- 0:00Speaker 1: This is India Today Podcasts. Hair loss is perhaps one
- 0:06of the major reasons why people are stressed today. It
- 0:10is perhaps one of the reasons why people are also
- 0:12anxious and depressed in certain cases.
- 0:14Speaker 2: Hair loss means hair fall and hair thinning. In most cases,
- 0:20hair fall is completely recoverable. That hair thinning part, after
- 0:26some time, if you don't get it treated, then it
- 0:29results into a permanent hair loss. You must have seen
- 0:35boys' hair go from the front to a U-shaped pattern.
- 0:40Speaker 1: Male pattern balding.
- 0:42Speaker 2: Exactly. There are girls whose central parting gets widened up
- 0:47or thinning of the ponytail. That is the more worrying sign.
- 0:51Speaker 1: There is some telltale sign. I should meet a doctor.
- 0:54So when you.
- 0:55Speaker 2: Bathe, comb, or brush with oil, you will see hair
- 0:57in your hand.
- 1:04Speaker 1: Somebody comes to you with male pattern balding. What is
- 1:07your recommendation to them?
- 1:09Speaker 2: Two USFD approved drugs for pattern baldness. It's the most
- 1:14essential treatment.
- 1:17Speaker 1: But there are some side effects also. The thing with
- 1:20monoxidil that I know is that as long as you're
- 1:22applying it, your hair will go through this hair fall phase,
- 1:25but then they will come back really nicely. But the
- 1:27minute you stop....
- 1:30Speaker 2: It's very interesting that in 1970s when we discovered minoxidil,
- 1:34it was actually used for blood pressure. Doctors started noticing
- 1:37that the hair is increasing.
- 1:39Speaker 1: Now the internet is divided over whether oiling is good
- 1:42for you or whether oiling is bad for you.
- 1:44Speaker 2: So in our science, oil has no role. So you
- 1:48get oil yourself. If you don't take it for 3-4 days,
- 1:49how does it happen? It becomes oily.
- 1:51Speaker 1: Yeah, yeah. Mix fenugreek, rosemary, and hibiscus. You should add
- 1:56all these things. You talked about rosemary oil.
- 1:59Speaker 2: There is one study that supports rosemary oil. The whole
- 2:05industry of rosemary oil started behind it. Those who come
- 2:10to us have already tried all this.
- 2:13Speaker 1: This is Bhumit who says that shampooing your hair frequently
- 2:18causes hair fall. If you are recommending a shampoo to someone...
- 2:22What do you look for in the ingredient list?
- 2:24Speaker 2: People make a lot of mistake in that. People just
- 2:26see the sulfate-free ad and they will buy the sulfate-free
- 2:29shampoo and then they will come to me for dandruff.
- 2:31And that is why my dandruff practice is increasing day
- 2:33by day because everybody is selling sulfate-free shampoos.
- 2:36Speaker 1: Everybody I know these days has a hair color on.
- 2:39What is a thing that should not be in your
- 2:41hair color?
- 2:42Speaker 2: Definitely, this is a very important question, I would say,
- 2:45especially in Indian context because we like our hair black.
- 2:49We can't live with white hair. Abhi jo regulations hai,
- 2:54that is banned ammonia completely. But there's another molecule in
- 2:58the colours, that is.
- 3:04Speaker 1: Hello and welcome to Doctor vs Internet. I'm Sonal Mehrotra
- 3:07Gapoor and welcome to our new studio. And yes, we
- 3:10listened to all your comments and all your feedback. And
- 3:12you guys said, what is this red background? To humne
- 3:15wo change kar diya. Hum aapke liye ek naya studio
- 3:17lekar aaye hain. Aur ek bahut hi behtereen guest ke saath.
- 3:21And on our topic... which is again on popular demand.
- 3:25Everything to do with your crowning glory. I'm talking about hair.
- 3:29Say hello to Dr. Ankur Sareen. How are you?
- 3:31Speaker 2: I'm good. How are you?
- 3:32Speaker 1: Very well. So when it comes to hair, I have
- 3:36a lot of questions. So this is going to be
- 3:38a very long podcast. But let me start by asking
- 3:43you this. Hair loss is perhaps one of the major
- 3:48reasons why people are stressed today. It is perhaps one
- 3:51of the reasons why people are also anxious and depressed
- 3:53in certain cases. It is a very serious problem. It
- 3:56has repercussions all over. And hair loss can happen because
- 4:01of various reasons. But I want you to address permanent
- 4:04hair loss as one. We understand that if you lose
- 4:07too much weight dramatically, if you're on GLP-1, postpartum, season change,
- 4:11in everything, You have seasonal hair fall, but all that
- 4:15hair actually comes back. But permanent hair fall is happening
- 4:19to you. First, how will I know that I will
- 4:22have to go and meet a dermatologist or a trichologist?
- 4:25How to tell?
- 4:26Speaker 2: You used a very good term, hair loss. It's actually
- 4:31not hair fall because some people call it hair fall.
- 4:34So hair loss means there can be hair fall and
- 4:37hair thinning as well. Most cases, the hair fall, which
- 4:42we scientifically call telogen effluvium, is completely recoverable. The other
- 4:48one is the hair thinning part. That hair thinning part,
- 4:53after some time, if you don't get it treated, then
- 4:56it results into a permanent hair loss. And then there
- 5:01are some other medical conditions, which are not that common.
- 5:06But they can also cause hair loss. Permanent hair loss.
- 5:10We call them scarring alopecia. Meaning scarring hair loss. So
- 5:15the most common is hair thinning. And the hair loss
- 5:20due to the hair thinning. Which we also call androgenetic alopecia.
- 5:23You must have seen boys' hair. it becomes a U-shaped
- 5:29pattern from the front.
- 5:30Speaker 1: Male pattern baldness.
- 5:31Speaker 2: Male pattern baldness, exactly. Or there are girls whose central
- 5:35parting gets widened up and girls will say a little
- 5:39bit reduction of hair from this part or thinning of
- 5:44the ponytail. That is the more worrying sign. If hair
- 5:49is falling, it is not that worrying because that is
- 5:51easily recoverable. But when the thinning starts, then you know
- 5:56that you're moving towards a permanent hair loss.
- 5:59Speaker 1: Okay, but how will you understand the difference in this?
- 6:02And in both cases, is the hair going from the
- 6:04roots or is it a case of hair damage also,
- 6:07which is why thinning is happening?
- 6:09Speaker 2: So hair thinning is mostly due to a hormone in you.
- 6:13And that hormone is called DHT, dihydrotestosterone, which is more
- 6:19in men because it is made from testosterone, which is
- 6:22in your blood and when it reaches the hair follicle,
- 6:26there it meets an enzyme called 5-alpha reductase that converts
- 6:30it into dihydrotestosterone and this dihydrotestosterone is responsible for To
- 6:36shorten the growth phase of the hair. Which is called
- 6:39the anagen phase. In science, it's called anagen phase. When
- 6:42our growth phase gets smaller, our hair gets thinner. So,
- 6:46when you sleep or wake up, your hair follicle also
- 6:49sleeps or wakes up. So, when it's time to wake up,
- 6:51it's called anagen phase. Anagen means growth phase. It means
- 6:54it's active. It's growing. It's getting metabolically thick. That is
- 7:00what we want. We want all our hair to be
- 7:02in the anagen phase. But, If it's a natural thing,
- 7:06then the natural thing needs rest. It will get tired. So,
- 7:09the hair follicle needs rest. When it gets dark, it's
- 7:13called telogen phase. So, telogen phase is the resting phase.
- 7:17And there's a phase in between which is called cataract phase.
- 7:19It's not that important. It's very small. Telogen phase is
- 7:22the resting phase. And what happens in this is that
- 7:24the hair is resting on your scalp. If you pull
- 7:28it easily, it will come out. At a given point
- 7:32of time, 90% of the hair are in the Growth phase.
- 7:36And 10% are in the resting phase. When this ratio changes, 60-40, 50-50,
- 7:41so when the resting phase hair grows, when you bathe, comb,
- 7:48brush with oil, you will see hair in your hand.
- 7:53So then we say that the patient is having terogen effluvium,
- 7:55you have to say hair is falling. Now this is
- 7:58easier to recover. But the other one that I said,
- 8:01where there's thinning of the hair shaft, in which the
- 8:04size of your hair follicle is shrinking, so you're moving
- 8:08towards a permanent hair loss.
- 8:11Speaker 1: Is there any telltale sign that you're in the phase
- 8:16of hair thinning and you should meet a doctor?
- 8:19Speaker 2: For girls, a lot of them notice widening of the partition.
- 8:25Before that, you can find out by seeing how your
- 8:27parents are. Because genetics is from there. So, if your parents,
- 8:33father has frontal balding, crown area balding, your mother has
- 8:37central parting widened hair. So, you know that you are
- 8:41at a higher risk than the general population for this
- 8:45kind of hair thinning. So, you can preemptively take some steps.
- 8:49You don't have to worry. People get very nervous. Already,
- 8:53people are very sensitive about hair.
- 8:54Speaker 1: Yeah, yeah.
- 8:55Speaker 2: But this is one way that you can judge. The
- 8:58second is, like I said, ponytail thinning. Patients will come
- 9:02and say, doc, ponytail is not that long anymore. Hair's
- 9:05length is not growing as much as it was before.
- 9:08Because of the shortening of the growth phase of the hair,
- 9:10which is called anagen phase. Which can be from 2
- 9:14years to 6 years. Now your hair is till here.
- 9:17Now someone's, maybe your growth phase is 4 years. If
- 9:20someone's hair is 6 years, which is the maximum usually,
- 9:24maybe their hair will come till the knees. Okay. And
- 9:27the one who will have 2 years, his hair will
- 9:29come further than this.
- 9:30Speaker 1: So what will define my hair?
- 9:32Speaker 2: This is genetics. Genetics, nutrition. But see, genetics has the
- 9:37major role. You cannot change genetics by changing your nutrition
- 9:41and lifestyle, right? But good genetics and bad lifestyle can
- 9:46be still bad hair.
- 9:48Speaker 1: Yeah.
- 9:49Speaker 2: So it's like that.
- 9:50Speaker 1: So when people say they are blessed with good hair,
- 9:52they actually mean that, right?
- 9:53Speaker 2: Definitely.
- 9:54Speaker 1: So all these viral videos on Instagram that I applied
- 9:57this concoction every day, oil and my hair has become
- 10:046-12 inches. All of that is not true. They probably
- 10:06already had good genes and they've just done the right
- 10:10thing by supporting that.
- 10:12Speaker 2: Right. So there is genetics and there is epigenetics, right?
- 10:15So how the environment is playing a role In supporting
- 10:20or not supporting that genetics, definitely that is important. And
- 10:23you talked about rosemary oil, so there is one study
- 10:27which supports rosemary oil for the hair growth. And behind that,
- 10:32the whole industry of rosemary oil or rosemary water has started.
- 10:38Speaker 1: Okay, so we have established that largely how your hair is,
- 10:42when it will fall, in which pattern it will fall,
- 10:45these define your genes. A lot of it is not
- 10:47in your hand. A good lifestyle, good protein intake, good
- 10:52stress levels can really amplify it or not. But that
- 10:56range will remain the same. So if I understand that
- 10:59my hair thinning has started, And I went to the doctor.
- 11:02What will you be able to do first? Will you
- 11:05prescribe me tests? Will you tell me treatments? Will you
- 11:07tell me to correct my diet? What will you do?
- 11:10Speaker 2: So, first thing is the history. So, when you come
- 11:13to me, I have to ask you some questions. How
- 11:15you feel about it? Since when it is happening? That
- 11:18gives us a good idea. Then the second thing is
- 11:21our examination. So, we have to examine the patient. Physical examination.
- 11:25That how the hair is. We commonly do a hair
- 11:28pull test. I like to do it a lot. In
- 11:30which we take around 60 hair strands and give it
- 11:33traction from the base to the top. And see how
- 11:38much hair is in our hands. So a lot of
- 11:41people get to know that hair is falling because if
- 11:44more than 5 hairs come in your hands. So then
- 11:47you know that. Now it's painful. No, it's not that painful.
- 11:52But you can say maybe... 3 or 4 out of 10, yeah.
- 11:56A little bit.
- 11:58Speaker 1: So, you pulled 60. If 4 or 5 are coming,
- 12:00then that's normal.
- 12:01Speaker 2: Yes, yes. If 2 or 3 are coming, then it's normal.
- 12:05But if more than 2 or 3 are coming, which
- 12:07are very common, then it is not normal. And also,
- 12:10we go by what the patient is saying. If the
- 12:12patient is saying that the hair fall is significantly increased
- 12:17than what it was before, then we like to proceed
- 12:21even if the hair pull test is negative, right? After that,
- 12:26we can narrow down the diagnosis once we have taken
- 12:29the history and physical examination. The next step is to
- 12:32recommend some tests. Some normal blood tests that can be done.
- 12:37And we can check the nutrition profile. These days, I've
- 12:41also started to tell patients that put your diet on
- 12:44these calculators that are available on the internet. Because that
- 12:48gives them an idea of how much protein they're having.
- 12:51Speaker 1: Protein! Protein is like everything now. Even for your hair.
- 12:56Speaker 2: Right, absolutely. Or one gram per kg. is a lot
- 13:00of India is vegetarian and a lot of them are
- 13:02not having that much amount of protein, right? So that
- 13:06is very important. So we get an idea from this,
- 13:09like what is going on.
- 13:10Speaker 1: Can you tell us some tests? Males and females, blood
- 13:13tests that they should get done if they want to
- 13:15understand the.
- 13:15Speaker 2: Source of their hair loss. So if you want to
- 13:17go to a dermatologist, so if you want to get
- 13:18a test beforehand done, you can do a CBC. you
- 13:21can do a vitamin D. CBC will tell you the
- 13:24hemoglobin levels and it will give you a picture of...
- 13:27Iron deficiency will also... Iron deficiency will be caught there.
- 13:29It will give you the size of the red blood cells.
- 13:32So if the PCV is down, you know that there
- 13:33is iron deficiency. If the nucleus of the red blood
- 13:36cells is bigger, then the doctor will know it is
- 13:38vitamin B12 deficiency. So a good idea one can make
- 13:41out from the CBC. Then we have the vitamin D,
- 13:45which is again very low in a lot of patients.
- 13:47So we need to supplement that. Then vitamin B12.
- 13:51Then there's TSH, thyroid. If you feel weakness, you get
- 13:54tired quickly, then you should definitely get a thyroid test done.
- 13:59And ferritin levels, that is the iron stores in your body.
- 14:03Those are also very important. And sometimes we want to
- 14:06keep it on the upper end for a better performance. CBC,
- 14:11vitamin D, B12, ferritin and T-acid. These are the five
- 14:14tests that are commonly done. Apart from this, if the
- 14:18hair fall still doesn't stop, then we go into the
- 14:20deeper testing with trace elements like zinc, selenium, manganese, chromium,
- 14:25which our body needs in trace amounts. And sometimes these
- 14:28also become deficient. Magnesium is deficient in many people. Zinc
- 14:32is deficient in many people. So we supplement them too
- 14:37and that improves the patient. Then there are some advanced
- 14:42tests like scalp biopsy. If we doubt as a doctor,
- 14:45we are not able to diagnose with just these steps,
- 14:49then we can send a small piece of the skin
- 14:53of the scalp to a dermatopathologist, the one who looks
- 14:57under the microscope and tells us that this could be ABC.
- 15:01And then we proceed accordingly. These are the most common
- 15:04tests that are done. Apart from that, for research purposes,
- 15:09we can check the diameter of the hair. If the
- 15:12diameter of the hair is decreasing, we take 10 hairs,
- 15:14we see them under the microscope. If we feel that
- 15:16there is variation in the diameter of the hair, that
- 15:20is a sign of the hair thinning because the diameter
- 15:23of the hair is different. So these are the tests
- 15:25that we can do. Yeah.
- 15:27Speaker 1: So for me, the takeaway from this is that if
- 15:29you're iron deficient, if you're B12 deficient, if you're vitamin
- 15:33D deficient, or if you're not taking the right amount
- 15:36of protein, you will have hair fall.
- 15:38Speaker 2: Definitely.
- 15:39Speaker 1: And a lot of the fix actually lies.
- 15:41Speaker 2: In what you're eating. Yes.
- 15:42Speaker 1: Okay. And what the fuel you're giving to your body.
- 15:44And that's one.
- 15:45Speaker 2: Exactly.
- 15:46Speaker 1: A lot of this hair fall that has now happened,
- 15:48if it is, say, genetically inclined, it will not regrow, right?
- 15:52So in order to grow that hair again, what are
- 15:55the kind of therapies which are out there in the market?
- 15:57Some very popular ones we see on Instagram as well.
- 16:00And there's a lot of misinformation. But I'm sure not
- 16:05like a right therapy needs the right patient. So how
- 16:07do you make that match? So say somebody comes to
- 16:10you with male pattern balding, what is your recommendation to them?
- 16:14Speaker 2: So if somebody has male pattern balding, first thing first
- 16:18is they have to do something at their home every
- 16:22day to fight the effect of that DHT. Okay. So
- 16:26for them, DHT blockers is the most essential treatment. Okay.
- 16:31So DHT blockers are two types. One is fenasteride and dutasteride.
- 16:36Then there are some injectable options available like bicalutamide, which
- 16:40we can put in the scalp. That is mostly done
- 16:43for females. So fenasteride and dutasteride patients can take at home.
- 16:47They will block their DHT and they have good effect.
- 16:50They are also USFD approved for.
- 16:52Speaker 1: DHT is basically a kind of.
- 16:55Speaker 2: It's a hormone.
- 16:56Speaker 1: Yeah.
- 16:56Speaker 2: It's a hormone. It's a more potent form of testosterone
- 16:59that is responsible for shortening of the growth phase of
- 17:03the hair.
- 17:03Speaker 1: But there are some side effects also.
- 17:05Speaker 2: So, finasteride and dutasteride because they reduce the DHT. So, theoretically,
- 17:10in your body, the effect of female hormones can increase
- 17:13more for a male. So, you can get breast enlargement. Okay.
- 17:19You can get loss of libido that is reduced desire
- 17:21to have sex.
- 17:23Speaker 1: Okay. Okay.
- 17:24Speaker 2: Now, the breast enlargement is very rare, super rare. Like
- 17:27in five years of my practice, I've had one that
- 17:31is of my clinic practice. One patient of breast enlargement.
- 17:38Loss of libido that is due to have sex, that
- 17:40is much more common. Also, I think it gets into
- 17:44patient's head. But the biggest meta-analysis, that is, if there
- 17:47are 50 studies, if we do a meta-analysis of all
- 17:49of them, what data came out after combining all the studies,
- 17:51in that it was found that 4% of people but
- 18:01still because people read it and you know it is
- 18:03such a psychological thing yeah having the sexual drive or
- 18:07maybe if they're having something else they might blame it
- 18:09on the drug so the overall percentage i would say
- 18:13in my practice would go up to 10 15 patients
- 18:17might say that doc i don't want to take it
- 18:18because i'm having uh sexual side effects oh i see
- 18:22and about women So for women, the sexual side effects
- 18:27could be there that they also might feel a reduced
- 18:31desire to have sex. But other side effects are not
- 18:35very concerning for women. If you have a history of
- 18:39breast cancer, a strong breast cancer in the family, like
- 18:41your first degree relative, then we avoid giving DHT blockers
- 18:46or we can give it and you know, we can
- 18:47ask the patient to be a bit you know, get
- 18:50her breast examined yearly.
- 18:52Speaker 1: Get your scans more regularly done. So this doesn't sound
- 18:56like a good option.
- 18:58Speaker 2: See, all medications have some side effects. Is there like
- 19:03no natural way to block DHT then? I'm the wrong
- 19:07person to ask for natural treatments. But yeah, no. See,
- 19:11there is like pumpkin seeds, saw tomato, which have shown
- 19:16in studies that they reduce DHT. But the thing is,
- 19:20when the patient is.
- 19:21Speaker 1: But I'm sure, I mean, I'm hoping this is not
- 19:23that kind of a study where they say, one kilo
- 19:24of food will be fine, but no one will be
- 19:26able to eat that.
- 19:27Speaker 2: Right.
- 19:27Speaker 1: Like, that's just proportion.
- 19:28Speaker 2: People don't... Right. I'm not sure of exact how, what
- 19:32is the amount. But there are some supplements available. And
- 19:35when patients are not going, when patients say, I don't
- 19:38want to take all this, then we do put them
- 19:41on that. But I, as a doctor, I'm not very sure.
- 19:44And I tell this to the patient. okay i'm not
- 19:46giving it to you but i'm not sure if it
- 19:48will work or not yeah so yeah and most of
- 19:51the patients who come to me are at a stage
- 19:53where um they need a proper treatment like an fda
- 19:57clear treatment right yeah yeah so that is what we believe.
- 20:00Speaker 1: In yeah yeah yeah so are there other options because
- 20:03this is i'm like taking it off this is like
- 20:05too much of a risk What is the next treatment available?
- 20:10I mean, we are in 2026. I'm sure there is
- 20:13machinery available. I'm sure there are medicines available.
- 20:17Speaker 2: That can, you know. Now coming back to, we'll go
- 20:20to minoxidil, but because we're talking about DHT blockers. So
- 20:24DHT blockers are now available in the injectable form as well. So,
- 20:29and they're also available with microneedling. So you do microneedling
- 20:33and you put the solution on the scalp and it
- 20:36penetrates more. When you do that, you're just giving the
- 20:40DHT blocker in a limited space on the scalp. So
- 20:44it does not go that much into the systemic circulation.
- 20:48When you take a pill, it goes into your gut.
- 20:49It goes into your legs as much as it goes
- 20:51into your head. So it's all equal. But when you
- 20:55are giving a local shot, local injection, so most of
- 20:58the effect of the drug will be limited to that area.
- 21:03Speaker 1: Yeah. So you will numb the head and you will
- 21:05inject it?
- 21:06Speaker 2: Yes, we'll put a numbing cream and we give shots
- 21:11of DHT blocker. They're called Dutasteride and Biclutamide in female cases.
- 21:16Speaker 1: Okay. Pretty popular?
- 21:20Speaker 2: It's not actually that popular. It's with only specialized dermatologists
- 21:25and specialized clinics that you'll get these services. It's not
- 21:28very popular. It's not still out there so much on
- 21:30the social media. Now it's out there.
- 21:34Speaker 1: No, that is good to know. Also, can I just
- 21:37use this time to say that if you go to
- 21:38any salon and they are promising anything to you, please don't.
- 21:42If you get any home treatment which is promising to
- 21:44do a laser for you or bring your hair back,
- 21:47please don't. Please always, always go to a certified doctor
- 21:51and let them only operate from you. We've had enough
- 21:54horror stories coming out from different parts of the country.
- 21:57Where a doctor has done something or a dermat has
- 22:00gone in to operate and, you know, all sort of
- 22:02stuff has happened. So, we've got to be very, very careful.
- 22:04Speaker 2: And these are Schedule X drugs, not even H, which
- 22:08is the minor form of prescription. X is like strongly regulated.
- 22:12You don't get these drugs outside India. Doctors also think
- 22:18five times before writing. Because they have so much documentation
- 22:21they have to do. Yes.
- 22:23Speaker 1: Yes. But if you are taking it, applying it locally
- 22:26on your head, like injecting it there, in how much
- 22:29time do you see hair reversal?
- 22:31Speaker 2: Around 4 months onwards, you start to see. Because the
- 22:34telogen phase, I told you, that is of 8 weeks. So,
- 22:37these medicines don't work on the telogen phase. It has
- 22:39left its growth phase. So, it has to shed. So,
- 22:43it is very interesting that after 8 weeks, the result
- 22:47will start coming to you. Even though the medication will
- 22:49come on the first day. So, that is why we
- 22:51tell patients that whenever you are taking treatment for hair fall,
- 22:53you have to wait for 2 months. After that, you
- 22:56have to judge whether the treatment is working or not.
- 22:59Speaker 1: Right, right. I think, and this is the phase in
- 23:02which a lot of people actually fall off track.
- 23:04Speaker 2: Yes.
- 23:04Speaker 1: And because, you know, we all know about this one product,
- 23:09which is on everybody's mind, monoxidil, right?
- 23:12Speaker 2: Yes.
- 23:12Speaker 1: Let's talk about monoxidil.
- 23:14Speaker 2: Yes, yes.
- 23:14Speaker 1: There is so much information, or should I say there
- 23:18is so much misinformation about monoxidil. Because... The thing with
- 23:22monoxidil that I know is that as long as you're
- 23:24applying it, your hair will go through this hair fall phase,
- 23:28but then they will come back really nicely. But the
- 23:30minute you stop.
- 23:32Speaker 2: Yes, it goes back.
- 23:33Speaker 1: Yeah. So is that true? Is that how it goes?
- 23:36And does that mean that people who are on monoxidil
- 23:38have to keep taking it all their life?
- 23:40Speaker 2: Okay. So let me tell you a little bit about monoxidil, yeah?
- 23:43So it's very interesting that in 1970s, when we discovered monoxidil,
- 23:46it was actually used for blood pressure, right? So doctor
- 23:50used to give the medication to reduce the blood pressure.
- 23:53It is a potassium channel agonist. So it dilates the
- 23:57blood vessel by working on that potassium channel. So this
- 24:02is what we were using as an anti-hypertensive. Doctors started
- 24:07noticing that the hair is increasing. okay and later on
- 24:10we discovered better drugs for hypertension which we can use
- 24:13at a lower dose and with the lesser side effects
- 24:16so we stopped using minoxidil and now minoxidil surfaced resurfaced
- 24:23in the last few years and a lot of different
- 24:25oral minoxidil brands have come up in.
- 24:27Speaker 1: The market right does that mean that people who are
- 24:30hypertensive should be careful about taking this drug No.
- 24:33Speaker 2: Actually this will reduce the blood pressure. Yeah, they have
- 24:36to let their doctor know. Yeah, so that they can
- 24:38reduce the dosage of the other drug if they're taking.
- 24:42But this is only mostly for the oral minoxidil. Yeah, okay.
- 24:45So mostly what we are using or most 80-90% of
- 24:49the people are using the topical minoxidil, application minoxidil. So
- 24:54minoxidil works by dilating the blood vessel. It increases the
- 24:57blood flow to the hair follicle because of which the
- 25:00anagen phase, the growth phase of the hair starts to
- 25:04increase and that leads to thicker hair. So it is
- 25:07also used to improve your hair texture and quality because
- 25:12it generates thicker hair even if you don't have the
- 25:14signs of hair androgenetic alopecia. Actually, a lot of dermatologists
- 25:20also use it just for, you know, better hair. Okay.
- 25:22Because they understand the science behind it.
- 25:23Speaker 1: Because they have to have great skin and good hair. Otherwise,
- 25:26no patients.
- 25:27Speaker 2: Otherwise, you're judged. Otherwise, you're judged online specially these days.
- 25:31Speaker 1: Yeah.
- 25:31Speaker 2: Yeah.
- 25:32Speaker 1: Okay. So, you use it?
- 25:35Speaker 2: I am using LED treatment these days. I'm off minoxidil. Yeah,
- 25:39but I've used it in the past.
- 25:41Speaker 1: Okay.
- 25:41Speaker 2: Yeah.
- 25:41Speaker 1: Okay.
- 25:42Speaker 2: Yeah.
- 25:42Speaker 1: I'm going to get to your routine also in a bit.
- 25:45I will want to understand what is it that you
- 25:48are doing.
- 25:48Speaker 2: Yes.
- 25:48Speaker 1: So that is minoxidil or there is more to the
- 25:51story because there are side effects to minoxidil as well, right?
- 25:55Speaker 2: Yes. So minoxidil side effects are actually very mild. Okay.
- 25:59So if somebody is not willing to take DHT blockers,
- 26:03so that's what I tell them that you go for
- 26:05minoxidil for now and we'll talk about DHT blockers later.
- 26:08Some people see very good result with minoxidil. So there
- 26:12is an enzyme in us that has to activate minoxidil
- 26:16once we put it. So if that enzyme is in
- 26:18higher concentration, so you will see better results with minoxidil.
- 26:23So two people using same percent of minoxidil can have
- 26:27different results. So for everybody, the treatment has to be
- 26:31very individualized. And minoxidil side effects are just, you know,
- 26:36facial hair growth, Maybe a little bit dizziness if you
- 26:39start because it is dilating the blood vessels. So your
- 26:44blood pressure will go down. So your heart may take
- 26:47a little bit more time to adjust to that pressure changes.
- 26:50Especially it's more common in girls and who start on
- 26:53a higher percentage straight away. This can easily be managed
- 26:57by starting at a lower percentage with a lesser amount
- 27:00and then your body get used to it. So then
- 27:03you will not have those side effects. But the side
- 27:05effects are minor for minoxidil. And I think there is
- 27:09a negative campaign against it on the internet by so
- 27:14many companies that are making non-minoxidil type stuff.
- 27:20Speaker 1: But you would still say that minoxidil is the holy grail?
- 27:23Speaker 2: There are only two US FDA approved drugs for patent baldness.
- 27:27One is minoxidil and other is the DHT blockers. So definitely,
- 27:31these are the two go-to drugs. I don't say that
- 27:34the others don't work. But when you're in trouble, you
- 27:38go to the most trusted solution. Yes.
- 27:41Speaker 1: Yes. These days I'm seeing a lot of reels online
- 27:44on better absorption of minoxidil or any topical cream for
- 27:47that matter. They do this needling, right?
- 27:50Speaker 2: Derma roller.
- 27:50Speaker 1: Derma roller.
- 27:51Speaker 2: And tretin cream they use around with it.
- 27:52Speaker 1: Yeah, so they're using it so that better absorption happens.
- 27:55I've seen some horror stories as well. People sort of
- 27:59hurting themselves, injuring themselves and it can lead to infection.
- 28:03It can go really, really wrong. Do you prescribe these rollers?
- 28:08Speaker 2: So, I don't prescribe the rollers in my regular OPD.
- 28:11But patients do ask these days that, doc, can I
- 28:14do a derma roller at home?
- 28:16Speaker 1: Yeah, because it's available on Blinkit.
- 28:17Speaker 2: Yes, yes, it is available.
- 28:18Speaker 1: In 10 minutes, you can have it and you've seen
- 28:20one reel, you want to try it out and you
- 28:22end up doing like more damage than good, right?
- 28:24Speaker 2: So, I say if you want to, it's not necessary
- 28:27at all. And you can go to a dermatologist. But see,
- 28:30not everybody can afford dermatological treatments, right? So for them,
- 28:35I say that you can do 0.25 mm, which is safer.
- 28:38So 0.25 mm and I teach them how to do it.
- 28:40So you have to do it without the pressure and
- 28:44just mild redness is the end point. Make sure that
- 28:49it does not bleed. So that is if it is bleeding,
- 28:52then it has to be done in the dermatologic office
- 28:54if you want to do it at that level. So
- 28:57at different levels, you are actually doing different things. So
- 29:01proper collagen stimulation will come at around 1 mm. More
- 29:04than 0.5, definitely. But at home, you can do 0.25,
- 29:07create a little bit of inflammation that gaps in the epidermis,
- 29:12that is the skin, so that the product gets better absorption.
- 29:18Speaker 1: You sound scary, but okay. I'm sure you have a
- 29:21lot of patients who come to you with... Dermaroller injuries
- 29:25or something or you have tried all this.
- 29:27Speaker 2: Yeah, definitely. Dermaroller injuries. I have had patients who have
- 29:30had scarring dermaroller injuries as well. Then there is a
- 29:33common problem called tram track appearance. You start seeing dots
- 29:38on the dermaroller. Yeah, yeah. a lot here to increase
- 29:42the hairline. So here on their forehead, dots are visible. So, yeah,
- 29:49it's something that you need to have a lot of knowledge.
- 29:52Speaker 1: Are there any other tools that can be used, say,
- 29:55for regular usage at home, which you think are safe
- 29:58and can help people also?
- 30:00Speaker 2: LED caps recently got FDA clearance. So they are one
- 30:04step below the evidence level of this one, DHT blockers
- 30:09and minoxidil. So because it got FDA clearance, doctors have
- 30:14also like now started prescribing it a lot. There are
- 30:18LED treatments that you can take in the clinic, which
- 30:21is a red light LED, which is around the wavelength
- 30:23of 630 nanometer. This works by improving the extracellular matrix,
- 30:29which is the collagen around your hair follicle. It improves that.
- 30:34It forms new blood vessels in it, reduces inflammation, increases
- 30:38growth factors, which improves the overall hair follicle's quality, its thickness. So,
- 30:47LED capsules just... And the best part, there's no side effect.
- 30:50Absolutely zero. So, 10 minutes, you can wear it, click it,
- 30:55and 10 minutes, you're done. You can watch it, watch TV,
- 30:58and while watching TV, you can do it.
- 31:00Speaker 1: Right, but you said 630 nanometers should be the wavelength, right?
- 31:06Speaker 2: And red light. Yeah. And these should be medical diodes.
- 31:11Medical diodes? Yes, medical diodes. The lights are diodes, so
- 31:15they should be medical diodes. And once you turn on
- 31:18its light, you will know the quality.
- 31:20Speaker 1: Okay, because again, there are a lot of things selling online,
- 31:23but they don't really work.
- 31:24Speaker 2: Yeah, so you have to get one which is a
- 31:26medical device. That will show the real benefit.
- 31:30Speaker 1: So how do I know that what is being sold
- 31:32online is medical grade or not?
- 31:35Speaker 2: So I think the best person to help you here
- 31:37will be your dermatologist. You can go to them and
- 31:40they can recommend you a good LED cap which is
- 31:44medical because they will be knowing that. I have just
- 31:47got one here for you just to show what is
- 31:50the difference in the quality. So this is the one
- 31:52that I am using right now. This is not sponsored
- 31:55by the company. I am just showing this to you
- 31:57organically for your information. So this is what we wear
- 32:01for 10 minutes. So I just wear it. I switch
- 32:03it on and see the power of the LEDs. Yeah. Right.
- 32:06So this is how the cap should be. And it
- 32:08should be 630 to 650 nanometer. And we wear this
- 32:12for 10 minutes. And after 10 minutes, it has an
- 32:16automatic cut off. Yeah. So this is what I am
- 32:19using currently. And I use it every day.
- 32:21Speaker 1: Okay.
- 32:22Speaker 2: Yes.
- 32:22Speaker 1: Put it on and show me how it looks.
- 32:28Speaker 2: I'll spoil my hair for the podcast so maybe some
- 32:31other day the thing with all skincare and hair care
- 32:33is consistency is very important it's not something that you
- 32:36can you know build in a month's time it takes
- 32:40uh if a patient of hair damage comes to us
- 32:42it will take me around six months to one year
- 32:45to uh give them that kind of improvement and i
- 32:48tell them that now our journey has started and you'll
- 32:51be my patient for.
- 32:52Speaker 1: At least a year but you have Essentially saying that
- 32:55if you want to look like as fabulous as people
- 32:59on screen.
- 33:00Speaker 2: Yes. On the big screen really.
- 33:01Speaker 1: It takes work.
- 33:02Speaker 2: They work a lot. That's what you're saying. They work
- 33:04a lot. It takes a lot of work. Starting from
- 33:07their diet to sleep to tracking their parameters. They're obsessed
- 33:12with it. But I'm not sure if everybody wants to
- 33:14do that much because not everybody is in front of
- 33:16the camera. Yeah. But it's good in a way that
- 33:21you're taking good care of yourself. Now, these days, the
- 33:24Gen Zs, they're not even drinking. Like, so many young
- 33:27people come to us and I'm really inspired by them.
- 33:30Speaker 1: Yeah.
- 33:31Speaker 2: We used to drink more in our college and now
- 33:34they're coming and they're saying, dog, I don't drink. No,
- 33:37you know, late nights.
- 33:38Speaker 1: Listen, they're doing other stuff, but.
- 33:42Speaker 2: But now things are changing. Now they want to do
- 33:46longevity and they want to live longer. They want to
- 33:49do high rocks and all those things, right? Yeah. So
- 33:52you need a very good level of fitness to do
- 33:54things like that.
- 33:55Speaker 1: Yeah. But what I know about LED light therapy or
- 33:59any light therapy for that matter is that you have
- 34:01to do it regularly. It's not like one day.
- 34:04Speaker 2: Every day.
- 34:05Speaker 1: It has to be done on a regular basis. That's
- 34:06the only way it reacts, right? Have you seen those
- 34:10videos online of people, their morning routine? It starts with
- 34:14them sitting in front of the red light and doing
- 34:16their meditation. They're on a vibration chair. And that's how
- 34:19it works.
- 34:19Speaker 2: People who have already made it in life probably. Yeah, yeah.
- 34:22And they have so much time. They have so much time.
- 34:25Cryo chambers and LED. So it's the next big thing.
- 34:27I think that's where the aesthetic market is also heading.
- 34:31And you can already see a few aesthetics clinics who
- 34:36have started to incorporate that. And I think it's going
- 34:39to only grow because people are really now investing into longevity.
- 34:46So there are definitely studies that are supporting these treatments.
- 34:51That is why they're coming up. There's really good evidence
- 34:55for LED and HBOT, that is hyperbaric oxygen therapy. For cryotherapy,
- 35:01the evidence is still not that high. But hyperbaric oxygen,
- 35:05we even use in our government colleges from our PG
- 35:09days for the healing of the wound. We used to
- 35:12make that environment there as well. This is what is
- 35:16called edge bots. Edge bot. Hyperbaric oxygen therapy.
- 35:21Speaker 1: So your face is basically attached to a machine that
- 35:23gives you.
- 35:23Speaker 2: Yeah, so the very fancy ones have come now. So
- 35:26even like one, probably half of the size of this room,
- 35:30where two people can think. can, sorry, sit and watch
- 35:34TV and, you know, enjoy the hyperbaric oxygen therapy.
- 35:37Speaker 1: Yeah, but what is it? It's just concentrated oxygen, right?
- 35:40Speaker 2: Oxygen, yes, yes, yes.
- 35:41Speaker 1: Ah, okay. Maybe people in Delhi need it.
- 35:44Speaker 2: Yeah, in India, we'd probably need it, yeah.
- 35:47Speaker 1: We need that. Okay, let me talk about the other
- 35:49basic that people have. And then again, another internet myth
- 35:53that I want you to really bust. On these concoctions
- 35:56that are there, everybody has, like, this oil... you know,
- 36:00kind of ritual that they like to do. Now, the
- 36:04internet is divided over whether oiling is good for you
- 36:07or whether oiling is.
- 36:08Speaker 2: Bad for you.
- 36:09Speaker 1: My mind was blown when a couple of years ago,
- 36:12I met a trichologist and he said, please stop oiling
- 36:14your hair. The best thing you can do is to
- 36:17stop making it oily because as Indians, there's scalps and
- 36:20then she analyzed my scalp as well. And she said, listen,
- 36:22especially in this time of the, when monsoon is here
- 36:25and you're already very sweaty or whatever, do not, you know,
- 36:28apply oil. Then I met another trichologist who said, no, no,
- 36:31it doesn't matter if it's a little bit of oiling.
- 36:33Speaker 2: So what is the truth?
- 36:33Speaker 1: Do you have to do oil or not? How much
- 36:36do you have to do? At what frequency?
- 36:38Speaker 2: So in our science, oil has no role. It is
- 36:42only a conditioner. So oil should be applied to the
- 36:47lower one third of the hair as a conditioner. And
- 36:51when it has done its work, you can wash it off.
- 36:55For some... One second. Yes.
- 36:57Speaker 1: Oil only on hair ends.
- 36:59Speaker 2: Yes.
- 37:00Speaker 1: And not on scalp.
- 37:01Speaker 2: Not on scalp, yes.
- 37:02Speaker 1: You're saying no champi.
- 37:03Speaker 2: No champi.
- 37:04Speaker 1: This one hour champi kia is not going to work.
- 37:07Speaker 2: No.
- 37:09Speaker 1: Okay, I'll sit with that.
- 37:12Speaker 2: So this is what happens in our OPD also. So
- 37:15we do not want to break that myth because it's
- 37:21so ingrained in people and it's so difficult to... so
- 37:25we we try to give this information in a more
- 37:29subtle way we say that put it in the lower
- 37:32one third don't do it here if you have dandruff
- 37:35then please don't do uh oiling because if you do
- 37:38oiling dandruff is mostly due to excess oil so your
- 37:43there's a fungus called malassezia that breaks the oil in
- 37:46your scalp into free fatty acids and when that free
- 37:50fatty acid concentration is not correct and uh that is
- 37:54the time that the fungus starts to grow. So that
- 37:57is why we say that don't put oil on the
- 38:00scalp if you have dandruff.
- 38:02Speaker 1: Yeah, people think if you have dandruff, if you have
- 38:04dry skin, then you should put more oil and they
- 38:05are spoiling it.
- 38:06Speaker 2: Right.
- 38:07Speaker 1: So oil should only be applied on.
- 38:09Speaker 2: The hair ends? Yes, because there you need the most conditioning.
- 38:11If you have natural oil, then scalp and face is
- 38:14one of the richest areas for oil glands, right? So
- 38:17you get oil yourself. If you don't wash your hair
- 38:19for 3-4 days, how does it happen? Hair? It becomes oily.
- 38:22Because that oil is coming, it's covering your upper hair.
- 38:24Because it's coming with hair strands. But below, the length,
- 38:29it's not reaching there. So that's why we do deeper conditioning.
- 38:32We have to apply more mask here. Split ends are down.
- 38:37Freeziness is down.
- 38:40Speaker 1: Because this is the part of the hair which is
- 38:41actually not alive, right?
- 38:43Speaker 2: Yeah, this is the part that has been produced... Earlier.
- 38:47Speaker 1: Earlier. Older hair.
- 38:48Speaker 2: Older hair.
- 38:49Speaker 1: Yeah.
- 38:49Speaker 2: So take care of it like that. Take care of
- 38:51old people.
- 38:51Speaker 1: Yeah. Take care of old hair. Like you take care
- 38:54of old people. That's a good one. So if you
- 38:57are doing oil, you have to apply it only on
- 38:59the bottom. Should you add all these things like fenugreek, rosemary,
- 39:05hibiscus in oil?
- 39:06Speaker 2: Or all of this is just a mix? If you
- 39:08like fragrance, then you should add it. But I don't
- 39:10think there is any evidence above that. And in our science,
- 39:15it doesn't have any role.
- 39:17Speaker 1: People also mix oil. One-third coconut oil, one-third mustard.
- 39:23Speaker 2: Some drops of rosemary.
- 39:24Speaker 1: They're also adding vitamin E. What's happening?
- 39:30Speaker 2: Home remedies. These are very popular in India. But scientifically,
- 39:35there is no evidence of this. So, we don't promote
- 39:39this at all. Anyway, those who come to us, they
- 39:40have already tried all this.
- 39:42Speaker 1: Oh.
- 39:44Speaker 2: Yes.
- 39:44Speaker 1: Okay, instead of details, we have a fun game.
- 39:47Speaker 2: Okay.
- 39:49Speaker 1: It's a take it or trash it on hair treatments DIY.
- 39:52Speaker 2: Right.
- 39:52Speaker 1: Okay, so how this game works is that.
- 39:54Speaker 2: It's going to be trashy.
- 39:58Speaker 1: So I'm going to give you some hair treatments DIY
- 40:02that people very popularly use in India. You have to
- 40:04tell me whether it works or does not work. So
- 40:06should you take it or should you trash it? Okay.
- 40:09So take it or trash it hair treatment edition. Let's start.
- 40:15Coconut oil keeping it overnight.
- 40:18Speaker 2: Trash it.
- 40:21Speaker 1: Onion juice?
- 40:22Speaker 2: Trash it.
- 40:23Speaker 1: Rice water rinse?
- 40:27Speaker 2: You can do it.
- 40:28Speaker 1: Can do it?
- 40:29Speaker 2: Yeah.
- 40:29Speaker 1: Conditioning?
- 40:30Speaker 2: It's not harmful. So, if it conditions your hair a
- 40:34little bit, you can do it. But it's not useful.
- 40:36Speaker 1: Okay.
- 40:38Speaker 2: Yeah.
- 40:39Speaker 1: Egg mask?
- 40:40Speaker 2: Egg mask, trash.
- 40:42Speaker 1: Fenugreek paste? Methi ke daano ka paste?
- 40:45Speaker 2: Trash.
- 40:47Speaker 1: Hibiscus?
- 40:48Speaker 2: Trash it.
- 40:49Speaker 1: Apple cider vinegar rinse.
- 40:51Speaker 2: Definitely trash it.
- 40:52Speaker 1: Okay. Beer mix.
- 40:55Speaker 2: It just acts like a conditioner, so you can do that.
- 40:58Speaker 1: You can?
- 40:59Speaker 2: Yeah.
- 40:59Speaker 1: Okay. Rosemary oil.
- 41:02Speaker 2: You can do it because there's some studies that are
- 41:04favoring it. Okay.
- 41:06Speaker 1: Castor oil.
- 41:08Speaker 2: Trash it.
- 41:09Speaker 1: Amla.
- 41:10Speaker 2: Trash it.
- 41:10Speaker 1: Curd mask on hair.
- 41:12Speaker 2: You can do. Okay.
- 41:14Speaker 1: Help me with one thing. Somebody watching this podcast right now,
- 41:17they don't aspire to have skin like a celebrity or
- 41:21they want to, they can't afford perhaps that much amount
- 41:25of time and effort into looking that good. Somebody just
- 41:29has about like five minutes, 10 minutes in the morning
- 41:31before they hit work or about five, 10 minutes before
- 41:34they hit the bed. What would you recommend they do
- 41:36to ensure that their hair is in good shape? So
- 41:39give me like a hair care morning routine and a
- 41:41hair care evening routine.
- 41:42Speaker 2: So in the morning, definitely start with washing your hair.
- 41:44For girls, maybe three times in a week. And for boys,
- 41:49every day. For most boys, because we sweat, right? Then
- 41:54nothing else to be done. You can apply your general
- 41:57wax or whatever you would like to apply to set
- 41:59your hair. There's no problem in that. Then in the night,
- 42:03you can use the, if doctor has given you some treatment,
- 42:08like some peptide serum you can use to improve your
- 42:10hair quality. You can take your supplements. If you have
- 42:14pattern baldness, you can apply minoxidil. That's about it.
- 42:18Speaker 1: Nothing, not that hair density peptides that are very popular
- 42:22these days.
- 42:22Speaker 2: Like I said, peptides. So peptides you can apply to
- 42:25maintain the quality of hair.
- 42:27Speaker 1: Are there some easy combinations that are out there?
- 42:30Speaker 2: Yes, yes, of course. There are decapeptides, octopeptides, and there's Redensil,
- 42:34there's Anagain, there's Seroxil, there's Menexil. Right. So these are
- 42:42the ones that you can use.
- 42:42Speaker 1: Basically, ask the doctor. We won't understand all this. No,
- 42:44these are available in.
- 42:46Speaker 2: A lot of serums nowadays.
- 42:48Speaker 1: There are... More brands that we know that are doing
- 42:51shampoos these days.
- 42:52Speaker 2: Yeah.
- 42:53Speaker 1: There are more brands that we know who are doing
- 42:55dry shampoo and then conditioners and masks and God knows
- 42:59serums and God knows what all. If you are recommending
- 43:04a shampoo to someone, what do you look for in
- 43:07the ingredient list? And is there something that you definitely
- 43:10don't want there?
- 43:12Speaker 2: So first is we need to choose the shampoo according
- 43:14to our scalp type. And our hair type, right? People
- 43:17make a lot of mistake in that. People just see
- 43:19the sulfate-free ad and they will buy the sulfate-free shampoo
- 43:22and then they will come to me for dandruff. And
- 43:24that is why my dandruff practice is increasing day by
- 43:26day because everybody is selling sulfate-free shampoos.
- 43:29Speaker 1: Okay.
- 43:29Speaker 2: So one has to see if the scalp is very oily,
- 43:34go for a shampoo that makes froth. a lot of froth,
- 43:38and that would be something with the sulfate. In the
- 43:40sulfate category, instead of sodium lauryl sulfate, which was in
- 43:45most of the older brands, conventional brands, sodium laureth sulfate,
- 43:51they are a bit more milder. So, they don't cause
- 43:53that much dryness. Okay. Then, if you feel that your
- 43:57scalp is very sensitive, and you easily get allergies, you
- 44:00start getting water from your eyes, some people are allergic usually,
- 44:05those people should stay away from paraben, paraben-based products.
- 44:08Speaker 1: Not everyone.
- 44:09Speaker 2: Other people can use it. Perfume can also be used
- 44:11by everyone. Especially, the category of patients, not that. If
- 44:18your hair is dyed or if you feel that your
- 44:22hair frizzes easily, then use a sulfate-free shampoo. Because it
- 44:27has less cleansing power. So your hair dye will stay longer.
- 44:31Your hair won't be too dry. So only in that case,
- 44:36you go for the sulfate-free shampoo.
- 44:38Speaker 1: Okay. Otherwise, for scalp, what do you recommend?
- 44:40Speaker 2: For the scalp, like I said, If you have an
- 44:42oily scalp, go with a sulfate-based shampoo. Go for sodium
- 44:46laureth sulfate. Okay. For somebody who is having not so
- 44:51oily scalp, they can go with a sulfate-free shampoo.
- 44:55Speaker 1: So, you're largely saying that the shampoo is for your scalp.
- 44:59Speaker 2: Right.
- 44:59Speaker 1: And the conditioner is for your hair ends.
- 45:01Speaker 2: Of course.
- 45:02Speaker 1: That's what you need to know.
- 45:03Speaker 2: Because shampoo is to clean. Cleanse. You don't want to
- 45:06clean your hair. You'll lose the... There's a beautiful layer
- 45:09of fat around your hair that gives it shine. So
- 45:13if you wash that away, so your hair will be
- 45:16not having luster. Right.
- 45:18Speaker 1: There are a lot of treatments nowadays available, especially like Olaplex, etc. Again,
- 45:22not to take a brand name, but they have these
- 45:24treatments which say that apply it for 10 minutes and
- 45:26then your bonds will get like rejoined. Does that happen?
- 45:30Does that work?
- 45:31Speaker 2: Only temporarily they can be rejoined. Temporary bonds can form,
- 45:35which will show improvement. Like you do hair spa, how
- 45:39your hair gets better for some type. So these are
- 45:41new hydrogen bonds that form. But the main bonds are
- 45:44the sulfur bonds in your hair. They cannot be formed.
- 45:48They can be broken by a treatment called chemical rebonding.
- 45:50That's why doctors say don't get chemical rebonding done. Because
- 45:53they are permanently broken. That breaks your hair structure. And
- 45:56these things will temporarily fix your hair structure. These treatments
- 46:00do fix your hair structure. But it's temporary. Once the
- 46:03product wasts off, the effect will go away. Oh.
- 46:08Speaker 1: So it doesn't really stay.
- 46:09Speaker 2: It doesn't really stay.
- 46:10Speaker 1: Unlike what the marketing says.
- 46:11Speaker 2: No. For things that stay is what you eat, your genetics.
- 46:16Speaker 1: Yeah.
- 46:16Speaker 2: And rest is all from the outside. It's cosmetic.
- 46:20Speaker 1: Yeah. But listen, we have to look at the realities
- 46:23of the times that we live in, right? People like
- 46:25me have to get their hair blow-dried every day. But
- 46:27even on a regular basis, now I see more and
- 46:29more people using hair dryer and using ironing machines, etc.
- 46:34That leads to damage, yes. But living without them is like,
- 46:38Like nobody can be fling stones anymore, right? Right, right, right.
- 46:40Speaker 2: No, of course, you should use them. You can use them.
- 46:4270 degree Celsius, heat more than 70 degree Celsius will
- 46:47start to denature your hair.
- 46:49Speaker 1: But you won't be able to get iron in less
- 46:52than 70 degree.
- 46:53Speaker 2: You won't be able to get iron, but.
- 46:54Speaker 1: It will blow dry.
- 46:56Speaker 2: Yes, it can blow dry. If you do iron regularly,
- 46:59then keep its temperature a little low. Because as I said,
- 47:02if you keep increasing the temperature, your sulfur bonds will
- 47:05start breaking. And your sulfur bonds will break. And the
- 47:08cuticle of your hair, the covering of your hair will break.
- 47:12That will expose your hair. So when you go out,
- 47:15environmental challenges will come to your hair. You will go
- 47:18to a humid area. You will go to Goa for holiday.
- 47:21So moisture can enter your hair. Then the texture of
- 47:26your hair will be completely gone. Okay.
- 47:30Speaker 1: So, these heat protection sprays, do they work? Yes, they work.
- 47:35Speaker 2: Again, that is also a temporary solution. If you wash it,
- 47:38it will come off. But it has silicone. Or it
- 47:41has polymers. So, they make a layer of your hair follicle,
- 47:45hair strand outside. So, they protect you from heat.
- 47:50Speaker 1: So, they can apply it.
- 47:51Speaker 2: This is like a sunscreen, you can say.
- 47:54Speaker 1: Ah.
- 47:54Speaker 2: Yeah. Okay.
- 47:55Speaker 1: But it also has side effects.
- 47:56Speaker 2: No, it doesn't have any side effects. It gets washed, right?
- 48:00Speaker 1: Okay, okay. So before we move forward, and I've got
- 48:03lots of questions that people have sent for you on
- 48:05our channel, I want to spend some time in discussing
- 48:11hair coloring. Everybody I know these days has a hair
- 48:14color on. Barely anybody has raw virgin hair left in
- 48:20this world. And the thing about hair coloring is that
- 48:23once you start it, you just can't stop, right? Tell me,
- 48:28What is a thing that should not be in your
- 48:30hair color? I know so many people who've got allergies,
- 48:35really bad ones across their body because they've used a
- 48:38bad hair color. I know so many people who've started
- 48:41balding from here because you're constantly getting a root touch
- 48:45up or something done. Is there a good way of
- 48:48getting a hair color done or something that you should
- 48:50watch out for?
- 48:51Speaker 2: Definitely. This is a very important question, I would say,
- 48:54especially in Indian context. Because we like our hair black.
- 48:58We can't live with white hair. Now, the regulations, that
- 49:03has banned ammonia completely. But there's another molecule in the colors,
- 49:09that is paraffinylenediamine. It's called PPD. PPD is a very
- 49:15common form of allergy. It's the most common form of allergy,
- 49:18at least I see in my practice. And there's a
- 49:22condition called LPP, lichen planus pigmentosis, also called Reel's melanosis,
- 49:27which is triggered by the PPD. So, these are the
- 49:33things that people don't even know about, but these things
- 49:35are happening in India. Yes. Patients are coming to doctors
- 49:41but no one is highlighting this to our government. That
- 49:45so many people are getting LPP in which black pigmentation
- 49:48comes in this area. And especially girls because they use
- 49:52it so they get it. Boys also get it. So
- 49:55it has a trigger. PPD.
- 49:58Speaker 1: LPP.
- 49:59Speaker 2: And LPP is such a problem which is It doesn't
- 50:02get cured easily. And it's even more difficult than laser.
- 50:06LPP has 100% results. It's like permanent damage. It's like
- 50:09permanent skin pigmentation. More common if you have higher melanin content.
- 50:15Speaker 1: So if you're darker in your skin tone.
- 50:17Speaker 2: If you're darker, it's more popular. And we should only
- 50:23use PPD-free dyes. And PPD-free dyes, somehow brands are still
- 50:27very less. There are brands, Indian brands, foreign brands too.
- 50:31And of course, people choose the PPD one or the
- 50:35saloon one because they don't know this. They think the
- 50:39long standing one is better. But it's actually not better
- 50:44for you. PPD free dyes are better for you. If
- 50:47I ever want to start dyeing, I will always use
- 50:50PPD free dye.
- 50:52Speaker 1: Got it. So it's just mentioned in the ingredients. This
- 50:56is what you should ask.
- 50:57Speaker 2: The companies that make PPD free dyes highlight this. This
- 50:59is PPD free.
- 51:02Speaker 1: Okay, okay. What about mehendi then? A lot of people
- 51:05use heena.
- 51:06Speaker 2: Yeah. So, naturally, there's no problem with that. But the
- 51:09brands that are available in the market, they also have PPD,
- 51:12long-staining gloves. They put it in. So, even they're saying
- 51:15that we're applying mehendi, but they don't know. They say,
- 51:18bring it, pack it, whatever you're using. So, sometimes we
- 51:21get PPD from that. I see.
- 51:24Speaker 1: So you have to use a good brand there and
- 51:25you have to check the ingredient list.
- 51:26Speaker 2: Good brand ka toh nahi, you have to check the ingredients.
- 51:29Speaker 1: I see.
- 51:30Speaker 2: I see.
- 51:30Speaker 1: Indigo color is getting very popular these days.
- 51:33Speaker 2: Indigo color is without PBT.
- 51:35Speaker 1: Yeah. But that also washes off very quickly, which is why.
- 51:38Speaker 2: But whatever you're buying, just read the label.
- 51:41Speaker 1: Okay. Okay. On that note, let me take some questions
- 51:45that our viewers have sent. Okay. This is Bhumit. Who says,
- 51:53does shampooing your hair frequently cause hair fall? No. This
- 51:57is Saurabh. He says, is scalp psoriasis caused by autoimmune
- 52:01disease and is it incurable?
- 52:03Speaker 2: Yes, psoriasis is caused by autoimmune disease. It is treatable.
- 52:07But we cannot say that we have cured you because
- 52:10it might come back at some point of her time.
- 52:13Speaker 1: There's another one which says, can you please ask if
- 52:15there's a natural way to block DHT to avoid male
- 52:18pattern baldness? We've already answered that question. So go back
- 52:21to the chapter and see it. There is one. Oil
- 52:26is a trend online. There is something called a Adivasi oil.
- 52:31Speaker 2: Yes. You never heard about it? No. It's so popular.
- 52:35Speaker 1: And?
- 52:35Speaker 2: So this is a brand which is from somewhere in
- 52:38the south. And it's from a village where everybody has...
- 52:43Luscious big hair. And I think that's mostly genetics. Probably
- 52:50that area people have thicker hair. Or they just get
- 52:55models who have thicker hair. But yeah, they've made a
- 52:59strong brand recall with that. There is no evidence to
- 53:04suggest any sort of hair oil is helping in growing
- 53:10thicker hair. And we should think if this would have happened,
- 53:14then would this secret be limited to India only? Don't
- 53:17people outside take this secret where there is a problem
- 53:20with hair fall and where there is a billion dollar industry?
- 53:23There is another ad in which an old man sells
- 53:27his oil. So if that old man's oil is so good,
- 53:34So why not take him to a multinational and sell
- 53:39him all over the world. These are all marketing gimmicks.
- 53:42These are all marketing gimmicks. There is a little sense,
- 53:45common sense.
- 53:46Speaker 1: Okay, there's another one. So there are a lot of
- 53:50questions on DHT but I'm very glad we've answered that
- 53:52in detail. Let's move to the next question. This is
- 53:56Praveen who says how to improve the quality of hair
- 53:59precisely the thickness of hair.
- 54:02Speaker 2: Yes.
- 54:02Speaker 1: Can it be improved?
- 54:03Speaker 2: Yes, thickness can be improved by increasing the anagen phase,
- 54:06I told you. Because if anagen gets thinner, it gets thinner. So,
- 54:12peptide serums can help you with that. Redensil can help
- 54:17you with that. Then there is minoxidil that can help you.
- 54:21There are clinic treatments that can help you, which include
- 54:24exosomes and PRP. And mesotherapy can help you. So these
- 54:31are the things.
- 54:31Speaker 1: Mesotherapy.
- 54:33Speaker 2: Yeah.
- 54:33Speaker 1: Tell us a little about that.
- 54:35Speaker 2: So mesotherapy comes from the word mesoderm. When we are
- 54:38being made in our body, there is a layer called mesoderm.
- 54:41Due to which a lot of our organs are made.
- 54:46So if we are doing the treatment of the organ
- 54:48made from that mesoderm, then we know it as mesotherapy.
- 54:51So the hair treatment that comes is usually mesotherapy. So,
- 54:55in mesotherapy, as we are saying in daily usage, it
- 55:01means giving your hair peptides, growth factors, vitamins, antioxidants, nutrition
- 55:10directly into the hair follicle by a form of injection
- 55:14or by the form of microneedling and then spreading it
- 55:18through the channels that you... Yes, yes, it works.
- 55:21Speaker 1: Hmm, hmm. Okay, this is interesting. This is Siddharth. He
- 55:24says my nutrition is very good. My protein intake is
- 55:28very close to 110. My weight is also very good.
- 55:32I still have hair loss. I don't use any such
- 55:35oil or shampoo. So how to stop the hair fall
- 55:39and decreasing hairline?
- 55:42Speaker 2: So if the hairline is receding, so definitely he needs
- 55:46to start on the proper treatment with his dermatologist that
- 55:50is minoxidil DHT blockers and whatever the doctor suggests because
- 55:53that is going to be lifelong.
- 55:54Speaker 1: It's probably a case of genetics now.
- 55:57Speaker 2: If he's having also hair fall along with it and
- 55:59he's fixed all his diet and everything and still it's
- 56:03been two months and he's still having that then he
- 56:05should definitely add a peptide serum to his hair care
- 56:09routine and he should get his blood investigations done and
- 56:12then meet a dermatologist so that deeper things can be
- 56:16looked at. You can also think about the cyclical hair
- 56:19therapy because some things that might be missed in the
- 56:22blood test because if you start doing all the tests
- 56:24like this, you'll have to pay a lot because the
- 56:26trace elements tests are very expensive. I think zinc test
- 56:29is around 3500 or 4000 rupees, right? So you don't
- 56:32want to get all things done. You can take the
- 56:34supplement instead.
- 56:35Speaker 1: All right, let's move to the next segment now. I
- 56:38want to talk about one thing which is now as
- 56:43popular and in fact should be an industry of its own.
- 56:47It is hair care supplements.
- 56:49Speaker 2: Right.
- 56:49Speaker 1: People are popping biotin and collagen like it's dal roti sabzi.
- 56:53Speaker 2: Right. Right.
- 56:55Speaker 1: Does it work? Does it not work? Because every time
- 56:57I look at research, I feel like the research is
- 57:00so weak. There is like marginal difference that it really makes.
- 57:05Speaker 2: Of course.
- 57:06Speaker 1: What is your analysis? Is the research not catching up
- 57:09with what you're seeing in your clinic? Or is there some...
- 57:13Is there... We should just like cancel all of this altogether.
- 57:16Speaker 2: Definitely, I think research is not catching up with the
- 57:19kind of variety there is in the market. And not
- 57:22everything is very high on research. And a lot of
- 57:25it is now PR also. Yeah. Biotin is coming to
- 57:30the main thing, you know, which is promoted for hair loss.
- 57:33The main supplement is biotin. Biotin is very essential. It
- 57:37is there to make keratin. It's involved in the pathway
- 57:40to make keratin. That is the hair protein. But, The
- 57:44deficiency of biotin is rare. So I do not ask
- 57:47patients to take biotin unless they're on hair growth treatments.
- 57:53If they're on hair growth treatments, I'll probably give it
- 57:55for three months and then just stop it because I
- 57:58don't want to overdo it. Biotin is known to cause
- 58:01abnormality in the lab reports. So if you have some
- 58:05other problem when you're getting a lab test done, so
- 58:08this is actually a warning by the FDA as well,
- 58:11that biotin can interfere with the results. So you can
- 58:14get a wrong result.
- 58:16Speaker 1: So if you're getting any test done and if you
- 58:18are on biotin, please let your doctor know that's number one.
- 58:20Because you can sort of amplify this. So overuse of
- 58:24biotin is a problem. And only those who do not
- 58:29have sufficient levels should take them.
- 58:30Speaker 2: That's what I'm saying. Yes, yes. Or you can just
- 58:32take it for a few months when you start your
- 58:35hair treatment. And after discussing with the doctor, then you
- 58:38can stop it. You don't have to keep taking it
- 58:42for your entire journey. There is something called a cyclical
- 58:46hair therapy that I really believe in and it has
- 58:49shown good results to my patients. So these are a
- 58:54box of nutritional supplements. So every day there's a different
- 58:58nutritional supplement. So you're not overdosing. At the same time,
- 59:01you are getting the recommended daily allowance. So there is iron,
- 59:07there is vitamin D, there is even trace element like
- 59:10zinc chromium but according to the recommended daily allowance so
- 59:12you do not exceed that and the supplements are rotated
- 59:16so that is something that you can do and we've
- 59:18seen a lot of benefit to patients who have hair
- 59:21fall how does one get that It's available with all dermatologists.
- 59:25You can take a prescription. You can then order it online.
- 59:28You can take from a doctor's clinic or pharmacy.
- 59:30Speaker 1: What's it called again?
- 59:32Speaker 2: It's called cyclical hair nutrition.
- 59:35Speaker 1: Okay. Tell me also about the other supplement which is
- 59:39very popular. Let's talk about collagen.
- 59:41Speaker 2: Yes.
- 59:42Speaker 1: There is marine. There is one that comes from pigs.
- 59:45One that comes from cows. There is plant-based also now.
- 59:48Does it really help?
- 59:49Speaker 2: Yeah, so collagen is basically a three amino acid clubbed
- 59:53together that forms the collagen. There are a few types
- 59:56of collagen that are present in all parts of our body.
- 59:59So collagen is basically a protein supplement. In studies, it
- 1:00:04has shown to improve the moisturization or the moisture retention
- 1:00:08in the face. And that's how it indirectly helps in wrinkles.
- 1:00:12So that is sort of true. For the hair, I
- 1:00:15don't recommend collagen products. Regularly I do it for the face.
- 1:00:20For the hair, I always concentrate more on protein intake
- 1:00:23so that your body gets enough protein so that it
- 1:00:25can make keratin of its own.
- 1:00:27Speaker 1: Of its own.
- 1:00:28Speaker 2: Yeah.
- 1:00:28Speaker 1: So these collagen, gummies, pops.
- 1:00:33Speaker 2: I'm totally against gummies. I get a lot of, you know,
- 1:00:38they contact me also, a lot of companies who have
- 1:00:40gummies to talk about their product. But I never do
- 1:00:44that because hair gummies is like full of sugar and
- 1:00:47all those things. And you... It's better to take just
- 1:00:51a tablet which is, you know, free of these artificial things,
- 1:00:54artificial flavors.
- 1:00:56Speaker 1: Got it. So, no biotin unless you're deficient.
- 1:01:01Speaker 2: Yes.
- 1:01:02Speaker 1: No collagen, it'll have no impact on your hair.
- 1:01:04Speaker 2: Not hair so much.
- 1:01:05Speaker 1: Gummies don't work either. Right. The only thing that you're
- 1:01:08saying in terms of supplements that works is the cyclical
- 1:01:11hair nutrition that your dermat can only prescribe.
- 1:01:15Speaker 2: Or you can take a supplement according to your blood report.
- 1:01:18Like if your vitamin D is less and everything else
- 1:01:21is fine.
- 1:01:21Speaker 1: Supplement what you are deficient of.
- 1:01:23Speaker 2: You don't need to take the full cyclical therapy. You
- 1:01:26can take just vitamin D. Right.
- 1:01:28Speaker 1: But I'm hearing a lot of people talking about these
- 1:01:31collagen shots.
- 1:01:32Speaker 2: Yes. Collagen shots. Yeah.
- 1:01:35Speaker 1: They're like in concentrated forms and they say you have
- 1:01:37to like drink it like that.
- 1:01:38Speaker 2: Probably they have made some way to make the collagen
- 1:01:41in the liquid form as well. And they're taking that.
- 1:01:43I think it should work the same.
- 1:01:44Speaker 1: Yeah with the same amount like marginal difference.
- 1:01:47Speaker 2: So the recommended collagen per day is 10 grams that
- 1:01:52is when one sachet we give 10 grams so if
- 1:01:55you're going for a collagen brand check that it is
- 1:01:58it should be preferably marine cod and tilapia fish are
- 1:02:02a good source of collagen then it should be at
- 1:02:05least 5 can be up to 10 grams per day
- 1:02:09and it should be a hydrolyzed collagen type 1 so
- 1:02:13look For all these things, if you're buying a collagen brand,
- 1:02:17take something which is also tested for heavy metals. Yeah.
- 1:02:20See their website if it says that.
- 1:02:22Speaker 1: Right.
- 1:02:23Speaker 2: Right.
- 1:02:24Speaker 1: So that is a good point for us to sort
- 1:02:26of jump into. So the therapies that are now available.
- 1:02:29Speaker 2: Right.
- 1:02:29Speaker 1: There is... I mean, I don't even know these names.
- 1:02:32There's PRP, there is PRF, there is exosome, there is
- 1:02:36mesotherapy that you spoke about, there is GFC, microneedling, QR678.
- 1:02:41There are so many therapies.
- 1:02:45Speaker 2: So I've got some of them which I use in
- 1:02:47my clinic here so that people understand it better. So
- 1:02:51we can take them one by one. Yeah, let's see. Okay,
- 1:02:54so we will just start with this. This is advanced
- 1:02:57skin complex from Exocobio. Again, this is not a sponsored session.
- 1:03:02This is a Korean exosome. What are exosomes? Exosomes are
- 1:03:09vesicles that are produced by living cells. Okay, so these
- 1:03:14have a lipid membrane. And inside this, growth factors, peptides, cytokines...
- 1:03:23There are many types of molecules which can help hair growth.
- 1:03:27These can be plant-derived, animal-derived, synthetic-derived. So these are the exosomes.
- 1:03:34These are not supposed to be injected. They are mostly
- 1:03:37done with microneedling and then they are massaged into the
- 1:03:39scalp so that they penetrate.
- 1:03:41Speaker 1: What are they doing?
- 1:03:42Speaker 2: So they are directing the hair follicle to produce thicker
- 1:03:47hair because it has growth factors, cytokines. These are signaling molecules.
- 1:03:54But is this for everyone? Anybody can get this done.
- 1:04:01This is a cosmetic, cosmeceutical treatment. Medical and cosmetic treatment.
- 1:04:07So anybody can get it done for thicker hair and
- 1:04:10for more volume of the hair.
- 1:04:11Speaker 1: How much does it cost and how long do you
- 1:04:13have to do it?
- 1:04:14Speaker 2: So the exosomes are also of different qualities. This is
- 1:04:18the most expensive brand, the one that I've got here.
- 1:04:21This will cost you around 25,000-30,000 per session.
- 1:04:25Speaker 1: Per session. And how many do you need?
- 1:04:28Speaker 2: Four.
- 1:04:29Speaker 1: Four.
- 1:04:29Speaker 2: Yeah.
- 1:04:30Speaker 1: It's not like one of those that you take and
- 1:04:32it's over. I mean, you have to then maintain it.
- 1:04:34Speaker 2: Right, right. Okay.
- 1:04:36Speaker 1: You have others.
- 1:04:38Speaker 2: Yes. So this is a PRP kit. Okay. This is
- 1:04:40the one that is, I think, most popular. This is
- 1:04:42also a lot more affordable. You can overcome its cost
- 1:04:47if you're not using a PRP specific tube. But I
- 1:04:52say that you use this one because you have to
- 1:04:55put it and then you have to take it back
- 1:04:57from the vial and inject it into the scalp. So
- 1:05:00you should use a good vial. So this is more affordable.
- 1:05:04PRP treatment is what?
- 1:05:05Speaker 1: You're basically extracting.
- 1:05:07Speaker 2: You're taking the blood, you're separating the platelets out of
- 1:05:10it and then that platelets have growth factors in them
- 1:05:13and that growth factor concentrate you're putting in the scalp.
- 1:05:17Speaker 1: Okay, but PRP sessions, you need a lot of them.
- 1:05:19Speaker 2: Yeah, you need six sessions.
- 1:05:21Speaker 1: Six sessions at least.
- 1:05:22Speaker 2: Four to six sessions, yeah.
- 1:05:23Speaker 1: But are there people who don't respond to PRP also?
- 1:05:29I've heard a lot of stories that we've done so
- 1:05:30many sessions and nothing has happened to our hair.
- 1:05:32Speaker 2: See, the right expectation has to be given. If you
- 1:05:34are doing PRP for androgenetic alopecia male pattern baldness, first
- 1:05:38you have to tell him that this is an additional treatment. Okay?
- 1:05:44This is an FD approved treatment. This is an additional treatment.
- 1:05:47That is the proper disclosure the doctor should give. That's
- 1:05:49the doctor's responsibility. Then, PRP... Now what people do is
- 1:05:53they sell PRP after doing main therapy. That is misleading.
- 1:05:57Speaker 1: Okay?
- 1:05:59Speaker 2: To earn money, PRP... But don't apply minoxidil at home.
- 1:06:03You have to tell the patient that minoxidil is main.
- 1:06:06DHT blockers are main. PRP is additional. If you want
- 1:06:10to go higher than that. Or if you don't find
- 1:06:11its result sufficient. Then you do this. That would be
- 1:06:14more ethical disclosure I think. Yeah. So you can get
- 1:06:18PRP done. And you should do it 4 to 6 times.
- 1:06:22And post that. If you have patent baldness. Because most
- 1:06:25of the people have patent baldness. So they have to
- 1:06:27maintain the results. If they are thinking that PRP is
- 1:06:29done and leave is for always. It is not like that.
- 1:06:34If a patient comes to me, I tell them that
- 1:06:35I am not their doctor. You are going to the
- 1:06:38wrong place. Because I know that later he will come
- 1:06:39to me and say that he is not a doctor.
- 1:06:42And all the results are not there.
- 1:06:43Speaker 1: So there is no magical treatment. PRP is not that.
- 1:06:46But both you said, exosome and PRP, both have growth
- 1:06:51factors coming out of different sources and induced by which
- 1:06:54your hair becomes thicker.
- 1:06:55Speaker 2: Yes. Then this is something very new. This is a
- 1:06:59hair filler. This has peptides. So, these are eight kinds
- 1:07:03of peptides in this particular brand. And again, peptides nourish
- 1:07:08the hair follicle. Peptides are proteins. So, they nourish the
- 1:07:10hair follicle, make it better, thicker. So, this can be used.
- 1:07:13Speaker 1: How do you use it?
- 1:07:14Speaker 2: You just inject it. They are already pre-filled syringes. You
- 1:07:17just inject them.
- 1:07:18Speaker 1: Again, which kind of person or what kind of hair loss?
- 1:07:22Speaker 2: This can be done for hair loss. This can be
- 1:07:24done to improve the hair texture also. Over the course
- 1:07:27of time, it will improve the hair texture also.
- 1:07:29Speaker 1: Side effects?
- 1:07:30Speaker 2: Just the pain during the injection, that's probably the only
- 1:07:33side effect. So, peptides are proteins. You will not have
- 1:07:36any side effects with them. Side effects for all three
- 1:07:39of them are nothing major. Okay. Yeah. This is also
- 1:07:42a very, very commonly used thing, a growth factor. This
- 1:07:47is imported from the USA. This also has peptides and
- 1:07:50a lot of other things. like good molecules you can
- 1:07:54say vitamins and minerals that you can directly inject into
- 1:07:57the scalp this is quite popular and a lot of
- 1:08:01people get this you know I have.
- 1:08:02Speaker 1: A worry you are showing all these things my worry
- 1:08:05is that people will procure it separately and go to
- 1:08:09their salons and say put this on my head you
- 1:08:12will do it yourself why is it so dangerous to.
- 1:08:16Speaker 2: Do that it is so dangerous because they don't have
- 1:08:18proper knowledge of what they are doing hmm They can
- 1:08:21put something wrong. Your product can be put in your vessels.
- 1:08:26Hair filler can be put in your vessels. Which can
- 1:08:28cause problems. You need to know the proper depth. You
- 1:08:32need to know the proper gapping between the treatments. These
- 1:08:39are all medical treatments. Ideally, they should not be available.
- 1:08:41Because these are regulated. But in our country, everything is available.
- 1:08:44In our country, everything is available.
- 1:08:46Speaker 1: Cost for everything.
- 1:08:46Speaker 2: That's true.
- 1:08:47Speaker 1: Okay. There's also something called PRF.
- 1:08:50Speaker 2: PRF is same as PRP. It's just that it is
- 1:08:52done in a vial which doesn't have anticoagulant. So it
- 1:08:55is thicker than the PRP. But the benefits and the
- 1:08:59profile is almost the same. Cost? Cost is low only
- 1:09:02because that's the cheapest kind of a therapy because it
- 1:09:05is made of your own blood. And you just need
- 1:09:09normal vials for it. Okay.
- 1:09:11Speaker 1: There are so many treatments. There is some laser therapy
- 1:09:14and laser cups. Is there one therapy that you think
- 1:09:18really works? If you have a genetic issue, that means
- 1:09:21you have a male pattern baldness or you have a
- 1:09:23female pattern baldness, what is the one therapy you would
- 1:09:26say is just upcoming and really showing good results?
- 1:09:30Speaker 2: So apart from the minoxidil and the DHT blockers, the
- 1:09:34injectable DHT blockers are very good because the side effects
- 1:09:38are less. They work on the same mechanism as your
- 1:09:41oral DHT blockers. So I would rate them really high
- 1:09:43and I've seen great results with it. But there are
- 1:09:46certain contraindications like we don't want to do a DHT
- 1:09:49blocker for a young married female. So for those patients,
- 1:09:53we can go to the other like these kind of
- 1:09:55treatment which are the hair growth treatment that don't work
- 1:09:58on the DHT part of it.
- 1:10:00Speaker 1: Why young married women? Because they could be conceiving and
- 1:10:02it's not used on pregnant women.
- 1:10:04Speaker 2: Yes, yes.
- 1:10:04Speaker 1: Okay, okay. Okay, which brings me to the one thing
- 1:10:08that anybody who has even typed hair loss or hair
- 1:10:12fall once, na, aapke algo mein ho aajayega.
- 1:10:14Speaker 2: Wo main ho.
- 1:10:17Speaker 1: Apart from you. Apart from you. Agar aapne kabhi bhi
- 1:10:20kisi reel ko like kar liya hai or share kar
- 1:10:22diya hai on hair loss or hair transplant, wo seedhe
- 1:10:25aapko lehkar jaate hain Turkey. Okay, everybody has watched on
- 1:10:29how you go to Turkey, you take one trip and
- 1:10:32you come back. You're a new person. You have new confidence.
- 1:10:36You have new hair. Okay, what is happening in Turkey?
- 1:10:40What are they doing in these spas?
- 1:10:43Speaker 2: There's nothing happening there. See, you have to go back
- 1:10:46a little bit in the history, how it started. Europe is,
- 1:10:50of course, more expensive. They have more money. For them
- 1:10:54to fly to Turkey and get a hair transplant done,
- 1:10:56which is a labor-intensive treatment, was cheaper. And that's how
- 1:11:00Turkey started to get a lot of patients from Europe
- 1:11:04and there must be some really good surgeons there who
- 1:11:06were doing a good job and slowly they've been able
- 1:11:09to build that kind of a brand for themselves and
- 1:11:11congratulations to them that they have really captured that market
- 1:11:16I would say. But for an Indian to go to
- 1:11:21Turkey doesn't make any sense whatsoever. There are wonderful hair
- 1:11:25transplant surgeons here in India and hair transplant is one
- 1:11:30thing where we are doing one of the best work.
- 1:11:33I would say we are doing it like in comparison
- 1:11:36to the top, you know, the countries, even maybe better
- 1:11:42than them because we have so many patients who want
- 1:11:45to get a hair transplant done. But again, in India,
- 1:11:48the problem is regulation. This problem is that you can
- 1:11:52end up into wrong hands. And that same problem is
- 1:11:55also in Turkey. I've seen patients with not so good transplants,
- 1:11:59especially the patients who come from Dubai to me. They
- 1:12:02get their transplant sometime done in Turkey, right? So they
- 1:12:05come to me and then I see their hairline and all.
- 1:12:07They're very artificial. So there's a lot of quackery in
- 1:12:10Turkey and there are a lot of unregulated hair transplants
- 1:12:13being done in India. So please check your doctor's reputation.
- 1:12:18Prefer a dermatologist or a plastic surgeon. for hair transplant
- 1:12:26and yeah I think if that's if you read the
- 1:12:29degree and go you should be.
- 1:12:30Speaker 1: Okay so you're saying that the kind of machinery or
- 1:12:37the kind of treatment that is being used in Turkey
- 1:12:39is the same that's available in India they're.
- 1:12:41Speaker 2: Not probably doing better now oh I see yeah I see.
- 1:12:45Speaker 1: But the entire transplant thing works in a way that
- 1:12:47you take out your own hair follicles and From different
- 1:12:51other parts of your body and you just insert that in, right?
- 1:12:53Like that's essentially.
- 1:12:55Speaker 2: You can say that's the summary. Yeah. Okay.
- 1:13:00Speaker 1: Can it go wrong also in certain cases?
- 1:13:04Speaker 2: See, if you're transplanting 4,000 follicles, you cannot mess up
- 1:13:08with 4,000 follicles. Maybe you're a very bad surgeon. You've
- 1:13:11damaged 1,000. 3,000 should grow. It shouldn't happen that 4,000
- 1:13:13out of 4,000 don't grow. But the bigger problem is
- 1:13:17the hairline design, the direction of the hair follicles, the
- 1:13:22safety of the anesthesia. Because long surgery, 8-hour surgery, so
- 1:13:26you need to have the right calculation of anesthesia that
- 1:13:31you're giving. That is where most of the deaths in
- 1:13:34air transplant have happened. Because of the wrong calculation of
- 1:13:39the amount of anesthesia that is given to the patient.
- 1:13:42There must be a monitoring system installed. if you're getting
- 1:13:45it done because mostly it's being done in the clinic
- 1:13:47not in a hospital right so they must have a
- 1:13:50monitoring system um with all vital monitors should be there
- 1:13:55and they must have a tie-up with a nearby hospital
- 1:13:57that something if happens then they can emergency they can
- 1:14:01go up to them so these things must be there
- 1:14:04should be a quality center don't take it lightly you're
- 1:14:07just going to get it done once or twice in
- 1:14:09it in your.
- 1:14:10Speaker 1: Lifetime twice why it doesn't work.
- 1:14:12Speaker 2: Once No, if you have more hair loss with time,
- 1:14:14then you might need it twice, right? So you should
- 1:14:17get it done right.
- 1:14:19Speaker 1: Okay, so who is the hair transplant really for? Who's
- 1:14:24a good candidate for it?
- 1:14:25Speaker 2: For anybody who has lost a patch of hair, where
- 1:14:30the skin is showing and there's no hair there, they
- 1:14:32can get a transplant done. But a lot of people
- 1:14:34get it done for increasing hair density also as we
- 1:14:37grow older.
- 1:14:38Speaker 1: Okay, and that works?
- 1:14:40Speaker 2: That works. It works really well, actually. It's one of
- 1:14:43the most satisfying treatments for us because it's a complete
- 1:14:47360 makeover and patients are very happy. And that gives
- 1:14:50us happiness.
- 1:14:52Speaker 1: Is this what celebrities are getting done?
- 1:14:54Speaker 2: I think most of the celebrities have gotten transplant done.
- 1:14:57It's not necessary that they're getting it done for baldness.
- 1:15:00Some get it done if they reach 45-50 years. They
- 1:15:03just want to add more density in their hairline. It
- 1:15:06can also be done for that. Okay.
- 1:15:08Speaker 1: But does it work for women?
- 1:15:09Speaker 2: Yes, transplant can be done for the women as well.
- 1:15:12Hair follicle is the same for men and women. And
- 1:15:15it grows in the next one year or so.
- 1:15:19Speaker 1: That's a very long period.
- 1:15:20Speaker 2: Right. No, but you start to see results from three
- 1:15:23months onward. But full thickness will come around one, one
- 1:15:25and a half years. Okay.
- 1:15:27Speaker 1: This sounds very messy, tiring, expensive and labor-intensive. There is
- 1:15:35no cream like this. We're all staying in 2026. I'm
- 1:15:41sure science has caught up.
- 1:15:43Speaker 2: Right. There's a treatment coming up called Clascoterone, which is
- 1:15:46an androgen receptor blocker. It blocks that receptor. That's why
- 1:15:50DHT can't work. It's already approved for acne and is
- 1:15:52in the market. And for this, in phase 3 trials,
- 1:16:06I think, for hair loss. So when that comes, we
- 1:16:08will have another treatment in our weaponry to fight against
- 1:16:13androgenetic alopecia. So really looking forward to that as well.
- 1:16:17Speaker 1: You will not prescribe it till the time it's got
- 1:16:18FDA approval, of course.
- 1:16:19Speaker 2: It's not available right now commercially. It's in phase 3
- 1:16:22right now. So once it is available commercially, then only
- 1:16:25I can prescribe.
- 1:16:26Speaker 1: Yeah, but that has not stopped people from using a
- 1:16:28lot of stuff.
- 1:16:29Speaker 2: Yeah.
- 1:16:30Speaker 1: Let me... throw at you some popular treatments that happen
- 1:16:36both in the salon and inside a dermat clinic and
- 1:16:38I want you to rate it on a scale of
- 1:16:401 to 10 10 being the highest and the best
- 1:16:43okay let's start with the easy one topical minoxidil 10
- 1:16:49minoxidil foam versus solution both are good okay topical finasteride
- 1:16:58topical finasteride is just 6 okay PRP.
- 1:17:03Speaker 2: Six.
- 1:17:04Speaker 1: PRF?
- 1:17:05Speaker 2: Four.
- 1:17:07Speaker 1: Exosome therapy?
- 1:17:10Speaker 2: Seven.
- 1:17:11Speaker 1: Mesotherapy?
- 1:17:13Speaker 2: Eight.
