- 0:00Hi, everyone. This is Lucas Voss with the
- 0:01Becker's Healthcare podcast series. Thanks so much for
- 0:04tuning in today. Very excited to have you.
- 0:07And I am excited and thrilled to be
- 0:09joined by Mark Johnson, senior vice president at
- 0:12Saris, who will talk to us about payment
- 0:15integrity. Very important topic. Mark, thanks so much
- 0:17for being here today. Thanks for having me,
- 0:19Lucas. Appreciate it. Mark, we'll start off with
- 0:21introductions here. If you just wanna give us
- 0:23a quick introduction to yourself and just your
- 0:26your background in health care and your career
- 0:28so far.
- 0:30Absolutely. I've been with,
- 0:32Cirrus now for a little over five years,
- 0:33but my background, you know, is around thirty
- 0:36years experience in the health care space. I've
- 0:38been in patient care at the bedside,
- 0:41into medical billing, payment integrity, insurance.
- 0:45Really, I've I've led significant product improvements in
- 0:49that thirty years at Cirrus. We're part of
- 0:51the CorVel Corporation.
- 0:53I've been at Blue Cross Blue Shield in
- 0:54Minnesota.
- 0:55I had a stint at UnitedHealth Group as
- 0:57well. My background includes patient care,
- 1:00payment integrity,
- 1:01vendor management,
- 1:03regulatory
- 1:04compliance.
- 1:05I've done some international
- 1:06operations startups as well.
- 1:09Educationally, I've got a bachelor's degree in physiology
- 1:12from the University of Minnesota
- 1:14and an MBA from Regis University.
- 1:17Go Gophers
- 1:18in in that case. Go Gophers. Go big
- 1:2110. Yeah. Ex exactly. We'll we'll go with
- 1:24that.
- 1:25Well, again, Mark, it's it's great to have
- 1:26you, and this is an important topic today.
- 1:29I was doing research for this podcast, and
- 1:32and this stat just really stood out to
- 1:34me. There's an estimation that up to eighty
- 1:38percent yes. You heard that right. Eighty percent
- 1:40of medical bills
- 1:42contain
- 1:43errors, which is staggering.
- 1:45As leaders in the payer space increasingly
- 1:48look to really optimize
- 1:50processes, we know that's happening, especially around billing
- 1:53and payments.
- 1:54What's the role of payment integrity
- 1:57from your perspective, and why is it so
- 2:00important?
- 2:01Yeah. Payment integrity
- 2:03in the payer space is incredibly important. It
- 2:05just ensures that claims are accurately
- 2:08processed
- 2:09and more importantly, accurately reimbursed
- 2:12to the providers.
- 2:13It helps prevent errors, fraud, waste, and abuse.
- 2:17Really, this is crucial
- 2:19because it helps maintain financial stability, not only
- 2:22for the payer, but for the providers being
- 2:24impacted.
- 2:25It also enhances the trust between the payers
- 2:27and the providers.
- 2:29And, you know, lastly, it ensures that the
- 2:31health care dollars are being used appropriately.
- 2:33That's why payment integrity is so important.
- 2:37Yeah. It's sort of a I view it
- 2:38sort of as a as a foundational
- 2:40piece. It's really the foundation that a lot
- 2:43of things stand on. And if that foundation
- 2:45isn't there,
- 2:46then there are going to be problems as
- 2:48you've outlined.
- 2:50And
- 2:51the process is is really key to this
- 2:54to this area,
- 2:55the process around,
- 2:58around everything that you've touched on.
- 3:00How can changing the process
- 3:03around payments
- 3:04reduce some of these numbers of errors, right,
- 3:0680?
- 3:07And what strategies have you found beneficial?
- 3:10Do you have any examples that that stood
- 3:12out to you and some of the results
- 3:13that that you see that that make a
- 3:15difference?
- 3:17Sure, Lucas. I'm I'm trying to think over
- 3:20thirty years, payment integrity really didn't exist thirty
- 3:22years ago. Right? And it's going through a
- 3:25lot
- 3:26of transition right now, a lot of growth.
- 3:29I can think of maybe four or five
- 3:31key strategies that payers might wanna contemplate.
- 3:35The first would be
- 3:36to centralize their payment integrity strategy.
- 3:40Take a centralized approach that integrates your prepay
- 3:43editing reviews,
- 3:44your post pay auditing capabilities,
- 3:47and as well as your innovation teams, your
- 3:49r and d teams in order to streamline
- 3:51processes
- 3:52and try to reduce those errors,
- 3:54especially prepayment.
- 3:56Secondly, I would say
- 3:58when you're thinking about artificial intelligence
- 4:01and machine learning, those are big buzzwords
- 4:03lately,
- 4:05really think about it as a complement to
- 4:07the clinical teams that you have deployed in
- 4:10your payment integrity space as well as your
- 4:12certified coders.
- 4:14Utilizing AI,
- 4:15machine learning for real time analysis of claims,
- 4:19proactively
- 4:20identifying high risk claims,
- 4:22and really helping your clinicians and your coders
- 4:25identify,
- 4:27those claims quickly can significantly
- 4:30enhance your accuracy and as well as throughput
- 4:33for the teams working those claims.
- 4:35Third strategy,
- 4:37contemplate a second pass claims editing process.
- 4:42A second pass vendor, second pass process, or
- 4:45even a final filter on claims
- 4:48can really help catch errors that are missed
- 4:50in primary editing phases.
- 4:52Just, again, reducing inappropriate payments from going out
- 4:55the door.
- 4:57Fourth, and this is a big one,
- 4:59coordination of benefits.
- 5:01Mhmm. We really need a strong coordination of
- 5:03benefits program. You may want to consider adopting
- 5:06a pause and pay approach
- 5:08with advanced analytics again, right, to ensure you're
- 5:11correct payment responsibility,
- 5:13eligibility validation before the claims are even paid.
- 5:16Just reduce that overpayment from going out the
- 5:19door
- 5:20and all of the overhead associated with the
- 5:22pay and chase model.
- 5:25Lastly,
- 5:26quality assurance programs.
- 5:29Establishing a robust
- 5:31QA program
- 5:32to conduct pre billing audits can really catch
- 5:35errors early and then just improve overall billing
- 5:38accuracy.
- 5:39I guess with these strategies,
- 5:42you know, health care payers
- 5:44can really enhance their payment integrity program and
- 5:46reduce those errors, really ultimately
- 5:49improving overall financial performance.
- 5:52Yeah. Absolutely. And just as a as a
- 5:54quick follow-up to that, should this be a
- 5:56a holistic approach as in we're focusing on
- 6:00all of these strategies, we're incorporating them all,
- 6:02or is there one focus area from the
- 6:04things that you just mentioned that that, organizations
- 6:08should look at?
- 6:10I all. I would I would be looking
- 6:12at all of these right now. If you
- 6:15had to prioritize,
- 6:17something, I would I would try to centralize
- 6:19your payment integrity strategy.
- 6:21You may have payers may have post pay
- 6:24services, prepay services,
- 6:27clinical utilization
- 6:29management services, all under different
- 6:32leaderships
- 6:33Mhmm. Or strategies.
- 6:35And some of those strategies may be at
- 6:37odds with each other. Make sure you're all
- 6:39aligned
- 6:40first.
- 6:40And then you then you can really start
- 6:43digging into
- 6:44solutions,
- 6:45vendor partners,
- 6:47like Cirrus
- 6:48or any other,
- 6:50technology play.
- 6:52Yeah. Absolutely.
- 6:53Which leads me to my next point, and
- 6:54this was one of your your strategies or
- 6:56one of the pieces of that you've mentioned
- 6:58that are really important right now. The modernization
- 7:01efforts
- 7:02efforts that are happening in the payer space.
- 7:04They are all over. You've mentioned the buzzwords.
- 7:05Right? AI. It's it's all over the place.
- 7:08Again, touch on how you see technology
- 7:10evolving and really making an impact on reducing
- 7:13errors and enhancing
- 7:15payment integrity.
- 7:17What are you looking forward to right beyond
- 7:20those buzzwords, and how should organizations prepare?
- 7:23Wow. That's it's such a big area. The
- 7:27technologies are just evolving
- 7:29so rapidly.
- 7:31I would say that if if you have
- 7:33an AI strategy,
- 7:35excellent. If you don't, get one quickly.
- 7:40Innovations such as artificial intelligence,
- 7:42digital payment systems,
- 7:46finance solutions, just embedded finance solutions are really
- 7:49transforming health care payments. Mhmm. Providers
- 7:52themselves
- 7:53are starting to look at these these technologies
- 7:56as well to help them with their revenue
- 7:58cycle.
- 7:59So while this these technologies
- 8:02I mean, all of this tech play
- 8:05can really help automate billing processes and reduce
- 8:08manual errors and just improve that rev cycle
- 8:11for both payers and and providers.
- 8:14It's still going to be important to have
- 8:17that
- 8:17human
- 8:18touch, that clinical
- 8:20overview, that coder overview.
- 8:22You don't want to get caught
- 8:25automating your your edits, automating your your solutions.
- 8:29Technology can help,
- 8:31but you shouldn't put all of your eggs
- 8:33in just in that baskets.
- 8:35You know, adopting advanced payment technologies,
- 8:38making sure that that cash is flowing
- 8:42smoothly and accurately between the payer and the
- 8:44provider. And I think that's that's really where
- 8:47you're gonna wanna be focused.
- 8:49Yeah. Absolutely.
- 8:51Well, Mark, thank you so much for sharing
- 8:53all of your insights here. Again, thirty years
- 8:55of experience. We really appreciate you taking the
- 8:57time today. Thanks for being here.
- 8:59I appreciate it, Lucas. Thank you for the
- 9:01time. I I really do.
