- 0:47Hello, everyone, and welcome to the Becker's Health
- 0:49Care podcast. We're thrilled today to be joined
- 0:51by John Young, chief strategy and development officer
- 0:54at Evernorth Health Services Home Based Care. John,
- 0:57thank you so much for joining us today.
- 0:59Yeah. Thank you. It's great to be here.
- 1:01So let's jump into today's conversation.
- 1:03Can you please tell us a little bit
- 1:04about yourself and your background?
- 1:07Yeah. So I'm with Evernorth Home Based Care,
- 1:10and we are, in home primary care group
- 1:13that does value based care work for health
- 1:16plans focusing mainly on their,
- 1:18critical and chronic care, patients
- 1:22that need more attention in the home, need
- 1:24clinical care and services in the home.
- 1:27And so I've been in health care for
- 1:2830 plus years now, and always have had
- 1:31a passion for looking at how might we
- 1:33deliver better care and better solutions
- 1:36to solve the big challenges that we have
- 1:38in health care. One of them is the
- 1:39rising chronic condition patients,
- 1:42and on top of that is just the
- 1:43escalating cost of care.
- 1:45Can you speak a little bit more about
- 1:47the mission of Evernorth home based care, and
- 1:49why is this mission important to both you
- 1:51and your team? Yeah. So for me, it
- 1:53goes way back.
- 1:54You know, primary care is essential
- 1:57to our health care system, and I think
- 2:00really powerful
- 2:01option for us to continue to double down
- 2:04on
- 2:04in order to decrease the the cost of
- 2:07care, but also improve the outcomes. Because
- 2:10a lot of the focus of our practice
- 2:13is taking a brick and mortar primary care
- 2:15group
- 2:16and truly bringing that into the home such
- 2:19that we can deliver as many visits, as
- 2:21many services,
- 2:23and as much care and other support that
- 2:25that individual needs
- 2:27so that they're not going to the urgent
- 2:29care or going to the emergency room or
- 2:32going up into the acute care
- 2:35because they're not getting primary care. And a
- 2:37lot of the members just have many issues,
- 2:40social determinants that they're dealing with.
- 2:42They also have,
- 2:44you know, other challenges,
- 2:46transportation,
- 2:48medications.
- 2:49Many of them are on 20 plus medications.
- 2:51They don't know how to balance that. So
- 2:52being in their home and helping in their
- 2:55home is important.
- 2:56For me, it goes way back. My dad
- 2:58was a pediatrician.
- 2:59Growing up in the seventies, I remember him
- 3:01doing house calls.
- 3:03And the way that he created trust
- 3:05and the way that he was able to
- 3:07individually serve each of his patients and our
- 3:10neighbors really resonated with me, and I think
- 3:12it's one of the biggest solutions that we
- 3:14have to to really double down on to
- 3:16improve health care. Right. Right. Yeah. Exactly. That
- 3:18that trust is really key there. So what
- 3:21what are some of the industry barriers and
- 3:22opportunities that your team is currently focused on?
- 3:26Yeah. I I think
- 3:27one is really just the the continued rise
- 3:30and complexity of the the patients and their
- 3:33needs.
- 3:34I think how do you how do you
- 3:35build the right
- 3:37care plan and clinical team
- 3:40that truly can impact in a meaningful way
- 3:43the the individuals that we serve. So just
- 3:45getting a handle of everything that's going on.
- 3:47How many chronic conditions can one person have?
- 3:50They can have 5, 6 plus. You know?
- 3:53And so how do you build that right
- 3:54team,
- 3:55ensure that the patient who can't get out
- 3:57of their home or struggles to get to
- 3:59any level of care, we can bring that
- 4:02into the door. So that's 1.
- 4:042, really building the trust with the individuals
- 4:06and the caregivers of those individuals
- 4:09such that they let us into their home.
- 4:11It's it's really critical.
- 4:13You can't do this virtually. You can't
- 4:16see everything that's going on in the home.
- 4:17You can't build a relationship with the folks
- 4:20that really know what's going on with the
- 4:22individuals, their caregivers,
- 4:23And so you have to build that trust.
- 4:25So we spend a lot of time focusing
- 4:27on, you know, gaining that trust, getting our
- 4:30teams into the home, and then building that
- 4:32relationship with them. You know, the third thing
- 4:35is, obviously, we we face, like everybody else,
- 4:38just supply chain issues with clinic clinical teams.
- 4:41And so, you know, we have the rising
- 4:43chronic conditions, building the trust so that we
- 4:45can truly become
- 4:47a partner in their home for their clinical
- 4:49care, and then ensuring and protecting our clinicians
- 4:52so they can practice at the top of
- 4:53their
- 4:54license. It sounds like there's a lot going
- 4:56on in your space right now.
- 4:58And,
- 4:59with your years of experience, what are some
- 5:01of the biggest pieces of advice you'd have
- 5:03for,
- 5:04your peers in the health care industry? Yeah.
- 5:06I think, you know,
- 5:08having been around the block for 30 years
- 5:10and really focusing on how do we transform
- 5:13our health system, how do we make it
- 5:15more efficient,
- 5:16effective, how do we get it to deliver
- 5:19better outcomes,
- 5:20mainly don't give up. We we will solve
- 5:22this at some point, I hope. And I
- 5:25hope we do it, you know, and I
- 5:26hope the the angle that we're taking with
- 5:28home based care
- 5:29and true,
- 5:30you know, MDs and MPs and pharmacists and
- 5:33RNs getting into the home and delivering care
- 5:36is one of the answers that's gonna help
- 5:38us maybe truly bend
- 5:40the cost curve, increase outcomes, and and bring
- 5:44value based care. So don't give up.
- 5:46I think the other thing is really think
- 5:48creatively. You know? The brick and mortar,
- 5:51the fee for service,
- 5:53the other challenges that we faced. I I
- 5:56mean, truly, they haven't changed for 30 years
- 5:58even though the big promises that CMS has
- 6:01made,
- 6:02the big initiatives that many of the payers
- 6:04have
- 6:05put forward,
- 6:06are are are slowly changing the dynamics of
- 6:09our health care system. But I think we
- 6:11really have to think a bit radical and
- 6:13think differently and and not be bound by
- 6:16the the biases that have held us, you
- 6:18know, kind of, you know, stuck for so
- 6:21long.
- 6:22So how does Evernorth home based care differentiate
- 6:25itself from other home based care providers in
- 6:27the industry?
- 6:28And, what what would you say are some
- 6:30unique advantages that you bring to your patients
- 6:32and health care partners?
- 6:33Yeah. I think, first,
- 6:35just being able to go into a true
- 6:38full,
- 6:38capitated global risk model
- 6:41is a a a really big advantage.
- 6:44We are truly putting,
- 6:46our money where our mouth is, you know,
- 6:48to say.
- 6:49And we we believe that
- 6:52providers
- 6:53and our clinical team
- 6:55should be allowed to make their decisions and
- 6:57not be bound by the OMI billing the
- 7:00right code, OMI,
- 7:02you know, billing something that I'm gonna get
- 7:03reimbursed for. So I think that's one of
- 7:06our our big advantages. I think to,
- 7:08being a part of Evernorth
- 7:11and the full suite of solutions that exist
- 7:14from the the benefits
- 7:16management's
- 7:17aspects in pharmacy and medical
- 7:19to all of the things that we do
- 7:20on our side of the house and the
- 7:22air delivery
- 7:24and pharmacy side.
- 7:26We're able to leverage the knowledge, for 1,
- 7:29the resources and support that give us a
- 7:31no another layer of clinical
- 7:34competencies and teams and skills that we can
- 7:36surround that patient by,
- 7:39as well as, you know, doing some of
- 7:40the virtual 247
- 7:42If it's critical and we need to get
- 7:44in touch or get someone to that home,
- 7:46we're able to do that. So, you know,
- 7:48a lot of those things are are really
- 7:50important. The other big thing is,
- 7:53you know, truly getting into the home,
- 7:56being a trusted part of the family caregiver
- 7:59team,
- 8:00and seeing all of the social determinants of
- 8:02health, and then building a care plan that's
- 8:05unique to each individual
- 8:07based on their conditions,
- 8:09based on their social determinants,
- 8:12based on their caregiver and other support, and
- 8:14becoming kind of that Sherpa of their ecosystem
- 8:17for their health.
- 8:19John, I wanna thank you again for joining
- 8:20us today on the Becker's Healthcare podcast. But
- 8:22before I let you go, do you have
- 8:23any final thoughts you'd like to leave with
- 8:24our listeners? Yeah. I think this is just
- 8:27an exciting time for health care, and it's
- 8:30an exciting time for leaders in health care
- 8:32to really rethink
- 8:33where should the clinical
- 8:35models be delivered,
- 8:37how is care shifting
- 8:39and getting into the home, and how am
- 8:41I as a a leader going to respond
- 8:44to that and become a champion of it
- 8:47versus protecting kind of the biases of the
- 8:49past. And I I just applaud everybody who's
- 8:51doing it, and, the energy's there. So let's
- 8:54get on it. Awesome. I really appreciate those
- 8:56insights today.
- 8:57John, thank you again for joining us today.
