- 2:05Speaker 3: gonna talking about implants a lot today. Yeah, thank you
- 2:09Speaker 3: for that clarification. Didn't want them to think it was
- 2:10Speaker 3: any other kind of implants. So we're gonna be talking
- 2:12Speaker 3: about dental implants this morning, and there's a uh, doctor Greeb,
- 2:16Speaker 3: you said that there were You're gonna get a lot
- 2:18Speaker 3: of questions about this, and this is one of those technologies.
- 2:20Speaker 3: We've covered it a few times on this show. This
- 2:22Speaker 3: is one of those things that if you're somebody who
- 2:24Speaker 3: was an implant curious at some point in the past,
- 2:28Speaker 3: it might be worth your while to take a look.
- 2:30Speaker 3: There's more options, there's more tools than ever in the
- 2:31Speaker 3: dentist toolbox.
- 2:32Speaker 5: Yeah, I mean, that's something that we've talked about, you know,
- 2:35Speaker 5: pretty regularly on the show. But however, we have almost
- 2:39Speaker 5: daily patients coming in and because of asking questions because
- 2:43Speaker 5: there's so much I don't know, if you want to say,
- 2:47Speaker 5: there's so much confusion around what a dental implant is,
- 2:50Speaker 5: how they can be used in your mouth to help
- 2:52Speaker 5: you out in what are you a candidate for them?
- 2:54Speaker 4: Are you not?
- 2:56Speaker 5: And then what do dental implants mean? Is it singular thing?
- 2:59Speaker 5: Is it ltiple teeth? What if you're using implants to
- 3:02Speaker 5: fix one tooth? What if you're using implants to fix
- 3:05Speaker 5: missing all of your teeth? So there are so many
- 3:07Speaker 5: different ways to to use those appropriately in the mouth,
- 3:12Speaker 5: and that's why I thought it would be a good
- 3:14Speaker 5: time to kind of go down the options because everyone
- 3:17Speaker 5: seems to come in with with a lot of questions.
- 3:19Speaker 5: They say, you know, I hear about implants, Is that
- 3:21Speaker 5: something I need to do now? Or they generally just
- 3:25Speaker 5: have a lot of dental problems, and maybe their mind
- 3:28Speaker 5: goes there because there's so much messaging and advertisement towards
- 3:33Speaker 5: that right now online and on television, and some some
- 3:37Speaker 5: people come in thinking that, you know, I'm I've been
- 3:40Speaker 5: away for the dentists for a couple of years. It
- 3:41Speaker 5: must be time for my dental implants, you know, So
- 3:43Speaker 5: we've got to talk about implants.
- 3:46Speaker 6: Well, let's talk about it.
- 3:47Speaker 4: Yeah exactly, So let's let's have that discussion.
- 3:49Speaker 3: So you get sometimes people that are a little overly
- 3:51Speaker 3: aggressive and seeking out the implants. It's like just get
- 3:54Speaker 3: a rind of all, just just pump something new in there.
- 3:56Speaker 3: Let's just let's go robot teeth.
- 3:57Speaker 5: Oh yeah, absolutely, yesterday.
- 4:01Speaker 3: It happens all the time.
- 4:02Speaker 4: Indeed.
- 4:03Speaker 5: So what you have getting right into that is patients
- 4:08Speaker 5: like I say that, notice that they're say they're getting
- 4:12Speaker 5: to be, you know, middle aged, and a few teeth.
- 4:14Speaker 4: Are starting to break down.
- 4:16Speaker 3: Guilty, there you go.
- 4:17Speaker 5: So you know, it happens to the best of us,
- 4:20Speaker 5: and they get discouraged and they say, you know, I've
- 4:24Speaker 5: had two or three teeth break on me the last year.
- 4:28Speaker 5: I haven't been to the dentist for a while, you know,
- 4:31Speaker 5: and I never really liked the color for my teeth.
- 4:33Speaker 5: I think it's just time to go to dental implants
- 4:36Speaker 5: like what I mean, this is what happens, and people
- 4:38Speaker 5: come in and say it, and it's and I don't
- 4:41Speaker 5: want to discredit what they're saying. It's just that they're
- 4:43Speaker 5: they're not they're they're not understanding the whole process and
- 4:47Speaker 5: what options there are for them.
- 4:49Speaker 4: So that's where I'm excited.
- 4:50Speaker 5: When they come in, we have the opportunity to have
- 4:52Speaker 5: a great discussion about it and to say, you know
- 4:55Speaker 5: what I'm looking at your teeth and they're not that bad,
- 4:58Speaker 5: and they're very relie.
- 5:00Speaker 3: Of your own teeth as a matter of fact.
- 5:01Speaker 4: So it's great news.
- 5:02Speaker 3: You know.
- 5:02Speaker 5: It's just because a lot of individuals, I think, just
- 5:05Speaker 5: kind of get down on themselves and they think there's
- 5:07Speaker 5: no other option for them at this time. But that's
- 5:09Speaker 5: what's great is that we love to sit down and
- 5:11Speaker 5: speak with our patients, do a work up of their
- 5:14Speaker 5: oral health, and to tell them many times, you know, hey, what,
- 5:19Speaker 5: guess what we can save what you got and it's
- 5:21Speaker 5: gonna be a lot less involved, a lot less expensive,
- 5:25Speaker 5: and you're gonna be in great shape. And that's a
- 5:27Speaker 5: wonderful discussion to have.
- 5:29Speaker 6: Now.
- 5:29Speaker 5: At the same time, there are some that have been
- 5:31Speaker 5: away from the dentists, maybe longer, maybe they have some
- 5:35Speaker 5: underlying conditions, they have long standing gum disease or something
- 5:38Speaker 5: of that nature. They come in and they say, I
- 5:41Speaker 5: think it's about time to get rid of these teeth,
- 5:44Speaker 5: and sometimes you say, you know what, You're correct, We're
- 5:47Speaker 5: gonna have to do that. So that's kind of the
- 5:50Speaker 5: point of that initial consultation and conversation that we have
- 5:53Speaker 5: with our patients, which we're very happy to do.
- 5:56Speaker 3: That's really it's really a wonderful thing to be able
- 5:58Speaker 3: to give the options for something like that, and also
- 5:59Speaker 3: to talk people back a little bit from, like you said,
- 6:02Speaker 3: maybe being down on themselves a little bit, or you know,
- 6:04Speaker 3: people will hear stuff through whisper networks all the time,
- 6:06Speaker 3: you know, and so you you somebody will start planning
- 6:09Speaker 3: seas in your head, the best thing you're going to
- 6:10Speaker 3: have to do is get implants, you know, or a
- 6:12Speaker 3: cousin got implants or something like and suddenly you think
- 6:14Speaker 3: that that's something that you have to do. So having
- 6:17Speaker 3: that conversation with someone and also doing it in in
- 6:19Speaker 3: an ethical manner to make sure the persons make the
- 6:22Speaker 3: right decisions with full uh decisions in front of them,
- 6:25Speaker 3: they know what they can and can't do, what the
- 6:27Speaker 3: results are going to be. All those things are really
- 6:29Speaker 3: necessary when it comes to having the discussion about implants.
- 6:32Speaker 3: And there's different types of implants and different types of
- 6:34Speaker 3: situations too. So having a doctor like doctor Garib or uh,
- 6:37Speaker 3: doctor Henley, that really is going to help for the
- 6:40Speaker 3: patient to isolate, Okay, here's what my situation is, here's
- 6:43Speaker 3: the type of implant that I can do, and what
- 6:45Speaker 3: it's going to mean for me in the long term.
- 6:46Speaker 5: And also, I think what drives a lot of that
- 6:48Speaker 5: thought process is the idea that implants last forever. That's
- 6:56Speaker 5: like you get those in, you know, and you're and
- 6:58Speaker 5: you're done, you know. And so you have patients that
- 7:01Speaker 5: come in that are thirty years old, forty years old,
- 7:04Speaker 5: and they say, you know what, I'm just done with
- 7:06Speaker 5: dealing with these teeth. I want something that's going to
- 7:08Speaker 5: last me forever. And I mean this is like really what.
- 7:11Speaker 3: People say, you never want to save again, just get
- 7:12Speaker 3: rid of all of my phone.
- 7:13Speaker 5: I mean, it's kind of kind of like that, right,
- 7:16Speaker 5: And that's a good analogy. And I and I have
- 7:19Speaker 5: that conversation, and there are some individuals that that may
- 7:23Speaker 5: be the best route, you know, I have that discussion. Say,
- 7:26Speaker 5: you know what, we can invest in what you have,
- 7:28Speaker 5: save a lot of your teeth, restore them otherwise, but
- 7:32Speaker 5: you're going to have to have maintenance. You know, you
- 7:34Speaker 5: have to come into your dentist for cleanings regularly. But
- 7:37Speaker 5: if you just know you for cleanings regularly, but if
- 7:39Speaker 5: you just know you're not going to do that, you
- 7:41Speaker 5: know you're not going to brush your teeth at home.
- 7:42Speaker 5: You're not going to floss. You know, maybe going to
- 7:46Speaker 5: implants is an option for is a better option for you.
- 7:49Speaker 5: And sometimes patients say, yeah, you know, that's what I'm
- 7:52Speaker 5: That's what I need because I know I'm not going
- 7:53Speaker 5: to maintain them now.
- 7:55Speaker 4: That is.
- 7:57Speaker 5: That that's the reality of the situation. That is not
- 8:01Speaker 5: recommended by any means, because truthfully, as we're doing more
- 8:06Speaker 5: and more implants in the industry in dentistry, we're seeing
- 8:12Speaker 5: that implants may not be forever and things that you
- 8:17Speaker 5: put on top of implants are definitely not forever. So
- 8:20Speaker 5: the younger you are going to a full implant option
- 8:23Speaker 5: in your mouth, the more I'm almost assuredly you will
- 8:29Speaker 5: have to do some significant maintenance on that down the line,
- 8:32Speaker 5: which is not inexpensive as well. So that's patients are
- 8:36Speaker 5: a lot of time thinking, you know, I invest heavily
- 8:38Speaker 5: right now into dental implants and I'm set for life. Well,
- 8:42Speaker 5: that may not be the case. So that's another part
- 8:44Speaker 5: of the conversation that we can have with patients, and
- 8:47Speaker 5: a lot of that depends upon which dental implant option
- 8:50Speaker 5: they go with. WHI which there are many.
- 8:53Speaker 3: One question that I just thought of that I don't
- 8:56Speaker 3: think I've asked before as far as the implants go,
- 8:58Speaker 3: if you're someone who's very active of that takes a
- 9:01Speaker 3: lot of like physical punishment some way, Like if you're
- 9:03Speaker 3: an athlete, maybe you play football or something like that,
- 9:05Speaker 3: or even a boxer or something. Right, is that something
- 9:09Speaker 3: that you should wait to your careers over before you
- 9:11Speaker 3: get an implant or is that something that you can
- 9:13Speaker 3: get and then monitor? I mean, like if what would
- 9:16Speaker 3: be the best circumstance for somebody like that, like, well,
- 9:18Speaker 3: you wanted to get it done.
- 9:19Speaker 5: It depends on your goals for sure, because as I
- 9:22Speaker 5: think I've mentioned before, there there is there's certain sports
- 9:26Speaker 5: where you lose a lot of teeth. Yeah, right, hockey, hockey, rugby,
- 9:29Speaker 5: And I know of a South American rugby team that
- 9:32Speaker 5: they've lost so many teeth that they had their their
- 9:35Speaker 5: team dentists start putting dental implants into the missing teeth
- 9:40Speaker 5: and creating a custom crown on top of it with
- 9:43Speaker 5: a bottle opener in it.
- 9:46Speaker 3: I believe it so.
- 9:47Speaker 5: Useful, Yeah, very useful, because he's like, they're going to
- 9:50Speaker 5: try to open bottles with their beer bottles with their.
- 9:52Speaker 4: Teeth anyway, So let's make them, give them a titanium.
- 9:55Speaker 3: I mean, that's right close to biohacking, right, like the
- 9:58Speaker 3: whole or you do things to your body for utility
- 10:00Speaker 3: or whatever, that's very close to biohacking.
- 10:02Speaker 5: So in that situation, those those guys uh in the
- 10:06Speaker 5: rug on the road on the rugby team, definitely are
- 10:09Speaker 5: okay with having implants while they're while they're still practicing
- 10:13Speaker 5: their sport, you know. But yeah, depends on the sport.
- 10:17Speaker 5: But that is back to that conversation about longevity and maintenance.
- 10:21Speaker 5: That is the benefit of dental implants is once you
- 10:23Speaker 5: go to those, it is titanium. They don't give like teeth,
- 10:27Speaker 5: dew and flex and move their rock solid and if
- 10:31Speaker 5: they heal well enough into your jawbone, they can take
- 10:34Speaker 5: a beating. So typically with athletes, it's it's pretty advisable
- 10:40Speaker 5: to go onto that if the situation lends itself to it.
- 10:43Speaker 3: Yeah, I just thought that was a I just popped
- 10:45Speaker 3: in my head. I saw Carl Lee in here yesterday.
- 10:47Speaker 3: I was like, I wonder what happened to like a
- 10:48Speaker 3: football player? You know, you get somebody like Jack Lambert,
- 10:50Speaker 3: you know, he he famously didn't have any teeth. What
- 10:52Speaker 3: if he wanted to have like all his pearly whites
- 10:54Speaker 3: in there, and what if he wanted to pop him
- 10:56Speaker 3: out before games? So he looked intimidating. But now talk
- 10:59Speaker 3: about bridges or something there. Well, let's start talking about
- 11:02Speaker 3: these things, because that's the important thing that we're talking
- 11:04Speaker 3: about this morning. Again, if you have questions for your
- 11:06Speaker 3: personal situation, you can give us a call at three
- 11:09Speaker 3: zero four three four to eight one three one. You
- 11:10Speaker 3: can text three zero four non three five five zero
- 11:12Speaker 3: zero eight. And and that's the thing about when it
- 11:15Speaker 3: comes to implants and situations is you do and you
- 11:19Speaker 3: and doctor Henley would know this obviously better than anyone.
- 11:21Speaker 3: There's certain circumstances you must see more commonly than others.
- 11:24Speaker 3: What are some of the things that match up pretty
- 11:26Speaker 3: well as far as situations that a patient may be
- 11:28Speaker 3: in and implants that are perfect for Yeah.
- 11:30Speaker 5: So I think think we should start with kind of
- 11:32Speaker 5: the simplest thing. You know, what happens with you have
- 11:36Speaker 5: one single tooth implants, we're basically replacing teeth. This is
- 11:40Speaker 5: the best thing we can do right now in our
- 11:43Speaker 5: current world situation and technology that we have to give
- 11:47Speaker 5: you back A tooth is with a dental implant, Doctor Henley,
- 11:50Speaker 5: want to go through the parts of a dental implant,
- 11:52Speaker 5: like a single.
- 11:52Speaker 6: Oh you want me to go deep dive and be
- 11:54Speaker 6: it nerd Oh, I'm gaining.
- 11:56Speaker 5: I mean, you know you love doing that thing. So
- 11:57Speaker 5: let's let's let's go with what a single tooth implant?
- 12:00Speaker 5: What components there are?
- 12:01Speaker 3: For sure? For sure?
- 12:02Speaker 6: And the reason I did correct you Dale. When it
- 12:05Speaker 6: comes to implant versus dental implant, we have a very
- 12:07Speaker 6: close friend called doctor Tim Kazinski in Detroit. He's placed
- 12:10Speaker 6: something like eighteen thousand implants and it's kind of an
- 12:13Speaker 6: inside joke now every time I say so or ask
- 12:16Speaker 6: him a question about implants, he goes dental implants.
- 12:18Speaker 3: He just corrects me.
- 12:19Speaker 6: You just make sure I know he places dental implants, Like,
- 12:22Speaker 6: I don't think he's an orthpeedic surgeon or anything, but
- 12:24Speaker 6: he just always says that.
- 12:24Speaker 3: So I'm always just likely.
- 12:27Speaker 4: Talk about implants.
- 12:28Speaker 5: I've gotten calls that the patients think that I am
- 12:31Speaker 5: a different type of an implant placement surgeon of some sort.
- 12:35Speaker 6: And we are not. We are dental implant dentist.
- 12:38Speaker 3: Yes, So moving on.
- 12:40Speaker 6: So when it comes to implants, if we're going to
- 12:42Speaker 6: replace a single tooth. There are three main parts of
- 12:46Speaker 6: a dental implant. There's the implant itself, which pretty much
- 12:49Speaker 6: when you think about the parts of a tooth, you
- 12:51Speaker 6: have the part that you see called the crown of
- 12:54Speaker 6: the tooth, and the part you don't see that's in
- 12:56Speaker 6: the bone, called the root. What the dental implant replaces
- 12:59Speaker 6: pretty much is the root, and so from that you
- 13:02Speaker 6: have something that sticks out called the abutment, and then
- 13:04Speaker 6: the crown sits on top of that, and a lot
- 13:06Speaker 6: of times you can put the abutment in the crown
- 13:08Speaker 6: together called screwtained. Anyway, I'm getting into the weeds here,
- 13:10Speaker 6: but pretty much the dental implant itself is not something
- 13:13Speaker 6: that you should see. If you see it, there's probably
- 13:16Speaker 6: a problem, and we kind of fix those problems in
- 13:18Speaker 6: our offices all the time. But the dental implant itself,
- 13:21Speaker 6: pretty much when you think about what part of the
- 13:23Speaker 6: tooth it is, it's the root. And then from there
- 13:26Speaker 6: we restore on top of it by putting a screw
- 13:28Speaker 6: and screwing a screw into the dental implant, which is
- 13:32Speaker 6: itself a screw into the bone. Anyway, it's Brent just
- 13:34Speaker 6: a mechanism of screwage to screw things together to create
- 13:38Speaker 6: something that looks like a tooth. But in a sense,
- 13:42Speaker 6: the single implant, there's three parts, the dental implant, the abutment,
- 13:45Speaker 6: and then the crown.
- 13:46Speaker 4: That's right.
- 13:46Speaker 5: And that's where that is a source of big confusion
- 13:48Speaker 5: because patients come in and say, you know, I'm ready
- 13:50Speaker 5: for my implant.
- 13:52Speaker 4: Well, the way you look at it like, well, we already.
- 13:54Speaker 5: Have implants in your head. What do you mean you're
- 13:56Speaker 5: ready for your implant? It's already there. Well, no, I
- 13:58Speaker 5: haven't gotten an implant yet. Well there they're talking about
- 14:01Speaker 5: the crown, the tooth part that you use, and you
- 14:03Speaker 5: see that doctor Henley was saying that part right there.
- 14:07Speaker 5: It's the crown that goes on top of the implant.
- 14:08Speaker 5: Didentially implants, I say to a lot of people, you
- 14:11Speaker 5: got to you can think of them as kind of.
- 14:13Speaker 4: A trailer hitch receiver. You know.
- 14:15Speaker 5: It's like something you can put different attachments into, you know,
- 14:19Speaker 5: So the implant itself goes into the bone, and then
- 14:22Speaker 5: you can either put that abutment that doctor Henley's talking about.
- 14:25Speaker 5: It's typically made out of titanium screwing to that implant body,
- 14:29Speaker 5: the implant itself within the bone, and then you can
- 14:32Speaker 5: use it like a regular tooth that we've prepared for
- 14:34Speaker 5: a crown.
- 14:35Speaker 4: You put a crown on top of it.
- 14:36Speaker 5: So that's the simple single tooth, you know, baseline, this
- 14:41Speaker 5: is a basic implant what it is.
- 14:43Speaker 6: And by simple, it's very complex.
- 14:46Speaker 4: But that's the simplest iteration of an implant.
- 14:50Speaker 6: It is not easy. We've been to how many hundreds
- 14:52Speaker 6: of out maybe thousands of hours of see now to
- 14:54Speaker 6: get this to do.
- 14:55Speaker 3: What we do, so that that's simply if you want
- 14:57Speaker 3: to replace one tooth, just want to So that's kind
- 15:00Speaker 3: of the basic thing. So you come in missing a tooth,
- 15:03Speaker 3: we can do with an implant instead of something that
- 15:05Speaker 3: comes in and out of your mouth, instead of a bridge.
- 15:07Speaker 3: But moving on from that, then we start talking about
- 15:11Speaker 3: you know, now I'm not just missing one, two or
- 15:15Speaker 3: three teeth scattered throughout my mouth that I want to
- 15:17Speaker 3: replace with dental implants. Now we're starting to talk about
- 15:21Speaker 3: replacing the entire arch. We call it either the top
- 15:25Speaker 3: jaw or the bottom jaw. You're missing all your teeth
- 15:27Speaker 3: or all of your teeth are in such bad shape
- 15:29Speaker 3: that we do have to remove them, and then we
- 15:31Speaker 3: start talking about how can we put teeth back into
- 15:34Speaker 3: your head? Basically now traditionally take out your teeth, we
- 15:38Speaker 3: can use dentures, right, And patients, you know, either tolerate
- 15:43Speaker 3: dentures or they hate dentures. You know, it's not much
- 15:45Speaker 3: in between. And some get along very well with them.
- 15:48Speaker 3: However many do not. So that's another great function of implants,
- 15:53Speaker 3: and another big reason that they were brought into the
- 15:55Speaker 3: dental world is to help patients that have that are
- 15:58Speaker 3: struggling with dentures. So, what do you say struggle? What
- 16:01Speaker 3: do you mean they have a hard time keeping them
- 16:03Speaker 3: in their mouth, They're not comfortable, they can't eat right.
- 16:05Speaker 3: When somebody is struggling with dentures, what are they recognizing
- 16:07Speaker 3: about themselves?
- 16:08Speaker 4: All of the above, the all of those things.
- 16:10Speaker 5: But typically a top denture kind of suctions up against
- 16:13Speaker 5: the roof of your mouth pretty well, and the top
- 16:15Speaker 5: ones stay up generally decently until they don't and then
- 16:19Speaker 5: if your bone changes or you just don't have the
- 16:21Speaker 5: right bone shape, you can't really get it to suction
- 16:25Speaker 5: up there. Maybe you gag some In those patients, they
- 16:28Speaker 5: have a really tough time because they can't even keep
- 16:30Speaker 5: a top denture in, which is unusual.
- 16:32Speaker 3: So you hear like a wet mouth, like sucking sounds
- 16:34Speaker 3: when they talk a lot, and stuff like that. Is
- 16:36Speaker 3: that usually what seems to happen.
- 16:37Speaker 5: Yeah, But really what happens with those patients is if
- 16:40Speaker 5: they're not able to use adhesive on their the denture paste,
- 16:44Speaker 5: the denture just flops down, like gravity just pulls it
- 16:47Speaker 5: down and there there's nothing to grab onto. That is
- 16:50Speaker 5: a little more abnormal. Normally we can make a denture
- 16:52Speaker 5: on the top that suctions up there decently, but nonetheless
- 16:55Speaker 5: that's the upper denture. The lower denture is the main issue,
- 16:58Speaker 5: is a really big problem. I mean the number of
- 17:01Speaker 5: patients out there that have full sets of dentures that
- 17:04Speaker 5: only where their top denture is probably i would say
- 17:06Speaker 5: probably sixty, yeah, fifty six?
- 17:08Speaker 3: Wow?
- 17:09Speaker 4: Really yeah, I mean so many?
- 17:10Speaker 6: Is that bad?
- 17:11Speaker 4: You'll notice if you notice someone that.
- 17:14Speaker 5: A lot of patients walk around with no teeth on
- 17:17Speaker 5: the bottom but teeth on the top to smile with,
- 17:19Speaker 5: because the bottom denture just no matter what you do
- 17:23Speaker 5: as a dentist, it's yeah, I mean, it's just hard.
- 17:26Speaker 5: It's hard to deal with. You have your tongue in there.
- 17:28Speaker 5: That's just kicking that denture around. You don't have as
- 17:31Speaker 5: much surface area for the denture to hold on to,
- 17:34Speaker 5: and as you talk and chew and speak, that bottom
- 17:37Speaker 5: denture is just kind of floating around. And patients that
- 17:39Speaker 5: get along well with them, they've learned good muscle control.
- 17:43Speaker 5: It's not so much that their jaw is holding the
- 17:45Speaker 5: denture in better as much as their cheeks and their
- 17:48Speaker 5: tongue kind of keep it down and they've learned to
- 17:50Speaker 5: deal with it. And some some patients do well enough
- 17:53Speaker 5: with them to get along with it. However, the World
- 17:57Speaker 5: Health Organization has labeled a regular denture as having a
- 18:01Speaker 5: disability wow because of that bottom denture, because it's just
- 18:06Speaker 5: almost impossible to get just right and to work well with.
- 18:10Speaker 5: So what is done as a result of that is
- 18:14Speaker 5: placing some dental implants on the bottom to hold that
- 18:18Speaker 5: bottom denture in place. So back to the World Health Organization,
- 18:21Speaker 5: they say regular dentures disability. If you put two implants
- 18:25Speaker 5: on a lower jaw and attach a denture to it,
- 18:28Speaker 5: that gets them out of that disability category. That's that's
- 18:31Speaker 5: the best way. That's the minimum amount of work to
- 18:34Speaker 5: restore a patient to full function in a sense. So
- 18:38Speaker 5: that brings us to how we can use dental implants
- 18:41Speaker 5: with your dentures.
- 18:42Speaker 3: And why this tool is so great because everything you
- 18:44Speaker 3: just describe for some people, some people maybe are okay
- 18:46Speaker 3: with it. You know, maybe they live a little bit
- 18:48Speaker 3: more reclusive lifestyle and they're not out and about. I mean,
- 18:51Speaker 3: that's fine. I mean some people have lifestyles that are
- 18:53Speaker 3: fine with stuff like that, but other people that would
- 18:56Speaker 3: be a big impediment. You know, if with having problems
- 18:58Speaker 3: with the lower denter and stuff like that. How we
- 19:00Speaker 3: have other implants that can help with this exactly.
- 19:02Speaker 5: So now we're going to go kind of basically from
- 19:04Speaker 5: the run up of what options we have in implants
- 19:08Speaker 5: with dentures. Let's say so you come in, you don't
- 19:10Speaker 5: have any teeth. Maybe you have a set of dentures
- 19:13Speaker 5: that are as we described. You like how they look,
- 19:16Speaker 5: they don't fit great. You like how they look and
- 19:20Speaker 5: fit in general, but you can't keep that bottom denture
- 19:22Speaker 5: in place. So in our offices, we have the option
- 19:26Speaker 5: of taking those dentures that you have ad placing surgically,
- 19:31Speaker 5: placing a few implants, and then using those same dentures
- 19:36Speaker 5: to reattach if we deem those dentures good enough to
- 19:39Speaker 5: do so. Now we reserve the right to say, you
- 19:41Speaker 5: know what, especially if we didn't make the dentry, you know,
- 19:43Speaker 5: we're not gonna put something on top of that that,
- 19:46Speaker 5: you know, we don't really know much about and we
- 19:47Speaker 5: don't think is going to be structurally sound enough for
- 19:50Speaker 5: you and give you a good, good result.
- 19:51Speaker 4: So that's up to the discretion of the doctor.
- 19:53Speaker 5: But sometimes with our own patients, we've made them dentures
- 19:56Speaker 5: and we say, you know, hey, we're going to make
- 19:58Speaker 5: these the best we can if but if it's not
- 20:00Speaker 5: working for you, we can upgrade you to the implants
- 20:03Speaker 5: at that time, and some patients take that option on.
- 20:06Speaker 5: So how to do that? You have a few different
- 20:09Speaker 5: ways to do it. There are there's one the kind
- 20:12Speaker 5: of most baseline option. They are called mini implants. Now
- 20:17Speaker 5: I talked about before how implants can be like a
- 20:21Speaker 5: hitch receiver. You can change the tops on them. With
- 20:24Speaker 5: many implants. You cannot many implants. They are very small,
- 20:28Speaker 5: they're thin. They're designed for patients that don't either financially
- 20:33Speaker 5: can't afford the larger implants or they don't have enough
- 20:38Speaker 5: bone for the larger, thicker implants, and you can only
- 20:41Speaker 5: use these very thin implants, but you have to put
- 20:44Speaker 5: in multiple ones. You have to put in at least
- 20:46Speaker 5: four of these many implants spaced out across the front jaw,
- 20:52Speaker 5: and they have a little ball that kind of sticks
- 20:55Speaker 5: up through the gums once the implants are inserted, and
- 20:58Speaker 5: then that little ball attaches to a small gasket inside
- 21:02Speaker 5: of the denture, which is something that we do in
- 21:06Speaker 5: the office after we place them, and that gives the
- 21:09Speaker 5: patient some very good stability to ventures that previously we're
- 21:13Speaker 5: floating around like like, uh, like boots on a rooster
- 21:17Speaker 5: what we call them?
- 21:18Speaker 4: You know that just there in that Texas?
- 21:22Speaker 6: Is that not a Texas?
- 21:23Speaker 3: I never heard that. I like it.
- 21:25Speaker 4: What do you think about a rooster wearing boots?
- 21:26Speaker 2: Right?
- 21:26Speaker 4: It doesn't fit.
- 21:28Speaker 6: Like it's high heels.
- 21:31Speaker 4: Yeah, there you go, you know, flopping around.
- 21:34Speaker 5: So if you got a denture flopping around, we can
- 21:37Speaker 5: put put those implants in and and get those connected.
- 21:40Speaker 3: Now. I can't get farn leg horn wearing boots out
- 21:43Speaker 3: of my mind.
- 21:45Speaker 4: You got the you got the vision.
- 21:46Speaker 5: Now, However, now that we have as many implants in place,
- 21:51Speaker 5: and those attachments are like little o ring gaskets that
- 21:54Speaker 5: we use to go down over that little ball joint,
- 21:56Speaker 5: that little ball attachment, so as you could imagine, there's
- 21:59Speaker 5: certain movement and flex to that, and that's these implant options,
- 22:03Speaker 5: the kind of entry level implant options are designed for
- 22:06Speaker 5: those bottom dentures in particularly, put two of the major
- 22:10Speaker 5: bigger implants in or four of those mini implants in place,
- 22:13Speaker 5: and that keeps those dentures from kind of popping out
- 22:15Speaker 5: and giving you more security, but it's not gonna lock
- 22:19Speaker 5: them in like rock solid.
- 22:21Speaker 6: It keeps some men, doesn't keep them stable.
- 22:23Speaker 5: Right, they're not, so that's definitely better than not having them.
- 22:27Speaker 5: But if patients are coming in think I'm gonna get
- 22:29Speaker 5: that mini implant option and I'm gonna you know, never
- 22:32Speaker 5: have to use any denture paste again, or these teeth
- 22:34Speaker 5: are going to be just locked in there, really tight tightly,
- 22:37Speaker 5: it's not gonna happen that way because if you can
- 22:40Speaker 5: imagine the very back teeth not having anything to lock
- 22:43Speaker 5: those in to hold on to, they're gonna move and
- 22:46Speaker 5: your gums are designed to kind of the dentures are
- 22:49Speaker 5: designed for your gums to take some of the pressure,
- 22:52Speaker 5: so you're gonna have some movement in there. But a
- 22:54Speaker 5: lot of patients have done this on get along super
- 22:57Speaker 5: well with the mini implants and it's a great introductory
- 23:00Speaker 5: option as far as implants go. So we got the
- 23:04Speaker 5: minis and then we can move on to the larger ones.
- 23:06Speaker 3: Let's do that. Let's tay of a break first, and
- 23:08Speaker 3: if folks want to get any questions, we do have
- 23:10Speaker 3: a question that we'll get to when we come back.

- 27:03Speaker 3: Before we went to the break, we're talking about dental
- 27:05Speaker 3: implants this morning. We did give a text line question
- 27:08Speaker 3: before we went to the break. This is from the
- 27:10Speaker 3: three zero four four to one zero? What's the pain
- 27:12Speaker 3: scale for an endplant? So I guess that just means
- 27:14Speaker 3: does it hurt?
- 27:16Speaker 4: Not at all?
- 27:17Speaker 3: Let me turn on your mind.
- 27:17Speaker 6: The pain scale for an implant, I would say is
- 27:22Speaker 6: zero to one. I mean maybe one, wow. Really yeah,
- 27:25Speaker 6: most often it's a zero, And I don't know why.
- 27:28Speaker 6: Necessarily why there's no response, usually no inflammation afterwards. I
- 27:32Speaker 6: don't have to give pain medicine, which I don't really
- 27:34Speaker 6: give serious pain medicine, but not even addvil and talent all.
- 27:38Speaker 6: Maybe I do give an antibodic after I placed implants,
- 27:40Speaker 6: just because of some research I've read. But really there's
- 27:43Speaker 6: no pain. The only pain you might have is if
- 27:46Speaker 6: you have to remove the tooth and then immediately place
- 27:48Speaker 6: the implant, because removal of teeth tends to be the
- 27:51Speaker 6: more inflammatory, a little more painful. But no, there's usually
- 27:55Speaker 6: no pain response.
- 27:56Speaker 2: Wow.
- 27:57Speaker 3: And so if that was a barrier, that certainly shouldn't
- 27:59Speaker 3: be something if you have.
- 28:00Speaker 5: An existing kind of missing tooth, there's enough bone there
- 28:03Speaker 5: to do to put the implant in. All we have
- 28:06Speaker 5: to do is make a small opening in the gum tissue,
- 28:08Speaker 5: and that is what hurts. It's like a pizza burn,
- 28:11Speaker 5: you know, like an injury to your gums that takes
- 28:14Speaker 5: a week or two to completely heal. But isn't that
- 28:17Speaker 5: painful for people? But you know, to doctor Henley's point
- 28:20Speaker 5: that the bone, when you put the implant into the bone,
- 28:22Speaker 5: there are less nerve endings within that bone itself, and
- 28:27Speaker 5: you don't sensation, don't have much of that pain sensation
- 28:30Speaker 5: or response. Now once in a while, if there's another
- 28:35Speaker 5: issue with thinness of bone or previous infection, there can
- 28:40Speaker 5: be times that there can be a pain response from
- 28:44Speaker 5: kind of something else other than the implant itself. So
- 28:48Speaker 5: there are times when implants can be tough. Or you know,
- 28:51Speaker 5: the other day placed eight implants on one patient at
- 28:55Speaker 5: one time. That's a lot of a lot of significant
- 28:59Speaker 5: surgery going on, so there's some discomfort associated with that afterwards,
- 29:03Speaker 5: just from kind of the like he's like doctor Henley saying,
- 29:06Speaker 5: the inflammation inflammatory response of the surgery itself, not so
- 29:10Speaker 5: much the implants in the bone though, So it is
- 29:11Speaker 5: fascinating when things go very well. There's very little pain
- 29:16Speaker 5: pain result.
- 29:16Speaker 3: And this is all done right in the office and
- 29:18Speaker 3: just local anesthet right.
- 29:19Speaker 4: Yeah, so sure, yeah.
- 29:20Speaker 5: And that's the other thing is that it's not so
- 29:22Speaker 5: painless that we can do it without oh.
- 29:25Speaker 3: Yeah, yeah, oh yeah, obviously got it a little bit
- 29:27Speaker 3: right aftermath, isn't isn't the yeah right, uncomfortable, but we yeah,
- 29:31Speaker 3: we numb you very well.
- 29:33Speaker 5: Some patients opt for sedation, which you do, both of
- 29:35Speaker 5: us do in our offices, and and that works really
- 29:38Speaker 5: well kind of the more significant procedures. But most times
- 29:41Speaker 5: we just numb up the area, take out a tooth,
- 29:43Speaker 5: if we need to put the implant in, suture it
- 29:46Speaker 5: all together and graft it and on good shape.
- 29:48Speaker 3: That and for a patient out there, that might be
- 29:50Speaker 3: perspective to get something like that. Something like that has
- 29:52Speaker 3: to give you some peace of mind, you know, you
- 29:53Speaker 3: don't have to worry about Sometimes when I approach things,
- 29:56Speaker 3: I had some cis removed from my head a few
- 29:58Speaker 3: years ago. It was fairly significant amount. There's like eleven
- 30:01Speaker 3: of them, and some of them were kind of big,
- 30:03Speaker 3: but they were benign. There was nothing wrong with them.
- 30:04Speaker 3: They weren't going to cause any problems. They were just
- 30:06Speaker 3: in my way, you know, and they messed with my
- 30:07Speaker 3: hair and stuff like that. So I went in for
- 30:09Speaker 3: the short and I kind of practiced ignorance. I was like,
- 30:12Speaker 3: I don't really know. I'll figure out after the surgery
- 30:14Speaker 3: is over, you know, I'll do what my doctor tells me.
- 30:15Speaker 3: Otherwise I don't really care. Well, I found out that
- 30:18Speaker 3: this was considerably more wound management than what I expected
- 30:21Speaker 3: on the other end of things, and I looked terrible.
- 30:23Speaker 3: I wasn't real sure exactly how bad it would be,
- 30:26Speaker 3: but I looked like I'd been through something very bad
- 30:29Speaker 3: and the wound recovery from that, and I got this
- 30:32Speaker 3: big package on how to take care of the wounds
- 30:34Speaker 3: and all this stuff. You know, I just wasn't expecting that. So,
- 30:37Speaker 3: although there was a benefit to it that I had
- 30:39Speaker 3: zero anxiety leading up to the surgery, because I didn't
- 30:42Speaker 3: do any research, it probably wasn't the smartest I did.
- 30:45Speaker 3: To be honest with you should probably educate yourself a
- 30:47Speaker 3: little bit more on that. However, that being said, the
- 30:49Speaker 3: peace of mind on something like this, there is no
- 30:51Speaker 3: wound care necessarily afterwards. It's going to be a pretty clean,
- 30:53Speaker 3: neat and tidy operation typically is.
- 30:56Speaker 5: And for instance, I remember years ago a patient came
- 31:01Speaker 5: in and we had to do two implants on which
- 31:03Speaker 5: is significant enough, I think in a similar area of
- 31:07Speaker 5: the mouth. And she had been through an implant procedure
- 31:12Speaker 5: years prior where she went to the surgeon and I
- 31:15Speaker 5: think they may have even had her hospitalized or inpatient
- 31:18Speaker 5: out patient type of thing, and that was under a
- 31:22Speaker 5: different protocol. And she came in to my office, I
- 31:26Speaker 5: mean in and out in an hour, maybe two hours,
- 31:29Speaker 5: all said and done, and she was like, Wow, this
- 31:32Speaker 5: was so easy, Like how I cannot believe that when
- 31:36Speaker 5: I had this done before, it was kind of made
- 31:38Speaker 5: such a big deal. You made such a big to
- 31:40Speaker 5: do about So yeah, we've we've learned a lot, We've
- 31:43Speaker 5: been able to make things a lot more kind of
- 31:45Speaker 5: use her friendly for patients.
- 31:46Speaker 6: Now and doing well in my office. I'm kind of
- 31:51Speaker 6: the I love to do guided surgery because I like
- 31:54Speaker 6: to do the surgery at home in my office on
- 31:57Speaker 6: a computer screen where I can actually place the implant
- 31:59Speaker 6: on a computer screen, send it off to our lab
- 32:02Speaker 6: that can fabricate a guided a surgical guide that allows
- 32:06Speaker 6: me to place the endplant. It takes fifteen to twenty
- 32:09Speaker 6: minutes per implant. It's just so quick. And because of that,
- 32:12Speaker 6: I have to make a little small incision and you
- 32:14Speaker 6: do this all the time as well, just making a
- 32:16Speaker 6: smaller incision into the gum, which feels like a pizza
- 32:19Speaker 6: burn as opposed to cutting the gums open and I
- 32:21Speaker 6: hate to use that term and creating a flap. It
- 32:24Speaker 6: just tends, to my opinion, heal quicker and be less painful.
- 32:28Speaker 4: So yeah, for sure.
- 32:29Speaker 6: Yeah, So the smaller amount of gum tissue you have
- 32:31Speaker 6: to remove as long as you know where you're placing it.
- 32:33Speaker 6: You can't always do that. Sometimes you do have the flap,
- 32:36Speaker 6: but it tends to make the procedure go quicker and
- 32:39Speaker 6: the healing go quicker as well.
- 32:40Speaker 3: Briefly, before we move on, I think that you addressed
- 32:43Speaker 3: at a moment ago. I want to make sure our
- 32:44Speaker 3: caller got the information though from the four one or
- 32:46Speaker 3: a three zero four to four to one zero. I
- 32:48Speaker 3: need a tooth in the back left pulled and an
- 32:50Speaker 3: endplant placed. Can I be sedated for that? I have
- 32:52Speaker 3: severe dental anxiety, Yeah.
- 32:53Speaker 5: For sure, and that's why we offer that in our offices.
- 32:56Speaker 5: Best things to come in, have the discussion with one
- 32:59Speaker 5: of our offices and and you know, make the selection
- 33:04Speaker 5: to have sedation performed. And that works very well to
- 33:08Speaker 5: have sedation done. Get a tooth out. Now, if you
- 33:12Speaker 5: have the tooth out, many times we can put the
- 33:14Speaker 5: implant in immediately that time. Sometimes we cannot. You also,
- 33:18Speaker 5: not to get too detailed, but a dental implant is,
- 33:22Speaker 5: like doctor Henley said, well, yeah, it's it's kind of
- 33:25Speaker 5: like a screw, a kind of big fat screw in
- 33:27Speaker 5: a sense, and it's cylindrical. Whereas when we take out teeth,
- 33:31Speaker 5: especially back tooth, as a caller saying, it actually has
- 33:34Speaker 5: two roots on it, maybe three roots, and the shape
- 33:38Speaker 5: of the void that it came out of is a
- 33:40Speaker 5: bit different than what the implant needs to go into.
- 33:43Speaker 4: So we have to reshape that area, that socket.
- 33:46Speaker 5: And a lot of times we can get enough bone,
- 33:48Speaker 5: have enough bone to grip our implant at the time
- 33:51Speaker 5: that we take the tooth out. Other times we have to,
- 33:54Speaker 5: you know, take the tooth out. There's just not enough
- 33:56Speaker 5: bone there to hold the implant, so we have to
- 33:58Speaker 5: put grafting material in and come back a few weeks later.
- 34:01Speaker 5: So that's the one caveat to it.
- 34:03Speaker 3: It's a great information as well a caller. If you're
- 34:05Speaker 3: interested in this and you want to give the Greeb
- 34:07Speaker 3: Dental Group a call, it's one hundred Great care, ONEYE
- 34:09Speaker 3: hundred great care. Iss the phone number. They'll get you
- 34:10Speaker 3: to the closest location to you. And there's a bunch
- 34:12Speaker 3: of them Pocus how Charles and Cross Lane, Saint albans
- 34:14Speaker 3: Tays Valley in Kanall City. So Greb Dental Group can
- 34:17Speaker 3: help you out. Have about three and a half minutes
- 34:19Speaker 3: left here. We got to get some high impact impact
- 34:22Speaker 3: dental impact talk out there. Oh, I got it.
- 34:24Speaker 6: So I want to discuss two patients yesterday and kind
- 34:27Speaker 6: of the dichotomy of how they came in and I
- 34:31Speaker 6: kind of after just discussing with them and giving them recommendations,
- 34:34Speaker 6: they flipped what they wanted to do. And so one
- 34:37Speaker 6: patient told me, for example, I kind of mentioned this
- 34:39Speaker 6: earlier that all her lower teeth just had to come out,
- 34:42Speaker 6: and I said, well, let's take a look. I don't
- 34:44Speaker 6: know for sure, but let's just take a look. And
- 34:46Speaker 6: by the end of that appointment, after taking interal photos,
- 34:49Speaker 6: after doing a digital scan and showing her on the
- 34:51Speaker 6: computer screen, after pulling up the X rays on the
- 34:53Speaker 6: TV in front of her and just showing her everything.
- 34:56Speaker 6: She kind of was not convinced, but just saying, oh,
- 34:58Speaker 6: I have other options. I don't have to take out
- 34:59Speaker 6: the On the flip side, I had a patient where
- 35:02Speaker 6: it was a broken tooth and I removed the broken piece,
- 35:05Speaker 6: and I said, we probably should remove this tooth. It's
- 35:08Speaker 6: it's kind of deep. It's right below the gun level,
- 35:11Speaker 6: not below the bone level yet, but it's it's tricky,
- 35:13Speaker 6: and we call that not maybe a non restorable tooth
- 35:16Speaker 6: where you probably shouldn't restore it. But this patient was
- 35:19Speaker 6: adamant that she wanted to save it, and so I said, well, okay,
- 35:23Speaker 6: let's save it.
- 35:24Speaker 3: Let's do it.
- 35:24Speaker 6: If you are adamant that you want to save it,
- 35:25Speaker 6: we have to go through this whole protocol of doing
- 35:27Speaker 6: a root canal and then putting a crown on it.
- 35:29Speaker 6: But if you're adamant, as I mean, as long as
- 35:31Speaker 6: you understand that this might not be the best option,
- 35:33Speaker 6: then you know, so we always have options. I sometimes
- 35:36Speaker 6: I go in thinking we for sure have to take
- 35:38Speaker 6: this out, and then we don't. Other times the patient
- 35:40Speaker 6: tells me we have to take this out, and I
- 35:41Speaker 6: convince them, no, we don't have to take it out.
- 35:43Speaker 6: And so to that caller out there, we need to
- 35:45Speaker 6: have a full discussion. Right, we don't know your situation,
- 35:48Speaker 6: and that's why we're here to listen. I think the
- 35:49Speaker 6: number one thing we do as dentist is listen to
- 35:52Speaker 6: make sure that we take care of you, because each
- 35:54Speaker 6: individual patient has their own concerns and their own things
- 35:56Speaker 6: that they need to discuss.
- 35:58Speaker 3: The one thing you guys do a great job of
- 36:00Speaker 3: it comes over on the radio, and of course I've
- 36:02Speaker 3: been to doctor stevens Off as many times is you
- 36:05Speaker 3: don't assume the patient has any particular amount of knowledge.
- 36:07Speaker 3: And sometimes for people in a medical field, I think
- 36:09Speaker 3: sometimes it's hard for you to segment your knowledge from
- 36:13Speaker 3: what is common knowledge and things that's common that you
- 36:15Speaker 3: were dealing every day. Sure, like the super advanced, I'm
- 36:17Speaker 3: sure the regular patient isn't going to know that. But
- 36:19Speaker 3: also some relatively common stuff in your field is even
- 36:23Speaker 3: very obscure to somebody that's not in your field. And
- 36:26Speaker 3: you guys do a great job with that communication and
- 36:28Speaker 3: getting over that barrier so people kind of understand the
- 36:31Speaker 3: full spectrum of what they're going into there. And I
- 36:32Speaker 3: think that's very beneficial at any medical office, especially because
- 36:36Speaker 3: nobody likes that that gray area. And what and something
- 36:39Speaker 3: that people don't like to do is ask questions. You know,
- 36:40Speaker 3: they don't like they think they're questioning authority, or they
- 36:43Speaker 3: don't they think they're going to look dumb or you
- 36:44Speaker 3: know whatever. It is. Sure you guys do a great
- 36:46Speaker 3: job of getting that information out so folks can make
- 36:48Speaker 3: an educated opinion. To make that give and take of
- 36:51Speaker 3: asking questions and getting answers a very natural process and
- 36:54Speaker 3: I think that folks really appreciate it.
- 36:55Speaker 5: Oh, we appreciate hearing that for sure. Becaus something we
- 36:57Speaker 5: strive to do. And that's another part of all that
- 37:00Speaker 5: technology that we have. Doctor Henley was describing, you know,
- 37:03Speaker 5: showing being able to educate the patient and uh, you know,
- 37:05Speaker 5: co discovering with them what's best for their for their needs.
- 37:08Speaker 5: But we we have a lot of different other implant
- 37:10Speaker 5: options we didn't get to today. Uh we talked about
- 37:13Speaker 5: the the ventures and attaching those and kind of scratching
- 37:17Speaker 5: the surface from that area, but there are more ways
- 37:20Speaker 5: we can make it so you have teeth that stay
- 37:22Speaker 5: in and don't come out. We might have to do
- 37:23Speaker 5: a part too, so we'll have to you know, delay
- 37:25Speaker 5: that maybe for our next show and put a little
- 37:27Speaker 5: bookmark in it now. But yeah, it's been it's been
- 37:30Speaker 5: a great discussion and there's there's always more to talk
- 37:32Speaker 5: about with implants.
- 37:33Speaker 3: Great information from the Greed Dental Group. As always the
- 37:35Speaker 3: fourth Tuesday of every month, the right here on five
