- 0:00Hello, and welcome to the Becker's Healthcare podcast.
- 0:02My name is Chanel Bongern. Today, I have
- 0:04the pleasure of speaking with Charles Ross,
- 0:06a chief strategy officer and partner at TCM
- 0:09Consulting and Management, who joins the podcast today
- 0:12to share insights to his background, Trancy is
- 0:14keeping an eye on a bit more. Charles,
- 0:15thank you so much for joining me today.
- 0:18Thank you. Glad to be here.
- 0:20Perfect. Well, to get us started out, could
- 0:21you please introduce yourself and tell us a
- 0:23bit about your background in TCM?
- 0:25Absolutely. Thank you. I am Charles
- 0:28Ross. I'm the chief strategy officer with TCM
- 0:31Consulting and Management.
- 0:32I actually began my career as a hospital
- 0:36discharge planner,
- 0:38back in the nineteen nineties. And
- 0:41the unit I worked on was a geriatric
- 0:43unit, and many of those
- 0:45patients
- 0:46were needing,
- 0:47skilled nursing
- 0:49home care post discharge. So I got, an
- 0:52introduction
- 0:53to that as a professional from the hospital
- 0:56side. And then eventually, I went on to
- 0:58work for
- 0:59a not for profit hospital owned skilled nursing
- 1:03facility
- 1:04in a case management role and an operations
- 1:06role and a marketing role. So those those
- 1:09sort of combined to bring me to
- 1:12my position now in strategy. I eventually worked
- 1:15my way up to,
- 1:16become a nursing home administrator.
- 1:19A great position.
- 1:21Learned a lot doing that as well.
- 1:24And about fifteen years ago,
- 1:27TCM was founded.
- 1:29It was actually founded
- 1:31originally as transitional
- 1:33care management, and we,
- 1:35use TCM, obviously, as the easy shortcut.
- 1:38But over time, we evolved and the company
- 1:41evolved.
- 1:43And over those fifteen years, we really have
- 1:45the opportunity
- 1:46as a company to do a lot. And
- 1:49I think that's what's kept us relevant today
- 1:52is doing, you know, focusing on different areas
- 1:54and doing different things.
- 1:56We originally
- 1:57were focused on building state of the art
- 1:59post acute freestanding
- 2:01transitional
- 2:02care centers
- 2:03and becoming a full service third party management
- 2:06company for those and other centers.
- 2:09We still do,
- 2:11full service third party management
- 2:14and that's still part of our business. But
- 2:16along the way,
- 2:17we have run a home health care company.
- 2:20We've managed a hospice company.
- 2:23We've helped design
- 2:24newly built, nursing homes and transitional care centers.
- 2:28And we've we've been and are currently a
- 2:30third party management
- 2:32company for,
- 2:34skilled nursing homes,
- 2:35intermediate care centers,
- 2:37behavioral
- 2:38health care
- 2:39centers,
- 2:40and even a specialized mental health,
- 2:43rehab center.
- 2:44And we've managed,
- 2:46both for profit and, not for profit
- 2:49entities.
- 2:50So we we have experience with both.
- 2:53We've also
- 2:54consulted
- 2:55to hospitals on their skilled nursing strategy
- 2:59and consulted specifically around creating skilled nursing units,
- 3:03with acute care and helping hospitals see if
- 3:07that's
- 3:08a good, fit for them.
- 3:10And we do quite a bit of clinical
- 3:13consulting
- 3:14nationally
- 3:15for skilled nursing, for assisted livings,
- 3:18senior livings. And we found a real sweet
- 3:21spot with, continuing care retirement communities that have
- 3:24skilled nursing units because,
- 3:27skilled nursing,
- 3:29has certainly a different set of demands from
- 3:31the rest of senior living products and a
- 3:34high level of regulation
- 3:36and a high level of clinical focus. So
- 3:39often groups will bring us in to help,
- 3:42look at their clinical needs for their CCRC
- 3:45skilled nursing units, but also how to integrate
- 3:48those skilled nurse units programs successfully
- 3:51with the rest of the CCRC.
- 3:54And we do back of the house full
- 3:56service financial management for senior livings, for assisted
- 4:00livings,
- 4:01and for nursing homes. So we've been able
- 4:04to
- 4:05become over the last, fifteen years,
- 4:08very versatile. I'm really,
- 4:10proud of that. And we've been able to,
- 4:12in some cases, anticipate
- 4:14the evolving market and other cases diversify
- 4:17because,
- 4:18you know, a need suddenly emerged and clients
- 4:21and customers were, you know, experiencing
- 4:24need and we were able to fulfill those
- 4:26needs, particularly with a lot of the financial
- 4:28work that we do as a company. We
- 4:30find that we can only get a relief
- 4:32for,
- 4:33clients who do front of the house. You
- 4:36know, they do an excellent job, but perhaps
- 4:38they need some help with finance and billing
- 4:42or what we're doing for one client, create
- 4:44a plan where we can transition them from
- 4:46us helping them with the back office to
- 4:48them hiring their own team and doing it
- 4:51themselves.
- 4:52I our biggest,
- 4:54divisions right now, in addition to the full
- 4:56service management, would be that back of the
- 4:59house financial management
- 5:01and our clinical consulting division. And then we
- 5:04also do, recruitment as well.
- 5:06And that's that's pretty much the summary of
- 5:09where we are as a company today.
- 5:11Wow. That's amazing. It sounds like your team
- 5:13is involved in a wide variety of efforts.
- 5:16And with that, I'm curious to hear what
- 5:18trends are you watching in health care currently?
- 5:20Oh, gosh. There there's so much and there's
- 5:22been so much that happened
- 5:24post COVID, but I do think
- 5:27there's still and and some of it's still
- 5:29lingering,
- 5:30significantly. I think there's still a post COVID
- 5:32workforce pressure,
- 5:34which particularly puts pressure on
- 5:37positions
- 5:38in nursing homes and senior living. And, and
- 5:41when we're looking for direct caregivers, there's definitely,
- 5:45that pressure that we and our clients are
- 5:47dealing with.
- 5:48Also, nationally, in the last ten years, there's
- 5:51been about a five percent rate of nursing
- 5:53home closing.
- 5:54And I'd say that that's not the big
- 5:57squeeze, but the squeeze is is less about
- 5:59closing, but more about,
- 6:01facilities that are left. How are they performing?
- 6:04There is a, you know, certainly a trend
- 6:08for people to find,
- 6:10alternatives
- 6:11to nursing home care. So what you have
- 6:13is typically,
- 6:14you know, those with the highest acuity or
- 6:16needing the most care will end up in
- 6:19nursing homes, and then there continue to be
- 6:21more and more options for those who may
- 6:23not need a nursing home level of care.
- 6:27Right now,
- 6:28about ten thousand people are turning 65
- 6:31each day,
- 6:32which will take us from sixty three million
- 6:35seniors in The US
- 6:36to something like seventy three million in 2030
- 6:39and
- 6:40ninety five million by some predictions in 2060.
- 6:44So the senior tsunami or silver tsunami is
- 6:47still at play,
- 6:48but there are more and more options for
- 6:50those people.
- 6:52I'd say additional trends
- 6:54would have some more specifically would to do
- 6:56with skilled nursing operators
- 6:58operating on very slim margins.
- 7:00And one of the way they often react
- 7:03to that is have leadership positions covering multiple
- 7:06roles
- 7:07and maybe fewer specialists and more generalists.
- 7:10That's often where we come come in hand,
- 7:13for our clients as we can provide some
- 7:15of that specialization
- 7:16that they may not have in house, particularly,
- 7:19let's say, with the clinical needs they might
- 7:22have.
- 7:23And all this does create clinical needs. We
- 7:26see increased needs for clinical training,
- 7:29increased needs around
- 7:31AR and revenue cycle management.
- 7:33Oh, and I there's one trend I have
- 7:35to mention that's sort of a trend in
- 7:36every industry now, and that is, AI.
- 7:39We have seen it really help and improve
- 7:42some processes.
- 7:43We're actually using an AI system
- 7:45with our admissions process,
- 7:47in the the buildings where we are a
- 7:50third party manager
- 7:51and seeing some some it being very, very
- 7:54helpful.
- 7:55And it does take pressure off in some
- 7:57areas, but I think we all have to
- 7:58watch that trend. Be careful. Don't become overreliant
- 8:02because we've seen many examples in our own
- 8:05work where,
- 8:06AI's answer isn't necessarily the end. It's a
- 8:09great tool, a great way to help process
- 8:11things, but I don't think we're at a
- 8:13point where we can rely on as overly
- 8:15heavily yet.
- 8:18Got it. Got it. Thank you so much
- 8:19for walking us through that. And now it's
- 8:21hard to believe that we're about a quarter
- 8:22of the way through 2026.
- 8:24And I'm curious to know, what are you
- 8:26most focused on and excited about going into
- 8:28the rest of 2026?
- 8:31I know. Time is is really flying.
- 8:34You know what's very interesting about the work
- 8:36we're doing in the post acute space and
- 8:38working with hospitals
- 8:39and and seeing some increased interest in,
- 8:43working with post acute, providers in a different
- 8:46way is back in, 2014, September 2014,
- 8:51myself and our chief clinical officer at the
- 8:53time wrote an article for Becker's, 10 things
- 8:56hospitals should know about their post acute providers.
- 8:59So it's kinda fun to be,
- 9:02still talking to hospitals and seeing how we
- 9:05we've evolved over the last decade.
- 9:07And so I've been really excited about all
- 9:11things TCM consulting and management that are
- 9:14kinda have moved away from our,
- 9:17traditional
- 9:18nursing home, you know, or as post acute
- 9:20care only
- 9:21focus, when you're really just looking at buildings,
- 9:25you know, specific nursing home buildings. And I'm
- 9:27really excited
- 9:29about how we're reengaging with acute care,
- 9:33reengaging with providers across the continuum.
- 9:37One project that we've seen increased interest, really
- 9:40starting the end of last year and this
- 9:42year
- 9:43is hospitals
- 9:44willing again to look at how skilled nursing
- 9:48fits with their strategy.
- 9:49And that goes beyond,
- 9:52what's been in play for many years, which
- 9:54is a hospital's,
- 9:55post acute network where they are engaged with
- 9:59community providers,
- 10:00but now actually looking inward
- 10:02at their current acute care beds and utilization
- 10:06and exploring why we are not having hospital,
- 10:10skilled nursing units would fit their strategy.
- 10:13And we really love,
- 10:15helping hospitals do that that assessment.
- 10:18There's, been a couple of systems, one in
- 10:21Illinois
- 10:22and, one in Indiana where we were,
- 10:25part of helping them
- 10:27explore whether or not having
- 10:30or keeping a hospital skilled nursing unit would
- 10:32fit with their long term strategy.
- 10:35We currently have a couple of other
- 10:37hospitals engaged in those discussions right now. So
- 10:40I'm really excited,
- 10:42to see where, that will go for us.
- 10:46Exciting stuff. And now I could tell in
- 10:49this short conversation that you're very driven and
- 10:52impactful in what you do. And with that,
- 10:54I'm curious to know what advice you would
- 10:55give to evolving leaders looking to have that
- 10:57same impact and drive in their careers as
- 10:59you have.
- 11:00Oh, great question.
- 11:02I would say, number one, be flexible.
- 11:06You survive by moving,
- 11:08being in a position to anticipate
- 11:11your clients' needs and looking at trends, but
- 11:14just definitely being flexible,
- 11:17being versatile.
- 11:18Really
- 11:19look number one at how to help
- 11:22those that you serve, whether it be the
- 11:24residents in a in a nursing facility
- 11:27or an investor who
- 11:29owns, you know, the business you're helping manage,
- 11:32but really,
- 11:33really get to the heart of of what
- 11:35your customer
- 11:36wants and look for win wins.
- 11:39And don't assume that others understand what the
- 11:41win win is. Sometimes you have to educate
- 11:44them.
- 11:44For example,
- 11:46a senior living
- 11:47community
- 11:48might just get excellent reviews, provide excellent care,
- 11:52have a fantastic,
- 11:54occupancy rate, doing very well. But if they're
- 11:58failing with, collecting the income, they're not gonna
- 12:01be able to provide the services they want
- 12:03to their their clients. So that's an idea
- 12:06where where our company can come in sometimes
- 12:08and create a win win
- 12:10for the business, and and everyone,
- 12:13does well. But I would say definitely also
- 12:16find a need, fill that need,
- 12:18don't be afraid to do something outside of
- 12:21your core competency
- 12:23because you can learn it, your team can
- 12:25learn it. And as you heard as my
- 12:27example of TCM consulting and management, what we've
- 12:30done over the last fifteen years, you know,
- 12:32there's been a wide breadth
- 12:34of work we've done. Some of it we
- 12:36continue to do. Some of it has evolved,
- 12:38you know, into different work. And that is
- 12:40what keeps things fresh and exciting.
- 12:42It's kinda funny. I started
- 12:44my career, you know, I told you about
- 12:46my first jobs, but I look back and
- 12:48at, age of nine, I was visiting my
- 12:51grandmother
- 12:52in a nursing home in rural East Texas,
- 12:55and we would make regular visits.
- 12:57And at that age, I kind of found
- 12:58it fascinating. I actually enjoyed,
- 13:01the visits and the people I met. And
- 13:03I wish I had, you know, video from
- 13:06those days and could compare
- 13:08where skilled nursing has, you know, has gone
- 13:11over the the decades because we've evolved
- 13:14so, so much. So, keep evolving. That's probably
- 13:17the best way to sum up my advice.
- 13:20I love it. That's excellent advice. Well, Charles,
- 13:22I wanna thank you for your time today.
- 13:24But before I let you go, is there
- 13:25anything else that listeners should know?
- 13:28I'd love if you,
- 13:30would reach out and follow us on LinkedIn,
- 13:33TCM Consulting and Management.
- 13:35We have a a LinkedIn page with, frequent
- 13:38posts. And you can also find us via
- 13:40our website,

- 13:48And thank you so much,
- 13:49for the time to speak with you today.
- 13:52Thank you as well. And I wanna thank
- 13:54you once again for joining me today and
- 13:56for sharing your insights on the Becker's Healthcare
- 13:58podcast. Thank you so much. Thank you.
