Latest / Fork U with Dr. Terry Simpson / Tofu Isn't a Statin: Food as Medicine
Transcript
- 0:03>> Dr. Terry Simpson: You've probably heard the phrase, food is
- 0:06medicine. It sounds good, right? Ancient
- 0:09wisdom, supposedly from Hippocrates, the father of
- 0:11medicine. Except here's the problem.
- 0:14Hippocrates never said it. Not once.
- 0:17It is not in any of the surviving writings. Zero
- 0:20evidence. Chances are someone a couple hundred years
- 0:23ago just slapped his name on it to give their
- 0:26idea more weight. And even Robert F.
- 0:28Kennedy, Jr. The chief grifter currently running
- 0:31Health and Human Services, quoted it as
- 0:35well. If RFK Jr. Is your historical reference,
- 0:38you're already in trouble. But the phrase stuck,
- 0:42and people cling to it like it is carved in marble. And
- 0:44as someone who's certified in culinary medicine, you
- 0:47might think I'd be all in on that idea. And sometimes
- 0:50I am. But food isn't the cure for everything.
- 0:53And let me be clear. Food is one of the
- 0:56most empowering tools we have. And
- 0:59I absolutely prescribe food for
- 1:02patients. The right food can prevent, slow, and sometimes
- 1:05even reverse some, but not all, chronic diseases.
- 1:08But here's the catch. While tofu, tempeh,
- 1:11almonds, and oats are fantastic, they're not going to
- 1:14replace your pharmacy.
- 1:16Which brings us to today's episode,
- 1:19the Portfolio Diet and why Tofu
- 1:22isn't a Statin.
- 1:26I am your Chief Medical Explanationist, Dr. Terry
- 1:29Simpson, and this is Fork U Fork
- 1:32University, where we make sense of the madness,
- 1:34bust a few myths, and teach you a little bit about food
- 1:38and medicine.
- 1:42The Portfolio Diet was designed by Dr.
- 1:45David Jenkins, the same scientist who gave us
- 1:48the glycemic H index. The idea
- 1:51is simple. Combine a portfolio of foods that each
- 1:54lower cholesterol just a little, like
- 1:56nuts. Plant proteins like soy.
- 1:59Viscous fiber like oats, barley, psyllium.
- 2:02Plant sterols from fortified foods. Together,
- 2:05they act on cholesterol recycling through what's called the
- 2:08enterohepatic circulation. Okay, here's
- 2:11how that works. Your liver makes cholesterol,
- 2:14it packages it into bile and and sends it
- 2:16down into your gut through your common bile duct.
- 2:19Normally, most of that bile and the cholesterol in it gets
- 2:22reabsorbed back into your bloodstream. But when
- 2:25you eat viscous fiber and plant sterols,
- 2:28those foods bind to the cholesterol and carry it out
- 2:31of your body. That's one of the reasons your bowel movements are
- 2:34brown. Bile is brown. And when fiber
- 2:36catches it and drags it out, you flush away
- 2:39some of the cholesterol with it. It's a win win. You feed
- 2:42your gut microbes and you keep cholesterol from sneaking
- 2:45back into circulation. The issue is
- 2:49can you stick to eating that much fiber every
- 2:51single day? That's the rub. It's a
- 2:54lot easier just to take Crestor like I do.
- 2:57There's some clinical data and here's where it gets
- 2:59impressive. The portfolio diet
- 3:02isn't just theory. It's been tested.
- 3:06A randomized control trial published in the Journal of the American
- 3:09Medical association compared the portfolio diet against
- 3:12standard low saturated fat diets in patients with high
- 3:15cholesterol. And here's what they found. If your starting
- 3:18LDL was about 171 with the
- 3:20portfolio diet, LDL dropped by
- 3:23about 13 or 14%, or about 24 to
- 3:2626 milligrams per deciliter over six months.
- 3:29In the most adherent patients,
- 3:32the reductions exceeded 20%
- 3:35at one year. Now this wasn't a one off.
- 3:38Multiple meta analysis and systematic reviews
- 3:41confirm these results showing that mean LDL, which
- 3:44is the bad cholesterol reduction of about
- 3:4717%. The portfolio diet also
- 3:49improved other cardiometabolic risk Factors including
- 3:52non HDL cholesterol, apolipoprotein
- 3:55D and the estimated 10 year coronary
- 3:58heart disease risk. So yes, the
- 4:00portfolio diet works. It is
- 4:03especially true in the most motivated adherent patients.
- 4:06The LDL reduction is actually comparable to first
- 4:09generation statins like lovastatin.
- 4:12But let's make this real. What does it take to
- 4:15get those results? So here's a day in the life of the
- 4:18portfolio diet. Breakfast bowl of
- 4:20oatmeal made with soy milk that gives you viscous fiber and
- 4:23soy protein. One Shot Snack handful of
- 4:26almonds, about 25 or 30 grams. Not Trail
- 4:29mix, which I love. The candy coated
- 4:32stuff. Nope, this is just plain almonds. Lunch A cup of
- 4:35lentil soup, maybe with a slice of whole grain bread.
- 4:38More fiber, more plant protein. Dinner
- 4:41Tofu stir fry with vegetables and barley on the side.
- 4:43That's more soy, more viscous fiber. Somewhere in
- 4:46the day you'll need plant sterols, usually in the form of a margarine
- 4:49spread, about 2 grams worth. That's how you
- 4:52hit the therapeutic doses. Soy protein,
- 4:55viscous fiber, nuts and sterols every day
- 4:58consistently. And that's the challenge. It
- 5:01takes planning and honestly, it takes
- 5:03motivation. It's not impossible, but it's
- 5:06harder than taking medicine.
- 5:09Now you might be thinking, wait a minute, isn't this basically the
- 5:12Mediterranean diet? And the answer is kind of
- 5:15both diets are plant forward, emphasize nuts, legumes,
- 5:17whole grains and fiber. Both lower LDL cholesterol, both
- 5:20reduce cardiovascular risk, but they are not
- 5:23identical. The Mediterranean diet is a broader
- 5:26lifestyle pattern. It has olive oil, fish, vegetables,
- 5:29legumes, limited red meat. It's flexible, delicious,
- 5:32and a lot more embedded in most of our cultures. The
- 5:34portfolio diet is more prescriptive.
- 5:37You need specific amounts of soy protein,
- 5:40viscous fiber, almonds, and plant
- 5:42siderols every single day. It's less about
- 5:45lifestyle and more about hitting cholesterol with targeted
- 5:48foods. If the Mediterranean diet is the whole restaurant,
- 5:51olive oil, fish, vegetables, joy, the
- 5:53portfolio diet is just the cholesterol lowering section
- 5:56of the menu. You can actually fit the portfolio diet
- 5:59in the Mediterranean framework. And when you do, you'll probably get the
- 6:02best of both worlds and maybe a lot of
- 6:05sadness. But now let's
- 6:08continue. Let's, let's do some comparison. Let's go to the
- 6:11world of pharmacy. Let's take statins like atorvastatin
- 6:14or rosuvastatin, which is Crestor, the medicine. I take
- 6:17those two typically lower LDL by
- 6:20double that amount, 30 to 50%. That's
- 6:23double or sometimes triple what you get on the portfolio diet.
- 6:26And they don't just change cholesterol numbers.
- 6:29They reduce heart attacks and strokes by 25 to
- 6:3240%. And here's the kicker.
- 6:35When you add zdia, also known as
- 6:37ezetimide, it also, it's another drug. It
- 6:40blocks cholesterol absorption in the gut. So
- 6:43that's basically the portfolio diet on high
- 6:45octane. On its own, ZD can lower
- 6:48LDL by about 15 or 20%, similar to the diet.
- 6:51But when you combine zdia with a statin, LDL
- 6:54drops another 20% on top of what the statin does.
- 6:57So a statin alone might give you a 50% reduction in
- 7:00LDL. At Zetia, you're at 65 or 70%. That's
- 7:03massive and far more consistent than anyone can
- 7:06manage with oatmeal and tofu alone. Let me give you a real life
- 7:09example. Me, I have familial
- 7:12hypercholesterolemia. My father had a heart
- 7:15attack when he was 55. My brother had a heart attack
- 7:17at age 50. I have high cholesterol.
- 7:20My LDL, the bad cholesterol, runs at about
- 7:23190 if I do nothing with it. But I
- 7:26take both Crestor, Rosuvastatin
- 7:29and Ezetimibe. And my LDL
- 7:32is in the 40s, which is remarkable
- 7:35because that is lower than the number we think that is
- 7:37needed to have me continue to form
- 7:40atherosclerotic plaque. It's great.
- 7:42So what about red yeast rice? There's a story here. And
- 7:45here's the before we had prescription statins,
- 7:48we had red yeast rice.
- 7:51It's a traditional Chinese food that produces something
- 7:54called Monacolin K, chemically
- 7:57identical to lovastatin, the very first
- 8:00statin drug. Sounds great, right? A, uh, natural
- 8:02statin, except the dose is
- 8:05unpredictable.
- 8:05One batch might have none, another might have near drug levels.
- 8:09Many samples are contaminated with a, uh, kidney
- 8:12toxin. And the FDA said if there's enough monoclin
- 8:15K to work, it's not a supplement, it's an unapproved drug.
- 8:19So even when it works, Red rice yeast
- 8:22is basically a first generation statin. And here's what that
- 8:25means. It's less effective than fourth generation
- 8:27and it has far more side effects than modern fourth
- 8:30generation statins. So meanwhile, Crestor
- 8:33and atorvastatin or Lipitor are more potent,
- 8:37safer and consistent. So if you want a
- 8:39statin, go to your doctor, not the supplement
- 8:42side. So here's the takeaways. The
- 8:45evidence is clear. The portfolio diets lower LDL
- 8:48by 17% at best, more if you stick with
- 8:51it. And that's real meaningful. And that's why
- 8:54diet always matters. The Mediterranean diet
- 8:57is broader, easier, proven to reduce events
- 9:00when followed long term. But statins, they're
- 9:03stronger, safer, have the best evidence for
- 9:06preventing heart attacks and strokes. And when you combine them with
- 9:08zdia, the pharmacologic version of the portfolio
- 9:11diet, you can lower LDL by nearly
- 9:1470%. The best outcomes happen
- 9:17when you do all three, though. Eat Mediterranean,
- 9:20weave in portfolio elements. Take a statin
- 9:23if your doctor prescribes it. Take Xetia if you can,
- 9:26or some of the newer Drugs like the PCSK9 inhibitors
- 9:29or bempeditic acid. Diet is
- 9:32always a foundation. It is the empowering thing that
- 9:35you can do. Medicine builds the house, and, uh,
- 9:37together they keep the roof from caving
- 9:40in.
- 9:44This has been episode one of a new series where we're
- 9:47calling Food isn't a Prescription Pad. And up
- 9:50next, we're going to revisit the dash diet and blood pressure.
- 9:53How beans, bananas and leafy greens can lower your numbers.
- 9:56But maybe not enough to toss out the pill bottle just yet.
- 9:59I'm Dr. Terry Simpson, your chief medical explanationist.
- 10:02Remember, tofu and almonds are great, but they're not
- 10:05Crestor. I have researched and
- 10:08written, obviously this episode, but
- 10:10remember, while I am a board certified
- 10:13physician, I am not your physician.
- 10:16This podcast is for education, not personal medical
- 10:19advice. If you need to talk about lowering your
- 10:22cholesterol, or if you have a history, family history, please
- 10:25see your own doctor. And if you need dietary advice,
- 10:27a registered dietitian before making any
- 10:30changes to your healthcare. Please don't see a chiropractor. They
- 10:33can't prescribe anything and they're oftentimes anti
- 10:36statin and anti whatever. Please don't see an eastern trained
- 10:39shaman, a real doctor. This is in
- 10:41their wheelhouse. Also,
- 10:44for more deeper dives into this, please see the
- 10:47blog associated with this podcast
- 10:49either@yourdoctorsorders.com or
- 10:52or4q.com and for more
- 10:54Mediterranean style types of foods go
- 10:57to my website terrysimpson.com
- 11:00thank you everybody. Have a good week.
- 11:07You know Ivo, some people think if they eat oatmeal,
- 11:09soy milk and almonds, they can ditch a statin. But
- 11:12that's like saying you can patch a leaky roof with duct tape.
- 11:15It'll hold until the microburst monsoon or
- 11:18whatever big storm hits you. And in this case, it. It's worse than
- 11:21a storm. It's like a heart attack or a stroke.
- 11:24So I'm going to keep my statin and let's keep both
- 11:27of our cholesterols down. Because you know what? We're coming up on
- 11:30100 episodes of 4Q and we've got
- 11:32about 900 more episodes to go before
- 11:35we can retire.
- 11:39Wait, hold on. I get retirement benefits with
- 11:41this gig. Sweet
- 11:46SA.
