- 0:00Hi, everyone. This is Brian Zimmerman with Becker's
- 0:02Healthcare. Thank you so much for tuning into
- 0:04the Becker's Healthcare podcast.
- 0:06Today, we're going to talk about revenue readiness
- 0:09and how RCM leaders can win with the
- 0:11EHR transition.
- 0:13Joining me for today's discussion is Jody Hillacre,
- 0:16senior director of EHR services at HealthRise. Jody,
- 0:19welcome back to the podcast, and and thanks
- 0:21for joining.
- 0:22Pleasure to be here, Brian. Good to talk
- 0:24to you again. Yep. Great great to have
- 0:26you back on, and we'll begin here sort
- 0:28of the usual way for folks that maybe
- 0:30didn't tune in to the last podcast or
- 0:32might not be as familiar with with you.
- 0:34Jody, can you introduce yourself and share a
- 0:36bit about your background and the work you're
- 0:38doing now? Sure. So as you stated, my
- 0:40name is Jody Hilliker, and I'm the senior
- 0:42director of EHR services at HealthRise.
- 0:44Been with the company for about four years.
- 0:47Great organization. Happy to be there.
- 0:50I have a,
- 0:52very
- 0:53strong clinical background, but,
- 0:56as I've worked with HealthRise for the last
- 0:58four years, I've learned a lot about how
- 1:00revenue cycle and clinical
- 1:04can be
- 1:05can work together to create a great outcome
- 1:08for any organization. So the last four years,
- 1:11I've been kind of working on that and
- 1:13and having a great time with it. So,
- 1:15here we are today.
- 1:17Excellent. Well, I'm excited to to pull from
- 1:19your experience here again and and get into
- 1:21it. I I the last time we spoke,
- 1:23I believe we explored the idea of the
- 1:25revenue ripple,
- 1:26starting with provider optimization for for RCM leaders
- 1:29preparing for an EHR go live.
- 1:32Can you share where that ripple truly does
- 1:35begin, and what are the most common, I
- 1:36I suppose,
- 1:37I guess, revenue blind spots organizations miss during
- 1:40that transition?
- 1:42What would you call out there?
- 1:44Sure. So the the the ripple really starts
- 1:47with provider experience. It starts the moment a
- 1:49clinician opens that chart, starts to document care,
- 1:53place orders,
- 1:54and tries to move efficiently throughout their day.
- 1:57If that workflow is clunky
- 1:59and documentation
- 2:01requirements aren't clear,
- 2:03revenue issues begin to start
- 2:05immediately, not weeks down the line like a
- 2:08lot of organizations
- 2:09think, well, we'll see this down the road.
- 2:12It's really starting immediately.
- 2:14So then you see delays in your coding,
- 2:17your missed charges, your denials, and slower cash
- 2:20flow,
- 2:21which isn't good for any organization.
- 2:23So one of the biggest blind spots is
- 2:25the organization
- 2:27thinking that, oh, this is mainly a technology
- 2:30event, and it's not really,
- 2:33you know, it's not it doesn't really involve
- 2:35me, and and that's not true. It's really
- 2:37an operational event that technology
- 2:40is supporting.
- 2:42So,
- 2:43you know, those are things that you have
- 2:44to think about. Another
- 2:46common miss that I've seen, Brian, very frequently
- 2:49is that
- 2:50a lot of organizations feel that if they've
- 2:52trained their end users, that that means that
- 2:54they're ready, and that's not the case.
- 2:57Readiness means that people can perform their job
- 2:59from day one. So it's really great to
- 3:01have,
- 3:03sessions prior to
- 3:05the actual go live where you put your
- 3:07end users in
- 3:09a situation where they can actually work through
- 3:12their workflow during the day, get a feel
- 3:14for what it's gonna be like, and they're
- 3:16they're really far more prepared.
- 3:18So I recommend operational readiness as being something
- 3:21that is high on the list when you're
- 3:23gonna do a boat live.
- 3:25Yeah. Such an important call out there that
- 3:27is more than a technology event and then
- 3:29really getting folks, I guess, early reps. Is
- 3:32is is that a fair way to think
- 3:33about it in terms of getting comfortable with
- 3:35with the operational workflow, Jody? Right. Absolutely. So
- 3:38what we do when what I recommend is
- 3:40that with operational readiness is that you run
- 3:43a lot of labs,
- 3:44what we call, you know, user labs where
- 3:47you bring your end users in and you
- 3:49let them work through the workflow in a
- 3:51test environment.
- 3:52Let them see what it's gonna be like
- 3:53from day to day. Let them ask their
- 3:56questions there rather than being panicked on that
- 3:58first day and wondering, oh my gosh. What
- 4:00am I gonna do tomorrow when we have
- 4:01this first day of go live? How am
- 4:03I going to learn to work with this?
- 4:05The more that we prep people prior to
- 4:08by having them a day in the life
- 4:11of is the most important thing,
- 4:13they they feel more comfortable. So day one
- 4:16isn't such a shock to them.
- 4:18Yeah. And and the preparedness goes a long
- 4:20way. That that can't be pro probably overstated.
- 4:24But at the same time, right, there's still
- 4:26variables and things are will change. So you've
- 4:29gotta watch this, I imagine, as you you
- 4:31begin to roll this out. I guess,
- 4:34Jody, what would be some indicators RCM team
- 4:36should monitor to to
- 4:38potentially identify
- 4:40workflow gaps or or documentation breakdowns before they
- 4:43start to to impact that cash flow, which
- 4:45as you pointed out, this can happen right
- 4:47away. So Right. What what what should folks
- 4:49be on the lookout for once they do
- 4:51go live?
- 4:52So there are a lot of early signals,
- 4:54Brian, that they can be looking for. They
- 4:56don't wait for the end of the month
- 4:58financial report to tell them that, oh my
- 4:59gosh, we're in trouble. So you can look
- 5:01for charge lag, unsigned charts,
- 5:04on on
- 5:05encounters that aren't closed,
- 5:07claim edits, coding, backlogs,
- 5:10encounter and encounter close times that are very,
- 5:14very lengthy,
- 5:15any kind of denial trends, and then provider
- 5:17pro productivity.
- 5:19And you should watch for behaviors.
- 5:21If you are,
- 5:22a manager or you're in the hierarchy position
- 5:25and you're walking through your clinic and you're
- 5:27seeing increased after hour work, frustrations,
- 5:31workarounds being developed, or teams just saying, you
- 5:34know, I'm I I don't like this system.
- 5:36This is taking twice as long. And it
- 5:38doesn't matter what what system they're working on,
- 5:40whether they're
- 5:41in Epic,
- 5:43you know, Meditech, Cerner.
- 5:45It does not really matter. It's if you're
- 5:47hearing those words that, you know, we don't
- 5:50like this system because it's taking too long,
- 5:52then those are signs that you're gonna have
- 5:54a a downstream problem with your with your
- 5:56revenue as you keep going. So the faster
- 5:59that you catch those workflow
- 6:01frictions,
- 6:02the faster you're gonna protect that cash flow.
- 6:05It's such a great call out to really
- 6:06be have your eyes open, right, when you're
- 6:08when you're walking through and and and figure
- 6:10things out and
- 6:12spot those issues.
- 6:13And can you sort of as you describe
- 6:16sort of some some of the proactive
- 6:18training, the the preparedness work folks can be
- 6:20doing here to get ready, can you expand
- 6:22more on what sort of this the the
- 6:25ownership of this looks like in terms of
- 6:27who's accountable for what and what kind of
- 6:30cross functional alignment,
- 6:32should should take place here to ensure that
- 6:34that performance that performance dip doesn't happen. Because
- 6:37I imagine that you've gotta get different different
- 6:39people
- 6:41to work together from different departments and also
- 6:43take some accountability and ownership here. Can you
- 6:45share a little bit more of what you
- 6:46think that should look like?
- 6:49Absolutely. So strong ownership means that everybody owns
- 6:52a piece of the go live success. It's
- 6:53not just an IT situation. Just as I
- 6:56said, it's not just an IT project. It's
- 6:58really an operations project that's supported by IT.
- 7:02So operation owns your workflows, making sure that
- 7:04everything flows correctly.
- 7:06Clinical leaders own adoption. They're responsible for making
- 7:10sure that their teams adopt to the EHR
- 7:12and that they get familiar with it and
- 7:13they feel comfortable with it. Revenue cycle,
- 7:17they own their downstream integrity,
- 7:19and then executive leadership pays a big plays
- 7:22a big part in making sure that they
- 7:25own the speed of decision
- 7:27making and accountability.
- 7:29If we are bringing
- 7:31as as end users
- 7:33or as managers are bringing to our leadership
- 7:36that we need
- 7:37to change something and they don't react,
- 7:40then you're gonna have an issue.
- 7:42And this should always happen before go live.
- 7:45So the best organizations
- 7:47are aligned before the go live on what
- 7:49success looks like. What are our productivity targets?
- 7:52What are our charge leg thresholds?
- 7:55What are the our escalation path?
- 7:58How do we do daily reporting? And who
- 8:00fixes what when something happens? If you have
- 8:02a clear path on that,
- 8:05you're going to win. If your ownership is
- 8:07vague and performance drops,
- 8:11there's gonna be an issue. If ownership is
- 8:13clear, the organization will recover quickly from any
- 8:16kind of problem that pops up.
- 8:18Yeah. And and the ultimate goal here too,
- 8:20I mean, the the go live is important.
- 8:23The preparedness
- 8:24thing is important as well. But you also
- 8:26wanna set up teams for for long term
- 8:28financial success here. And you've shared examples previously
- 8:32where,
- 8:33optimization can really lead to measurable revenue improvement
- 8:37without adding provider hours because that's another thing,
- 8:40of course, folks are are pursuing is trying
- 8:42to,
- 8:43achieve this and and get revenue improvement without,
- 8:46you know, adding to the plates of of
- 8:48of providers.
- 8:50For executives listening today, how should they think
- 8:52about investing in provider support and workflow optimization
- 8:56as they're looking at that long term financial
- 8:58sustainability
- 8:59and success?
- 9:00So if I were a leader in an
- 9:02organization that's about to go live, I would
- 9:05look at provider support as a margin protection.
- 9:08When providers can move very efficiently throughout their
- 9:11day, document cleanly, close encounters on time,
- 9:15the organization
- 9:16benefits
- 9:17everywhere. Revenue capture,
- 9:19throughput, patient access,
- 9:21retention of their patients, retention of the providers.
- 9:25It's a margin protection.
- 9:27You don't need massive investments
- 9:30when you're, you know, when you're looking at
- 9:31provider optimization.
- 9:33It's just to make sure that you target
- 9:36the workflow, redesign if you need one,
- 9:39specialty optimization for those specialty providers, making sure
- 9:42that they have the templates and the tools
- 9:44that they need,
- 9:45reduced inbox burden, show them how to use
- 9:48tools so they can reduce the inbox burden
- 9:51that they have. Hands on support, like I
- 9:54I think I mentioned in the last podcast
- 9:56is finding a team of people that will
- 9:58sit down with your providers and make sure
- 9:59that they have everything that they need to
- 10:02use the EHR efficiently and effectively,
- 10:04and then spending the time on optimization and
- 10:07training workflows,
- 10:08making sure that if you change something that
- 10:11you actually train the providers on the change
- 10:13that you made and talk to them about
- 10:15the changes that you want to make and
- 10:17make sure that it is something that's going
- 10:19to improve their day to day processes.
- 10:22Jody, thank you so much. And it really
- 10:24is a pleasure to have you come on
- 10:25and share so many,
- 10:27tangible,
- 10:28actionable piece of advice with with our listeners.
- 10:31We'll close here by just
- 10:33asking you, you know, for for RCM leaders
- 10:36out there that are thinking about their next,
- 10:38EHR milestone or or optimization phase.
- 10:41What final piece of advice do you wanna
- 10:43leave them with before we sign off today?
- 10:45I would say don't confuse complexity with more
- 10:49con control.
- 10:50A lot of organizations
- 10:52will add multiple layers, reports,
- 10:55approvals, rules when performance dips,
- 10:58you know, trying to make it more complex
- 11:00when it it really could be just a
- 11:02simple solution.
- 11:03The better answer is simplifying the work,
- 11:06clarifying accountability,
- 11:07and fixing the root causes. Get right to
- 11:10the root cause. Get it fixed.
- 11:12Stay focused on what truly moves the performance.
- 11:15Don't lose touch with what frontline teams are
- 11:18experiencing every day. Make sure that you get
- 11:21down into the nitty gritty, and you talk
- 11:23to your teams to see what exactly
- 11:26is is happening. Why are you experiencing
- 11:29roadblocks? What are you feeling?
- 11:32My my strongest advice is don't treat ready
- 11:35operational readiness as a line item. Treat it
- 11:38as a necessity.
- 11:39It is something that you have to do.
- 11:42Start to engage
- 11:43operational readiness at the very beginning of your
- 11:46organization's
- 11:47launch into the EHR process.
- 11:50Productivity loss, denial volume, provider frustration, delayed cash,
- 11:55and rework
- 11:56can follow organizations
- 11:58for months after an implementation
- 12:00if you don't jump on them and if
- 12:01you're not prepared ahead of time. So, again,
- 12:04you know, my favorite my favorite speech, Brian,
- 12:07find a partner
- 12:09that's based in experience and execution. Make sure
- 12:12that you've got a team that's not just
- 12:14about price and promises. Make sure that they
- 12:16come in and really get engaged. Get down
- 12:19and down with your teams and sit down
- 12:21with them and find out what their pain
- 12:23points are. Work with them.
- 12:25The right partner should understand both or operations
- 12:29and,
- 12:30revenue cycle. And that, I think, is
- 12:32another problem that a lot of organizations have.
- 12:34They'll pick an org to work with that
- 12:37only has one or the other, and you
- 12:39should have somebody that has the whole you
- 12:41know, that can fill the whole basket.
- 12:43Strong partner doesn't help launch the system,
- 12:47but they protect performance while you do it.
- 12:49So that's that my gist is just to
- 12:52make sure that operational readiness is is not
- 12:55just a line item, but it's something that
- 12:57you dive into.
- 12:58Yep. And and and if you're diving into
- 13:00it as an an executive for for a
- 13:02health system,
- 13:03your expectation should be the partner's gonna dive
- 13:05into it with you. Correct?
- 13:07Absolutely. You should be hand in hand, and
- 13:10you you're looking for a partner that's just
- 13:12not gonna be a yes, man. I I
- 13:14really have a strong feeling about that. You're
- 13:17looking for a partner that's gonna be prescriptive
- 13:19and that's gonna say, from our experience,
- 13:22this is what we've seen in the past
- 13:24and this is what we recommend. Whether the
- 13:26organization decides to go with that or not
- 13:28is totally up to them. But pick a
- 13:30partner that's gonna prescribe for you what they've
- 13:33seen and what they recommend that you do,
- 13:35and and listen to their advice because sometimes
- 13:38when you've gone through 400 of these like
- 13:40our organization has,
- 13:42you have a little bit of experience there,
- 13:44and you know what the pitfalls are. So
- 13:45you should really take advantage of that and
- 13:47and utilize them as a partner, not just
- 13:50as an employee.
- 13:52Yeah. Jody, thank you so much once again
- 13:54for for taking the time to come on
- 13:56the podcast.
- 13:57Always a pleasure to speak with you. You
- 13:58too, Brian. Thank you so much for your
- 14:00time. We also wanna thank our podcast sponsor,
- 14:03HealthRise.
