- 0:00Everyone. This is Molly Gammell with Becker's Healthcare.
- 0:01Thank you so much for tuning in to
- 0:03the Becker's Healthcare podcast series. Today, we'll be
- 0:05talking about how virtual care, especially in primary
- 0:07and mental health,
- 0:09can improve access, patient outcomes, and operational efficiency
- 0:12amid rising demand and provider shortages.
- 0:15Joining me for today's discussion is Susanna Feinstein,
- 0:18product manager for behavioral health at AmWell. Susanna,
- 0:20thank you so much for joining me and
- 0:22being my guest. Thank you so much for
- 0:24having me. Well well, to get us started,
- 0:25Susanna, can you can you please share more
- 0:27about yourself and then a bit more too
- 0:29about AmWell?
- 0:30Of course.
- 0:31So, hi. My name is Susanna Feinstein, and
- 0:34I lead our behavioral health strategy here at
- 0:36AmWell as a product manager for digital behavioral
- 0:39health.
- 0:40My focus is really on expanding
- 0:43access to high quality mental health care through
- 0:46digital innovations.
- 0:48We're we're really working closely with health plans,
- 0:50providers, and employers
- 0:52to deliver scalable evidence based behavioral health solutions
- 0:57that meet people where they are. So whether
- 0:59that's at home, at work, or on the
- 1:01go.
- 1:03My background is I'm a licensed clinical social
- 1:05worker,
- 1:06and I'm very passionate about this topic. So
- 1:08thank you for having me.
- 1:10Well, I I feel like, Suzanne, there has
- 1:11been so many
- 1:13a proliferation of options for people when it
- 1:15comes to digital health solutions. And interestingly,
- 1:18research shows patients still want more. They want
- 1:21greater access to digital health tools
- 1:23to really help them manage their health. Many
- 1:25organizations, including some of the ones you mentioned
- 1:28there, health plans, employers, providers are are working
- 1:31to meet this demand, just seeing it as
- 1:32such an opportunity for patient satisfaction experience, creating
- 1:36efficiency
- 1:37even amid stacking gaps.
- 1:39Let's step back a bit just to start.
- 1:41What are some key trends that you're seeing
- 1:43in the virtual care space, especially in primary
- 1:45care?
- 1:46And I'm curious what opportunities and challenges you've
- 1:49really seen emerge.
- 1:51Great question. Yeah. It's a huge issue. Over
- 1:53one hundred sixty million Americans
- 1:55live in mental health professional shortage areas.
- 1:58So at AmWell, we're really addressing this by
- 2:01offering a flexible
- 2:03tech enabled platform
- 2:04that connects individuals
- 2:06with licensed therapists, psychologists,
- 2:09psychiatrists,
- 2:10and, of course, our digital programs as well.
- 2:12Our solution really includes everything from live therapy
- 2:16and psychiatry to more of that self guided
- 2:19cognitive behavioral therapy or CBT modules on the
- 2:22go.
- 2:23And our goal there is really to help
- 2:25to close the gap of access while supporting
- 2:28a range of acuity levels, so the low
- 2:30to moderate
- 2:32all the way up to the moderate to
- 2:33severe acuity levels. And one major trend, as
- 2:36you said, like, what are some major trends?
- 2:39One major trend that we've seen is the
- 2:41shift toward whole person care.
- 2:43So that's really the concept of integrating behavioral
- 2:46health into broader care journeys and pathways.
- 2:50We're also seeing increased demand for asynchronous
- 2:53tools like digital CBT
- 2:56or cognitive behavioral therapy,
- 2:58which offers flexibility
- 3:00and privacy.
- 3:02Patients really want care that's convenient to them,
- 3:05personalized, and stigma free, and technology is really
- 3:09enabling that and making that a possibility.
- 3:12At Amwell, we're combining the human care with
- 3:15digital tools to create that seamless experience
- 3:18that supports both the prevention and the treatment,
- 3:21and that's really a trend that we're seeing,
- 3:23that expectation that people want that blended care
- 3:26model, more of a longitudinal care journey,
- 3:29that supports them again through all the the
- 3:31stages of their mental health journey.
- 3:34Mhmm. That stat you just mentioned, as of
- 3:37this year, a 160,000,000
- 3:38Americans living in areas facing mental health provider
- 3:41shortages.
- 3:43And that's that's a staggering statistic
- 3:46in itself. And then I imagine even compounded
- 3:48by wait times in areas that don't have
- 3:51shortages, but there's still some access problems.
- 3:53You you hit on this a bit, but
- 3:55can you go a little bit more in-depth
- 3:56about how you really see virtual care
- 3:59playing an important role in addressing these access
- 4:01issues?
- 4:03Of course. Yeah. So kind of let's start
- 4:05with some context to ground what we're talking
- 4:07about. Right now in The US,
- 4:09we know that over one in five adults
- 4:11is experiencing a mental illness every single
- 4:15year. We also know that the demand for
- 4:17mental health care has skyrocketed
- 4:19as a result of that, especially post the
- 4:21pandemic or since the pandemic.
- 4:23But the number of licensed therapists and providers
- 4:26has not caught up. And as you mentioned,
- 4:28many millions of Americans
- 4:30are living in areas facing mental health provider
- 4:33shortages or rural areas,
- 4:35so it's really a compounding effect. Mhmm. There
- 4:38are whole counties in The United States without
- 4:40a single practicing psychologist or psychiatrist.
- 4:44And as we know, the wait lists are
- 4:46stretching for months.
- 4:47And for therapists, we also know that the
- 4:49burnout is really real. We are overloaded,
- 4:53under resourced, and constantly asked to do more
- 4:56and more and more, so our plates are
- 4:58getting very full.
- 4:59As a result, providers really need tools that
- 5:02scale care without sacrificing the quality of the
- 5:05care.
- 5:06So these virtual tools are going to augment
- 5:09the care that they're providing, not automate. And
- 5:11that's really where AmWell's digital behavioral health solutions.
- 5:16And today, what we're focusing on especially is
- 5:18Silver Cloud or digital behavioral health. That's where
- 5:21we enter into the picture
- 5:23as well as AmWell's whole platform of delivering
- 5:25care.
- 5:27AmWell's digital behavioral health is a digital mental
- 5:29health platform,
- 5:31which offers evidence based programs for conditions like
- 5:34depression,
- 5:35anxiety,
- 5:36stress,
- 5:37insomnia, and even chronic illness and substance use.
- 5:40And here's a little bit how it works.
- 5:42Basically, a client will be enrolled into the
- 5:44program either in the self guided or supported
- 5:47program with a coach, a real live human
- 5:50being.
- 5:51They can access their CBT based content, including
- 5:54interactive modules,
- 5:56journaling prompts, videos, etcetera on their own time,
- 5:59and then communicate asynchronously
- 6:02with the coach, the real live behavioral health
- 6:04prepared human beings.
- 6:06And what's behind the scene is that their
- 6:08their provider can really monitor their progress,
- 6:12send them messages,
- 6:13assign them extra content and modules, and get
- 6:16data insights in real time about how they're
- 6:19progressing through.
- 6:20Again, we're we're HIPAA compliant.
- 6:23We integrate with electronic health records,
- 6:25and we have multilingual options including Spanish.
- 6:29And we're already being used by major US
- 6:31health care systems,
- 6:33colleges, counseling centers, and private practices,
- 6:36so health systems, health plans, providers.
- 6:40And it's seeing we're he seeing huge impact,
- 6:43especially in integrated care settings, such as primary
- 6:47care providers
- 6:48who want to offer behavioral health support,
- 6:51and then telehealth groups looking to scale without
- 6:53burning out their clinicians like you mentioned,
- 6:56as well as those rural areas where in
- 6:59person care is very much limited.
- 7:01And so, you know, one university counseling center,
- 7:04for example, saw a 25
- 7:07increase in student support capacity
- 7:10after implementing digital behavioral health, and that's without
- 7:13including an additional staff member.
- 7:16So, again, just to send the message home
- 7:19is that it's flexible,
- 7:20it's affordable,
- 7:21and it fits the regulatory and practical needs
- 7:24of US providers who are very much, struggling
- 7:27with shortages
- 7:29and burnout and all of the other things
- 7:31we mentioned.
- 7:32It's such a comprehensive
- 7:33answer to my question. Thank you so much
- 7:35for expanding on that.
- 7:37I I also wanna go back to something
- 7:38you had mentioned as a trend, but I
- 7:40think it sounds like it's really part of
- 7:42the mission of AmWell
- 7:43given that broader platform you had mentioned. The
- 7:45behavioral health piece is is one piece of
- 7:47it.
- 7:48But can you talk a bit about how,
- 7:50for so long, physical and mental health care
- 7:52were treated in silos. How are you seeing
- 7:54from animals' point of view
- 7:56improvements in behavioral health, whether it's access or
- 7:59outcomes, really contribute to overall improvements in patient
- 8:02outcomes, care delivery, overall cost? You you mentioned
- 8:05a great staff from a university there. Are
- 8:07there any other success stories or wins that
- 8:10come to mind that illustrate how these are
- 8:11connected?
- 8:12Yeah. Yeah. Awesome question. So we know that
- 8:14the behavioral health crisis is not going to
- 8:16go away. In fact, as you mentioned, really,
- 8:19the demand continues to grow and the provider
- 8:22supply is staying flat. So digital therapeutics like
- 8:25digital behavioral health at AmWell,
- 8:27they don't replace the providers. Again, they amplify
- 8:30them. They're helping us to work smarter,
- 8:32deliver more equitable care, and meet patients where
- 8:35they are.
- 8:36And so if you're a provider, as many
- 8:37of you out there are, this is about
- 8:39really preserving your energy while scaling your impact.
- 8:43So getting into a couple specific advantages like
- 8:45you mentioned for therapists, physicians, and health systems,
- 8:48and overall improvements in patient outcomes.
- 8:52Our tools are really allowing you to extend
- 8:54your capacity without that burnout.
- 8:57We know that these digital programs that run
- 8:59asynchronously
- 9:00will mean that your patients can work through
- 9:03their modules
- 9:04on their own time
- 9:05while you oversee the progress. And so as
- 9:08a result, a single provider can support three
- 9:10to five times as many patients
- 9:13with lower administrative burden.
- 9:15So really imagine freeing up your calendar
- 9:18while still ensuring that your patients are engaged
- 9:21between the sessions.
- 9:23And, you know, that really comes into play
- 9:24with the man measurement based care, making that
- 9:27easier, making value based care and measurement care
- 9:30easier,
- 9:32because we track usage,
- 9:34symptom change, and engagement automatically.
- 9:36So that's a really huge win for documentation,
- 9:40outcome reporting, and value based contracts.
- 9:43For example, a provider can see that a
- 9:45patient reduced their GAD seven scores by five
- 9:48points after four weeks of guided work, and
- 9:51that's something that without these digital tools, we
- 9:53wouldn't really be able to see the outcomes
- 9:55in real time like that, allowing us to
- 9:57scale.
- 9:59And then just, you know, research has shown
- 10:00that more than eighty percent of users complete
- 10:03at least one full module.
- 10:05And that's because the platform is intuitive, it's
- 10:08stigma free, and it's designed for mobile usage.
- 10:11And for younger or tech savvy populations,
- 10:15this is really what they expect for mental
- 10:17health services.
- 10:18And then for the providers
- 10:20working with health plans, especially Medicaid and commercial
- 10:23plans,
- 10:24we are working towards billable services.
- 10:27So we're working towards codes for reimbursement of
- 10:29that asynchronous and coached,
- 10:31guided therapy support.
- 10:33And so that's really why,
- 10:36therapists and providers are using these tools.
- 10:39They're seeing that it saves time. They're seeing
- 10:42that it supports clients between the sessions.
- 10:44They're seeing the data outcomes,
- 10:47measurable
- 10:48outcomes in mood all in one place.
- 10:51And, you know, they're even seeing that if
- 10:53they have a wait list, they can use
- 10:54Silver Digital Behavioral Health as a bridge to
- 10:58serve more clients without overbooking.
- 11:01And so, again, if you really wanna extend
- 11:03that reach, this is a tool that will
- 11:04help to do so. And one of our
- 11:06colleagues to your point do you have any
- 11:08case studies?
- 11:09One of our client colleagues said using digital
- 11:12behavioral health was like adding another therapist to
- 11:15the team without hiring anyone.
- 11:18So, again, we're not replacing providers. We're just
- 11:20allowing you to wrap your arms around more
- 11:22of your population
- 11:24and making them feel cared for in between
- 11:26visits, adjunct to therapy in so many different
- 11:29ways.
- 11:30Suzanne, thank you so much. Is there anything
- 11:32else? I think you painted such a rich
- 11:34picture of where AmWell fits in,
- 11:37in this space and the the the needs
- 11:39it's meeting so so clearly. The need is
- 11:42great, to understand how this is impacting
- 11:45patients and care teams,
- 11:46is really remarkable. Is there anything else that
- 11:49you wanna leave our listeners with as we
- 11:50wind down today?
- 11:52Yeah. Yeah. My last point that I'd like
- 11:54to emphasize is just that behavioral health is
- 11:56really no longer a siloed service.
- 11:58It's central to the overall health of organizations
- 12:02and to the individual
- 12:03members within a population.
- 12:05And so organizations that embrace digital innovation and
- 12:08invest in this integrated scalable solutions,
- 12:11those will be the ones that are best
- 12:13positioned to meet the rising demand and deliver
- 12:16meaningful impact.
- 12:18And so at AmWell, we're excited to be
- 12:20at the forefront of that transformation,
- 12:22and we're excited to see how our digital
- 12:24behavioral health tools, which are evidence based and
- 12:26clinically
- 12:28validated, can be used at scale to really
- 12:30support you and your patients
- 12:33and the members that you need help with.
- 12:35And this is really not a generic app.
- 12:37It's a therapeutic
- 12:39ecosystem grounded in the research. So that's really
- 12:41where I'd like to leave you is that
- 12:43this is evidence based. It's proven to work.
- 12:45There are research papers, white papers about us,
- 12:49and we're really here to help.
- 12:51Fantastic.
- 12:51Thank you so much, Susanna, for your time
- 12:54and your insights today. We also wanna thank
- 12:56our podcast sponsor, AmWell. You can tune in
