- 0:51- This is Laura Dierdo with the Becker's Healthcare
- 0:53- podcast. I'm thrilled today to be joined by
- 0:55- Thomasina Wilkins, chief revenue officer at Sinai Chicago.
- 0:59- Thomasina, it's a pleasure to have you on
- 1:00- the podcast today.
- 1:02- Thank you for having me.
- 1:05- Absolutely. Now I'm excited for our conversation. I
- 1:08- know that we've got a lot going on
- 1:09- at Sinai Chicago, and it'll be great to
- 1:11- dig in there as well as get your
- 1:13- perspective on the future of leadership too. But
- 1:15- before we do, can you introduce yourself and
- 1:17- just tell us a little bit more about
- 1:18- Sinai Chicago?
- 1:20- Definitely.
- 1:21- So my name is Thomas Wilkins. I am
- 1:24- the first chief revenue officer
- 1:26- for Sinai Chicago.
- 1:29- Sinai Chicago is one of the biggest and
- 1:31- largest safety net hospitals in the state of
- 1:34- Illinois. We are mission driven,
- 1:36- and we definitely
- 1:38- put a
- 1:40- a deep focus on mission driven care
- 1:43- that it's more than medicine. It is definitely
- 1:46- personal.
- 1:49- I love that. And I I think it
- 1:50- really just brings the humanness in health care,
- 1:53- down to every single thing that you're doing,
- 1:55- whether you're on the clinical side, administrative side,
- 1:57- finance side, anything. It it seems like that's
- 2:00- a a great motto to run by.
- 2:03- Absolutely.
- 2:04- Now what are some of the trends that
- 2:06- you're watching currently?
- 2:08- So as chief revenue officer, I am definitely
- 2:11- watching a lot of trends. The
- 2:14- I would say that the the major one
- 2:16- that we are watching is state and federal
- 2:19- policy, also denials management as well.
- 2:23- And what are the trends in denial, specifically
- 2:25- in the areas of,
- 2:28- medical necessity as well as authorization.
- 2:31- So those two are at the top of
- 2:33- the list, denials management,
- 2:35- as well as,
- 2:36- state and federal
- 2:38- rules and,
- 2:39- regulations and policies that may impact,
- 2:43- our ability to be able to provide care.
- 2:48- Absolutely. I know there's a variety of things
- 2:50- happening, whether it's, you know, HR one connected
- 2:52- to some of the things nationally and the
- 2:54- federal government as well as in Chicago, how,
- 2:57- you know, some of the especially Medicaid
- 3:00- rules have been changing and and whatnot. So,
- 3:03- from your perspective, what are some of the
- 3:04- ways that you are, working with your teams
- 3:08- to
- 3:08- meet the challenges that are either,
- 3:11- have already begun or coming down the pipe
- 3:13- in the next couple years,
- 3:15- when you're thinking about, you know, these different
- 3:17- policies and trying to make sure that you're
- 3:19- able to continue to bring revenue into the
- 3:21- organization and serve patients,
- 3:23- who very much need those services.
- 3:26- Absolutely.
- 3:26- So
- 3:27- first and foremost, I wanna tell you about
- 3:29- an exciting,
- 3:31- I would say an exciting
- 3:33- approach
- 3:34- that Sinai Chicago takes to everything, and that's
- 3:37- mission driven care.
- 3:39- So we know that, you know, up and
- 3:41- coming, you know, policies like HR one, Medicare
- 3:45- redetermination,
- 3:46- One of the biggest challenges for us is
- 3:48- that we serve a vulnerable
- 3:50- community,
- 3:51- some of the most vulnerable, you know, patients
- 3:54- in, the state,
- 3:56- whereas they have to choose between,
- 3:58- you know, food, medication,
- 4:00- rent. And now with Medicaid coming under scrutiny
- 4:03- and deter redetermination
- 4:05- as well as,
- 4:07- you know, new rules and regulations
- 4:09- for, you know, making sure that they are,
- 4:12- they are in alignment with what's required.
- 4:15- We have to take a a different approach
- 4:17- to how we care for our patients and
- 4:18- how we're assisting and supporting them. So right
- 4:21- now, we are ramping up at Sinai Chicago
- 4:23- to be able to do that, not only
- 4:25- from a care perspective to ensure
- 4:28- that, you know, access
- 4:29- to care is there, but also
- 4:31- making sure that we have community health care
- 4:33- workers in place
- 4:35- to be able to answer those hard questions
- 4:37- about,
- 4:38- you know, what are the redeterminations?
- 4:40- What do I need to do? And cyanide
- 4:42- Chicago stands strong
- 4:44- to be able to lead
- 4:45- that initiative at our hospital.
- 4:48- So when you pick up the phone and
- 4:49- you call for an appointment, you not you
- 4:51- not only get, you know, care, but you
- 4:53- also get,
- 4:54- you know, an engagement that asks you, you
- 4:57- know, what else do you need? Do you
- 4:58- need food? Do you need rent
- 5:00- assistance and support with, you know, Medicaid redetermination?
- 5:04- So we're looking at, you know, the full
- 5:06- picture of care, not only from a clinical
- 5:09- perspective, but we take a holistic
- 5:11- approach,
- 5:13- to full care and access to care. So
- 5:16- those are the things that we're we're trying
- 5:18- to do right now,
- 5:20- a collaborative
- 5:21- approach to not only help the patients that
- 5:23- we have, but the community as well.
- 5:28- I love it. And I think it's so,
- 5:30- beneficial for the community to have an institution
- 5:32- like Sinai Chicago who is looking at them
- 5:35- holistically
- 5:35- and trying to make sure that they have
- 5:37- the right services they need and access to
- 5:40- all the different things that can help them,
- 5:44- grow and develop.
- 5:46- Now when you think about things that you
- 5:48- are are working on right now, what are
- 5:49- you most focused on and excited about? And
- 5:52- I'd love to hear about anything you're most
- 5:53- proud of from the last six to eighteen
- 5:55- months as well.
- 5:56- So yes. So as the first chief revenue
- 5:59- officer for Sinai Chicago,
- 6:01- I will share with you that this is
- 6:03- an exciting time for me to serve,
- 6:06- at a safety net hospital.
- 6:08- As you know, my my focus over the
- 6:10- last twenty five years as being a subject
- 6:13- matter expert, not only in, you know, 20
- 6:16- of the 50 states internationally,
- 6:18- but also in the role that I serve
- 6:20- right now.
- 6:22- Collaboration
- 6:23- when it comes to patient financial services
- 6:26- is definitely leading the forefront at Sinai Chicago.
- 6:30- So
- 6:31- one of the things that I am excited
- 6:32- about is
- 6:35- the revenue cycle is all of us,
- 6:37- initiative.
- 6:38- And that means, you know, from the time
- 6:40- that the patient calls in to schedule an
- 6:43- appointment,
- 6:45- at the time that the patient receives
- 6:47- we're we're trying to get an authorization for
- 6:50- their care.
- 6:52- We take a holistic approach where at every
- 6:55- step of the way,
- 6:57- revenue cycle is all of us, and we
- 6:59- are all partners
- 7:01- in our patients' care. So
- 7:03- you may see that on the clinical side
- 7:05- that,
- 7:06- the employees are called caregivers, but our patient
- 7:09- financial services,
- 7:11- employees are called caregivers as well. The reason
- 7:13- being is because
- 7:15- revenue cycle is all of us and that
- 7:17- we partner across the organization to make sure
- 7:20- that there is a holistic approach to that.
- 7:22- So all of the leaders work together to
- 7:24- ensure that the patient experience
- 7:27- is top notch in there.
- 7:29- And then it's all of our responsibility
- 7:31- to ensure
- 7:32- that our patients have the highest quality of
- 7:34- care that we possibly can provide. And so
- 7:36- I'm excited about that as, you know, Sinai
- 7:39- Chicago's chief revenue officer
- 7:41- because this is an important approach
- 7:44- that that needs to be there in this
- 7:46- day and age. We are also,
- 7:49- embarking apart upon artificial intelligence.
- 7:52- So there's a lot of new and exciting
- 7:54- things that's that's coming
- 7:57- to our organization.
- 7:58- And I just wanna put that out there.
- 8:00- But,
- 8:02- as a team, as a leadership team, and
- 8:04- all of the caregivers who work at Sinai
- 8:06- Chicago,
- 8:07- this is an all of us approach. So,
- 8:10- the partnership in terms of collaboration
- 8:13- and operational efficiency is there, and I'm proud
- 8:16- to serve for this organization,
- 8:18- and the community based off of that.
- 8:22- That's amazing to hear. And, you know, I
- 8:24- love that tagline
- 8:25- that all of us approach it all in,
- 8:28- together
- 8:28- in making sure that everybody knows the mission
- 8:31- and is living it out and to have
- 8:33- everyone consider themselves caregivers in their role and
- 8:36- in what their care they're providing for the
- 8:38- community is so critical.
- 8:40- And and from your perspective, as you've been
- 8:42- making this transformation and working with your teams
- 8:46- in order to realize this this vision and
- 8:48- approach,
- 8:49- what have you worked well for you? What,
- 8:51- how have you communicated this? And what have
- 8:53- you seen and noticed among the teams as
- 8:56- you've gone through this process of really bringing
- 8:58- everybody together and partnering across the organization?
- 9:02- Absolutely. So,
- 9:04- in my background, of course, you know, most
- 9:06- folks know that I am cross functional, so
- 9:08- that's clinical finance as well as IT.
- 9:12- And I think that getting that message out
- 9:14- there and bridging the gap is important, not
- 9:16- only at Sinai Chicago, but at other health
- 9:19- care organizations
- 9:20- as well and understanding that it's a it's
- 9:23- a partnership. It starts with mission,
- 9:26- and it starts with partnership. So those two,
- 9:31- I would say, strategies
- 9:33- really drive home what excellence in care is.
- 9:37- So I would say that collaborative approach,
- 9:40- is definitely communicated every single day, and that's
- 9:42- what we strive for at Sinai Chicago.
- 9:47- Fantastic.
- 9:48- And, you know, you mentioned,
- 9:50- AI as well coming in exciting things for
- 9:53- the organization. I know AI is something that
- 9:56- many hospitals and systems are trying to figure
- 9:58- out and make sure they're optimizing the right
- 10:01- way and looking at adoption too. So
- 10:03- how have you been,
- 10:05- approaching the technology and AI as you've been
- 10:08- selecting, you know, what technology to use and
- 10:10- then incorporate it into the workflow? What's that
- 10:12- been like for you, and what have you
- 10:14- learned? What what's ahead?
- 10:16- Oh, yeah. So right, Tari, you know, I
- 10:19- have shared about my approach to
- 10:22- holistic care,
- 10:25- and it is proactive loss mitigation. Proactive loss
- 10:28- mitigation
- 10:28- means at the start of the process, even,
- 10:31- you know, when a patient is thinking about,
- 10:33- hey, should I choose
- 10:35- side eye Chicago
- 10:37- as my place of care?
- 10:39- Artificial intelligence plays an important role in that
- 10:42- as well. So we are
- 10:44- embarking upon implementing that organization wide.
- 10:49- Most recently,
- 10:50- you know, we have you know, we've gone
- 10:52- to Epic. We used to be on a
- 10:54- different system. So Sinai Chicago
- 10:56- is definitely new, with Epic. We're probably about,
- 10:59- you know, three years in.
- 11:01- So artificial intelligence
- 11:03- is just a way for us to have,
- 11:05- you know, additional tools. But I do think
- 11:08- that, you know, the,
- 11:11- warm hand off and the approach
- 11:13- by utilizing that as a tool would just
- 11:16- hide in, you know, the experience for our
- 11:18- patients. So,
- 11:20- that's every step of the way. That means
- 11:22- in operations. That means in clinical care. That
- 11:25- also means in,
- 11:27- patient financial services too.
- 11:29- So we are improving every single day on
- 11:32- our workflows and our operational practices,
- 11:35- mostly because that we are a mission driven
- 11:37- care
- 11:38- organization. And so,
- 11:41- AI just improves
- 11:44- and just makes us stronger.
- 11:46- But, definitely, the caregivers behind,
- 11:49- the tools that are, I would say, in
- 11:51- place,
- 11:53- are just as important as well.
- 11:58- Absolutely.
- 11:59- That makes a lot of sense and, you
- 12:01- know, it is definitely something that is so
- 12:03- cool about health care right now because you
- 12:06- have the technology that really augments what the
- 12:08- human workforce is doing and and can superpower
- 12:11- them in a way, it seems like, to
- 12:13- really get more done and then serve, patients
- 12:16- in a more
- 12:17- personalized and meaningful way. So that's amazing to
- 12:20- hear.
- 12:21- Absolutely. Now
- 12:23- yeah.
- 12:24- Yeah. Now before we wrap up, I wanted
- 12:26- to get your advice on leadership as well.
- 12:29- What would you tell evolving leaders today who
- 12:31- are looking to get in a role like
- 12:32- yourself or really just make a career,
- 12:35- and continue to lead in the health care
- 12:37- space?
- 12:39- I would say, you know, partnership.
- 12:42- Lead with partnership.
- 12:44- Understand that, you know, being a caregiver at
- 12:47- the end of the day, whether you serve
- 12:48- in an IT space, an operational space,
- 12:51- or inpatient financial services,
- 12:54- bringing that all together is important. Collaboration
- 12:57- is important to strategy.
- 12:59- And at the end of the day,
- 13:00- you know, staying focused on the mission, which
- 13:02- is ensuring that our patients have access to
- 13:05- care and working across the organization together
- 13:08- as a collaborative team to ensure
- 13:11- success in the highest quality of care organization
- 13:14- wide, I say, you know, that's where success
- 13:17- comes from. That's where a heightened patient experience
- 13:21- comes from.
- 13:22- And if you are looking to, you know,
- 13:24- serve in a leadership position,
- 13:27- this is one of the things that I've
- 13:28- learned. I started out at the front desk,
- 13:32- you know, serving. I've also served on the
- 13:34- the IT side as well as clinical and
- 13:37- financial.
- 13:39- And being mission driven and understanding at the
- 13:41- end of the day
- 13:43- that collaboration and approach and shared,
- 13:47- philosophy is what drives success, is what drives
- 13:50- mission, what drives,
- 13:52- you know, the the passion every single day
- 13:54- for me to stay in this particular role.
- 13:57- And then keeping in in in mind, you
- 13:59- know,
- 14:01- the patient experience as well. And if in
- 14:03- the event that the first approach didn't work,
- 14:06- I would say
- 14:08- recalibrate what that looks like and also engage,
- 14:11- you know, the patients for,
- 14:14- surveys
- 14:14- and,
- 14:16- experiences as well to see what they think.
- 14:18- But,
- 14:19- to those leaders out there, you know, godspeed
- 14:22- to you.
- 14:23- At Sinai Chicago,
- 14:25- care is provided by all of us regardless
- 14:28- of, you know,
- 14:30- what role we serve in, and I think
- 14:32- that mission driven approach
- 14:34- is why, you know, we're one of the
- 14:36- best in,
- 14:38- in care.
- 14:41- That's fantastic.
- 14:42- Thomasina, thank you so much for joining us
- 14:44- on the podcast today. This has been an
- 14:46- inspiring conversation. It's truly remarkable to hear about
- 14:49- all the things you're doing here at Sinai
- 14:50- Chicago, and I love the way that you're
- 14:53- transforming the culture, especially on the finance side.
- 14:56- So thank you so much for your time
- 14:57- today, and I look forward to connecting with
- 14:59- you again soon.
- 15:00- Thank you.
