- 0:01Good morning, Good morning, good morning, and welcome to ask the doctor.
- 0:03And the doctor is Carrie Williams with med Club bartles Direct primary Care.
- 0:08Some folks have heard it, but they don't know exactly what it means,
- 0:11but it pretty much means what it says. It does. It means that
- 0:13that patient contracts directly with that position for primary care. And you do that
- 0:19at med Club Bartlesville, right here in Bartlesville, across from Jimmy's Egg on
- 0:23seventy five. Yes, and we've been doing it in Bartlesville since twenty seventeen.
- 0:28Now. This is kind of a cool model. It's something that got
- 0:30started really up in Kansas, didn't it. It did, Yeah, and
- 0:33it started kind of when Obamacare was coming about and people were looking for different
- 0:38ways to you know, affordable see a doctor and make it work not only
- 0:43for the patient but for the doctor and for the health of a community.
- 0:45As a matter of fact, Yes, and doctors are a lot happier because
- 0:49they're not dealing with a lot of insurance companies and administration. No. In
- 0:54fact, you have one monthly fee and that takes care of a lot of
- 0:58things. It doesn't take care of everything, but by value takes care of
- 1:00a pretty good swap of it. It does, and you know what to
- 1:04expect. There's transparency, Oh yeah, very much. So it's all right
- 1:07there in black and white. Plus you'll tell them too, yes, And
- 1:11also at mccleb we offer discounted labs and prescriptions and you know how much it's
- 1:15going to cost before you leave. My goodness, sakes, so all of
- 1:19the mystery is taken out and you can get down to medicine. Yes,
- 1:23Oh, how rare is that? That is very rare in our medical climents
- 1:29today? Oh my goodness. You know, I was taking a look at
- 1:32some of these facts and figures here. Whenever I need a medical constitution or
- 1:38looked at it for anything I call, I'm either there that afternoon or the
- 1:45next day. We try very hard, ninety nine times out of a one
- 1:48hundred, but going the traditional route, we got some late times that are
- 1:53kind of I wasn't aware, and I'm glad that I didn't go that route.
- 1:57Well, intraditional practices, Yeah, there's nearly a thirty day wait to
- 2:02get an appointment, and as a new patient it will be even longer.
- 2:07And if you go to a specialty clinic, it will be over thirty days,
- 2:12more like thirty five or forty days to get an appointment because there's not
- 2:15enough doctors in the United States. Well, you know, I just took
- 2:19a look at a chart there, and you're absolutely correct. When we talk
- 2:23about primary care workforce projections, we start out with what looks like a lot
- 2:29of doctors, but in comparison to how many people there are, the ratio
- 2:35pardon my grammar, ain't that great, No, sir, it's not.
- 2:38In primary care physicians they're basically what populates rule Oklahoma, rural America, and
- 2:46there's not enough of them. And because our population as a nation is growing,
- 2:54so you ratio it and doctors can lose see so many patients today and
- 3:00that limits how many patients they have. Indeed, and when we talk about
- 3:07the demand for doctors, it's it's almost off the scale and it all comes
- 3:12down in numbers. But it's nice to know that you're here and you're abailable.
- 3:16Yes, And you know, because of a decrease ratio of physician to
- 3:24patients in the United States, because of the increase in our populations, are
- 3:29filling in with other medical providers like nurse practitioners and physicians assistants. Now I
- 3:36look at family positions. Yeah, maybe you get six percent a year coming
- 3:42into the field, but the increase is thirteen percent. Thirty. Wow,
- 3:49kind of losing ground there. We are losing ground goodness ages a general,
- 3:53internal physicians it's almost double that's a that's crazy. And the ratios that go
- 4:01on here, nurse practitioners in primary care there seems to be a bunch of
- 4:05those. But the increase in demand is only sixteen percent. So we've got
- 4:10like a little crossing there of the on the diagram as far as supply and
- 4:15demand. But it all depends on what you're looking for it does, and
- 4:17what that patient's goals are. And one of the questions I asked my patients
- 4:23with age, do you think you're going to get healthier? And Andrews absolutely
- 4:28not no, And you think your medical conditions are going to get less complicated
- 4:33with age with age? And so you need someone with experience that has spent
- 4:43a lot of time with patients, and it's seen a lot of patients.
- 4:46By the time we complete residency, which in family medicine is three years,
- 4:51we've seen a lot of patients. We've worked a lot of hospitals, We've
- 4:56worked a lot of clinics, and I did a fellowship in emergency medicine and
- 5:02also can do that and am boards ordified in hospice and pality medicine. It's
- 5:09so seeing that many patients, having much experience, you can kind of weed
- 5:15through the complications a lot more quickly. You can now death rates. I'm
- 5:23just taking a look at that because you know, I'm sixty four and I
- 5:28didn't quite really care about this until you turned like sixty two. I don't
- 5:30know why. I all aden sixty two was like, hey, I better
- 5:33check up on this. But it's amazing here in the United States. You
- 5:38think you're going to live forever, well, none of us get out of
- 5:40here alive. But there are hard facts and hard numbers and real reasons why
- 5:45you should really be in good hands of a physician knows what's going on.
- 5:49Well, you know, with age, you know, ourselves get damaged just
- 5:56by injury and stress and DNA mutations, and our body has a lessoned ability
- 6:02to correct those problems, so the disease processes will creep up on you.
- 6:09And you know, the deterioration of the organs in the United States, cardiovascular
- 6:15disease is the number one cause of death. Cancer is right behind it.
- 6:19So those two together of all the death in the United States, contribute to
- 6:24over forty percent. And a lot of times we can kind of control the
- 6:30encroachment of that. We can we can start doing prementitive messages. And a
- 6:34lot of people will not address their health concerns until they have a catastrophic event,
- 6:41they have their heart attack, their stroke, their cancer. They won't
- 6:45keep up with their maintenance. They will spend more time on their car,
- 6:49changing the oil, doing the maintenance on their car than they will themselves.
- 6:54And then when the catastrophic event happens, then it's expensive. Wow. Wait,
- 6:59that is that? That is something else? You know, when we
- 7:01think that our body is what we have that's going to take us to where
- 7:05we need to go, that folks wally to think about their conveyances more than
- 7:10you know, what's keeping them alive, what's keeping you you? And we've
- 7:13got to change the mind shift here on this. It's hard, yeah,
- 7:16because you have to come to groups with your own mortality, and most people
- 7:21don't like doing it. No one likes doing it. Let's do it that
- 7:26way. But it's an inevitable event and a lot to die. So let's
- 7:31see if we can keep the misery out of it, if we can anticipate
- 7:34it, if we can ward off, we can catch things early and take
- 7:38care of them. Now, the model you have is ideal for what catching
- 7:43things early and to get ahead of things because with the way you're set up
- 7:48one feed monthly fee, you can get checked in just about any old time
- 7:54we want making the appointment. You're usually there in the day or half a
- 7:57day, and you give your patients anywhere from a half hour to an hour
- 8:03of your time each time they're there in order to get things explained, examined
- 8:07properly and help them help themselves in getting ahead of the curve of what's coming
- 8:11ahead. Any education of it is a lot and the understanding that the patient
- 8:18has of the medical system, the system, the system, and people go,
- 8:24well, that's not right, they shouldn't do it that way. I'm
- 8:26sorry they do do it that way, and we have to learn to live
- 8:28with it. But we have to also learn how to manage it. Wow
- 8:35and what I also have a degree in a master's in healthcare administration to try
- 8:41to understand this and to understand the system and break it down and put it
- 8:48into fanal English for us common folk to how this all operates and how it's
- 8:52going to affect you. Absolutely, and for a doctor to speak English,
- 8:56let alone Oki English, it's difficult. We've spent a lot of years learning
- 9:01to speak that high pollutant vocabulary. But the thing is is that when people
- 9:09are looking for answers, they're looking for direct response, in the very direct
- 9:15communication. And with med Club Bartlefield, everything is based on communication and very
- 9:20direct communicating. Yes, and we have several means of communication, not only
- 9:24a face to face appointment time, but we text and email. I do
- 9:28telephone appointments. There are time times I'm going to really almost manage to come
- 9:33in because I need to see something, examine something. But a lot of
- 9:37the questions, a lot of the labyordies and things are done via the telephone
- 9:43or face to face something like that. Also, some of medicine isn't what
- 9:46it appears. Do tell things like metabolic problems, they don't show up until
- 9:54there's a problem. We can kind of anticipate them. Also, as a
- 9:58primary care position, I'm a gatekeeper. Most the information comes through me or
- 10:05to me, and so getting different specialists very information and getting hospital information plus
- 10:11my information and being able to put a canelative together and see trends, see
- 10:20reasons, see timings of problems is really important. It is, it truly
- 10:26is. Now how can folks get started with becoming a member of med Club,
- 10:33Artilesfel. We'll go to our website at www dot medclub dot clinting.
- 10:39That website will answer almost every question you may have about direct primary care and
- 10:43med Club. Also, there's a page on there to sign up as a
- 10:48patient, and my electronic medical record will let me know that someone new has
- 10:54signed up and we will get back with you. Good. And you offer
- 10:58an opportunity for folks to come in once they've gone through that portion, come
- 11:03in and just ask questions and that's great. A free there is an appointment
- 11:09available that we don't provide medical advice, but we answer the questions about direct
- 11:15primary care in mid club. Sure, so you know everything what to expect
- 11:20and why see the best of our ability, just the best of our ability
- 11:24now with this one month fee, that is all kinds of checkups, and
- 11:28that's visits, that's consultations and everything else. However, you do provide some
- 11:33other service at reduced costs. Like labs them right there. Yes, in
- 11:39our office we have lab pick up every day, blood work every day we're
- 11:45open and we do blood work you'reine, and some other things like pap smears
- 11:52and the contract with the lab. We also have discounted and that is greatly
- 11:56discounted. And we also have discounted prescriptions. As a direct primary care doctor,
- 12:03we're going to buy a group with other direct primary care practices and can
- 12:07buy purchase prescription medications that are very discounted rate and we don't have the markup
- 12:13of the retail pharmacies. Another bonus for the patient. Yes, and for
- 12:18I have some patients who are are snowbirds who go to different states. We
- 12:24mail them prescriptions that are a small fee. So it's the one stop shop
- 12:31works on so many different levels for so many different people. It does,
- 12:35And the main thing is accessibility and time. Time is everything. It's fun.
- 12:43It's something that if you're a patient, you want someone who's able to
- 12:48take the time to explain exactly what's going on with you in a way that
- 12:52you'll understand it, and you can't do that in five minutes. I've been
- 12:56on the receiving end of that work well and being able to explain it having
- 13:03resources there. Some people are very visual. They need to see pictures and
- 13:09understand it. They need to know where certain organs are located on their body
- 13:15and to understand what certain things mean. If I poke you here and it
- 13:20shows up over here, it means so and so. Yeah. So it's
- 13:24great that we have that time to get that. That will explain And I'm
- 13:30going to tell you folks full transparency. I'm a patient of doctor Williams.
- 13:33She'lldn't even haul out the grave anatomy and and they'll show you exactly what's going
- 13:37on, and she'll tell you where to find other information so you can do
- 13:43some homework on your own and really absorb it in your own way and in
- 13:46your own time as well. On top of the long but that's the key
- 13:52to medicine is understanding and it has been an enigma to so many people,
- 13:56and they use great big words and getting past that. I send almost every
- 14:03new patient, I will send some type of link to a website of some
- 14:07kind for them to start their learning. Because a lot of medicine has turned
- 14:11into online college classes where you have to do a lot of it yourself to
- 14:16get caught up. And stay caught up. Any things is if medical advance
- 14:24has changed that is pertinent to that patient. Sometimes out of the blue,
- 14:26you'll get something from me that is relevant to you, and that's because the
- 14:31doctor really takes the time to get to know each and every individual patient.
- 14:35You don't have thousands of patients like other clinics do that Club Barbelsfield. You
- 14:41have a set number and that's by design. Yes, we limit our number
- 14:45of patients at seven hundred and we're still accepting new patients. And in my
- 14:50old practice and a traditional method, I had over three thousand. How'd you
- 14:56keep tracked? You don't. You have a staff and my staff is limited
- 15:03where I'm at and a lot of the time I'm the one doing things because
- 15:09and more than anything that trust for the time and the communications is built both
- 15:15ways. And our phone number is nine one eight two one four eight two
- 15:20zero zero. Very good. And of course the website, the all important
- 15:24website to get the ball rolling. Yes, it's www dot medclub dot clinic
