- 0:00Hi you
- 0:00guys, and
- 0:00welcome
- 0:00back for
- 0:01another
- 0:01episode of
- 0:02the Eye
- 0:02Believe
- 0:02Podcast.
- 0:03I'm joined
- 0:04with doctor
- 0:04Carol
- 0:04Shields
- 0:05from Wills
- 0:05Eye
- 0:05Hospital,
- 0:06and today
- 0:06we're going
- 0:07to talk
- 0:07about
- 0:07pediatric
- 0:08ocular
- 0:08melanoma,
- 0:09as well
- 0:09as some of
- 0:10the myths
- 0:10and, myths
- 0:10busting
- 0:11around
- 0:11pregnancy
- 0:12and ocular
- 0:12melanoma.
- 0:12So Doctor
- 0:13Shields,
- 0:13thanks for
- 0:14joining us
- 0:14again. Yes.
- 0:15Thank you
- 0:15for
- 0:15having me.
- 0:16I was going
- 0:16to start
- 0:17with
- 0:17pediatric
- 0:18ocular
- 0:18melanoma
- 0:19if you want
- 0:19me to.
- 0:20Yeah,
- 0:20I think
- 0:20that
- 0:21sounds like
- 0:21an awesome
- 0:21place
- 0:22to start.
- 0:22Let's
- 0:22just cover
- 0:23some of the
- 0:23current
- 0:23research
- 0:24in
- 0:24pediatric
- 0:24ocular
- 0:25melanoma.
- 0:26So when we
- 0:26talk about
- 0:27ocular
- 0:27melanoma,
- 0:29we're
- 0:29talking
- 0:29about
- 0:29conjunctiva
- 0:30melanoma
- 0:31as well
- 0:32as Uveal
- 0:32melanoma.
- 0:33So I'm
- 0:33going to
- 0:33just start
- 0:33with
- 0:34conjunctiva
- 0:34melanoma.
- 0:36It's pretty
- 0:37rare
- 0:38to have
- 0:39conjunctiva
- 0:40melanoma
- 0:41in
- 0:41children.
- 0:41So when you
- 0:42see a child
- 0:43with a
- 0:43pigmented
- 0:44spot on
- 0:44the eye
- 0:45it's going
- 0:46to be
- 0:47nevus nevus
- 0:48nevus nevus
- 0:49and nevus.
- 0:50It's very
- 0:51rarely
- 0:52going to
- 0:52be
- 0:53melanoma.
- 0:54So don't
- 0:54worry.
- 0:55And most
- 0:56nevi occur
- 0:57right at
- 0:57the limbus.
- 0:58They tend
- 0:59to have
- 0:59their onset
- 1:00when the
- 1:00child's
- 1:012, 3,
- 1:024 or 5
- 1:03years old.
- 1:04They tend
- 1:04to get
- 1:04a little
- 1:04more
- 1:05pigmented
- 1:05as
- 1:06they head
- 1:06into
- 1:06puberty,
- 1:07and it's
- 1:08a good idea
- 1:08to
- 1:08have them
- 1:09resected
- 1:09just
- 1:10because
- 1:10nevus
- 1:11can
- 1:12transform
- 1:12into
- 1:13melanoma.
- 1:14So if you
- 1:15look at all
- 1:15pediatric
- 1:16conjunctival
- 1:17tumors,
- 1:18only 1%
- 1:19are
- 1:20melanoma.
- 1:20And that's
- 1:21in our
- 1:21practice.
- 1:22So imagine,
- 1:24you know,
- 1:24the real
- 1:25practice,
- 1:25the real
- 1:26world.
- 1:27It's way
- 1:27less
- 1:28than 1%.
- 1:29So
- 1:29I wouldn't
- 1:29worry
- 1:30about it.
- 1:30But it's
- 1:31a good idea
- 1:31to have a
- 1:32pigmented
- 1:32spot on the
- 1:33surface of
- 1:34the eye
- 1:34surgically
- 1:35removed
- 1:35just
- 1:35in case.
- 1:36Now
- 1:37I'll move
- 1:38on to
- 1:38Uveal
- 1:39melanoma.
- 1:40Hey again,
- 1:42it's
- 1:42totally
- 1:42rare to see
- 1:44Uveal
- 1:45melanoma
- 1:46in a kid.
- 1:47If you
- 1:47look at
- 1:47all uveal
- 1:48melanoma,
- 1:51only 1%
- 1:52occurs
- 1:53under the
- 1:53age of 20.
- 1:55And if you
- 1:56look at
- 1:56those
- 1:57children
- 1:58under
- 1:58the age
- 1:58of 20,
- 1:59they're
- 1:59mostly
- 2:00teenagers,
- 2:03and
- 2:06they tend
- 2:06to have
- 2:06approximately
- 2:08the same
- 2:08prognosis
- 2:09as older
- 2:10adults.
- 2:10But I
- 2:10do feel
- 2:12they have a
- 2:12little
- 2:12better
- 2:13prognosis.
- 2:15Why?
- 2:16Because
- 2:16when you do
- 2:17genetic
- 2:18testing
- 2:19of Uveal
- 2:19melanoma
- 2:20in
- 2:20children,
- 2:21you're
- 2:22more likely
- 2:22to get
- 2:23negative
- 2:24genetics.
- 2:25That means
- 2:26they're
- 2:26going to be
- 2:26A or B
- 2:27the Cancer
- 2:28Genome
- 2:28Atlas A
- 2:29or B,
- 2:29which means
- 2:31lower risk
- 2:31for
- 2:32mutation.
- 2:33We found that
- 2:34a long time
- 2:34ago,
- 2:358, 10
- 2:36years ago,
- 2:36we
- 2:36published
- 2:37a report
- 2:38on 1,059
- 2:39eyes that
- 2:40we did
- 2:40genetic
- 2:41sampling,
- 2:41and
- 2:41we found
- 2:42increasing
- 2:43age
- 2:45correlates
- 2:46with worse
- 2:47genetic
- 2:48mutation.
- 2:49So kids
- 2:49have better
- 2:50genetic
- 2:50mutation
- 2:51or less
- 2:52genetic
- 2:52mutation
- 2:53compared to
- 2:53older
- 2:54adults.
- 2:55So
- 2:55when we see
- 2:56melanoma in
- 2:57kids, yes,
- 2:59it happens.
- 3:01Usually
- 3:02they're
- 3:02over three
- 3:02millimeters
- 3:03in
- 3:03thickness.
- 3:04Occasionally
- 3:06they're
- 3:06mistaken
- 3:07as nevus or
- 3:09could this
- 3:09be
- 3:09retinoblastoma
- 3:10believe it
- 3:11or not.
- 3:12Or,
- 3:12could
- 3:13this be
- 3:13Coates
- 3:13disease
- 3:14or Von
- 3:15Hippel-Lindau
- 3:15disease?
- 3:15I mean
- 3:16there's
- 3:16all kinds
- 3:17of
- 3:17diagnoses
- 3:18that get
- 3:18sent to us,
- 3:19but
- 3:20you can
- 3:21make the
- 3:21diagnosis
- 3:22with
- 3:22a good ultrasound.
- 3:23And
- 3:24if you're
- 3:24not sure,
- 3:25maybe a
- 3:25good MRI,
- 3:26you'll see
- 3:27it
- 3:27enhances,
- 3:28and
- 3:28then you
- 3:28treat them
- 3:30usually
- 3:30with
- 3:30radiation
- 3:31if you
- 3:31can't you
- 3:32treat them
- 3:32with
- 3:33eye
- 3:33removal,
- 3:34but
- 3:35we always
- 3:35get genetic
- 3:36testing
- 3:37beforehand
- 3:38because
- 3:38we want
- 3:39to
- 3:39prognosticate
- 3:40for the
- 3:40patients.
- 3:41And, for
- 3:42example,
- 3:42today I saw
- 3:44like a
- 3:4412 year
- 3:45old girl
- 3:45who had
- 3:46a small
- 3:46ciliary
- 3:47body
- 3:47melanoma.
- 3:48We did
- 3:49genetic
- 3:49testing.
- 3:50It was the
- 3:50Cancer
- 3:51Genome
- 3:51Atlas
- 3:51Group A
- 3:52I was able
- 3:53to tell
- 3:53her, hey,
- 3:54that's low
- 3:55risk.
- 3:55You're
- 3:56likely to
- 3:56do fine
- 3:57long term.
- 3:58You're
- 3:58doing well.
- 4:00We'll keep
- 4:00a check
- 4:00on you
- 4:01once or
- 4:01twice
- 4:02a year.
- 4:03So keep
- 4:05in mind
- 4:05that
- 4:05melanoma
- 4:06of the uvea
- 4:07can occur
- 4:08in kids but
- 4:10generally
- 4:11and
- 4:11believe it
- 4:12when you
- 4:12see it
- 4:13and send
- 4:13them.
- 4:14If you're
- 4:14not sure,
- 4:15send them
- 4:16to an
- 4:16ocular
- 4:16oncologist
- 4:17to look
- 4:18and make
- 4:19a diagnosis.
- 4:20You never
- 4:20want to
- 4:21presume
- 4:21anything
- 4:22in a child.
- 4:23You don't
- 4:24want to
- 4:24presume
- 4:24retinoblastoma.
- 4:25You don't
- 4:26want to
- 4:26presume
- 4:26Coates
- 4:27disease.
- 4:27You want to
- 4:28be 100%
- 4:29sure.
- 4:30Is it
- 4:30melanoma or
- 4:31is it not
- 4:31melanoma?
- 4:34So
- 4:34are there
- 4:34any current
- 4:35specifically
- 4:36pediatric
- 4:36treatments
- 4:37that you're
- 4:37aware of
- 4:38that are
- 4:38just unique
- 4:38to the
- 4:39pediatric
- 4:39realm?
- 4:39and if not,
- 4:40then do
- 4:40you have
- 4:41hope that
- 4:42there
- 4:42would be
- 4:42some
- 4:42level of,
- 4:43For
- 4:43example,
- 4:44maybe the
- 4:44Bel-sar
- 4:45drug that
- 4:45you
- 4:45mentioned,
- 4:45something
- 4:46that
- 4:46could help
- 4:46to preserve
- 4:47vision
- 4:47because I'm
- 4:47sure for a
- 4:48younger
- 4:48child,
- 4:49preserving
- 4:49the vision
- 4:50as well
- 4:50as the life
- 4:51is also
- 4:51higher on
- 4:52the list
- 4:52because
- 4:52there's so
- 4:53much
- 4:53younger.
- 4:54Right?
- 4:54You know,
- 4:55today
- 4:55we had a
- 4:56five year
- 4:56old child
- 4:58who had
- 4:58a big
- 4:59ciliary
- 5:00body
- 5:00melanoma,
- 5:01but he had
- 5:01ocular
- 5:02melanocytosis.
- 5:04So that's
- 5:04what
- 5:04promoted
- 5:05his risk
- 5:06for
- 5:06melanoma
- 5:07at such a
- 5:08young age.
- 5:09He required
- 5:10eye
- 5:10enucleation,
- 5:12but
- 5:12one thing
- 5:12about kids,
- 5:14is
- 5:14kids just
- 5:15have a
- 5:15more robust
- 5:17system
- 5:18body.
- 5:19They
- 5:20respond to
- 5:20things
- 5:21more
- 5:21robustly.
- 5:22And
- 5:22the same
- 5:23thing is
- 5:23they
- 5:23respond
- 5:24to
- 5:25radiation
- 5:26more
- 5:26robustly.
- 5:27When you
- 5:27put
- 5:27a plaque on
- 5:28their eye,
- 5:29they're at
- 5:29risk to
- 5:30get worse
- 5:31radiation
- 5:32retinopathy.
- 5:34So for
- 5:34kids,
- 5:35I think
- 5:35it's really
- 5:35important
- 5:36that we
- 5:37give them
- 5:37the
- 5:37prophylactic
- 5:38anti-VEGF
- 5:39Avastin,
- 5:40and that
- 5:41we put
- 5:41in a mini
- 5:42sector,
- 5:42PRP,
- 5:43to reduce
- 5:45their
- 5:45VEGF drive
- 5:46because
- 5:46their VEGF
- 5:47drive
- 5:47is going
- 5:48to be
- 5:48powerful,
- 5:49much more
- 5:50powerful
- 5:50than an
- 5:5180 year old
- 5:52VEGF drive.
- 5:53So
- 5:54we always
- 5:55watch
- 5:55these kids
- 5:56really
- 5:56closely
- 5:57and
- 5:57make sure
- 5:57we don't
- 5:58miss
- 5:59any
- 5:59treatments
- 6:00to reduce
- 6:02the vegf
- 6:02drive.
- 6:04Yes, I
- 6:04do think
- 6:05you
- 6:05mentioned
- 6:06Bel-sar
- 6:06down the
- 6:07road.
- 6:07Bel-sar
- 6:08would be
- 6:08one heck
- 6:09of a good
- 6:09treatment
- 6:10for small
- 6:11melanoma
- 6:12in a child.
- 6:13You know,
- 6:13if
- 6:13you have a
- 6:13small
- 6:14melanoma
- 6:14it has say
- 6:15three risk
- 6:16factors.
- 6:16You know
- 6:17the child
- 6:17is going
- 6:17to have
- 6:18a 34%
- 6:19chance
- 6:19for
- 6:20transformation
- 6:21into
- 6:21melanoma.
- 6:22with
- 6:22growth.
- 6:23yeah.
- 6:23We could treat
- 6:24that child
- 6:24with
- 6:25Bel-sar
- 6:25and
- 6:26hopefully
- 6:27protect
- 6:27the vision
- 6:28and
- 6:28hopefully,
- 6:29you know,
- 6:30cure the
- 6:30cancer
- 6:31and put it
- 6:31to sleep.
- 6:32But,
- 6:33you know,
- 6:33time
- 6:33will tell
- 6:34and we have
- 6:34to get
- 6:35Bel-sar
- 6:35FDA
- 6:36approved
- 6:36right now.
- 6:37We can't treat
- 6:38anyone
- 6:39under
- 6:39the age
- 6:39of 18
- 6:41with
- 6:41Bel-sar
- 6:41And I know
- 6:42that's
- 6:42definitely
- 6:42one of
- 6:42the things
- 6:43in the
- 6:43pediatric
- 6:43ocular
- 6:44melanoma
- 6:44community
- 6:45that,
- 6:45kind of rubs,
- 6:46because
- 6:46those
- 6:46parents
- 6:47are like,
- 6:47well,
- 6:47we want
- 6:47these
- 6:48clinical
- 6:48trials,
- 6:48whether
- 6:49it's for
- 6:49metastatic
- 6:49treatment
- 6:50for
- 6:50adjuvant
- 6:50treatment.
- 6:51We want
- 6:51these
- 6:52trials
- 6:52available
- 6:52for
- 6:52the kids
- 6:53because
- 6:53we want them
- 6:54to be
- 6:54okay, too.
- 6:55So
- 6:55I feel it's
- 6:56so
- 6:56reassuring
- 6:56to hear
- 6:56you talk
- 6:57a little
- 6:57bit more
- 6:57about the
- 6:58genomics
- 6:58of the tumor
- 6:59and how
- 7:00kids have
- 7:00a much
- 7:00better
- 7:01chance
- 7:01of having
- 7:01those
- 7:02better
- 7:02prognostication
- 7:03factors.
- 7:04So that was
- 7:04that was
- 7:05really helpful.
- 7:05is there
- 7:06anything else
- 7:06you want
- 7:06to say
- 7:06to
- 7:07pediatric?
- 7:07Yeah.
- 7:08So I mean,
- 7:09I can
- 7:10say a few
- 7:11words on,
- 7:12you know,
- 7:12retinoblastoma
- 7:13and how
- 7:14we have
- 7:15completely
- 7:17changed
- 7:17retinoblastoma
- 7:18in the
- 7:18past,
- 7:21since
- 7:211994,
- 7:23using
- 7:23chemical
- 7:24therapy
- 7:24and I think
- 7:25we're on
- 7:26the cusp
- 7:27with uveal
- 7:28melanoma.
- 7:28I really think
- 7:30in the next
- 7:30few years
- 7:31things are
- 7:31going to
- 7:32shift,
- 7:33but
- 7:33right now,
- 7:34with
- 7:35retinoblastoma,
- 7:37we can cure
- 7:38the child.
- 7:39We can save
- 7:40the child's
- 7:41life.
- 7:41I mean,
- 7:41when I
- 7:42started,
- 7:43there was
- 7:43a 20%
- 7:45risk for
- 7:46metastatic
- 7:46disease
- 7:47with
- 7:47retinoblastoma.
- 7:48Now it's
- 7:501% in
- 7:51our hands.
- 7:52It's less
- 7:52than 1%.
- 7:54Kids just
- 7:54don't die
- 7:55from
- 7:55this cancer,
- 7:56and we
- 7:57prevent
- 7:58the disease
- 7:59from
- 8:00transmitting
- 8:00to the
- 8:01next
- 8:01generation,
- 8:02isn’t
- 8:03that cool?
- 8:04That's
- 8:05so novel.
- 8:05That's
- 8:05so cool.
- 8:06Because
- 8:06retinoblastoma
- 8:08is, in
- 8:09one third
- 8:09of cases,
- 8:10a heritable
- 8:11disease.
- 8:12So now
- 8:12what we do
- 8:13is we do
- 8:14in vitro
- 8:15fertilization,
- 8:17and we only
- 8:17fertilize
- 8:18the female
- 8:19with
- 8:20fertilized
- 8:21eggs
- 8:22that do not
- 8:23carry
- 8:23the mutation.
- 8:24So the
- 8:25transmission
- 8:26stops
- 8:27with the
- 8:28next
- 8:28generation.
- 8:29That's
- 8:29incredible.
- 8:30I mean.
- 8:31I remember
- 8:31taking
- 8:32family
- 8:32trees,
- 8:33back
- 8:33in the 1980s
- 8:34when I was
- 8:35just
- 8:35starting,
- 8:36and, oh,
- 8:38Aunt Elsie
- 8:39died of
- 8:39retinoblastoma
- 8:40at age two.
- 8:41Oh,
- 8:42Uncle
- 8:42Morton,
- 8:43he died
- 8:43at age
- 8:44five from
- 8:44retinoblastoma.
- 8:45Oh,
- 8:46and then
- 8:46my mom
- 8:46had it, and
- 8:47she
- 8:47survived,
- 8:48and she
- 8:48had me.
- 8:51That's
- 8:51going to
- 8:51all be
- 8:51done.
- 8:53Yeah.
- 8:53So we've
- 8:54we've done
- 8:55a lot
- 8:56with
- 8:56retinoblastoma.
- 8:57And
- 8:58you
- 8:58mentioned,
- 8:59oh, we need
- 9:00FDA
- 9:00approval.
- 9:01Well,
- 9:01we sort of
- 9:02don't
- 9:04have any
- 9:04FDA
- 9:05approval
- 9:05for any
- 9:06of
- 9:06the drugs
- 9:06we use
- 9:07in
- 9:07retinoblastoma
- 9:09care.
- 9:10We don't have
- 9:10any
- 9:10approval
- 9:11for the use
- 9:12of
- 9:12vincristine
- 9:12Etoposide,
- 9:13Carboplatin,
- 9:14Melphalan,
- 9:14Topotecan
- 9:16for
- 9:16retinoblastoma.
- 9:18We're
- 9:18using it
- 9:19all in
- 9:19kind of a
- 9:20sympathetic
- 9:20way,
- 9:21and
- 9:22we know it
- 9:22works,
- 9:23and we've
- 9:23done enough
- 9:24trials
- 9:24to
- 9:24show that
- 9:25it works
- 9:26but
- 9:26we don't
- 9:26have FDA
- 9:27approval.
- 9:27We're just
- 9:28using it
- 9:28because
- 9:29it's been
- 9:30shown
- 9:31thanks
- 9:31to Judy
- 9:32Kingston
- 9:33in London,
- 9:33who first
- 9:34did it,
- 9:35and thanks
- 9:35to Kaneko
- 9:36in Japan,
- 9:37who first
- 9:37did IAC.
- 9:39We learned
- 9:39those drugs
- 9:40work,
- 9:40we started
- 9:41using them,
- 9:41there's no
- 9:42FDA
- 9:42approval
- 9:43and kids
- 9:43have lived
- 9:45enormously,
- 9:47thanks to
- 9:48some great
- 9:49observers
- 9:50who contributed
- 9:51to
- 9:51the field.
- 9:51So I think
- 9:53even though
- 9:53Bel-sar
- 9:54might not
- 9:54be approved
- 9:55for use in,
- 9:56you know,
- 9:56under
- 9:56age 18,
- 9:58we could
- 9:58probably
- 9:58use it
- 9:59in a
- 9:59sympathetic
- 10:00way for
- 10:01young
- 10:01patients.
- 10:03So you
- 10:03mentioned
- 10:03a little
- 10:03bit of
- 10:04some of the
- 10:06changes
- 10:06in
- 10:06retinoblastoma
- 10:07helping
- 10:08to lead
- 10:08to this
- 10:08kind
- 10:09of cusp
- 10:09of research
- 10:09for the
- 10:10the way
- 10:10that you
- 10:11treat Uveal
- 10:11melanoma.
- 10:12do
- 10:12you want to
- 10:12touch
- 10:13on that
- 10:13a little
- 10:13bit?
- 10:15Yeah.
- 10:15So,
- 10:16you know,
- 10:16back in
- 10:16the day
- 10:17in 1994
- 10:18when Judy
- 10:19Kingston
- 10:19from,
- 10:20Saint
- 10:21Bartholomew's
- 10:22Hospital
- 10:22in London,
- 10:23England,
- 10:23made
- 10:25an
- 10:25observation,
- 10:26she said,
- 10:27oh, I treat
- 10:28neuroblastoma
- 10:29with VEC
- 10:30(Vincristine,
- 10:30Etoposide,
- 10:31Carboplatin),
- 10:32maybe I'll
- 10:33use that
- 10:33for
- 10:33retinoblastoma.
- 10:34She did,
- 10:35and
- 10:35she saw a
- 10:35dramatic
- 10:36response.
- 10:37So
- 10:37all of us
- 10:38in the U.S.
- 10:39heard
- 10:39about this,
- 10:40and all
- 10:41three of us
- 10:41in North
- 10:42America
- 10:42started
- 10:43using it.
- 10:44And
- 10:46it was
- 10:46scary
- 10:47because
- 10:47we would
- 10:48put these
- 10:48kids on
- 10:49systemic
- 10:50chemotherapy,
- 10:51never
- 10:51knowing
- 10:52what we
- 10:52were going
- 10:52to see
- 10:53in the
- 10:53operating
- 10:53room,
- 10:54a
- 10:55little bit
- 10:55worried
- 10:55that,
- 10:56you know,
- 10:56some kids
- 10:57might
- 10:57get worse.
- 10:59But
- 10:59on the contrary,
- 11:00almost
- 11:00everybody
- 11:01got better.
- 11:03And now
- 11:03we're
- 11:03saving more
- 11:05eyes than
- 11:05ever.
- 11:07You know,
- 11:07only 5%
- 11:08of kids now
- 11:09require
- 11:09enucleation,
- 11:11and only 1%
- 11:13develop
- 11:14metastatic
- 11:14disease.
- 11:15And the
- 11:16whole world
- 11:17of
- 11:17chemotherapy,
- 11:19whether we
- 11:19give it
- 11:19intravenous
- 11:21intra
- 11:22arterial
- 11:23to behind
- 11:23the eye
- 11:24or inject
- 11:25the vitreous
- 11:26or inject
- 11:27the aqueous,
- 11:30so
- 11:30many eyes
- 11:30are saved
- 11:31and kids
- 11:32march
- 11:33out of
- 11:33this
- 11:33hospital
- 11:34so happy
- 11:35and parents
- 11:36so proud
- 11:37that
- 11:38their child
- 11:39is doing
- 11:39well.
- 11:40So, today
- 11:41I saw
- 11:41a family
- 11:42from
- 11:43Wisconsin.
- 11:45I treated
- 11:45their
- 11:46young boy
- 11:46when he was
- 11:47four years
- 11:48old.
- 11:49He had advanced
- 11:50retinoblastoma.
- 11:51We gave him
- 11:52Intra-arterial
- 11:53chemotherapy,
- 11:54Intravitreal
- 11:55chemotherapy.
- 11:56He's now
- 11:5716 years
- 11:58old,
- 11:59driving
- 12:00a car.
- 12:01He has
- 12:02no
- 12:02recurrence
- 12:03and he's
- 12:04doing
- 12:04awesome,
- 12:05and he
- 12:05still has
- 12:06his eye
- 12:07and he's
- 12:08he's a
- 12:09very neat
- 12:10little
- 12:11guy.,
- 12:11young guy,
- 12:14thankful
- 12:15that he has
- 12:16his life
- 12:16and
- 12:17his eye,
- 12:17and he has
- 12:18some vision
- 12:18out of
- 12:18that eye.
- 12:20This
- 12:20is, you
- 12:21know,
- 12:21this is
- 12:22how the
- 12:23whole field
- 12:23of
- 12:24retinoblastoma
- 12:24has been
- 12:25flipped.
- 12:26And I
- 12:26do think
- 12:27we're going
- 12:28to see
- 12:28a similar
- 12:30maybe
- 12:30slower flip
- 12:32for
- 12:32melanoma
- 12:33because
- 12:33we don't
- 12:34get answers
- 12:35with
- 12:35melanoma
- 12:36as fast
- 12:36as we do
- 12:37with
- 12:37retinoblastoma.
- 12:38Retinoblastoma
- 12:39you get
- 12:39answers
- 12:40in a month.
- 12:41If
- 12:41a treatment
- 12:42is not
- 12:42going
- 12:43to work
- 12:43you're
- 12:43going
- 12:43to know
- 12:44in a month.
- 12:44Hence,
- 12:45we always
- 12:46see these
- 12:46kids back
- 12:47when we're
- 12:47doing a new
- 12:48treatment
- 12:48one month
- 12:49later.
- 12:50With
- 12:50melanoma,
- 12:51usually
- 12:51don't
- 12:51get answers
- 12:52like for
- 12:52four months
- 12:53or six
- 12:53months.
- 12:54So it's
- 12:54going to be
- 12:55a little
- 12:55bit slower
- 12:56to get
- 12:57some of the,
- 12:57you know,
- 12:58results
- 13:00We're
- 13:00poised
- 13:01to have
- 13:02a lot of
- 13:03new
- 13:03therapies,
- 13:04and
- 13:06we're
- 13:06poised
- 13:07to have
- 13:08kind of a
- 13:08game
- 13:09changer now
- 13:10with uveal
- 13:10melanoma
- 13:12and,
- 13:14you know,
- 13:14I think
- 13:15it's going
- 13:15to impact
- 13:16pediatrics
- 13:16as well as
- 13:18adult uveal
- 13:18melanoma.
- 13:20Thank you
- 13:20so much.
- 13:22Let's shift
- 13:23gears a
- 13:23little bit.
- 13:24This is
- 13:24maybe a
- 13:25hot topic
- 13:25for some of
- 13:26our younger
- 13:26ocular
- 13:27melanoma
- 13:27adults.
- 13:30patients
- 13:30in the community
- 13:31who worry
- 13:32about
- 13:32future
- 13:33pregnancy
- 13:33or worry
- 13:34about
- 13:34whether or
- 13:35not
- 13:35pregnancy
- 13:36has a
- 13:36risk for,
- 13:37you know,
- 13:37causing
- 13:38metastases
- 13:38or just
- 13:39any of
- 13:39those kinds
- 13:39of things,
- 13:40as well as
- 13:41the risks
- 13:42around
- 13:42being
- 13:42pregnant
- 13:43while
- 13:43you're
- 13:43being
- 13:43treated
- 13:44with ocular
- 13:44melanoma.
- 13:45So do
- 13:46you want to
- 13:46just talk
- 13:46a little bit
- 13:47about some
- 13:47of the
- 13:47myths
- 13:47versus the
- 13:48the facts
- 13:49around
- 13:50pregnancy
- 13:50and ocular
- 13:50melanoma?
- 13:51Let's
- 13:51just start
- 13:52with
- 13:52primary
- 13:52treatment
- 13:52first.
- 13:53Yeah.
- 13:56So uveal
- 13:57melanoma
- 13:58can
- 13:58definitely
- 13:58occur
- 13:59in younger
- 14:00patients,
- 14:02and it
- 14:02can occur
- 14:04during the
- 14:06years when
- 14:07pregnancy
- 14:08is possible
- 14:09and it can
- 14:10occur
- 14:10during
- 14:10pregnancy,
- 14:12but
- 14:12we don't
- 14:13think
- 14:13pregnancy
- 14:14has
- 14:15any impact
- 14:16on the
- 14:17development
- 14:17of uveal
- 14:18melanoma.
- 14:20Uveal
- 14:20melanoma
- 14:21most often
- 14:21occurs
- 14:22in fair
- 14:22skinned
- 14:23people.
- 14:24We
- 14:24call that
- 14:25Fitzpatrick
- 14:26skin type
- 14:261 or 2
- 14:27like you
- 14:28and me,
- 14:29fair skin
- 14:29people and
- 14:32pregnancy
- 14:32does
- 14:33not lead
- 14:33to the
- 14:33development
- 14:34of uveal
- 14:34melanoma.
- 14:35That's
- 14:35a myth,
- 14:37and
- 14:37pregnancy
- 14:38does not
- 14:39portray
- 14:40or cause
- 14:41a higher
- 14:42risk for
- 14:42metastatic
- 14:43disease.
- 14:44That's
- 14:44a myth.
- 14:45The
- 14:46one thing
- 14:46that's
- 14:46a truth
- 14:48A patient
- 14:48who's
- 14:48pregnant
- 14:49tends
- 14:50to retain
- 14:51more
- 14:51bodily fluids,
- 14:53and
- 14:54when they
- 14:54have a
- 14:55melanoma in
- 14:55the eye,
- 14:56they tend
- 14:56to have
- 14:56more fluid
- 14:57with the
- 14:58melanoma
- 14:58in the eye,
- 15:00that's
- 15:00okay.
- 15:01The fluid
- 15:01goes away
- 15:02with
- 15:02treatment,
- 15:03no big
- 15:03deal.
- 15:04But,
- 15:04they're not
- 15:05going
- 15:05to get
- 15:06any higher
- 15:06risk of
- 15:07metastatic
- 15:08disease
- 15:09than a
- 15:09non-pregnant
- 15:10person.
- 15:10Now
- 15:10how do we know
- 15:11this.?
- 15:11Well we did
- 15:12a study
- 15:12on this
- 15:13many years
- 15:14ago,
- 15:15I think
- 15:15it was
- 15:16again
- 15:16in
- 15:17the 1990s.
- 15:18We looked
- 15:19at I think,
- 15:19it was 16
- 15:21pregnant
- 15:21patients
- 15:22and
- 15:23compared
- 15:23them to
- 15:25non-pregnant
- 15:26patients
- 15:27in the
- 15:27same age
- 15:28group
- 15:29and
- 15:29we found
- 15:29that they
- 15:30had the
- 15:30same
- 15:31incidence
- 15:32of
- 15:32metastatic
- 15:32disease.
- 15:33Then,
- 15:34Dan Albert,
- 15:35when
- 15:35he was
- 15:35up at
- 15:37Mass Eye
- 15:37and Ear,
- 15:38he looked
- 15:39at pregnant
- 15:41patients
- 15:42who had
- 15:42juvenile
- 15:42melanoma,
- 15:43who were
- 15:43pregnant
- 15:44and came
- 15:45to
- 15:46enucleation.
- 15:46He looked
- 15:47at surface
- 15:48markers on
- 15:49the
- 15:49melanoma.
- 15:50Do
- 15:50they have
- 15:50estrogen
- 15:51or
- 15:51progesterone,
- 15:55receptors
- 15:56that maybe
- 15:58the
- 15:58progesterone
- 15:59going up
- 15:59in
- 15:59pregnancy
- 16:00led to the
- 16:01development
- 16:01of melanoma
- 16:02and he said
- 16:03no
- 16:03receptors.
- 16:04So
- 16:05they don't
- 16:05have
- 16:06normal
- 16:07receptors
- 16:08that are
- 16:09found in
- 16:09pregnant
- 16:10women okay.
- 16:11So
- 16:12we really
- 16:13don't
- 16:15think
- 16:15pregnancy
- 16:16has any
- 16:16impact
- 16:17on the
- 16:17development
- 16:17or
- 16:20the risk
- 16:20for
- 16:20metastasis
- 16:21of
- 16:21melanoma.
- 16:22However,
- 16:23if
- 16:23a pregnant
- 16:24patient
- 16:25has a
- 16:25melanoma,
- 16:26we try not
- 16:27to
- 16:28irradiate
- 16:28during the
- 16:28first
- 16:29trimester
- 16:30because
- 16:33we
- 16:33don't
- 16:33want to
- 16:34expose
- 16:34the fetus
- 16:35to any
- 16:35radiation.
- 16:36If we have to
- 16:37irradiate,
- 16:38we will have
- 16:41the mother
- 16:42wear lead
- 16:44the entire time
- 16:46she has
- 16:46radiation
- 16:47on.
- 16:47So we
- 16:48give her
- 16:49our lead
- 16:49vest and
- 16:50a lead
- 16:51skirt
- 16:52so that way
- 16:53the radiation
- 16:54to her
- 16:55eye can't
- 16:56penetrate
- 16:56the lead
- 16:57vest,
- 16:57so it's not
- 16:58going
- 16:58to get
- 16:59to the fetus,
- 17:01and we
- 17:01do
- 17:01everything,
- 17:02all of
- 17:02our surgery
- 17:03under local
- 17:04anesthesia.
- 17:04No general
- 17:05anesthesia.
- 17:07Even if
- 17:07she's in
- 17:08second
- 17:08trimester
- 17:09and even
- 17:10sometimes
- 17:10in third
- 17:11trimester,
- 17:12we still do
- 17:13the lead
- 17:13vest on
- 17:15the mother,
- 17:16just for
- 17:17peace
- 17:18of mind,
- 17:19you know?
- 17:20so when we
- 17:22come in
- 17:23with our
- 17:23Geiger
- 17:23counter
- 17:24to check
- 17:25for
- 17:25radiation
- 17:26in a patient
- 17:27with
- 17:27melanoma,
- 17:28if
- 17:28we're this
- 17:29far away,
- 17:30we usually
- 17:31can't
- 17:31detect it
- 17:32in arm
- 17:33length
- 17:34but
- 17:34as soon as
- 17:34you get
- 17:35a little
- 17:35bit closer,
- 17:37like a fist
- 17:37length
- 17:38away,
- 17:39you can detect
- 17:39it.
- 17:40So imagine
- 17:41you're only
- 17:42going
- 17:42to detect
- 17:43a
- 17:44little bit
- 17:44of
- 17:44radiation
- 17:45in this
- 17:46aura,
- 17:46about
- 17:47the size
- 17:47of the
- 17:47head,
- 17:48and
- 17:49the
- 17:49radiation
- 17:50does not
- 17:50cause
- 17:51any damage
- 17:51to the
- 17:52opposite
- 17:52eye, to
- 17:53the brain,
- 17:53to
- 17:53the mouth,
- 17:54to the
- 17:54sinuses.
- 17:55So it's
- 17:56so minor
- 17:57that it's
- 17:58not going
- 17:58to cause
- 17:59any risk
- 18:00to
- 18:00the fetus,
- 18:01but we
- 18:01still, just
- 18:02for peace
- 18:03of mind,
- 18:04put a lead
- 18:05vest
- 18:06and a
- 18:06lead skirt
- 18:07on the mom,
- 18:08and
- 18:08in the past,
- 18:09and we
- 18:09sometimes
- 18:10still
- 18:10do this
- 18:11instead
- 18:11of having
- 18:12the mom
- 18:12wear it
- 18:14heavy on
- 18:14her abdomen
- 18:15that's
- 18:15already
- 18:16big.
- 18:17We put that,
- 18:19you know,
- 18:19in
- 18:19a hospital
- 18:20how
- 18:20you have,
- 18:21like,
- 18:21the dinner
- 18:22tray, that
- 18:23little
- 18:23moving
- 18:23dinner
- 18:24tray.
- 18:25We put that
- 18:25over
- 18:26the mother
- 18:27and
- 18:27we just lay
- 18:28the lead on
- 18:28top of that
- 18:29so
- 18:30that it's
- 18:30not on her
- 18:31abdomen,
- 18:32it's on
- 18:32the dinner
- 18:33tray, so
- 18:34it can't
- 18:35go through
- 18:35that.
- 18:37But a lot
- 18:37of it
- 18:37is myth,
- 18:39and
- 18:41not fact.
- 18:43This year
- 18:43at the
- 18:44Academy,
- 18:44though,
- 18:44I am giving
- 18:45a talk on
- 18:47ocular
- 18:47oncology
- 18:48and
- 18:48pregnancy.
- 18:50And
- 18:51you know
- 18:51what I'm
- 18:51talking
- 18:51about
- 18:53basically
- 18:54two things.
- 18:56Yeah,
- 18:56I'll talk
- 18:56about
- 18:57melanoma,
- 18:57but
- 18:57I'll say
- 18:58it's mostly
- 18:58myth.
- 18:59But with
- 19:00choroidal
- 19:01hemangioma,
- 19:02a pregnant
- 19:03woman gets
- 19:05lots of
- 19:05fluid, so
- 19:07choroidal
- 19:07hemangioma
- 19:08can get
- 19:09worse
- 19:09during
- 19:09pregnancy.
- 19:11So,
- 19:12if you have
- 19:12a choroidal
- 19:13hemangioma,
- 19:13it might
- 19:14become more
- 19:14leaky
- 19:15during
- 19:16pregnancy
- 19:17and
- 19:17what do we
- 19:18do?
- 19:19We usually
- 19:19wait for
- 19:20the patient
- 19:21to become
- 19:22unpregnant
- 19:23because
- 19:24the fluid
- 19:24tends to go
- 19:25away.
- 19:26I'll never
- 19:27forget
- 19:2920
- 19:29years ago
- 19:30I had a Hispanic
- 19:32lady
- 19:32who had a
- 19:33choroidal
- 19:33hemangioma.
- 19:34She had
- 19:35total
- 19:35retinal
- 19:36detachment
- 19:37and we
- 19:37said, let's
- 19:38sit tight
- 19:39till she
- 19:39delivers
- 19:40the baby.
- 19:40She delivered
- 19:41the baby.
- 19:42All her
- 19:42fluid
- 19:42went away
- 19:45because all
- 19:45that
- 19:45fluid that
- 19:46you retain
- 19:47goes away.
- 19:48So it's
- 19:49good
- 19:49to wait
- 19:49on that.
- 19:49The same
- 19:50with
- 19:50meningioma.
- 19:51That's the
- 19:52second
- 19:52tumor.
- 19:53Meningioma
- 19:54is
- 19:54if they're
- 19:54in
- 19:54the brain
- 19:55or around
- 19:56the nerve
- 19:56of the eye.
- 19:57They can
- 19:58get worse
- 19:58with water
- 20:00retention
- 20:00during
- 20:00pregnancy,
- 20:01but then
- 20:02that goes away
- 20:03once they
- 20:03deliver
- 20:04the baby.
- 20:06I think
- 20:06as far
- 20:06as
- 20:06pregnancy
- 20:07and uveal
- 20:07melanoma
- 20:08goes,
- 20:08I think
- 20:08hearing you
- 20:09talk about
- 20:09some of
- 20:09those
- 20:10studies
- 20:10that you
- 20:10shared
- 20:11around,
- 20:12just
- 20:12showing
- 20:12that we
- 20:13don't see
- 20:13any of
- 20:14those risk
- 20:14factors
- 20:15for
- 20:15progesterone
- 20:15or
- 20:16estrogen,
- 20:16showing
- 20:17that
- 20:17pregnancy
- 20:18would
- 20:18exacerbate,
- 20:18say,
- 20:19metastatic
- 20:19growth
- 20:20or the
- 20:20primary
- 20:20tumor
- 20:21growth.
- 20:21That's
- 20:22hugely
- 20:22reassuring
- 20:23and I'm
- 20:23sure
- 20:23that the
- 20:23people
- 20:24listening,
- 20:24we can share
- 20:25that with
- 20:25our friends
- 20:25just to
- 20:26make sure
- 20:26that they
- 20:26know,
- 20:27there's,
- 20:27there's so
- 20:28many ways
- 20:28and so
- 20:29many things
- 20:29that
- 20:29especially
- 20:30when we're
- 20:30a younger
- 20:30patient
- 20:31that ocular
- 20:31melanoma,
- 20:32uveal
- 20:32melanoma
- 20:33can just
- 20:33feel very
- 20:34scary
- 20:34and
- 20:34uncertain.
- 20:35Right,
- 20:35and bottom
- 20:37line,
- 20:37early
- 20:38detection
- 20:39makes a
- 20:39difference.
- 20:41Really
- 20:41important!
- 20:42So you
- 20:43mentioned
- 20:43not
- 20:43treating
- 20:44in the
- 20:44first trimester.
- 20:45So if you
- 20:45find
- 20:46something
- 20:46that is
- 20:46early like
- 20:47a small
- 20:48nevus,
- 20:49let's just
- 20:49hypothetically
- 20:50speak here.
- 20:50would
- 20:51you still
- 20:51want
- 20:52to wait
- 20:52for
- 20:53a couple
- 20:53of months?
- 20:53Do you have
- 20:54that kind
- 20:54of window
- 20:54of time
- 20:55to work
- 20:55with,
- 20:55with the
- 20:55first
- 20:56trimester
- 20:56versus
- 20:57the second
- 20:57in a
- 20:57pregnancy?
- 20:58patient.
- 20:59Yes.
- 20:59We have so
- 21:00much better
- 21:02treatment
- 21:02options.
- 21:03We have
- 21:03so much
- 21:04more
- 21:04freedom
- 21:04for
- 21:05treatment
- 21:05second
- 21:06and third
- 21:07trimester.
- 21:09First
- 21:09trimester,
- 21:09we really
- 21:10want to be
- 21:11super
- 21:12careful
- 21:12with the
- 21:12fetus
- 21:13because,
- 21:15you know,
- 21:17cells
- 21:17are
- 21:18dividing
- 21:18into the
- 21:18liver,
- 21:19into the eye,
- 21:20into
- 21:20different
- 21:21organ
- 21:22structures
- 21:22and we
- 21:22don't
- 21:22want to
- 21:23disturb
- 21:23any of that.
- 21:24So if
- 21:25we saw
- 21:25a choroidal
- 21:26nevus
- 21:26that was
- 21:26a bit
- 21:27suspicious
- 21:28in the
- 21:28first trimester,
- 21:29we would
- 21:29probably
- 21:30bring
- 21:30the mother
- 21:30back
- 21:31in, say
- 21:312 to 3
- 21:32months,
- 21:33hopefully
- 21:34get them
- 21:34into the
- 21:34second
- 21:35trimester,
- 21:35and then
- 21:36if we
- 21:36needed to
- 21:37do
- 21:37treatment,
- 21:37whether
- 21:38it be
- 21:40low dose
- 21:41radiotherapy
- 21:42or standard
- 21:42dose
- 21:43radiotherapy
- 21:44versus
- 21:45maybe
- 21:45transpupillary
- 21:46thermotherapy,
- 21:47we would
- 21:47do it,
- 21:48okay?
- 21:50so much
- 21:51more
- 21:51freedom
- 21:52in the
- 21:52second
- 21:52trimester
- 21:54and
- 21:54of course
- 21:54a
- 21:54difference,
- 21:55much more
- 21:55freedom.
- 21:56By the time
- 21:56you get in
- 21:57the third
- 21:57trimester,
- 21:58most of the
- 21:58organ
- 21:58systems
- 21:59are already
- 21:59developed.
- 22:00It's
- 22:00mostly just
- 22:01growth of
- 22:02the organ
- 22:03systems
- 22:03at that
- 22:03point.
- 22:05This
- 22:05has been
- 22:06so, so
- 22:06neat to
- 22:07listen to.
- 22:07So Dr.
- 22:08Shields,
- 22:08I'm
- 22:08so thankful
- 22:09to you,
- 22:09so thankful
- 22:10for your
- 22:10expertise
- 22:11and we here
- 22:12at A Cure
- 22:12In Sight,
- 22:13and through
- 22:13the Eye
- 22:13Believe
- 22:13Podcast
- 22:14listening
- 22:14community,
- 22:15are so
- 22:15grateful
- 22:15for
- 22:16everything
- 22:16that
- 22:16you've shared
- 22:17and
- 22:17just the
- 22:17continued
- 22:18efforts
- 22:19that you
- 22:19and your
- 22:19team make
- 22:20in
- 22:20research,
- 22:20in
- 22:21training,
- 22:21the next
- 22:21generation
- 22:22of ocular
- 22:22oncologists.
- 22:24that you do
- 22:24a lot of
- 22:25medical
- 22:25teaching
- 22:25with your
- 22:26fellows
- 22:26and your
- 22:26students,
- 22:27so we are
- 22:28so thankful
- 22:28for
- 22:28everything
- 22:29that you do
- 22:29for the
- 22:29ocular
- 22:30melanoma
- 22:30community
- 22:31every
- 22:32single day.
- 22:33Well, thank
- 22:35you and
- 22:36our
- 22:36whole team
- 22:37feels
- 22:37honored.
- 22:38I couldn't
- 22:38do this
- 22:38without all
- 22:39the
- 22:40technicians
- 22:40and the
- 22:41front
- 22:41desk,
- 22:42back desk
- 22:43counselors,
- 22:44you
- 22:44name it.
- 22:45We all work
- 22:45together
- 22:46as a team.
- 22:47You know,
- 22:47it's not
- 22:48just one
- 22:48person
- 22:49taking care
- 22:49of
- 22:49the patient.
- 22:50It is
- 22:50really an
- 22:51entire team
- 22:52and for us,
- 22:53it's
- 22:54a great honor
- 22:55and we will
- 22:56continue
- 22:57to strive
- 22:57to do
- 22:58our best.
- 23:00Well,
- 23:00for those
- 23:01of you
- 23:01listening,
- 23:01please
- 23:01make sure
- 23:02to share
- 23:02this
- 23:02episode
- 23:03with any of
- 23:03your
- 23:03friends
- 23:04and
- 23:04anyone else
- 23:04in the
- 23:04ocular
- 23:05melanoma
- 23:05community
- 23:06and we
- 23:07are so
- 23:07delighted
- 23:08to have Dr.
- 23:08Shields
- 23:09here.
- 23:09We hope
- 23:09we'll
- 23:09get to have
- 23:09you back
- 23:10at a future
- 23:10date,
- 23:10maybe
- 23:11to talk
- 23:11about
- 23:11something
- 23:12new or
- 23:12something
- 23:13a little
- 23:13more
- 23:13specific.
- 23:14Maybe
- 23:14we could
- 23:14talk about
- 23:15Bel-sar,
- 23:15or we could
- 23:15talk about
- 23:16Daro,
- 23:16a little
- 23:17bit more
- 23:17in depth
- 23:18but
- 23:18it's
- 23:18been such
- 23:18a pleasure,
- 23:19and we
- 23:19look
- 23:19forward to
- 23:20having
- 23:20you back
- 23:20next time.
- 23:22Great!
- 23:22Thank you,
- 23:22Danet!
- 23:22Tonight,
- 23:23this was
- 23:23really a,
- 23:24a fun
- 23:25conversation.
- 23:26I really
- 23:26appreciate
- 23:27it.
- 23:27Thanks to
- 23:28all of.
