- 0:00- This is Scott Becker with the Beckers HealthCare
- 0:03- podcast.
- 0:04- Today's discussion is we need more doctors.
- 0:07- So so here's the context. We're huge fans
- 0:10- of our physician community.
- 0:12- We believe... I believe that we badly need
- 0:15- more specialists and more doctors.
- 0:17- We have horrendous shortages in many areas.
- 0:20- This is absolutely horrible when you or a
- 0:22- family member has a complex issue.
- 0:25- It's absolutely horrendous in behavioral health too where
- 0:27- we're so short psychiatrists
- 0:29- and and
- 0:30- and psychologists.
- 0:32- I'm increasingly annoyed in our physician societies in
- 0:35- two core places, and and I get pushback
- 0:37- from physicians this.
- 0:39- You know, first, I saw several physician societies
- 0:43- just issue a press release essentially saying that
- 0:46- we need to slow down the use of
- 0:48- AI in medicine.
- 0:49- We're at a spot where quite frankly... I
- 0:52- I don't know if I agree or disagree,
- 0:54- but our physician community is so overwhelmed, and
- 0:57- we are so short providers
- 0:59- that I viewed the least as incredibly territorial
- 1:02- and self serving.
- 1:03- Not helpful.
- 1:04- I get the point, but we need more
- 1:07- doctors, and we need technologies too. And slowing
- 1:10- down this evolution is a bad, bad idea.
- 1:13- Second...
- 1:14- And here's where I think I'm probably more
- 1:15- upset with the physician societies
- 1:17- and and and some physicians.
- 1:20- The physician societies seem so against shortening the
- 1:23- pathway to become a physician
- 1:25- that by necessity, in some places for good
- 1:27- and some places for bad, we're going to
- 1:29- end up by default with a huge amount
- 1:32- of work here provided by nurse practitioners,
- 1:35- PAs, and CRNAs.
- 1:37- What what... What's happened now is the medical
- 1:39- education path is four years of college,
- 1:42- four years of medical school, then four years
- 1:45- of residency, and often a fellowship.
- 1:47- So what's happened, it's harder and harder to
- 1:49- get a great deal of the best and
- 1:51- brightest to go down that path. There's no
- 1:54- question in my mind that that total path
- 1:56- of twelve years
- 1:58- could probably be shortened to
- 2:01- ten
- 2:02- or nine or eight years. In fact, in
- 2:04- other countries, people go right to medical school
- 2:07- out of high school, and they end up
- 2:08- shortening that path by four years. Even in
- 2:11- our country, we used to have the six
- 2:12- year premed programs where people combined college and
- 2:15- medical school into six years, cutting off two
- 2:18- years of the program.
- 2:19- I'm also sure we cut off the fourth
- 2:21- year of medical school without that much harm
- 2:23- and at least six months of residency without
- 2:25- that much harm, but instead we're making it
- 2:26- longer and longer.
- 2:28- What this is leading to... Now I was
- 2:30- in a discussion today with a physician,
- 2:32- so upset that CRNAs are doing more of
- 2:34- the work than anesthesiologist,
- 2:36- But you can't have it both way. You
- 2:38- can't have a fourteen year training path for
- 2:40- anesthesiologists,
- 2:43- have more CRNAs produced each year.
- 2:46- Right now, we've got 62,000 or so CRNAs
- 2:48- in the country, 51,000 or so anesthesiologists.
- 2:51- You you can't have both ways. You can't
- 2:53- make it really hard to become an anesthesiologist
- 2:55- and then be upset when the work moves
- 2:57- someplace else. It's absolutely ass backwards. I think
- 2:59- the professional societies have a lot of blame
- 3:01- in this. I think they've gotta get on
- 3:03- board with trying to make it a little
- 3:05- bit easier to become a physician so we
- 3:07- get the best and brightest constantly into medicine.
- 3:09- Thank you for listening to the Becker's Healthcare
- 3:12- podcast. We hope you enjoy this. Thank you
- 3:14- so much.
