- 0:00Hey
- 0:00everyone
- 0:00I am

- 0:02podcast
- 0:03with
- 0:03a cure
- 0:03in site.
- 0:04And
- 0:04we have
- 0:04a fabulous
- 0:05guest
- 0:05today.
- 0:05We have
- 0:06Tara
- 0:06McCannel.
- 0:07Hi Tara.
- 0:07Thanks
- 0:07for
- 0:07being here.
- 0:09Thank you
- 0:09so much,
- 0:09Megan.
- 0:10It's
- 0:10a real
- 0:11honor
- 0:11to be
- 0:11part of
- 0:12the cure
- 0:12in sight
- 0:13and
- 0:13to share
- 0:14what
- 0:14I know
- 0:14with you.
- 0:16We are so excited
- 0:17to have
- 0:17you share
- 0:17what
- 0:18you know
- 0:18with us.
- 0:21So I can
- 0:21just
- 0:22start.
- 0:22I'll just
- 0:23jump in
- 0:23and just
- 0:24introduce myself
- 0:25and
- 0:26you know
- 0:26where I
- 0:27come
- 0:27from.
- 0:27So I'm
- 0:28a professor
- 0:29of
- 0:29ophthalmology
- 0:30at the
- 0:30University
- 0:31of
- 0:31California,
- 0:32Los
- 0:32Angeles,
- 0:32at the
- 0:33stein eye
- 0:33Institute,
- 0:34and I'm
- 0:35the
- 0:35director
- 0:35of ocular
- 0:36oncology
- 0:37and
- 0:38excited
- 0:38that this
- 0:39is
- 0:39actually
- 0:39my 20th
- 0:40year at
- 0:41UCLA.
- 0:42And,
- 0:43I've
- 0:43really
- 0:44learned
- 0:44so much
- 0:45about
- 0:46what's
- 0:46involved
- 0:47in
- 0:47helping
- 0:48people
- 0:48who are
- 0:49diagnosed
- 0:49with
- 0:50ocular
- 0:50melanoma,
- 0:51both in
- 0:51treatments,
- 0:52but also,
- 0:53helping
- 0:53to
- 0:54disseminate
- 0:54information,
- 0:56and sort
- 0:56of really
- 0:57focusing
- 0:57a lot
- 0:57of our
- 0:58academic
- 0:58research
- 0:59on how
- 1:00the more
- 1:01you know
- 1:02about
- 1:02what
- 1:03the ocular
- 1:03melanoma
- 1:04is, why
- 1:05you're
- 1:05getting
- 1:05treated
- 1:06a certain
- 1:06way,
- 1:07and
- 1:08better
- 1:08understanding
- 1:09of the
- 1:09cancer
- 1:10really
- 1:10helps
- 1:11people to
- 1:11do
- 1:11better.
- 1:11So I'm
- 1:12hoping to
- 1:13help
- 1:13further
- 1:13that
- 1:14goal.
- 1:15and I
- 1:15think
- 1:15what
- 1:16a cure
- 1:16in sight
- 1:17does is
- 1:17really
- 1:17fantastic
- 1:18because
- 1:18it brings
- 1:19all this
- 1:19information
- 1:20together
- 1:21for
- 1:21patients.
- 1:23So I
- 1:24wanted to
- 1:25speak
- 1:25about,
- 1:26what's
- 1:27involved
- 1:27in
- 1:28brachytherapy
- 1:29surgery
- 1:30and also
- 1:31just
- 1:32other
- 1:32surgery
- 1:33and
- 1:33procedures
- 1:34that can
- 1:34happen to
- 1:35the eye.
- 1:35And,
- 1:35Megan,
- 1:36please
- 1:37interrupt
- 1:37me.
- 1:38just
- 1:38jump in
- 1:38if
- 1:39there's
- 1:39something
- 1:39that you
- 1:40need me
- 1:41to help
- 1:41explain
- 1:42further,
- 1:42I'd
- 1:43be more
- 1:43than
- 1:43happy to
- 1:43do that.
- 1:44So,
- 1:44you know,
- 1:45I want
- 1:45to.
- 1:45We want to
- 1:46have this
- 1:46as casual
- 1:47and easy
- 1:48to
- 1:48understand
- 1:49as
- 1:49possible.
- 1:50When
- 1:50a person
- 1:51gets
- 1:51treated
- 1:52for
- 1:52ocular
- 1:52melanoma,
- 1:53there are
- 1:53really
- 1:54two major
- 1:56categories
- 1:56of
- 1:56treatment.
- 1:57As
- 1:58most of
- 1:58you know.
- 1:59One is
- 2:00removal
- 2:00of the eye
- 2:01or what
- 2:02we call
- 2:02a
- 2:02enucleation,
- 2:04and
- 2:04the other
- 2:05is some
- 2:06sort of
- 2:06treatment
- 2:06to
- 2:06the eye
- 2:07that saves
- 2:08the eye
- 2:08and can
- 2:08save
- 2:09some of
- 2:09the
- 2:09vision.
- 2:10And I'm
- 2:11really
- 2:11pleased
- 2:12that it's
- 2:13actually
- 2:13becoming
- 2:14less
- 2:14common
- 2:15now for
- 2:15patients
- 2:16to
- 2:16need
- 2:17to have
- 2:17a
- 2:17enucleation,
- 2:18because
- 2:18we are
- 2:19finding
- 2:19ocular
- 2:20melanoma
- 2:20much
- 2:21earlier
- 2:22than we
- 2:22had been,
- 2:23even
- 2:24when I
- 2:24started
- 2:25practicing.
- 2:25it's it's
- 2:26really
- 2:27uncommon
- 2:27to have
- 2:28a person
- 2:28whose
- 2:29cancer
- 2:29has
- 2:29gotten
- 2:30so
- 2:30advanced
- 2:30in
- 2:30the eye
- 2:31that we
- 2:31don't
- 2:31have
- 2:31a lot of options.
- 2:33So
- 2:33I think,
- 2:34you know,
- 2:34early
- 2:34detection,
- 2:35getting
- 2:36ocular,
- 2:36getting
- 2:37just eye
- 2:38exams
- 2:38where
- 2:39you're
- 2:39dilated
- 2:40and
- 2:40you're
- 2:40checked.
- 2:41and
- 2:41also a
- 2:42lot of times
- 2:43now
- 2:43when you
- 2:44go to
- 2:44a
- 2:44health
- 2:45care
- 2:45officer
- 2:46and
- 2:46an eye
- 2:47care
- 2:47provider,
- 2:47you
- 2:47get these
- 2:48photos
- 2:48that are
- 2:48done.
- 2:49And those
- 2:50have been
- 2:50a really
- 2:50excellent
- 2:51screening
- 2:51tool and
- 2:52allow us
- 2:53to find
- 2:54melanoma,
- 2:54the eye
- 2:54when it's
- 2:55very
- 2:55small.
- 2:55So
- 2:56fortunately,
- 2:57it's a,
- 2:57it's a
- 2:58smaller
- 2:58percent
- 2:59of
- 2:59the people
- 3:00who have
- 3:00ocular
- 3:00melanoma
- 3:01that need
- 3:01to have
- 3:02the
- 3:02enucleation.
- 3:04And then
- 3:04when
- 3:05we're
- 3:05able to
- 3:05save the eye,
- 3:06which is
- 3:06in
- 3:07most of
- 3:07the cases,
- 3:08we
- 3:09the most
- 3:09common
- 3:09way to
- 3:10treat
- 3:10ocular
- 3:10melanoma
- 3:11is using
- 3:11some type
- 3:12of radiation.
- 3:13because
- 3:13the radiation
- 3:14destroys
- 3:15the
- 3:15cancer,
- 3:16excellently,
- 3:17without a doubt.
- 3:18I think
- 3:18that's
- 3:18the one thing
- 3:19that
- 3:19we're
- 3:19very
- 3:20certain
- 3:20about
- 3:20with
- 3:20ocular
- 3:21melanoma
- 3:21is that
- 3:22radiation
- 3:23will kill
- 3:23it.
- 3:24And
- 3:25there's
- 3:25a few
- 3:26different
- 3:26types
- 3:26of
- 3:26radiation.
- 3:27But
- 3:28the most
- 3:28common
- 3:29type of
- 3:30radiation
- 3:30treatment
- 3:30is when
- 3:31people
- 3:31have
- 3:32what's
- 3:32called
- 3:33brachytherapy
- 3:34plaque
- 3:34or when
- 3:35they're
- 3:35seeds
- 3:35of
- 3:35radiation
- 3:36in a carrier,
- 3:37which is
- 3:38like this
- 3:38disc that
- 3:38gets
- 3:39placed
- 3:39on the
- 3:39outside
- 3:40of
- 3:40the eye
- 3:40behind
- 3:41where
- 3:41the tumor
- 3:42is.
- 3:42That's
- 3:42the most
- 3:43common
- 3:43way that
- 3:44radiation
- 3:44is, is
- 3:45given,
- 3:46a second
- 3:46way that
- 3:47radiation
- 3:47is given
- 3:48is what
- 3:48we call
- 3:49proton
- 3:49beam
- 3:50therapy.
- 3:50And
- 3:51there are
- 3:51far fewer
- 3:52centers
- 3:53that
- 3:53offer
- 3:53this
- 3:53treatment.
- 3:54But
- 3:54it involves
- 3:55radiation,
- 3:55so can be
- 3:57also
- 3:57successful
- 3:58at
- 3:58treating
- 3:58the
- 3:58melanoma.
- 4:00in terms
- 4:00of just
- 4:00talking
- 4:01about
- 4:01what
- 4:01happens
- 4:02in these
- 4:02surgeries.
- 4:03I want to
- 4:03talk
- 4:04about,
- 4:04you know,
- 4:04in terms
- 4:05of
- 4:05enucleation.
- 4:06if
- 4:06a person
- 4:07needs to
- 4:07have
- 4:07enucleation,
- 4:08this can
- 4:08definitely
- 4:09be very
- 4:10traumatic
- 4:10because
- 4:11you're
- 4:11like,
- 4:12you're
- 4:12losing
- 4:12an organ.
- 4:13It's like
- 4:14losing
- 4:14a limb.
- 4:15And it's
- 4:16the
- 4:17difficult
- 4:17the most.
- 4:17The
- 4:18difficult
- 4:18thing
- 4:18about it
- 4:19is
- 4:19sort of
- 4:19getting
- 4:19over
- 4:20the shock
- 4:20of
- 4:21having to
- 4:22go
- 4:22through
- 4:22this
- 4:22where
- 4:22your eye
- 4:23is
- 4:23taken
- 4:23out.
- 4:24however,
- 4:25I think
- 4:26it's
- 4:26important
- 4:26to
- 4:26understand
- 4:27that if
- 4:29the enucleation
- 4:29is
- 4:29required
- 4:30because
- 4:31everything
- 4:32is too
- 4:32big,
- 4:33there's not
- 4:33much
- 4:33vision
- 4:34in the eye
- 4:34and
- 4:35there's
- 4:35not
- 4:35really
- 4:35anything else
- 4:36we can
- 4:36do.
- 4:37it is
- 4:38far more
- 4:38straightforward
- 4:39and kind
- 4:40of makes
- 4:41a lot
- 4:41more
- 4:41sense
- 4:41than
- 4:42really
- 4:42doing
- 4:42everything
- 4:43possible
- 4:44to try
- 4:44to save
- 4:44the eye,
- 4:45to try
- 4:45to radiate
- 4:46and then
- 4:47see if
- 4:47we can
- 4:47keep
- 4:48anything.
- 4:49Because
- 4:50enucleation
- 4:50is a very
- 4:51simple
- 4:52surgery.
- 4:53You have
- 4:53an operation
- 4:54that's
- 4:55about an hour.
- 4:56they
- 4:57they're able
- 4:57to
- 4:58replace
- 4:58the globe,
- 4:59which is
- 5:00the eyeball
- 5:01with
- 5:01like a
- 5:01silicone
- 5:02ball
- 5:02that fills
- 5:03the space.
- 5:03After
- 5:04a couple
- 5:04weeks of
- 5:05healing,
- 5:06you can get
- 5:06fitted
- 5:06with the
- 5:07prosthesis
- 5:08that
- 5:08looks
- 5:09incredibly
- 5:10lifelike.
- 5:12and we
- 5:12call this
- 5:12sort
- 5:12of the
- 5:13artificial
- 5:13eye.
- 5:14But
- 5:14really,
- 5:14it's
- 5:14like a
- 5:15jumbo
- 5:15contact
- 5:15lens that
- 5:16goes in
- 5:16place,
- 5:17and
- 5:18it's very
- 5:18low
- 5:18maintenance.
- 5:19We have
- 5:20this idea
- 5:20that
- 5:21if you
- 5:21have
- 5:21to have a
- 5:21fake eye,
- 5:22you have to
- 5:23take it
- 5:23out
- 5:23every night
- 5:24and
- 5:24put it
- 5:24by your
- 5:25bedside
- 5:25table.
- 5:26And I
- 5:26don't
- 5:26know
- 5:27where
- 5:27this
- 5:27notion
- 5:27came
- 5:28from,
- 5:28but
- 5:28everybody
- 5:28assumes
- 5:29that
- 5:29that's
- 5:29what
- 5:30what's
- 5:30involved.
- 5:31But
- 5:31really
- 5:32it's very
- 5:33low
- 5:33maintenance.
- 5:34You might
- 5:34only have
- 5:35it
- 5:35removed
- 5:36a couple
- 5:36times
- 5:37a year
- 5:37for
- 5:37polishing
- 5:38and fitting,
- 5:39and then
- 5:39you just
- 5:40leave it
- 5:40in there
- 5:41and carry
- 5:41on.
- 5:42And I've
- 5:43just been
- 5:43impressed
- 5:44with the
- 5:44artistry
- 5:45of the
- 5:45Ocularist
- 5:46who
- 5:47actually
- 5:47build
- 5:47and
- 5:47create
- 5:48these
- 5:48prosthesis
- 5:49And I
- 5:50have been
- 5:50at the
- 5:51microscope
- 5:51in the exam
- 5:52room and
- 5:52looking
- 5:52at the
- 5:53artificial
- 5:53eye
- 5:54thinking,
- 5:54whoops,
- 5:54this is the wrong eye
- 5:56because
- 5:56it's so
- 5:57good.
- 5:57And until
- 5:57you see
- 5:58it under
- 5:58close
- 5:59magnification,
- 5:59you can tell
- 6:00that it's
- 6:01not
- 6:01really
- 6:02the
- 6:02real thing.
- 6:03So very
- 6:04straightforward.
- 6:05And
- 6:05sometimes
- 6:05when
- 6:05people
- 6:06are
- 6:06diagnosed
- 6:06with a
- 6:07really
- 6:07large
- 6:08ocular
- 6:08melanoma,
- 6:09there can also
- 6:10be pain
- 6:11associated
- 6:12with it.
- 6:12And once
- 6:13the eyes
- 6:14removed,
- 6:14all that
- 6:15pain and
- 6:15discomfort
- 6:16immediately
- 6:17goes away.
- 6:18And so
- 6:19it allows
- 6:19you
- 6:19to just
- 6:20carry on
- 6:20and not
- 6:21have this
- 6:21drag
- 6:22on your eye.
- 6:23And
- 6:23I know
- 6:24that
- 6:24sometimes
- 6:24when
- 6:25people
- 6:25have a
- 6:25very
- 6:26large
- 6:26melanoma
- 6:26that is
- 6:27treated
- 6:27with
- 6:27radiation
- 6:28in an
- 6:28attempt
- 6:29to save
- 6:29the eye,
- 6:29sometimes
- 6:30there
- 6:30can be
- 6:30this
- 6:31prolonged
- 6:31period of
- 6:32a lot
- 6:33of
- 6:33discomfort
- 6:34because
- 6:34there's
- 6:34so much
- 6:35tissue
- 6:35that was
- 6:36damaged
- 6:36with the
- 6:36radiation
- 6:37treatment
- 6:38because
- 6:38things
- 6:38were
- 6:38so big
- 6:39and
- 6:39there can be
- 6:40sort of
- 6:40this
- 6:40ongoing,
- 6:41painful
- 6:42period
- 6:43until the
- 6:44person
- 6:44may
- 6:44eventually
- 6:45need
- 6:45enucleation.
- 6:46So
- 6:46I think
- 6:47if it's
- 6:47recommended
- 6:48that
- 6:48enucleation
- 6:49is
- 6:49the best
- 6:50hope
- 6:51for
- 6:52getting
- 6:52through
- 6:52this,
- 6:52and
- 6:53treatment
- 6:53it is
- 6:53a very
- 6:54straightforward
- 6:55procedure,
- 6:55and
- 6:56a lot of
- 6:57it is
- 6:57just
- 6:57understanding
- 6:58what's
- 6:58involved.
- 7:00you're
- 7:00100%
- 7:01right.
- 7:01Some of the prosthetics
- 7:03look so
- 7:03good,
- 7:04you
- 7:04really
- 7:04cannot
- 7:05tell.
- 7:06And, it's
- 7:07just it's
- 7:07really,
- 7:08really
- 7:08impressive.
- 7:11Yeah.
- 7:11I don't
- 7:11know how
- 7:12they,
- 7:13the ocularist
- 7:14do
- 7:14the work
- 7:14they do,
- 7:15but,
- 7:16you know,
- 7:16basically
- 7:16they take
- 7:17a lot
- 7:17of photos
- 7:17of your
- 7:18other
- 7:18eye, and
- 7:19just
- 7:20create
- 7:20this, and
- 7:21it's
- 7:21just,
- 7:21it's very
- 7:22difficult
- 7:23to tell,
- 7:23if
- 7:24this is
- 7:24your eye
- 7:25or not,
- 7:25and
- 7:25people
- 7:25don't
- 7:26have to
- 7:26walk
- 7:26around
- 7:27wearing
- 7:27a black
- 7:28patch
- 7:28for
- 7:28the rest
- 7:29of their
- 7:29life,
- 7:29I think
- 7:30that's
- 7:30another
- 7:31image
- 7:32that
- 7:32comes
- 7:32to mind.
- 7:33we have
- 7:33really
- 7:33great
- 7:34ways
- 7:34of
- 7:35helping
- 7:36make
- 7:36people
- 7:37look,
- 7:37as they
- 7:38should,
- 7:38you know,
- 7:38as normal
- 7:39as
- 7:39possible
- 7:40after
- 7:40this
- 7:40sort of a
- 7:41sort of
- 7:41a
- 7:41procedure.
- 7:43the fact
- 7:43that they
- 7:43can
- 7:44match it
- 7:44to your
- 7:44other
- 7:44eye, it's
- 7:46really
- 7:46just it
- 7:47blows
- 7:47your mind
- 7:48how
- 7:48the artistry
- 7:49and the
- 7:50talent
- 7:50that
- 7:50goes into
- 7:51creating that.
- 7:51But
- 7:51they really
- 7:52are
- 7:52beautiful.
- 7:53Yeah.
- 7:54I think
- 7:54that's a
- 7:54that's a
- 7:55lovely
- 7:55word.
- 7:55They're
- 7:56beautiful.
- 7:56It's
- 7:56a beautiful
- 7:57work
- 7:57of art.
- 7:57Like
- 7:57the other day
- 7:58I had,
- 7:59I was,
- 7:59I was
- 7:59looking,
- 8:00I was taking,
- 8:01someone's
- 8:01prosthesis
- 8:02and I was
- 8:02looking at it
- 8:03and
- 8:03you know,
- 8:03I when
- 8:04I take about
- 8:04I always
- 8:04like,
- 8:05wash
- 8:05it first
- 8:05and then
- 8:06I'm
- 8:06looking
- 8:06at this
- 8:06thing,
- 8:07I'm like,
- 8:07this is like
- 8:08I'm
- 8:08holding
- 8:08an eye
- 8:09in my
- 8:09hand.
- 8:10It's it's
- 8:11really
- 8:11amazing.
- 8:12So,
- 8:12so really
- 8:13excellent
- 8:13work.
- 8:14and some
- 8:14people
- 8:15need
- 8:15enucleation
- 8:16from the beginning,
- 8:17but
- 8:17sometimes
- 8:18people
- 8:19after
- 8:19treatment
- 8:20and
- 8:20the eye
- 8:20is not
- 8:21doing
- 8:21very
- 8:21well.
- 8:22As
- 8:22I
- 8:22mentioned,
- 8:22sometimes
- 8:23it can
- 8:23be
- 8:23painful
- 8:24and not
- 8:24really
- 8:24giving
- 8:25vision
- 8:25back.
- 8:26if I sort of
- 8:26think
- 8:27that,
- 8:27you know,
- 8:27maybe
- 8:27in the
- 8:28not too
- 8:28distant
- 8:28future
- 8:29we might be
- 8:29talking
- 8:30about
- 8:30this.
- 8:30I try to
- 8:31bring it
- 8:31up early
- 8:32and just
- 8:32keep
- 8:33talking about it
- 8:33so that
- 8:34people
- 8:34can start
- 8:35thinking
- 8:36about
- 8:36this
- 8:37possibility
- 8:38and, and
- 8:39really
- 8:39adjusting
- 8:40to it
- 8:40a little bit
- 8:41because
- 8:41it's it's
- 8:41definitely
- 8:42it can be
- 8:42a shock
- 8:43if
- 8:43someone
- 8:43if
- 8:44you've
- 8:44come to
- 8:44an appointment
- 8:45and
- 8:45you're
- 8:46told that
- 8:46you have
- 8:46to have
- 8:46the eye
- 8:47removed.
- 8:47So,
- 8:48you know,
- 8:48just just
- 8:49understanding
- 8:50as much
- 8:50about it
- 8:51as
- 8:51possible
- 8:51can really
- 8:52make it a
- 8:53more easy
- 8:54transition.
- 8:55if you
- 8:55actually
- 8:56need to
- 8:56have it.
- 8:58I want to
- 8:59talk a
- 8:59bit about
- 9:00what
- 9:00happens
- 9:00in
- 9:01brachytherapy
- 9:01eye
- 9:01surgery,
- 9:02and
- 9:02certainly
- 9:03people
- 9:03who have
- 9:04already
- 9:05had the
- 9:05treatment
- 9:06may,
- 9:06you know,
- 9:07be
- 9:07already.
- 9:07Well
- 9:07beyond
- 9:08this.
- 9:08But for
- 9:08people
- 9:09who
- 9:09this is
- 9:09new for,
- 9:10but also
- 9:11people
- 9:11who
- 9:11have had
- 9:12the treatment
- 9:12and maybe
- 9:13didn't
- 9:13quite
- 9:13understand
- 9:14what happened,
- 9:15I think
- 9:16it's
- 9:16helpful
- 9:16to
- 9:16kind of
- 9:17go over
- 9:17what
- 9:18happens
- 9:18in
- 9:18surgery.
- 9:19So I'm
- 9:19going to
- 9:19I'm going
- 9:20to talk
- 9:20about
- 9:20break
- 9:21the
- 9:21brachytherapy
- 9:22plaque
- 9:22because
- 9:22that's
- 9:23the most
- 9:23common.
- 9:23And then
- 9:24also a
- 9:24little bit
- 9:25about
- 9:25proton
- 9:25beam
- 9:26after.
- 9:27So
- 9:28one of
- 9:28the
- 9:28things
- 9:29that
- 9:29doesn't
- 9:29happen is
- 9:30the eye
- 9:31gets
- 9:31taken out
- 9:32and
- 9:32put on
- 9:33a surgical table.
- 9:34And then
- 9:34we do
- 9:35surgery
- 9:35and
- 9:35then put
- 9:36the eye
- 9:36back so
- 9:37the eye
- 9:37stays in
- 9:38the head
- 9:39the whole time.
- 9:40This
- 9:40is it's
- 9:41really
- 9:41possible
- 9:41to do
- 9:42this type
- 9:42of
- 9:42surgery
- 9:43because
- 9:43of
- 9:44isolating
- 9:45the
- 9:45muscles
- 9:45and
- 9:45really
- 9:46getting
- 9:46a lot
- 9:46of
- 9:46exposure
- 9:47during
- 9:47surgery.
- 9:48And
- 9:48when people
- 9:49have
- 9:49brachytherapy
- 9:50plaque
- 9:50surgery,
- 9:50we use
- 9:51we use
- 9:51anesthesia.
- 9:53I think
- 9:53some
- 9:54centers
- 9:54will do
- 9:54general
- 9:55anesthesia.
- 9:56I know
- 9:56our center.
- 9:57I like to use
- 9:58a lot of
- 9:58local
- 9:58anesthesia.
- 9:59I try
- 9:59to avoid
- 9:59generals
- 10:00unless
- 10:01people
- 10:01are
- 10:01really young
- 10:02or just
- 10:02totally
- 10:02can't
- 10:02tolerate
- 10:03it.
- 10:04but
- 10:04either
- 10:04way,
- 10:05the
- 10:05person is
- 10:05very
- 10:05comfortable,
- 10:07and
- 10:07the surgery
- 10:08can last,
- 10:09it's
- 10:09different
- 10:09at
- 10:09every place.
- 10:10But
- 10:10can be
- 10:10is
- 10:11as short
- 10:12as 30
- 10:12minutes
- 10:13to
- 10:13sometimes
- 10:14an hour,
- 10:14an hour
- 10:14and a
- 10:14half,
- 10:15depending
- 10:15on
- 10:16everything
- 10:16that's
- 10:16happening,
- 10:17depending
- 10:17on,
- 10:18the
- 10:18complexity
- 10:19of the surgery.
- 10:20But
- 10:20when you
- 10:20come to
- 10:21surgery,
- 10:21it's
- 10:22usually
- 10:23in most
- 10:23places.
- 10:23Now it's
- 10:24all outpatient.
- 10:25I think
- 10:26that
- 10:26the last
- 10:27I heard
- 10:27the centers,
- 10:28it used
- 10:28to be
- 10:28inpatient,
- 10:29meaning
- 10:29staying
- 10:30in the
- 10:30hospital
- 10:31have
- 10:31switched.
- 10:32And so,
- 10:32most of
- 10:33the time
- 10:33this is
- 10:34an
- 10:34outpatient
- 10:34procedure.
- 10:35So,
- 10:36you don't
- 10:36have
- 10:36to stay
- 10:37in the hospital.
- 10:38And
- 10:38what happens
- 10:39in
- 10:39the surgery
- 10:40is that
- 10:40the
- 10:40plaque,
- 10:41which is
- 10:42customized
- 10:43and built
- 10:43to the
- 10:44specifications
- 10:46of the
- 10:46individual
- 10:47melanoma,
- 10:48the plaque
- 10:48is placed
- 10:49on the
- 10:50eye on
- 10:50the
- 10:50outside
- 10:51of the eye,
- 10:53and we
- 10:53get
- 10:53to the
- 10:53outside
- 10:54of the eye
- 10:55by
- 10:55opening
- 10:56up the
- 10:56superficial
- 10:57tissues.
- 10:58So
- 10:58we call
- 10:58that the
- 10:58conjunctiva,
- 10:59the
- 10:59and the,
- 11:00which is
- 11:00sort
- 11:00of the
- 11:01superficial
- 11:01tissue
- 11:01overlying
- 11:02the white
- 11:02part of
- 11:03the eye.
- 11:04And the
- 11:04muscles
- 11:05of
- 11:05the eye.
- 11:05There's
- 11:06four main
- 11:06muscles
- 11:07that we
- 11:08look for.
- 11:08And
- 11:09they're
- 11:09really
- 11:09ones,
- 11:11that help
- 11:11the eye
- 11:11look up,
- 11:12look down
- 11:12and look
- 11:12right to
- 11:13look
- 11:13left.
- 11:13And so
- 11:14each of
- 11:14those
- 11:15four
- 11:15muscles
- 11:15is
- 11:16identified
- 11:17at
- 11:17the time
- 11:17of surgery.
- 11:18And
- 11:18we put
- 11:18a suture
- 11:20around
- 11:20those muscles
- 11:21so that
- 11:21we can
- 11:21turn
- 11:22the eye.
- 11:22And you
- 11:22can turn
- 11:23the eye
- 11:23way over
- 11:24so that
- 11:25you can
- 11:25get
- 11:25to the
- 11:25very
- 11:25back.
- 11:26So it's
- 11:26possible
- 11:27to put
- 11:28the
- 11:28breaking
- 11:28for
- 11:28if you're
- 11:29the
- 11:29melanomas
- 11:30at
- 11:30the close
- 11:31to the
- 11:31macula
- 11:32or very
- 11:32posterior,
- 11:33we call
- 11:34that,
- 11:34we can
- 11:35turn
- 11:35the eye
- 11:36and
- 11:36expose
- 11:37that area
- 11:37very well
- 11:38with just
- 11:39using our
- 11:39strings
- 11:40and
- 11:40instruments
- 11:41to help
- 11:41spread
- 11:41the
- 11:42tissues
- 11:42to get
- 11:42right
- 11:43where we
- 11:43needed
- 11:43to be.
- 11:44and
- 11:45because
- 11:45people
- 11:46are have
- 11:46anesthesia,
- 11:47you're
- 11:47really
- 11:47not
- 11:48feeling
- 11:48any of
- 11:49this
- 11:49movement
- 11:50around
- 11:50the eye.
- 11:51when we do
- 11:52local
- 11:52anesthesia,
- 11:53when a
- 11:53person
- 11:53is
- 11:54sort of
- 11:54in
- 11:54twilight,
- 11:55we add
- 11:55additional
- 11:56anesthesia
- 11:56around
- 11:57the eyes.
- 11:57So
- 11:58the eyes
- 11:58bathed
- 11:59in, in
- 12:00sort of
- 12:01xylocaine
- 12:01type
- 12:02solutions,
- 12:02which
- 12:03really
- 12:03numbs
- 12:03everything,
- 12:04you can
- 12:05also be
- 12:05sort of
- 12:05semi awake
- 12:06and you
- 12:06might
- 12:07hear
- 12:07people
- 12:07talking
- 12:08or
- 12:08sometimes
- 12:08our patients
- 12:09kind
- 12:09of talk
- 12:09along
- 12:10with us,
- 12:10but
- 12:10whatever
- 12:11works.
- 12:12So
- 12:12once we
- 12:13find the
- 12:13muscles,
- 12:14we can
- 12:14turn
- 12:14the eye
- 12:15and then
- 12:15in order
- 12:16to find
- 12:17where
- 12:17that
- 12:18melanoma
- 12:18is,
- 12:19there's a
- 12:20couple
- 12:20ways,
- 12:21and
- 12:21we use
- 12:21a bit of
- 12:21both,
- 12:22but
- 12:22you can
- 12:23do
- 12:23something
- 12:23called
- 12:24trans
- 12:24illumination,
- 12:25which is
- 12:25getting
- 12:26a really
- 12:26strong
- 12:26light
- 12:27and
- 12:27shining
- 12:28it
- 12:28through
- 12:28the
- 12:28dilated
- 12:29pupil,
- 12:29just like you,
- 12:30like if
- 12:31you
- 12:31imagine
- 12:31a pumpkin
- 12:32like
- 12:32a jack
- 12:33o
- 12:33lantern.
- 12:34if you put
- 12:35a candle
- 12:35inside,
- 12:36if
- 12:36there's
- 12:36something
- 12:36dark on
- 12:37the wall
- 12:38of the
- 12:38of the
- 12:39pumpkin,
- 12:39it will be black
- 12:40and
- 12:40it will
- 12:41block
- 12:41light.
- 12:42So
- 12:42a
- 12:42melanoma
- 12:43does
- 12:43the same
- 12:44sort
- 12:44of thing.
- 12:44So if
- 12:45you shine
- 12:45a light
- 12:46and you,
- 12:46you
- 12:47expose
- 12:47the wall
- 12:48of the eye
- 12:48with the
- 12:49light
- 12:49glowing
- 12:49through
- 12:50it,
- 12:50you can see
- 12:51this dark
- 12:52area
- 12:52where the
- 12:53melanoma
- 12:53is.
- 12:54sometimes
- 12:55melanomas
- 12:55are
- 12:56very tiny
- 12:57and
- 12:57sometimes
- 12:58they
- 12:58don't
- 12:58have
- 12:58a lot
- 12:58of pigment.
- 12:59So this,
- 13:00this way
- 13:01of, of
- 13:01using
- 13:01trans
- 13:02illumination,
- 13:03it
- 13:03doesn't
- 13:04always work.
- 13:04So what
- 13:05I always
- 13:05find is
- 13:06helpful
- 13:06together
- 13:07with
- 13:07trans
- 13:07illumination
- 13:08is also
- 13:08to, to
- 13:09actually
- 13:09look.
- 13:10So we put
- 13:11on the
- 13:11indirect
- 13:12ophthalscope
- 13:13which is
- 13:13sort of
- 13:13that
- 13:13miner's
- 13:14light
- 13:15that
- 13:15we use
- 13:15with a lens.
- 13:16And
- 13:16you can
- 13:16actually
- 13:16see where
- 13:17the
- 13:18melanoma
- 13:18is and
- 13:19you can
- 13:19use a
- 13:19little,
- 13:20what
- 13:20we call
- 13:20a little
- 13:20depressor
- 13:21on the
- 13:21outside
- 13:22of
- 13:22the eye
- 13:22to see
- 13:23and mark
- 13:23where
- 13:23the very
- 13:24edge of
- 13:24it
- 13:24starts.
- 13:25And
- 13:25so
- 13:25then you
- 13:25can mark
- 13:26that
- 13:26on the eye
- 13:26wall.
- 13:27And
- 13:27usually
- 13:28if you do
- 13:28that
- 13:29together
- 13:29with the
- 13:30trans
- 13:30illumination,
- 13:31you're
- 13:32most of
- 13:32the time
- 13:33you're
- 13:33there.
- 13:33But I'm
- 13:34always
- 13:34surprised
- 13:35that
- 13:36sometimes
- 13:36the trans
- 13:37illumination
- 13:37isn't
- 13:38exactly
- 13:38accurate
- 13:39or eye
- 13:40mark
- 13:40at all.
- 13:41And
- 13:41it looks
- 13:42and it
- 13:42looks
- 13:42like
- 13:42I've got it
- 13:43exactly
- 13:43in place.
- 13:44And so
- 13:46many
- 13:46centers
- 13:46also
- 13:47bring
- 13:47their
- 13:47ultrasonographer
- 13:48or do
- 13:48intraoperative
- 13:49ultrasonography.
- 13:50So that's
- 13:51just a
- 13:51third way
- 13:52you can
- 13:52increase
- 13:53the
- 13:53accuracy
- 13:53of the placement.
- 13:54So
- 13:55we always
- 13:55check.
- 13:56And I
- 13:56think
- 13:56most
- 13:56centers
- 13:57now do
- 13:58an
- 13:58ultrasound
- 13:59during
- 13:59the surgery
- 14:00where
- 14:01after
- 14:01everything
- 14:01is in
- 14:02place
- 14:02we look
- 14:03and say,
- 14:04you
- 14:04know, is
- 14:04this is
- 14:04everything
- 14:05accurate?
- 14:06Is
- 14:06the plaque,
- 14:07is it
- 14:08lying is
- 14:09or the
- 14:09dimensions
- 14:10usually
- 14:11sort of
- 14:11front
- 14:11to back
- 14:12and from
- 14:13side to
- 14:13side.
- 14:13We usually
- 14:14have two
- 14:14major
- 14:15dimensions
- 14:15on
- 14:15ultrasound.
- 14:16We want to
- 14:17make sure
- 14:17that the
- 14:17plaque
- 14:18is
- 14:18centered
- 14:18over
- 14:19the tumor.
- 14:19And
- 14:19there's
- 14:20no areas
- 14:21of
- 14:21what we
- 14:21call edge
- 14:22miss or
- 14:22parts of
- 14:23the tumor
- 14:23that are
- 14:24not being
- 14:24properly
- 14:24covered
- 14:25by the
- 14:25plaque.
- 14:26And I
- 14:26also
- 14:26think
- 14:27it's
- 14:27really
- 14:27important
- 14:27to
- 14:28this is a part
- 14:29that is
- 14:29really
- 14:30critical,
- 14:30because
- 14:30we need
- 14:31to make
- 14:31sure that
- 14:32the
- 14:32treatment
- 14:32is as
- 14:33accurate,
- 14:33as
- 14:33accurate
- 14:34as can
- 14:34be.
- 14:35So
- 14:35we really
- 14:36take
- 14:36our time.
- 14:36We measure,
- 14:37we look,
- 14:37and
- 14:37if it doesn't
- 14:38look
- 14:38quite
- 14:38where
- 14:39we want it
- 14:39to be,
- 14:39or
- 14:40maybe
- 14:40it's a
- 14:40little
- 14:40lopsided,
- 14:41this is the opportunity
- 14:42to move it.
- 14:43So then
- 14:43you can move it
- 14:44so it
- 14:45shift.
- 14:46Look
- 14:46again
- 14:47by
- 14:47ultrasound.
- 14:48And a lot
- 14:49of
- 14:49centers
- 14:50in
- 14:50addition
- 14:50to ours
- 14:51have
- 14:51published
- 14:51on how
- 14:52ultrasound
- 14:52actually
- 14:53can help
- 14:53increase
- 14:54the
- 14:54accuracy
- 14:55of the
- 14:55brachytherapy
- 14:56treatment.
- 14:57and
- 14:58I think
- 14:58ultrasound
- 14:58is
- 14:58really critical.
- 14:59And
- 15:00even though
- 15:00I have
- 15:00all these
- 15:01years of
- 15:01doing
- 15:02this,
- 15:02I
- 15:02don't think
- 15:02I've ever
- 15:03had
- 15:03a time
- 15:04where
- 15:04I thought,
- 15:04you know,
- 15:04I don't
- 15:04need
- 15:04ultrasound
- 15:05anymore
- 15:06because
- 15:06I'm
- 15:06always
- 15:06surprised
- 15:07that if I
- 15:08if I do
- 15:08the trans
- 15:09illumination,
- 15:09we put it
- 15:10on,
- 15:10it's
- 15:10like,
- 15:10whoa,
- 15:10this
- 15:11didn't
- 15:11this wasn't
- 15:11this was off.
- 15:12So
- 15:12the ultrasound
- 15:13allows
- 15:14you
- 15:14to confirm.
- 15:16But also
- 15:17sometimes
- 15:18there may
- 15:18be a
- 15:18delay
- 15:19from
- 15:19the last
- 15:20ultrasound
- 15:20measurement
- 15:21to the
- 15:21time of
- 15:22surgery.
- 15:23So
- 15:23performing
- 15:24ultrasound
- 15:24also
- 15:25gives you
- 15:25an
- 15:25opportunity
- 15:26to
- 15:26remeasure
- 15:27the
- 15:28melanoma.
- 15:28And
- 15:28if you
- 15:29find that
- 15:29it's
- 15:30grown
- 15:30a little bit,
- 15:30you can give
- 15:31those
- 15:32extra
- 15:32numbers
- 15:33to the
- 15:33physics
- 15:33team.
- 15:34And we
- 15:34usually
- 15:35in
- 15:35situations
- 15:36like
- 15:36this,
- 15:36we may
- 15:37find that
- 15:37we're
- 15:37adding
- 15:38additional hours
- 15:39or
- 15:39sometimes
- 15:40an
- 15:40additional
- 15:40day
- 15:41to the
- 15:41brachytherapy
- 15:42treatment
- 15:43to make
- 15:43sure that
- 15:43everything
- 15:44is as
- 15:44accurate
- 15:45as can
- 15:45be.
- 15:46So
- 15:47all these
- 15:47things
- 15:47happen.
- 15:48Usually
- 15:48there's a
- 15:48lot of
- 15:49people
- 15:49in the
- 15:50operating
- 15:50room
- 15:50during
- 15:51brachytherapy.
- 15:52We have
- 15:52our
- 15:52radiation
- 15:53physicist,
- 15:54we have
- 15:54our
- 15:54radiation
- 15:55oncologist,
- 15:57in
- 15:57addition
- 15:57to all
- 15:57the
- 15:58technical
- 15:58team,
- 15:58in our
- 15:59case,
- 15:59our ultrasonographer
- 16:00comes in
- 16:01for all
- 16:01the cases
- 16:02in
- 16:02addition
- 16:03to all
- 16:03the
- 16:03people
- 16:04learning
- 16:04and
- 16:05observing.
- 16:05So it's
- 16:05a very
- 16:06busy,
- 16:06we like
- 16:07to call
- 16:07it.
- 16:07It's
- 16:07a party
- 16:08in the
- 16:08operating room.
- 16:09So
- 16:09there's
- 16:09lots of
- 16:10people
- 16:10who are
- 16:10watching
- 16:11and
- 16:11learning,
- 16:12and
- 16:12there's
- 16:12usually
- 16:13lots of
- 16:13conversation
- 16:14because
- 16:14people
- 16:15want to
- 16:15understand
- 16:16all the
- 16:16different
- 16:17steps,
- 16:17even,
- 16:17you know,
- 16:18outside
- 16:18of the patient.
- 16:19But
- 16:19all the people
- 16:20who are
- 16:20there
- 16:21are
- 16:21usually
- 16:22there's
- 16:22always
- 16:22people
- 16:23learning
- 16:23and
- 16:24visitors
- 16:24and such.
- 16:25So once
- 16:27everything
- 16:27is in
- 16:27place,
- 16:28then
- 16:28we can
- 16:29put the
- 16:29we can
- 16:30put
- 16:30everything
- 16:30back and
- 16:31the
- 16:32plaque
- 16:32remains
- 16:33sutured
- 16:33on to
- 16:34the eye.
- 16:35and
- 16:35there's,
- 16:36there's
- 16:36different
- 16:36ways to
- 16:37suture
- 16:37the plaque,
- 16:38but we do
- 16:38use
- 16:39stitches
- 16:40that hold
- 16:41the, the,
- 16:42the edge
- 16:42of the
- 16:42plaque
- 16:43against
- 16:44the sclera,
- 16:44which is
- 16:45the white
- 16:46non
- 16:46vascular
- 16:47part of
- 16:47the eye
- 16:48that
- 16:48we're we.
- 16:48So
- 16:48things to
- 16:50it's it's
- 16:51just this
- 16:51in a
- 16:51similar
- 16:52type of
- 16:52surgery
- 16:52where
- 16:53we do
- 16:53in
- 16:53retinal
- 16:54diseases
- 16:55doing
- 16:55retinal
- 16:55detachment
- 16:56surgery.
- 16:56We
- 16:57you know
- 16:57I also do
- 16:59surgical
- 16:59retina.
- 16:59If
- 17:00people
- 17:00have
- 17:00retinal
- 17:00attachments
- 17:01we often use
- 17:02a scleral
- 17:02buckle.
- 17:02So we so
- 17:04we sew
- 17:04sewing
- 17:05things
- 17:05on to
- 17:05sclera
- 17:06is not
- 17:06an unusual
- 17:07or
- 17:07rare thing.
- 17:08So
- 17:08putting on
- 17:09a plaque
- 17:09and
- 17:09sewing
- 17:10that on
- 17:10is
- 17:11something
- 17:11very
- 17:11doable.
- 17:12We have sutures
- 17:12that are
- 17:13very good
- 17:13for that.
- 17:14one of
- 17:14the ways,
- 17:15one
- 17:15of the
- 17:15things
- 17:15that
- 17:15we do
- 17:16is we,
- 17:16we so
- 17:17our plaques
- 17:17on a way
- 17:18that
- 17:18we can
- 17:18remove
- 17:19them in
- 17:19the
- 17:19office.
- 17:21so
- 17:21there's,
- 17:22there's
- 17:22different
- 17:22ways to
- 17:22do that.
- 17:23I think
- 17:23most
- 17:23people
- 17:24will
- 17:24come back
- 17:24for a
- 17:25second
- 17:25surgery
- 17:26that's
- 17:26usually
- 17:26much
- 17:26shorter
- 17:27just
- 17:27to remove
- 17:28the
- 17:28plaque.
- 17:29it's so
- 17:29fascinating.
- 17:30It's just
- 17:31amazing
- 17:32what
- 17:32modern
- 17:33medicine
- 17:33is doing.
- 17:33It
- 17:33really is.
- 17:34I think
- 17:34the
- 17:35the question
- 17:35I have
- 17:35right
- 17:36now is,
- 17:36after you
- 17:38stitch
- 17:38the plaque
- 17:39on the I
- 17:41and
- 17:41they go
- 17:41home, can
- 17:42they feel
- 17:42that
- 17:43in there
- 17:44That's
- 17:45a great question.
- 17:46So
- 17:47the further
- 17:47back
- 17:48the plaque
- 17:49is, so
- 17:49the more
- 17:50posterior
- 17:51the
- 17:51melanoma
- 17:52is
- 17:53there's
- 17:53nothing
- 17:53really
- 17:53to feel
- 17:54or even
- 17:54see back
- 17:55there.
- 17:56and often
- 17:57there's,
- 17:57there's
- 17:57not just
- 17:58the eye
- 17:59and
- 17:59there's
- 18:00so many,
- 18:00there's
- 18:01so much
- 18:01soft
- 18:01tissue
- 18:02in the orbit,
- 18:03which is
- 18:03like the
- 18:03eye
- 18:03socket
- 18:04where the
- 18:04eyeball
- 18:04sits.
- 18:05And
- 18:06because
- 18:06things
- 18:06are being
- 18:06moved
- 18:07and
- 18:07turned,
- 18:08there's
- 18:08a lot
- 18:08of
- 18:08swelling,
- 18:09in,
- 18:10in all the
- 18:10tissues
- 18:11in the eye.
- 18:11So we
- 18:12often
- 18:12give
- 18:13people,
- 18:13some
- 18:14a little
- 18:14bit of
- 18:14steroid
- 18:15during
- 18:15the surgery
- 18:15just
- 18:16to help
- 18:16that
- 18:16swelling
- 18:17go down
- 18:18and
- 18:18not be.
- 18:18so tense
- 18:19afterwards?
- 18:19Sometimes
- 18:19when
- 18:20everything's
- 18:20very
- 18:20swollen,
- 18:20it can be
- 18:21very
- 18:21painful
- 18:21as well.
- 18:22if people
- 18:23feel
- 18:23anything,
- 18:24it's
- 18:24really
- 18:24just
- 18:24the bit
- 18:25of
- 18:25swelling
- 18:26and a
- 18:26little
- 18:26bit of
- 18:26inflammation
- 18:27after the
- 18:28surgery.
- 18:29The funny thing
- 18:29is, even
- 18:30if the
- 18:31plaque
- 18:31is at
- 18:31the very
- 18:31front
- 18:32of the eye,
- 18:32and
- 18:32sometimes
- 18:33if you
- 18:33have like
- 18:34a, a
- 18:35tumor
- 18:36that
- 18:36involves
- 18:36the
- 18:37ciliary
- 18:37body,
- 18:38which is
- 18:38towards
- 18:38the very
- 18:39front
- 18:39of the eye,
- 18:40you can
- 18:40see
- 18:41oftentimes
- 18:42you'll
- 18:42see some
- 18:42of the
- 18:42gold from
- 18:43the front
- 18:44with
- 18:44the eye
- 18:44open.
- 18:45You'll
- 18:45see some
- 18:45a gold
- 18:46part
- 18:46of the
- 18:46plaque
- 18:47showing.
- 18:47And it's
- 18:48really
- 18:48quite
- 18:48amazing
- 18:49that that
- 18:49doesn't
- 18:50cause
- 18:50additional
- 18:51discomfort.
- 18:52And even
- 18:53sometimes
- 18:53we have
- 18:53our
- 18:54plaques
- 18:54hanging
- 18:55over
- 18:55the cornea,
- 18:56which is
- 18:56the front
- 18:57of the eye,
- 18:57which is a
- 18:58very,
- 18:58very
- 18:58sensitive
- 18:59structure.
- 19:00as long
- 19:00as
- 19:00everything
- 19:01is, is
- 19:01still
- 19:02and in
- 19:02place,
- 19:03there
- 19:03really
- 19:03isn't
- 19:03any
- 19:04discomfort
- 19:04from
- 19:05that.
- 19:05And so,
- 19:05so
- 19:06actually,
- 19:06I don't
- 19:06think
- 19:07people
- 19:07really
- 19:07feel the
- 19:08plaque
- 19:08at all.
- 19:09They may
- 19:09feel some
- 19:10swelling
- 19:11or
- 19:11just all
- 19:12the
- 19:12tissues
- 19:12being a
- 19:12little
- 19:13bit
- 19:13altered,
- 19:14but, I
- 19:14don't
- 19:15know.
- 19:15One has
- 19:15said, oh,
- 19:15I feel
- 19:16the edge.
- 19:16And it's,
- 19:17it's
- 19:17actually
- 19:17correlates.
- 19:18We, we
- 19:19really,
- 19:20amazingly
- 19:21don't
- 19:21feel the
- 19:22plaque
- 19:23on the eye.
- 19:25That is amazing
- 19:25because
- 19:26you get
- 19:26a tiny
- 19:26little
- 19:26eyelash
- 19:27in there
- 19:27and it
- 19:27drives
- 19:28you
- 19:28crazy.
- 19:29But
- 19:29the fact
- 19:29that
- 19:29you guys
- 19:30can put
- 19:30a whole
- 19:30plaque
- 19:31in there
- 19:31and
- 19:31they don't
- 19:31feel it,
- 19:32it's
- 19:32really
- 19:33blows
- 19:33your mind.
- 19:34I think
- 19:35it's it's
- 19:35just,
- 19:36because
- 19:36different
- 19:37parts of
- 19:37the eye
- 19:38have
- 19:38different,
- 19:39sensitivity
- 19:40and
- 19:40different
- 19:40innervation
- 19:41And the
- 19:41cornea
- 19:42being
- 19:42the most
- 19:43exposed
- 19:43part
- 19:44is it's
- 19:44like one
- 19:45of the
- 19:45most
- 19:45sensitive
- 19:46places
- 19:46with the
- 19:46dense
- 19:47nerve,
- 19:48receptors
- 19:49in the
- 19:49body.
- 19:49So that,
- 19:50I mean,
- 19:50for our
- 19:50protection.
- 19:51So if
- 19:52anything
- 19:52touches
- 19:52the eye,
- 19:53it's
- 19:53going to
- 19:53affect
- 19:53our vision.
- 19:54So
- 19:54we react,
- 19:55you know,
- 19:56in a
- 19:56very
- 19:56hyper
- 19:57reactive
- 19:57way.
- 19:57So like
- 19:58an
- 19:58eyelash.
- 19:58Exactly.
- 19:59As you
- 19:59say.
- 20:00even
- 20:00sometimes
- 20:00with
- 20:01the stitches.
- 20:02some
- 20:02people
- 20:02feel
- 20:03the stitches
- 20:03afterwards,
- 20:05but
- 20:05I feel
- 20:06like
- 20:06there's a
- 20:07lot of
- 20:07times
- 20:08I'm
- 20:08really
- 20:08amazed
- 20:09that even
- 20:10if
- 20:10there's
- 20:10little
- 20:10strings
- 20:11hanging
- 20:11that are
- 20:12about to
- 20:12dissolve,
- 20:13people
- 20:14just it
- 20:14really
- 20:15just is
- 20:15part of
- 20:16the eye.
- 20:17But, you
- 20:17know, it
- 20:17also,
- 20:18there's
- 20:18little
- 20:18details
- 20:18too.
- 20:19So
- 20:19there's
- 20:19different
- 20:19types
- 20:20of suture
- 20:20material
- 20:21and
- 20:22suture
- 20:22material
- 20:23that is
- 20:23more,
- 20:24sort
- 20:24of what
- 20:24we call
- 20:25hydrophilic
- 20:25or
- 20:26sort of
- 20:26water
- 20:26loving.
- 20:27those
- 20:27tend
- 20:28to be
- 20:28less
- 20:28noticeable,
- 20:30as
- 20:30opposed
- 20:30to
- 20:30certain
- 20:31types
- 20:31of
- 20:31stitches
- 20:32that are
- 20:33caused
- 20:33more
- 20:33irritation.
- 20:34So there
- 20:34definitely
- 20:34can be
- 20:35variations
- 20:36in
- 20:37materials
- 20:38that
- 20:38people
- 20:38use, in
- 20:39surgery.
- 20:40But
- 20:40the plaque
- 20:41itself
- 20:41usually
- 20:42does not,
- 20:43is not
- 20:43felt.
- 20:44and, and
- 20:45really
- 20:45for that
- 20:45week
- 20:47that the
- 20:47plaque
- 20:48is in
- 20:48place,
- 20:48it's
- 20:49normal
- 20:50to feel
- 20:51to have
- 20:51that same
- 20:52discharge
- 20:53coming
- 20:53out of
- 20:53the eye
- 20:54like
- 20:54tearing.
- 20:55And it's
- 20:55normal
- 20:56to
- 20:56kind of
- 20:56look in
- 20:57the
- 20:57mirror
- 20:57and see
- 20:58everything
- 20:58is
- 20:58totally
- 20:58swollen,
- 20:59like
- 21:00you were
- 21:00in
- 21:00a fight
- 21:01and
- 21:01everything
- 21:01looking
- 21:01red.
- 21:03but all
- 21:03that,
- 21:04once that
- 21:05week is over
- 21:06and
- 21:06the plaque
- 21:06is out,
- 21:07you know,
- 21:07everything
- 21:08heals
- 21:08very
- 21:09quickly.
- 21:09Like
- 21:09within
- 21:10a matter
- 21:10of even
- 21:12a couple
- 21:12of weeks
- 21:13later,
- 21:13when
- 21:13you look
- 21:14at the
- 21:14surface
- 21:15of
- 21:15the eye
- 21:16under
- 21:16the microscope,
- 21:17you can't
- 21:17even see
- 21:18where
- 21:19those
- 21:19incisions
- 21:20in the
- 21:20conjunctiva
- 21:21were.
- 21:21You can
- 21:22see
- 21:22little.
- 21:22The
- 21:22tissue is
- 21:23irregular
- 21:24because
- 21:24it's
- 21:24healing,
- 21:25but the
- 21:25healing
- 21:25happens
- 21:26very quickly.
- 21:27but
- 21:28it definitely
- 21:28can feel
- 21:29like a
- 21:29very
- 21:29prolonged,
- 21:30uncomfortable,
- 21:31I mean,
- 21:32uncomfortable,
- 21:33not
- 21:33so much
- 21:34in the
- 21:34sense of
- 21:35being painful,
- 21:35but just
- 21:37everything
- 21:37being so
- 21:38out of
- 21:38sorts
- 21:39and the
- 21:39swelling
- 21:40and
- 21:40looking
- 21:41red and
- 21:41feeling
- 21:42bumpy.
- 21:42I mean,
- 21:43that's
- 21:43all very
- 21:44normal.
- 21:45what isn't
- 21:46normal,
- 21:46though,
- 21:47is if
- 21:47you have
- 21:48a lot
- 21:49of pain,
- 21:49like
- 21:50if it
- 21:50feels
- 21:50very
- 21:50painful,
- 21:52especially
- 21:52if you
- 21:52have pain
- 21:53that
- 21:53develops
- 21:54during
- 21:55the
- 21:55the few
- 21:55days that
- 21:56the plaque
- 21:57is in
- 21:57place
- 21:57that wasn't
- 21:58there
- 21:58before.
- 21:59If you
- 21:59notice
- 21:59anything
- 22:00that
- 22:01seems
- 22:01unusual
- 22:02or
- 22:02uncomfortable,
- 22:03always
- 22:03call your
- 22:05ophthalmologist.
- 22:06Don't
- 22:06just
- 22:06kind of
- 22:07wait.
- 22:07And you
- 22:08know,
- 22:08if you
- 22:09know,
- 22:09pain that
- 22:09develops
- 22:10during
- 22:11the treatment
- 22:12period
- 22:12needs
- 22:12to be
- 22:13reported.
- 22:13I mean,
- 22:14it's
- 22:14possible,
- 22:15you know,
- 22:15what
- 22:15are the
- 22:15reasons
- 22:16that
- 22:16we want
- 22:17to
- 22:17watch out
- 22:17for?
- 22:17You could be
- 22:18developing
- 22:19high
- 22:19pressure
- 22:20in the
- 22:20eye.
- 22:22you
- 22:22could be
- 22:22developing
- 22:23an
- 22:23infection,
- 22:23although
- 22:24very
- 22:24uncommon,
- 22:25but
- 22:25still,
- 22:25I mean,
- 22:26it happens.
- 22:27So
- 22:27if you
- 22:28have any
- 22:28unusual
- 22:29pain,
- 22:29always
- 22:29call.
- 22:30don't.
- 22:30It's
- 22:31better
- 22:31to be
- 22:31safe than
- 22:31sorry if.
- 22:33And but
- 22:33the thing
- 22:33that we
- 22:33find is
- 22:34sometimes
- 22:34people
- 22:35take take
- 22:35photos
- 22:36with
- 22:36their
- 22:36phone
- 22:37of what
- 22:37it looks like
- 22:38and
- 22:38they say,
- 22:38is this
- 22:38normal?
- 22:39Or is,
- 22:39you know,
- 22:40there's
- 22:40a string
- 22:41I see
- 22:41here,
- 22:42you know,
- 22:42is this
- 22:43should I
- 22:43be
- 22:43concerned?
- 22:44So
- 22:45I always
- 22:45encourage
- 22:46people
- 22:46to, to
- 22:47sort of
- 22:47engage
- 22:48with your
- 22:49ophthalmologist,
- 22:50like
- 22:50reach
- 22:50out to
- 22:51the team
- 22:51like ask
- 22:52questions.
- 22:53you don't
- 22:53want to
- 22:54be kind
- 22:54of
- 22:54worrying
- 22:55during
- 22:55that
- 22:55week.
- 22:56and, and,
- 22:57you know,
- 22:57pain
- 22:58you in
- 22:59terms
- 22:59of it's
- 23:00normal
- 23:00to be
- 23:00uncomfortable,
- 23:02but
- 23:02that should,
- 23:03even as
- 23:03the time
- 23:04goes by,
- 23:05should
- 23:05ease or
- 23:06remain
- 23:06stable.
- 23:08and
- 23:08as soon
- 23:08as the
- 23:08plaque
- 23:09comes out
- 23:10with each
- 23:10passing
- 23:10day,
- 23:11it should
- 23:11start to
- 23:11feel
- 23:11better
- 23:12and
- 23:12better.
- 23:13So if
- 23:13anything
- 23:14doesn't
- 23:14quite fit
- 23:15that,
- 23:15or you're
- 23:15not sure,
- 23:16always
- 23:17call.
- 23:17I can't
- 23:18stress
- 23:18that
- 23:18enough.
- 23:18There's
- 23:19there's
- 23:19always
- 23:19someone
- 23:20available
- 23:20to answer
- 23:21questions
- 23:22at major
- 23:22centers.
- 23:23and so
- 23:24definitely
- 23:25use them.
- 23:26So
- 23:26that's
- 23:26why
- 23:26we have
- 23:27all
- 23:27the people
- 23:27that we do
- 23:28so that
- 23:29we can
- 23:29help
- 23:29people
- 23:30and bring
- 23:31people
- 23:31in.
- 23:31If there's
- 23:31any
- 23:32reason
- 23:32to double
- 23:32check
- 23:33something
- 23:33during
- 23:33the
- 23:33treatment.
- 23:35and then,
- 23:36while
- 23:37the
- 23:37plaque is
- 23:37in place.
- 23:39You know,
- 23:39often
- 23:40I find
- 23:40that
- 23:40sometimes
- 23:41people
- 23:42think
- 23:42that,
- 23:43you know,
- 23:43when the
- 23:43plaque
- 23:44comes out
- 23:44that
- 23:44melanoma
- 23:45is going
- 23:45to be gone.
- 23:46So
- 23:46what actually
- 23:47happens
- 23:47is the
- 23:48the
- 23:48radiation
- 23:49destroys
- 23:50the DNA
- 23:52in the
- 23:52melanoma
- 23:53tissue
- 23:53and does
- 23:53destroys
- 23:54the DNA
- 23:55in
- 23:55anything
- 23:55in its
- 23:56path.
- 23:56But
- 23:56because
- 23:57cancer
- 23:58cells
- 23:58are more
- 23:59rapidly
- 23:59dividing,
- 24:00their DNA
- 24:01is
- 24:02more sensitive.
- 24:03And so
- 24:04the those
- 24:05cancer
- 24:05cells are
- 24:06very
- 24:06prone to
- 24:07and
- 24:07they're
- 24:07very
- 24:08responsive
- 24:08to the
- 24:08radiation
- 24:09treatment.
- 24:10but right
- 24:11when the
- 24:11plaque
- 24:11comes
- 24:12out,
- 24:12things
- 24:13generally
- 24:13look
- 24:14about
- 24:14the same.
- 24:15They
- 24:15don't,
- 24:15it
- 24:15doesn't
- 24:16you
- 24:16really
- 24:16can't
- 24:17tell much
- 24:18difference
- 24:18in just a
- 24:19few days.
- 24:20However,
- 24:21often
- 24:22times
- 24:22by about
- 24:23a month
- 24:23afterwards,
- 24:24there
- 24:25are the
- 24:25the tumor
- 24:26can start
- 24:27looking
- 24:27different.
- 24:28one thing
- 24:28to point out
- 24:29is that
- 24:29depending
- 24:30on
- 24:30how big
- 24:31your tumor
- 24:31is to
- 24:32start
- 24:32with,
- 24:32that
- 24:33depends.
- 24:33That will
- 24:34determine
- 24:34how much
- 24:35change
- 24:35we see.
- 24:36So
- 24:37a person
- 24:37who has a
- 24:38very
- 24:38large
- 24:38melanoma
- 24:39will see
- 24:40a lot
- 24:40of
- 24:40changes
- 24:41faster
- 24:42in the
- 24:42recovery
- 24:43period.
- 24:43If
- 24:44you start off
- 24:44with a
- 24:44really
- 24:45tiny
- 24:45melanoma,
- 24:46we might not
- 24:47see
- 24:47very much
- 24:48right
- 24:48after
- 24:48the treatment.
- 24:49And
- 24:49it might
- 24:50take
- 24:51several
- 24:51visits
- 24:52to
- 24:52actually
- 24:53see
- 24:54reduction
- 24:55and
- 24:55leaking
- 24:56or even a
- 24:57little
- 24:57shriveling.
- 24:58So
- 24:58the smaller
- 24:59something
- 24:59is,
- 25:00the longer
- 25:00it's
- 25:01going
- 25:01to take
- 25:01to see
- 25:01much
- 25:02difference.
- 25:03But
- 25:03the bigger
- 25:03it is,
- 25:04the faster
- 25:05we see
- 25:05response.
- 25:07And so
- 25:07that's
- 25:07something
- 25:08to keep
- 25:08in mind
- 25:08because
- 25:08I, I know
- 25:09that it's
- 25:10normal
- 25:11when
- 25:11you get
- 25:11checked
- 25:11afterwards
- 25:12to kind
- 25:13of be
- 25:14hoping
- 25:14that I
- 25:14hope that
- 25:15it's
- 25:15shrunken,
- 25:15I hope
- 25:16that it's
- 25:16gone.
- 25:17But,
- 25:17you know,
- 25:18one thing
- 25:18to point out
- 25:19is that
- 25:19really
- 25:20whether
- 25:20if it
- 25:21if it's
- 25:22decreased
- 25:22in size
- 25:23or about
- 25:24the same
- 25:24size,
- 25:25that's
- 25:25kind of
- 25:25the same,
- 25:26in terms
- 25:27of the
- 25:27ocular
- 25:28oncologist
- 25:28perspective,
- 25:29when
- 25:29we're
- 25:29looking,
- 25:30if it
- 25:31doesn't
- 25:31shrink
- 25:31a little
- 25:32bit,
- 25:32it
- 25:33doesn't
- 25:33matter
- 25:33one way
- 25:34or the
- 25:34other
- 25:34to us.
- 25:34So, so
- 25:35we don't
- 25:35feel bad
- 25:36at all.
- 25:36So
- 25:36I want to
- 25:37point out
- 25:37that
- 25:38that's
- 25:38not
- 25:39necessarily
- 25:40like
- 25:40bad news
- 25:41if we don't
- 25:41see
- 25:42shrinkage
- 25:42and even,
- 25:43you know,
- 25:44years
- 25:44after
- 25:44follow
- 25:45up,
- 25:45we might see
- 25:46some
- 25:46reduction.
- 25:47And then
- 25:47it's the
- 25:47same
- 25:48measurements
- 25:49for
- 25:49a lot of
- 25:49time,
- 25:50then
- 25:50make it
- 25:50a little
- 25:50smaller.
- 25:51But,
- 25:52you know,
- 25:52as long
- 25:52as it's,
- 25:53you know,
- 25:53responded
- 25:54from its
- 25:54original
- 25:55state,
- 25:55you know,
- 25:56we're
- 25:56we're
- 25:56ahead.
- 25:59most
- 25:59times,
- 26:00you know,
- 26:00most
- 26:01centers
- 26:01do
- 26:02anywhere
- 26:02from 4
- 26:03to 7 days
- 26:05of a
- 26:05brachytherapy
- 26:06treatment.
- 26:06And
- 26:07you know
- 26:07the
- 26:07question.
- 26:08You know
- 26:08what?
- 26:09You know,
- 26:09we see
- 26:09a lot
- 26:09of people
- 26:10for
- 26:10second
- 26:10opinions.
- 26:11And
- 26:11people
- 26:11are
- 26:11wondering,
- 26:12why would I
- 26:13have,
- 26:13like,
- 26:14why do I have
- 26:15to have
- 26:15X number
- 26:16of days
- 26:16versus
- 26:16another?
- 26:17Well,
- 26:17it
- 26:18usually
- 26:18is just
- 26:18based on
- 26:19logistics.
- 26:20And for
- 26:21example,
- 26:22the ocular
- 26:23surgeon
- 26:23might put
- 26:24the plaque
- 26:24in on
- 26:24a Monday
- 26:25and it's
- 26:25convenient
- 26:26to
- 26:26take it
- 26:26out on
- 26:26a Friday.
- 26:27You know,
- 26:28at
- 26:28our center,
- 26:29we,
- 26:29we always
- 26:29plan for
- 26:30seven day
- 26:30plaques
- 26:31just
- 26:31so that
- 26:32we
- 26:32come back
- 26:33to
- 26:33the day
- 26:33that
- 26:34I have
- 26:34assigned
- 26:35to me
- 26:35to do
- 26:35surgery
- 26:36at our center.
- 26:37And,
- 26:38just
- 26:38because
- 26:39we have
- 26:39a lot
- 26:39of number
- 26:40of days
- 26:40doesn't
- 26:41mean
- 26:41we have
- 26:41any more
- 26:42radiation.
- 26:43So what
- 26:43the physics
- 26:44the physicists
- 26:45do is
- 26:45they
- 26:46determine
- 26:46how much
- 26:46radiation
- 26:47is needed
- 26:48to kill
- 26:48a
- 26:48melanoma
- 26:49of
- 26:49that size,
- 26:50and then
- 26:51they look
- 26:51at the
- 26:52amount
- 26:52of time
- 26:52that they
- 26:52have to
- 26:53to have
- 26:53the
- 26:54radiation
- 26:55be
- 26:55released.
- 26:55And so
- 26:56when you
- 26:57have a
- 26:57longer
- 26:57number
- 26:58of days,
- 26:58usually
- 26:58the
- 26:59radiation
- 26:59release
- 26:59is slower.
- 27:01If you have a
- 27:01very if
- 27:02you're
- 27:02only,
- 27:02say,
- 27:02putting it
- 27:03in for
- 27:03three
- 27:03days,
- 27:04then
- 27:05our physics,
- 27:05our physicists
- 27:06like to
- 27:06call,
- 27:07they call
- 27:07that
- 27:07like
- 27:07a very
- 27:08hot
- 27:08plaque,
- 27:08because
- 27:09there's
- 27:09much more
- 27:10radiation
- 27:10coming
- 27:11out,
- 27:11because
- 27:11all of it
- 27:11has to
- 27:12come out
- 27:12in
- 27:12three days.
- 27:14So
- 27:14it really
- 27:14just
- 27:14depends
- 27:15on
- 27:15planning.
- 27:16The amount
- 27:17of
- 27:17radiation
- 27:17that is
- 27:18used is
- 27:19really
- 27:19based on
- 27:20the tumor
- 27:21dimensions
- 27:22that the
- 27:22radiation
- 27:23team
- 27:23get
- 27:24when they
- 27:24construct
- 27:25the
- 27:25plaque.
- 27:26and
- 27:26sometimes
- 27:27I
- 27:27mentioned
- 27:28before
- 27:28that,
- 27:28you know,
- 27:29some
- 27:29ultrasound
- 27:30can help
- 27:30give more
- 27:31information
- 27:31during
- 27:32the surgery,
- 27:33for
- 27:33example,
- 27:34a larger
- 27:34size
- 27:34melanoma.
- 27:35We
- 27:36we always
- 27:36remeasure them
- 27:37because
- 27:38they
- 27:38the bigger
- 27:39the tumor
- 27:40is like
- 27:40the more
- 27:41cancer
- 27:41cells
- 27:41there
- 27:41are.
- 27:42So
- 27:42the higher
- 27:42the
- 27:42likelihood
- 27:43that it
- 27:43can
- 27:43increase
- 27:44in size.
- 27:45if you
- 27:46if you
- 27:46have a
- 27:46delay in
- 27:47treatment,
- 27:47like
- 27:48if it's
- 27:48say that
- 27:49you're
- 27:49supposed
- 27:49to go,
- 27:50but
- 27:50you were
- 27:50delayed
- 27:50a couple of weeks.
- 27:51So really
- 27:52confirming
- 27:53the
- 27:53measurement
- 27:54at
- 27:54the time
- 27:55of
- 27:55surgery
- 27:55is
- 27:55really important.
- 27:56Because
- 27:56if we add
- 27:56extra
- 27:57days,
- 27:58it's to
- 27:58help make
- 27:59sure
- 27:59we get
- 27:59the treatment.
- 28:00And then
- 28:01there's
- 28:01another
- 28:01reason
- 28:01sometimes
- 28:02we add
- 28:02a day
- 28:03or so is
- 28:04because
- 28:05when we
- 28:05look
- 28:05at it
- 28:06by
- 28:06ultrasound,
- 28:07sometimes
- 28:08these
- 28:08breakthrough
- 28:08therapy
- 28:09plaques,
- 28:10they don't
- 28:10fit
- 28:11perfectly
- 28:12around
- 28:12the
- 28:12roundness
- 28:13of the eye.
- 28:13And
- 28:13sometimes
- 28:14when
- 28:14you see
- 28:15by
- 28:15ultrasound,
- 28:16you can see
- 28:16that
- 28:16the plaque
- 28:17might be
- 28:17a little
- 28:17bit away
- 28:18from
- 28:18the eye
- 28:19wall.
- 28:20And so
- 28:20what
- 28:20we do
- 28:21is we
- 28:21measure
- 28:22that
- 28:22space
- 28:23and
- 28:23recalculate
- 28:24because
- 28:25we don't
- 28:25want to
- 28:25under
- 28:26treat
- 28:26just
- 28:26because
- 28:26the
- 28:26plaque
- 28:27is a
- 28:27little
- 28:27bit away
- 28:28from
- 28:28the eye
- 28:28wall.
- 28:29and
- 28:30the reason
- 28:30for this
- 28:31is
- 28:31because
- 28:31there's,
- 28:31there's,
- 28:32there's
- 28:32a lot
- 28:32of
- 28:32tissues
- 28:33in the,
- 28:34in
- 28:34the eye
- 28:34space,
- 28:35and
- 28:35especially
- 28:36when
- 28:36the tumors
- 28:37are very
- 28:38posterior
- 28:38at the
- 28:38back
- 28:39of the eye
- 28:39or the
- 28:39very tiny,
- 28:40sometimes
- 28:41on
- 28:41ultrasound,
- 28:41we can see
- 28:42some
- 28:42tissue
- 28:42between
- 28:43the plaque
- 28:44and
- 28:44the eye
- 28:44wall.
- 28:45And even
- 28:46if you go
- 28:46and try
- 28:46to move
- 28:47the tissue,
- 28:47sometimes
- 28:48you just
- 28:48can't.
- 28:49Nothing
- 28:49really
- 28:50moves
- 28:50depending
- 28:51on
- 28:51where it is.
- 28:51So
- 28:51we just
- 28:52recalculate
- 28:53the treatment.
- 28:53So
- 28:54a lot of
- 28:54adjustments
- 28:54can
- 28:55happen
- 28:55to the
- 28:56treatment
- 28:56plan at
- 28:57the time
- 28:57of
- 28:57surgery.
- 28:58and it's
- 28:59important
- 28:59to
- 28:59kind of
- 29:00look for
- 29:00things
- 29:01and allow
- 29:01for being
- 29:02flexible
- 29:03if
- 29:03it's going
- 29:04to
- 29:04result in
- 29:04better
- 29:05treatment.
- 29:07While
- 29:08they have
- 29:08it on
- 29:09whether
- 29:10it's
- 29:10four days
- 29:10or
- 29:10seven
- 29:11days or
- 29:11whatever,
- 29:11what
- 29:12can they
- 29:12expect?
- 29:13Like are
- 29:13they,
- 29:14are they going
- 29:14to feel
- 29:15nauseous?
- 29:15Are
- 29:15they
- 29:15allowed
- 29:15to drive?
- 29:16Do
- 29:16they need
- 29:16a special
- 29:17diet?
- 29:17Like
- 29:17what's
- 29:18that time
- 29:19with the
- 29:19patch on
- 29:20going to
- 29:20look
- 29:20like?
- 29:22Yeah,
- 29:22that's a
- 29:22great question.
- 29:25so we
- 29:26generally
- 29:27recommend
- 29:28well,
- 29:28first
- 29:28of all
- 29:28I'm going
- 29:29to just
- 29:29go real
- 29:29basic.
- 29:30So
- 29:30are really
- 29:31are
- 29:31physicists
- 29:32recommend
- 29:32that
- 29:33people
- 29:33don't go
- 29:34into
- 29:34crowds of
- 29:35people.
- 29:36and, and
- 29:36I
- 29:37always say
- 29:37if you're
- 29:37around
- 29:38people
- 29:38or pets,
- 29:39just wear
- 29:40the lead
- 29:40shield
- 29:41and, they
- 29:42we often
- 29:43provide
- 29:43most
- 29:43centers
- 29:43provide
- 29:44literally
- 29:45like a
- 29:45pirate
- 29:45patch
- 29:46with a
- 29:46lead
- 29:46on it
- 29:46to,
- 29:47to reduce
- 29:48or to
- 29:48really
- 29:48to
- 29:49prevent
- 29:50any
- 29:50extraneous
- 29:51radiation
- 29:52from
- 29:52coming
- 29:52around.
- 29:54and, and,
- 29:55you know,
- 29:55it's
- 29:56important
- 29:56to
- 29:56keep in
- 29:56mind that
- 29:58the
- 29:58amount
- 29:59the
- 29:59eyeball
- 29:59is so
- 29:59small,
- 30:01a
- 30:01melanoma,
- 30:01even a
- 30:02you know,
- 30:03the size
- 30:03of tumors
- 30:04is very
- 30:04relative
- 30:05but
- 30:05relative,
- 30:06the whole
- 30:06body
- 30:07very tiny.
- 30:08So
- 30:08the amount
- 30:08of
- 30:08radiation
- 30:09that even
- 30:09can be
- 30:10detected
- 30:10close
- 30:11to
- 30:11a person
- 30:12with the
- 30:12brachytherapy
- 30:13plaque,
- 30:14even a
- 30:14treating
- 30:15a large
- 30:15tumor is
- 30:16very low.
- 30:17But
- 30:18because,
- 30:19you know,
- 30:19like most
- 30:19physics
- 30:20and
- 30:20radiation
- 30:21oncology
- 30:21departments,
- 30:22we
- 30:22recommend
- 30:23just
- 30:23the same
- 30:24protocols,
- 30:25whether
- 30:25you're
- 30:25having,
- 30:26you know,
- 30:27whatever
- 30:27the organ
- 30:27is.
- 30:28So
- 30:28they always
- 30:28recommend
- 30:29wearing
- 30:29the lead
- 30:30being,
- 30:31a certain
- 30:31amount,
- 30:32a certain
- 30:32amount of
- 30:33distance
- 30:33away
- 30:33from people.
- 30:34But
- 30:35I just
- 30:35sort of
- 30:36simplify
- 30:36it to
- 30:37if there
- 30:37if people
- 30:38around
- 30:38you
- 30:38put it
- 30:38on,
- 30:39if you're
- 30:39by
- 30:39yourself,
- 30:40have it
- 30:40open.
- 30:42and and
- 30:43just
- 30:43try to
- 30:44minimize
- 30:44that, but
- 30:45not
- 30:45to be alarmed
- 30:46if
- 30:46suddenly
- 30:47your
- 30:47family
- 30:47member
- 30:48is
- 30:48sitting
- 30:48on your
- 30:48lap and
- 30:50oops,
- 30:50we forgot
- 30:50about
- 30:51that,
- 30:51you can
- 30:51just,
- 30:52you know,
- 30:52nothing,
- 30:53nothing
- 30:54is going
- 30:54to
- 30:54happen.
- 30:54And we've
- 30:55never
- 30:55heard of
- 30:56a case
- 30:56of
- 30:56someone
- 30:57developing
- 30:57a problem
- 30:58with
- 30:59a partner
- 30:59or
- 30:59somebody
- 31:00getting
- 31:00exposed.
- 31:01But we
- 31:01just
- 31:02do it
- 31:02for
- 31:02safety.
- 31:03most
- 31:04people
- 31:04feel
- 31:05nauseous.
- 31:06If people
- 31:07feel
- 31:07nauseous
- 31:08the day
- 31:09after
- 31:09surgery,
- 31:10it's
- 31:10in my
- 31:11experience,
- 31:11it's
- 31:11usually
- 31:12the
- 31:12anesthesia
- 31:13that's
- 31:14wearing
- 31:14off.
- 31:14And,
- 31:15and I
- 31:15find
- 31:16that,
- 31:16you know,
- 31:16when
- 31:17we're
- 31:17checking
- 31:17people
- 31:17the first
- 31:18day after
- 31:18surgery,
- 31:19a
- 31:19lot of people,
- 31:20not
- 31:20a lot of
- 31:20people,
- 31:20but some
- 31:21people,
- 31:21especially
- 31:21younger
- 31:22people,
- 31:22young men
- 31:23tend
- 31:23to be
- 31:24kind of
- 31:25not like
- 31:25they're
- 31:26kind of
- 31:26passing
- 31:26out
- 31:27or not
- 31:27feeling
- 31:27good.
- 31:28it's it's
- 31:29a lot
- 31:29of it
- 31:30is the
- 31:30anesthetic
- 31:31that is
- 31:31that is
- 31:32still in
- 31:32the system.
- 31:33And that
- 31:34usually
- 31:34passes
- 31:35after
- 31:35about a day.
- 31:36So
- 31:36I would
- 31:36tell
- 31:37people
- 31:37to be
- 31:38just,
- 31:38you know,
- 31:39and in
- 31:39terms of diet
- 31:40and what
- 31:41you should
- 31:41eat,
- 31:41don't
- 31:41don't
- 31:42rush
- 31:42anything
- 31:43because
- 31:43if you're
- 31:44kind of
- 31:45a little
- 31:45bit on a
- 31:46roller
- 31:46coaster
- 31:47in terms
- 31:48of nausea
- 31:48and not
- 31:48feeling
- 31:49great
- 31:49the very
- 31:50next day
- 31:50after
- 31:51surgery,
- 31:51you want to
- 31:51take it
- 31:52easy.
- 31:52So go.
- 31:54You know,
- 31:54you
- 31:54probably
- 31:55people
- 31:55have been
- 31:56sort of,
- 31:56you know,
- 31:56no food
- 31:57or drink
- 31:57before
- 31:58midnight
- 31:58and
- 31:58people
- 31:58have been
- 31:59eating up
- 32:00until the
- 32:00surgery.
- 32:01So go
- 32:01really
- 32:01slow.
- 32:02Lots of
- 32:02fluids.
- 32:03you know,
- 32:04don't
- 32:04don't
- 32:04rush into
- 32:05a big meal
- 32:05right
- 32:06after the
- 32:06surgery.
- 32:07Just go
- 32:07slowly,
- 32:08and
- 32:09sort of
- 32:09build up
- 32:09your appetite.
- 32:10Wait
- 32:10till you
- 32:10really
- 32:11have a
- 32:11strong
- 32:11appetite
- 32:12because
- 32:12it's not
- 32:13unusual
- 32:14to feel
- 32:14a bit off
- 32:15after
- 32:15you have
- 32:16a procedure.
- 32:17And
- 32:17much of
- 32:17it isn't.
- 32:18As
- 32:18I said,
- 32:18it's
- 32:18mainly
- 32:19the anesthetic.
- 32:19It's not
- 32:20necessarily
- 32:20the,
- 32:21you know,
- 32:22manipulating
- 32:23the eye
- 32:23itself,
- 32:24although
- 32:25some
- 32:25people,
- 32:26if you
- 32:26feel
- 32:26really
- 32:27nauseous
- 32:27or
- 32:28unwell,
- 32:29we really
- 32:29we have
- 32:29to
- 32:30make sure
- 32:31that
- 32:31the eye
- 32:31is doing
- 32:32okay.
- 32:32And
- 32:32that's
- 32:32why we
- 32:33check
- 32:33people
- 32:33the day
- 32:34after
- 32:34surgery
- 32:35to check
- 32:35the
- 32:36intraocular
- 32:36pressure.
- 32:37Now
- 32:37sometimes
- 32:38the
- 32:38pressure
- 32:38can go up
- 32:39just
- 32:40having
- 32:40surgery.
- 32:41So
- 32:41we want to
- 32:41make sure
- 32:42if that
- 32:42is the
- 32:43case,
- 32:43that we
- 32:43treat it
- 32:44during
- 32:44the
- 32:44post-operative
- 32:45period.
- 32:47usually
- 32:48with each
- 32:48passing
- 32:49day the
- 32:50the person
- 32:51should
- 32:51feel a
- 32:51little
- 32:52bit more
- 32:52comfortable
- 32:53and,
- 32:53you know,
- 32:54the
- 32:54swelling
- 32:54will get
- 32:55less.
- 32:55and I
- 32:56encourage,
- 32:57you know,
- 32:57if,
- 32:57if
- 32:58the
- 32:58eye is
- 32:58so
- 32:58swollen
- 32:59that it
- 33:00can't
- 33:00even move
- 33:01when the,
- 33:02when the
- 33:02patch
- 33:02comes off
- 33:03and it's
- 33:03being
- 33:03checked
- 33:04that
- 33:04a person
- 33:05do
- 33:05a short
- 33:05course
- 33:06of, of
- 33:06of
- 33:06steroids,
- 33:07like,
- 33:07like a
- 33:08medrol
- 33:08dose pack
- 33:09because
- 33:10the pain
- 33:10when
- 33:11tissues
- 33:11are
- 33:11swollen
- 33:12can be
- 33:12excruciating.
- 33:13And it's
- 33:14very safe
- 33:15to have
- 33:15something
- 33:16to just
- 33:16to
- 33:16get you
- 33:16to feel
- 33:17okay
- 33:18afterwards.
- 33:19we use
- 33:20solu
- 33:20medrol
- 33:21intravenously
- 33:22for
- 33:23during
- 33:23our operations
- 33:24and ever
- 33:25like a
- 33:26colleague
- 33:26of mine.
- 33:27was doing
- 33:28this for
- 33:28scleral
- 33:29buckles,
- 33:29and I
- 33:30started
- 33:30doing it,
- 33:31I don't
- 33:31know,
- 33:31maybe 12
- 33:32years
- 33:32ago.
- 33:33And it's
- 33:33just
- 33:33it's day
- 33:34and
- 33:34night.
- 33:35We used
- 33:35to have
- 33:36so much
- 33:36trouble
- 33:36taking
- 33:37the patch off
- 33:37because
- 33:37people
- 33:38were
- 33:38so sore
- 33:38because
- 33:39of the
- 33:39swelling.
- 33:39And now
- 33:40people
- 33:40walk in
- 33:41like like
- 33:42nothing's
- 33:42happened.
- 33:42So
- 33:43it really
- 33:43makes a
- 33:44difference.
- 33:44So really
- 33:45kind of,
- 33:45you know,
- 33:45there's
- 33:46little
- 33:46things
- 33:46you can
- 33:46do.
- 33:46And
- 33:46I also
- 33:47tell
- 33:47people
- 33:47to use
- 33:48a lot of ice.
- 33:49So
- 33:50the swelling
- 33:51can be
- 33:51if it's
- 33:52a lot of
- 33:52inflammatory
- 33:53swelling.
- 33:53So
- 33:54if you have
- 33:54like ice
- 33:55chips
- 33:55or frozen
- 33:56peas
- 33:56in a bag,
- 33:57put it
- 33:57in a Ziploc
- 33:58bag and
- 33:58just
- 33:58hold it
- 33:59over
- 33:59your eye.
- 34:00That'll
- 34:00really
- 34:01help with
- 34:01the swelling.
- 34:02And
- 34:02if you
- 34:02use do
- 34:03a lot
- 34:03of icing
- 34:05of
- 34:05the eye,
- 34:06you will
- 34:06probably
- 34:08you need
- 34:08to use
- 34:08less
- 34:09Tylenol
- 34:09or Advil,
- 34:10and I
- 34:11recommend
- 34:11everybody
- 34:12use
- 34:12Tylenol
- 34:12and Advil
- 34:13if they're
- 34:14uncomfortable.
- 34:15we have
- 34:15not
- 34:16had to
- 34:16use
- 34:17narcotics
- 34:18for many
- 34:19years
- 34:19now.
- 34:20and so
- 34:21there are
- 34:21measures
- 34:22that you
- 34:22can do
- 34:23to help
- 34:23alleviate
- 34:24the sort
- 34:24of
- 34:24post-operative
- 34:25pain and
- 34:25discomfort.
- 34:27think the
- 34:27the last
- 34:28part of
- 34:28it
- 34:28would be
- 34:28what's
- 34:29their
- 34:29activity
- 34:29level?
- 34:30Are they
- 34:30should
- 34:30they be
- 34:31kind of
- 34:31laying
- 34:31low, not
- 34:32driving,
- 34:33or
- 34:33can they
- 34:34do
- 34:34anything.
- 34:35Yeah,
- 34:36that's
- 34:36a great question.
- 34:37So
- 34:37you know
- 34:38I tell
- 34:38people
- 34:39to just
- 34:39you know,
- 34:39if you're
- 34:40feeling
- 34:40well just
- 34:42go back
- 34:42and do
- 34:42your
- 34:43physical
- 34:43activities.
- 34:44There's
- 34:44really,
- 34:45no reason
- 34:46to
- 34:46kind of
- 34:47rest
- 34:48and lay
- 34:48in bed
- 34:49and
- 34:49really
- 34:50chill
- 34:50out.
- 34:51I
- 34:51find that
- 34:51when the
- 34:52more
- 34:52people
- 34:53kind of
- 34:53get back
- 34:54to doing
- 34:54things
- 34:55moving
- 34:55around
- 34:56their home
- 34:56or,
- 34:57you know,
- 34:57doing
- 34:58light
- 34:58exercise,
- 34:59the people
- 35:00recover
- 35:01much
- 35:01better.
- 35:02You know,
- 35:02it's it's
- 35:03know
- 35:03nowadays
- 35:04especially
- 35:05we used
- 35:05to do
- 35:06a lot
- 35:06more
- 35:06positioning
- 35:07in
- 35:08retinal
- 35:08surgery.
- 35:09Have
- 35:09people be
- 35:09face down
- 35:10for two
- 35:10weeks
- 35:10or do
- 35:11even,
- 35:11you know,
- 35:12way back
- 35:12when people
- 35:13have
- 35:13to be
- 35:13in
- 35:13patients
- 35:14to have
- 35:14cataract
- 35:15surgery?
- 35:15We've
- 35:16really
- 35:16moved
- 35:16away from
- 35:17that.
- 35:17Now.
- 35:17People
- 35:18don't
- 35:18even
- 35:18position
- 35:19a lot of times
- 35:20for
- 35:20retinal surgery.
- 35:21and
- 35:22so it's
- 35:23really
- 35:23important
- 35:24to get
- 35:24people,
- 35:25you know,
- 35:25feeling
- 35:25good
- 35:26as soon
- 35:26as
- 35:26possible
- 35:26and doing
- 35:28things
- 35:28that
- 35:28they feel
- 35:28comfortable.
- 35:29I tell people,
- 35:29if you're
- 35:30feeling
- 35:30if
- 35:31something
- 35:31you're
- 35:31doing
- 35:31makes you
- 35:32feel
- 35:32uncomfortable,
- 35:33like even
- 35:33you
- 35:33mentioned
- 35:33driving,
- 35:34technically
- 35:35you can
- 35:35drive
- 35:36legally,
- 35:37but it's
- 35:38really
- 35:38a matter
- 35:38of
- 35:39comfort.
- 35:39So once
- 35:40you're
- 35:40comfortable
- 35:40moving
- 35:41around
- 35:41your home,
- 35:42it is
- 35:42probably
- 35:43is it
- 35:44safe
- 35:44to go
- 35:45very
- 35:45short
- 35:45distances
- 35:46if you want
- 35:47to, if
- 35:47you're
- 35:47just if
- 35:47you're
- 35:47feeling
- 35:48unwell
- 35:48or not
- 35:49good,
- 35:49or
- 35:49you feel off
- 35:50because
- 35:51your eye
- 35:51is
- 35:51really blurry,
- 35:52then
- 35:52definitely
- 35:53have
- 35:53people
- 35:53help you,
- 35:54you know,
- 35:54and wait
- 35:55till
- 35:55you feel
- 35:56comfortable
- 35:56before
- 35:57venturing
- 35:58back into
- 35:59all the
- 35:59regular
- 35:59activities.
- 36:00But in
- 36:01general,
- 36:01I
- 36:02recommend
- 36:03people
- 36:03don't
- 36:03lay low
- 36:04for that
- 36:05week.
- 36:06it's just
- 36:06important
- 36:07to
- 36:08stay active,
- 36:08you know,
- 36:09I have
- 36:09some people,
- 36:10if you feel
- 36:10really
- 36:10great
- 36:11the day
- 36:12the plaque
- 36:12comes
- 36:12off, then
- 36:13you can
- 36:13go for
- 36:14a walk
- 36:14or,
- 36:15you know,
- 36:15do some
- 36:16light
- 36:16exercise.
- 36:17it's just
- 36:18really
- 36:18important
- 36:18to just
- 36:19keep
- 36:19yourself
- 36:19moving
- 36:20and not
- 36:20completely
- 36:21kind of
- 36:22regress
- 36:22into it,
- 36:23because
- 36:23then
- 36:23you
- 36:23have
- 36:23to kind
- 36:24of climb
- 36:24out of
- 36:24that
- 36:25afterwards.
- 36:25And, it's
- 36:26just
- 36:26so much
- 36:27better
- 36:27for your
- 36:28overall
- 36:30wellness
- 36:30and just
- 36:31feeling
- 36:31good
- 36:31about
- 36:32things
- 36:32to be
- 36:32a little
- 36:33more
- 36:33active.
- 36:34the main
- 36:35difference
- 36:35with
- 36:35brachytherapy
- 36:36and
- 36:36proton
- 36:37therapy
- 36:37is that
- 36:38in
- 36:38brachytherapy
- 36:39the radiation
- 36:39comes
- 36:40from the
- 36:41plaque
- 36:42sort of
- 36:42behind
- 36:43the eye
- 36:43wall to
- 36:44the tumor,
- 36:44which is
- 36:45on
- 36:45the eye
- 36:45wall in
- 36:47proton
- 36:47beam
- 36:48therapy
- 36:48or
- 36:49any kind
- 36:49of
- 36:50external,
- 36:51radiation
- 36:52therapy.
- 36:53the radiation
- 36:54comes
- 36:54through
- 36:55the front
- 36:55of
- 36:55the eye
- 36:56and then
- 36:56culminates
- 36:57at
- 36:57the tumor
- 36:58to treat
- 36:58the
- 36:58tumor.
- 36:59So,
- 36:59proton
- 37:00therapy,
- 37:01involves,
- 37:02and I
- 37:03used to
- 37:03in my
- 37:04training,
- 37:04I, my
- 37:05training
- 37:05was at
- 37:06the
- 37:06Massachusetts
- 37:07Eye and
- 37:07your
- 37:07infirmary
- 37:08at
- 37:08Harvard
- 37:08Medical School,
- 37:09and
- 37:09we had
- 37:10the
- 37:10northwest
- 37:11pro
- 37:11northeast
- 37:11proton
- 37:12beam
- 37:12center,
- 37:13where,
- 37:13when
- 37:14I was
- 37:14a fellow
- 37:14in
- 37:14training,
- 37:15we spent
- 37:16a lot of
- 37:16time with
- 37:17people
- 37:18who were
- 37:18having
- 37:19proton
- 37:19beam and
- 37:19kind of
- 37:20going
- 37:20through
- 37:20everything.
- 37:21So
- 37:21I feel,
- 37:22I mean,
- 37:22if it's
- 37:22like
- 37:22yesterday,
- 37:23but what?
- 37:24So
- 37:24people,
- 37:25you still
- 37:25need
- 37:26to have
- 37:26a surgery
- 37:27to mark
- 37:28the
- 37:28borders
- 37:28of
- 37:28the tumor.
- 37:29And in
- 37:30brachytherapy
- 37:31surgery
- 37:31you have
- 37:32the
- 37:32surgery.
- 37:33You find
- 37:33the tumor,
- 37:34you
- 37:34put the
- 37:34plaque
- 37:35in place
- 37:36for
- 37:36proton
- 37:36therapy,
- 37:37you have a
- 37:37surgery,
- 37:38you find the tumor
- 37:39and you
- 37:39put
- 37:39markers
- 37:40at the
- 37:41edge of
- 37:41the tumor.
- 37:42And these
- 37:42markers
- 37:43will can
- 37:44be
- 37:44detected
- 37:45on, on
- 37:46X-rays
- 37:47that
- 37:47they use
- 37:48at
- 37:48the proton
- 37:49beam.
- 37:49So
- 37:49you have
- 37:50a surgery
- 37:50to put
- 37:51markers
- 37:51and
- 37:51usually 1
- 37:52or 2
- 37:52weeks
- 37:53later,
- 37:54then
- 37:54you go
- 37:54for your
- 37:55sessions
- 37:55at the
- 37:56proton
- 37:56proton
- 37:57beam
- 37:57facility.
- 37:58And
- 37:58usually
- 37:58it's it's
- 37:59like five
- 38:00days,
- 38:00five
- 38:01consecutive
- 38:01days
- 38:02where
- 38:02each
- 38:02treatment
- 38:03may be
- 38:04about
- 38:04a minute,
- 38:05very short.
- 38:06But
- 38:06during
- 38:07those
- 38:07radiation
- 38:08treatments,
- 38:09you're
- 38:09awake.
- 38:11We don't use
- 38:11anesthesia
- 38:12for the
- 38:13actual
- 38:13radiation
- 38:14sessions.
- 38:14We do
- 38:15for the
- 38:15surgical part,
- 38:16but for
- 38:16the
- 38:16radiation
- 38:17sessions,
- 38:18it's
- 38:18important
- 38:19that you are.
- 38:20You are
- 38:20very,
- 38:20very
- 38:21still
- 38:21because
- 38:22you
- 38:23if you
- 38:23move or
- 38:24blink or
- 38:24move
- 38:25around,
- 38:26then
- 38:26the really
- 38:26the path
- 38:27of the
- 38:27radiation
- 38:28isn't
- 38:28as accurate.
- 38:29So in
- 38:30order to
- 38:30do that,
- 38:31to
- 38:31prepare
- 38:32for the
- 38:32radiation
- 38:32sessions,
- 38:33most
- 38:34places
- 38:34will have
- 38:35create
- 38:36sort of
- 38:36a frame
- 38:36for your
- 38:37face,
- 38:37which
- 38:37includes,
- 38:38certainly
- 38:39at the
- 38:39the
- 38:40northeast
- 38:40proton
- 38:40beam
- 38:40center,
- 38:41there was
- 38:41a bite
- 38:41block
- 38:42to clench
- 38:43your teeth around
- 38:44so
- 38:44you could
- 38:44hold
- 38:44your face
- 38:45really,
- 38:46really
- 38:46still.
- 38:46And there
- 38:47was
- 38:47something
- 38:47that,
- 38:48light
- 38:49or an
- 38:49indicator
- 38:49that you
- 38:50would
- 38:50look
- 38:50at to
- 38:50keep
- 38:51your gaze
- 38:51steady.
- 38:52And then
- 38:53there
- 38:53would be
- 38:53a monitor.
- 38:54There
- 38:54would be
- 38:54somebody
- 38:54monitoring.
- 38:55And
- 38:55certainly
- 38:56at
- 38:56the center
- 38:57I trained
- 38:57at,
- 38:57it was the fellow.
- 38:58So
- 38:59we would
- 38:59we would
- 38:59help the
- 39:00patients,
- 39:01open
- 39:01the eyelids
- 39:02and
- 39:02we would
- 39:02monitor
- 39:02that.
- 39:03The eye
- 39:04was
- 39:04exactly
- 39:04centered.
- 39:05And,
- 39:06you know,
- 39:06people
- 39:07were not
- 39:07sort of
- 39:08wandering
- 39:08the gaze.
- 39:09If
- 39:09a person
- 39:09started
- 39:10to drift,
- 39:10we
- 39:10we turned
- 39:11off,
- 39:12we were able
- 39:12to stop
- 39:13the treatment
- 39:14and then
- 39:14kind of
- 39:15get
- 39:15things
- 39:15on track
- 39:16again.
- 39:17and
- 39:17sometimes,
- 39:18you know,
- 39:18sometimes
- 39:19can be
- 39:19difficult.
- 39:19So
- 39:20we'd
- 39:20have to
- 39:20kind of
- 39:20stop and
- 39:21make sure
- 39:21that
- 39:21people
- 39:22knew what
- 39:22they
- 39:22should be
- 39:22looking
- 39:23straight
- 39:23at.
- 39:24but
- 39:24certainly
- 39:24the
- 39:24centers
- 39:25that have
- 39:25a lot
- 39:25of
- 39:25experience
- 39:26with
- 39:26proton
- 39:26beam,
- 39:27they're,
- 39:27they're
- 39:27really,
- 39:28you know,
- 39:28they're
- 39:28really
- 39:29good at
- 39:29making
- 39:29sure that
- 39:30everything's
- 39:30in place.
- 39:31And,
- 39:31you know,
- 39:31remember
- 39:32the
- 39:32radiation
- 39:32team was
- 39:33was
- 39:34incredibly
- 39:34excellent.
- 39:35Everybody
- 39:35knew
- 39:35how to
- 39:36just get
- 39:36things
- 39:36just
- 39:37right.
- 39:37So there
- 39:38there's
- 39:38a series
- 39:39of
- 39:39treatments.
- 39:39And so
- 39:40the end
- 39:40of
- 39:41about say
- 39:42it's
- 39:42usually
- 39:42that I'm
- 39:43familiar
- 39:43with
- 39:43about
- 39:43five
- 39:44sessions.
- 39:44that
- 39:44would be
- 39:45the end
- 39:45of the
- 39:45radiation
- 39:46treatment.
- 39:47and then
- 39:48the same
- 39:48monitoring
- 39:49happens.
- 39:50We,
- 39:50we
- 39:50see you
- 39:51afterwards.
- 39:52We then
- 39:52we start
- 39:53doing
- 39:53ultrasounds
- 39:54to
- 39:54confirm
- 39:55that
- 39:55there's
- 39:55been good
- 39:56treatment
- 39:56response.
- 39:58so, so
- 39:58proton
- 39:58beam
- 39:59therapy
- 39:59is very
- 40:00good at,
- 40:00destroying
- 40:01melanoma.
- 40:01No
- 40:01question.
- 40:02and
- 40:02I think
- 40:03that most
- 40:03centers
- 40:04know the
- 40:05interesting
- 40:06thing
- 40:06that I've
- 40:06learned
- 40:07over
- 40:07the years
- 40:07is that
- 40:07everybody
- 40:08does
- 40:08things
- 40:08a little
- 40:08bit
- 40:09different.
- 40:10And
- 40:10when I
- 40:11visit
- 40:11colleagues
- 40:12and
- 40:12see what
- 40:12they're doing,
- 40:13I learn
- 40:14that
- 40:14there's
- 40:14different,
- 40:14you know,
- 40:15people
- 40:15use
- 40:15slightly
- 40:15different
- 40:16techniques.
- 40:16Some people
- 40:17even
- 40:18measure
- 40:18for the
- 40:19Reiki
- 40:19therapy,
- 40:19like
- 40:20slightly
- 40:20differently.
- 40:21you know,
- 40:22it's just
- 40:22amazing
- 40:23the amount
- 40:23of
- 40:23variation
- 40:24that
- 40:24can happen
- 40:25that
- 40:25that's
- 40:26involved
- 40:26in this.
- 40:26Some people
- 40:27really
- 40:27like
- 40:28to take
- 40:28muzzles
- 40:28off,
- 40:29I don't like
- 40:30to take
- 40:30muscles off
- 40:30because
- 40:31it's just
- 40:31extra
- 40:31stuff.
- 40:32And I
- 40:32don't,
- 40:33you
- 40:33know,
- 40:33that
- 40:33that's
- 40:33another thing.
- 40:33I'll
- 40:33mention
- 40:34that
- 40:34sometimes
- 40:34in order
- 40:35to put
- 40:35the
- 40:35brachytherapy
- 40:36plaque
- 40:36in place,
- 40:37a muscle
- 40:38may have
- 40:38to be
- 40:38moved
- 40:39in order
- 40:39to
- 40:40to
- 40:40position
- 40:41the plaque.
- 40:42I find
- 40:42that when
- 40:43muscles
- 40:44are are
- 40:45on the eye,
- 40:45it really
- 40:46helps
- 40:46exposure
- 40:46because
- 40:47you need
- 40:47the muscles
- 40:47to
- 40:48turn the eye.
- 40:48If
- 40:48the eye
- 40:49has no
- 40:49muscles,
- 40:50no.
- 40:50You're
- 40:51just kind
- 40:51of like
- 40:52it's hard
- 40:52to just
- 40:52move the
- 40:53eyeball,
- 40:54but
- 40:54the muscles
- 40:54are kind
- 40:55of like
- 40:55the
- 40:55handles
- 40:56that help
- 40:56help
- 40:57expose
- 40:57and
- 40:58allow,
- 40:58tissues
- 40:58in place.
- 40:59But
- 40:59when we
- 40:59do have
- 41:00to move
- 41:00a muscle,
- 41:01usually
- 41:01if
- 41:01I have
- 41:01to.
- 41:02So right
- 41:03where
- 41:03the muscle is
- 41:04and I
- 41:04can't get
- 41:05the
- 41:05plaque
- 41:05there,
- 41:05well,
- 41:06what
- 41:06what happens
- 41:07is
- 41:08we will
- 41:09the
- 41:09muscle,
- 41:09each
- 41:10muscle of
- 41:10each of
- 41:11the four
- 41:11eye
- 41:11muscles
- 41:12is
- 41:12attached
- 41:13sort of
- 41:14at what
- 41:15we
- 41:15call the
- 41:15equator
- 41:16of
- 41:16the eye,
- 41:16which is
- 41:17just past
- 41:17where you
- 41:17can
- 41:18see it
- 41:18in the
- 41:18eye
- 41:19socket.
- 41:19And so
- 41:20you find
- 41:20where
- 41:20it's
- 41:21attached.
- 41:22And you,
- 41:22you use
- 41:23scissors
- 41:24to snip
- 41:24the attachment,
- 41:25but
- 41:26you sew
- 41:26the ends
- 41:27in place.
- 41:29And
- 41:29there's
- 41:29many
- 41:30different
- 41:30ways to
- 41:30sort of
- 41:31what
- 41:31we call
- 41:31this
- 41:31insert
- 41:32the muscle
- 41:32and then
- 41:33put it
- 41:33back.
- 41:34I like to
- 41:34use
- 41:34leave
- 41:35this
- 41:35the stitch
- 41:36in place.
- 41:36So
- 41:36when the plant
- 41:37comes out
- 41:37I just
- 41:38pull it
- 41:38back
- 41:39and tie
- 41:39it
- 41:40and then
- 41:40the muscles
- 41:40back in
- 41:41its place.
- 41:42and what
- 41:43we found,
- 41:43and
- 41:44certainly
- 41:45when we
- 41:45looked
- 41:45at our
- 41:46cases,
- 41:46who
- 41:47the people
- 41:47need
- 41:47to have
- 41:48the
- 41:48muscles
- 41:48moved
- 41:48just
- 41:49inserting
- 41:49a muscle
- 41:49does
- 41:50not have
- 41:50any
- 41:51effect
- 41:51on double
- 41:52vision
- 41:52afterwards.
- 41:53double
- 41:54vision
- 41:54is more
- 41:55sort of
- 41:55the
- 41:55location
- 41:56of
- 41:56the tumor,
- 41:57you know,
- 41:57the
- 41:58usually
- 41:58more in
- 41:58the
- 41:59posterior
- 41:59or part
- 41:59of the eye.
- 42:00But
- 42:00if you have
- 42:00to have
- 42:01a muscle
- 42:01removed,
- 42:02it's a
- 42:02very,
- 42:03very safe
- 42:03type of a
- 42:05procedure
- 42:05to help
- 42:05facilitate
- 42:06the
- 42:06surgery.
- 42:08You
- 42:09mentioned
- 42:09putting
- 42:09markers
- 42:10in the eye.
- 42:10What does that
- 42:11mean?
- 42:12Like,
- 42:12are you actually
- 42:12putting
- 42:13something
- 42:13in or
- 42:13are you
- 42:14just
- 42:14marking?
- 42:14Do
- 42:14you have to
- 42:15take it
- 42:15back out?
- 42:15What is that.
- 42:16Oh yes.
- 42:17Thank you,
- 42:17thank you
- 42:18I didn't
- 42:19I didn't
- 42:19speak
- 42:19completely,
- 42:20but so
- 42:20we use
- 42:21for
- 42:22proton
- 42:22beam
- 42:22markers.
- 42:23We
- 42:23actually.
- 42:24So what's
- 42:25called
- 42:25tantalum
- 42:26clips
- 42:27and
- 42:27they're
- 42:27like
- 42:28little metal.
- 42:30they're like
- 42:30metal,
- 42:30like like
- 42:31a, it's
- 42:32like a
- 42:32little fasten
- 42:33these little
- 42:34fasteners,
- 42:34like into
- 42:34the
- 42:35back of
- 42:35the dress
- 42:35has that
- 42:36extra
- 42:36fastening.
- 42:37It's a
- 42:37little
- 42:38tiny
- 42:38thing
- 42:38like that
- 42:38that you
- 42:39sew on
- 42:39with
- 42:39like a
- 42:40circle
- 42:40in it
- 42:40that
- 42:41you can
- 42:41you, you
- 42:42just
- 42:42so it
- 42:43in place
- 42:43and
- 42:44they are
- 42:45they're
- 42:45radio
- 42:46opaque
- 42:46so
- 42:47that when
- 42:47you are
- 42:48doing
- 42:48the proton
- 42:49beam
- 42:49treatment,
- 42:49you can actually
- 42:50see the
- 42:51markers
- 42:51and you
- 42:52line up
- 42:52the markers
- 42:53to line
- 42:54up the,
- 42:54the proton
- 42:54beam,
- 42:55and
- 42:55that's
- 42:55what.
- 42:56And they
- 42:56stay there
- 42:56forever.
- 42:57Actually,
- 42:58they never have
- 42:58to
- 42:58come out.
- 42:59They will never
- 43:01set off.
- 43:02like
- 43:02you can
- 43:02have
- 43:02an MRI.
- 43:03They're
- 43:04not
- 43:04they
- 43:04don't
- 43:04cause
- 43:04trouble.
- 43:05They don't
- 43:05set off
- 43:05metal
- 43:06detectors.
- 43:07and
- 43:07it's just
- 43:08the
- 43:08nature
- 43:08of that.
- 43:09It's
- 43:09called
- 43:09tantalum.
- 43:10and
- 43:11that's
- 43:11sort
- 43:11of the
- 43:12from what
- 43:12I
- 43:12understand,
- 43:12is the
- 43:13universal
- 43:13material
- 43:14that is
- 43:14used
- 43:15to, to,
- 43:15to
- 43:16the markers
- 43:16to place,
- 43:17to
- 43:17put them
- 43:18in place.
- 43:18And
- 43:18you don't
- 43:19feel them
- 43:19afterwards.
- 43:20They
- 43:21I don't think
- 43:22I've ever
- 43:22heard
- 43:22of a case
- 43:23of them
- 43:23coming
- 43:23out, but
- 43:24it's
- 43:24possible
- 43:24that they
- 43:26could
- 43:26just work
- 43:26themselves
- 43:27out.
- 43:27But
- 43:27but
- 43:27that's
- 43:27very
- 43:28uncommon.
- 43:29And
- 43:29they don't
- 43:29change,
- 43:30you know,
- 43:30they're
- 43:31very
- 43:31they're
- 43:31inert.
- 43:32I mean,
- 43:32so they
- 43:32don't
- 43:33really
- 43:33cause
- 43:33any
- 43:34trouble
- 43:34or
- 43:34additional
- 43:35inflammation
- 43:35of
- 43:36the eye.
- 43:36and
- 43:37the eye
- 43:37can have
- 43:37ultrasounds
- 43:38and
- 43:39really
- 43:39just,
- 43:41you know,
- 43:41very,
- 43:41very
- 43:42unusual
- 43:43activity
- 43:43afterwards.
- 43:44and we,
- 43:45we leave
- 43:45those
- 43:45markers
- 43:46or we
- 43:46call them
- 43:47clips.
- 43:48They
- 43:48stay in
- 43:48place
- 43:49forever.
- 43:51And
- 43:51you won't
- 43:51set off
- 43:52a metal
- 43:52detector
- 43:53or
- 43:53anything
- 43:53like
- 43:53that.
- 43:54You
- 43:54just go
- 43:54about
- 43:55your
- 43:55life.
- 43:56Yeah.
- 43:56But
- 43:56I should
- 43:56mention
- 43:57you
- 43:57mentioned
- 43:57metal
- 43:57detectors
- 43:58and going
- 43:59through
- 43:59like TSA.
- 44:00So
- 44:01if you happen
- 44:01to have
- 44:02a
- 44:02radioactive
- 44:03plaque
- 44:03on
- 44:03your eye,
- 44:04you will set
- 44:05off TSA
- 44:07alarms.
- 44:08So
- 44:08if you're
- 44:08planning
- 44:09to fly
- 44:10in that
- 44:10period
- 44:11where
- 44:11the plaque
- 44:12is on
- 44:12the eye,
- 44:13make sure
- 44:13you have
- 44:13a letter
- 44:14from
- 44:15maybe
- 44:15the physicist
- 44:16or
- 44:17somebody.
- 44:18Make sure
- 44:18you have a letter
- 44:19because
- 44:20it will.
- 44:20So we've
- 44:21we have
- 44:21we have
- 44:22patients
- 44:22who fly
- 44:22in,
- 44:23who are
- 44:23not
- 44:24who are
- 44:24not too far.
- 44:24So
- 44:24they'll fly
- 44:25like from
- 44:25Nevada,
- 44:26they'll
- 44:26fly
- 44:26back home
- 44:27or
- 44:27they'll
- 44:27drive.
- 44:28And we
- 44:28recommend
- 44:29most
- 44:29people
- 44:29stay
- 44:30nearby
- 44:30just for
- 44:30that
- 44:31time,
- 44:31just
- 44:31because
- 44:31most
- 44:32people
- 44:32don't
- 44:32want
- 44:32to be
- 44:32far away.
- 44:33But
- 44:34occasionally
- 44:34we've had
- 44:35that
- 44:35happen
- 44:35in the past.
- 44:35So just
- 44:37be aware,
- 44:37be
- 44:38prepared
- 44:39so that
- 44:39you can
- 44:39make your
- 44:40flight.
- 44:41So
- 44:43I'm sure
- 44:43this is
- 44:44a very
- 44:44complicated
- 44:45question,
- 44:45so feel
- 44:46free to,
- 44:46make this
- 44:47as simple
- 44:48as you
- 44:48need to,
- 44:49but
- 44:49how do you decide
- 44:50which one
- 44:51to do?
- 44:52How do you know
- 44:53what's
- 44:53the best
- 44:53choice
- 44:54for
- 44:54somebody,
- 44:55or do
- 44:55they
- 44:55have a
- 44:55say
- 44:56in it?
- 44:56Or do
- 44:56you
- 44:57kind of
- 44:57tell them
- 44:57which
- 44:58one?
- 44:59Yeah,
- 45:00that's a
- 45:00great
- 45:00question.
- 45:03and the
- 45:03truth is.
- 45:06What's
- 45:07the
- 45:07treatment
- 45:08that it's
- 45:08going
- 45:08to be
- 45:08recommended
- 45:09to
- 45:09a patient
- 45:10is going
- 45:11to be
- 45:11what the
- 45:11center
- 45:12does.
- 45:13And
- 45:14there's
- 45:15I can
- 45:15think of
- 45:16one
- 45:16center
- 45:17that does
- 45:18maybe
- 45:18two centers
- 45:19that do
- 45:20both
- 45:20brachytherapy
- 45:21and
- 45:21proton
- 45:22beam,
- 45:23but most
- 45:24places do
- 45:25exclusively
- 45:25one or
- 45:26the
- 45:26other.
- 45:27And
- 45:30so it's,
- 45:32you know,
- 45:33many times
- 45:34people
- 45:34find
- 45:34themselves
- 45:35referred
- 45:35to a
- 45:36particular
- 45:36oncologist
- 45:37and
- 45:38they have
- 45:39what's
- 45:39recommended.
- 45:40But also
- 45:41we have
- 45:41people
- 45:42who have
- 45:43have
- 45:43researched
- 45:44each of
- 45:44the centers
- 45:45all
- 45:46across
- 45:46the country
- 45:47and are
- 45:48visiting
- 45:48proton
- 45:48beam
- 45:49centers
- 45:49and,
- 45:50talking
- 45:50to
- 45:51people.
- 45:51So,
- 45:52you know,
- 45:53it's
- 45:54it really
- 45:55it's
- 45:55variable.
- 45:56And, and,
- 45:56you know,
- 45:56I have
- 45:57my own
- 45:57view
- 45:58of it.
- 45:58You know,
- 45:58I did
- 45:59all my prior
- 46:00to coming
- 46:01to UCLA,
- 46:01I did
- 46:01all my
- 46:02training
- 46:02at, at
- 46:03it,
- 46:03Massachusetts
- 46:04and
- 46:04for me
- 46:05and all
- 46:05they had
- 46:05was
- 46:05proton
- 46:06beam.
- 46:06And I
- 46:06was
- 46:07convinced
- 46:07that
- 46:08we had
- 46:08to get
- 46:08a proton,
- 46:09particle
- 46:10accelerator
- 46:10at UCLA
- 46:11in
- 46:11order to
- 46:11treat
- 46:12melanoma
- 46:12because I
- 46:13was so
- 46:13impressed
- 46:14with the
- 46:14results
- 46:15and etc..
- 46:15Then when
- 46:16I came
- 46:16to UCLA,
- 46:17which
- 46:18had been
- 46:18doing
- 46:19break
- 46:19therapy
- 46:19plaques
- 46:20for long
- 46:20before
- 46:21I was
- 46:21there, I,
- 46:22I was
- 46:23okay,
- 46:24fine,
- 46:24we'll do plaques,
- 46:25but
- 46:25we need
- 46:25to think
- 46:26about
- 46:26getting a
- 46:26proton
- 46:27beam
- 46:27set up
- 46:28at some
- 46:28point.
- 46:28So.
- 46:29But
- 46:29after a
- 46:29while,
- 46:30I was
- 46:30just
- 46:31I mean,
- 46:31I love
- 46:32doing
- 46:32brachytherapy
- 46:33plaques
- 46:33and
- 46:34I've been do
- 46:34it.
- 46:34And
- 46:34I just
- 46:34think
- 46:35it's
- 46:35such an
- 46:36elegant
- 46:36way and a
- 46:37relatively
- 46:38simple
- 46:38way
- 46:39to treat
- 46:39the problem.
- 46:40You know,
- 46:41we don't
- 46:41need a
- 46:41big
- 46:42particle
- 46:42accelerator
- 46:43and,
- 46:44you know,
- 46:44the
- 46:45the side
- 46:45effect
- 46:45profiles
- 46:46are
- 46:46slightly
- 46:47different
- 46:47between
- 46:48the two
- 46:49treatments.
- 46:51in
- 46:51addition,
- 46:52I think
- 46:52it's,
- 46:53you know,
- 46:53you
- 46:53really want it
- 46:54the
- 46:54most important
- 46:55thing
- 46:55that
- 46:55matters
- 46:55at number
- 46:56one is
- 46:57accuracy.
- 46:58So,
- 46:59you know,
- 47:00if you
- 47:00look at
- 47:00there's
- 47:01not a lot
- 47:01of proton
- 47:01beam
- 47:02centers,
- 47:02actually,
- 47:03the vast
- 47:03majority
- 47:04of places
- 47:05that
- 47:05treat
- 47:06ocular
- 47:06melanoma
- 47:07do
- 47:07brachytherapy
- 47:08plaque.
- 47:08So among
- 47:10the few
- 47:10centers
- 47:11that do
- 47:11proton
- 47:12beam,
- 47:12what I've
- 47:13learned
- 47:13from my
- 47:13colleagues
- 47:14who do
- 47:14proton
- 47:15beam and
- 47:15also
- 47:15colleagues
- 47:16in
- 47:16Europe,
- 47:16there's
- 47:17some
- 47:17centers,
- 47:17major
- 47:18centers
- 47:18there
- 47:18that the
- 47:18proton
- 47:19beam is
- 47:19that each
- 47:20proton
- 47:20beam
- 47:21is
- 47:22different.
- 47:22It's
- 47:22collimated
- 47:23differently.
- 47:24It's
- 47:24it's it's
- 47:25calibrated
- 47:26differently.
- 47:26And so
- 47:27they
- 47:27all work
- 47:28a little
- 47:28differently
- 47:29there.
- 47:30Not as
- 47:30I think
- 47:31that
- 47:31there's
- 47:31less
- 47:31uniformity.
- 47:33So
- 47:33it's important
- 47:34if you're
- 47:35going to
- 47:35a proton
- 47:35beam
- 47:36center
- 47:36that you
- 47:37you know,
- 47:38you
- 47:38can ask
- 47:39about
- 47:39what are the,
- 47:39you know,
- 47:40what
- 47:40are the
- 47:40success
- 47:41rates,
- 47:41what is
- 47:41your
- 47:41recurrence
- 47:42rate,
- 47:42and all
- 47:43sorts
- 47:43of
- 47:43things.
- 47:44I mean,
- 47:44these are
- 47:44things
- 47:44people
- 47:45should
- 47:45ask
- 47:45anybody,
- 47:46actually,
- 47:46you know,
- 47:47what is
- 47:47what is
- 47:48not just
- 47:48what
- 47:48is the
- 47:49success
- 47:49rate,
- 47:49but
- 47:49what is
- 47:50your
- 47:50success
- 47:51rate,
- 47:51because
- 47:52every
- 47:52center
- 47:53has a
- 47:53different
- 47:53rate of
- 47:54success
- 47:55and
- 47:55different
- 47:56it's all
- 47:57there's
- 47:57so many
- 47:58details
- 47:58that go
- 47:58into this.
- 47:59But with
- 48:00proton
- 48:00beam
- 48:00there's
- 48:01a little
- 48:01more
- 48:01variability.
- 48:02And
- 48:03I think
- 48:03a center
- 48:04when
- 48:04I'm very
- 48:05biased to
- 48:05my old
- 48:05training
- 48:06center.
- 48:06But
- 48:07the Northeast
- 48:08proton
- 48:08proton
- 48:08beam
- 48:08center,
- 48:09they had
- 48:09phenomenal,
- 48:11success
- 48:11rates.
- 48:12They and
- 48:12they were
- 48:13very good
- 48:13about
- 48:14publishing.
- 48:14They published
- 48:15their data
- 48:16over many
- 48:16decades,
- 48:17like
- 48:17starting
- 48:18in
- 48:18the 70s.
- 48:19their success
- 48:19rate is
- 48:20very high
- 48:20for
- 48:21their treatment.
- 48:22And so
- 48:22they have
- 48:23a
- 48:23they have
- 48:23a
- 48:23well-oiled
- 48:24machine.
- 48:24I think
- 48:24there's
- 48:25other places
- 48:25that are
- 48:26sort of
- 48:26thinking
- 48:27of
- 48:27introducing
- 48:27proton
- 48:28beam,
- 48:28but
- 48:29you know,
- 48:29there's
- 48:30it's
- 48:30it's all
- 48:31there's
- 48:31so
- 48:31many details.
- 48:32So
- 48:32I think,
- 48:33you know,
- 48:34part of
- 48:35when you
- 48:36get
- 48:36diagnosed
- 48:37and
- 48:37you're
- 48:37learning
- 48:38about
- 48:38the
- 48:38treatment
- 48:38options
- 48:39is you,
- 48:40first
- 48:40of all,
- 48:40talk to
- 48:41the person
- 48:41that
- 48:41you're in
- 48:42front
- 48:42of first
- 48:43and
- 48:43find out
- 48:43know
- 48:44what?
- 48:44What
- 48:45do they
- 48:45do.
- 48:45What are the
- 48:46recommended.
- 48:46What do they
- 48:47think
- 48:47about
- 48:47proton
- 48:48beam.
- 48:48And you
- 48:49know,
- 48:49if
- 48:49it was their
- 48:50eye, what
- 48:50would
- 48:50they want
- 48:51to be
- 48:51doing.
- 48:53I know
- 48:53for using
- 48:54brachytherapy,
- 48:55brachytherapy
- 48:56allows
- 48:57you know,
- 48:57at UCLA
- 48:58we also
- 48:59do
- 49:00we place
- 49:01silicone
- 49:01oil
- 49:02in the eye,
- 49:02which
- 49:02helps
- 49:03minimize
- 49:03the radiation
- 49:04to
- 49:04healthy
- 49:04tissues.
- 49:05And
- 49:05that can
- 49:05only
- 49:06be done
- 49:06with
- 49:06brachytherapy
- 49:07because
- 49:07you can't
- 49:08if you
- 49:08put
- 49:08silicone
- 49:09oil
- 49:09in the eye
- 49:09and do
- 49:10proton
- 49:10you're
- 49:10going to
- 49:11you'll
- 49:11affect
- 49:12the radiation
- 49:13going to
- 49:13the tumor.
- 49:13So
- 49:14you can't
- 49:14it
- 49:14doesn't
- 49:14really
- 49:15work
- 49:15with
- 49:16proton
- 49:16proton
- 49:17beam
- 49:17therapy
- 49:18has
- 49:18you know
- 49:19it's been
- 49:19published.
- 49:20But
- 49:20there's a
- 49:20lot of it
- 49:20is a
- 49:21little
- 49:21bit more
- 49:22eyelid
- 49:22eyelash
- 49:23issues,
- 49:24more
- 49:24dry eye
- 49:26with
- 49:26proton
- 49:26therapy.
- 49:27But
- 49:27again,
- 49:28you need
- 49:28to ask
- 49:29the center
- 49:30that
- 49:30you're at
- 49:30and
- 49:30find out.
- 49:31You know
- 49:31what
- 49:32what is
- 49:33what what
- 49:33what
- 49:34should I
- 49:34expect.
- 49:35I know
- 49:36there's a
- 49:36proton
- 49:37beam
- 49:38center on
- 49:38in
- 49:39Northern
- 49:39California
- 49:40that
- 49:40a lot
- 49:41of
- 49:41different
- 49:41people
- 49:41use.
- 49:42and so
- 49:44it's it
- 49:44all
- 49:44depends
- 49:45on
- 49:45the details.
- 49:46So
- 49:47I think
- 49:49most of
- 49:49the time
- 49:49we don't
- 49:49have
- 49:50a choice
- 49:50like you
- 49:50asked,
- 49:50like,
- 49:51should
- 49:51can I
- 49:51choose
- 49:52what
- 49:52I want?
- 49:53I think
- 49:53in
- 49:54order to
- 49:54make
- 49:54a choice,
- 49:54you have to
- 49:55understand,
- 49:56what would be
- 49:57the pros
- 49:57and cons?
- 49:58You know,
- 49:58what
- 49:58would be
- 49:59the
- 49:59advantage
- 49:59of doing
- 50:00this
- 50:00versus
- 50:00that?
- 50:01Or,
- 50:01you know,
- 50:02should I
- 50:02go to
- 50:02a center
- 50:03that has a
- 50:03proton
- 50:03beam?
- 50:04Should I
- 50:04go to
- 50:05a center
- 50:05where
- 50:06there's
- 50:06brachytherapy?
- 50:07And I
- 50:07think
- 50:07the most
- 50:08important
- 50:08thing is
- 50:09to to go
- 50:09to a
- 50:10place
- 50:10that has
- 50:10a lot
- 50:11of
- 50:11experience
- 50:12in what
- 50:12they do
- 50:12and
- 50:13have them
- 50:13share
- 50:14what they
- 50:15what
- 50:15their
- 50:15experiences,
- 50:16and
- 50:16then ask
- 50:17what
- 50:17the opinion
- 50:18is of
- 50:18your
- 50:18doctor.
- 50:20You know,
- 50:20I feel like
- 50:21so
- 50:22many of
- 50:22my
- 50:22colleagues
- 50:23do
- 50:23slightly
- 50:23different
- 50:24things,
- 50:24but,
- 50:25you know,
- 50:25they're
- 50:25all
- 50:25very excellent.
- 50:27and you
- 50:27just
- 50:28want to
- 50:28do
- 50:29the due
- 50:29diligence,
- 50:30do your
- 50:30research,
- 50:31find out
- 50:31what is
- 50:32done.
- 50:33I think
- 50:34brachytherapy
- 50:34is pretty
- 50:35tried
- 50:36and true.
- 50:36It
- 50:37you know,
- 50:37as I said
- 50:37before,
- 50:38all
- 50:38radiation
- 50:39treatment
- 50:39will kill
- 50:39the
- 50:40melanoma.
- 50:41and
- 50:41if you
- 50:42are in
- 50:42a position
- 50:43to get
- 50:43a few
- 50:43opinions
- 50:44or to
- 50:44choose,
- 50:45then
- 50:46definitely
- 50:47do
- 50:47the
- 50:47research,
- 50:48learn
- 50:48about
- 50:49as
- 50:49much as
- 50:49you can,
- 50:49understand
- 50:50the differences.
- 50:51and,
- 50:51and
- 50:52if it's,
- 50:52there's
- 50:52a lot of
- 50:53there
- 50:53is
- 50:53literature
- 50:53kind of
- 50:54loosely
- 50:54comparing
- 50:55the two
- 50:56types of
- 50:57treatment
- 50:57and the
- 50:58comparisons
- 50:58that we
- 50:59published
- 50:59paper
- 50:59about a
- 51:00long
- 51:00time ago,
- 51:01looking
- 51:01at all
- 51:01the data
- 51:03in the
- 51:03two
- 51:03treatments,
- 51:03and
- 51:04they worked
- 51:05similarly.
- 51:06But, you
- 51:07really
- 51:07have
- 51:07to ask,
- 51:08ask very
- 51:09pointed
- 51:10questions
- 51:10to your,
- 51:11your
- 51:11oncologist.
- 51:12you know,
- 51:12there's
- 51:13there's
- 51:13no such
- 51:13thing
- 51:13as
- 51:13the only
- 51:14way to
- 51:14treat
- 51:14something.
- 51:15And even
- 51:16among
- 51:17centers
- 51:17that use
- 51:18the same
- 51:18treatments,
- 51:19there
- 51:19can be
- 51:19differences.
- 51:20so it's
- 51:21a hard
- 51:21question
- 51:21to answer.
- 51:22and
- 51:23but
- 51:24I think
- 51:24that,
- 51:24you know,
- 51:25the most
- 51:25important
- 51:25thing is
- 51:25you
- 51:25get the
- 51:26treatment
- 51:26and,
- 51:27you know,
- 51:28when you're
- 51:28getting
- 51:29opinions
- 51:29and doing
- 51:30research
- 51:30and
- 51:30learning
- 51:30about
- 51:31all the
- 51:31different
- 51:31options,
- 51:32I always
- 51:33hope
- 51:33people
- 51:33don't
- 51:33take
- 51:33too much
- 51:34time,
- 51:34really
- 51:35do it
- 51:35in as
- 51:36quick
- 51:36a time
- 51:36as you
- 51:36can, and
- 51:37then ask
- 51:38your
- 51:38ocular
- 51:39oncologist
- 51:39to help
- 51:41give you
- 51:41suggestions
- 51:41for
- 51:42second
- 51:42opinions.
- 51:43You know
- 51:43you
- 51:43don't.
- 51:44Don't
- 51:44feel bad
- 51:44about
- 51:45saying,
- 51:45well,
- 51:45can you
- 51:46give me 2
- 51:47or 3
- 51:47people
- 51:48that
- 51:48I could
- 51:48see a
- 51:49second
- 51:49opinion
- 51:49so that
- 51:49I can
- 51:50learn
- 51:50more
- 51:50information
- 51:51about my
- 51:52choices?
- 51:53That's my
- 51:53it
- 51:54would be
- 51:54my
- 51:54advice.
- 51:55I think
- 51:56that's
- 51:56really
- 51:56smart
- 51:56too.
- 51:57And don't
- 51:57be afraid
- 51:57of
- 51:58offending
- 51:58your doctor.
- 51:59Like
- 51:59I think
- 51:59that they
- 52:00want you
- 52:00to get
- 52:00second
- 52:01opinions
- 52:01and
- 52:01that's
- 52:01such a
- 52:02normal thing.
- 52:03But
- 52:03people
- 52:04are
- 52:04sometimes
- 52:04maybe
- 52:05a little
- 52:05afraid
- 52:05too,
- 52:06because
- 52:06they
- 52:06don't
- 52:06want to
- 52:06offend
- 52:07anybody.
- 52:07And I
- 52:07think you
- 52:07need to,
- 52:08put you
- 52:09first
- 52:10and
- 52:10understand
- 52:10that
- 52:10they know
- 52:11you're
- 52:11going
- 52:11to get
- 52:11a second
- 52:12opinion
- 52:12and
- 52:12they're
- 52:12not
- 52:12going to
- 52:13be offended.
- 52:13And
- 52:13if they are
- 52:14offended,
- 52:15maybe
- 52:15that's
- 52:15not
- 52:15even the
- 52:15right
- 52:16doctor
- 52:16for you.
- 52:17I 100%
- 52:18agree.
- 52:19You know,
- 52:20when
- 52:20if your doctor
- 52:21looks at
- 52:21you like,
- 52:21why would you
- 52:22get a
- 52:22second
- 52:22opinion
- 52:23after
- 52:23I've
- 52:23told you
- 52:24what I
- 52:24said?
- 52:24I mean,
- 52:25you know,
- 52:25that's
- 52:26not
- 52:26that's
- 52:26not the
- 52:27attitude
- 52:27of
- 52:27someone.
- 52:28You
- 52:28aren't
- 52:28looking
- 52:28after
- 52:29you.
- 52:29I'm
- 52:29just
- 52:29going to
- 52:29say it.
- 52:30If you
- 52:30if
- 52:30someone
- 52:31gives you
- 52:31a hard
- 52:31time with
- 52:31second
- 52:32opinion,
- 52:32then
- 52:32then just
- 52:33turn around
- 52:34and and
- 52:35find
- 52:35somebody
- 52:35else,
- 52:36you know,
- 52:36you need
- 52:37someone
- 52:37who's
- 52:37going
- 52:37to be
- 52:38responsive
- 52:39and
- 52:39understand
- 52:39while you
- 52:40have
- 52:40questions
- 52:40and
- 52:41someone
- 52:41who
- 52:41you can
- 52:41work
- 52:42with,
- 52:42you know,
- 52:42and this
- 52:43I mean,
- 52:44today,
- 52:45you know,
- 52:45maybe 25
- 52:46years
- 52:46ago, no
- 52:47one would
- 52:48we don't
- 52:48we
- 52:48wouldn't
- 52:48really
- 52:49know
- 52:49how to
- 52:50get the
- 52:50knowledge.
- 52:51But
- 52:51there's
- 52:51so much
- 52:52good
- 52:52information
- 52:53available
- 52:53now
- 52:54that it
- 52:54it really
- 52:55is.
- 52:55The
- 52:55responsibility
- 52:57is on
- 52:57patients
- 52:58to, you
- 52:59know,
- 52:59question
- 52:59their
- 52:59doctors,
- 53:00you know,
- 53:00don't
- 53:01you don't
- 53:01have
- 53:01to go to
- 53:02the person
- 53:02that
- 53:02you were
- 53:03referred
- 53:03to.
- 53:03You can
- 53:04maybe
- 53:05there's
- 53:05two
- 53:05maybe
- 53:06there's
- 53:06I mean
- 53:06there's
- 53:06not a
- 53:07lot of
- 53:07in
- 53:07colleges,
- 53:08but
- 53:08you know,
- 53:09sometimes
- 53:10you might
- 53:10have
- 53:10options,
- 53:11maybe
- 53:11there's
- 53:11like one
- 53:12or 2 or
- 53:133 people
- 53:14in an area
- 53:14that you can
- 53:15you can go
- 53:16to.
- 53:16And,
- 53:17you know,
- 53:17even if
- 53:17you like
- 53:18your
- 53:18oncologist,
- 53:19and
- 53:19you're
- 53:19thinking
- 53:20you're
- 53:20probably
- 53:20going
- 53:20to go
- 53:20there,
- 53:21you can
- 53:21certainly
- 53:22get a
- 53:22second
- 53:22opinion
- 53:23just to
- 53:23learn
- 53:24more.
- 53:24You know,
- 53:25there's
- 53:25you can
- 53:25get a
- 53:26second
- 53:26opinion
- 53:26and just
- 53:27decide
- 53:27to stick
- 53:27with the
- 53:28first
- 53:28one.
- 53:28It's not,
- 53:29like
- 53:29you're
- 53:30leaving
- 53:30your
- 53:30doctor.
- 53:31And
- 53:32again,
- 53:33you know,
- 53:33doctors,
- 53:34I think
- 53:34we're,
- 53:34we're
- 53:35becoming
- 53:35better
- 53:36at,
- 53:37make it
- 53:37making
- 53:38this
- 53:38normal,
- 53:38getting
- 53:38and
- 53:39second
- 53:39opinions.
- 53:39But
- 53:39I do
- 53:40know it's
- 53:40still
- 53:40hard for
- 53:41some
- 53:42physicians
- 53:43to be
- 53:43open
- 53:44minded
- 53:44about
- 53:44second
- 53:44opinions,
- 53:45but
- 53:46that's
- 53:46just
- 53:46too bad.
- 53:47They're going
- 53:47to have
- 53:47to learn,
- 53:48because,
- 53:48you know,
- 53:49people,
- 53:49it's all
- 53:50about
- 53:50the patient
- 53:51and what
- 53:51what
- 53:51you feel
- 53:52comfortable
- 53:52about.
- 53:53and
- 53:53really
- 53:53just,
- 53:54you know,
- 53:54even if
- 53:55you
- 53:55belong to
- 53:55a health
- 53:56plan
- 53:56that
- 53:56might
- 53:56have a
- 53:57limited,
- 53:58you know,
- 53:58limited
- 53:58options,
- 53:59oftentimes
- 53:59you can
- 54:00ask for a
- 54:00second
- 54:00opinion,
- 54:01even
- 54:02if you
- 54:02have
- 54:02health
- 54:03insurance
- 54:03where
- 54:04only a
- 54:04particular
- 54:05acute
- 54:05oncologist
- 54:06is
- 54:06in-network.
- 54:07So ask,
- 54:07you know,
- 54:08the worst
- 54:08that can happen
- 54:08is they
- 54:09can just
- 54:09say no.
- 54:09But,
- 54:10you know,
- 54:10you
- 54:10definitely
- 54:11can ask
- 54:11and say,
- 54:11this is a
- 54:12rare
- 54:12condition.
- 54:13Few
- 54:13people
- 54:14have
- 54:14experience
- 54:14in this.
- 54:15And
- 54:15I feel
- 54:16that
- 54:16because I
- 54:16have this
- 54:17rare
- 54:17condition,
- 54:18I need
- 54:18to be
- 54:19approved
- 54:19for a
- 54:20second
- 54:20opinion
- 54:20at
- 54:20center X.
- 54:23So
- 54:23definitely,
- 54:23definitely
- 54:24you have
- 54:25a lot of
- 54:26leeway,
- 54:26especially
- 54:27if
- 54:27this is
- 54:27rare.
- 54:27If
- 54:28this is
- 54:28extremely
- 54:28common,
- 54:29then
- 54:30I mean
- 54:30there's
- 54:30usually
- 54:30a lot
- 54:31more
- 54:31providers,
- 54:31but
- 54:32this is
- 54:32a case
- 54:33where
- 54:33in ocular
- 54:33melanoma.
- 54:34it's very
- 54:35important
- 54:35to get a
- 54:36second
- 54:36opinion.
- 54:37And
- 54:37you need
- 54:38to be
- 54:38with
- 54:38a provider
- 54:39that
- 54:39you trust
- 54:40and that
- 54:41relationship
- 54:41is important.
- 54:42So even
- 54:42if you
- 54:44think
- 54:44that you
- 54:45do agree
- 54:45with what
- 54:45they're
- 54:45saying,
- 54:46if
- 54:46you don't
- 54:46feel that
- 54:47connection,
- 54:47it might
- 54:48be worth
- 54:48exploring
- 54:49another
- 54:49doctor
- 54:49just
- 54:49to find
- 54:50somebody
- 54:50that is
- 54:51a good
- 54:51fit
- 54:52for you.
- 54:54Yeah,
- 54:54I think
- 54:54that's
- 54:55that's
- 54:55so true.
- 54:56and,
- 54:57you know,
- 54:57you're
- 54:58you're
- 54:58you're
- 54:59never
- 54:59going
- 54:59to regret
- 55:00getting a
- 55:00second
- 55:00opinion
- 55:01unless
- 55:01you,
- 55:02you took
- 55:02a really
- 55:02long time
- 55:03to get
- 55:03it.
- 55:03And by
- 55:04really,
- 55:04you know,
- 55:04don't do
- 55:05all
- 55:05your opinions
- 55:07within
- 55:08like,
- 55:09you know,
- 55:10the
- 55:10majority
- 55:10of people
- 55:11have
- 55:11about 2
- 55:12to 4
- 55:12weeks.
- 55:13You know,
- 55:13this
- 55:13isn't
- 55:14something
- 55:14you have
- 55:14to treat
- 55:14tomorrow.
- 55:15You know,
- 55:16this is
- 55:16you know,
- 55:17there's
- 55:17usually
- 55:17time
- 55:18with
- 55:18ocular
- 55:18melanoma.
- 55:19you know,
- 55:20you don't
- 55:20want to
- 55:21leave it
- 55:21a long
- 55:21time.
- 55:22And
- 55:22obviously
- 55:23each case
- 55:23is a
- 55:23little
- 55:23different.
- 55:24Some people,
- 55:25we have to
- 55:25go faster
- 55:26than
- 55:26others.
- 55:26But the
- 55:27the
- 55:27majority
- 55:28of people,
- 55:28we have some leeway
- 55:30so that
- 55:30you don't
- 55:30have
- 55:30to rush
- 55:31into
- 55:31anything.
- 55:35Yeah,
- 55:35I think
- 55:35that's
- 55:35really
- 55:36important.
- 55:38let me think
- 55:38about
- 55:39other
- 55:39aspects
- 55:39of,
- 55:40treatment.
- 55:41you know,
- 55:41I think
- 55:42that one
- 55:43of the
- 55:43things
- 55:43that
- 55:43people
- 55:44often
- 55:44wonder
- 55:44about is,
- 55:45you know,
- 55:45after
- 55:46the
- 55:46treatment
- 55:47and
- 55:47having
- 55:47the
- 55:47surgery
- 55:48and
- 55:48procedures,
- 55:48people,
- 55:49want to know
- 55:50what
- 55:50is what
- 55:50what
- 55:51happens
- 55:51after,
- 55:52you know,
- 55:52what do
- 55:53I need
- 55:53to have
- 55:54more
- 55:54radiation?
- 55:55I'm going
- 55:55to have
- 55:56to have
- 55:56additional
- 55:57treatments.
- 55:57And,
- 55:58usually
- 55:59the answer
- 55:59is no.
- 56:00it's
- 56:00usually
- 56:01one
- 56:01surgery,
- 56:01one
- 56:02radiation.
- 56:03But it's
- 56:03important
- 56:04to still
- 56:05do
- 56:05the evaluations
- 56:07and still
- 56:08and I
- 56:08and I
- 56:09really
- 56:09strongly
- 56:10recommend
- 56:10that
- 56:11people
- 56:11stay with
- 56:12their
- 56:13oncologist
- 56:14if they can,
- 56:14because
- 56:15we have
- 56:15seen like
- 56:16nine
- 56:17years
- 56:17later,
- 56:18someone
- 56:18can start
- 56:19to get
- 56:19a recurrence.
- 56:20And
- 56:21if that wasn't
- 56:21if no one
- 56:22was
- 56:22really
- 56:22looking
- 56:23for that,
- 56:23it
- 56:23would have
- 56:24just
- 56:24sort of
- 56:25gone out
- 56:25of control.
- 56:26But
- 56:26if you
- 56:26find
- 56:26something
- 56:27like a
- 56:27recurrence
- 56:28later
- 56:29that's
- 56:29very
- 56:29small
- 56:30and very
- 56:30treatable
- 56:30in an eye
- 56:31with good
- 56:31vision,
- 56:32you can have
- 56:32a second
- 56:33brachytherapy
- 56:33plaque.
- 56:34You know,
- 56:34it's
- 56:35becoming
- 56:35pretty
- 56:35normal
- 56:36now.
- 56:36In
- 56:36the olden
- 56:37days,
- 56:37we used to say
- 56:38if
- 56:38the tumor
- 56:39comes
- 56:39back,
- 56:39we have to
- 56:39take out
- 56:40the eye.
- 56:40But,
- 56:41you know,
- 56:41we would
- 56:41we would
- 56:42take out
- 56:42the eye
- 56:42if
- 56:42there was
- 56:43really
- 56:43no vision.
- 56:44But
- 56:44if there's
- 56:45vision
- 56:46and it's
- 56:47very
- 56:47treatable,
- 56:47it's
- 56:48you know,
- 56:49what
- 56:49I've learned
- 56:49is that
- 56:50the eye
- 56:50does not
- 56:51completely
- 56:52fall
- 56:52apart
- 56:53with a
- 56:53second
- 56:53dose of
- 56:53radiation.
- 56:55there's
- 56:55this
- 56:55notion
- 56:56still.
- 56:56I mean,
- 56:56we were
- 56:56very
- 56:57first,
- 56:57the
- 56:57very first time
- 56:58I remember
- 56:59doing a
- 56:59second
- 56:59plaque.
- 57:00Our
- 57:00radiation
- 57:00team was
- 57:01like,
- 57:01well,
- 57:01we can't do
- 57:02that.
- 57:02We
- 57:02can't do
- 57:02that.
- 57:03I said,
- 57:03well,
- 57:03what's
- 57:03the option
- 57:04if
- 57:04we're going
- 57:05to take
- 57:05the eye
- 57:05out?
- 57:05So, so
- 57:06we, we
- 57:07everyone
- 57:07was very
- 57:07surprised
- 57:08that the
- 57:08patient
- 57:09tolerated
- 57:09it
- 57:09very well.
- 57:10And you
- 57:10know, now
- 57:11when
- 57:11we do see
- 57:12a
- 57:12recurrence
- 57:13and the
- 57:13vision is
- 57:13good,
- 57:13most
- 57:14people
- 57:14tend to
- 57:14maintain
- 57:15their vision.
- 57:17so
- 57:17a second
- 57:18dose of
- 57:18radiation
- 57:19is not a,
- 57:19not a,
- 57:20not
- 57:20necessarily
- 57:21a vision
- 57:22threatening
- 57:22sentence.
- 57:23So,
- 57:23you know,
- 57:24make
- 57:24sure
- 57:24you get
- 57:24followed
- 57:25up,
- 57:25make sure
- 57:25you
- 57:26monitor
- 57:26the eye.
- 57:27And the
- 57:28other
- 57:28thing to
- 57:29when
- 57:29you're
- 57:29getting
- 57:30these eye
- 57:30evaluations,
- 57:31not don't
- 57:32just
- 57:32focus
- 57:32on the
- 57:33melanoma
- 57:33eye, but
- 57:34make sure
- 57:35that the
- 57:35ophthalmologist
- 57:36is
- 57:36looking
- 57:36carefully
- 57:37at your
- 57:37other eye.
- 57:38Because
- 57:39there are
- 57:39so many
- 57:40conditions
- 57:41of
- 57:41the eye
- 57:41that
- 57:42are far
- 57:42more
- 57:42common
- 57:43than
- 57:43ocular
- 57:43melanoma,
- 57:44that are
- 57:45so
- 57:45treatable.
- 57:46And,
- 57:46you know,
- 57:47number
- 57:47one is
- 57:48cataract.
- 57:49and then
- 57:49glaucoma.
- 57:51And then
- 57:52there's
- 57:52a host
- 57:52of
- 57:52various
- 57:53retinal
- 57:53conditions.
- 57:54But even
- 57:55just
- 57:55because
- 57:56your eye
- 57:56had a
- 57:56melanoma
- 57:57in it,
- 57:57and
- 57:58if you
- 57:58develop
- 57:58like a
- 57:58macular
- 57:59hole or
- 57:59something
- 58:00different,
- 58:00you can have
- 58:01the treatment
- 58:02that
- 58:02everybody
- 58:02else has.
- 58:03sometimes
- 58:04I hear
- 58:05that, oh,
- 58:05this is a
- 58:06melanoma.
- 58:06I don't
- 58:07touch it.
- 58:07Just
- 58:07leave it.
- 58:08You know,
- 58:09it's been
- 58:09through a lot.
- 58:10But
- 58:10if there's
- 58:11a way
- 58:11to
- 58:11make your
- 58:11vision
- 58:12better
- 58:13because
- 58:13that
- 58:13eye has
- 58:14a condition
- 58:15that is more common
- 58:16and that
- 58:17we know
- 58:17how to
- 58:17fix, then
- 58:18you
- 58:18should
- 58:18definitely
- 58:19have
- 58:19surgery.
- 58:20And
- 58:20the other thing
- 58:21is,
- 58:21you know,
- 58:22cataract.
- 58:23I often
- 58:23find
- 58:24people
- 58:24saying,
- 58:24oh, my
- 58:25doctor
- 58:25told me
- 58:25that
- 58:26I should
- 58:26I should
- 58:26wait
- 58:27as long
- 58:27as
- 58:27possible
- 58:28to have
- 58:28cataract
- 58:28in
- 58:29my good
- 58:29eye
- 58:29and maybe
- 58:30never
- 58:30have
- 58:30cataract
- 58:31surgery,
- 58:31because
- 58:32what if
- 58:32something
- 58:32happened
- 58:33to my my,
- 58:35my good
- 58:35eye?
- 58:35And I
- 58:36just
- 58:36think
- 58:36that's
- 58:36just I
- 58:37would
- 58:37flip it
- 58:38around
- 58:38and say
- 58:38that in
- 58:39order
- 58:39for you
- 58:39to
- 58:39function
- 58:40well,
- 58:41if you do
- 58:41start
- 58:42to have
- 58:42a vision
- 58:43degrading
- 58:43condition
- 58:44that's
- 58:44happening
- 58:45in your
- 58:45better
- 58:45eye,
- 58:46you need
- 58:47to be
- 58:47all the
- 58:48more
- 58:48proactive
- 58:49about
- 58:49getting
- 58:49it
- 58:49treated
- 58:50so that
- 58:50you can
- 58:51continue
- 58:51and do
- 58:52well.
- 58:52Because,
- 58:53you know,
- 58:53this is
- 58:54cataract
- 58:54is such a
- 58:55such a
- 58:56common
- 58:56and
- 58:56highly
- 58:57successful
- 58:57surgery
- 58:59that to
- 58:59to
- 58:59sort of
- 59:00not to
- 59:00deny
- 59:01people
- 59:01to have
- 59:01cataract
- 59:02surgery.
- 59:03just
- 59:03because
- 59:04there
- 59:04might
- 59:04be a
- 59:04remote
- 59:05risk that
- 59:06something
- 59:06might
- 59:06happen,
- 59:07is
- 59:07just not
- 59:08appropriate,
- 59:09in
- 59:09my opinion.
- 59:09And
- 59:10I think
- 59:10it's
- 59:10really
- 59:10outdated
- 59:11to tell
- 59:11people
- 59:12not to
- 59:12have
- 59:12surgery
- 59:13that
- 59:13we're so
- 59:13good at,
- 59:15because
- 59:15of this
- 59:16rare
- 59:16risk.
- 59:17And,
- 59:17and I
- 59:17and I try
- 59:17to
- 59:18talk to
- 59:18people
- 59:18about it
- 59:18and
- 59:18those people
- 59:19who just
- 59:19don't
- 59:20want
- 59:20anything
- 59:20done.
- 59:20And,
- 59:21you know,
- 59:21it's
- 59:21completely
- 59:22your choice.
- 59:23But it's
- 59:24important
- 59:24not to
- 59:25necessarily
- 59:25have the
- 59:26fears
- 59:27of your
- 59:27ophthalmology
- 59:28team.
- 59:29You know,
- 59:30trickle
- 59:31on to you
- 59:31if it's
- 59:31going to
- 59:32be result
- 59:33in
- 59:33impaired
- 59:33vision.
- 59:34So
- 59:34there's
- 59:35a lot
- 59:35of things
- 59:35you can
- 59:35do.
- 59:36And the
- 59:36other
- 59:36thing
- 59:36I'd
- 59:37mention
- 59:37is
- 59:37glaucoma.
- 59:38there's about
- 59:398 to
- 59:409% of
- 59:41everybody
- 59:41who gets
- 59:42a break.
- 59:42A therapy
- 59:42plaque
- 59:43will
- 59:43develop a
- 59:43pressure
- 59:44issue
- 59:44with
- 59:44their eye
- 59:45down
- 59:45the road
- 59:46at
- 59:46some point.
- 59:47and I
- 59:47find that
- 59:47a lot
- 59:48of people
- 59:48are
- 59:48scared
- 59:49to treat
- 59:49glaucoma
- 59:50in a
- 59:50melanoma
- 59:51eye.
- 59:52And
- 59:52unfortunately,
- 59:53I've seen
- 59:53people
- 59:53who've
- 59:53lost
- 59:54a lot of vision
- 59:55because
- 59:55their
- 59:56ophthalmologist
- 59:56was too
- 59:57afraid
- 59:57to do
- 59:58the
- 59:58standard
- 59:58glaucoma
- 59:59surgeries
- 1:00:00in a
- 1:00:00melanoma
- 1:00:01eye.
- 1:00:02And
- 1:00:02I think
- 1:00:02that's
- 1:00:02just
- 1:00:03really
- 1:00:03unfortunate.
- 1:00:04But, you
- 1:00:05know,
- 1:00:06again,
- 1:00:07if if
- 1:00:08when
- 1:00:08I see
- 1:00:08people
- 1:00:09regularly
- 1:00:09always
- 1:00:09ask about
- 1:00:10what else
- 1:00:10is
- 1:00:10happening
- 1:00:11in their
- 1:00:11eye care,
- 1:00:11who's
- 1:00:12checking
- 1:00:12this,
- 1:00:12who's
- 1:00:13doing
- 1:00:13that, and
- 1:00:13sometimes
- 1:00:14I have
- 1:00:14to get
- 1:00:14on the phone
- 1:00:15and call
- 1:00:15their
- 1:00:15ophthalmologist
- 1:00:16and say,
- 1:00:16hey, you
- 1:00:16know,
- 1:00:17it's
- 1:00:17completely
- 1:00:17safe.
- 1:00:18This
- 1:00:18tumor
- 1:00:18is dead.
- 1:00:19You know,
- 1:00:19please,
- 1:00:19do you
- 1:00:20know,
- 1:00:21glaucoma
- 1:00:21surgery.
- 1:00:21So
- 1:00:22the vision,
- 1:00:22we're
- 1:00:23not
- 1:00:23going to
- 1:00:23lose
- 1:00:23vision
- 1:00:24from
- 1:00:24something
- 1:00:25else.
- 1:00:25So
- 1:00:26I
- 1:00:26encourage
- 1:00:27people to
- 1:00:28know,
- 1:00:29ask
- 1:00:29questions
- 1:00:30not just
- 1:00:30about the
- 1:00:31cancer
- 1:00:31part,
- 1:00:31but
- 1:00:32about everything
- 1:00:32in their
- 1:00:33eye
- 1:00:33health.
- 1:00:35Yeah,
- 1:00:35I mean,
- 1:00:36that
- 1:00:36makes
- 1:00:36sense.
- 1:00:36You want to
- 1:00:37preserve
- 1:00:37any
- 1:00:37vision
- 1:00:38you have.
- 1:00:38So yeah,
- 1:00:39I mean,
- 1:00:40I think
- 1:00:41that
- 1:00:41that's
- 1:00:41wonderful
- 1:00:42that
- 1:00:43everything
- 1:00:43is
- 1:00:43progressing
- 1:00:44the way it is
- 1:00:44and
- 1:00:45they're
- 1:00:45starting
- 1:00:45to see
- 1:00:45that
- 1:00:46they can
- 1:00:46save
- 1:00:46the vision.
- 1:00:47I mean,
- 1:00:47that's
- 1:00:49the
- 1:00:49ultimate
- 1:00:50thing is,
- 1:00:51you know,
- 1:00:51obviously
- 1:00:53getting
- 1:00:53the
- 1:00:53cancer
- 1:00:54to be,
- 1:00:56what's
- 1:00:56the word.
- 1:00:56Not
- 1:00:58not cure.
- 1:00:59Cure
- 1:01:00killed
- 1:01:01kill the
- 1:01:01melanoma.
- 1:01:03You
- 1:01:03just kill
- 1:01:04it.
- 1:01:04I don't
- 1:01:04I don't
- 1:01:05there's
- 1:01:05no fancy.
- 1:01:05Words
- 1:01:06like
- 1:01:07there.
- 1:01:08That
- 1:01:08dead.
- 1:01:09Yes.
- 1:01:10Yeah.
- 1:01:10To kill
- 1:01:11the cancer.
- 1:01:11But then
- 1:01:12we also
- 1:01:12need to
- 1:01:12preserve
- 1:01:13quality
- 1:01:13of life
- 1:01:14and vision.
- 1:01:14And so
- 1:01:15you don't
- 1:01:15just
- 1:01:16want to
- 1:01:16stop there.
- 1:01:17So
- 1:01:17I think
- 1:01:18that's
- 1:01:18great.
- 1:01:18And that,
- 1:01:19you know,
- 1:01:20technology
- 1:01:20is
- 1:01:21progressing
- 1:01:21enough
- 1:01:22that
- 1:01:22they can
- 1:01:22do more
- 1:01:23and more.
- 1:01:23It's
- 1:01:24really
- 1:01:24just
- 1:01:24fascinating.
- 1:01:25And I
- 1:01:26mean,
- 1:01:26what
- 1:01:27a time
- 1:01:27to be
- 1:01:27alive.
- 1:01:27Like the
- 1:01:28technology
- 1:01:29is to
- 1:01:29explore.
- 1:01:31Yeah.
- 1:01:31And then
- 1:01:31there's
- 1:01:31so much,
- 1:01:32so much
- 1:01:32happening
- 1:01:33and you
- 1:01:34know,
- 1:01:34repair of
- 1:01:35I mean,
- 1:01:35I think,
- 1:01:36I think
- 1:01:36in the
- 1:01:36field
- 1:01:37of
- 1:01:37retinal
- 1:01:37detachment
- 1:01:38even and
- 1:01:38sometimes
- 1:01:39eyes with
- 1:01:40the
- 1:01:40melanoma
- 1:01:40right
- 1:01:41after
- 1:01:41treatment,
- 1:01:42sometimes
- 1:01:42you can
- 1:01:43get a
- 1:01:43retinal detachment.
- 1:01:44And I
- 1:01:44find that
- 1:01:45a lot
- 1:01:45of times
- 1:01:45doctors
- 1:01:46don't
- 1:01:46really
- 1:01:46want to
- 1:01:46treat it.
- 1:01:48And,
- 1:01:48and I
- 1:01:48that
- 1:01:48really
- 1:01:49bugs me
- 1:01:49because
- 1:01:49we,
- 1:01:50we know
- 1:01:50how to
- 1:01:50fix
- 1:01:51retinas.
- 1:01:52and so,
- 1:01:53so if you
- 1:01:53find
- 1:01:54that,
- 1:01:54you know,
- 1:01:54your
- 1:01:54vision
- 1:01:55is down
- 1:01:55and you,
- 1:01:55you're asking,
- 1:01:56why
- 1:01:56am I not
- 1:01:57seeing?
- 1:01:57And then
- 1:01:58you hear
- 1:01:58that
- 1:01:58there's a
- 1:01:59retinal detachment,
- 1:02:00you can
- 1:02:00say,
- 1:02:00well,
- 1:02:00how about
- 1:02:01how about
- 1:02:01we fix
- 1:02:01the retina
- 1:02:02because,
- 1:02:03you know,
- 1:02:03people do
- 1:02:04that.
- 1:02:04That's a
- 1:02:04normal
- 1:02:05thing.
- 1:02:06and I
- 1:02:06find that
- 1:02:06people
- 1:02:06are
- 1:02:06always
- 1:02:07reluctant
- 1:02:08to do
- 1:02:08that. So.
- 1:02:09So I
- 1:02:09encourage
- 1:02:10people
- 1:02:11to,
- 1:02:11you know,
- 1:02:11ask
- 1:02:12specifically
- 1:02:13like,
- 1:02:13why is my vision
- 1:02:14decreased
- 1:02:15if it is
- 1:02:16or why
- 1:02:17am I
- 1:02:17seeing
- 1:02:18this?
- 1:02:18And once
- 1:02:18you
- 1:02:18understand
- 1:02:19what
- 1:02:19it is
- 1:02:19like,
- 1:02:19ask,
- 1:02:20what are we going
- 1:02:21to do
- 1:02:21about it?
- 1:02:21You know,
- 1:02:22we're
- 1:02:22going
- 1:02:22to just
- 1:02:22leave it
- 1:02:22there or,
- 1:02:23you know,
- 1:02:24do
- 1:02:24I have to
- 1:02:24live
- 1:02:24with this
- 1:02:25or can
- 1:02:25this be
- 1:02:25fixed?
- 1:02:26And
- 1:02:27I just
- 1:02:28I find
- 1:02:28that
- 1:02:28a lot
- 1:02:29of still,
- 1:02:29there has
- 1:02:29to be
- 1:02:30a lot
- 1:02:30of work
- 1:02:30amongst
- 1:02:31the
- 1:02:32ophthalmology
- 1:02:32community
- 1:02:33to let
- 1:02:34people
- 1:02:35know.
- 1:02:35And,
- 1:02:36you know,
- 1:02:36lot of times
- 1:02:36I give
- 1:02:37talks
- 1:02:37in our
- 1:02:38annual
- 1:02:38meetings
- 1:02:39and
- 1:02:39things
- 1:02:39about all
- 1:02:40the
- 1:02:40different
- 1:02:40things
- 1:02:40that
- 1:02:40we have
- 1:02:41to make
- 1:02:43better
- 1:02:43vision.
- 1:02:43And then
- 1:02:44ocular
- 1:02:44melanoma
- 1:02:45eyes,
- 1:02:45because
- 1:02:45we have
- 1:02:46so many
- 1:02:47incredible
- 1:02:48techniques.
- 1:02:48As you
- 1:02:49say,
- 1:02:49there are
- 1:02:49so many
- 1:02:50advances.
- 1:02:50It
- 1:02:50doesn't
- 1:02:51have
- 1:02:51to be
- 1:02:51an
- 1:02:51advancement
- 1:02:52in eye
- 1:02:52cancer,
- 1:02:53but an
- 1:02:54an
- 1:02:54advancement
- 1:02:55in any
- 1:02:55type of
- 1:02:56eye care
- 1:02:57is is
- 1:02:58translatable
- 1:02:58to an eye
- 1:02:59with a
- 1:02:59melanoma.
- 1:03:00You know,
- 1:03:00there's
- 1:03:00so much
- 1:03:01that
- 1:03:01people
- 1:03:01can
- 1:03:01benefit
- 1:03:02from.
- 1:03:02but
- 1:03:03I also
- 1:03:04often
- 1:03:04find that
- 1:03:04it's hard
- 1:03:05to get
- 1:03:05the eye
- 1:03:06doctors
- 1:03:06to be
- 1:03:06open
- 1:03:07about it,
- 1:03:07because I
- 1:03:08feel
- 1:03:08like,
- 1:03:08honestly,
- 1:03:09you know,
- 1:03:10an eye
- 1:03:10with a
- 1:03:10melanoma
- 1:03:11is a
- 1:03:11little
- 1:03:11second
- 1:03:12class
- 1:03:12citizen
- 1:03:12eye.
- 1:03:13And and
- 1:03:14we have
- 1:03:14to
- 1:03:14just let
- 1:03:14people
- 1:03:15know
- 1:03:15that it's
- 1:03:16we have to
- 1:03:16do
- 1:03:16we have to
- 1:03:17fix that.
- 1:03:17If
- 1:03:17you have
- 1:03:17macular
- 1:03:18degeneration,
- 1:03:19you get
- 1:03:20macular
- 1:03:20degeneration.
- 1:03:21And I
- 1:03:21with a
- 1:03:21melanoma
- 1:03:22will
- 1:03:22treat
- 1:03:22that.
- 1:03:22You know,
- 1:03:23we're not
- 1:03:23just
- 1:03:23going to
- 1:03:23leave it.
- 1:03:24Of course
- 1:03:24I mean
- 1:03:25we have
- 1:03:25like
- 1:03:26that.
- 1:03:26I think
- 1:03:26that's
- 1:03:26the area
- 1:03:27we have.
- 1:03:27The
- 1:03:27incredible
- 1:03:28progress
- 1:03:28in the
- 1:03:29last few
- 1:03:29years is
- 1:03:30the
- 1:03:30treatments
- 1:03:30we have
- 1:03:31for age
- 1:03:31related
- 1:03:31macular
- 1:03:32degeneration,
- 1:03:33where
- 1:03:33we can
- 1:03:33improve
- 1:03:34vision
- 1:03:34and
- 1:03:34people
- 1:03:35are
- 1:03:35driving
- 1:03:36as they
- 1:03:36get older
- 1:03:37with
- 1:03:37these
- 1:03:37diseases.
- 1:03:38I mean,
- 1:03:38those diseases
- 1:03:39are
- 1:03:39fairly
- 1:03:40common
- 1:03:40and
- 1:03:40they can happen
- 1:03:41if you have
- 1:03:41a
- 1:03:41melanoma
- 1:03:42and and
- 1:03:43you got
- 1:03:43to get
- 1:03:43that
- 1:03:43treated.
- 1:03:44And so,
- 1:03:45so
- 1:03:45just, you
- 1:03:46know,
- 1:03:46again,
- 1:03:46I think
- 1:03:47a lot of
- 1:03:47it is
- 1:03:48just for
- 1:03:48the
- 1:03:48audience
- 1:03:49is to ask
- 1:03:50questions.
- 1:03:51If
- 1:03:51something
- 1:03:51is
- 1:03:52changing
- 1:03:52in
- 1:03:52your vision,
- 1:03:52you want to know
- 1:03:53why,
- 1:03:54you know,
- 1:03:54if
- 1:03:54something
- 1:03:55is
- 1:03:55a little
- 1:03:55worse.
- 1:03:56Your ophthalmologist
- 1:03:57needs to
- 1:03:57tell you
- 1:03:58why and
- 1:03:59what
- 1:03:59you're
- 1:03:59going
- 1:03:59to do about it
- 1:04:01if you can.
- 1:04:01Because
- 1:04:02yes,
- 1:04:02there are
- 1:04:02some things
- 1:04:03we don't
- 1:04:03have
- 1:04:03treatments
- 1:04:04for, but
- 1:04:05there are
- 1:04:05so many
- 1:04:05things
- 1:04:06that are
- 1:04:06treatable.
- 1:04:07So if you
- 1:04:08have a
- 1:04:08treatable
- 1:04:08condition,
- 1:04:09you have
- 1:04:10to push
- 1:04:10to get
- 1:04:11the treatment.
- 1:04:11And
- 1:04:12if your doctor
- 1:04:12is
- 1:04:13reluctant
- 1:04:13again,
- 1:04:14get
- 1:04:14a second
- 1:04:15opinion,
- 1:04:15you know,
- 1:04:15see
- 1:04:16someone
- 1:04:16who, who
- 1:04:17has the
- 1:04:17ability
- 1:04:18and
- 1:04:19willingness
- 1:04:19to help
- 1:04:20you see
- 1:04:20your best.
- 1:04:21And
- 1:04:21that's
- 1:04:21really
- 1:04:21what it's
- 1:04:22all about,
- 1:04:22as you
- 1:04:23say, it's
- 1:04:23just
- 1:04:23really,
- 1:04:24you know,
- 1:04:24making
- 1:04:24use
- 1:04:25of all
- 1:04:25the
- 1:04:25advances
- 1:04:26that
- 1:04:26we have
- 1:04:26and
- 1:04:27making
- 1:04:27sure
- 1:04:27that they
- 1:04:28apply to
- 1:04:28your eye
- 1:04:29health.
- 1:04:30And we
- 1:04:30talk a
- 1:04:31lot about
- 1:04:31advocating
- 1:04:32for yourself
- 1:04:32that
- 1:04:33you're
- 1:04:33your best
- 1:04:33advocate.
- 1:04:34And
- 1:04:34this is,
- 1:04:35excuse
- 1:04:35me,
- 1:04:35this is one
- 1:04:36of those
- 1:04:36situations
- 1:04:37where
- 1:04:37if you
- 1:04:37don't go
- 1:04:38ask those
- 1:04:38questions
- 1:04:39and you
- 1:04:39don't
- 1:04:39advocate
- 1:04:40for yourself,
- 1:04:40no
- 1:04:41one else
- 1:04:41is going
- 1:04:41to do it.
- 1:04:43That's
- 1:04:43absolutely
- 1:04:44right.
- 1:04:44Yeah.
- 1:04:45And
- 1:04:45you have
- 1:04:45to feel
- 1:04:45really
- 1:04:46comfortable
- 1:04:46doing
- 1:04:46that.
- 1:04:47You have
- 1:04:47to think
- 1:04:47this is
- 1:04:47for you.
- 1:04:48You know,
- 1:04:48this is,
- 1:04:49you know,
- 1:04:50it's
- 1:04:50your eye
- 1:04:50and
- 1:04:51your life
- 1:04:51and
- 1:04:53you have to be
- 1:04:54you have to
- 1:04:55be
- 1:04:55comfortable
- 1:04:55just speaking
- 1:04:56up for
- 1:04:56yourself
- 1:04:57and, and,
- 1:04:59you know,
- 1:05:00talk to
- 1:05:00people.
- 1:05:01You know,
- 1:05:01I always
- 1:05:01recommend
- 1:05:02people
- 1:05:02bring
- 1:05:02somebody
- 1:05:03to their
- 1:05:03appointment
- 1:05:04because,
- 1:05:04you know,
- 1:05:05even,
- 1:05:05you know,
- 1:05:05I've been
- 1:05:05a patient
- 1:05:06before,
- 1:05:06and
- 1:05:06I often
- 1:05:07find that
- 1:05:08when
- 1:05:08something
- 1:05:08is
- 1:05:09happening
- 1:05:09to you,
- 1:05:10you
- 1:05:10kind of
- 1:05:10you kind
- 1:05:11of lose
- 1:05:11a little
- 1:05:11bit, you
- 1:05:12lose
- 1:05:12your voice
- 1:05:12a little bit.
- 1:05:13So bring
- 1:05:14someone
- 1:05:14with you,
- 1:05:15a good
- 1:05:15friend,
- 1:05:15a family
- 1:05:16member or
- 1:05:16partner
- 1:05:17and
- 1:05:18have them
- 1:05:19there
- 1:05:19together
- 1:05:19with you
- 1:05:20to make
- 1:05:21sure that
- 1:05:21you got
- 1:05:22through
- 1:05:22all the
- 1:05:22questions
- 1:05:23on your list
- 1:05:23or that
- 1:05:24you know
- 1:05:24the
- 1:05:25person
- 1:05:25coming
- 1:05:25with you
- 1:05:26is
- 1:05:27you got a
- 1:05:27two sets
- 1:05:28of brains
- 1:05:28there
- 1:05:28that are
- 1:05:29analyzing
- 1:05:30the information
- 1:05:30you're getting,
- 1:05:31and
- 1:05:31they can ask
- 1:05:32questions
- 1:05:32too.
- 1:05:33So
- 1:05:33I think
- 1:05:33that's
- 1:05:34very helpful
- 1:05:35because
- 1:05:35sometimes
- 1:05:37if you're
- 1:05:37just by
- 1:05:37yourself,
- 1:05:38you might
- 1:05:38you might
- 1:05:39not feel
- 1:05:39strong
- 1:05:40enough.
- 1:05:41But
- 1:05:41when you
- 1:05:41have
- 1:05:41someone
- 1:05:41with you,
- 1:05:42I
- 1:05:43find that
- 1:05:43if I
- 1:05:43ever come
- 1:05:44with
- 1:05:44someone
- 1:05:44to a
- 1:05:45medical
- 1:05:45examination,
- 1:05:47I, I'm
- 1:05:47right
- 1:05:47there,
- 1:05:48ready to
- 1:05:49interrogate
- 1:05:50and
- 1:05:50find out
- 1:05:50everything.
- 1:05:51So
- 1:05:51bring a
- 1:05:52close
- 1:05:52friend.
- 1:05:52I think
- 1:05:53that's
- 1:05:53a tip
- 1:05:53I'd
- 1:05:53recommend.
- 1:05:55Yeah,
- 1:05:55and even
- 1:05:55if they
- 1:05:56can't go
- 1:05:56in
- 1:05:56person,
- 1:05:56you can always
- 1:05:57put them
- 1:05:57on
- 1:05:57FaceTime
- 1:05:58on your phone
- 1:05:58and
- 1:05:59have them
- 1:05:59not be
- 1:06:00right
- 1:06:00there
- 1:06:00with you
- 1:06:00on your
- 1:06:01phone.
- 1:06:02Exactly.
- 1:06:02That's
- 1:06:02a great thing
- 1:06:03to bring.
- 1:06:05Bring
- 1:06:05people
- 1:06:05by
- 1:06:05FaceTime
- 1:06:06in
- 1:06:07person.
- 1:06:07It's,
- 1:06:08you know,
- 1:06:08there's
- 1:06:08so much
- 1:06:09you can
- 1:06:09do, and
- 1:06:10I
- 1:06:10encourage
- 1:06:10everyone
- 1:06:11to try
- 1:06:11to do
- 1:06:12the most
- 1:06:12they can
- 1:06:13to really
- 1:06:14engage.
- 1:06:15You know,
- 1:06:15it's all
- 1:06:15about
- 1:06:16your eye
- 1:06:16and
- 1:06:16your health
- 1:06:17and so
- 1:06:18much
- 1:06:18people
- 1:06:18can do.
- 1:06:19And
- 1:06:20in this
- 1:06:20experience,
- 1:06:21not not
- 1:06:22only in
- 1:06:22the very
- 1:06:22beginning,
- 1:06:23but,
- 1:06:24you know,
- 1:06:24even
- 1:06:24like a
- 1:06:25decade
- 1:06:25later,
- 1:06:25if
- 1:06:26there's
- 1:06:26more
- 1:06:26information
- 1:06:27or more
- 1:06:27knowledge,
- 1:06:27it's it's
- 1:06:29I find
- 1:06:29that it's
- 1:06:30it's
- 1:06:30really
- 1:06:31it really
- 1:06:31is
- 1:06:32important
- 1:06:32for
- 1:06:32people
- 1:06:32to keep
- 1:06:33learning,
- 1:06:33even
- 1:06:34if you
- 1:06:34think
- 1:06:34you're
- 1:06:35your
- 1:06:35old hat.
- 1:06:35I mean,
- 1:06:36there's
- 1:06:36always
- 1:06:36something
- 1:06:37else.
- 1:06:38Right?
- 1:06:38You know.
- 1:06:39Absolutely.
- 1:06:40That's
- 1:06:40100%
- 1:06:41true.
- 1:06:42I think
- 1:06:42a lot of
- 1:06:43people
- 1:06:43like,
- 1:06:43it's
- 1:06:43really
- 1:06:44hard to
- 1:06:44get that
- 1:06:44kind of
- 1:06:44information
- 1:06:45in your
- 1:06:46eye
- 1:06:46appointments.
- 1:06:47yeah.
- 1:06:47I feel like
- 1:06:48there's
- 1:06:48little
- 1:06:49pieces
- 1:06:49that
- 1:06:49I talk
- 1:06:50to people
- 1:06:50about,
- 1:06:50but just
- 1:06:51sort of
- 1:06:51really
- 1:06:52focusing
- 1:06:52on all
- 1:06:52the
- 1:06:53things
- 1:06:53you can
- 1:06:53do, like
- 1:06:54literally
- 1:06:55like mind
- 1:06:56and body
- 1:06:56and
- 1:06:57physical
- 1:06:57health
- 1:06:58and so
- 1:06:59many
- 1:06:59things
- 1:06:59that
- 1:06:59relate
- 1:07:00to people
- 1:07:00doing
- 1:07:00better.
- 1:07:01So I
- 1:07:01so appreciate
- 1:07:02that.
- 1:07:02I just
- 1:07:03really,
- 1:07:04you know,
- 1:07:04I'm
- 1:07:04excited
- 1:07:05to
- 1:07:05reach out
- 1:07:06to people
- 1:07:07and be
- 1:07:07a resource,
- 1:07:08and I'm
- 1:07:08happy to
- 1:07:08if a
- 1:07:09topic
- 1:07:09comes up
- 1:07:09that
- 1:07:10you need
- 1:07:10any
- 1:07:10help on
- 1:07:11or I can
- 1:07:12speak
- 1:07:12about,
- 1:07:13please
- 1:07:13don't
- 1:07:13hesitate
- 1:07:14to ask
- 1:07:14me.
- 1:07:14I feel like
- 1:07:15I
- 1:07:16really
- 1:07:16want to
- 1:07:16share
- 1:07:17everything
- 1:07:17that I've
- 1:07:17learned
- 1:07:18and
- 1:07:18and what
- 1:07:18I think
- 1:07:19would
- 1:07:20help
- 1:07:20people
- 1:07:20who are
- 1:07:21going
- 1:07:21through
- 1:07:21this just
- 1:07:22to do
- 1:07:22their
- 1:07:22very just
- 1:07:23to have a
- 1:07:23make this
- 1:07:24something
- 1:07:25really,
- 1:07:26you know,
- 1:07:26like an
- 1:07:27opportunity
- 1:07:28to learn
- 1:07:28new
- 1:07:28things
- 1:07:29and to
- 1:07:29just,
- 1:07:30you know,
- 1:07:31really
- 1:07:31engage.
- 1:07:31I think
- 1:07:32that's,
- 1:07:32that's
- 1:07:32really
- 1:07:33my,
- 1:07:33my
- 1:07:34mission.
- 1:07:34I feel
- 1:07:34it's
- 1:07:34important
- 1:07:35that we,
- 1:07:35we do
- 1:07:36all
- 1:07:36we can.
- 1:07:36And,
