- 0:00This is Laura Dirda with the Becker's Healthcare
- 0:02Podcast.
- 0:03I'm thrilled today to be joined by Simon
- 0:05Nazarian, executive vice president and chief digital technology
- 0:08officer at City of Hope, Nassim
- 0:10Eftikari,
- 0:12who is the chief AI and analytics officer
- 0:14at City of Hope. Nasim, Simon, thank you
- 0:16so much for joining us on the podcast
- 0:17today.
- 0:18Happy to be here. Great. Thanks for having
- 0:20us.
- 0:21Fantastic. Well, I'm looking forward to learning more
- 0:23about everything you're doing at City of Hope.
- 0:25I know it's a fantastic organization, really innovative
- 0:28and on the cutting edge of what's happening
- 0:30in cancer care today and more. So I'm
- 0:32excited to dig in. But before we do,
- 0:34can you tell me a little bit more
- 0:35about yourself? Give us a a bit of
- 0:37your backgrounds and then, a little bit about
- 0:39City of Hope too.
- 0:41Fantastic.
- 0:42I can start.
- 0:43Hi. My name is Simon Nazarian,
- 0:45and, I'm the executive vice president and chief
- 0:48digital and technology officer
- 0:50for City of Hope. And,
- 0:53you know, it's a exciting role we're playing
- 0:55today,
- 0:56especially, since City of Hope,
- 0:58was founded in 1913.
- 1:01We've been focused on providing compassionate care to
- 1:04people in need.
- 1:07And, you know, we really focus on what's
- 1:09best for our patients, and that guides everything
- 1:11that we do.
- 1:12And in the past several years, City of
- 1:15Hope, has experienced tremendous growth.
- 1:18We're one of the largest and most advanced
- 1:21cancer research and treatment organizations in The US
- 1:24and, ranked among the nation's top cancer centers
- 1:27in The US. We're very proud of that.
- 1:30You know, City of Hope's, NCI designated
- 1:33comprehensive
- 1:34cancer center
- 1:36received the highest rating, from the NCI,
- 1:39and that puts us, City of Hope, in
- 1:41the top echelon of the nation's,
- 1:44you know, 57
- 1:45NCI designated comprehensive cancer centers.
- 1:48And,
- 1:49the
- 1:50and we are system wide, so we are
- 1:52in five major metropolitan
- 1:53areas,
- 1:55including LA, Orange County, Chicago, Atlanta, and Phoenix.
- 1:59And, 86,000,000
- 2:01people live in
- 2:02communities with close access to one of our
- 2:04facilities.
- 2:06K?
- 2:07And, you know, in my role, I,
- 2:10you know, oversee,
- 2:11digital technology enterprise platforms
- 2:14that really support,
- 2:16care at scale. So anything that has to
- 2:18do with technology rolls up to my organization.
- 2:21And, you know, in partnership with Nasim, really
- 2:23focused on how innovation actually moves from
- 2:27a idea to adoption, you know, in a
- 2:29very complex health system.
- 2:31Right? And we wanna make sure that, you
- 2:33know, we build a durable infrastructure
- 2:36and just, you know, not one off pilots
- 2:38that don't scale.
- 2:40Thank you, Simon, for that great introduction of
- 2:42City of Hope. I'm Nasim,
- 2:44chief AI and analytics officer.
- 2:46I
- 2:47lead,
- 2:48meaningful application of machine learning AI, under Simon's
- 2:52leadership
- 2:53in all areas of patient care,
- 2:55research, precision medicine, and operations.
- 2:58I've been with the company for nine years
- 3:01now, and,
- 3:03and my background is in computer science and
- 3:05machine learning.
- 3:07That's amazing to hear. And, you know, really
- 3:09great to have that experience at City of
- 3:11Hope and then be able to support its
- 3:14growth, which has been, you know, outstanding over
- 3:16the last several years.
- 3:18I'm curious. Could you tell me a little
- 3:19bit more about the most important initiative that
- 3:21you've led in the last year or so?
- 3:23What did you do, and what were the
- 3:24results?
- 3:26Well,
- 3:27I would say that, you know, we have
- 3:29a very strong focus on, governance and our
- 3:32operating model when it comes,
- 3:35to advance the an emerging technology. So,
- 3:38the most in important initiative wasn't just a
- 3:41single tool.
- 3:42It was really establishing the, you know, conditions
- 3:45for AI to scale responsibly,
- 3:48within our organizations.
- 3:50So, and and really focusing
- 3:52on,
- 3:54reframing
- 3:54our governance
- 3:56as an enabler, not a barrier. So, I
- 3:58would dare say as a partner, not a
- 4:00police.
- 4:01So, you know, help clarify the decision rights,
- 4:05you know, embedding the right guardrails,
- 4:07and,
- 4:09really moving governance closer to where the work
- 4:11is actually happening.
- 4:13So this is not a bolt on anymore
- 4:15for us. It's within the structure of our
- 4:17organization.
- 4:19And,
- 4:20you know, we wanna make sure that, we
- 4:22do the right things for,
- 4:24for our folks and really focus on scale
- 4:27and velocity
- 4:28and, really partner with our organization.
- 4:31Nassim, anything you wanna add?
- 4:34Sure. So I,
- 4:36I wanna,
- 4:37add a little bit to the governance piece.
- 4:40We we've been,
- 4:41we've been giving a lot of thought to
- 4:44how do we safely and responsibly
- 4:47apply these technologies, but also balance the speed
- 4:49of innovation.
- 4:50Like Simon said, we have safe and responsible
- 4:53use of AI.
- 4:54We have had a lot of, like, you
- 4:56know, very,
- 4:57tangible exciting initiatives that we've been leading over
- 5:00the past year, like using generative AI for
- 5:03patient onboarding,
- 5:04for clinical trial matching,
- 5:07and and many other things in digital pathology
- 5:09and precision medicine and others.
- 5:12But one of the most important things that
- 5:14I keep thinking about day in day out
- 5:16is and and it ties into governance a
- 5:19little bit and scale and velocity is how
- 5:23do we
- 5:24really orchestrate
- 5:25all of these different AI tools and these
- 5:27days, AI agents
- 5:29that every organization
- 5:30is adopting for point solutions.
- 5:33So
- 5:34we we wanna be, we wanna get ahead
- 5:36of it so that
- 5:38in, like,
- 5:39in a year or so, we don't have,
- 5:42tens, hundreds, thousands of agents running around the
- 5:45organization, each doing their own thing, and they
- 5:48cannot talk to each other.
- 5:49So I think, in terms of governance and
- 5:52also technology,
- 5:54a big top of mind for me is
- 5:56now how do we manage,
- 5:59all these AI agents and tools, not just
- 6:01agents, but AI tools and agents. How do
- 6:04we orchestrate them to work effectively with one
- 6:07another
- 6:08and, and optimize the whole system rather than
- 6:10just one part of, like, a
- 6:13a local optimization of we solve this problem,
- 6:16but then we shift the bottleneck somewhere else,
- 6:19and then we have to solve that other
- 6:20problem.
- 6:22That makes a lot of sense. You know,
- 6:24it's truly fascinating to see all of the
- 6:25different possibilities that are out there with AI,
- 6:28but you're right. Being able to have the
- 6:29governance process and having visibility into everything that's
- 6:33going on, the different agents, and
- 6:36sharing that with the entire organization so you
- 6:38can smartly and effectively,
- 6:41incorporate
- 6:42AI without too much risk coming back to
- 6:44you. So, I think that's critical and so
- 6:46helpful to understand that process that you've taken
- 6:49and really built into,
- 6:50what you're doing at City of Hope. Now
- 6:52looking ahead for the next year or so,
- 6:54where are some of your big priorities and
- 6:55headwinds that you're focused on? What's really top
- 6:58of mind for you?
- 6:59Oh, man.
- 7:01A fantastic question.
- 7:03I think Nasim really hit it. A major
- 7:07priority is, scaling agency across the organization, but
- 7:11responsibly.
- 7:12It's
- 7:14really important for us to, empower our researchers,
- 7:17our clinicians,
- 7:19and our teams,
- 7:21to be able to create and run their
- 7:23own agents. Right? You know, they for them
- 7:25to
- 7:26be trained and,
- 7:27skilled to act with confidence,
- 7:30to,
- 7:31you know, use all of our intelligent tools
- 7:33responsibly,
- 7:34and make decisions closer, to the work. And,
- 7:38you know, one of my mantras at the
- 7:40office here is that, innovation is federated,
- 7:43and, we believe that,
- 7:46this really will help us, make that come
- 7:49true because
- 7:50we have to rethink the decision rights
- 7:53when we scale this agency and not just
- 7:55deploying a basic technology. Right?
- 7:58And the headwind here is, you know, most
- 8:00organizations
- 8:01are built for a centralized permission.
- 8:04Right? And, this work, you know, AI specifically
- 8:08requires distributed responsibility.
- 8:11So how do we
- 8:12shepherd the organization and work together in order
- 8:15to go through that kind of a change
- 8:17management?
- 8:18If it's if it's Simon's priority, then it's
- 8:20my priority. So on on my end,
- 8:23to enable this, one of the biggest thing,
- 8:26this year,
- 8:27is AI, literacy, fluency, education.
- 8:32How do
- 8:33we educate our workforce to
- 8:36work off of a shared knowledge base so
- 8:39that they can have this agency that, Simon
- 8:42is aspiring to? When we started,
- 8:45after the, AI boom,
- 8:47suddenly AI got democratized and everyone
- 8:50with very, like,
- 8:52in a very user friendly fashion could interact
- 8:54with AI, and that was amazing. That raised
- 8:56a lot of awareness about possibilities,
- 8:58but also, I think, exposes
- 9:01us and any other health care organization to
- 9:03a lot of risks. So we had to,
- 9:06hug AI close for some time
- 9:08and started educating, educating, educating so that people
- 9:12know if you're using this AI tool, it's
- 9:14great. It can make you more productive. But
- 9:16when you put something in it, it goes
- 9:18on the cloud. The data is now publicly
- 9:19available.
- 9:20So you don't put PHI into these publicly
- 9:23available tools. You should be using our enterprise
- 9:26instances of
- 9:27of, these AI assistants. So we started,
- 9:30using every tool that we have available to
- 9:33us from from emails that Simon and I
- 9:35send out to formal trainings
- 9:37and certifications, and we're still doing that. So
- 9:40I think that for me,
- 9:42I want to,
- 9:44I want all of our workforce to have
- 9:47that level of, fluency in AIs that we
- 9:50can really
- 9:51give agency to everyone to then build their
- 9:54own agents and don't cause, or or
- 9:57be less prone to causing an unintended harm.
- 10:00So that's AI education top of mind for
- 10:03me.
- 10:04That makes a lot of sense. And, you
- 10:05know, it's so critical for the organization. I
- 10:08think for some, it might be an easy
- 10:10leap, and for others, it takes a lot
- 10:12more to really help them understand what the
- 10:14AI is capable of doing and then understand
- 10:17the risks and and being able to act
- 10:19appropriately because of it.
- 10:21And and so I think that's really critical
- 10:23and cool to understand, you know, how you're
- 10:25developing those programs and then spreading it across
- 10:28the organization.
- 10:29And and within that, kind of mindset, what
- 10:31do you think the hardest thing about all
- 10:33of this, that you'll have to do in
- 10:35the coming year will be?
- 10:37The hardest thing,
- 10:39Simon, I'm going to steal your thunder here.
- 10:42The hardest thing has always been and will
- 10:45I I think for the foreseeable future, it
- 10:47will always be change management, adoption,
- 10:50changing the culture so that
- 10:53so that these tools
- 10:55make the intended impact that we're hoping they
- 10:58would make.
- 11:00Bringing,
- 11:01onboarding AI, tools,
- 11:03always requires a lot of changes to people's
- 11:06day to days and workflows, and we're talking
- 11:08about people who are already
- 11:10extremely busy and,
- 11:12and a lot of time set in their
- 11:14own ways.
- 11:15So, I think that's, something that in our,
- 11:19executive leadership team, and enterprise,
- 11:22technology group, we're always thinking about change management
- 11:26and how do we change the mindset and
- 11:28that make that culture shift that AI can
- 11:31be successful. Because I think the technology is
- 11:33there for the most part for a lot
- 11:35of things that we wanna accomplish, but, does
- 11:37it really get adopted and used and make
- 11:40the impact? That's the big question.
- 11:42And, and you've done a great job, Nassim,
- 11:45in in really facilitating the organization because,
- 11:48you know, we really wanna you know, our
- 11:50focus is on ensuring quality, trust, and usability,
- 11:56that key pace as these systems are growing.
- 11:59So and technology is really helping, you know,
- 12:01reduce the cognitive and administrative burden
- 12:05or, for our teams rather than adding to
- 12:07it.
- 12:08Right? And, when you move from a
- 12:11a tech enabled mindset
- 12:14to a tech forward organization,
- 12:17that requires new ways of working, you know,
- 12:19new decision making norms, and a lot of
- 12:21cultural adoptions. And,
- 12:23you know, and and, frankly, success depends on
- 12:26trust,
- 12:27you know, clarity and champion,
- 12:29and the champions across the organization.
- 12:32You know, this is not something you mandate.
- 12:34You gotta work together and then ownership move
- 12:36it forward. And
- 12:39it's been a a really interesting journey for
- 12:41Nassim and I as we, shepherd this through
- 12:44the organization.
- 12:46And that's amazing to hear. And, you know,
- 12:48truly, it sounds like,
- 12:50that move from the tech enabled mindset to
- 12:52the tech forward
- 12:54organization is something that so many health systems
- 12:56and hospitals and health care organizations across the
- 12:59country are are looking to do. And so
- 13:01your framework here in in some of the
- 13:03ways that you're looking at it from the
- 13:05leadership perspective,
- 13:06really is so helpful to understand.
- 13:08I'm curious. Before we wrap up, I wanna
- 13:11talk about growth too. Where do you see
- 13:12some of the best opportunities for organizational growth,
- 13:15especially as you're bringing in more of this
- 13:17education, more of these opportunities,
- 13:20and more,
- 13:21of that
- 13:22tech forward mindset,
- 13:24across the board?
- 13:26You know,
- 13:28for me,
- 13:29it's really focusing on, you know, what I
- 13:32call blended intelligence
- 13:34when we talk about organizational growth,
- 13:37especially in health care,
- 13:38right, to bring the best knowledge
- 13:42to the point of decisioning and letting, you
- 13:44know, human wisdom apply the context values
- 13:47and the judgment
- 13:49at that
- 13:50point. So and, you know, this is where
- 13:52AI,
- 13:53really has the,
- 13:55capability
- 13:56to amplify human,
- 13:58wisdom,
- 13:59right, not to replace it.
- 14:01And that's where we want to be and
- 14:03make sure that we're very responsible,
- 14:05about it.
- 14:06K? And the one last thing is, I'll
- 14:08add, and I'll turn it over to Nassim,
- 14:10is that this is also the opportunity where,
- 14:12as we discussed before,
- 14:15for, innovation to be invisible.
- 14:18Right? So if we are doing the right
- 14:20things for our patients and our providers
- 14:22in a way that,
- 14:23we're allowing them to practice their medicine at
- 14:26the top of their license and our patients
- 14:28to have the best experience,
- 14:30and,
- 14:31we'll take advantage of it. Thanks, Simon. So
- 14:34Simon,
- 14:36hit on the one of the most important
- 14:38aspects of growth, which is enabling people to
- 14:41be,
- 14:42not only more productive, but also enjoy what
- 14:45they do day to day and keep, take
- 14:47away the mundane,
- 14:49repetitive parts of their workflow so that they
- 14:51can operate at the top of their lessons.
- 14:53So that's one aspect of growth.
- 14:55Another aspect of growth, at least for City
- 14:57of Hope, is patient access.
- 14:59We want to we want more patients to
- 15:02have access to,
- 15:04the world class care and research that we
- 15:06provide, and we are using a lot of
- 15:09technologies, including AI technologies,
- 15:12to make that possible so that in every
- 15:14community center,
- 15:15even outside of City of Hope, people can
- 15:17have access through our technology
- 15:20to the world class care and research that
- 15:22we provide.
- 15:23That's been something that has been top of
- 15:25mind,
- 15:26always for me over the past nine years
- 15:28and and so and finally, technology is there
- 15:31to be able to enable it in a
- 15:33very reliable
- 15:35fashion. So that's also something that we are
- 15:37focusing on.
- 15:39I love it. Thank you so much, Nasim
- 15:40and Simon, for joining us on the podcast
- 15:42today, and I'm looking forward to seeing you
- 15:44both as well at our annual meeting. I
- 15:45know you'll be
- 15:47doing a session, a fireside chat,
- 15:49with one of our editorial leaders just about
- 15:51everything we talked about here today and digging
- 15:54deeper into some of these areas that are
- 15:56so important when you think about technology and
- 15:59AI in health care and clinical care as
- 16:01well. It's just, incredible to see the opportunities
- 16:04and how you've been able to really maneuver
- 16:06around it at City of Hope. So I'm
- 16:07looking forward to learning more and seeing you
- 16:09in a couple weeks.
- 16:10Thank you. Looking forward to it. Same here.
- 16:12Thank you so much.
