- 0:02- Pharmacy has long been viewed as a cost
- 0:03- center, but that framing is starting to shift,
- 0:06- and chief pharmacy officers are driving the change.
- 0:09- Don Wright, pharmacy is one of the most
- 0:11- powerful financial and clinical levers the health system

- 0:59- Hello. This is Arianna Porto Latin with the
- 1:01- Becker's Dental and DSO review podcast.
- 1:03- I'm thrilled to be joined today by doctor
- 1:05- Scott Law, the cofounder of Smile Doctors. Doctor
- 1:07- Law, thank you so much for being here
- 1:09- today. It's a pleasure to have you on
- 1:10- our podcast. Hey. Thanks for having me. Happy
- 1:12- to join.
- 1:13- Great. To start us off, can you introduce
- 1:15- yourself for our listeners and tell us a
- 1:17- little bit about your background and give a
- 1:18- brief intro to small doctors as well?
- 1:21- Absolutely. Yeah. So I'm an orthodontist
- 1:23- by trade.
- 1:25- Graduated dental school in 2006
- 1:27- and practiced as a GP for a short
- 1:29- time before going back to ortho school. And
- 1:30- then,
- 1:31- it was in 2015
- 1:34- that I helped cofound
- 1:36- Smile Doctors, which is an orthodontic
- 1:39- only,
- 1:40- support organization or an OSO in the DSO
- 1:43- world. So,
- 1:44- we've we started with about with a handful
- 1:47- of practice. I think we had about nine
- 1:48- locations to start, and we're currently at 600
- 1:53- All across the all across The US. Yeah.
- 1:55- I think we're in about thirty thirty seven
- 1:57- states.
- 1:58- Awesome. Alright. First question for you here.
- 2:01- How have Smile Doctors,
- 2:03- growth strategies evolved over the years into what
- 2:05- it is today? Yeah. So I think initially,
- 2:07- it was it was kinda
- 2:09- we've always attracted doctors who have wanted to
- 2:11- be a part of something bigger than themselves,
- 2:13- and orthodontics is such a special part of
- 2:16- dentistry. I think we get to,
- 2:18- be the be a lot of the happy
- 2:19- place where people see their their teeth transform
- 2:22- and their lives transform in that way just
- 2:24- by opportunities
- 2:26- and and, be just just a smile changes
- 2:29- everything. I think it's a part of every
- 2:30- bit of our life
- 2:32- from every bite of food, every kiss, every
- 2:34- job interview, and,
- 2:36- things have changed over the last few years
- 2:38- just in
- 2:39- that we're we're a large part acquisition,
- 2:42- but we've also had
- 2:44- great success with De Novos. So where we
- 2:46- have,
- 2:48- strategies to do
- 2:50- startup practices
- 2:51- in markets adjacent to our current footprint. And
- 2:54- so,
- 2:55- we are we've also changed too to becoming
- 2:58- a 100%
- 2:59- asset
- 3:00- purchase, strategy to one where we are very
- 3:03- much a joint venture. And so doctors retain
- 3:06- that that ownership in their local practice as
- 3:08- well as having
- 3:10- ownership at the at the hold code level.
- 3:13- And so that that's been a big benefit.
- 3:15- Doctors love it. It keeps that entrepreneurial,
- 3:19- bit about them really strong, and we have
- 3:21- seen the positive effects of that.
- 3:24- Great.
- 3:25- And I'd love to know what have been
- 3:27- the biggest lessons or takeaways from the last
- 3:29- few years, for the company as far as,
- 3:32- m and a, network expansion, growth, etcetera, anything
- 3:35- that you can share there? Yeah. Absolutely. I
- 3:37- think we've always sought doctors who care a
- 3:40- great deal about the future. So we're not
- 3:42- an organization that that,
- 3:45- finds it really attractive when a doctor is
- 3:47- looking to just retire, sell their practice, and
- 3:49- then, you know, set sail in the sunset.
- 3:51- So we're we're one that
- 3:53- we want doctors who wanna wanna dive in,
- 3:56- make us better, see how our systems processes,
- 4:00- our workflows, everything can can lean in,
- 4:04- to make them better. And then that that
- 4:06- they share a little bit of the burden.
- 4:08- I think practice ownership is stressful,
- 4:11- and a lot of doctors reach a point
- 4:13- where they're redlining
- 4:14- in all areas of life at life at
- 4:16- practice, life at home. And
- 4:19- so having taking that approach to where they're
- 4:21- this is a true partnership, and,
- 4:24- they're able
- 4:25- to grow things there and do what they
- 4:27- want with their life. It's it's an awesome
- 4:29- piece. But
- 4:30- part partly is has been, I think, the
- 4:33- focus on the joint venture or the JV
- 4:35- model. That way, it's it's really we've seen
- 4:39- tremendous growth there. And then when doctors
- 4:42- join us and they they've had ideas to
- 4:44- do a second or a third location,
- 4:47- we can help them do that through de
- 4:49- novos. So it's been been a great benefit
- 4:52- and a really, really positive success for us.
- 4:55- Okay.
- 4:56- And something I've spoken with quite a few,
- 4:58- executives and, orthodontists about has been the growth
- 5:02- in adult orthodontics over the last few years.
- 5:06- I'd love to know, what do you attribute
- 5:08- the growth in this market to?
- 5:10- Right. So a couple of things. I think
- 5:12- all of us being on Zoom and camera
- 5:14- all day, we're looking at ourselves and looking
- 5:16- at our at our teeth, right, right, shining
- 5:19- right back at us. And so we see
- 5:21- that a lot. And as we're as we're
- 5:23- aging,
- 5:25- we're we're trying to see ways that we
- 5:26- can stay younger, look more youthful. And so
- 5:28- orthodontics
- 5:29- is is one way there where everybody wants
- 5:31- a nice straight white bright smile.
- 5:33- And so that's one. I think the attention
- 5:35- to it. I think there's also been some
- 5:37- very positive moves in the industry.
- 5:40- The
- 5:41- the plastics technology is that's come along. It's
- 5:44- just made it easier to move teeth, easier
- 5:47- to get your teeth straight, so you don't
- 5:48- have to do the bracket and wire. And
- 5:49- then there's been some some others where
- 5:52- the I truly think that the do it
- 5:54- yourself aligner movement brought a ton of attention
- 5:58- to what's possible.
- 5:59- And that opened up for, at least for
- 6:01- Smile Doctors, our Smile Express,
- 6:05- offering,
- 6:06- where where it's only two visits,
- 6:09- to the to the practice, and they get
- 6:12- their aligners. They're monitored remotely,
- 6:14- and and people can get their their, you
- 6:17- know, their concrete or their aesthetic treatment
- 6:20- completed,
- 6:21- in in a very short time. And then
- 6:24- always have the option to upgrade to complete
- 6:26- comprehensive treatment if they if they want if
- 6:28- their goals are bigger. But this has been
- 6:30- a very, very positive thing for us, and
- 6:32- Smile Express is is,
- 6:34- has been a really good
- 6:36- thing. Yeah.
- 6:38- And are there any other shifts that you
- 6:40- can predict will take place in the adult
- 6:42- ortho market over the next two to three
- 6:44- years?
- 6:45- It's that's interesting. I think as people get
- 6:47- busier and busier, their their time demands
- 6:51- are, you know, everyone has constraints. We have
- 6:54- obligations. We have we have places we need
- 6:56- to be. So the more convenient that we
- 6:58- make it, I think that's how pay when
- 7:00- patients win, I think we win as an
- 7:02- industry.
- 7:03- And so it's it's that. It's kind of
- 7:05- a shift away from the paternalistic
- 7:08- treatment model where the patients were there to
- 7:10- serve the doctor. I think that was that
- 7:12- was true of of yesteryear.
- 7:14- And more and more as we make it
- 7:16- convenient,
- 7:17- accessible, affordable
- 7:19- to, especially adult patients, we see that they're
- 7:22- happy. They get the results they want. Doctors
- 7:24- are happy because we're able to treat at
- 7:26- a very high level,
- 7:27- deliver on
- 7:29- on our treatment goals.
- 7:31- And when that's the case and there and
- 7:33- there's business profitability, it is a it's all
- 7:36- green lights all the way around. And so
- 7:38- that's that, I think, is only gonna continue
- 7:40- to grow in the adult segment.
- 7:42- I think, of course, I'm biased, but I
- 7:44- think Smile Express is taking off.
- 7:47- And as more and more eyes get there,
- 7:49- they patients
- 7:50- want to be overseen by an orthodontist. They
- 7:53- wanna know that if something is not going
- 7:55- right,
- 7:56- they can go into a brick and mortar
- 7:58- and and be taken care of. So while
- 8:00- they want the convenience,
- 8:02- they they also wanna know, hey. There's a
- 8:04- fallback that I have a doctor
- 8:06- who sees me. I know where they are.
- 8:08- I know I can be seen if I
- 8:09- need to. And so I think that's probably
- 8:11- on the horizon,
- 8:13- a whole lot more industry wide, and Smile
- 8:16- Doctors is the first the first to lead
- 8:18- out with it.
- 8:19- Great. And I'd love to dive in a
- 8:21- little bit deeper,
- 8:23- into the patient expectations
- 8:24- portion,
- 8:25- also technology. So how would you say,
- 8:29- these two items are changing the orthodontic experience
- 8:31- currently,
- 8:32- And how is Smile Doctors approaching these changes?
- 8:35- Right. I think we're leaning into it. One
- 8:38- of our competitive advantages
- 8:40- at Smile Doctors has always been, even from
- 8:43- before we began, I think we'll it's true
- 8:45- for a lot of our doctors, our our
- 8:46- founding doctors for sure, and why so many
- 8:49- join us is we love people first. We
- 8:51- straighten teeth second. And that's always been the
- 8:54- case, and those people can be patients. They
- 8:56- can be team members. They can be doctors.
- 8:58- They can be anybody involved in in the
- 9:01- straightening of teeth.
- 9:02- And so that's always been our approach.
- 9:05- But
- 9:06- as
- 9:07- as we look and go, well,
- 9:10- how are things shifting? With AI, I think
- 9:13- more and more is becoming impersonal.
- 9:16- There are great efficiencies.
- 9:17- And so people wanna book appointments online. They
- 9:21- they they wanna save time.
- 9:23- But we always have to ask ourselves, what
- 9:25- are we then doing with the extra time?
- 9:27- How how can we make and build
- 9:30- better and deeper connections?
- 9:32- And so that's been our approach, is to
- 9:34- lean into relationships. How can we help
- 9:37- our dental referral partners? How can we help
- 9:40- them grow their
- 9:41- I think for too long, orthodontics has been
- 9:44- kind of a one way street where orthodontics
- 9:47- orthodontists come to,
- 9:49- the general dentist and say, hey. Please send
- 9:51- us patients.
- 9:52- And
- 9:53- I I don't think that orthodontists get the
- 9:56- credit for the patients that they send back
- 9:58- who need a dental home.
- 10:00- Because many times, I know in Smile Doctors,
- 10:02- we get patients who they see our marketing.
- 10:04- They they come in, and they haven't seen
- 10:06- a dentist for a long time. They don't
- 10:07- have a dental home. And so we're able
- 10:09- to form deeper relationships with our referring dentists
- 10:13- as we send them patients to that they
- 10:15- can take care of and
- 10:17- they can love on, and then we can
- 10:19- we can straighten their teeth. I think it's
- 10:21- a win win all the way around for,
- 10:24- all aspects from the patient, their outcomes,
- 10:27- their their their level of dental care and
- 10:29- health, and then just big, beautiful smiles that
- 10:32- they get to go share all throughout their
- 10:34- life. So that's that's really how we're we're
- 10:37- leveraging technology.
- 10:38- We're the largest
- 10:41- AI
- 10:42- user in in dentistry in the world to
- 10:44- my knowledge. We're the number one user in
- 10:47- of dental monitoring,
- 10:49- and we're also the number one user of
- 10:51- Align Technologies
- 10:52- and and aligners. And so we use that
- 10:55- as well as all of our business processes
- 10:57- on the back end to really streamline things
- 10:59- and be the the very best business
- 11:02- and
- 11:03- and dental
- 11:05- and orthopractus
- 11:06- that we can be.
- 11:09- Super interesting.
- 11:11- Well, that's all I have for you today,
- 11:12- doctor Ludd. Thank you so much for joining
- 11:13- us. It's been a pleasure speaking with you,
- 11:15- learning more about Smile Doctors, and I look
- 11:17- forward to connecting with you again in the
- 11:18- future. I appreciate it. Thank you for having
- 11:21- me on. Thank you so much. Have a
- 11:22- good day. Bye bye. You too.
