- 0:02Speaker 1: necessarily reflect the views and opinions of five ADWCHS it's
- 0:06Speaker 1: employees or WVRC Media.
- 0:21Speaker 2: And very pleas a good morning to you.
- 0:23Speaker 3: It is eight twenty three and you are listening to
- 0:26Speaker 3: your Voice of Charleston five ADWHS. I'm Del Cooper on
- 0:29Speaker 3: this Tuesday morning. Man, it is just gorgeous out there.
- 0:32Speaker 3: It's like homeostasis outside. You walk outside and everything is
- 0:36Speaker 3: like it feels like nature, Like there's no humidity, the
- 0:39Speaker 3: temperature is exactly where it should be it is. It
- 0:42Speaker 3: feels really nice out there. I was just amazed when
- 0:44Speaker 3: I walked out this morning, you didn't have that little
- 0:46Speaker 3: bit of hit of humidity that was gone. It wasn't
- 0:48Speaker 3: so chilly where you felt like it was just like homeostasis.
- 0:51Speaker 2: It was like good stuff outside.
- 0:53Speaker 3: And speaking of good stuff, we'd like to bring into
- 0:55Speaker 3: the studio this morning from gree Dental Group joining us
- 0:58Speaker 3: as they usually do together. Doctor Stephen Garib is here
- 1:01Speaker 3: and doctor Carson Henley. Doctor Stephen, how you doing this morning?
- 1:03Speaker 4: Doing great? Thank you very much. I am enjoying the weather.
- 1:06Speaker 4: It's uh didn't feel so sticky.
- 1:09Speaker 2: This morning. No, it's nice. It's nice.
- 1:11Speaker 3: I like I like those those addum mornings where it's
- 1:13Speaker 3: still warm enough where you don't need a jacket or anything,
- 1:15Speaker 3: you don't have to worry about any of that stuff,
- 1:16Speaker 3: but there's still no humidity. It's kind of nice. Doctor
- 1:20Speaker 3: Carson's with us as well. How are you doing this morning?
- 1:22Speaker 5: Just like miscongeniality, what's your favorite date? It's like April
- 1:26Speaker 5: twenty first, right anyway, So when it comes up to
- 1:31Speaker 5: homeostasis for the body, it's like, what ninety eight degrees?
- 1:33Speaker 5: So when you said homeostasis, I'm like, it's not homeostace,
- 1:38Speaker 5: but I know you with the weather, it feels natural
- 1:41Speaker 5: to be outside my scientific, nerdy mind somewhere on man.
- 1:45Speaker 4: That's that's that's a very Carson comment.
- 1:52Speaker 3: We are here this morning taking your well, they're taking
- 1:55Speaker 3: your dental questions about your dental hygiene, any questions that
- 1:58Speaker 3: you might have for yourself, were a loved one, your child,
- 2:00Speaker 3: or for maybe an elderly person in your family that's
- 2:02Speaker 3: looking for some help. We're going to be covering a
- 2:04Speaker 3: bunch of topics this morning, but you can help scare
- 2:06Speaker 3: that conversation as well and give us a call this
- 2:07Speaker 3: morning three zero four, three four five fifty fifty eight
- 2:10Speaker 3: three four five fifty eight fifty eight. You can also
- 2:11Speaker 3: text us this morning three zero four nine three five
- 2:13Speaker 3: five zero zero eight. Three zero four nine, three five
- 2:16Speaker 3: five zero zero eight is our text line number. I
- 2:18Speaker 3: want to talk this morning kind of progressive discussion about
- 2:23Speaker 3: dental hygiene and teeth and so forth. This morning, I
- 2:25Speaker 3: want to start a little bit about and this is
- 2:28Speaker 3: this is really a good topic because we we talk
- 2:30Speaker 3: a lot about things that you can do to repair teeth.
- 2:33Speaker 3: There's all kinds of different things that you guys bring
- 2:35Speaker 3: up on the show. Folks could come in and get
- 2:38Speaker 3: evaluated for different types of repair and things like that.
- 2:40Speaker 3: But sometimes it's like a like a like a tire
- 2:44Speaker 3: that you patched. You know, eventually you just have to
- 2:46Speaker 3: replace the tire. And maybe that's not the best analogy
- 2:48Speaker 3: in the world, but when it comes to taking care
- 2:52Speaker 3: of your teeth, patching the problems that are in your mouth,
- 2:54Speaker 3: and different things along those lines, there comes a point
- 2:56Speaker 3: where you have to determine if continuing down that path
- 2:59Speaker 3: is the best or maybe you have to look for
- 3:01Speaker 3: another program you can you can come up with something
- 3:02Speaker 3: better that's right.
- 3:03Speaker 4: Yeah, So I mean what we see, it's one of
- 3:05Speaker 4: the most common things that come into the office is
- 3:08Speaker 4: the patient comes in and there's a tooth that has
- 3:12Speaker 4: a significant break in it or a significant cavity that's
- 3:16Speaker 4: pretty large. Maybe it's had some fillings done before. And
- 3:19Speaker 4: the question is, you know, doc, can we patch this
- 3:22Speaker 4: up and fix it and get us through for a
- 3:24Speaker 4: little bit longer or fix it completely that way, or
- 3:29Speaker 4: do we have to say, you know, we've done enough
- 3:30Speaker 4: of that and we really have to go to the
- 3:32Speaker 4: next step. And then we talk about what are the
- 3:34Speaker 4: next steps. And you can take this however far you'd
- 3:37Speaker 4: like to go. With one single tooth, a corner of
- 3:40Speaker 4: the mouth. We call it a quadrant, you know, we
- 3:42Speaker 4: break things up into four sections, or you can talk
- 3:44Speaker 4: about entirely all of your teeth are starting to do this.
- 3:47Speaker 4: What do we do in general for a total plan?
- 3:50Speaker 2: But the.
- 3:53Speaker 4: Bottom line is that I do use the car analogy
- 3:56Speaker 4: a lot of times. You know, tires are getting bald,
- 3:59Speaker 4: or you have a hole and a tire, we can
- 4:01Speaker 4: patch it, and those patches last for a period of time,
- 4:04Speaker 4: but it's not a permanent solution. So we can typically
- 4:08Speaker 4: patch up a tooth, but we have to know that
- 4:10Speaker 4: this may not be the best thing for the tooth
- 4:12Speaker 4: or for you in the long term. So after that,
- 4:15Speaker 4: once we've kind of filled it to its maximum amount,
- 4:20Speaker 4: what can happen is the tooth then is in danger
- 4:24Speaker 4: of fracturing or breaking or becoming weak and needing a
- 4:27Speaker 4: root canal down down the line if we leave it
- 4:31Speaker 4: in that condition. So the next step typically would be
- 4:35Speaker 4: kind of upgrade it in a sense to the next
- 4:38Speaker 4: more complex treatment would be a crown on that tooth.
- 4:41Speaker 4: So if you filled it all that you can then
- 4:44Speaker 4: a crown. What that is some people call them caps,
- 4:46Speaker 4: and they it covers the entire tooth three hundred and
- 4:49Speaker 4: sixty degrees with a strong porcelain material. Sometimes metal material
- 4:54Speaker 4: like a gold we can use as well, but it
- 4:56Speaker 4: holds the tooth together and protects it from future breaking. Now,
- 5:00Speaker 4: if the decay is too deep and goes into the
- 5:03Speaker 4: center of the tooth, sometimes that requires us to do
- 5:05Speaker 4: a root canal as well. So then you have the question, Okay,
- 5:08Speaker 4: is this tooth decayed or damage so far that even
- 5:11Speaker 4: a root canal and a crown are not advisable. And
- 5:15Speaker 4: sometimes you get to the point to where you say,
- 5:17Speaker 4: you know, we could do a root canal and a
- 5:19Speaker 4: crown on this tooth, but it might not last that long.
- 5:23Speaker 4: So then we're in the discussion of, well, should we
- 5:25Speaker 4: consider a dental implant at that point, or extraction and
- 5:29Speaker 4: maybe a dental bridge or partial. So that kind of
- 5:32Speaker 4: opens up to an entirely different set of options in
- 5:37Speaker 4: a sense for patients.
- 5:40Speaker 2: And that.
- 5:42Speaker 3: And that ongoing relationship is really why it's beneficial to
- 5:45Speaker 3: see your Dennis on a regular basis to get problems
- 5:47Speaker 3: address as they come up, because if you're having that conversation,
- 5:49Speaker 3: of course, if somebody comes out off that you've never
- 5:51Speaker 3: seen before, you can get to that point of helping
- 5:54Speaker 3: somebody out. But it really helps things quite a bit
- 5:56Speaker 3: if you're a patient of someone if you see somebody
- 5:58Speaker 3: all the time, because it really helps you get to
- 6:01Speaker 3: know that person and you can advise them probably a
- 6:03Speaker 3: lot more closely.
- 6:04Speaker 5: Doctor Steven No comment. That was such such a complete answer,
- 6:10Speaker 5: creat job, I might clapping inside my brain.
- 6:12Speaker 4: Wow, I've impressed doctor Henley.
- 6:16Speaker 2: You always impressed me.
- 6:17Speaker 5: But that was like, there's like nothing else to add
- 6:19Speaker 5: I mean, it was really good.
- 6:20Speaker 4: Well, you know, to your point though, about having someone
- 6:23Speaker 4: have that relationship with their dentist, because that's what kind
- 6:26Speaker 4: of hurts me the most for the patient is when
- 6:29Speaker 4: they come in and they're focusing on this one tooth.
- 6:33Speaker 4: This one tooth is breaking down. It's hurting me, you know. Okay,
- 6:35Speaker 4: we can do all of these things. We go through
- 6:36Speaker 4: that process, what should we do? But then they have
- 6:40Speaker 4: five other teeth that are in very similar shape. But
- 6:43Speaker 4: the way that the body is, they're not hurting them
- 6:45Speaker 4: at this point right now, it's just that one tooth
- 6:47Speaker 4: that's bothering them. So then we have to start talking about, Okay,
- 6:50Speaker 4: really we need to get you into a comprehensive plan
- 6:53Speaker 4: to keep this from happening in two months for the
- 6:55Speaker 4: next tooth, and then two months for the next tooth,
- 6:57Speaker 4: and then the next tooth.
- 6:57Speaker 6: You know.
- 6:58Speaker 4: So so really that drives home the point that you're making.
- 7:01Speaker 4: You know, let's keep things regular, stay regular with your
- 7:04Speaker 4: dental checkups, and in the long run, that will avoid
- 7:09Speaker 4: a lot of more comprehensive and expensive treatment.
- 7:12Speaker 3: And that goes for people that only go to the
- 7:14Speaker 3: dentist when there's a when there's a presenting problem. If
- 7:17Speaker 3: you're constantly chasing these things instead of addressing the issue
- 7:20Speaker 3: at the root cause to keep it from happening in
- 7:21Speaker 3: the future, you just want to constantly going in with pain,
- 7:24Speaker 3: Hey doc, hey, doc, hey doc. At a point, there's
- 7:26Speaker 3: something you can do about that. There there's actual maintenance
- 7:28Speaker 3: you can do that keep that from happening for sure. Yeah,
- 7:31Speaker 3: let's talk about if you get to that point where
- 7:34Speaker 3: you've tried these different things, you have some problems with
- 7:36Speaker 3: your with your teeth, dental hygiene, different things along those
- 7:39Speaker 3: lines where whether it be genetic or something else that's happened.
- 7:43Speaker 3: We've talked about the on the show, we've done with
- 7:45Speaker 3: dedicated shows to it before. But you do have implants
- 7:47Speaker 3: and teeth in a day, and that's something that of
- 7:50Speaker 3: course we need to set the expectations for that, as
- 7:52Speaker 3: we always do with that discussion.
- 7:54Speaker 4: Reject about this coming in. I mean, it's it's amazing,
- 7:57Speaker 4: but yeah, so that's one of the most common questions
- 7:59Speaker 4: like I have coming into the office is what I'll
- 8:02Speaker 4: hear about this teeth in a day? You know, can
- 8:03Speaker 4: we do that? And how's that work? And really it's
- 8:06Speaker 4: funny because a lot of patients come in and they say,
- 8:08Speaker 4: surely that can't happen. I mean, that's crazy. You can't
- 8:11Speaker 4: get teeth in a day. And I'll find that funny
- 8:14Speaker 4: because really I expect patients to come in demanding that
- 8:17Speaker 4: I expect to get teeth in a day. But the
- 8:19Speaker 4: more common thing is I hear about this in no
- 8:21Speaker 4: way that works.
- 8:22Speaker 2: Well.
- 8:22Speaker 4: The answer is it can work in certain situations depending
- 8:26Speaker 4: on what you choose. So the idea of teeth in
- 8:30Speaker 4: a day, you can explain that a few different ways.
- 8:33Speaker 4: Let's go classically. We've had teeth in a day for
- 8:37Speaker 4: a long time in dentistry, and we would call those
- 8:40Speaker 4: immediate dentures. So what happens there is you come in,
- 8:43Speaker 4: you have a set of bad teeth. You can have
- 8:46Speaker 4: those teeth taken out in that same day, a denture
- 8:49Speaker 4: put in into your mouth that day. Now, anyone that
- 8:52Speaker 4: wears dentures knows that that is not a perfect solution
- 8:58Speaker 4: because when you have a set of teeth that need removed,
- 9:01Speaker 4: when we send it to the lab to make those
- 9:03Speaker 4: immediate dentures we call them, they have to remove the
- 9:06Speaker 4: teeth kind of virtually or on the model outside of
- 9:10Speaker 4: the mouth and guess how the gums are going to respond,
- 9:14Speaker 4: and guess how much bone is needing to be removed
- 9:16Speaker 4: to take those teeth out, So that denture is never
- 9:19Speaker 4: going to be a perfect fit. Even if it were
- 9:22Speaker 4: a perfect fit, your gums will swell and recede and shrink,
- 9:26Speaker 4: and the bone will compress and change over a period
- 9:29Speaker 4: of six to eight weeks after having the teeth taken out,
- 9:32Speaker 4: then that denture would have to be re lined. Now
- 9:35Speaker 4: that's important to know talking about anything else forward. Once
- 9:38Speaker 4: you remove teeth, things change. So if we're going to
- 9:42Speaker 4: go from that type of a teeth in a day
- 9:44Speaker 4: to the more modern style that people talk about is
- 9:48Speaker 4: taking out your teeth, putting dental implants in place, and
- 9:51Speaker 4: then putting teeth on those implants that day that we
- 9:55Speaker 4: do all the time in the office can and we
- 9:58Speaker 4: can do it in the right situation. But the important
- 10:02Speaker 4: thing is that whatever we put into your mouth that day,
- 10:05Speaker 4: taking out our teeth, putting the dental implants, in attaching
- 10:08Speaker 4: teeth to those implants, then the bone and the gums
- 10:11Speaker 4: are going to change. And whatever we put in does
- 10:14Speaker 4: give you teeth that day, but it is a temporary
- 10:16Speaker 4: bridge of some sort that will be replaced in twelve weeks.
- 10:20Speaker 4: Once everything heals well and the implants are integrated, confirmed
- 10:24Speaker 4: to be very solid. So some patients are candidates for that,
- 10:28Speaker 4: others are not, and that's for a discussion about if
- 10:31Speaker 4: you're a candidate for implants and what not to. But
- 10:35Speaker 4: then some patients do get a little confused, as doctor
- 10:39Speaker 4: Henley and I were talking about before, if you were
- 10:42Speaker 4: to choose a different implant option, such as snap indentures
- 10:48Speaker 4: to implants. This is something we explain to patients upfront,
- 10:54Speaker 4: very very completely. We explain to them, but then sometimes
- 10:59Speaker 4: even after we do the procedure, they say, well, I
- 11:01Speaker 4: thought these teeth were going to stay on these implants today.
- 11:04Speaker 4: The issue is that when you take out teeth, put
- 11:06Speaker 4: in implants and then expect to snap a denture to them,
- 11:10Speaker 4: the implants that you put in that day are not
- 11:13Speaker 4: healed well enough to withstand that snapping on and off
- 11:16Speaker 4: pressure because you could yank those implants. That makes sense.
- 11:19Speaker 4: Whereas with the bridge that we talked about earlier, it's
- 11:21Speaker 4: all splintered together. It's kind of it's a bit of
- 11:24Speaker 4: engineering within dentistry that we use for that. It all
- 11:27Speaker 4: links it together in arch form and that can typically
- 11:30Speaker 4: withstand the chewing pressures during that healing period of time.
- 11:33Speaker 4: But if you choose to do the option of a
- 11:36Speaker 4: snap in denture, we have to basically put you into
- 11:38Speaker 4: that immediate denture that we talked about before while the
- 11:41Speaker 4: implants heal for twelve weeks. After twelve weeks we confirm
- 11:45Speaker 4: the implants have healed well enough to withstand those pressures
- 11:48Speaker 4: of snapping on and off, then we modified the denture.
- 11:51Speaker 4: We snap the implants to that denture. The denture to
- 11:54Speaker 4: the implants, I should say at that time.
- 11:56Speaker 3: And when you listen to the explanation here, it's obvious
- 12:01Speaker 3: that this is a complicated measure that you have to
- 12:04Speaker 3: put into place because of the knock on effect or
- 12:07Speaker 3: the domino effect. But once you remove something from your mouth,
- 12:09Speaker 3: this happens, This happens, this happens, and you have to
- 12:12Speaker 3: you have to make sure that as it's healing and
- 12:14Speaker 3: as it's getting into place, that it's going to heal
- 12:15Speaker 3: correctly so it can take the load of all of
- 12:17Speaker 3: these different things. And I can understand, you know, what
- 12:20Speaker 3: happens with a lot of people is they read or
- 12:22Speaker 3: hear something that they hope they understand, and they get
- 12:26Speaker 3: that in their mind and then regardless of what somebody
- 12:28Speaker 3: tells them. They're they're casting it in their mind to
- 12:31Speaker 3: fit within that box they've already kind of made, and
- 12:33Speaker 3: so it seems like that they're having everything confirmed to them,
- 12:35Speaker 3: which if they're actually listening, they would find out that
- 12:38Speaker 3: there was a different path there. That's really easy for
- 12:40Speaker 3: us to do, as you know, when we're not experts
- 12:42Speaker 3: in those situations and stuff like that.
- 12:43Speaker 4: So human nature, it really is you kind of fit
- 12:47Speaker 4: that make the information fit your reality in your in
- 12:50Speaker 4: your head. So it does happen. But we try to
- 12:52Speaker 4: show show models, show pictures, explain things as well as
- 12:56Speaker 4: we possibly can. But I mean, in dental implants, they're
- 12:59Speaker 4: they're kind of three levels of If we're talking about
- 13:02Speaker 4: comprehensive implant treatment to treat an entire mouth, you can
- 13:06Speaker 4: go from you know, the classic venture setup to using
- 13:11Speaker 4: implants to snap the dentures in somehow all the way
- 13:13Speaker 4: up to the bridge that stays in place. So it
- 13:16Speaker 4: depends on which category you want to go into it,
- 13:18Speaker 4: and a lot of times that that depends upon the
- 13:21Speaker 4: patient patient's financial ability as well.
- 13:23Speaker 2: And that makes sense.
- 13:24Speaker 3: So I mean, it's all about having a spectrum of
- 13:27Speaker 3: options that can be put in front of you, and
- 13:29Speaker 3: the doctors help you do that what you're willing and
- 13:31Speaker 3: able to do on yourself, and then what helps the
- 13:34Speaker 3: best for your dental hygiene and for your health and
- 13:37Speaker 3: things along those lines. You can address each of those
- 13:40Speaker 3: things in turn in the best possible way. But the
- 13:43Speaker 3: bottom line is, if you have a situation that you've
- 13:46Speaker 3: been trying to tackle for a while, or maybe you've
- 13:50Speaker 3: not found the right path, it's it's worth giving it
- 13:52Speaker 3: a shot to get consultation to see what that is.
- 13:55Speaker 3: Because technology has changed, different dental offices have different solutions
- 13:59Speaker 3: for so you can check those things out. If you

- 17:12Speaker 3: So we tease before the break that if you're looking
- 17:14Speaker 3: into your smile, how you can make your smile better? Uh,
- 17:18Speaker 3: And I think the first thing, and I know that
- 17:20Speaker 3: we've had this discussion before, so you have to set
- 17:23Speaker 3: the expectation on what a better smile is because as
- 17:26Speaker 3: doctor Steve, one of the first times that we that
- 17:28Speaker 3: we did a show we taught that it always stuck
- 17:30Speaker 3: in my head was talking about people with too white
- 17:32Speaker 3: a teeth, you know, and if they come in with
- 17:34Speaker 3: this expectation that they're they when they want to smile
- 17:36Speaker 3: and give off like radiation, you know, I guess that
- 17:39Speaker 3: if they demanded that, that you that you're there to
- 17:41Speaker 3: serve the patient ultimately, but you're gonna explain to folks
- 17:45Speaker 3: that there is such thing as a naturally white smile,
- 17:47Speaker 3: and there's this such thing as something that looks like
- 17:49Speaker 3: an action figure, and and they're there, you want to
- 17:51Speaker 3: hopefully go towards the natural look. We call it chick
- 17:55Speaker 3: lit tea chick lit that's what it was.
- 17:57Speaker 2: That's right. Yeah.
- 17:57Speaker 5: Yeah, So so when the patient wants the super vibrant
- 18:01Speaker 5: white I always double down and kind of double check
- 18:04Speaker 5: and kind of show them photos online and go, are
- 18:07Speaker 5: you sure right, because this is what you're gonna get
- 18:09Speaker 5: and if that's what they want, then we can provide it. However,
- 18:12Speaker 5: most patients go, oh yeah, I don't want that. I
- 18:15Speaker 5: want I want something like that. But not necessarily that
- 18:17Speaker 5: I want a little less vibrancy, Like I don't want
- 18:20Speaker 5: to be emanating the sun as I walk around Charleston.
- 18:23Speaker 2: Right.
- 18:24Speaker 5: So there is that idea that we want to provide
- 18:28Speaker 5: what's best for the patient, but sometimes the patient doesn't
- 18:31Speaker 5: necessarily always know what is best for themselves, right, So
- 18:34Speaker 5: we need to kind of walk them through that. I
- 18:36Speaker 5: always call myself the navigator of the situation, and they're
- 18:39Speaker 5: the driver. So if they want to go, they press
- 18:41Speaker 5: the gas pedal. If they want to stop, they press
- 18:43Speaker 5: the brake pedal, but they cannot go in any direction.
- 18:46Speaker 5: I don't guide them down. Now, there are multiple paths.
- 18:48Speaker 5: There's a journey, right, and so they get to pick
- 18:51Speaker 5: which path they choose, and there are so many different ways.
- 18:54Speaker 5: And what's really interesting is we might get halfway down
- 18:57Speaker 5: the treatment plan or we're doing something else, sudden they go, actually,
- 19:00Speaker 5: I want to shift. I want to change a little bit,
- 19:02Speaker 5: Like Okay, let's fine. As long as you understand that
- 19:04Speaker 5: we're going to have to do this and this to
- 19:06Speaker 5: change it, we might you know, increase the cost just
- 19:08Speaker 5: a little bit because they're doing this instead of that,
- 19:10Speaker 5: And so we really work well, I'm trying to stay
- 19:15Speaker 5: humble here, but I think we do a really good job,
- 19:17Speaker 5: especially doctor Stephen, walking through the patients. Our staff are
- 19:20Speaker 5: trained to make sure they understand fully what they're getting
- 19:23Speaker 5: themselves into. We always take photos beforehand. I take photos
- 19:27Speaker 5: after a lot as well, so I can show them
- 19:29Speaker 5: the before and after and hand them kind of a
- 19:31Speaker 5: piece of paper or I print out there before and
- 19:33Speaker 5: after and go congratulations, like this is a new day
- 19:37Speaker 5: for you. Doctor's orders go out there and smile, or
- 19:41Speaker 5: we take some digital impressions where they can actually take
- 19:44Speaker 5: a look at their smile with us. X rays are
- 19:47Speaker 5: important as well, so we take all of it and
- 19:50Speaker 5: kind of formulate it and try to cater to each
- 19:52Speaker 5: and every patient because if you just do a cookie
- 19:55Speaker 5: cutter treatment plan, they can read right through that, and
- 19:58Speaker 5: so we want to make sure that they know that
- 19:59Speaker 5: this is made for them, that they can choose what
- 20:02Speaker 5: they want, and then we can eventually give them the
- 20:05Speaker 5: smile they want or give them a better healthy smile.
- 20:08Speaker 5: And that's our goal.
- 20:09Speaker 3: The thing that strikes me, and with what you're talking
- 20:12Speaker 3: about here, with somebody whitening their teeth other cosmetic procedures,
- 20:15Speaker 3: they might get the caution reminds me a little bit
- 20:19Speaker 3: of like the lower around like genie wishes or something.
- 20:22Speaker 3: You know, commonly when somebody, if somebody gets tricked by
- 20:25Speaker 3: a genie into making oh, I want the whitest teeth possible. Oh,
- 20:28Speaker 3: you will have the whitest and then suddenly like they
- 20:30Speaker 3: can't eat anything because they have some surface on their
- 20:33Speaker 3: teeth that keeps them from consuming the food or whatever.
- 20:35Speaker 2: Right, but they have really white teeth.
- 20:36Speaker 3: You know, you want to be careful what you ask
- 20:38Speaker 3: for because it may not be exactly what you actually want.
- 20:41Speaker 3: And so consulting with professionals and consulting with your dentists
- 20:44Speaker 3: or something like that, we can give you perfectly naturally
- 20:47Speaker 3: white teeth and it will look wonderful. You don't want
- 20:49Speaker 3: these these chick lit teeth or whatever. And you can
- 20:51Speaker 3: help people along those lines and make sure that they
- 20:54Speaker 3: have a natural look about their mouth as you're doing
- 20:56Speaker 3: cosmatic stuff like and I think that's really important because
- 20:58Speaker 3: if somebody just walks in and they tell you top
- 21:01Speaker 3: line what they want, and somebody's like, okay, I'll give
- 21:03Speaker 3: that to you without any any exploratory questions at all,
- 21:07Speaker 3: the expectation and what is delivered might be different.
- 21:11Speaker 4: For sure, and you need to get your spouse on board.
- 21:14Speaker 2: Oh great question, great point.
- 21:17Speaker 4: Yeah, I hat this happened in multiple different ways where
- 21:20Speaker 4: you have one spouse come in and insist that they
- 21:23Speaker 4: want to have the brightest, whitest teeth in the world,
- 21:26Speaker 4: and so you deliver what the patient would want, and
- 21:30Speaker 4: then the next round of family photos comes up and
- 21:35Speaker 4: their teeth are much whiter than the rest of their family.
- 21:38Speaker 4: And so then either the rest of the family wants
- 21:41Speaker 4: to have whiter teeth or the spouse is like the
- 21:46Speaker 4: patient may come back and say, my teeth are too white.
- 21:48Speaker 4: You know, what are we going to So then that's
- 21:51Speaker 4: a different conversation, or like, my teeth I love them,
- 21:55Speaker 4: but my husband or my wife doesn't really, you know,
- 21:58Speaker 4: it's paling comparisons. So maybe the the husband or wife
- 22:00Speaker 4: has gets their teeth whiter as well, or it causes
- 22:04Speaker 4: marital strife. So it is an interesting thing. You got
- 22:07Speaker 4: to you gotta consider a lot of difference.
- 22:10Speaker 2: You've seen it all. That's very true.
- 22:11Speaker 3: As we talk about improving smiles, it's not, of course,
- 22:14Speaker 3: just about how how white your smile is. It's also
- 22:16Speaker 3: about the structure of the smile and your teeth and
- 22:18Speaker 3: things like that. So what's some of the other things
- 22:20Speaker 3: that somebody that's looking for a better smile quote unquote,
- 22:23Speaker 3: what are some of the other things they can look into,
- 22:24Speaker 3: So things.
- 22:25Speaker 4: That we can do to improve smile basic from the
- 22:28Speaker 4: bottom kind of the lowest type of treatment, the less
- 22:31Speaker 4: least type of treatment would be whitening your teeth. You know,
- 22:34Speaker 4: so if you have natural teeth that are a little dingy,
- 22:36Speaker 4: of course we can whiten them and brighten them up.
- 22:38Speaker 4: We can straighten them as well. That makes a big
- 22:40Speaker 4: difference for a lot of patients. Many times they just
- 22:43Speaker 4: our teeth are out of line and you can't clean
- 22:45Speaker 4: them as well, and they don't bleach quite as well.
- 22:47Speaker 4: So if we straighten them up and bleach a little
- 22:49Speaker 4: bit while they're straightening their teeth with something like in visiline,
- 22:52Speaker 4: then that works very well for the patient. Uh, those
- 22:55Speaker 4: are all very conservative. You're not damaging teeth in any way.
- 22:59Speaker 4: The next love would be cosmetic bonding. People call it
- 23:02Speaker 4: or tooth colored filling material we put on their composite
- 23:05Speaker 4: veneers we call them as well, few different words ways
- 23:08Speaker 4: to describe that. But where we are custom forming filling
- 23:12Speaker 4: material on top of the patient's tooth to change the
- 23:15Speaker 4: color and change the shape. So that's another option as well,
- 23:18Speaker 4: and that's very conservative as well. We generally just have
- 23:21Speaker 4: to roughen the tooth up a little bit to get
- 23:23Speaker 4: our filling material to stick. It doesn't, we can always
- 23:25Speaker 4: remove it, and it's very conservative type of treatment. Next
- 23:29Speaker 4: we go up from there to porcelain veneers, where we
- 23:32Speaker 4: do have to reduce the teeth a little bit, a
- 23:34Speaker 4: couple millimeters or fractions of millimeters and to make room
- 23:38Speaker 4: for porcelain to then be bonded onto the tooth structure.
- 23:42Speaker 4: The laboratory makes those outside of the mouth. It makes
- 23:44Speaker 4: them as ideal as possible and the color and the
- 23:46Speaker 4: shape that the patient would like, and then we put
- 23:49Speaker 4: the veneers on there. Then if the teeth are broken
- 23:52Speaker 4: down and need extra structure or amounts of the tooth
- 23:57Speaker 4: built back, then we could start talking about crown so
- 24:00Speaker 4: that cover the entire tooth. That could be a little
- 24:03Speaker 4: more aggressive, depending on the situation with the patient how
- 24:05Speaker 4: much tooth we'd have to remove, but that strengthens and
- 24:09Speaker 4: changes the shape of the teeth as well.
- 24:11Speaker 2: And then of.
- 24:11Speaker 4: Course bridging between teeth is an option, and dental implants
- 24:15Speaker 4: would be the final option if we were removing teeth
- 24:18Speaker 4: and then making an entire bridge or a set of dentures.
- 24:22Speaker 4: Everything that we do doctor Mitchrie from years ago to
- 24:24Speaker 4: always say all of our dentistry should be cosmetic dentistry.
- 24:27Speaker 4: We try to make every treatment modality look as good
- 24:31Speaker 4: as it possibly can for the patient.
- 24:33Speaker 3: That makes a lot of sense, and for a patient,
- 24:35Speaker 3: I'm sure that that like to hear that as well.
- 24:38Speaker 3: Not only are you doing the best possible thing for
- 24:40Speaker 3: the patient, but each time you want to make it
- 24:41Speaker 3: look as cosmetically whole as you possibly can, whatever that
- 24:45Speaker 3: means in the particular situation. And again, this is something
- 24:49Speaker 3: that if you have a general goal of hey, I
- 24:52Speaker 3: want a better smile, this is just one of those situations.
- 24:54Speaker 3: We're having a consultation with your dentist. It's going to
- 24:56Speaker 3: help you out a lot in that way, because that
- 24:57Speaker 3: can mean a lot of different things, almost positively mean
- 25:01Speaker 3: something different to each person. So it's good to talk
- 25:03Speaker 3: to your dentist and come up with a plan for
- 25:05Speaker 3: something along those lines, right, and.
- 25:07Speaker 5: Even getting a good cleaning, no joke, there is a
- 25:10Speaker 5: lot of stain out there that if you let one
- 25:12Speaker 5: of my lovely hygienists spend an hour with you, we
- 25:15Speaker 5: can get a lot of that stain off. We can
- 25:17Speaker 5: polish it off, and even if we can't get it
- 25:19Speaker 5: all off, we can do some simple feelings to kind
- 25:22Speaker 5: of make your teeth look even more vibrant. And so
- 25:26Speaker 5: before we've been whiten, let's clean right, let's make sure
- 25:28Speaker 5: that you have a nice healthy smiles. Yeah, but definitely,
- 25:33Speaker 5: I've had patients come in being like I have all this,
- 25:35Speaker 5: like my teeth are so yellow.
- 25:36Speaker 2: I sheed whitening.
- 25:37Speaker 5: I'm like, no, no, no, you need a cleaning and
- 25:39Speaker 5: so and then once that cleaning happens or go, you
- 25:41Speaker 5: were right, My teeth looks so much better now, right,
- 25:43Speaker 5: all that stuff that was on there, it's off, and
- 25:45Speaker 5: I'm like, yeah, let's keep it off, right, and so on.
- 25:48Speaker 5: We once we had that conversation, then we hopefully give
- 25:51Speaker 5: them the tools and techniques to keep it off, because
- 25:55Speaker 5: we're not going home with them the brush their teeth.
- 25:56Speaker 2: They need to do that at home.
- 25:57Speaker 3: When a hunder Great Care is a number for a
- 25:59Speaker 3: gree dental group, if you want to give them a call.

- 26:03Speaker 3: to the closest location to you. Pocus Al Charles and
- 26:04Speaker 3: Cross Lane, Saint Albans Tays Valley, Kanol City one eight hundred.
- 26:08Speaker 3: Great Care is a number of The web address is
- 26:10Speaker 3: GREEB Dentalgroup dot com. That g h A R e
- 26:12Speaker 3: e B GREEB Dentalgroup dot com. I want to talk
- 26:14Speaker 3: a little bit about the kiddos this morning as well.
- 26:16Speaker 3: And if I have an eight year old and so
- 26:18Speaker 3: this has been a constant uh uh. I won't call
- 26:21Speaker 3: it a battle, but uh my kid goes through through
- 26:24Speaker 3: phases with with brushing her teeth.
- 26:25Speaker 2: It's very strange.
- 26:26Speaker 3: Sometimes it's all about the competition and so you know,
- 26:29Speaker 3: we'll we'll rush to brush our teeth together and make
- 26:31Speaker 3: sure we get all the spot you know, like it
- 26:33Speaker 3: becomes a very healthy uh uh thing that we that
- 26:37Speaker 3: we do together. And then sometimes she just wants to
- 26:39Speaker 3: push back on it and she's like, I don't feel
- 26:40Speaker 3: like brushing my teeth tonight.
- 26:42Speaker 8: Uh.
- 26:42Speaker 3: We get past those things, but kids bring up a
- 26:45Speaker 3: very specific kind of challenge because for one thing, everything
- 26:49Speaker 3: for kids they put sugar in.
- 26:50Speaker 2: I don't know what to deal.
- 26:51Speaker 3: But everything we have she has to take daily allergy
- 26:55Speaker 3: medicine and she prefers the liquid over the over the
- 26:57Speaker 3: pills and she takes it every day, which is fine.
- 27:00Speaker 3: Has tons of sugar in it, I mean just the
- 27:01Speaker 3: just just a stinking yeah, of course, but tons of
- 27:04Speaker 3: sugar in and not just a little bit, tons of sugar.
- 27:06Speaker 3: And so that's one of the one of the the
- 27:09Speaker 3: balancing ocs and one of the challenges for your kids
- 27:11Speaker 3: dental hatching. You have to really sort of evaluate what
- 27:14Speaker 3: they eat because they don't they don't know any better their.
- 27:16Speaker 4: Kids, Yeah, for sure, so but they should, right, come on,
- 27:20Speaker 4: kids pick it up. But yeah, I mean that's a
- 27:23Speaker 4: that's a common common issue, and we have patients to
- 27:25Speaker 4: come in and bring there first of all, let's say
- 27:27Speaker 4: it's worth seeing that the ADA and we recommend your
- 27:31Speaker 4: children come in. When do they come into the dentists first?
- 27:33Speaker 4: When they have to get their get their first tooth.
- 27:35Speaker 4: Now that's could be a six months of age, which
- 27:39Speaker 4: we're not going to bring them in and sit them
- 27:40Speaker 4: down for a full hour and you know, take all
- 27:42Speaker 4: this time and wear them out, you know, just to
- 27:45Speaker 4: worry about a couple of teeth. But it's about getting
- 27:47Speaker 4: them used to the environment, getting used to the sounds
- 27:50Speaker 4: and smells and sensations of getting your teeth clean and
- 27:53Speaker 4: coming to the dentist. So we want to start that
- 27:55Speaker 4: good experience early. But patients that their children in maybe
- 28:02Speaker 4: when it's later they feel like they've been brushing their
- 28:04Speaker 4: teeth well enough and regularly and doing all of the
- 28:08Speaker 4: right things, and they come in and sometimes we say,
- 28:10Speaker 4: you know that your child has a lot of cavities
- 28:12Speaker 4: in here, and that isn't necessarily due to a lack
- 28:17Speaker 4: of proper home care. Because teeth have these little cracks
- 28:22Speaker 4: in them. Everyone's open the mouth and looked and seeing
- 28:24Speaker 4: these little cracks on the chewing surfaces of their teeth. Well,
- 28:27Speaker 4: those little fissures we call them. If you look at
- 28:30Speaker 4: them under a very high intensity microscope, the toothbrush bristle
- 28:35Speaker 4: has no chance of getting down to the bottom of
- 28:37Speaker 4: these grooves in the teeth. So what can happen is
- 28:41Speaker 4: if you're not having regular dental checkups and it's been
- 28:44Speaker 4: a long time since your child's been to the dentist,
- 28:46Speaker 4: it's not uncommon for cavities to have settled into those
- 28:50Speaker 4: cracks of the teeth and then grow kind of silently
- 28:54Speaker 4: and then when they come in, we have to do
- 28:56Speaker 4: some fillings to get those out of there, no matter
- 28:58Speaker 4: what type of home care that you have. Now, also
- 29:00Speaker 4: in between the teeth where the teeth touch, if your
- 29:03Speaker 4: child does have the habit of drinking a lot of juice,
- 29:07Speaker 4: you think, oh, juice is good. You know it's natural
- 29:09Speaker 4: and all this. Well, juice has a lot of sugar
- 29:11Speaker 4: in it. Sticky candies of course as well, will do
- 29:14Speaker 4: that and things the kids love to eat. But that
- 29:18Speaker 4: can start cavites in between the teeth very easily, and
- 29:21Speaker 4: those can go hidden for a long time as well
- 29:22Speaker 4: until we take our X rays and we can use
- 29:25Speaker 4: our X ray vision to see between the teeth and
- 29:28Speaker 4: see that well, there are cavities here starting as well.
- 29:30Speaker 4: So all of this just to say that no matter
- 29:34Speaker 4: how well you do on your own at home, cavities
- 29:37Speaker 4: can still grow very very silently in your child's mouth
- 29:40Speaker 4: without the proper intervention. That's where things like sealants come in,
- 29:44Speaker 4: fluoride treatments come in to help strengthen those teeth as well.
- 29:48Speaker 5: Yeah, and this is the PSA for all those parents
- 29:50Speaker 5: out there. Doctor Miranda Kulaski has actually mentioned that one
- 29:54Speaker 5: of the worst types of snacks to chew on is goldfish,
- 29:59Speaker 5: which I was not super aware of.
- 30:01Speaker 2: I kind of knew that.
- 30:02Speaker 5: It was bad because you know, it's literally made of carbohydrates,
- 30:05Speaker 5: which is sugar. But whenever you chew it, especially if
- 30:08Speaker 5: you if these little kids are chewing one at a
- 30:10Speaker 5: time slowly and then they just keep popping them in.
- 30:13Speaker 5: It's kind of like a candy that you just keep eating,
- 30:16Speaker 5: and once it's crushed in my microscopically sized, it just
- 30:20Speaker 5: sticks into those fissures of the teeth and it's very
- 30:23Speaker 5: hard to remove unless you brush immediately. So she's actually
- 30:26Speaker 5: seen so many patients with cavities and the number one
- 30:30Speaker 5: reason is what she can consider why it's happening.
- 30:34Speaker 2: It's goldfish. Wow. Wow. Does that go for cheese its too?
- 30:40Speaker 4: Probably would do a similar thing.
- 30:42Speaker 3: Because we kind of switched from goldfish to to cheese
- 30:45Speaker 3: its in our family.
- 30:46Speaker 2: But I guess we have all had the burnt cheese its. Yes,
- 30:51Speaker 2: toasted are particularly good. Yeah, I'm kind of a cheese
- 30:56Speaker 2: at mark. I'm kind of a big fan of the
- 30:58Speaker 2: cheese its. Yeah, my kid is too.
- 31:00Speaker 3: Now I'm like, well, I've screwed up your teeth forever.
- 31:03Speaker 3: And because I haven't give you a cheese.
- 31:04Speaker 2: Ites and moderation.
- 31:05Speaker 4: Yeah, as doctor Hanley saying, it's it's about that constant
- 31:08Speaker 4: chronic exposure. You got to have your give your mouth
- 31:11Speaker 4: a break to balance things out. But cheese, it's goldfish
- 31:16Speaker 4: if you're if you're just snacking on them all day long,
- 31:18Speaker 4: that's that's just as well as chewing on you know,
- 31:21Speaker 4: candy all day long as well.
- 31:23Speaker 3: Since we are in kind of the back to school
- 31:24Speaker 3: era right now, let's talk a little bit about that
- 31:26Speaker 3: and what kids can eat kind of as a neutral
- 31:29Speaker 3: snack or at least not that bad of a snack
- 31:31Speaker 3: if you send them to school with something, or if
- 31:32Speaker 3: they have a choice to get something while they're at school,
- 31:34Speaker 3: or something like that. Here at work, it's relatively easy.
- 31:36Speaker 3: I can bring I love baby carrots. I can bring
- 31:38Speaker 3: baby carrots to work and keeping them fridge. I don't
- 31:40Speaker 3: like them room temperature. But if they're cold, they're snappy,
- 31:42Speaker 3: they're good. They give you the crunch and all those things.
- 31:45Speaker 3: Maybe in your lunchbox, those would survive, but also maybe not.
- 31:48Speaker 3: And once they get a little warm or whatever, they're
- 31:50Speaker 3: not going to be all that great. What's some good
- 31:52Speaker 3: things that that kids can can snack on, that you
- 31:54Speaker 3: can give them not only at school, but around the
- 31:56Speaker 3: house or something along.
- 31:57Speaker 4: The thing that I want to drive home is about drinks,
- 31:59Speaker 4: that's all sure you kids now they're sipping on energy
- 32:02Speaker 4: drinks all day.
- 32:03Speaker 2: Long, or so much sugar, so much sugar.
- 32:05Speaker 4: Sugar and acids on there as well. And again it's
- 32:07Speaker 4: the chronic exposure. I tell my patients, you know, hey,
- 32:09Speaker 4: I'm not going to tell you can never have a coke,
- 32:12Speaker 4: but have it at lunch and be done, don't. I mean,
- 32:14Speaker 4: so many come in sipping on mountain dew all day long.
- 32:16Speaker 4: Well maybe in Appalachia we call that mountain dew mouth,
- 32:19Speaker 4: and it just wrecks the teeth. So you have to
- 32:22Speaker 4: have the limit the exposure, whether it's a snack or
- 32:25Speaker 4: a drink of some sort. So for of course, drinks
- 32:29Speaker 4: safe safe things to drink. For staining, I talk about
- 32:33Speaker 4: drinking things clear. You know, water of course is going
- 32:35Speaker 4: to be the best thing, but but there are many
- 32:37Speaker 4: drinks out there that are clear, drinks that aren't going
- 32:39Speaker 4: to stain your teeth as well cosmetically. But then for
- 32:43Speaker 4: for sugars, again limit the time of the exposure. But
- 32:48Speaker 4: you can have something that has some sugar and acid
- 32:51Speaker 4: in it, but give your mouth some time to balance
- 32:53Speaker 4: out any other.
- 32:55Speaker 3: My clear liquid can't be sprite right, and it's and
- 32:59Speaker 3: be done, you know, but that's don't drink it all
- 33:02Speaker 3: day long.
- 33:02Speaker 7: Well.
- 33:02Speaker 3: One thing that that that has really helped us out
- 33:05Speaker 3: a lot is that Caprice on of all things have
- 33:08Speaker 3: come out with. They're just flavored water instead of the
- 33:10Speaker 3: juice pouches. They just it's just flavored water. It's just
- 33:12Speaker 3: like if you had the additive that you get from
- 33:14Speaker 3: the little pouch and drop into your water, so all
- 33:15Speaker 3: that it had there's nothing in it. There's no sugar
- 33:17Speaker 3: in it, nothing at all. You can get them with
- 33:18Speaker 3: electrolytes if you really want it, but you don't even
- 33:20Speaker 3: need that. My daughter loves those and so she doesn't
- 33:23Speaker 3: even actually drink them all out of the pouch. Now
- 33:24Speaker 3: I'll pour them into because they're the only kids version
- 33:26Speaker 3: of that that I can find. They're not that expensive.
- 33:28Speaker 3: She loves those, and it has helped, Like I can
- 33:30Speaker 3: visually tell that it has helped her because she's she
- 33:33Speaker 3: basically doesn't drink anything that sugar anymore. She almost exclusively
- 33:36Speaker 3: drinks water.
- 33:37Speaker 5: Frankly, I would say the only downside of that, most
- 33:39Speaker 5: likely is that it's an acidic liquid a little bit, right,
- 33:43Speaker 5: But obviously almost everything we eat is not going to
- 33:45Speaker 5: be a pH of seven or neutral. It's going to
- 33:48Speaker 5: be a little basic, a little app acidic.
- 33:50Speaker 2: Right.
- 33:51Speaker 5: I just had some sparkling water. You know what actually
- 33:54Speaker 5: carbonates it? Carbonic acid. Yeah, Like we're always drinking a
- 33:58Speaker 5: little bit of acid. But as long as you or
- 34:00Speaker 5: doing it not all day, you're you're you're you're limiting
- 34:03Speaker 5: your your exposure to it. I think I think it
- 34:06Speaker 5: can be safe. But I didn't know about that Caprice
- 34:08Speaker 5: sunthing that that's really cool and you look into.
- 34:10Speaker 3: It what we discovered, and it's really been something because
- 34:12Speaker 3: she she loves others. She likes milk, and she just
- 34:14Speaker 3: does drink water and different things like that. But her
- 34:16Speaker 3: big thing was like just like like pouches of juice.
- 34:20Speaker 3: She loved pouches of juice because they were really easy
- 34:21Speaker 3: and it was just the convenience of it. Once we
- 34:23Speaker 3: gave her the convenience of water in that same pouch,
- 34:26Speaker 3: she she loves it. It's fine, And it wasn't the
- 34:28Speaker 3: sugar so much. A's just the convenience.
- 34:30Speaker 5: I have a funny story about that. This is not
- 34:32Speaker 5: about kids, so kids don't tell about an adult. She
- 34:35Speaker 5: came to me and said, I love to drink wine,
- 34:37Speaker 5: but it's staining my teeth.
- 34:38Speaker 2: What can I do.
- 34:39Speaker 5: I'm like, don't drink wine. And then she goes, you
- 34:41Speaker 5: know what, I'm gonna keep drinking wine, but I use
- 34:43Speaker 5: a straw, and I'm like, go for it. Trying to
- 34:46Speaker 5: pass it by the.
- 34:47Speaker 4: Teeth some people, some people have success with that with
- 34:50Speaker 4: coffee as well, you know, and they drink coffee through
- 34:52Speaker 4: a strata avoids staining and oh weird. All right, all right,
- 34:56Speaker 4: but healthy snacks, uh, not the not to avoid that
- 35:01Speaker 4: as much. But cheeses, yogurts, those are all healthy things
- 35:03Speaker 4: that can help apps, all right, of course, fruits.
- 35:06Speaker 3: And vegetables, that's all great stuff. GREEB Dental Group, one line,
