:27know, as a team,
- 4:30we now have,
- 4:33a lot of projects,
- 4:34what we have prioritized.
- 4:36And then really,
- 4:38as we continue,
- 4:39to evaluate these projects
- 4:41periodically to say, you know, what is important.
- 4:44Obviously,
- 4:45anything that is, patient safety
- 4:48or
- 4:49or or regulatory,
- 4:50I mean, those always are up there. But
- 4:52even, you know, financial,
- 4:54projects that that affects the bottom line. So
- 4:57anything that we can be efficient, anything that
- 4:59we can grow our services,
- 5:01these three kinda
- 5:03always,
- 5:04rise to the top, but not forgetting everything
- 5:06else that we need to do,
- 5:08you know, from, from an employee engagement perspective.
- 5:11And, you know, if you do those things
- 5:14first, financials always, financials always follow. But you
- 5:18also have to make sure you're prioritizing or
- 5:20reprioritizing
- 5:21or adding new projects timely. So this way,
- 5:24you're not stuck in a in a document,
- 5:27or a project list all the time.
- 5:30For sure. And as you look into 2026,
- 5:32are there any new projects or new things
- 5:34that your team is prioritizing that you're really
- 5:36looking forward to?
- 5:38I mean, there are, many projects that we're
- 5:40looking at,
- 5:41our pump, IV pump in our operability with
- 5:44our EMR.
- 5:45We're thinking about, many other improvements to our
- 5:49financial assistance with our patients. So we're looking
- 5:52at applications. We're looking at,
- 5:55many other other initiatives from an inventory management
- 5:58perspective to streamline those things so that we're
- 6:00not,
- 6:02spending a lot of time reconciling,
- 6:05invoices and payments and all that stuff,
- 6:08that that that really takes hours and hours,
- 6:11and and a lot of resources on the
- 6:13back end.
- 6:14So,
- 6:15you know and then,
- 6:17you know, like I said, the big thing
- 6:19is the three forty b changes.
- 6:21The all of that is is is a
- 6:23big big to do because
- 6:25and we know it's not gonna stop here.
- 6:26I know there's another 15 drugs coming up
- 6:28in
- 6:29on 01/01/1927
- 6:30and then continued,
- 6:32every year, but more drugs coming on board.
- 6:34So we need to have systems in place.
- 6:36We're looking at,
- 6:38you know, how how other systems can come
- 6:40into play, more integration, more interoperability
- 6:43so that we're not,
- 6:46chasing. And then the other piece is day
- 6:48data
- 6:49is is the key where we're normalizing a
- 6:51lot of our data, building a lot of
- 6:53dashboards out, and getting prepared for the AI,
- 6:57in, you know,
- 6:58I call it the AI invasion, but it's
- 7:00more how do how do we leverage AI?
- 7:02How do we u utilize AI
- 7:05so that we can become continually become more
- 7:07efficient?
- 7:09Mhmm.
- 7:09For sure. Thank you for sharing. And now
- 7:12stepping back a bit to a more broader
- 7:14picture, where do you see the biggest opportunities
- 7:16for pharmacy to lead, innovate, or expand its
- 7:19impact within health systems?
- 7:21I mean, pharmacy definitely now you know, if
- 7:24you compare ten, fifteen years ago to what
- 7:26it is today, it's it's literally apples and
- 7:28oranges.
- 7:29Pharmacy now is at the forefront,
- 7:32you know, whether you talk clinically, whether you
- 7:34talk quality, whether you talk safety.
- 7:36And, you know, you know, now,
- 7:40revenue generation and,
- 7:42all the medication access initiatives that we're working
- 7:45on value based care,
- 7:46you know, now with addition of CAR T
- 7:49cell therapy and or cell and gene therapy,
- 7:52pharmacy is now
- 7:54really at the forefront,
- 7:55leading a lot of those efforts to make
- 7:58sure that the health systems
- 8:00are now up there,
- 8:02trying to provide those, you know, streams of,
- 8:05care, I call it.
- 8:07And and then really you wanna make sure
- 8:09you're viable financially. So
- 8:11negotiating with pharma, negotiating with payers, negotiating with,
- 8:15you know, within our providers,
- 8:17you know, hiring the right provider network, creating
- 8:19the specialty,
- 8:21and assisting in
- 8:24those spectrums with our health system so that
- 8:26we continue to become that center of excellence
- 8:29in many facets
- 8:34opportunities to grow and, you know, again, improve
- 8:36our health systems
- 8:37and and access to our patients.
- 8:40Mhmm. And where would you say pharmacy's influence
- 8:43is growing the fastest right now?
- 8:45I think it's, the more if if you
- 8:47look at
- 8:48specialty pharmacy continues to grow, community pharmacies,
- 8:51with Meds2Beds and all the other initiatives that
- 8:54are out there. Obviously,
- 8:56from an inpatient perspective, everything that we're doing,
- 8:59I think that continues to evolve.
- 9:02But a lot of clinical care antimicrobial stewardship
- 9:04is another big area. You know, in the
- 9:06in the initial years antimicrobial stewardship was more
- 9:09inpatient driven, but now we're seeing a lot
- 9:12more value on the outpatient side with,
- 9:14many,
- 9:16OPAD and COPAD
- 9:17and and all these additional stuff that goes
- 9:20on. Obviously, that requires resources, but that's a
- 9:22big quality impact on our patients.
- 9:24And and and, you know, making sure we're
- 9:27taking the right antibiotics.
- 9:29So these things are really a big area.
- 9:31And I think the other big area of
- 9:33focus is home infusion.
- 9:36This is a big, big opportunity for health
- 9:37systems to look at,
- 9:39where home infusion can can play a big
- 9:42role in providing out closing their rec you
- 9:45know, their last step of the care. So
- 9:47that that's another big area where we're we're
- 9:50focusing on as well. Mhmm. Thank you for
- 9:52sharing. And for pharmacy to fully capitalize on
- 9:55those opportunities you just discussed, what does pharmacy
- 9:58need to demonstrate today to be seen as
- 10:00a growth driver, not just a cost center
- 10:02within health systems?
- 10:04Yeah. I mean, you know, we have already
- 10:06demonstrated over the last few years of what
- 10:08pharmacy can bring to the table.
- 10:11Not only,
- 10:12revenue and all that, but also services,
- 10:15our patient satisfaction,
- 10:17you know, our retention rate with our patients,
- 10:20our provider satisfaction, you know, how we're reducing
- 10:23the administrative burden for our providers so that
- 10:26they can focus on seeing more patients.
- 10:29So a lot of that, really, and then,
- 10:31you know,
- 10:32even growing new business,
- 10:34plans,
- 10:35writing those business plans that are more effective
- 10:38and and and seeing that, you know, we're
- 10:40not only cost efficient, but also growing
- 10:43services in areas that we have not ventured
- 10:45into.
- 10:46I think a lot of that focus for
- 10:48pharmacy is gonna be in those areas.
- 10:50Mhmm.
- 10:51And how would you say that conversation has
- 10:53changed over time?
- 10:55Definitely.
- 10:56You know,
- 10:57if if I had to think, you know,
- 11:00again, many years ago,
- 11:02bringing pharmacy initiatives
- 11:03when we were
- 11:05specifically a call center
- 11:07to what it is today is night and
- 11:09day difference. How things have evolved, discussions are
- 11:11involved,
- 11:13and and and on how,
- 11:15we approach,
- 11:16subject. I think chief pharmacy officers now have
- 11:19a larger role to play on how to
- 11:21grow services,
- 11:23how to build that infrastructure, how to build,
- 11:26you know, all the things that that are
- 11:28necessary from a medication management perspective.
- 11:31I mean, these things have really changed the
- 11:33way of how we operate,
- 11:35and look at pharmacy,
- 11:37and and and and also,
- 11:40to show,
- 11:41the leadership
- 11:43that what pharmacy can bring to the table,
- 11:45you know, being at that
- 11:48seat,
- 11:48trying to gain trust,
- 11:51and and show
- 11:52demonstrate,
- 11:53and demonstrate the results, that come out of
- 11:56it.
- 11:57That really shows, you know, where pharmacy can
- 12:00make inroads and really deliver on the results.
- 12:02So this,
- 12:03overall, you know, I call it the three
- 12:05sixty
- 12:06or the quadruple aim.
- 12:08You know, we've delivered on every front, whether
- 12:11it's patient satisfaction, provider satisfaction, financial,
- 12:14bottom line, etcetera.
- 12:16This this this has shown where,
- 12:19you know, where pharmacy can make an impact,
- 12:21and this continues to evolve as as we
- 12:24continue to grow our services.
- 12:26Definitely.
- 12:27And as you look ahead, what's one change
- 12:29or shift do you think would make the
- 12:30biggest difference for pharmacy over the next few
- 12:33years?
- 12:34I think I think, pharmacy getting involved in
- 12:36many of the other service areas.
- 12:38I I think, you know, by showing what
- 12:41we can do,
- 12:42you know, improving our systems, aligning,
- 12:45and, you know, not and not getting stuck
- 12:46in the stereotyping of systems or, you know,
- 12:50going outside of the industry to see what
- 12:52is out there. And I think, taking a
- 12:54big leap forward on that those fronts, I
- 12:57think it's gonna help. And then with AI
- 12:59coming in, I think that's gonna be a
- 13:01great, you know, initiative, I would say.
- 13:05We have to be, careful,
- 13:07you know, while we wanna leverage AI to
- 13:09really improve our
- 13:11operations, become more efficient, all of that. But
- 13:14we also wanna do it right,
- 13:17that,
- 13:17that that information
- 13:21kinda, you know,
- 13:23is is more safer
- 13:24and it provides more, you know, appropriate timing
- 13:27for our staff.
- 13:29You know, this is an opportunity where you
- 13:31can reallocate staff to do other things.
- 13:34And as I spoke about staffing shortages,
- 13:37and drug shortages and pharmacy deserts,
- 13:40these are gonna continue to be a challenge.
- 13:42So for leaders,
- 13:44it's how you assess, how you make an
- 13:46impact,
- 13:47leveraging technology, leveraging AI, All these things are
- 13:50gonna be a big, big, challenge for us.
- 13:53But it's a way of, mitigating,
- 13:56these these, challenges as well.
- 13:59Mhmm. And in terms of AI, what are
- 14:02some air areas of AI governance that Baptist
- 14:04Health is looking into for 2026?
- 14:09Definitely,
- 14:11we we at Baptist, what we've done is
- 14:13we have, proactively created our AI
- 14:16infrastructure. We have a com we have committees.
- 14:18We have pharmacy is a big part of
- 14:20it. And and really what we have done
- 14:22is, you know, anytime we wanna bring an
- 14:25application that provides some type of AI functionality,
- 14:28then we really go through the process of
- 14:30vetting them out to see what what that
- 14:32what that advantage
- 14:33of bringing an AI
- 14:35application in it and then really look at
- 14:37what's the ROI. You know, what what are
- 14:39we trying to achieve,
- 14:41by bringing this AI application in? And then
- 14:43really, if we can start measuring outcomes, that's
- 14:46even a better benefit. So that's another focus
- 14:49point on, you know, what are the data
- 14:51metrics that we can collect and then really
- 14:53show what the value of it is. Because
- 14:55I think, you used to wanna bring an
- 14:57application in without, some type of ROI in
- 15:00place.
- 15:01So, whether it's improving quality of care, whether
- 15:04it's improving our employee satisfaction, or or even,
- 15:07you know, end of the day, some financial
- 15:09gains. I think
- 15:11there needs to be some outcomes,
- 15:13that we need to measure so we can
- 15:15drive and bring in more AI applications
- 15:17in all the different facets of what we
- 15:19practice pharmacy in.
- 15:21Definitely.
- 15:22Well, Milesh, this has been such an insightful
- 15:24conversation. Thank you for sharing your perspective and
- 15:26experience with us. That's all for today's episode
- 15:29of the Becker's pharmacy leadership podcast. A big
- 15:31thank you again to Nilesh for sharing his
- 15:33insights on the headwinds pharmacy leaders are currently
- 15:35navigating. Thanks for listening, and we'll see you
- 15:38next time. Thank you.
