- Yoel Bakas: A Theranostics dose is not like a normal slot on your treatment schedule. And that affects how a no-show impacts your program. Take the Lutetium that most programs use, Pluvicto and Lutathera The dose is a fixed amount. ordered for one patient on a specific day. with about a five day shelf life because the isotope is decaying the entire time. It's expensive and it can't sit on the shelf waiting for a better day. So when a patient doesn't show up or there's an issue with the labs. you may not be able to backfill that open slot. You're holding an expensive decaying dose with a very short window. Maybe you can reschedule inside of that window and maybe you can't. Either way, it's real exposure that a regular radonc patient does not carry. And in the vendor performa, what you see is something different. the margin per treatment. What it does not show you. Is how often it gets wasted. And what it costs to cover supervision when your one authorized user is not available. Most programs run thin on that coverage, and many don't price it until the reality bites. Protecting margin here is not a billing trick. It's operating discipline. The scheduling, the rescheduling protocol. The authorized user coverage The capture mechanics. Those are the things that decide whether the service line holds its margin. And it's exactly the work that we do at Medsolve before the program launches the service line. Make someone show you dose calculations. And the authorized user coverage plan before you sign. If no one can, that's your answer.
