- 0:00Hello, everyone. This is Jacob Emerson with the
- 0:02Becker's Payer Issues podcast. Thrilled today to be
- 0:05joined by a special guest. Doctor Todd May
- 0:08is vice president and medical director at Health
- 0:11Net. Doctor May, thank you so much for
- 0:14taking the time to be with me on
- 0:15the podcast today.
- 0:16Jacob, thank you for having me. I'm I'm
- 0:18very pleased to be here.
- 0:20I'll just mention,
- 0:21importantly that I am a practicing family physician,
- 0:25and I've been practicing
- 0:27primary care for over thirty years. So I
- 0:29I have deep, deep experience in the in
- 0:31this area and, a lot of interest. Fantastic.
- 0:34And I appreciate you you diving in,
- 0:37to your career background, Todd. And before we
- 0:39get started with everything you want we wanna
- 0:41talk with you about, can you tell us
- 0:42a little bit more about your current role
- 0:45at Health Net and what you do?
- 0:48Sure. Sure. I,
- 0:50I am the executive representing the commercial line
- 0:53of business. I am involved in in all
- 0:55things,
- 0:56clinical, sup supporting supporting the business.
- 1:00I'm very deeply involved in population health management,
- 1:03which is our,
- 1:04approach to optimizing the care for,
- 1:08all of our members, across all lines of
- 1:10business,
- 1:11deeply involved in quality improvement,
- 1:13health equity,
- 1:15looking at,
- 1:16member engagement platforms, digital tools,
- 1:20that kind of thing to to
- 1:22maximize engagement
- 1:23of our members in their in their own
- 1:25health care.
- 1:27Fantastic. So, Todd, we're we're here today to
- 1:29talk with you about a really big and
- 1:31special initiative.
- 1:34If multiple insurers are are coming together to
- 1:37or or have. They've launched a shared value
- 1:40based payment model for reimbursing primary care physicians
- 1:43in California.
- 1:45That initiative includes collaboration from Aetna, Blue Shield
- 1:48of California,
- 1:49and, of course, HealthNets
- 1:51along with 11 providers
- 1:53across the state.
- 1:54So talk to us a little bit about
- 1:56this. What what does this mean? What were
- 1:58some of the key factors in bringing all
- 2:01these stakeholders together?
- 2:03Well, Jacob, first, thank you for recognizing how
- 2:06big of a deal this is. This is
- 2:08unique, innovative,
- 2:09groundbreaking work.
- 2:11And and, you know, there's been we have
- 2:13a we have a fragmented health care system.
- 2:15Right, and there's been realization for the health
- 2:18plans
- 2:19for some time that to have a real
- 2:20impact on broad systems like health care delivery
- 2:23and primary care specifically
- 2:25that we can work together to align with
- 2:28a cohesive multi plan strategy.
- 2:30And and this demonstration project
- 2:33stems from a partnership,
- 2:34agreement that we made, in July 2022
- 2:38with HealthEd and, and, and several other payer
- 2:41organizations.
- 2:42And we've all been deeply engaged, in this
- 2:45work,
- 2:46from the very beginning.
- 2:47It was
- 2:48California
- 2:49Quality Collaborative, the CQC,
- 2:52and Integrated Healthcare Association, IHA,
- 2:55who brought us together. They were the conveners,
- 2:57of this group, and they can continue to
- 3:00facilitate
- 3:01the project.
- 3:03And,
- 3:04you know, there's real value,
- 3:06in this approach when
- 3:07offering provider organizations a cohesive, aligned strategy
- 3:12that really helps them transform
- 3:14into becoming higher functioning
- 3:16advanced primary care practices.
- 3:18And and a high functioning primary care
- 3:22is is a foundation for a high quality
- 3:24and cost effective health care system.
- 3:26And when this happens, we're better able to
- 3:28meet the needs of patients and improve health
- 3:31outcomes
- 3:32that really matter. For for example,
- 3:35you know, the sweet spot for for primary
- 3:37care
- 3:38are are managing chronic conditions like
- 3:41diabetes and and high blood pressure. And,
- 3:44you know, why are these important? Well, these
- 3:46are
- 3:47two of the most prevalent conditions in The
- 3:48United States and their
- 3:50underlying causes of cardiovascular
- 3:52disease, which is the primary,
- 3:54cause of,
- 3:55death in our country. And we also focus
- 3:57on
- 3:58prevention, like cancer screening vaccinations,
- 4:02that kind of thing that really impacts impacts
- 4:04health.
- 4:06Absolutely. And and, Todd, let's dive into this
- 4:08a little bit more because this new model
- 4:10is shifting
- 4:12individual payment systems to a single shared system
- 4:15that rewards
- 4:16high quality care.
- 4:18How does that approach differ from the traditional
- 4:21fee for service models we all know,
- 4:23and what are some of the specific metrics
- 4:25or outcomes that are being used to measure
- 4:27and reward,
- 4:29physicians' performances?
- 4:31Yeah. You know, value based care differs from
- 4:34traditional health care by prior prioritizing patient outcomes
- 4:38and the quality of care over the volume
- 4:41of services provided. And that and that means,
- 4:44our providers are financially rewarded for achieving positive
- 4:46health results for their patients
- 4:48rather than the number of services delivered.
- 4:51In the traditional long standing fee for service
- 4:54models,
- 4:55providers are paid for the volume of preserve
- 4:58services they provide. And, you know, that that's
- 5:01irrespective of the impact
- 5:03on or or outcomes that they achieve. I
- 5:04mean, whether they do any good or not
- 5:06doesn't actually factor into the equation. They get
- 5:09paid for
- 5:10doing things.
- 5:11So this shift,
- 5:12really encourages a more holistic and proactive
- 5:16approach to patient care. We're focusing on preventioning,
- 5:19prevention, mind managing chronic conditions,
- 5:22and really looking at the overall overall health
- 5:24status of patients
- 5:26rather than just treating acute issues or or
- 5:28whatever they, happen to come in with that
- 5:31day.
- 5:32And, you know, HealthNet and other health plans
- 5:34have been engaged in
- 5:36in this value based purchasing,
- 5:39arrangement for some time. This is not a
- 5:41new concept.
- 5:42But we we all did as individual plans
- 5:45with individual models and priorities,
- 5:47you know, so help, you know, provider groups
- 5:50just have,
- 5:51a bunch of different health plans,
- 5:53providing different incentives and that kind of thing.
- 5:55So so what we're doing here with this
- 5:57demonstration project
- 5:59that's that's really revolutionary
- 6:02is this multi plan alignment.
- 6:04We're all rowing in the same direction, and
- 6:06the practices know,
- 6:08what's expected. And we're we're really amplifying the
- 6:11impact,
- 6:12that we have in producing high quality,
- 6:14high value results.
- 6:17And, you know, the the health plans all
- 6:19came together with CQC, NIAJ.
- 6:21We developed this together from the very beginning
- 6:24from the conception design payment models.
- 6:26We developed the metrics, which I'll get to
- 6:29in a moment.
- 6:30We got the implementation plan, then we're gonna
- 6:32assess the outcomes.
- 6:34Now in terms of metrics,
- 6:36we jointly developed key performance metrics that reflect,
- 6:40high quality and high value member outcomes.
- 6:43And these metrics this is really important. These
- 6:45are closely aligned with the priority metrics
- 6:49that our major purchasers
- 6:51have identified
- 6:53as well as key leaders in the industry.
- 6:56So at a high level, these metrics include
- 6:59health outcomes and prevention.
- 7:01Talked about diabetes, high blood pressure,
- 7:04immunizations, and cancer screenings.
- 7:06We're looking at patient reported outcomes and focusing
- 7:09on
- 7:10depression status.
- 7:12We're looking at patient safety,
- 7:15looking at minimizing high risk drug combinations.
- 7:19We're looking at patient experience using industry standard
- 7:22surveys.
- 7:23That's really key. We really hope to move
- 7:25the dial, the the needle there,
- 7:28and also high value care, which is, you
- 7:30know, minimizing you know, with with really strong,
- 7:33primary care, we hope to minimize
- 7:36emergency departments and, inpatient hospital utilization.
- 7:40Those are the those are the key metrics
- 7:42that we are targeting and that we are
- 7:44rewarding
- 7:45the practices
- 7:46for achieving.
- 7:48Absolutely. And going off of that, Todd, with
- 7:50everything you just said and and these and
- 7:52how these metrics were developed,
- 7:54why were small
- 7:55independent practices chosen as the starting point for
- 7:59this new model?
- 8:00And have you learned any lessons so far,
- 8:03being only just a short amount of time,
- 8:05since implementation?
- 8:07Yeah. That that's a really great question. So
- 8:09so like I said, primary care is the
- 8:11foundation of a high functioning
- 8:13health system.
- 8:14And we have while while we have
- 8:17growing
- 8:18consolidation in in health care delivery,
- 8:20we still have, you know, a vast, vast
- 8:23number of small independent practices out there providing
- 8:27a substantial portion of primary care.
- 8:30Now this advanced primary care model is an
- 8:32integrated, comprehensive,
- 8:34patient centered, team based care model, and we're
- 8:37supporting these practices
- 8:39in in adopting
- 8:41this broader,
- 8:42broader approach to care. It's not just contingent
- 8:45on that doctor in the room,
- 8:47but it's more of a team approach. So
- 8:49everybody in the office,
- 8:51can contribute
- 8:52to, this population health,
- 8:55management approach. And together, they look at all
- 8:58the patients,
- 8:59use data,
- 9:00identify their needs, and then proactively
- 9:03reach out to to their patients and close
- 9:06their care care gaps.
- 9:08Now we've we've focused on these small independent
- 9:10practices because they're they're largely under resourced,
- 9:14and they they lack the size and infrastructure
- 9:17to implement these changes on their own. I
- 9:19mean, it takes work. It takes effort to
- 9:21do this, to to change your,
- 9:24your, you know, standard way of doing things.
- 9:27Now larger provider groups tend to be better
- 9:29resourced,
- 9:31to enact these kinds of changes. So,
- 9:35with the with the, demonstration project, we're excited,
- 9:39with where we're, where we're at and what
- 9:41we're seeing.
- 9:42I'll just mention we're calling this a demonstration
- 9:45project instead of a pilot, and there's a
- 9:47reason for that.
- 9:49Pilots are, like, initial attempts to just test
- 9:52out a new idea,
- 9:53But this this project is based on science
- 9:56and experience, and this methodology has been proven.
- 10:00We we know that advanced primary care works,
- 10:03so we we are we're moving ahead beyond
- 10:06just a a test pilot.
- 10:09And so far, we've,
- 10:11we've learned that health plans can indeed come
- 10:13together collaboratively,
- 10:15get in the same room, elbow to elbow,
- 10:18align around key innovations and operational interventions,
- 10:22and, we've been able to successfully
- 10:25engage targeted practices,
- 10:28to commit to practice transformation
- 10:31in in adopting this advanced primary care model.
- 10:34And we're also finding high interest from these
- 10:37practices. They're eager to receive support. They wanna
- 10:40be helped. They wanna do better.
- 10:42They know it's hard work and and that
- 10:43they can't do it on their own. So
- 10:45they really appreciate,
- 10:47both the financial resources, but also
- 10:50the the training and coaching,
- 10:52that we're providing,
- 10:54to to make this transition.
- 10:57Yeah. And, Todd, let me follow-up with a
- 10:59point, one of those last points you just
- 11:01made that this is really bringing together a
- 11:03lot of organizations that in the past may
- 11:05have been directly competing
- 11:07with one another. So how do you see
- 11:09this demonstration
- 11:10project setting a precedent for for other insurers,
- 11:14for other providers throughout the country?
- 11:17Well, like I say I mean, this is
- 11:18what's groundbreaking.
- 11:19Right? I mean, who would imagine,
- 11:22the health plans
- 11:24competitors, yes, getting together literally in the same
- 11:27room
- 11:28and talking this all through, working this all
- 11:30out, coming to
- 11:32common
- 11:33agreements,
- 11:34with a focus on
- 11:36you know, we we we keep we keep
- 11:38the business separate. Right? This is this is
- 11:40really about
- 11:42quality. When you get down to it, we're
- 11:44all it's all about quality, improving outcomes, improving
- 11:48the care, improving the value of care that
- 11:51the people of California receive.
- 11:53And we can all agree on that. Right?
- 11:55I mean, there there's
- 11:57we we should we should be
- 11:59all,
- 12:01promoting that across the board for all Californians,
- 12:03and that's what we're doing here. And we
- 12:05found that through this alignment,
- 12:08we can really amplify our impact and make
- 12:11a difference, and it just makes a lot
- 12:13of sense. Now what we what we hope
- 12:15to do with this demonstration
- 12:17project is,
- 12:19show that, yes, indeed, health plans can do
- 12:22this. They can align. They can come together
- 12:25with strength in numbers
- 12:27and the and the amplification
- 12:29that we can,
- 12:32really help practices,
- 12:35adopt this model and provide,
- 12:38higher value care
- 12:39with it. Obviously,
- 12:41the long term goal here, demonstrate this works,
- 12:44and then scale it. Scale it across the
- 12:46across the state, maybe across the country,
- 12:49get other plans to to come on board
- 12:52and
- 12:53and really, really move the dial.
- 12:56Yeah. I mean, it's really exciting to to
- 12:58think about the potentials here, Todd. But let
- 13:00me ask you one last question about some
- 13:02of the technical
- 13:03aspects of this model,
- 13:05because as you've said throughout our our conversation
- 13:07today, you're really looking to address reimbursement challenges,
- 13:10improve health equity, and boost health outcomes among
- 13:13your your members,
- 13:14particularly those who may have been underserved
- 13:17in the past. So
- 13:19how are you specifically addressing some of those
- 13:21disparities in care, and how does a tool
- 13:24like the like the Coziva platform play in
- 13:26ensuring that those goals can be met?
- 13:30Great.
- 13:32So
- 13:33the the key here,
- 13:35is data.
- 13:36Timely, accurate, and actionable data is the key
- 13:39to all performance improvement efforts across the board,
- 13:42and that's especially,
- 13:44true with health equity. And so we're making
- 13:47it easier for the provider organizations to access
- 13:51their patient data.
- 13:52You know, we we like I mentioned, we
- 13:54have a fragmented system.
- 13:56Our data, our information, our medical records
- 13:59is equally
- 14:01fragmented, and that makes it really difficult to
- 14:03get a comprehensive view
- 14:05of the care that patients have had and
- 14:08identify their needs.
- 14:10So
- 14:11with this demonstration project,
- 14:14the three plans together
- 14:16are providing access to a uniform data platform,
- 14:20a single source of data, a single source
- 14:22of information
- 14:23where the pro practices can view key patient
- 14:27clinical information for our members
- 14:29for all of the plans
- 14:31in one place, on one dashboard.
- 14:34And
- 14:35access to and use of accurate data foundation
- 14:37of health equity work, and the way that
- 14:40that works is we stratify
- 14:42data. So we have a you know, look
- 14:45at a practice. They have their full panel
- 14:47patients. It's a diverse,
- 14:50array of folks in in any given practice.
- 14:53What you do in health equity is you
- 14:54take that data and then stratify
- 14:57by race, ethnicity,
- 14:59language,
- 15:00and other parameters,
- 15:02and then you compare the groups. And that
- 15:04reveals
- 15:05disparities
- 15:06in the services that that are provided.
- 15:09It uncovers
- 15:10things that you would not see unless you
- 15:13look at the data and stratify the data.
- 15:16So then we can identify those gaps in
- 15:18care,
- 15:18and then practices
- 15:20can proactively,
- 15:22address those gaps to mitigate health, health disparities.
- 15:27So the, you know, the goal here is
- 15:30visibility,
- 15:31see where the gaps of care are,
- 15:33take action,
- 15:34proactively
- 15:35outreach to specific especially
- 15:38underserved members. Now these are folks who may
- 15:41have fewer resources
- 15:43and fewer opportunities to access this care on
- 15:45their own. So
- 15:47by reaching out, bringing them in, or delivering
- 15:50the services to them, we can promote health
- 15:52equity.
- 15:54Fantastic. Well, Todd, we've got a lot of
- 15:56other health plan and health system leaders listening
- 16:00in right now. Any other final thoughts or
- 16:03final bits of advice you wanna offer them
- 16:05based off of everything we've just chatted about?
- 16:08Yeah. Well, first of all,
- 16:10watch this. Pay attention to what we're doing
- 16:12here. We follow this along. We'll we'll obviously,
- 16:16give status reports as we go, and,
- 16:20and we'll,
- 16:21share the results, that we demonstrate. We hope
- 16:23to show very positive results.
- 16:25So that's one, keep a close eye on
- 16:27this.
- 16:28And two,
- 16:30don't be afraid to jump in. Don't you
- 16:32know, it's it I know it seems foreign,
- 16:34sounds,
- 16:36sounds counterintuitive
- 16:37for competitors
- 16:39to get together
- 16:40and do good work together,
- 16:42but it makes sense. Okay. Then we're not,
- 16:45violating any
- 16:46any business comp competition practices or
- 16:50sharing proprietary information. There's none of that. We
- 16:53are focused on
- 16:55improving the health outcomes of all Californians, and
- 16:58everybody can get behind that.
- 17:01Wonderful. Well, doctor May, thank you so much
- 17:03for taking the time to sit down with
- 17:05us and for sharing your insights with our
- 17:07listeners. We really appreciate it.
- 17:10Jacob, thank you very much for having me
- 17:11today.
