- 0:00let's
- 0:00give a big
- 0:00shout out.
- 0:01And,
- 0:01welcome
- 0:01to Doctor
- 0:01Moser from
- 0:02Honor
- 0:02Health
- 0:02Research
- 0:03Institute.
- 0:10Welcome,
- 0:10everyone.
- 0:11And thanks
- 0:11for coming
- 0:11both in
- 0:11person
- 0:12and
- 0:12online.
- 0:12we saw
- 0:13most of
- 0:13you guys
- 0:13yesterday.
- 0:14I think
- 0:14we even
- 0:14have
- 0:15more in
- 0:15attendance
- 0:15today,
- 0:16which is
- 0:16great.
- 0:17today
- 0:17the talks
- 0:18are more
- 0:18kind of
- 0:19medically
- 0:19focused.
- 0:20so
- 0:20we actually
- 0:20have a
- 0:20really
- 0:21good line
- 0:21up of
- 0:21people
- 0:22coming.
- 0:22we have a lot
- 0:23of
- 0:23different
- 0:23physicians
- 0:24and other
- 0:24people
- 0:24coming in
- 0:25to speak
- 0:25today.
- 0:25So
- 0:25it should
- 0:25be, a
- 0:26pretty
- 0:26good
- 0:26discussion.
- 0:27Most of
- 0:28the
- 0:28discussions
- 0:28today.
- 0:28We'll have
- 0:29slides to
- 0:29help
- 0:30guide it.
- 0:30But
- 0:30we really want
- 0:31these to
- 0:31be
- 0:31discussion.
- 0:32So if you
- 0:32have
- 0:32questions
- 0:33raise
- 0:33your hand.
- 0:34Most of it
- 0:34will be
- 0:34a panel
- 0:35format.
- 0:36And so
- 0:36we really
- 0:37want
- 0:37this to
- 0:37be
- 0:37interactive.
- 0:38So
- 0:38you guys
- 0:38can get
- 0:38the
- 0:38information
- 0:39that best
- 0:39serves
- 0:40you.
- 0:40when
- 0:40we're,
- 0:41talking
- 0:41about
- 0:42liver
- 0:42directed
- 0:42therapy,
- 0:43there's,
- 0:44two kind
- 0:45of ways
- 0:45that we
- 0:45can treat
- 0:46tumors in
- 0:46the liver.
- 0:47One are
- 0:47directly.
- 0:48sticking
- 0:48needles
- 0:49or probes
- 0:50into the
- 0:50liver to,
- 0:51treat
- 0:51the tumors.
- 0:52And then
- 0:52there's
- 0:52also the
- 0:53transarterial
- 0:54approach
- 0:54that
- 0:54we can
- 0:55use,
- 0:55taking
- 0:56advantage
- 0:56of the
- 0:57liver's
- 0:57unique
- 0:58dual blood
- 0:58supply
- 0:59to
- 0:59allow us
- 0:59to treat
- 1:00tumors
- 1:01through
- 1:01the
- 1:01hepatic
- 1:01artery.
- 1:02while
- 1:02letting
- 1:03the portal
- 1:03vein,
- 1:04which
- 1:04supplies
- 1:05most of
- 1:05the blood
- 1:06flow to
- 1:06the liver,
- 1:07to kind
- 1:07of keep
- 1:08the liver
- 1:08alive.
- 1:09we'll kind
- 1:09of break
- 1:09this talk
- 1:10briefly
- 1:10into two
- 1:11subsets
- 1:12of ways
- 1:12to treat.
- 1:13so we'll
- 1:13first talk
- 1:14about the
- 1:14percutaneous
- 1:15treatments
- 1:16for liver
- 1:17tumors.
- 1:17the
- 1:17mainstay
- 1:18of this
- 1:19treatment
- 1:19for
- 1:19percutaneous
- 1:20therapy is
- 1:21ablation.
- 1:22it
- 1:22could be
- 1:22either
- 1:23heat based
- 1:23or
- 1:24freezing
- 1:25based.
- 1:25So
- 1:26cryoablation
- 1:27or either
- 1:27microwave
- 1:28radiofrequency
- 1:29ablation
- 1:30where
- 1:30we're
- 1:30using
- 1:31temperature
- 1:32to create
- 1:33direct
- 1:33tissue
- 1:33damage
- 1:34and kill,
- 1:35whatever
- 1:35is in
- 1:35the tumor.
- 1:36with those
- 1:37temperature
- 1:37effects.
- 1:38the pros of
- 1:39this are
- 1:39it's,
- 1:40efficacy
- 1:40is similar
- 1:41to
- 1:41that of,
- 1:42if you
- 1:42go in
- 1:42and
- 1:42have the
- 1:43surgically
- 1:43removed,
- 1:44the cons
- 1:45are it
- 1:45does
- 1:46require
- 1:46that
- 1:46it has
- 1:47to be
- 1:47a small,
- 1:48relatively
- 1:48small
- 1:48size.
- 1:49And,
- 1:50we can only
- 1:50treat
- 1:51a certain
- 1:51amount
- 1:52based on,
- 1:52the size
- 1:53and kind
- 1:53of the
- 1:54tissue
- 1:54destruction
- 1:55that we
- 1:55cause.
- 1:55one thing
- 1:56that
- 1:56we also
- 1:57participate
- 1:57in is
- 1:57clinical
- 1:58trials
- 1:58where
- 1:58we're
- 1:58doing,
- 1:59intratumoral
- 2:00injection,
- 2:00where,
- 2:01we
- 2:01directly
- 2:02inject,
- 2:02whatever
- 2:03medicine
- 2:04they
- 2:04come up
- 2:05with for
- 2:05us,
- 2:06directly
- 2:06into the
- 2:06tumor
- 2:07to try
- 2:07and induce
- 2:08an immune
- 2:08response
- 2:09or, kind
- 2:10of
- 2:10affected
- 2:10directly
- 2:11into the
- 2:11tumor
- 2:12itself.
- 2:13so that's
- 2:14very well
- 2:14tolerated.
- 2:15and,
- 2:15very small
- 2:16needles.
- 2:16And it's
- 2:16very,
- 2:17straightforward
- 2:18for us
- 2:18to, to
- 2:18get the,
- 2:19the
- 2:20medication
- 2:20inside
- 2:21the tumor.
- 2:22and that's
- 2:22kind of
- 2:22all done
- 2:23on a
- 2:23clinical
- 2:23trial
- 2:24basis
- 2:24there are several,
- 2:25and then
- 2:25one of
- 2:26this is
- 2:26one of
- 2:26the new
- 2:27kind of
- 2:27cutting
- 2:27edge
- 2:28therapies
- 2:28that we're
- 2:29really
- 2:29excited
- 2:30about.
- 2:30coming on
- 2:31is,
- 2:32histotripsy
- 2:33So this is
- 2:33an
- 2:33ultrasound
- 2:34based
- 2:34technology
- 2:35where
- 2:35they give
- 2:36high
- 2:36frequency,
- 2:37high
- 2:37amplitude,
- 2:38pulses
- 2:39that,
- 2:39causes
- 2:40kind of
- 2:40direct
- 2:41vaporization
- 2:42of
- 2:43but what's
- 2:44really
- 2:44exciting
- 2:44is it's,
- 2:45completely,
- 2:46noninvasive.
- 2:47So it's
- 2:48all done
- 2:48without
- 2:49any
- 2:49incision.
- 2:50and very
- 2:51well
- 2:51tolerated
- 2:51just
- 2:51because
- 2:52there's,
- 2:52no
- 2:52incision
- 2:53or
- 2:53anything.
- 2:54And it
- 2:55still
- 2:55allows
- 2:55you to
- 2:55to treat
- 2:56tumors
- 2:56inside
- 2:57the liver.
- 2:57those are
- 2:57the
- 2:58percutaneous
- 2:59options
- 2:59that we
- 2:59have.
- 3:00so other
- 3:00things
- 3:00that
- 3:00we can
- 3:01work
- 3:02through
- 3:02are,
- 3:02transarterial
- 3:03approaches
- 3:04to,
- 3:04treating
- 3:04these
- 3:04tumors.
- 3:05there
- 3:05are many
- 3:07different
- 3:07ways.
- 3:08There's
- 3:08not a ton
- 3:08of data
- 3:09on most of
- 3:09them.
- 3:10I don't
- 3:10really
- 3:11have
- 3:11experience
- 3:12with,
- 3:12because
- 3:12this
- 3:12is done
- 3:13by the,
- 3:13surgeons,
- 3:14they can
- 3:15implant
- 3:15a,
- 3:16infusion
- 3:16catheter
- 3:17that the
- 3:18directly
- 3:18infuses,
- 3:19chemotherapy,
- 3:21liver
- 3:22through
- 3:22a port
- 3:23just
- 3:23beneath
- 3:23the skin.
- 3:24and where
- 3:24you can
- 3:25have
- 3:25kind of
- 3:25weekly
- 3:26infusions
- 3:26to, to
- 3:27treat
- 3:27directly
- 3:28into the,
- 3:28liver
- 3:29vasculature.
- 3:32one of
- 3:32the kind
- 3:33of
- 3:33mainstays
- 3:33of liver
- 3:34directed
- 3:34therapy
- 3:35that we've
- 3:35had for
- 3:35many years
- 3:36is
- 3:36transarterial
- 3:37embolization.
- 3:37there
- 3:38are many,
- 3:38many
- 3:38different
- 3:39ways to
- 3:39do this.
- 3:40It can be,
- 3:41what
- 3:41we call
- 3:41bland.
- 3:42So just,
- 3:43kind of
- 3:44impeding
- 3:45the blood
- 3:45flow to
- 3:45the
- 3:45tumors.
- 3:46the
- 3:46embolic effect,
- 3:48or it
- 3:48can be
- 3:48combined
- 3:49with many
- 3:50different
- 3:50chemotherapy
- 3:51drugs to,
- 3:52also
- 3:52deposit
- 3:53the
- 3:53chemotherapy
- 3:54directly
- 3:55into the
- 3:55liver
- 3:56tumors
- 3:56themselves.
- 3:58in the
- 3:58setting of
- 3:58uveal
- 3:59melanoma,
- 3:59there's
- 3:59not
- 4:00a lot of,
- 4:00data
- 4:00to show
- 4:01which
- 4:02combination
- 4:03works
- 4:03best.
- 4:04and the
- 4:04biggest
- 4:05kind of
- 4:05downside
- 4:05of this
- 4:06procedure
- 4:06is it
- 4:07can be
- 4:07painful,
- 4:08just
- 4:08because
- 4:09we are
- 4:09intentionally
- 4:10cutting
- 4:10off the
- 4:10blood
- 4:11supply.
- 4:11So
- 4:11it can be,
- 4:12quite
- 4:13painful
- 4:13if we
- 4:13if we're
- 4:14doing a
- 4:15large area
- 4:15of the
- 4:16liver.
- 4:16the other
- 4:17potential
- 4:17transarterial
- 4:18therapy
- 4:19is immuno
- 4:20embolization,
- 4:21where
- 4:22you try
- 4:22and induce
- 4:23an immune
- 4:23response
- 4:24by kind of
- 4:24directly,
- 4:25infusing
- 4:27these
- 4:27medications,
- 4:28the
- 4:29granulocyte
- 4:30macrophage
- 4:31colony
- 4:31stimulating
- 4:32factor
- 4:33directly
- 4:33into the
- 4:33liver
- 4:34to try
- 4:34and induce
- 4:35the immune
- 4:35response,
- 4:36of
- 4:36the tumors.
- 4:40So
- 4:40I'm sure
- 4:40most of
- 4:41you maybe
- 4:42have heard
- 4:42of y90
- 4:43radio
- 4:43embolization.
- 4:43That's
- 4:44kind
- 4:44of the,
- 4:45kind of
- 4:45mainstay
- 4:46of
- 4:47kind of
- 4:47treatment,
- 4:49currently
- 4:50for, a
- 4:50lot of
- 4:50liver
- 4:51directed
- 4:51therapies.
- 4:52and what
- 4:53that is,
- 4:53it's a,
- 4:54special
- 4:54radioisotope
- 4:55called
- 4:55yttrium 90
- 4:57that
- 4:57we can
- 4:58directly
- 4:58inject to
- 4:59the liver.
- 5:00and
- 5:01what it does
- 5:02is it
- 5:02deposits
- 5:03either
- 5:03resin or
- 5:05glass
- 5:05microspheres
- 5:06directly
- 5:07into
- 5:07the tumor.
- 5:08they
- 5:09release
- 5:09radiation
- 5:10directly
- 5:10into those
- 5:11tumor
- 5:11cells
- 5:12that only
- 5:12penetrates
- 5:13about
- 5:13two millimeters.
- 5:14so
- 5:15we can give
- 5:16high doses
- 5:16of
- 5:16radiation,
- 5:17directly
- 5:18to the
- 5:18liver
- 5:18without,
- 5:19causing
- 5:19effects
- 5:20elsewhere
- 5:20in the body.
- 5:22with y90,
- 5:23it's all
- 5:23about
- 5:23being able
- 5:24to get
- 5:25adequate
- 5:25dose to
- 5:26the tumor.
- 5:26So, in
- 5:27general,
- 5:28this works
- 5:28better
- 5:28for,
- 5:29kind of
- 5:30fewer
- 5:30number of
- 5:31lesions
- 5:31where
- 5:31we can
- 5:32really
- 5:32give high
- 5:32doses
- 5:33of
- 5:33radiation
- 5:33directly
- 5:34to the
- 5:34tumor.
- 5:34and it's
- 5:35actually
- 5:36very,
- 5:36quite well
- 5:37tolerated,
- 5:37from,
- 5:38from a
- 5:38symptomatic
- 5:39standpoint
- 5:39as well.
- 5:41And then,
- 5:41finally,
- 5:42one of,
- 5:43things
- 5:43that I'm
- 5:44very
- 5:44excited
- 5:44about and,
- 5:45a lot
- 5:46of us are
- 5:46very
- 5:46excited
- 5:46about
- 5:47is
- 5:47percutaneous
- 5:48hepatic
- 5:48perfusion
- 5:49with
- 5:49the hepzato kit.
- 5:50and so
- 5:51the principles
- 5:51of that
- 5:52are to,
- 5:52saturate
- 5:53the whole
- 5:54liver
- 5:54with
- 5:55melphalan
- 5:56with the
- 5:56chemotherapy
- 5:56agent,
- 5:57and then
- 5:58isolate
- 5:59all the
- 5:59blood
- 5:59flow,
- 6:00return
- 6:00out of
- 6:01the liver,
- 6:02to
- 6:02allow us
- 6:02to,
- 6:03filter out
- 6:04the chemotherapy
- 6:05prior
- 6:05to
- 6:05giving the
- 6:05blood
- 6:06back,
- 6:06to
- 6:06minimize
- 6:07the
- 6:07systemic
- 6:08toxicity
- 6:09of the
- 6:09chemotherapy
- 6:10agent.
- 6:11so that's,
- 6:12one of
- 6:12the things
- 6:13we're very
- 6:13excited
- 6:14about
- 6:14that's
- 6:14shown,
- 6:15good
- 6:15efficacy.
- 6:16in liver
- 6:16metastases.
- 6:18So
- 6:18I have a
- 6:18couple
- 6:18cases
- 6:19here,
- 6:20from
- 6:20Doctor ma
- 6:21at
- 6:22Wisconsin,
- 6:23kind of
- 6:23showing
- 6:23the
- 6:24efficacy
- 6:24of this
- 6:25was a
- 6:25hepzato
- 6:26case,
- 6:27where
- 6:27you can see,
- 6:28you
- 6:28can see
- 6:29here
- 6:29this is the
- 6:30a large
- 6:30tumor,
- 6:31in the
- 6:31liver
- 6:32here.
- 6:33you can
- 6:33see round
- 6:33here and
- 6:34this is,
- 6:36during the
- 6:37second
- 6:37cycle of
- 6:38the
- 6:39hepzato
- 6:39treatment,
- 6:40and
- 6:40you can
- 6:40see that
- 6:41it's much
- 6:41smaller
- 6:41even after
- 6:42one,
- 6:42therapy.
- 6:43so
- 6:43there was
- 6:44also a
- 6:44corresponding,
- 6:45decrease
- 6:46in the
- 6:46circulating
- 6:47tumor DNA.
- 6:48I think
- 6:48that was
- 6:48a really
- 6:48good
- 6:48rundown
- 6:49about the
- 6:50different
- 6:50liver
- 6:50directed
- 6:51options
- 6:51that are
- 6:52available.
- 6:53And, like
- 6:53Doctor
- 6:53low said,
- 6:55the great
- 6:55thing is
- 6:56there's
- 6:56a ton
- 6:56of
- 6:56options.
- 6:57The bad
- 6:57thing is
- 6:58we don't
- 6:58always
- 6:59have
- 6:59strong
- 7:00data about
- 7:00which
- 7:00is best.
- 7:01Obviously,
- 7:02hepzato is
- 7:03the only
- 7:04FDA
- 7:04approved
- 7:05liver
- 7:05directed
- 7:05therapy
- 7:06specifically
- 7:06for uveal
- 7:07melanoma,
- 7:07so
- 7:08we do have
- 7:08really
- 7:09strong
- 7:09data for
- 7:09that.
- 7:10But in
- 7:10terms of
- 7:11Ablations
- 7:12histotripsy
- 7:14histotripsy
- 7:14is
- 7:15approved
- 7:15for any
- 7:16liver
- 7:16metastasis.
- 7:17Is that
- 7:17correct?
- 7:18Jason.
- 7:18So not
- 7:18necessarily
- 7:19uveal
- 7:20but uveal
- 7:20your goes
- 7:20to the liver.
- 7:21So
- 7:21it is approved
- 7:21in that
- 7:22regard.
- 7:22But
- 7:22immunoembolization
- 7:24radioembolization,
- 7:25all of these
- 7:25things
- 7:25have data
- 7:26to support
- 7:27their use.
- 7:27But
- 7:27they've
- 7:27never
- 7:28really
- 7:28been
- 7:28compared
- 7:28head
- 7:29to head.
- 7:29So
- 7:29it can get
- 7:30very
- 7:30confusing
- 7:31as
- 7:31a patient
- 7:32to try to
- 7:32figure
- 7:32out, well,
- 7:33which one
- 7:33should I
- 7:34do.
- 7:34because
- 7:34there's
- 7:35not great
- 7:35data,
- 7:36if you
- 7:36go see
- 7:37four
- 7:38different
- 7:38physicians,
- 7:38you might
- 7:39get six
- 7:39different
- 7:39answers.
- 7:40And that's
- 7:40because we
- 7:41don't
- 7:41necessarily
- 7:42always
- 7:42know what
- 7:42the best
- 7:43option is,
- 7:43because
- 7:44there's
- 7:44just
- 7:44limited
- 7:44data in
- 7:45rare
- 7:45tumors.
- 7:46So I think
- 7:46that was
- 7:46a really
- 7:46great
- 7:47overview.
- 7:47and we'll
- 7:48start
- 7:48with
- 7:48Doctor
- 7:48Hadfield.
- 7:49you're
- 7:49seeing a
- 7:49patient
- 7:49with uveal
- 7:50melanoma
- 7:51there's
- 7:51the
- 7:51systemic
- 7:52therapies
- 7:52that
- 7:52we have
- 7:53of
- 7:53kimmtrak
- 7:54and the
- 7:55clinical
- 7:56trials
- 7:56that are
- 7:56going on
- 7:57with
- 7:57ideaya
- 7:58and
- 7:58replimune
- 7:59And
- 8:00in octoriff
- 8:01but
- 8:02how do you
- 8:02approach
- 8:03a patient.
- 8:03When
- 8:03would you
- 8:04say, okay,
- 8:05I need
- 8:05to give
- 8:05this
- 8:05person
- 8:06systemic
- 8:06therapy,
- 8:07or when
- 8:08would you
- 8:08say, what
- 8:09liver
- 8:09directed
- 8:09option may
- 8:10be best
- 8:10for this
- 8:10patient?
- 8:11know,
- 8:11that was a
- 8:11wonderful
- 8:12overview.
- 8:12Of of
- 8:12liver
- 8:13directed
- 8:13therapies.
- 8:13And
- 8:13I think
- 8:14when you're
- 8:14you're
- 8:14meeting
- 8:15patients
- 8:15for
- 8:15the first
- 8:15time,
- 8:16there's
- 8:16a few
- 8:17things
- 8:17to kind of
- 8:17keep in
- 8:17mind.
- 8:18liver
- 8:18directed
- 8:19therapies
- 8:19are often
- 8:21technically
- 8:22challenging
- 8:22and in
- 8:23different
- 8:23ways.
- 8:24So,
- 8:24for
- 8:24instance,
- 8:25Sometimes
- 8:25patients
- 8:26just.
- 8:26Aren't
- 8:26good
- 8:26candidates
- 8:26for
- 8:27procedures.
- 8:27and
- 8:27they may
- 8:28be more
- 8:28amenable
- 8:28to
- 8:28systemic therapy.
- 8:30and that's
- 8:30something
- 8:30to
- 8:30consider.
- 8:31one thing
- 8:31that does
- 8:32come up,
- 8:32quite
- 8:32a bit,
- 8:32in
- 8:33my clinic,
- 8:33and I'm
- 8:33not sure
- 8:34if this
- 8:34resonates
- 8:34with
- 8:35anyone
- 8:35here,
- 8:35but
- 8:35oftentimes
- 8:36when
- 8:36I talk
- 8:36about
- 8:36liver
- 8:37directed
- 8:37therapy
- 8:37in
- 8:37conjunction
- 8:38with
- 8:38interventional
- 8:39radiology
- 8:39colleagues
- 8:40or
- 8:40surgical
- 8:41colleagues,
- 8:41one of the concerns
- 8:43is that
- 8:43when you
- 8:43you treat
- 8:44something
- 8:44just in
- 8:44the liver,
- 8:45it's only
- 8:46going to
- 8:46have an
- 8:46effect
- 8:47in the
- 8:47liver.
- 8:47And
- 8:47I think
- 8:47now we're
- 8:48starting
- 8:48to see
- 8:48some data
- 8:49that,
- 8:49if you
- 8:50treat
- 8:51liver
- 8:51directed
- 8:51lesions.
- 8:52particularly.
- 8:53With
- 8:53these.
- 8:53immune
- 8:54agonists
- 8:54and
- 8:54clinical.
- 8:54Trials,
- 8:55like, the.
- 8:55replimune
- 8:56study,
- 8:56you can
- 8:57actually,
- 8:57incite a
- 8:58more
- 8:58systemic
- 8:59immune
- 8:59response.
- 8:59And that's
- 9:00borne out
- 9:01with
- 9:01clinical
- 9:01data
- 9:02that's now
- 9:02becoming
- 9:03available.
- 9:03So,
- 9:04I think,
- 9:05it's
- 9:05certainly
- 9:05dependent
- 9:06on patient
- 9:07preference.
- 9:07some
- 9:07some patients
- 9:08don't
- 9:08want to
- 9:09undergo
- 9:09procedures
- 9:09and
- 9:09they're
- 9:10more
- 9:10comfortable
- 9:10with
- 9:10systemic
- 9:10therapies.
- 9:11but
- 9:12I think
- 9:12that's my
- 9:12general
- 9:13approach.
- 9:14So
- 9:14to expound
- 9:14on that.
- 9:15when
- 9:15you're
- 9:15looking
- 9:16at someone
- 9:16with just
- 9:16only liver
- 9:17Mets,
- 9:17I think
- 9:17liver
- 9:17directed
- 9:18therapy is
- 9:18reasonable.
- 9:19But you're
- 9:19absolutely
- 9:19right.
- 9:20there is a chance
- 9:21that it
- 9:21can spread
- 9:22outside
- 9:22the liver.
- 9:22And
- 9:22obviously
- 9:23you're
- 9:23not going
- 9:23to be
- 9:23treating
- 9:24that with
- 9:25uveal
- 9:25melanoma
- 9:26sometimes.
- 9:26And only
- 9:27the disease
- 9:27will stay
- 9:28in the
- 9:28liver
- 9:28for
- 9:28a couple
- 9:29of years.
- 9:29And
- 9:30I think
- 9:30a lot of
- 9:31the time,
- 9:31one of
- 9:31the things
- 9:32that can
- 9:32predict
- 9:32that
- 9:32is the
- 9:33volume
- 9:33of disease.
- 9:34So
- 9:34typically
- 9:35if I see
- 9:35someone
- 9:35with a
- 9:36high
- 9:36volume
- 9:37of disease
- 9:37in
- 9:37the liver,
- 9:38that makes me
- 9:39wonder
- 9:39if we
- 9:39should be
- 9:39doing
- 9:40a liver
- 9:40directed
- 9:40therapy,
- 9:41but also
- 9:42systemic
- 9:43therapy
- 9:43at the
- 9:43same time,
- 9:44or
- 9:44planning
- 9:44to
- 9:45sequence
- 9:45them very
- 9:45quickly.
- 9:46I think in
- 9:46general,
- 9:47people
- 9:47with very
- 9:48limited
- 9:48disease
- 9:48in
- 9:49the liver
- 9:49typically
- 9:50are more
- 9:50likely
- 9:50to benefit
- 9:51from the
- 9:51liver
- 9:51directed
- 9:52therapy
- 9:52long term,
- 9:53just
- 9:53because
- 9:53they're
- 9:54probably
- 9:54less
- 9:54likely
- 9:55to have it
- 9:55spread
- 9:55outside
- 9:56the liver.
- 9:56when we're
- 9:57differentiating,
- 9:58Jason
- 9:58did a
- 9:59really
- 9:59good job
- 9:59talking
- 9:59about the
- 10:00percutaneous
- 10:01stuff
- 10:01where we
- 10:02stick
- 10:02a needle
- 10:03in and
- 10:03inject
- 10:03or treat 1
- 10:04or 2
- 10:04lesions
- 10:05versus,
- 10:06the
- 10:07embolization
- 10:08and
- 10:08the more
- 10:08hepzato
- 10:09which
- 10:09treats the
- 10:09entire
- 10:10liver.
- 10:10Jason,
- 10:11would
- 10:11you want
- 10:11to comment
- 10:11on how
- 10:12you would
- 10:13look at on
- 10:13a scan
- 10:14to say,
- 10:14I can do
- 10:15this
- 10:15with a
- 10:16Perccutaneous
- 10:16ablation
- 10:17versus
- 10:17I need
- 10:17to
- 10:18treat the
- 10:18entire
- 10:18liver
- 10:19with
- 10:19hepzato
- 10:19or an
- 10:19embolization
- 10:20or
- 10:20something
- 10:20like that.
- 10:21That's a
- 10:21great
- 10:22question.
- 10:23the biggest
- 10:23thing
- 10:23is size
- 10:25and tumor
- 10:26number.
- 10:27if
- 10:27if you have
- 10:28somebody
- 10:28that has
- 10:29one, two,
- 10:31metastases
- 10:32in
- 10:32the liver
- 10:32that are
- 10:33smaller
- 10:33than three
- 10:33centimeters
- 10:34is kind of
- 10:34our
- 10:34cutoff.
- 10:35then I
- 10:36think the,
- 10:37approach
- 10:38would
- 10:38be to
- 10:39try and
- 10:39treat
- 10:39both
- 10:40of those,
- 10:41with
- 10:41ablation
- 10:41because
- 10:42we know
- 10:42we have
- 10:43data just
- 10:43in general
- 10:44with
- 10:44ablation
- 10:44that,
- 10:45it can be
- 10:46roughly
- 10:47equivalent
- 10:47to,
- 10:48having
- 10:49surgical
- 10:49resection.
- 10:49it's all
- 10:50about kind
- 10:51of number
- 10:51of tumor.
- 10:52Now once
- 10:52you start
- 10:52getting
- 10:53multiple
- 10:53tumors,
- 10:53then then
- 10:55you
- 10:55have
- 10:55to kind
- 10:55of start
- 10:56thinking
- 10:56of more
- 10:57distribution
- 10:57of the
- 10:58therapy,
- 10:59where you
- 10:59can kind
- 10:59of treat,
- 11:02the liver
- 11:03in
- 11:03totality,
- 11:04because,
- 11:05you can
- 11:05only treat
- 11:062 or 3
- 11:06lesions,
- 11:07with the
- 11:08percutaneous
- 11:08ablation
- 11:09technologies.
- 11:10Yeah,
- 11:11that's a
- 11:11great question.
- 11:11So I'll
- 11:12just
- 11:12repeat it
- 11:12so
- 11:12everyone
- 11:13on zoom
- 11:13can hear.
- 11:14with these
- 11:14local
- 11:14therapies
- 11:15we can
- 11:15treat the
- 11:16disease
- 11:16in the liver.
- 11:17But
- 11:17as we've
- 11:17talked
- 11:17about,
- 11:18there's
- 11:18a chance
- 11:18it could
- 11:19spread
- 11:19outside.
- 11:19So
- 11:19is there
- 11:20any
- 11:20thought
- 11:20about
- 11:21trying
- 11:21to combine
- 11:22systemic
- 11:22and liver
- 11:23directed
- 11:24therapies.
- 11:25we do that
- 11:25for
- 11:25some patients
- 11:26just
- 11:26because
- 11:27if they
- 11:27have a
- 11:27high
- 11:27disease,
- 11:28a burden,
- 11:28we feel
- 11:29like the
- 11:29systemic
- 11:30therapy
- 11:30may not be
- 11:31strong
- 11:31enough to
- 11:31shrink it.
- 11:32So we'll
- 11:32give the
- 11:33liver
- 11:33directed
- 11:33therapy
- 11:34with that.
- 11:35In terms
- 11:36of data,
- 11:36we are getting
- 11:37some data.
- 11:38So I think
- 11:39it's Kevin
- 11:39Kim at
- 11:40Sutter
- 11:40Health
- 11:41has done
- 11:41a clinical
- 11:41trial
- 11:42for a
- 11:42while
- 11:42combining
- 11:43ipi-nivo
- 11:44with radio
- 11:44embolization.
- 11:45I don't
- 11:45believe
- 11:46we've
- 11:46seen the
- 11:46full
- 11:46results
- 11:47from that.
- 11:48I know
- 11:48early on
- 11:49if you did
- 11:49them both
- 11:50at
- 11:50the same
- 11:50time, that
- 11:51there's
- 11:52a little
- 11:52risk for
- 11:52toxicity,
- 11:53so
- 11:53staggering
- 11:54them out,
- 11:54it's
- 11:54probably
- 11:54a little
- 11:54bit
- 11:55better.
- 11:55There's
- 11:55also a
- 11:56couple
- 11:56of
- 11:56different
- 11:57clinical
- 11:57trials
- 11:58going on
- 11:58with
- 11:58Delcath
- 11:59right now
- 12:00in Europe,
- 12:01where
- 12:01they're
- 12:01combining
- 12:02both
- 12:02the hepzado kit
- 12:03So
- 12:04the percutaneous
- 12:04hepatic
- 12:05perfusion
- 12:06with
- 12:06nivolumab
- 12:07and in
- 12:07ipilimab
- 12:08for
- 12:08patients
- 12:09with you
- 12:09for
- 12:09melanoma.
- 12:10So people
- 12:10are
- 12:11looking
- 12:11at that.
- 12:11We don't have the data.
- 12:13But
- 12:14I think
- 12:14for
- 12:14the patients
- 12:15where
- 12:15you say
- 12:15I need to
- 12:16treat
- 12:16stuff
- 12:16outside
- 12:17the liver,
- 12:18but also
- 12:18there's
- 12:18a lot
- 12:19of disease
- 12:19in the liver.
- 12:19And I
- 12:20needed to
- 12:20bulk it
- 12:21or shrink
- 12:21it
- 12:21because
- 12:21it's going
- 12:22to start
- 12:22causing
- 12:22side
- 12:23effects
- 12:23such
- 12:23as pain
- 12:24or other
- 12:25issues.
- 12:25Doing
- 12:26the combination
- 12:27is a
- 12:27reasonable
- 12:27approach,
- 12:28but
- 12:29as everything
- 12:29in
- 12:29medicine
- 12:30more,
- 12:31maybe
- 12:31better,
- 12:32but more
- 12:32also maybe
- 12:33more
- 12:33toxic too.
- 12:34So
- 12:34we have to
- 12:35balance
- 12:35out all of
- 12:36those
- 12:36different
- 12:36things.
- 12:36But I'm
- 12:37really
- 12:37looking
- 12:37forward to
- 12:37some of
- 12:38those
- 12:38trials
- 12:38reaching
- 12:39out to
- 12:39really
- 12:40show us
- 12:40if there's
- 12:40benefit
- 12:41and
- 12:41synergy
- 12:41between
- 12:42the two.
- 12:42when
- 12:42cancer
- 12:43starts, it
- 12:43starts in
- 12:43one place.
- 12:44in terms
- 12:45of uveal
- 12:45melanoma,
- 12:46the cancer
- 12:46started in
- 12:47your eye.
- 12:48and then,
- 12:49unfortunately,
- 12:50as the
- 12:50cancer
- 12:50progresses,
- 12:51it figures
- 12:52out ways
- 12:52to hide
- 12:52from your
- 12:53immune
- 12:53system,
- 12:53your immune
- 12:54system
- 12:55typically
- 12:55kills
- 12:56bacteria
- 12:56and
- 12:56viruses.
- 12:57It also
- 12:57kills
- 12:58cancer
- 12:58cells.
- 12:59And when
- 13:00the cancer
- 13:00figures
- 13:00out how to
- 13:01get around
- 13:01your
- 13:01immune
- 13:01system,
- 13:02it can
- 13:03then
- 13:03spread to
- 13:03other places.
- 13:04And that's
- 13:04typically
- 13:04through
- 13:05your
- 13:05lymphatics
- 13:06and
- 13:06your blood.
- 13:07And
- 13:07then it
- 13:07it shows
- 13:08up as
- 13:09is tumors
- 13:09and other
- 13:10parts of
- 13:10your body.
- 13:10uveal
- 13:11melanoma
- 13:11specifically
- 13:12has the
- 13:13highest
- 13:13propensity
- 13:13to to
- 13:14spread to
- 13:14your liver
- 13:15most of the time.
- 13:16But
- 13:17generally
- 13:17speaking,
- 13:18the cancer
- 13:18cells have
- 13:19started
- 13:19somewhere.
- 13:20And
- 13:20they're
- 13:20then
- 13:21dispersed
- 13:21throughout
- 13:21your body.
- 13:22It's just
- 13:22you're
- 13:22seeing
- 13:23the tumor
- 13:23somewhere
- 13:23else.
- 13:24And that's
- 13:24why
- 13:24things
- 13:25like
- 13:25circulating
- 13:25tumor DNA,
- 13:26which is
- 13:27a
- 13:27biomarker
- 13:28of the
- 13:28tumor,
- 13:29is so
- 13:29important
- 13:29in
- 13:30the blood
- 13:30for many
- 13:31of these
- 13:31treatments.
- 13:31That's
- 13:32so there's
- 13:32this.
- 13:32Type
- 13:33of seed
- 13:33and soil.
- 13:34Thought
- 13:35about
- 13:35metastasis,
- 13:36cancer
- 13:37spreading.
- 13:37We
- 13:37don't have
- 13:38a great
- 13:38understanding
- 13:39of why
- 13:39some
- 13:39cancers
- 13:40spread to
- 13:40certain
- 13:41places
- 13:41and others
- 13:41not,
- 13:42not to
- 13:42different
- 13:42parts of
- 13:43the body.
- 13:43And that's
- 13:43still
- 13:43something
- 13:44we're
- 13:44we're kind
- 13:44of
- 13:44learning.
- 13:45but I
- 13:45would love
- 13:45to get
- 13:46Doctor
- 13:46ma
- 13:46thoughts
- 13:46about,
- 13:47when
- 13:47looking at
- 13:48a patient
- 13:48who needs
- 13:49liver
- 13:49directed
- 13:49therapy,
- 13:50how does he
- 13:51choose
- 13:51hepzado
- 13:51out of
- 13:52versus,
- 13:52an ablation
- 13:53or some
- 13:53of these
- 13:53other
- 13:54different,
- 13:54local
- 13:55therapies
- 13:55that are
- 13:55out there?
- 13:57I
- 13:57believe I
- 13:57heard
- 13:58from,
- 13:59doctor
- 13:59Moser
- 14:00about,
- 14:00one of
- 14:00the case
- 14:01examples
- 14:01that
- 14:02we had
- 14:02displayed
- 14:03up there.
- 14:03what was
- 14:04interesting
- 14:04was,
- 14:05we were one
- 14:05of the
- 14:05very few,
- 14:07centers
- 14:07in the
- 14:07US that,
- 14:08had
- 14:08hepzado
- 14:09available
- 14:09when it became
- 14:10FDA
- 14:10approved.
- 14:10we treated
- 14:11the number
- 14:11of
- 14:11patients
- 14:12at this
- 14:12point.
- 14:13one of the case
- 14:14examples
- 14:14that we
- 14:15submitted
- 14:15or had
- 14:16displayed
- 14:16was
- 14:18a patient
- 14:18who
- 14:19we found
- 14:20actually
- 14:21had a
- 14:21pretty
- 14:22significant
- 14:22response
- 14:23to
- 14:23treatment.
- 14:24after one
- 14:24cycle
- 14:25of therapy
- 14:26this was
- 14:26actually
- 14:26our first
- 14:27patient.
- 14:27And so,
- 14:28we were
- 14:29fortunate
- 14:30to
- 14:30actually
- 14:30have
- 14:31one of
- 14:32our
- 14:32interventional
- 14:33radiologists.
- 14:34is able
- 14:34to obtain
- 14:35images
- 14:36at
- 14:36the time
- 14:36of the
- 14:36procedure.
- 14:37And so
- 14:37by
- 14:37the time
- 14:38the
- 14:38patient
- 14:38received
- 14:39their
- 14:39second
- 14:40dose
- 14:40or prior
- 14:41to their
- 14:41second
- 14:42dose,of
- 14:42hepzado
- 14:43we were
- 14:43actually
- 14:43able to
- 14:45visualize
- 14:45how much
- 14:46their
- 14:46disease
- 14:47burden,
- 14:47the
- 14:48metastatic
- 14:48uveal
- 14:49melanoma
- 14:50burden in
- 14:50a liver,
- 14:50was able
- 14:51to shrink,
- 14:52almost
- 14:53greater
- 14:53than 50%
- 14:54after a
- 14:54six week
- 14:55interval
- 14:55therapy.
- 14:57And, we're
- 14:57pleased
- 14:58to say
- 14:58that he
- 14:58continues,
- 14:59to respond
- 15:00well
- 15:00to therapy
- 15:01and, and
- 15:01receive
- 15:02their
- 15:02fourth
- 15:02dose,
- 15:03this past
- 15:04week.
- 15:05and so,
- 15:06what's
- 15:06great,
- 15:06as opposed
- 15:07to some of
- 15:08the other
- 15:08systemic
- 15:08therapy
- 15:09options,
- 15:10is that
- 15:10with this
- 15:11being sort
- 15:12of a liver
- 15:12directed
- 15:13therapy,
- 15:13he's
- 15:14actually
- 15:14kind of
- 15:14feeling
- 15:15minimal,
- 15:15toxicity
- 15:16associated
- 15:17with it.
- 15:17Great.
- 15:18thanks for
- 15:18that.
- 15:18Vince, and
- 15:19in addition,
- 15:20we were
- 15:20discussing
- 15:21how to
- 15:21choose
- 15:21liver
- 15:22directed
- 15:22therapies
- 15:23and
- 15:23with your
- 15:23experience
- 15:24with the
- 15:24Hepzado
- 15:24kit
- 15:25Could you
- 15:25walk us
- 15:26through,
- 15:26when
- 15:27you look
- 15:27at a patient,
- 15:27what guides
- 15:28you to
- 15:28say?
- 15:29I think
- 15:29we should
- 15:30do an
- 15:30embolization
- 15:31or an
- 15:31ablation
- 15:32or hepzado
- 15:33choose
- 15:34the patients
- 15:35that you
- 15:35think
- 15:36would
- 15:36benefit
- 15:36most from
- 15:37hepzado
- 15:37versus
- 15:38these other
- 15:38liver
- 15:38directed
- 15:39therapies?
- 15:41Yeah,
- 15:41that's a
- 15:41great
- 15:41question.
- 15:43oftentimes
- 15:45when I
- 15:45read,
- 15:46a patient
- 15:46with
- 15:47metastatic
- 15:47melanoma,
- 15:48in
- 15:48my clinic,
- 15:49we often
- 15:50times
- 15:50review
- 15:50the
- 15:50imaging
- 15:51together
- 15:51and,
- 15:52and also
- 15:53discuss
- 15:53the case,
- 15:54within a
- 15:54multidisciplinary
- 15:56setting.
- 15:57so we have
- 15:57specialists,
- 15:58including,
- 15:58surgeons,
- 15:59radiation
- 16:00oncologist,
- 16:01interventional
- 16:02radiology
- 16:03present
- 16:03at these
- 16:03tumor
- 16:04board.
- 16:04And,
- 16:05we'll
- 16:05extensively
- 16:06review
- 16:06the
- 16:07imaging,
- 16:07particularly
- 16:08the liver.
- 16:09and at
- 16:09that
- 16:09point,
- 16:10we'll kind
- 16:10of assess,
- 16:11for
- 16:11example,
- 16:11how much,
- 16:12disease
- 16:13involvement
- 16:13is there
- 16:14specifically
- 16:15in the
- 16:15liver,
- 16:15for cases
- 16:16where
- 16:16we're
- 16:16seeing,
- 16:17maybe one,
- 16:18maybe
- 16:18up to
- 16:19five
- 16:19lesions,
- 16:20where
- 16:20it is
- 16:21quite
- 16:21accessible
- 16:22by the
- 16:22surgeon.
- 16:23we'll
- 16:23often
- 16:24times
- 16:24consider a
- 16:25resection
- 16:25of the
- 16:26disease.
- 16:27by
- 16:27surgery.
- 16:28But, for
- 16:29some cases
- 16:30where,
- 16:30surgery
- 16:30may not
- 16:31be
- 16:31permissible,
- 16:32and
- 16:32we want to
- 16:33take what
- 16:33we call
- 16:33a more
- 16:34palatable
- 16:34approach,
- 16:35less
- 16:35invasive.
- 16:36I will
- 16:37sometimes
- 16:37consider
- 16:38just using
- 16:38radiation
- 16:39therapy
- 16:39alone,
- 16:41for cases
- 16:41where
- 16:42there's
- 16:42maybe
- 16:42multifocal
- 16:43disease
- 16:44these
- 16:44factors,
- 16:45extensive
- 16:46disease
- 16:47involvement
- 16:47of the
- 16:48liver,
- 16:48will often
- 16:49at that
- 16:49point
- 16:50really
- 16:50think
- 16:50about,
- 16:51using sort
- 16:52of THP
- 16:52or
- 16:54perfusion
- 16:54therapy
- 16:55with
- 16:55hepzado
- 16:56and one of
- 16:57the nice
- 16:57advantages
- 16:58of hepzado
- 16:58is that
- 16:59the
- 16:59ability
- 16:59to
- 17:00saturate
- 17:01or treat
- 17:01the entire
- 17:02liver,
- 17:02with
- 17:03chemotherapy
- 17:03in
- 17:03multiple
- 17:04and
- 17:05in those
- 17:05cases, you
- 17:06will also
- 17:07assess
- 17:07the patient's
- 17:08general
- 17:08candidacy
- 17:09for
- 17:09therapy
- 17:10because as
- 17:11of right
- 17:11now,
- 17:11the FDA
- 17:12approval
- 17:12for
- 17:13hepzado
- 17:13that, less
- 17:13than
- 17:1450% of
- 17:15the liver
- 17:15has been
- 17:16by the
- 17:17disease
- 17:17itself.
- 17:18We also have
- 17:19to
- 17:19make sure
- 17:19that
- 17:19there
- 17:19is no
- 17:20other
- 17:20sites
- 17:20of disease
- 17:21elsewhere
- 17:22outside
- 17:22the liver
- 17:23in order
- 17:24to be
- 17:24a candidate.
- 17:25but
- 17:25if there is
- 17:26disease
- 17:26outside
- 17:27the liver,
- 17:27we'll
- 17:28oftentimes
- 17:28need to
- 17:28consider
- 17:29definitive
- 17:30surgery
- 17:30or
- 17:30radiation.
- 17:31For
- 17:31example,
- 17:31if there's
- 17:32metastatic
- 17:33disease
- 17:33in the
- 17:33lungs,
- 17:34bones
- 17:34or skin,
- 17:36those do
- 17:36need to be
- 17:37treated
- 17:37before
- 17:37consideration.
- 17:38hepzado
- 17:40But
- 17:40generally
- 17:40speaking,
- 17:41for the
- 17:42liver,
- 17:42only
- 17:42disease
- 17:43will
- 17:43definitely
- 17:43strongly.
- 17:44Consider
- 17:44a liver
- 17:44directed
- 17:45therapy.
- 17:46generally
- 17:47speaking,
- 17:47because
- 17:48we know
- 17:48how
- 17:49extensive
- 17:50and how
- 17:50morbid
- 17:52liver
- 17:52involvements
- 17:53by cancer
- 17:53can be.
- 17:54and so
- 17:55it's
- 17:55important
- 17:55to really,
- 17:56be
- 17:57aggressive
- 17:57with
- 17:58treating
- 17:58the disease
- 17:58as much
- 17:59as
- 17:59possible.
- 18:00and
- 18:01especially
- 18:02for,
- 18:02melanoma,
- 18:03where 90,
- 18:0495% of
- 18:05patients
- 18:05have,
- 18:07disease
- 18:07involving
- 18:07the liver
- 18:08at the
- 18:08time of
- 18:08stage four
- 18:09diagnosis.
- 18:11it's a
- 18:11significant
- 18:12cause for
- 18:13white
- 18:13patients
- 18:13overall
- 18:14decline.
- 18:15and
- 18:16.........
- 18:17if
- 18:17the disease
- 18:18itself
- 18:18is not,
- 18:19well
- 18:19managed,
- 18:20quickly
- 18:20and
- 18:20efficiently.
- 18:21Great.
- 18:21I think
- 18:22that was a
- 18:22great
- 18:22discussion.
- 18:23Any questions
- 18:24from the
- 18:24audience
- 18:25or any
- 18:26questions
- 18:27from
- 18:27the zoom
- 18:28back
- 18:28in the back?
- 18:28How
- 18:29rapidly.
- 18:30Can the
- 18:31cancer.
- 18:31Grow
- 18:31within
- 18:32the liver?
- 18:33So if
- 18:33someone
- 18:33is.
- 18:33Scanned
- 18:34every six
- 18:34months.
- 18:35Month one,
- 18:36they're
- 18:36clear.
- 18:37Month six
- 18:38could it
- 18:38be more
- 18:38than.
- 18:3950% of
- 18:39the liver
- 18:40involved
- 18:40with
- 18:41cancer?
- 18:41Typically
- 18:42not.
- 18:42we know
- 18:43from the
- 18:43updated
- 18:44coog
- 18:442.1 study
- 18:45which
- 18:45Doctor
- 18:46Harbor
- 18:46we'll
- 18:47talk about
- 18:47in a
- 18:48prerecorded,
- 18:49lecture
- 18:49later
- 18:49today
- 18:50that
- 18:51if you
- 18:51have uveal
- 18:52melanoma
- 18:53and you're
- 18:53getting
- 18:54scans
- 18:54the median
- 18:55time to
- 18:56developing
- 18:56disease
- 18:57in the
- 18:57liver,
- 18:58it depends
- 18:59a little
- 18:59bit
- 18:59on your
- 18:59genetic
- 19:00risk.
- 19:00But
- 19:00the earliest
- 19:01is kind
- 19:01of a year
- 19:02and
- 19:02a half.
- 19:02And
- 19:02sometimes
- 19:03it's up
- 19:03to 2 to 2
- 19:04in a year
- 19:05or 2 or 2
- 19:05and a
- 19:06half
- 19:06years.
- 19:07Now
- 19:07obviously
- 19:08there's
- 19:08extremes.
- 19:09Have I
- 19:09seen
- 19:09people go
- 19:10from
- 19:10nothing
- 19:11to
- 19:11significant
- 19:12disease
- 19:12in six
- 19:12months?
- 19:13Yes,
- 19:13but that's
- 19:14very rare
- 19:15most
- 19:15of the time.
- 19:16And if you
- 19:17have,
- 19:18kind of
- 19:18intermediate
- 19:19or low
- 19:19risk, it's
- 19:20going
- 19:20to grow
- 19:20fairly
- 19:21slow.
- 19:22And so
- 19:22there
- 19:23won't be
- 19:23much
- 19:24change
- 19:24within
- 19:25six
- 19:25months.
- 19:25If
- 19:25you're a
- 19:26high risk,
- 19:26most of us
- 19:27will do
- 19:27scans
- 19:28every
- 19:28three
- 19:28months
- 19:29after your
- 19:29treatment.
- 19:30Although
- 19:30those are
- 19:31those
- 19:31guidelines
- 19:31are
- 19:32very soft.
- 19:32And so
- 19:33there's a
- 19:33lot of
- 19:34variability
- 19:34there.
- 19:35And so
- 19:35we
- 19:36wouldn't
- 19:36catch
- 19:36that.
- 19:36But it's
- 19:37very rare
- 19:37that
- 19:37someone's
- 19:38cancer
- 19:38grows
- 19:38that fast.
- 19:40Unfortunately.
- 19:40The reality
- 19:41is
- 19:42if you're
- 19:43if your
- 19:43cancer
- 19:44is growing
- 19:44that fast,
- 19:46that's a
- 19:46reflection
- 19:47of just
- 19:47how
- 19:47aggressive
- 19:48the cancer
- 19:48is.
- 19:49And
- 19:49it means
- 19:50that
- 19:50you're
- 19:50less
- 19:50likely
- 19:51to respond
- 19:51to
- 19:51the therapy.
- 19:52So
- 19:52a lot of
- 19:52people in
- 19:53those
- 19:53situations
- 19:53will
- 19:54think, oh,
- 19:54I should
- 19:55have done
- 19:55something
- 19:55different.
- 19:56I should have
- 19:56scanned
- 19:56sooner.
- 19:57But that
- 19:58may not
- 19:58have
- 19:58always
- 19:58changed
- 19:59anything
- 19:59because at
- 20:00the end,
- 20:00at the
- 20:01end of the day,
- 20:01it was the
- 20:02cancer's
- 20:02aggressiveness,
- 20:03not
- 20:04something
- 20:05you did
- 20:05or your
- 20:05physician
- 20:06did or
- 20:06something
- 20:07like that.
- 20:07Okay,
- 20:08so I'll
- 20:08throw a
- 20:08question
- 20:09to Doctor
- 20:09Low then.
- 20:09So
- 20:12most of
- 20:12us these
- 20:12days are
- 20:13doing
- 20:13surveillance
- 20:14scans
- 20:14for
- 20:14patients
- 20:15who have
- 20:15treated
- 20:15uveal
- 20:15melanoma.
- 20:16So we are
- 20:16catching
- 20:17patients
- 20:17with liver
- 20:18metastases
- 20:19when they
- 20:19may have 1
- 20:20or 2 liver
- 20:21metastases
- 20:22or
- 20:23very low
- 20:23disease
- 20:24burden.
- 20:25So when
- 20:26we're
- 20:26seeing
- 20:26these
- 20:26patients,
- 20:26a
- 20:27lot of times
- 20:27we consider
- 20:27them for
- 20:28a liver
- 20:28directed
- 20:28therapy.
- 20:29If
- 20:30a patient
- 20:30were to
- 20:30ask you
- 20:31and say
- 20:31okay,
- 20:32of all
- 20:32the liver
- 20:32directed
- 20:33therapies
- 20:33ablation
- 20:34embolization,
- 20:35either
- 20:35radio
- 20:36or immuno,
- 20:37immobilization
- 20:37or hepzado
- 20:37kit
- 20:38what are the side
- 20:39effects
- 20:39and
- 20:39what do you think
- 20:40is kind of
- 20:40the most
- 20:41side
- 20:42effect one
- 20:43or what
- 20:44is the
- 20:44easiest
- 20:44one to do?
- 20:45just kind
- 20:46of help
- 20:46guide that
- 20:46discussion
- 20:47about
- 20:48what is it
- 20:48like for
- 20:49the patients
- 20:49to go
- 20:49through
- 20:50when
- 20:50we think
- 20:50they're a
- 20:50good
- 20:50candidate
- 20:51for any of
- 20:51these
- 20:51liver
- 20:52directed
- 20:52therapies?
- 20:53that's a
- 20:53really good point.
- 20:53They all
- 20:54do have
- 20:54their own
- 20:55kind of
- 20:55separate,
- 20:57unique
- 20:57side
- 20:57effects
- 20:58for each
- 20:58treatment.
- 20:59So if
- 21:00we're
- 21:00talking
- 21:00about,
- 21:01ablation,
- 21:02first of
- 21:02all, in
- 21:03general,
- 21:03they do
- 21:04require
- 21:04the procedure
- 21:05to be done
- 21:05under
- 21:06general
- 21:06anesthesia.
- 21:07they often
- 21:07will
- 21:08require
- 21:08overnight
- 21:09admission
- 21:09to
- 21:09the hospital.
- 21:10they're
- 21:10generally
- 21:11well
- 21:11tolerated,
- 21:11but they
- 21:12they can
- 21:12be
- 21:13painful.
- 21:13You can have
- 21:14discomfort,
- 21:14from from
- 21:15the
- 21:16ablation
- 21:16itself.
- 21:17and they,
- 21:18do have
- 21:18an
- 21:19infection
- 21:19risk
- 21:20as well.
- 21:20just
- 21:20because,
- 21:21we're
- 21:22destroying
- 21:22the,
- 21:23the area
- 21:23that
- 21:23we're,
- 21:24we're
- 21:24either
- 21:24heating
- 21:25or
- 21:25cooling,
- 21:26in the
- 21:26liver.
- 21:27now,
- 21:27when we're
- 21:28talking
- 21:28about
- 21:28transarterial
- 21:29therapies,
- 21:30as I said,
- 21:31the chemo
- 21:32embolization
- 21:33or,
- 21:34kind
- 21:34of the
- 21:34embolic
- 21:35technology
- 21:36or
- 21:36treatments,
- 21:37they
- 21:37tend to
- 21:38be,
- 21:38painful.
- 21:40just
- 21:40because,
- 21:41we are
- 21:41intentionally
- 21:42blocking
- 21:42off the
- 21:43blood flow
- 21:43to, to
- 21:44whatever
- 21:45area of
- 21:45the liver
- 21:46we decide.
- 21:46So,
- 21:47I tell
- 21:48patients
- 21:48that,
- 21:49you
- 21:49will have
- 21:50pain,
- 21:50and
- 21:51we just
- 21:51have to do
- 21:52our best
- 21:52to, manage
- 21:53that, for
- 21:53the, week
- 21:54or so
- 21:55it
- 21:55takes for
- 21:55your body
- 21:56to,
- 21:56to recover
- 21:57from that,
- 21:58y90 is
- 22:00very well
- 22:00tolerated
- 22:01from a
- 22:01pain
- 22:01standpoint.
- 22:03the most
- 22:04common
- 22:04thing
- 22:04people
- 22:05have is,
- 22:05is kind of
- 22:06just a
- 22:06generalized
- 22:07flu like
- 22:08illness.
- 22:08So just
- 22:09a few days
- 22:10of,
- 22:10fatigue,
- 22:12nausea,
- 22:13things
- 22:13like that.
- 22:14Now, there
- 22:14are some
- 22:15serious
- 22:15side
- 22:15effects
- 22:16that can
- 22:16happen
- 22:17from y90
- 22:18you can get
- 22:19ulcerations
- 22:19in your
- 22:20stomach,
- 22:21intestines.
- 22:22you can
- 22:22get
- 22:22radiation
- 22:23fibrosis
- 22:24to your lungs.
- 22:25but
- 22:25those are
- 22:26things
- 22:26that we,
- 22:27are very
- 22:28careful
- 22:29about
- 22:30not causing.
- 22:31And so
- 22:31that's why
- 22:32anyone
- 22:33we do
- 22:33actually
- 22:33has a
- 22:34procedure
- 22:34beforehand
- 22:35where we
- 22:36completely
- 22:37map out
- 22:37the blood
- 22:38flow
- 22:38to the liver,
- 22:39and find
- 22:40any of
- 22:40those
- 22:40little
- 22:41arteries
- 22:41that could
- 22:42potentially
- 22:43go to
- 22:43the
- 22:43stomach
- 22:44that could
- 22:44cause
- 22:44problems.
- 22:45And we do
- 22:47actually
- 22:47quantify
- 22:48how much,
- 22:49radiation
- 22:50will be
- 22:50given to
- 22:51the lungs
- 22:51as well.
- 22:52during
- 22:53our, our
- 22:53planning
- 22:54phase.
- 22:55And, with
- 22:56the
- 22:56Hepzado,
- 22:57we've,
- 22:59seen that,
- 22:59overall
- 23:00the,
- 23:00the systemic
- 23:01effects
- 23:02are quite
- 23:03mild.
- 23:03Most
- 23:04commonly
- 23:04people
- 23:05will have
- 23:05some,
- 23:06gi upset
- 23:07or nausea
- 23:08related
- 23:08to it.
- 23:10and, that
- 23:11also
- 23:11requires
- 23:11an
- 23:12overnight
- 23:12admission
- 23:13to
- 23:13the hospital
- 23:14just for,
- 23:14for
- 23:14monitoring
- 23:15and,
- 23:16we can
- 23:16induce,
- 23:17low
- 23:17platelets,
- 23:18different
- 23:19things
- 23:20in the
- 23:20blood
- 23:20profile
- 23:21that we
- 23:21require
- 23:22blood
- 23:22transfusions
- 23:23and things
- 23:23like that
- 23:23with
- 23:24hepzado
- 23:24as well.
- 23:25So
- 23:25one of the treatments
- 23:26that's
- 23:26commonly
- 23:27used for
- 23:27early
- 23:28or low
- 23:29volume
- 23:29disease
- 23:30in the
- 23:30liver
- 23:30at
- 23:30a lot of
- 23:31centers
- 23:31around
- 23:31the US
- 23:32is immuno
- 23:32embolization,
- 23:33which is
- 23:33given kind
- 23:34of
- 23:35repetitively
- 23:35every
- 23:36eight
- 23:36weeks
- 23:37for up
- 23:37to
- 23:37sometimes
- 23:386-10
- 23:39treatments.
- 23:39Is there
- 23:40any risk
- 23:40to
- 23:41repetitively
- 23:42embolising
- 23:42your liver
- 23:43every
- 23:43two months
- 23:44for that
- 23:44number of
- 23:44treatments,
- 23:45or do
- 23:46you
- 23:46typically
- 23:46see that
- 23:46it's well
- 23:47tolerated
- 23:47throughout?
- 23:49so it's
- 23:50usually
- 23:51well
- 23:51tolerated.
- 23:52And given
- 23:53a few
- 23:53weeks
- 23:53separation,
- 23:54the liver
- 23:55recovers,
- 23:56the immuno
- 23:58embolization
- 23:59doesn't
- 23:59appear
- 24:00to have as
- 24:02toxic
- 24:02effects
- 24:03on the
- 24:03blood
- 24:03vessels.
- 24:04So
- 24:04whenever
- 24:04we're
- 24:04going in
- 24:05to do a
- 24:05transarterial
- 24:06treatment,
- 24:07when
- 24:07we look
- 24:08at the
- 24:08small
- 24:08blood
- 24:08vessels
- 24:09in the liver,
- 24:10if we do
- 24:10multiple
- 24:11of those
- 24:12procedures
- 24:12where
- 24:12we give
- 24:13the chemo
- 24:13embolization,
- 24:15that can
- 24:15really
- 24:15affect,
- 24:17the blood
- 24:17vessels
- 24:17over time.
- 24:18But the
- 24:18Immuno
- 24:18embolization
- 24:19doesn't
- 24:20seem to,
- 24:21to do that
- 24:21sort of
- 24:22allows
- 24:22us to,
- 24:22to kind
- 24:23of do
- 24:23the
- 24:24treatment
- 24:24repetitively.
- 24:25more so
- 24:25the issue
- 24:26with
- 24:26repeated
- 24:27trans
- 24:28arterial
- 24:28treatments
- 24:28is we
- 24:29start
- 24:29running
- 24:29out of
- 24:30access
- 24:30sites.
- 24:30So because
- 24:31we can
- 24:31only
- 24:31use the
- 24:32wrist or,
- 24:33or the
- 24:34arteries
- 24:34in the
- 24:34groin.
- 24:35So,
- 24:35there's
- 24:36kind of
- 24:36we have to
- 24:37be
- 24:38judicious
- 24:38about
- 24:39where
- 24:39we're
- 24:39accessing
- 24:40for these
- 24:40procedures
- 24:41just
- 24:41to kind of
- 24:42let the,
- 24:42the access
- 24:43arteries
- 24:43heal.
- 24:44whenever
- 24:45we're
- 24:45kind of,
- 24:46doing
- 24:47consecutive
- 24:47treatments
- 24:48over, over
- 24:48time.
- 24:49For any
- 24:49last
- 24:50questions
- 24:51from the
- 24:51audience
- 24:52before
- 24:52we
- 24:53wrap this
- 24:53session
- 24:53up early.
- 24:55That's a
- 24:56great
- 24:56question.
- 24:56So just
- 24:57to repeat
- 24:57that for
- 24:57zoom.
- 24:58So if we
- 24:58treat
- 24:58a lesion
- 24:59in
- 24:59the liver,
- 25:00does it
- 25:00go away
- 25:00on the scans
- 25:01or is
- 25:02something
- 25:02still
- 25:02there
- 25:03most of
- 25:03the time
- 25:04we will
- 25:04see some
- 25:05shrinkage
- 25:05or some
- 25:05changes,
- 25:06but
- 25:06there will be
- 25:07something
- 25:08there.
- 25:08I think
- 25:09one of
- 25:09the things
- 25:09we've
- 25:10learned
- 25:10about
- 25:11uveal
- 25:11melanoma,
- 25:12liver
- 25:12metastases
- 25:13in
- 25:13the past
- 25:13couple of years
- 25:14is that
- 25:15the way
- 25:15the liver
- 25:16heals,
- 25:16the scans
- 25:17never
- 25:18look like
- 25:19it's fully
- 25:20gone away.
- 25:20But
- 25:21if we do
- 25:21other
- 25:21tests like
- 25:22circulating
- 25:22tumor DNA,
- 25:23which
- 25:23we'll talk
- 25:24about
- 25:24later,
- 25:25or these
- 25:25other
- 25:25tests
- 25:26to say,
- 25:26is there
- 25:27cancer
- 25:27still in
- 25:27the body,
- 25:28we're
- 25:28finding
- 25:29that
- 25:29those may
- 25:29be more
- 25:30accurate
- 25:30for
- 25:30determining
- 25:31response,
- 25:32because
- 25:32the liver
- 25:33doesn't
- 25:33always
- 25:33shrink
- 25:34as much
- 25:34as we
- 25:34would
- 25:34expect.
- 25:35if you
- 25:36treat
- 25:36something
- 25:36in the lung,
- 25:36it tends
- 25:37to fully
- 25:37go away.
- 25:38We just don't
- 25:38see that
- 25:39in the liver.
- 25:40So usually
- 25:40there
- 25:41may be
- 25:41some
- 25:41shrinkage,
- 25:42but
- 25:42otherwise
- 25:43there's
- 25:43normally
- 25:43something
- 25:44there.
- 25:44Either
- 25:44it's
- 25:44scars,
- 25:45it becomes
- 25:46hollow and
- 25:46cystic.
- 25:47It just
- 25:48changes
- 25:48textures.
- 25:49It loses
- 25:49enhancement.
- 25:50but
- 25:51usually
- 25:51the scans
- 25:51don't
- 25:52completely
- 25:52go away.
- 25:53I
- 25:54have a question
- 25:54just based
- 25:55on.
- 25:55I've had
- 25:55a few
- 25:56patients
- 25:56reach
- 25:56out to me
- 25:57who've had
- 25:57recent
- 25:58treatment,
- 25:58and,
- 25:59I've
- 25:59actually
- 25:59was
- 26:00talking
- 26:00to Doctor
- 26:01Johnny
- 26:01John,
- 26:01which
- 26:01I don't
- 26:02see him
- 26:02in here,
- 26:03so
- 26:03we may not
- 26:04have him
- 26:04to defer
- 26:05to.
- 26:05but I
- 26:06was
- 26:06curious.
- 26:07procedurally,
- 26:08when
- 26:09we talk
- 26:09about
- 26:09immuno
- 26:09embolization
- 26:11and chemo
- 26:11embolization,
- 26:12like in
- 26:13Philly,
- 26:13where
- 26:13Doctor
- 26:14soto and
- 26:14Doctor
- 26:14Orloff
- 26:15have
- 26:15performed
- 26:15that with
- 26:16doctor
- 26:16O'Donnell,
- 26:17there's
- 26:17this
- 26:17period
- 26:17of time
- 26:18where
- 26:18you're
- 26:18required
- 26:18to lay
- 26:19on
- 26:19your back.
- 26:20is that
- 26:21the case
- 26:21with
- 26:21hepzado
- 26:22kit?
- 26:22Why or
- 26:22why not?
- 26:23because
- 26:23I've heard
- 26:23mixed
- 26:24things
- 26:24from
- 26:24various
- 26:25different
- 26:25interventional
- 26:25radiology
- 26:26teams.
- 26:27the short
- 26:28answer
- 26:28is yes,
- 26:29there is.
- 26:29There
- 26:30will be
- 26:30a, minimum
- 26:32kind of
- 26:32three hours
- 26:33that
- 26:33you'll be
- 26:34in bed.
- 26:35Okay.
- 26:35with,
- 26:35with
- 26:36the hepzato
- 26:36kit.
- 26:37And
- 26:37the biggest
- 26:37thing is
- 26:38the
- 26:39arterial access.
- 26:40So
- 26:40any time
- 26:40you stick
- 26:41to
- 26:41the artery,
- 26:42we put a little
- 26:43sheath in,
- 26:44and
- 26:44we have
- 26:44to be
- 26:44very
- 26:44careful
- 26:45when
- 26:45we put
- 26:45people on
- 26:46bedrest.
- 26:46They can't
- 26:46bend their
- 26:47hip,
- 26:47during
- 26:48that time,
- 26:48just
- 26:48because,
- 26:49bleeding
- 26:50can occur
- 26:50and
- 26:51you can
- 26:51get
- 26:51hematomas
- 26:52and things
- 26:52like that
- 26:52at the,
- 26:53at the
- 26:53access
- 26:53site.
- 26:54so we're
- 26:54all, for
- 26:56the
- 26:56hepzado,
- 26:56it's,
- 26:57for access
- 26:58point.
- 26:58So it's
- 26:59both,
- 26:59sides of
- 27:00the neck,
- 27:00both
- 27:01groin,
- 27:02an
- 27:02arterial
- 27:02and venous
- 27:03in the groin.
- 27:04so there
- 27:04is,
- 27:05minimum
- 27:05three
- 27:06hour,
- 27:06we have
- 27:06devices
- 27:07we can use
- 27:07to plug
- 27:08up
- 27:08the hole
- 27:08in the artery.
- 27:10but
- 27:10that still
- 27:11requires
- 27:11three
- 27:12hours of
- 27:12of of bed
- 27:13rest,
- 27:13minimum.
- 27:14Okay.
- 27:14People are
- 27:14about six.
- 27:15Yeah.
- 27:16I was going
- 27:16to say
- 27:16I'm.
- 27:16I'm
- 27:17hearing a
- 27:17more
- 27:17common
- 27:17number is
- 27:18maybe
- 27:18about
- 27:18six hours
- 27:19of, of
- 27:19time.
- 27:20And
- 27:20is that
- 27:20going
- 27:20to kind
- 27:21of include
- 27:21some period
- 27:22of I mean,
- 27:22I just
- 27:23know
- 27:23anesthesia.
- 27:24Let's
- 27:24pretend,
- 27:24you're
- 27:24under
- 27:24anesthesia.
- 27:25That time
- 27:26counts
- 27:26as some of
- 27:27the time
- 27:27you're
- 27:27under
- 27:27as long
- 27:28as the
- 27:28the
- 27:29catheters
- 27:29have been
- 27:29removed.
- 27:31That's a
- 27:31good
- 27:31question.
- 27:32So I'm
- 27:32counting it
- 27:33when
- 27:33we get
- 27:33all the
- 27:33catheters
- 27:34out.
- 27:34So
- 27:34we're all,
- 27:35this is
- 27:35still
- 27:36fairly
- 27:36new.
- 27:37So, and
- 27:38each
- 27:38patient
- 27:38we're
- 27:38finding
- 27:39is a
- 27:39little bit
- 27:39different.
- 27:39And what
- 27:40they
- 27:40require
- 27:41from a
- 27:42blood
- 27:42product
- 27:42standpoint
- 27:43afterwards,
- 27:43whether
- 27:44they need
- 27:44platelets
- 27:45or,
- 27:46plasma,
- 27:47to take
- 27:48correct
- 27:49any
- 27:49of the coagulopathy
- 27:50that
- 27:51needs to
- 27:51be
- 27:51corrected
- 27:52before
- 27:52we can
- 27:52safely
- 27:53remove
- 27:54all those
- 27:54access
- 27:54points.
- 27:56so I think
- 27:56the more
- 27:57and more
- 27:57we do,
- 27:58the more
- 27:58we'll see
- 27:58and
- 27:59the more
- 27:59we'll
- 27:59be able
- 27:59to kind of
- 28:00shrink
- 28:00that that
- 28:01time to
- 28:01a time
- 28:02frame down
- 28:02where
- 28:02we can
- 28:03start,
- 28:03they have
- 28:03closure
- 28:04devices
- 28:04for veins
- 28:05that we
- 28:05can use,
- 28:06to kind of
- 28:07minimize
- 28:08that time
- 28:08where
- 28:09people
- 28:09are kind
- 28:10of stuck
- 28:10in bed.
- 28:10Yeah.
- 28:11And that
- 28:11makes
- 28:11sense too,
- 28:12the
- 28:12procedure
- 28:12would,
- 28:13as you go
- 28:13from new
- 28:14to doing
- 28:14the
- 28:14procedure
- 28:15to more,
- 28:15seasoned,
- 28:16I suppose
- 28:16that
- 28:16you would
- 28:17get
- 28:17faster,
- 28:17you'd get
- 28:17a little
- 28:18more
- 28:18efficient.
- 28:18The team
- 28:19kind of
- 28:19figures
- 28:19out
- 28:19the best
- 28:20ways to
- 28:20do
- 28:20everything.
- 28:21anyway.
- 28:21I feel like
- 28:22that helps
- 28:22answer
- 28:22my
- 28:23question.
- 28:23That
- 28:23there is
- 28:23some
- 28:24level of
- 28:24you
- 28:24have to
- 28:24lay flat,
- 28:25even in
- 28:26hepzado
- 28:26kit
- 28:26Any of the
- 28:26trans
- 28:27arterial
- 28:27therapies.
- 28:28There will
- 28:28be a,
- 28:293 to 6
- 28:30hour bed
- 28:30rest
- 28:31hours,
- 28:31unless
- 28:32you go
- 28:32through
- 28:32the rest,
- 28:33which
- 28:33you can
- 28:33do, which
- 28:34had.
- 28:34But for
- 28:35absorber
- 28:35you can go
- 28:36through
- 28:36the rest.
- 28:37Thank
- 28:37you so
- 28:37much,
- 28:38Doctor
- 28:38Lowe.
- 28:40Great.
- 28:40So unless
- 28:41there's
- 28:41any other
- 28:41questions,
- 28:42we'll wrap
- 28:42up
- 28:42the session
- 28:43a couple
- 28:43minutes
- 28:43early.
- 28:44All right.
- 28:45Thank
- 28:45you guys.
