- 1:05- revenue officer at Sinai Chicago, which includes four
- 1:08- hospitals,
- 1:08- several community clinics, and a nationally recognized research
- 1:12- institute
- 1:12- serving Chicago's West And Southwest Sides. Thomasina, it's
- 1:16- a pleasure to have you on the podcast
- 1:17- today.
- 1:18- Thank you for having me. Before we begin,
- 1:20- can you tell us a little bit about
- 1:22- your role and your background?
- 1:25- Yes. Absolutely.
- 1:26- So my name is Thomasina Wilkins. I'm also
- 1:29- known as TW around the country.
- 1:32- I have over twenty five years of experience.
- 1:34- I stopped counting after twenty five.
- 1:38- I was a subject matter expert in, 20
- 1:40- of the 50 states, including the state of
- 1:43- Illinois as well as internationally as well.
- 1:46- I am also an author of a substantial
- 1:50- amount of operating policies and procedures for health
- 1:54- care institutions,
- 1:56- which include medical academic centers,
- 1:59- physicians' offices.
- 2:01- And I was also recently named, one of
- 2:04- the top,
- 2:06- women in health IT by Becker's Healthcare for
- 2:09- 2026.
- 2:11- My role specifically,
- 2:13- at Sinai Chicago,
- 2:15- I oversee accounts receivable,
- 2:17- clinical documentation
- 2:18- integrity,
- 2:19- patient financial services,
- 2:22- and a host of different areas.
- 2:25- So I am cross functional,
- 2:27- financial, clinical, as well as IT.
- 2:30- And on a day to day basis, I
- 2:32- make sure that
- 2:34- all of our,
- 2:35- financial workflows,
- 2:37- are collaboratively,
- 2:39- done with our clinical,
- 2:41- team members,
- 2:42- to ensure white glove service as well as
- 2:45- excellence in care.
- 2:48- Well, it's great to have you joining us
- 2:49- today.
- 2:50- And so to start us off, I want
- 2:53- to,
- 2:54- see if you could give us a state
- 2:55- of the union. What has gone well this
- 2:57- year, and what is... What are you focused
- 2:59- on for the rest of 2026?
- 3:02- Absolutely.
- 3:03- So at Sinai Chicago,
- 3:05- we are definitely
- 3:06- mission driven,
- 3:07- and we are doing an absolutely amazing job,
- 3:10- and we are blazing the trail.
- 3:12- Mission and service to the community has increased.
- 3:16- Sinai Chicago is limit... Eliminating the barriers to
- 3:20- health disparities
- 3:21- one day at a time, one patient at
- 3:23- a time.
- 3:26- It's it's not just patient care. It's personal
- 3:29- for us.
- 3:30- We are highly established in a community, and
- 3:33- our mission driven service
- 3:35- will continue
- 3:37- throughout 2026.
- 3:38- And that's what we're focused on,
- 3:40- reducing disparities,
- 3:42- making sure that our patients have access to
- 3:44- care.
- 3:45- And collaboratively,
- 3:46- even with other health care institutions, even in
- 3:49- the neighborhood,
- 3:50- working together to ensure a healthy community.
- 3:53- And so what are the biggest headwinds that
- 3:55- you're keeping an eye on right now in
- 3:57- the industry?
- 3:59- Okay. So Sinai Chicago is one of the
- 4:02- largest
- 4:03- private, not for profit
- 4:05- hospital... Safety net hospitals.
- 4:08- So the number one thing that we're looking
- 4:10- at is HR one, the big beautiful bill
- 4:13- and the major Medicaid changes that are occurring
- 4:16- in 2027.
- 4:18- Why?
- 4:19- Because our highest,
- 4:21- patient demographic is our Medicaid recipients.
- 4:25- We have some patients who have to choose
- 4:27- between
- 4:28- their meals,
- 4:30- prescription drugs,
- 4:32- paying co pays, deductibles.
- 4:34- So HR one will have a major impact
- 4:38- in terms of the redeterminations,
- 4:40- the work requirements,
- 4:42- the administrative burdens, not only just for the
- 4:45- state, but also for our patients as well.
- 4:48- In our community, one of the barriers that
- 4:50- we have is access to technology.
- 4:53- So
- 4:54- we are heavily looking at that as to
- 4:57- how we can engage our community, how we
- 5:00- can continue to eliminate those barriers,
- 5:03- and that is what we are preparing for.
- 5:06- More than 60%
- 5:07- of our client base or patient base is
- 5:10- Medicaid dependent.
- 5:12- So our community health care workers are being
- 5:15- aligned to be able to get out into
- 5:17- the community
- 5:18- to
- 5:19- provide that service to our patients,
- 5:22- specifically
- 5:23- at their homes and in the community.
- 5:25- Our financial counselors are working to be able
- 5:28- to train in the understanding of what does
- 5:31- this mean for our patient,
- 5:33- community
- 5:34- as well as what paperwork do they need,
- 5:36- what are the requirements,
- 5:37- as... And we're also ramping up on making
- 5:40- sure that we are ready for those patients
- 5:43- who might need
- 5:44- alternative funding, whether that be
- 5:47- financial
- 5:49- assistance, you know, or charity care as well.
- 5:51- So we are looking at HR one. We
- 5:54- are looking at
- 5:55- the,
- 5:57- changes in Medicaid,
- 5:58- and we are ramping up for that to
- 6:00- make sure that we are able to provide
- 6:03- services to our patients
- 6:05- as well as that stellar,
- 6:07- service to our community.
- 6:09- Right now, we're already doing that day by
- 6:11- day,
- 6:12- and, we wanna be able to continue
- 6:15- that high quality of care and service throughout
- 6:18- 2026
- 6:19- and 2027
- 6:20- as well.
- 6:21- Yeah. So, obviously, certainly, big challenge ahead, but
- 6:24- on the flip side of that, what are
- 6:25- you excited about going into 2027?
- 6:29- What am I personally excited about for 2027?
- 6:32- I will tell you that I am excited
- 6:35- about
- 6:36- AI.
- 6:37- I am excited about artificial intelligence,
- 6:41- specifically in health care.
- 6:44- This is one of the things that Sinai
- 6:45- Chicago is blazing the trail doing.
- 6:48- We are at aligning our people, our processes,
- 6:52- as well as our technology, and our vendors
- 6:54- as well. So we're looking at our tech
- 6:56- stack too
- 6:57- and seeing how we can advance to be
- 6:59- able
- 7:00- to advance our clinical care,
- 7:03- our operational
- 7:04- support, as well as our patient financial services
- 7:07- engagements as well, not only on-site, but in
- 7:09- the community as well. So I am excited
- 7:12- about this
- 7:13- because I know that where we are now
- 7:17- is where we are, but where we will
- 7:19- be in 2027
- 7:21- is even better. And why?
- 7:23- Because we're looking at how we can improve
- 7:26- every single day
- 7:28- and recalibrating
- 7:29- that in our people processes as well as
- 7:32- technology
- 7:34- and our service in clinical care
- 7:37- each day.
- 7:38- And I guess on a similar note, where
- 7:41- do you see the best opportunities for growth
- 7:43- over the next two to three years?
- 7:45- So over the next two to three years,
- 7:48- the best opportunities
- 7:49- for growth
- 7:51- is to be able to advance in your
- 7:54- technologies and your tool sets that you're being
- 7:56- able to
- 7:57- to, utilize right now. So growth in terms
- 8:01- of what can we offer the community in
- 8:03- terms of financial assistance.
- 8:05- I understand and know that there's gonna be
- 8:08- high deductibles. There's gonna be our patients who
- 8:11- don't have access to the care deductibles.
- 8:14- So I see that the level of partnership
- 8:18- with our vendors,
- 8:19- the level of partnership
- 8:21- with our senators and
- 8:23- our,
- 8:24- political,
- 8:26- community
- 8:27- is gonna be a well...
- 8:30- It's it's gonna be an advancement for us.
- 8:32- So that's where our opportunity lies
- 8:35- to align with our community leaders,
- 8:38- to align with our vendors, to ensure that
- 8:40- we have the best in class service and
- 8:42- support,
- 8:43- and to also advance our service models as
- 8:46- well, as well as services that we offer
- 8:48- today
- 8:50- and advancing those services and providing more,
- 8:53- in the upcoming months and years.
- 8:56- And so the last question I had for
- 8:58- you today is, who do you admire as
- 9:00- a leader from any industry and why?
- 9:03- Okay. So this one is a big one
- 9:05- for me.
- 9:06- I am a cross functional leader. So I
- 9:08- serve as chief revenue officer for Sinai Chicago.
- 9:12- But my background is so diverse that I
- 9:14- will tell you that I admire
- 9:17- those leaders who are blazing the trail in
- 9:19- health care operations.
- 9:21- That is the IT leader who has figured
- 9:23- out the design to ensure innovation in our
- 9:27- current process...
- 9:28- Current processes and our seamless actions to make
- 9:31- them seamless. Right?
- 9:33- And I admire the clinical leaders who have
- 9:35- actionably created advancements in medicine
- 9:38- so that our patients continue to receive the
- 9:41- highest quality and care.
- 9:42- I also admire those financial leaders who blaze
- 9:46- the trail in ensuring
- 9:48- risk management
- 9:49- and financial stability so we can see more
- 9:52- patients.
- 9:53- That also means that I admire our service
- 9:56- staff and leaders too. That means our security,
- 9:59- our EVS, and our engineers.
- 10:02- Why? Because they keep our doors open, and
- 10:04- they keep us safe and compliant each day.
- 10:08- So
- 10:08- all of these leaders play
- 10:10- a key role in ensuring our community stays
- 10:13- consistently healthy,
- 10:15- and disparities
- 10:16- are reduced.
- 10:17- So I recognize
- 10:19- and I admire
- 10:20- all of the health care operations leaders because
- 10:23- it takes a village through mission and service
- 10:26- to be able to heal our community through
- 10:28- partnership.
- 10:29- Well, thank you so much for joining us
- 10:31- today, Thomasina. It's been a pleasure speaking with
- 10:33- you, and I look forward to talking with
- 10:34- you again down the line. Thank you.
